Description
Foreliggende oppfinnelse vedrører anvendelse av monoestere av probukol for fremstilling av medikamenter for behandling av kardiovaskulær sykdom og inflammasj onssykdom.
OPPFINNELSENS BAKGRUNN
Kardiovaskulær sykdom er for tiden den fremste dødsårsak i De Forente Stater. Tilnærmet 90% av kardiovaskulær sykdom diagnostiseres i dag som aterosklerose. Kardiovaskulære sykdommer er koblet til flere årsaksfaktorer som omfatter hyperkolesterolemi, hyperlipidemi og ekspresjon av VCAM-1 i vaskulære endotelceller.
Hyperkolesterolemi og hvperlipidemi
Hyperkolesterolemi er en viktig risikofaktor forbundet med kardiovaskulær sykdom. Serumlipoproteiner er lipidbærere i sirkulasjonen. Lipoproteiner klassifiseres ut fra tetthet: chylomikroner, "very low-density"-lipoproteiner (VLDL), "low density"-lipoproteiner (LDL) og "high-density"-lipoproteiner (HDL). Chylomikroner deltar primært i transport av triglyserider og kolesterol i kosten fra tarm til fettvev og lever. VLDL leverer endogent syntetiserte triglyserider fra leveren til fettvev og andre vev. LDL transporterer kolesterol til perifere vev og regulerer det endogene kolesterolnivå i disse vev. HDL transporterer kolesterol fra perifere vev til leveren. (1983); Miller, Ann. Rev. Med. 31, 97 (1980)). Hos pasienter med lave LDL-nivåer utvikles aterosklerose sjeldent.
Forhøyet kolesterolnivå er forbundet med en rekke sykdomstilstander, heriblant restenose, angina, cerebral arterosklerose og xantom. Det er ønskelig å tilveiebringe en fremgangsmåte for reduksjon av plasmakolesterol hos pasienter med eller utsatt for å utvikle restenose, angina, cerebral arteriosklerose, xantom og andre sykdomstilstander forbundet med forhøyet kolesterolnivå.
Dersom det er fastlagt at hyperkolesterolemi skyldes forhøyet LDL (hyperlipedemi), forsøkes reduksjon av LDL-nivået ved diettbehandling. Det er flere medikamentklasser som hyppig benyttes for reduksjon av LDL-nivået, heriblant gallesyresekvestranter, nikotinsyre (niacin), og 3-hydroksy-3-metylglutaryl-koenzym A (HMG CoA)-reduktase-inhibitorer. Probukol og fibratderivatene benyttes noen ganger som tilleggsbehandling, vanligvis i kombinasjon med andre medikamenter. HMG CoA-reduktaseinhibitorene kalles statiner eller vastatiner. Statinene er blant de mest effektive midler som i dag foreligger på markedet for hyperkolesterolemi, og omfatter pravastatin (Pravchol, Bristol Myers Squibb), atrovastatin (Warner Lambert/Pfizer), simvastatin (Zocor, Merck), lovastatin (Mevacor, Merck) og fluvastatin (Lescol).
For mange pasienter vil diettbehandling pluss et av de hypolipidemiske midler være tilstrekkelig. For pasienter med et utgangsnivå av LDL-kolesterol høyere enn 200 mg/dl, må imidlertid behandlingen redusere LDL-nivået med 50% eller mer. Selv om et enkelt middel fra tid til annen kan gi denne grad av LDL-reduksjon, er det mye vanligere å se reduksjoner på bare 20 til 30%. For en pasient med heterozygot familiær hyperkolesterolemi med et LDL-kolesterol på 200 til 400 mg/dl, er det nødvendig med en kombinasjon av to, og av og til tre, hypolipidemiske medikamenter for å oppnå et LDL-kolesterolnivå som er lavere enn 100 mg/dl. Det foreligger bevismateriale som tyder på at de aterogene virkninger av "low density"-lipoprotein (LDL) delvis kan formidles ved oksidativ modifisering av dette. Probukol er vist å besitte kraftige antioksidante virkninger og å blokkere oksidativ modifisering av LDL. I samsvar med disse funn har probukol faktisk blitt vist å redusere utviklingen av aterosklerose hos LDL-reseptordefisiente kaniner, som diskutert i Carew et al., Proe. Nati. Acad. Sei. U.S.A. 84:7725-7729 (1987). Sansynligvis er probukol effektivt siden forbindelsen er svært lipidløselig og transporteres av lipoproteiner, noe som beskytter dem mot oksidativ skade.
Probukol er kjemisk beslektet med de hyppig benyttede mattilsetningsstoffene 2,[3]-tert-butyl-4-hydroksyanisol (BHA) og 2,6-di-tert-butyl-4-metylfenol (BHT). Probukol's fulle kjemiske navn er 4,4'-(isopropylidenditio)bis(2,6-di-tert-butylfenol).
I dag benyttes probukol først og fremst for å redusere serumkolesterolnivået hos hyperkolesterolemiske pasienter. Probukol tilføres hyppig i form av tabletter som er tilgjengelige under varemerket "Lorelco". Uheldigvis er probukol nærmest uløselig i vann og kan derfor ikke injiseres intravenøst. Probukol er faktisk vanskelig for cellene å absorbere in vitro grunnet dårlig blandbarhet med buffere og medier for cellekultur. Fast probukol absorberes dårlig i blodet og utskilles i alt vesentlig i uendret form. Videre absorberes tablettformen av probukol med signifikant forskjellig hastighet og i forskjellige mengder av forskjellige pasienter. I en undersøkelse (Heeg et al., Plasma av forskjellige pasienter. I en undersøkelse (Heeg et al, Plasma Levels of Probucol in Man After Single and Repeated Oral Doses, La Nouvelle Presse Medical, 9:2990-2994
(1980)), ble toppnivået av probukol i serum funnet å variere med opp til en faktor på 20 fra pasient til pasient. I en annen undersøkelse observerte Kazuya et al., J. Lipid Res. 32; 197-204 (1991) inkorporering av mindre enn tilnærmet 1 ug probukol/10<6> celler ved inkubasjon av endotelceller i 24 timer med 50 fim probukol.
