Abstract
Aerosol por condensación que contiene un producto sedante-hipnótico seleccionado de entre el grupo que comprende zaleplón, zolpidem y zopiclona, en donde: a) el citado aerosol por condensación comprende partículas que comprenden menos del 10 por ciento en peso de productos de degradación sedantes-hipnóticos; y b) el citado aerosol por condensación tiene un MMAD de menos de 5 mum.
Description
Suministro de productos
sedantes-hipnóticos a través de una vía de
inhalación.
La presente invención está relacionada con el
suministro de productos sedantes-hipnóticos a través
de una vía de inhalación. Específicamente, la misma está
relacionada con aerosoles que contienen productos
sedantes-hipnóticos que son utilizados en terapia de
inhalación.
Existe un determinado número de composiciones
comercializadas actualmente como productos
sedantes-hipnóticos. Las composiciones contienen al
menos un ingrediente activo que proporciona los efectos terapéuticos
observados. Entre los ingredientes activos proporcionados en
composiciones sedantes-hipnóticas se encuentran el
zolpiden, el zaleplón y la zopiclona.
El documento WO 0224158 escribe formulaciones de
aerosol de polvo seco de zolpidem y zaleplón. La patente US5388574
describe aerosoles que comprenden fármacos contra la ansiedad o la
hiperactividad.
Resulta aconsejable proporcionar una nueva vía
de administración para los productos
sedantes-hipnóticos, la cual produzca rápidamente
concentraciones pico en plasma del compuesto. La provisión de la
citada vía constituye un objetivo de la presente invención.
La presente invención está relacionada con el
suministro de productos sedantes-hipnóticos a través
de la vía de inhalación. Específicamente, la misma está relacionada
con aerosoles que contienen productos
sedantes-hipnóticos, los cuales son utilizados en
terapia de inhalación.
En un aspecto de la composición de la presente
invención, el aerosol comprende partículas que tienen al menos un
5% en peso de un producto sedante-hipnótico.

suministrado da lugar a un pico de concentración en plasma de un
producto sedante-hipnótico en el mamífero, en menos
de 1 hora. Preferiblemente, la concentración pico en plasma se
alcanza en menos de 0,5 horas. Más preferiblemente, la concentración
pico en plasma se alcanza en menos de 0,2, 0,1, 0,05, 0,02, 0,01 ó
0,005 horas (medición arterial).
Habitualmente, el aerosol por condensación
suministrado se utiliza para tratar el insomnio.
En otro aspecto procedimental de la presente
invención, uno de los productos zaleplón, zolpidem o zopiclona es
suministrado a un mamífero a través de una vía de inhalación. El
procedimiento comprende: a) calentar una composición, en donde la
composición comprende al menos el 5 por ciento en peso de zaleplón,
zolpidem o zopiclona, para formar un vapor; y: b) dejar que el
vapor se enfríe, formando como consecuencia un aerosol por

