Abstract

Description
심장혈관질병은 현재 미국에서 주요한 사망원인이다. 심장혈관질환의 약 9퍼센트가 아테롬성 동맥경화증으로 진단되고 있다. 심장혈관질환은 과콜레스테롤혈증, 과지질혈증, 혈관내피세포에서의 VCAM-1을 포함하는 몇몇 병원인자와 관련되어 있다. 혈청의 지질단백질은 순환계에 있어서 지질의 담체이다. 지질단백질은 이들의 밀도에 따라 분류된다: 킬로마이크론 (chylomicrons), 초저밀도 리포단백질 (VLDL: very low-density lipoproteins), 저밀도 지질단백질 (LDL: low-density lipoproteins), 및 고밀도 지질단백질 (HDL: high-density lipoproteins). 킬로마이크론은 주로 장으로부터 식이 트리글리세라이드와 콜레스테롤을 지방조직 및 간으로 전달하는데 참여한다. 많은 환자에 있어서는 식이요법에 더하여 하나의 저지질혈증 작용제로 충분할 것이다. 그러나, 초기 콜레스테롤 수준이 200mg/dl이상인 환자에게 있어서는 LDL수준을 50%이상을 감소시키는 치료가 필요하다. 단일 작용제로도 종종 이정도의 LDL저하를 나타내는 경우도 있으나 단지 20-30%의 감소를 나타내는 것이 훨씬 일반적이다. 따라서, 200-400mg/dl의 LDL 콜레스테롤을 갖는 가족의 이형접합체를 갖는 환자에 있어서는, 100mg/ml보다 낮은 수준의 LDL 콜레스테롤 수준을 달성하기 위해서 2 또는 종종 3가지 저지혈증 약물의 함께 사용할 필요가 있을 것이다. 담즙산 격리제 레진과 니코틴산의 병용으로 LDL수준을 45% 내지 55%로, 레진(resin)과 스타틴(statin)의 병용으로 약 50-60%로, 니코틴산과 스타틴의 병용으로 약 50-55%로, 그리고 담즙산 결합 레진, 스타틴 및 티코틴산을 함께 사용하는 3가지 약물 치료로 70%만큼 감소시킬 수 있다.
증거에 의하면, LDL의 아테롬발생성 효과는 LDL의 산화적 변형을 통해 부분ㅈ적으로 매개되는 듯하다. 프로부콜은 강력한 항산화제 특성을 가지며 LDL의 산화적 변형을 방해하는 것으로 알려져 있다. 이러한 발견과 같은 맥락에서, Carew et al. Proc.Natl.Acad. Sci. VCAM-1 의 발현

"생리적으로 절단가능한 이탈기"이라 함은 이 그룹이 부착된 분자로부터 생체내(in vivo)에서 절단가능한 부분을 말하며, 유기 또는 무기 음이온, 약리적으로 허용가능한 양이온, 아실(아세틸, 프로피오닐, 및 부티릴과 같은 (알킬)C(O)를 포함하나 이에 한정되는 것은 아님), 알킬, 포스페이트, 설페이트 및 설포네이트를 포함한다.
아테롬성 동맥경화증과 다른 심장혈관 및 염증질환의 치료용으로는, 영향받는 자리로 위치하기에 적합한 친지성(lipophlicity)을 갖는 프로부콜 모노에스테르를 선택해야 한다. 상기 화합물은 지방 축적소와 같은 낮은 전환율을 갖는 영역으로 분획화되어서는 안된다. 심장혈관질환의 치료를 위한 바람직한 일례에서, 상기 화합물의 약동역학은 중혈성 심장마비나 신부전증에 의해서 크게 영향받아서는 안된다.
상기 활성 화합물 또는 이들의 혼합물을 경구로 및 정맥으로를 포함하는 전신으로 또는 경피로를 포함하는 국소적인 방법등을 포함하나 이에 한정되지 않는, 적절한 방법으로 투여한다. 일반적인 범위의 투여량은 이틀에 한번 내지 하루에 2회내지 수회의 범위에 이르는 투여량 계획에 따라서 0.1-500mg/kg체중일 것이다. 투여기간은 단지 한번 투여하거나, 2 내지 6 개월 코스에 걸쳐서 주어진 1일 투여량을 하루에 1회 내지 2회 투여할 수 있다.
심장혈관의 치료에 있어서, 또한 관상동맥이나 다른 동맥의 혈관형성술대신에 또는 이에 이어서 살포 풍선 카테터(perfusion balloon catheter)를 사용하여 혈관벽에 상기 화합물을 직접 투여할 수도 있다. 예로서, 상기 화합물을 1-500mM 또는 상기 화합물의 혼합므로 포함하는 생리적으로 허용가능한 용액을 2-5mL을 1-5 대기압하에서 투여할 수 있다. 그런후에, 이후의 6개월동안의 코스에 걸쳐서 재협착증의 위험이 최대인 동안, 상기 활성성분을 다른 적절한 경로로 투여량 계획에 따라 투여할 수 있다.