U.S. patent nr. 5,262,439 tildelt Parthasarathy beskriver løselige analoger av probukol i hvilke en eller begge hydroksylgrupper er erstattet med estergrupper som gjør forbindelsen vannløselig. I en utførelse er det løselige derivat utvalgt fra gruppen som består av mono- eller di-ravsyreester, glutarsyreester, adipinsyreester, korksyreester, talksyreester, azelainsyre eller maleinsyre av probukol. I en annen utførelse er probukolderivatet en mono- eller di-ester i hvilken esteren inneholder en alkyl- eller alkenylgruppe som omfatter en funksjonell gruppe utvalgt fra gruppen som består av en karboksylsyregruppe, en amingruppe, et salt av en amingruppe, amidgrupper og aldehydgrupper.
En serie med franske patenter viser at visse probukolderivater er hypokolesterolemiske og hypolipemiske midler: Fr 2168137 (bis 4-hydroksyfenyltioalkanestere); Fr 2140771 (tetralinylfenoksyalkansyreestere av probukol); Fr 2140769
(benzofuryloksyalkansyrederivater av probukol); Fr 2134810 (bis-(3-alkyl-5-t-alkyl-4-tiazol-5-karboksy)fenyltio)alkaner; Fr 2133024 (bis-(4-nikotinoyloksyfenyltio)propaner; og Fr 2130975 (bis(4-(fenoksyalkanoyloksy)-fenyltio)alkaner).
U.S. patent nr. 5,155,250 beskriver at 2,6-dialkyl-4-silylfenoler er antiaterosklerotiske midler. De samme forbindelser beskrives som serumkolesterolreduserende midler i PCT-publikasjon nr. WO 95/15760, publisert 15. juni 1995. U.S. patent nr. 5,608,095 viser at alkylerte 4-silylfenoler inhiberer peroksidering av LDL, redusere plasmakolesterol og inhiberer ekspresjon av VCAM-1, og at de således er anvendbare ved behandling av aterosklerose.
Ekspresjon av VCAM- 1
Adhesjon av leukocytter til endotelet representerer en grunnleggende og tidlig begivenhet i kardiovaskulær sykdom såvel som i et bredt utvalg av inflammatoriske tilstander, heriblant autoimmunforstyrrelser og bakterielle og virale infeksjoner. Rekrutteringen av leukocytter til endotelet begynner når induserbare adhesjonsmolekylreseptorer på overflaten av endotelceller interagerer med motreseptorer på immunceller. Vaskulære endotelceller avgjør hvilken type leukocytter (monocytter, lymfocytter eller neutrofiler) som rekrutteres ved selektiv ekspresjon av rekrutteres ved selektiv ekspresjon av spesifikke adhesjonsmolekyler, feks. vaskulærcelleadhesjonsmolekyl-1 (VCAM-1), intercellulært adhesjonsmolekyl-1 (ICAM-1) og E-selektin (ELAM). I det tidligste stadium av den aterosklerotiske lesjon skjer en lokalisert endotelekspresjon av VCAM-1 og selektiv rekruttering av mononukleære leukocytter som uttrykker integrinmotreseptoren VLA-4. Grunnet selektiv ekspresjon av VLA-4 på monocytter og lymfocytter, men ikke på neutrofiler, er VCAM-1 viktig for formidling av den selektive adhesjon av mononukleære leukocytter. VCAM-1 deltar som en formidler ved kroniske inflammatoriske forstyrrelser som astma, reumatoid artritt og autoimmun diabetes. VCAM-1 uttrykkes i dyrkede humane vaskulære endotelceller etter aktivering med lipopolysakkarid (LPS) og cytokiner som interleukin-1 (IL-1) og tumornekrosefaktor (TNF-a). Disse faktorer er ikke selektive for aktivering av celleadhesjonsmolekyleks-presjon.
Påfølgende overføring av leukocytter til skummakrofager fører til syntese av et bredt utvalg av inflammatoriske cytokiner, vekstfaktorer og kjemoattraktanter som bidrar til å videreføre den rekruttering av leukocytter og blodplater, proliferasjon av glatte muskelceller, endotelcellaktivering og syntese av ekstracellulær matriks som er karakteristisk for en modnende aterosklerotisk plaque.
Molekylær analyse av de regulatoriske elementer i det humane VCAM-1-gen som kontrollerer dets ekspresjon tyder på en viktig rolle for nukleær faktor-kB (NF-kB), en transkripsjonsreguleringsfaktor, eller et NF-kB-lignende bindingsprotein i oksidasjons-reduksjonssensitiv regulering av VCAM-1-genekspresjon. Transkripsjonsfaktorer er proteiner som aktiverer (eller undertrykker) genekspresjon i cellekjernen ved å bindes til spesifikke DNA-sekvenser som kalles "enhancerelementer" som generelt ligger nær det området i genet som betegnes "promoteren", fra hvilket RNA-syntesen initieres.
Promoterene for både VCAM-1 og ICAM-1 er klonet og karakteriserte. For eksempel, omfatter begge promotere flere DNA-sekvenselementer som kan binde transkripsjonsfaktoren NF-kB. Iademarco, M.F. et al, J. Biol. Chem. 267,16323-16329

Nukleær faktor-kB er en allestedsnærværende uttrykt transkripsjonsfaktor med flere subenheter som aktiveres i flere celletyper av en stor og mangfoldig gruppe av inflammatoriske midler, feks. TNFa, IL-IB, bakterieendotoksin og RNA-virus. Den spiller en nøkkelrolle ved formidling av inflammatoriske signaler og andre stressignaler til kjernens regulatoriske apparat. Selv om de nøyaktige biokjemiske signaler som aktiverer NF-kB er ukjente, kan denne transkripsjonsfaktor integrere i en felles molekylær reaksjonsvei mange av risikofaktorene og de "årsaksmessige" signaler for aterosklerose, feks. hyperlipidemi, røyking, hypertensjon og diabetes mellitus.