mamífero en una única inspiración. Más preferiblemente, entre 1 mg
y 10 mg de fármaco son suministrados al mamífero en una única
inspiración.
Habitualmente, el aerosol por condensación
suministrado da lugar a un pico de concentración en plasma de
zaleplón, zolpidem o zopiclona en el mamífero, en menos de 1 hora.
Preferiblemente, la concentración pico en plasma se alcanza en
menos de 0,5 horas. Más preferiblemente, la concentración pico en
plasma se alcanza en menos de 0,2, 0,1, 0,05, 0,02, 0,01 ó 0,005
horas (medición arterial).
Habitualmente, el aerosol por condensación
suministrado se utiliza para el tratamiento del insomnio.
En un aspecto del kit de la presente invención,
se proporciona un kit para el suministro de un producto
sedante-hipnótico a través de una vía de inhalación,
que comprende: a) una composición que comprende al menos el 5% por
ciento en peso de un producto sedante-hipnótico; y
b) un dispositivo que forma un aerosol
sedante-hipnótico a partir de la composición, para
ser inhalado por el mamífero. Preferiblemente, la composición
comprende al menos el 20 por ciento, el 30 por ciento, el 40 por
ciento, el 50 por ciento, el 60 por ciento, el 70 por ciento, el 80
por ciento, el 90 por ciento, el 95 por ciento, el 97 por ciento, el
99 por ciento, el 99,5 por ciento, el 99,99 por ciento o el 99,97
por ciento en peso de un producto
sedante-hipnótico.
El dispositivo contenido en el kit comprende: a)
un elemento para calentar la composición de producto
sedante-hipnótico para formar un vapor; b) un
elemento que permite que el vapor se enfríe para formar un aerosol;
y c) un elemento que permite que el mamífero inhale el aerosol.
En otro aspecto del kit de la presente
invención, se proporciona un kit para el suministro a un mamífero
de zaleplón, zolpidem o zopiclona a través de una vía de inhalación,
que comprende: a) una composición que comprende al menos el 5% por
ciento en peso de zaleplón, zolpidem o zopiclona; y b) un
dispositivo que forma un aerosol de zaleplón, zolpidem o zopiclona
a partir de la composición, para ser inhalado por el mamífero.
Preferiblemente, la composición comprende al menos el 20 por ciento,
el 30 por ciento, el 40 por ciento, el 50 por ciento, el 60 por
ciento, el 70 por ciento, el 80 por ciento, el 90 por ciento, el 95
por ciento, el 97 por ciento, el 99 por ciento, el 99,5 por ciento,
el 99,9 por ciento o el 99,97 por ciento en peso de zaleplón,
zolpidem o zopiclona.
El dispositivo contenido en el kit comprende: a)
un elemento para calentar la composición de zaleplón, zolpidem o
zopiclona para formar un vapor; b) un elemento que permite que el
vapor se enfríe para formar un aerosol; y c) un elemento que
permite que el mamífero inhale el aerosol.
La Figura 1 muestra una vista transversal de un
dispositivo utilizado para suministrar aerosoles
sedantes-hipnóticos a un mamífero, a través de una
vía de inhalación.
"Diámetro aerodinámico" de una partícula
determinada, se refiere al diámetro de una gotita esférica con una
densidad de 1 g/mL (la densidad de agua), que tiene la misma
velocidad de sedimentación que la partícula determinada.
"Aerosol" se refiere a una suspensión de
partículas sólidas o líquidas en un gas.
"Densidad de masa de fármaco en aerosol" se
refiere a la masa de producto sedante-hipnótico por
unidad de volumen de aerosol.
"Densidad de masa de aerosol" se refiere a
la masa de material particulado por unidad de volumen de
aerosol.
"Densidad de partícula de aerosol" se
refiere al número de partículas por unidad de volumen de
aerosol.
"Partícula amorfa" se refiere a una
partícula que no contiene más del 50 por ciento en peso de una
forma cristalina. Preferiblemente, la partícula no contiene más del
25 por ciento en peso de una forma cristalina. Más preferiblemente,
la partícula no contiene más del 10 por ciento en peso de una forma
cristalina.
"Aerosol por condensación" se refiere a un
aerosol formado por vaporización de una sustancia seguida de
condensación de la sustancia en un aerosol.
"Densidad de masa de fármaco en aerosol
inhalable" se refiere a la densidad de la masa de fármaco en
aerosol producida por un dispositivo de inhalación y suministrado en
un volumen corriente en un paciente típico.
"Densidad de masa en aerosol inhalable" se
refiere a la densidad de masa de aerosol producida por un
dispositivo de inhalación y suministrado en un volumen corriente en
un paciente típico.
"Densidad de partícula de aerosol
inhalable" se refiere a la densidad de partícula del aerosol, de
partículas de tamaño comprendido entre 100 nm y 5 micras, producidas
por un dispositivo de inhalación y suministradas en un volumen
corriente de paciente habitual.
"Diámetro aerodinámico medio de la masa" o
"MMAD" de un aerosol se refiere al diámetro aerodinámico para
el cual la mitad de la masa particulada del aerosol está constituida
por partículas con un diámetro aerodinámico superior al de MMAD y
la mitad por partículas con un diámetro aerodinámico inferior al de
MMAD.
"Velocidad de formación de aerosol" se
refiere a la masa de material en forma de partículas aerosolizadas
producida por un dispositivo de inhalación por unidad de tiempo.
"Velocidad de formación de partícula de
aerosol inhalable" se refiere al número de partículas de tamaño
comprendido entre 100 nm y 5 micras producido por un dispositivo de
inhalación por unidad de tiempo.
"Velocidad de formación de aerosol para
fármaco", se refiere a la masa de producto aerosolizado
sedante-hipnótico producida por un dispositivo de
inhalación por unidad de tiempo.
"Velocidad de sedimentación" se refiere a
la velocidad terminal de una partícula de aerosol que sufre
sedimentación gravitacional en el aire.
"Producto de degradación
sedante-hipnótico" se refiere a un compuesto que
resulta de una modificación química de un producto
sedante-hipnótico. La modificación, por ejemplo,
puede ser el resultado de una reacción inducida termalmente o
fotoquímicamente. Entre las citadas reacciones se incluyen, sin que
ello represente ninguna limitación, la oxidación y la
hidrólisis.
"Volumen corriente de paciente habitual" se
refiere a 1L para un paciente adulto y 15 mL/kg para un paciente
pediátrico.
"Vapor" se refiere a un gas, y "fase
vapor" se refiere a una fase gaseosa. El término "vapor
térmico" se refiere a una fase vapor, aerosol, o mezcla de fases
vapor-aerosol, formada(s) preferiblemente por
calentamiento.
"Zaleplón" se refiere al compuesto
N-[3-(3-cianopirazolo[1,5-a]pirimidin-7-il]fenil]-N-etilacetamida,
que es una base libre.
"Producto de degradación de zaleplón" se
refiere a un compuesto que resulta de una modificación química de
zaleplón. La modificación puede ser, por ejemplo, el resultado de
una reacción inducida termalmente o fotoquímicamente. Entre las
citadas reacciones se incluyen, sin que ello suponga ningún límite,
la oxidación y la hidrólisis. El C_{13}H_{9}N_{5}
(destilación y desamidación para proporcionar un resto anilina no