장기간 치료는 위험이 높은 환자에 있어서 진전된 손상의 전개를 방지하기 위해서 사용된다. 장기간 치료는 수년동안 0.5-500 mg/kg체중 범위의 투여량을 이틀에 1회 내지 매일 3회씩 간격으로 투여할 수 있다.
또한, 임상적으로 중요한 재협착증을 초래할 수 있는 비정상적인 증식성 및 염증성 반응을 제거하거나 감소시키기 위한 수단으로서 관상동맥 혈관형성술의 바로직전이나 후에 상기 활성 화합물을 투여할 수 있다.
아스피린과 같은 혈소판 응집 저해제; 쿠마딘과 같은 항혈전증제; 바라파밀, 딜티아젠 및 니페디핀과 같은 칼슘 채널 차단제; 캅토프릴 및 엔알로프릴과같은 앤지오텐신 전환효소(ACE); 및 프로판알올, 테르부탈올 및 라베탈올과 같은 베타-차단제를 포함하는 심장혈관 질환의 치료에 사용되는 다른 약물과 함께 상기 활성화합물을 투여할 수도 있다. 또한 상기 화합물을 이부프로펜, 인도메타신, 페노프로펜, 메펜아민산, 플루펜아민산, 설리다크와 같은 비스테로이성 항염증제와 함께 투여할 수도 있다. 상기 화합물을 또한 코르티코스테로이드와 함께 투여할 수도 있다.
피하에 이식된 펠렛(150mg/kg/일을 방출하는 펠렛)을 이용하여 투여되는 MSE는 마우스 모델의 폐에서 VCAM-1과 MCP-1의 LPS-유도 발현을 차단한다.
6주동안 경구로 투여된 MSE(150mg/kg/일)는 뉴질랜드 화이트 래빗 모델에서 전체 혈장의 ApO-B와 HDL 콜레스테롤의 수준을 저하시킨다. 혈장 콜레스테롤에 대한 상기 효과에 아테롬성 동맥경화증 손상 형성, 매크로파지 축적 및 VCAM-1 발현을 뚜렷한 저해가 수반된다.
2주간 MSE의 경구투여로 콜레스테롤 섭취 C57 블랙과 apo-E-녹아웃 마우스 무델에서 HDL에는 영향을 주지않으면서 apo-B함유 지질단백질을 선택적으로 저하시킨다. 과콜레스테롤혈증을 갖는 시노몰거스(cynomolgous) 원숭이 모델에서 2주간 MSE의 경구투여로 전체 혈장 및 LDL콜레스테롤을 저하시키나 HDL에는 영향이 없었다.
박테리아 에임즈 테스트(Ames test)결과, MSE는 돌연변이제가 아니다. 2주간 래트에게 1000/mg/kg/의 MSE를 경구 투여한 결과 사망도 없고 혈청 전해질 및 적혈구 용적율(hematocrit)수치에도 영향이 없었다. 혈청 LDH, 알칼라인 포스파타제, SGOT 및 SGPT의 상승이 관찰되나, 비치료 군과 통계학적으로 다르지 않으며 간의 형태나 조직생리적 변화도 수반되지 않았다.
염증성 피부 질환의 치료를 위한 국소 적용을 위해서, 부착된 자리에서 충분한 치료적 효과가 나타나기에 충분한 양으로 피부에 의해 흡수되도록 선택된 화합물을 제제화하여야 한다. 프로부콜 모노에스테르는 생리적으로 허용가능해야 한다. 일반적으로, 적어도 치료지수가 2, 바람직하게는 적어도 5 또는 10인 화합물이 허용가능하다. 상기 치료지수는 EC50/IC50으로 정의되며, 여기서 EC50은 VCAM-1의 발현을 50% 저해시키는 상기 화합물의 농도이고 IC50은 표적 세포의 50%에 독성인 화합물의 농도를 의미한다. 본분야의 당업자에게 알려진 것과 같이, 직접적인 세포수 측정, 트립판 블루 배제, 또는 3H-티미딘 포함과 같은 다양한 대사 활성도의 연구 등을 사용하여 세포 독성을 측정할 수 있다.