Aktivering av NF-kB i vaskulære endotelceller ved forskjellige signaler kan spesifikt inhiberes med antioksidanter som N-acetylcystein og pyrrolidinditiokarbamat. Dette har ført til den hypotese at oksygenradikaler spiller en viktig rolle i aktiveringen av NF-kB via en ikke-definert oksidasjons-reduksjonsmekanisme. Siden et NF-kB-lignende enhancerelementer også regulerer transkripsjonen av VCAM-1-promoteren på en oksidasjons-reduksjonssensitiv måte, ble det fremstilt en hypotese om at oksidativt stress i den aterosklerotiske lesjon kan spille en rolle i regulering av VCAM-1 - genekspresjonen via dette oksidasjons-reduksjonssensitive transkripsjonsreguleringsprotein. U.S. patentskrift nr. 5,380,747 (PCT/US93/10496) beskrev for første gang at ekspresjon av VCAM-1 i vaskulære endotelceller kan inhiberes ved tilførsel av en klasse av ditiokarbamater som omfatter
omfatter pyrrolidinditiokarbamat. Disse ditiokarbamatene er således anvendbare ved behandling av kardiovaskulær sykdom og har nå blitt vist å signifikant redusere forekomst av aterosklerotiske lesjoner hos hyperkolesterolemiske kaniner.
Det har blitt foreslått at modifisering av "low-density"-lipoprotein (LDL) til oksidativt modifisert LDL (ox-LDL) ved reaktive oksygentyper er den sentrale begivenhet som initierer og viderefører aterosklerose. Steinberg, et al., N. Engl. J. Med. 1989; 320:915-924. Oksidert LDL er en kompleks struktur som består av i det minste flere, kjemisk forskjellige oksiderte materialer som hver for seg eller i kombinasjon kan modulere cytokinaktivert adhesjonsmolekylgenekspresjon. Fettsyrehydroperoksider som linoleylhydroperoksid (13-HPODE) produseres fira frie fettsyrer ved lipoksygenaser og er en viktig bestanddel av oksidert LDL.
Det er foreslått at dannelse av oksiderte lipider skjer ved cellens lipoksygenasesystem og at de oksiderte lipider deretter overføres til LDL. Deretter skjer en propageringsreaksjon i LDL i mediet, katalysert av transisjonsmetaller og/eller sulfhydrylforbindelser. Tidligere undersøkelser har vist at fettsyremodifisering ved dyrkede endotelceller kan endre cellenes følsomhet overfor oksidasjonsskader. PCT/US95/05880 beskrev at polyumettede fettsyrer og deres hydroperoksider induserer ekspresjon av VCAM-1, men ikke ICAM-1 eller E-selektin i humane endotelceller fra aorta, via en mekanisme som ikke formidles av cytokiner eller andre ikke-cytokinsignaler. Dette var en grunnleggende oppdagelse av en viktig og tidligere ukjent biologisk reaksjonsvei i VCAM-1-formid-lede immunresponser. Det ble også rapportert i PCT/US95/05880 at induksjon av VCAM-1 ved polyumettede fettsyrer og deres hydroperoksider undertrykkes av ditiokarbamater, heriblant pyrrolidinditiokarbonat.
Siden kardiovaskulær sykdom for tiden er den fremste dødsårsak i De Forente Stater, foreligger det et behov for å tilveiebringe nye terapiformer for behandling av denne. Det er mål å tilveiebringe nye midler som samtidig kan behandle hyperkolesterolemi og hyperlipidemi, og som kan inhibere ekspresjon av VCAM-1 i vaskulære endotelceller.
Det er derfor et formål med foreliggende oppfinnelse å tilveiebringe et preparat for behandling av kardiovaskulær sykdom.
Det er et videre formål med foreliggende oppfinnelse å tilveiebringe et preparat for behandling av kardiovaskulær sykdom som samtidig kan behandle hyperkolesterolemi og hyperlipidemi.
Oppsummering
Foreliggende oppfinnelse omfatter følgelig anvendelse av en behandlingseffektiv mengde av en monoester av probukol valgt fra en monoravsyreester av probukol, en monoglutarsyreester av probukol, en monoadipinsyreester av probukol, en monokorksyreester av probukol, en monotalgsyreester av probukol, en monoazelainsyreester av probukol, og en monomaleinsyreester av probukol, eller et far-masøytisk akseptabelt salt derav for fremstilling av et medikament for behandling av en kardiovaskulær sykdom.
Det er funnet at monoestere av probukol er effektive i samtidig å redusere kolesterol, senke LDL, og inhibere ekspresjon av VCAM-1, og at disse forbindelser således er anvendbare som sammensatte kardiovaskulære midler. Siden forbindelsen viser tre viktige vaskulærbeskyttende aktiviteter samtidig, kan pasienten ta et medikament istedet for flere medikamenter for å oppnå samme virkning. Dette bør forsterke behandlingsformens ensartethet og gjøre det lettere for pasienten.
Det var overraskende å finne at monoesteren av probukol inhiberer VCAM-1, tatt i betraktning at probukol selv ikke signifikant grad påvirker VCAM-1-ekspresjonen, selv om stoffet er en kraftig antioksidant. Diestere av probukol påvirker heller ikke i vesentlig grad VCAM-1-ekspresjonen, heller ikke statiner.
Det er også blitt funnet at monoravsyreesteren av probukol reduserer HDL kun i liten grad hos kaniner og ikke påvirker HDL hos mus og aper. I motsetning til dette, reduserer probukol LDL kun i liten grad og reduserer HDL betydelig. Statiner reduserer LDL og kan påvirke HDL eller ikke gjøre det.
Det er videre oppdaget at monoestere av probukol, nærmere bestemt monoravsyreesteren av probukol (heri betegnet "MSE"), selektivt inhiberer TNF-indusert VCAM-1- og MCP-1-genekspresjon, men ikke ICAM-1 i humane endotelceller fira aorta. MSE påvirker ikke NF-kB-aktiveringen. MSE benyttes heri som illustrerende for monoestere av probukol. Anvendelse av MSE som en illustrasjon er kun for å forenkle diskusjonen.
Siden oppdagelsen av at monoestere av probukol og nærmere bestemt, monoravsyreesteren av probukol blokkerer den induserte ekspresjon av endotelcellens overflateadhesjonsmolekyl VCAM-1, er de anvendbare ved behandling av enhver sykdom som formidles av VCAM-1, heriblant aterosklerose, post-angioplastisk restenose, koronarsykdommer, angina og andre kardiovaskulære sykdommer, såvel som ikke-kardiovaskulære inflammatoriske sykdommer som formidles av VCAM-1. inflammatoriske sykdommer som formidles av VCAM-1. Forbindelsene kan også benyttes ved behandling av hjertetransplantatrejeksjon.
Forbindelsen som beskrives heri er anvendbare både i primær medisinsk behandling og tilleggsbehandling av kardiovaskulær sykdom. Forbindelsene benyttes i primærbehandling av feks. koronarsykdomstilstander som omfatter aterosklerose, post-angioplastisk restenose, koronarsykdommer og angina. Forbindelsene kan tilføres for å behandle sykdom i små kar som ikke kan behandles ved kirurgi eller angioplasti, eller annen karsykdom hvor kirurgi ikke er mulig. Forbindelsene kan også benyttes for stabilisering av pasienter før revaskulariseringsbehandling.
Ved korrekt bruk tilveiebringer oppfinnelsen som beskrives heri, muligheten for medisinsk å "helbrede" aterosklerose ved å forebygge utvikling av nye lesjoner og å få etablerte lesjoner til å gå tilbake.
Oppfinnelsen omfatter videre anvendelse av en behandlingseffektiv mengde av en monoester av probukol valgt fra gruppen bestående av en monoravsyreester av probukol, en monoglutarsyreester av probukol, en monoadipinsyreester av probukol, en monokorksyreester av probukol, en monotalgsyreester av probukol, en monoazelainsyreester av probukol, og en monomaleinsyreester av probukol, eller et far-masøytisk akseptabelt salt derav for fremstilling av et medikament for behandling av en inflammatorisk sykdom.
I denne utførelse kan forbindelsene som beskrives heri benyttes ved behandling av inflammatoriske hudsykdommer som formidles av VCAM-1, nærmere bestemt humane endotelforstyrrelser som formidles av VCAM-1, som omfatter, men ikke er begrenset til, astma, psoriasis, ekematøs dermatitt, Kaposi's sarkom og multippel sklerose, såvel som proliferative forstyrrelser hos glatte muskelceller.

Detaljert beskrivelse av oppfinnelsen
I. Definisjoner
For anvendelse for fremstilling av et medikament for behandling av aterosklerose og andre kardiovaskulære sykdommer og inflammasjonssykdommer bør det velges monoestere av probukol som har passende lipofilisitet til at de lokaliseres i det påvirkede setet. Forbindelsen bør ikke kompartmentaliseres til områder med lav omsetning, f.eks. fettdepoter. For fordelaktig behandling av kardiovaskulær sykdom bør forbindelsens farmakokinetikk ikke dramatisk påvirkes av kongestiv hjertesvikt eller nyresvikt.
Den aktive forbindelse eller en blanding av forbindelsene kan tilføres på en hver egnet måte, heriblant systemisk, heriblant oralt eller intravenøst, eller topisk, heriblant transdermalt. Et generelt doseringsområde vil ligge mellom 0,1 til 500 mg/kg kroppsvekt med et doseringsskjema som varierer fra en gang annenhver dag til to eller flere ganger pr. dag. Omfanget av doseringen vil variere fra en enkelt dose gitt kun en gang til to daglige doser gitt i løpet av to til seks måneder.
Ved kardiovaskulær behandling kan forbindelsene også tilføres direkte til karveggen ved å benytte perfusjonsballongkatetere etter eller i stedet for koronar eller annen
arteriell angioplasti. Som et eksempel, kan 2-5 ml av en fysiologisk aksepterbar løsning som inneholder tilnærmet 1 til 500 mM av forbindelsen eller blandingen av forbindelser tilføres ved et trykk på 1-5 atmosfærer. Deretter tilføres de aktive forbindelser i løpet av de neste seks måneder under perioden med maksimal risiko for restenose via andre egnede tilførselsveier og doseringsskjemaer.
Relativt kort tids behandling med de aktive forbindelser kan benyttes for å gi "krymping" av koronarsykdomslesjoner som ikke kan behandles hverken ved angioplasti eller kirurgi. Et eksempel på kort tids behandling er to til seks måneder med en dose som varierer mellom 0,5 og 500 mg/kg kroppsvekt, gitt ved tidsrom som varie-varierer mellom 0,5 og 500 mg/kg kroppsvekt, gitt ved tidsrom som varierer fra en gang annenhver dag til tre ganger daglig.