실시예 1: 사람 동맥 내피 세포에서 VCAM-1의 발현

도 3 사람의 HAEC에서 MCP-1에 대한 50μM의 프로부콜 및 TNF과, 10μM의 프로부콜의 모노숙신산 에스테르(MSE)의 효과를 비교한 막대 그래프이다. 1시간동안 MCP-1단독으로나 10 마이크로몰의 MSE와 함께 상기 세포를 처리하였다. 상기 세포배양배지를 수화하여 색깔에 기초한 ELISA분석법을 사용하여 MCP-1의 양을 정량화하는데 사용하였다. 예시된 바와 같이, 프로부콜 모노에스테르는 프로부콜 자체보다 큰 정도로 MCP-1의 발현을 저해하였다. 실시예 4: 생체외 사람 동맥 내피 세포(HAEC)에서 유전자 발현에 미치는 프로부콜 모노숙신산 에스테르의 영향
도 4는 LPS로 처리한 ApoE녹아웃 마우스의 폐에서 분리된 RNA로부터 VCAM-1및 MCP-1 유전자 발현의 노던 블랏 분석이다. 마우스를 MSE, 프로부콜 및 위약을 90일간 400mg을 방출하는 피하펠렛으로 투여하였다. 1주일후에, 1mg/kg의 LPS을 내복막으로 이들을 자극하였다. 2시간후에, 상기 동물을 희생하여 RNA 분석을 위하여 페를 냉동하였다. 변성된 1.0% 아가로스 겔 전기영동하고, 나일론 막으로 전이하고 마우스 JE-특이적 32P라벨된 cDNA 프로브로서 혼성화하여 상기 RNA를 크기로 분별하였다. 이어서 상기 막을 벗기고 마우스 VCAM-1 특이적 cDNA로 혼성화한후에, 베타-액틴 특이적 cDNA 프로브와 혼성화하였다.
실시예 5: 지질 섭취 토끼의 혈장에서 콜레스테롤에 미치는 프로부콜 모노숙신산 에스테르의 효과
도 5는 지질 섭취 토끼에서 전체 콜레스테롤 및 지질단백질 콜레스테롤 수준에 대한 MSE와 프로부콜의 효과를 나타내는 막대그래프이다. 토끼를 0.5% wt/wtㅡNSE 또는 프로부콜을 함유하는 고지방함유 음식(chow)(0.5% 콜레스테롤 및 3% 코코넛 오일)을 3주간 섭취시켰다. 대조 동물에게는 약물을 첨가하지 않은 상기 음식과 동일한 것을 섭취시켰다. 빠른 상 액체 크로마토그래피로서 전체 혈장으로부터 지질단백질을 분리하고 콜레스테롤 함량을 분석하였다. MSE는 결과적으로 지질단백질 부분에 있어서 통계적으로 상당한 감소를 나타냈으나 프로부콜은 단지 HDL콜레스테롤만을 감소시켰다.(p〈0.05)
실시예 6: 3주간 고콜레스테롤 식이를 섭취한 토끼의 혈장에서 프로부콜과 MSE 약물수준의 효과를 비교
3주간 0.5% wt/wt 농도로 고지방함유 식이(0.5%의 콜레스테롤과 3%의 코코넛 오일)로 토끼에게 MSE와 프로부콜을 섭취시켰다. 상기 약물을 혈장으로부터 추출하고 고압액체 크로마토그래피로 분석하였다. 상기 실시예에서 나타낸 것과 같이 상기 화합물들이 혈장 콜레스테롤과 지질단백질 수준에 미치는 영향이 상당히 다른그는 하지만, 나타낸 바와 같이 프로부콜과 MSE의 수준은 유사하였다.
실시예 7: NF-kB 활성화에 대한 MSE의 효과
1시간, 2시간, 또는 4시간중 어느 하나의 기간동안, TNF 단독으로나 25 마이크로몰의 MSE 또는 PDTC와 함께 사람의 동맥 내피 세포를 처리하였다. 상기 세포를 세척하고 핵추출물을 준비하여 VCAM-1 프로모터 프로브로 겔이동분석을 수행하였다. MSE는 NF-kB의 활성화에 영향이 없으나 PDTC는 NF-kB의 활성화를 저해하였다.
실시예 8: 콜레스테롤 섭취 토끼에서 콜레스테롤 수준에 대한 6주후에 MSE의 효과
고지방 및 고콜레스테롤(0.5%) 식이를 단독으로 또는 0.5% wt/wt의 MSE 또는 프로부콜을 6주간 약 150mg/kg/일로 뉴질랜드 화이트 토끼에 섭취시켰다. 도 8은 6주후에 지질 섭취 토끼에 있어서, 전체 콜레스테롤, LDLc, VLDLc, ILDLc, HDLc 및 TG에 대한 MSE의 영향을 나타내는 막대 그래프이다. 6주후에, 빠른 상 액체 크로마토그래피로 전체 혈장으로부터 지질단백질 부분을 분리하고 콜레스테롤 및 트리글리세라이드 함량을 분석하였다. 실시예 8에서 나타낸 바와 같이, VLDL 및 IDL에 잇어서 콜레스테롤뿐만 아니라 전체 콜레스테롤을 프로부콜보다는 MSE의 처리가 더욱 저하시켰다.
실시예 9: 고콜레스테롤혈증 토끼에서 아테롬성 동맥경화증의 진전에대한 MSE의 효과
실시예 8에서 기술한 토끼를 희생하여 동맥을 얻었다. 상기 동맥을 수단-4로 염상하여 염색된 정도를 분석하였다. 도 9는 지질 섭취 토끼에 있어서, MSE로 치료한 것과 치료하지 않은 경우에 손상된 대동맥 면적의 퍼세트을 나타내는 그래프이다. MSE를 처리한 상기 토끼의 동맥은 훨씬 덜 염색되었으며, 이는 프로부콜 모노숙신상 에트테를로 처리한 것에서 아테롬성 동맥 경화증의 감소를 나타낸다.
VCAM-1 또는 Ram-11 항원에 대한 항체를 사용하여 매크로파지 축적이나 VCAM-1 발현에 대해 상기 동맥의 단면을 면역 염색하였다. MSE처리는 VCAM-1 발현과 매크로파지 축적을 뚜렷이 감소시켰다(즉 약 75%이상). 비슷한 실험에서, 동일한 투여량의 프로부콜은 훨씬 덜 효과적이었다(VCAM-1 발현과 매크로파지 축적이 25%보다 적게 감소되었다).
실시예 10: 과콜레스테롤혈증을 갖는 원숭이에서 MSE가 LDL을 가역적으로 감소시켰다.
MSE를 처리하기 전에 고지방 및 고콜레스테콜 식이를 섭취시켜 시노몰거스 원숭이를 과콜레스테롤혈증으로 만들었다. 그런후에, 상기 원숭이에게 경구로 2주간 MSE(100mg/kg/일)를 투여하였다. 이기간동아 원숭이에서 혈청 LDL 콜레스테롤 퍼세트가 4 내지 60% 만큼 감소하였다. 약물의 투여를 종료하고 29일째 혈청 콜레스테롤을 점검하였다. 상기 콜레스테롤 수치는 처리하기전 수준으로 회복되었고 이를 유지하였다.