Lengre tids behandling kan benyttes for å forebygge utvikling av fremskredne lesjoner hos svært utsatte pasienter. En langtidsbehandling kan strekke seg over år med doser som varierer mellom 0,5 og 500 mg/kg kroppsvekt, tilført ved tidspunkter som varierer fra en gang annenhver dag til tre ganger daglig.
De aktive forbindelser som anvendes kan også tilføres i tidsrommet umiddelbart før og umiddelbart etter koronar angioplasti som et middel for å redusere eller fjerne den unormale proliferative og inflammatoriske respons som i dag fører til klinisk signifikant restenose.
De aktive forbindelser som anvendes kan tilføres sammen med andre medikamenter som benyttes ved behandling av kardiovaskulær sykdom, heriblant inhibitorer av blodplateaggregering, feks. aspirin, antitrombotiske midler som koumadin, kalsiumkanalblokkere som varapamil, diltiazem og nifedipin, angiotensinkonverterende enzym(ACE)-inhibitorer som kaptopril og enalopril, og B-blokkere som propanalol, terbutalol og labetalol. Medikamentene kan også tilføres i kombinasjon med ikke-steroide antiinflammatoriske midler som ibuprofen, indometacin, fenoprofen, mefenaminsyre, flufenaminsyre, sulindac. Medikamentet kan også tilføres sammen med kortikosteroider.
MSE tilført ved subkutant implanterte kulter (kuler som frigir 150 mg/kg/dag) blokkerer LPS-indusert genekspresjon av VCAM-1 og MCP-1 i lunger i en musemodell.
Oral tilførsel av MSE (150 mg/kg/dag) i seks uker reduserer kolesterolnivået i totalplasma, ApO-B-holdig lipoprotein og HDL i en New Zealand White kaninmodell. Virkningene på plasmakolesterol følges av en markant inhibering av dannelse av aterosklerotiske lesjoner, makrofagakkumulering og VCAM-1-ekspresjon.
Oral tilførsel av MSE i to uker reduserer selektivt ApOB-holdige proteiner hos kolesterolforede C57 sorte mus og ApOE-"knock out"-musemodeller uten å påvirke HDL. Oral tilførsel av MSE i to uker i en hyperkolesterolemisk hundeapemodell reduserer kolesterolnivået i totalplasma og LDL uten å påvirke HDL.
MSE er ikke mutagent i den bakterielle Ames-analyse. Oral tilførsel av MSE i en dose på 1000/mg/kg/dag i to uker til rotter førte ikke til dødsfall og hadde ingen virkning på elektrolytt- og hematokrittverdier i serum. Forhøyet LDH, alkalisk fosfatase, SGOT og SGPT i serum ble observert, men var ikke statistisk forskjellig fra den ubehandlede gruppe og ikke ledsaget av endringer i levermorfologi eller -histopatologi.
For topisk tilførsel ved behandling av inflammatoriske hudforstyrrelser bør det oppnådde medikamentet utformes slik at det absorberes av huden i en mengde som er tilstrekkelig til å gi en terapeutisk virkning på det påvirkede sete. Monoesteren av probukol må være fysiologisk aksepterbar. Generelt er medikamenter med en terapeutisk indeks på minst 2, og fortrinnsvis på minst 5 eller 10, aksepterbare. Den terapeutiske indeks defineres som EC50/IC50, hvori EC50 er den konsentrasjon av forbindelsen som inhiberer ekspresjonen av VCAM-1 med 50% og IC50 er den konsentrasjon av forbindelsen som er toksisk for 50% av målcellene. Cellulær toksisitet kan måles ved direkte celletall, trypanblåtteksklusjon og forskjellige undersøkelser av den metabolske aktivitet, feks. 3H-tymidininkorporering, som kjent blant fagfolk.
Oppfinnelsen illustreres videre i eksemplene nedenfor, som benytter MSE som modellforbindelse. Enhver annen monoester av probukol som definert heri kan benyttes for behandling av kardiovaskulær sykdom og inflammasjonsforstyrrelser på idet vesent-lige den samme måte.
Eksempel 1. VCAM-l-ekspresjon i humane endotelceller fra aorta
Figur 1 er et søylediagram som sammenligner virkningen av monoravsyreesteren av probukol og probukol i konsentrasjoner på 2,5 nm, 5 nm, 10 nm og 100 nm på VCAM-l-ekspresjon i humane endotelceller fra aorta in vitro, som prosent av VCAM-1 - ekspresjonen som induseres av TNF alene. Eksempel 2. ICAM-l-ekspresjon i humane endotelceller fra aorta

Eksempel 3. MCP-l-ekspresjon i humane endotelceller fra aorta
Figur 3 er et søylediagram som sammenligner 10 fim av monoravsyreesteren av probukol, 50 fim probukol og TNF på MCP-l-ekspresjon i humane endotelceller fra aorta (HAEC). Cellene ble behandlet enten med TNF alene eller sammen med 10 fim av monoravsyreesteren av probukol i 4 timer. Celledyrkningsmediet ble høstet og benyttet for kvantitering av MCP-1-mengden ved benyttelse av en fargebasert ELISA-analyse. Som vist, inhiberte monoesteren av probukol ekspresjon av MCP-1 i større grad enn probukol alene. MCP-1 er et kjemoattrakterende protein som rekrutterer monocytter til en aterosklerotisk lesjon.
Eksempel 4. Virkning av monoravsyreester av probukol på genekspresjon i
humane endoteceller fra aorta in vitro
Figur 4 er en Northern-blotanalyse av VCAM-1- og MCP-1-genekspresjon fra RNA isolert fra lungene hos LPS-behandlede ApoE-"knock out"-mus. Musene ble tilført MSE, probukol og placebo subkutant i en 400 mg kule for frigivelse over 90 dager. Etter en uke ble musene behandlet intraperitonealt med 1 mg/kg LPS. Etter 2 timer ble dyrene avlivet og lungene nedfrosset for RNA-isolering. RNA ble størrelsesfraksjonert ved denaturerende 1,0% agarosegelelektroforese, overført til en nylonmembran og hybridisert med en JE-spesifikk <32>P-merket cDNA-probe fra mus. Membranen ble deretter renset og hybridisert med et VCAM-1-spesifikt cDNA fra mus og deretter en B-aktinspesifikk cDNA-probe fra kylling.
Eksempel 5. Virkning av monoravsyreester av probukol på kolesterol i plasma
hos lipidforede kaniner
Figur 5 er et søylediagram som viser virkningen av monoravsyreesteren av probukol og probukol på totalkolesterol og kolesterolnivå i lipoprotein i plasma hos lipidforede kaniner. Kaninene ble foret med for med høyt fettinnhold (0,5% kolesterol og 3% kokosnøttolje) tilsatt 0,5% vekt/vekt MSE eller probukol i 3 uker. Eksempel 6. Sammenligning av virkningen av MSE- og probukol-medikament-nivå i plasma hos kaniner foret med en diett rik på kolesterol i 3
uker
MSE eller probukol ble tilført til kaniner i for med høyt fettinnhold (0,5% kolesterol og 3% kokosnøttolje) ved en konsentrasjon på 0,5% vekt/vekt i 3 uker. Medikamentene ble ekstrahert fra plasma med eter og analysert ved høytrykksvæskekromatografi. Som vist, tilsvarte nivået av probukol nivået av MSE, selv om forbindelsene hadde signifikant forskjellig virkning på plasmakolesterol og lipoproteinnivå, som vist i eksemplene ovenfor.
Eksempel 7. Virkning av MSE på NF-kB-aktivering
Humane endotelceller fra aorto ble behandlet med TNF alene eller i kombinasjon med 25 (im MSE eller PDTC over et tidsrom på enten 1 time, 2 timer eller 4 timer. Cellene ble vasket og kjerneekstrakter fremstilt for utførelse av gelskiftanalyse med en VCAM-1-promoterprobe. Det ble fastslått at MSE ikke påvirker NF-kB-aktiveringen mens PDTC inhiberer aktiveringen av NF-kB.
Eksempel 8. Virkning av MSE på kolesterolnivået etter 6 uker hos kolesterolforede kaniner
New Zealand White-kaniner ble foret med for med høyt fett og høyt kolesterol (0,5%) alene eller sammen med 0,5% (vekt/vekt) (tilnærmet 150 mg/kg/dag) av enten AGE-3 eller probukol i 6 uker. Figur 8 er et søylediagram som viser virkningen av monoravsyreesteren av probukol på totalkolesterol, LDLc, VLDLc, ILDLc, HDLc, og triglyserider (TG) hos lipidforede kaniner etter 6 uker. Etter 6 uker ble lipoproteinfraksj onene separert fra helplasma ved hurtigfase væskekromatografi og analysert for kolesterol- og triglyseridinnhold. Som vist i eksempel 8, ble totalkolesterol såvel som kolesterol i VLDL og IDL redusert mer ved behandling med AGE-3 enn med probukol.
Eksempel 9. Virkning av AGE-3 på forløp av aterosklerose hos hyperkolesterolemiske kaniner
Kaninene beskrevet eksempel 8 ble avlivet og aorta tatt ut. Aorta ble farget med sudan-4 og omfanget av farging analysert. Figur 9 er en kurve som viser prosent aortaoverflateareal som er dekket av lesjoner i MSE-behandlede og ubehandlede lipidforede kaniner. Aorta fra kaniner som ble gitt AGE-3 hadde mye mindre farging, noe som viser redusert aterosklerose hos kaninene behandlet med monoravsyreesteren av probukol.
Aortasnitt ble immunfarget for VCAM-l-ekspresjon eller makrofagakkumulering ved benyttelse av antistoffer mot VCAM-1 eller Ram-11-antigen. AGE-3-behandling ga en markant reduksjon av VCAM-1-ekspresjonen og makrofagakkumuleringen (dvs. tilnærmet mer enn 75%). I et tilsvarende eksperiment var probukol i samme dose mye mindre effektivt (mindre enn 25% reduksjon av VCAM-l-ekspresjonen og makrofagakkumuleringen) .
Eksempel 10. AGE-3 reduserer LDL reversibelt hos hyperkolesterolemiske aper Hundeaper ble gjort hyperkolesterolemiske før AGE-3-tilførsel ved tilførsel av et for med høyt fett/kolesterol. Apene ble så tilført AGE-3 (100 mg/kg/dag) oralt i 2 uker. Prosent serum-LDL-kolesterol avtok i et område fra 4 til 60% hos apene over dette tidsrommet. Tilførsel av medikamentet ble så stanset, og serumkolesterol kontrollert på dag 29. Kolesterolnivået vendte tilbake til nivået før behandling og holdt seg der.

Eksempel 15
Figur 15 er et søylediagram som viser virkningen i rotter av 2 ukers oral tilførsel av monoravsyreesteren av probukol (1000 mg/kg/dag) sammenlignet med en kontroll på albumin, kreatinin, urinsyre og total bilirubin i tilfeldige enheter.
Man har merket seg at det er forskjell i virkningen av MSE og probukol i mus sammenlignet med kaniner og aper når det gjelder virkningen på totalkolesterol og LDL. MSE er signifikant mer effektivt når det gjelder å redusere både kolesterol og LDL hos kaniner og aper, enn hos mus. MSE synes å ha den samme virkning som probukol hos mus, dvs. minimal virkning om noen på disse to faktorer. MSE inhiberer imidlertid ekspresjon av VCAM-1 i alle undersøkte arter.