전체 콜레스테롤과 LDL의 효과면에서, 마우스 대 토끼와 원숭이에서 MSE 및 프로부콜의 효과에 상이가 있음을 주목할 것이다. MSE는 마우스에 비해서 토끼 및 원숭이에서 콜레스테롤과 LDL 모두에 대해서 감소효과가 더 크다. MSE는 마우스에서, 프로부콜과 동일한 효과, 즉 최소의 효과를 갖는 것 같으며, 어쨋든 이들 두 인자에 영향을 가진다. 그러나, MSE는 모든 테스트 종에서 VCAM-1의 발현을 저해한다.
약리적 조성물
VCAM-1에 의해 매개되는 심장혈간질환 및 염증성 상태본명세서에 기술한 어떠한 질병을 가진 사람, 말, 고양이, 소 및 다른 동물들, 특히 포유동물에게 유효량의 상기 기술한 화합물을 하나이상 또는 허용가능한 유도체 또는 이들의 염을 약리적으로 허용가능한 담체나 희석제로 하여 투여함으로써 이러한 질병을 치료할 수 있다. 활성물질을 적합한 경로, 예를 들면 경구, 비경구, 경피, 정맥, 피하, 또는 국소적으로 투여할 수 있다.

약리적으로 허용가능한 담체 또는 희석제에, 치료대상 환자에게 심각한 독성효과를 나타내지 않으면서치료적으로 유효한 양을 환자에게 전달하기에 충분한 양으로 활성화합물을 포함한다. 상술한 모든 조건에 대한 활성성분의 바람직한 투여량은 하루에 약 0.1-500 mg/kg, 바람직하게는 약 1-100 mg/kg범위이다. 전달될 모화합물의 중량에 기초하여, 약리적으로 허용가능한 유도체의 유효투여량의 범위를 결정할 수 있다. 만약 상기 유도체가 그자체로 활성을 나타내면, 상기 유도체의 중량을 사용하거나 본기술분야의 전문가에게 알려진 다른 수단으로 유효 투여량을 추정할 수 있다.

국소적용에 적합한 운반체나 담체는 알려져 있으며, 로션, 현탁액, 연고, 크림, 겔, 팅크(tincture), 스프레이, 분말, 풀형태, 서방형 경피 패치, 천식을 위한 에어로졸, 및 직장의, 협막의, 코의 또는 구강의 점막에 사용하기 위한 좌약형태를 포함한다.