III. Farmasøytiske preparater
Mennesker, hester, hunder, storfe og andre dyr, og spesielt pattedyr, som lider av en av sykdomstilstandene beskrevet heri, heriblant kardiovaskulære forstyrrelser og inflammatoriske tilstander formidlet av VCAM-1, kan behandles ved tilførsel til pasienten av en effektiv mengde av en eller flere av de ovenfor identifiserte forbindelser eller et farmasøytisk aksepterbart derivat eller salt av denne i et farmasøytisk aksepterbart bærestoff eller fortynningsmiddel. De aktive stoffer kan tilføres ved enhver egnet tilførselsvei, feks. oralt, parenteralt, intravenøst, intradermalt, subkutant eller topisk.
Som benyttet heri, viser begrepet farmasøytisk aksepterbare salter eller komplekser til salter eller komplekser som bibeholder den ønskede biologiske aktivitet av de ovenfor identifiserte forbindelser og viser minimale uønskede toksikologiske virkninger. Eksempler på slike salter er (a) syreaddisjonssalter dannet med uorganiske syrer (feks. saltsyre, hydrogenbromid, svovelsyre, fosforsyre, salpetersyre og lignende), og salter dannet med organiske syrer som eddiksyre, oksalsyre, vinsyre, ravsyre, eplesyre, askorbinsyre, benzosyre, garvesyre, palmesyre, alginsyre, polyglutaminsyre, naftalensulfon-syre, naftalendisulfonsyre og polygalakturonsyre, (b) baseaddisjonssalter dannet med polyvalente metallkationer som sink, kalsium, vismut, barium, magnesium, aluminium, kobber, kobolt, nikkel, kadmium, natrium, kalium og lignende, eller med et organisk kation dannet fra N,N-dibenzyletylendiamin, D-glukosamin, ammonium, tetraetylammonium eller etylendiamin, eller (c) kombinasjoner av (a) og (b), feks. et sinktannatsalt eller lignende.
Den aktive forbindelse inngår i det farmasøytisk aksepterbare bærestoff eller fortynningsmiddel i en mengde som er tilstrekkelig til å tilføre til en pasient en terapeutisk effektiv mengde uten at alvorlige toksiske virkninger oppstår hos den behandlede pasient. En foretrukket dose av den aktive forbindelse for alle de ovenfor nevnte tilstander ligger i området fra tilnærmet 0,1 til 500 mg/kg, fortrinnsvis 1 til 100 mg/kg pr. dag. Det effektive doseringsområdet av de farmasøytisk aksepterbare derivater kan beregnes ut fra vekten av utgangsforbindelsen som skal tilføres. Dersom derivatet viser aktivitet i seg selv, kan den effektive dose beregnes som ovenfor ved å benytte vekten av derivatet, eller ved andre midler kjent blant fagfolk.
For systemisk tilførsel kan medikamentet med fordel tilføres i enhver egnet enhetsdoseform, deriblant en doseform som inneholder 1 til 3000 mg, fortrinnsvis 5 til 500 mg, av den aktive bestanddel pr. enhetsdoseform. En oral dose på 25-250 mg vil vanligvis være passende. Den aktive bestanddel bør tilføres slik at det oppnås toppkonsentrasjoner i plasma av den aktive forbindelse på tilnærmet 0,1 til 100 mM, fortrinnsvis tilnærmet 1 til 10 mM. Dette kan feks. oppnås ved intravenøs injeksjon av en løsning eller utforming av den aktive bestanddel, om ønskelig i saltvann eller et vandig medium, eller tilførsel som en bolusinjeksjon av den aktive bestanddel.
Konsentrasjonen av aktiv forbindelse i medikamentpreparatet vil avhenge av absorb-sjonshastighet, fordelingshastighet, inaktiveringshastighet og utskillelseshastighet av medikamentet såvel som av andre faktorer som er kjent blant fagfolk. Det bør bemerkes at doseverdiene også vil variere med omfanget av tilstanden som skal lettes. Det bør videre forstås at det spesifikke doseringsskjema for en gitt pasient bør justeres over tid ifølge individuelle behov og den profesjonelle vurdering til personen som tilfører eller overvåker tilførselen av preparatene, og at konsentrasjonsområdene som gis heri kun er eksempler. Den aktive bestanddel kan tilføres en gang, eller oppdeles i et antall mindre doser for tilførsel etter forskjellige tidsrom.
Orale preparater vil fortrinnsvis omfatte et inert fortynningsmiddel eller et spisbart bærestoff. De kan innkapsles i gelatinkapsler eller sammenpresses til tabletter. For oral terapeutisk tilførsel kan den aktive forbindelse inkorporeres med eksipienser og benyttes i form av tabletter, drops eller kapsler. Farmasøytisk kompatible bindemidler og/eller adjuvanter kan inngå som del av preparatet.
Tablettene, pillene, kapslene, dropsene og lignende kan inneholde enhver av følgende bestanddeler eller forbindelser av tilsvarende natur: et bindemiddel som mikrokrystallinsk cellulose, tragantgummi eller gelatin, en eksipiens som stivelse eller laktose, et disintegrasjonsmiddel som alginsyre, primogel eller maisstivelse, et smøremiddel som magnesiumstearat eller Sterotes, et glidemiddel som kolloidalt magnesiumstearat eller Sterotes, et glidemiddel som kolloidalt silisiumdioksid, et søtningsmiddel som sukrose eller sakkarin, eller et smaksmiddel som peppermynte, metylsalicylat eller appelsinsmak. Dersom enhetsdoseformen er en kapsel kan den i tillegg til stoffer av typene ovenfor inneholde et flytende bærestoff som f.eks. en fettolje. I tillegg, kan enhetsdoseformer inneholde forskjellige andre materialer som modifiserer doseringsenhetens fysiske form, f.eks. belegg av sukker, shellak eller andre enteriske midler.
Preparatene som oppnås ved anvendelsen ifølge oppfinnelsen kan tilføres som bestanddel av en eliksir, suspensjon, sirup, kjeks, tyggegummi eller lignende. En sirup kan i tillegg til de aktive forbindelser inneholde sukrose som søtningsmiddel og visse konserveringsmidler, fargestoffer og smaksstoffer.
Preparatene kan også tilføres sammen med andre aktive stoffer som ikke svekker den ønskede virkning eller med materialer som supplementerer den ønskede virkning, f.eks. antibiotika, antifungale midler, antiinflammatoriske midler eller antivirale forbindelser. Preparatene kan tilføres sammen med lipidreduserende midler som probukol og nikotinsyre, inhibitorer av blodplateaggregering som aspirin, antitrombotiske midler som coumadin, kalsiumkanalblokkere som varapamil, diltiazem og nifedipin, angiotensinkonverterende enzym (ACE)-inhibitorer som captopril og enalopril, og B-blokkere som propanalol, terbutalol og labetalol. Preparatene kan også tilføres i kombinasjon med ikke-steroide antiinflammatoriske midler, f.eks. ibuprofen, indometacin, aspirin, fenoprofen, mefenaminsyre, flufenaminsyre og sulindac. Preparatet kan også tilføres sammen med kortikosteroider.
Løsninger eller suspensjoner som benyttes for parenteral, intradermal, subkutan eller topisk tilførsel kan omfatte følgende bestanddeler: et sterilt fortynningsmiddel, f.eks. injeksjons vann, en saltløsning, ikke-flyktige oljer, polyetylenglykoler, glyserol, propylenglykol eller andre syntetiske løsemidler, antibakterielle midler som benzylalkohol eller metylparabener, antioksidanter som askorbinsyre eller natriumbisulfit, gelateringsmidler som etylendiaminotetraeddiksyre, buffere som acetater, sitrater eller fosfater, og midler for justering av tonisitet, f.eks. natriumklorid eller dekstrose. Det parenterale preparat kan innkapsles i ampuller, engangssprøyter eller flerdoseampuller fremstilt av glass eller plast.
Egnede transportører eller bærestoffer for topisk tilførsel er kjent og omfatter hudvann, suspensjoner, salver, kremer, geler, tinkturer, sprayer, pulvere, pastaer, transdermale plastere for langsom frigivelse, aerosoler for astma og stikkpiller for tilførsel til rektalt, vaginalt, nasalt eller oralt slimvev.
Tykningsmidler, bløtemidler og stabilisatorer kan benyttes for fremstilling av topiske preparater. Eksempler på tykningsmidler omfatter petrolatum, bivoks, xantangummi, eller polyetylenglykol, fuktgivende midler som sorbitol, mykemidler som mineralolje, lanolin og lanolinderivater, eller squalen. Et antall løsninger og salver er kommersielt tilgjengelige.
Naturlige eller kunstige smaksstoffer eller søtemidler kan tilsettes for å forbedre smaken av topiske preparater som tilføres for lokal virkning på slimhinneoverflater. Inerte fargestoffer kan tilsettes, særlig når det gjelder preparater som er utformet for tilførsel til orale slimhinneoverflater.
Preparatene kan fremstilles med bærestoffer som beskytter forbindelsen mot hurtig frigivelse, f.eks. et preparat for kontrollert frigivelse, heriblant implantater og mikroinnkapslede tilførselssystemer. Biodegraderbare, biokompatible polymerer kan benyttes, f.eks. etylenvinylacetat, polyanhydrider, polyglykolsyre, kollagen, polyortoestere og polymelkesyre. Mange fremgangsmåter for fremstilling av slike preparater er patentert eller generelt kjent blant fagfolk.
Ved intravenøs tilførsel er foretrukne bærestoffer fysiologisk saltvann eller fosfatbufret saltvann (PBS).

I en annen utførelse fremstilles de aktive medikamentene med bærestoffer som vil beskytte forbindelsen mot hurtig eliminering fra kroppen, f.eks. et preparat for kontrollert frigivelse, heriblant implantater og mikroinnkapslede tilførselssystemer. Biodegraderbare, biokompatible polymerer kan benyttes, f.eks. etylenvinylacetat, polyanhydrider, polyglykolsyre, kollagen, polyortoestere og polymelkesyre. Fremgangsmåter for fremstilling av slike preparater vil være åpenbare blant fagfolk. Materialene kan også erholdes kommersielt fra Alza Corporation og Nova Pharmaceuticals, Inc. Liposomsuspensjoner kan også være farmasøytisk aksepterbare bærestoffer. Claims (37)
1.
Anvendelse av en behandlingseffektiv mengde av en monoester av probukol valgt fra gruppen bestående av en monoravsyreester av probukol, en monoglutarsyreester av probukol, en monoadipinsyreester av probukol, en monokorksyreester av probukol, en monotalgsyreester av probukol, en monoazelainsyreester av probukol, og en monomaleinsyreester av probukol, eller et farmasøytisk akseptabelt salt derav for fremstilling av et medikament for behandling av en kardiovaskulær sykdom.

Anvendelse ifølge krav 1, der monoesteren er monoravsyreester av probukol.

Anvendelse ifølge krav 1, der monoesteren er monoglutarsyreester av probukol.

Anvendelse ifølge krav 1, der monoesteren er monoadipinsyreester av probukol.

Anvendelse av en behandlingseffektiv mengde av en monoester av probukol valgt fra gruppen bestående av en monoravsyreester av probukol, en monoglutarsyreester av probukol, en monoadipinsyreester av probukol, en monokorksyreester av probukol, en monotalgsyreester av probukol, en monoazelainsyreester av probukol, og en monomaleinsyreester av probukol, eller et farmasøytisk akseptabelt salt derav for fremstilling av et medikament for behandling av en inflammatorisk sykdom.