zolpidem. La modificación puede ser, por ejemplo, el resultado de
una reacción inducida termalmente o fotoquímicamente. Entre las

(eliminación de amida) constituye un ejemplo de producto de
degradación.
"Zopiclona" se refiere al éster

zolpidem. La modificación puede ser, por ejemplo, el resultado de
una reacción inducida termalmente o fotoquímicamente. Entre las
citadas reacciones se incluyen, sin que ello suponga ningún límite,
la oxidación y la hidrólisis. Entre los ejemplos de productos de
degradación se incluyen la
2-amino-5-cloropiridina
y la 1-metilpiperazina.
Para formar los aerosoles de la presente
invención se utiliza cualquier procedimiento que resulte adecuado.
No obstante, un procedimiento preferido conlleva el calentamiento de
una composición que comprende un producto
sedante-hipnótico para formar un vapor, seguido de
enfriamiento del vapor, de tal forma que el mismo condensa para
proporcionar un aerosol que comprende un producto
sedante-hipnótico (aerosol por condensación). La
composición es calentada de una de entre 4 posibles formas: como
compuesto activo puro (a saber, zaleplón, zolpidem o zopiclona
puros); en forma de mezcla de compuesto activo y de un excipiente
farmacéuticamente aceptable; en forma de sal del compuesto activo
puro; y, en forma de mezcla de sal de compuesto activo puro y un
excipiente farmacéuticamente aceptable.
Los productos
sedantes-hipnóticos en forma de sal (por ejemplo, de
zaleplón, zolpidem o zopiclona) o bien se encuentran disponibles
comercialmente o se obtienen a partir de la correspondiente base
libre utilizando procedimientos bien conocidos en la técnica. Para
proceder a la aerosolización, se pueden utilizar una diversidad de
sales farmacéuticamente aceptables. Entre las citadas sales se
encuentran sin que ello suponga ninguna limitación, las siguientes:
las sales de ácido clorhídrico, las de ácido bromhídrico, las de
ácido acético, las de ácido maleico, las de ácido fórmico y las de
ácido fumárico.
Los excipientes farmacéuticamente aceptables
pueden ser volátiles o no volátiles. Los excipientes volátiles,
cuando son calentados, son concurrentemente volatilizados,
aerosolizados e inhalados con el producto
sedante-hipnótico. Diversas clases de los citados
excipientes son conocidos en el estado de la técnica e incluyen, sin
que ello suponga ninguna limitación, disolventes gaseosos, fluidos
supercríticos, disolventes líquidos y disolventes sólidos. La
siguiente lista proporciona ejemplos de materiales soporte dentro de
las clases: agua; terpenos, tales como el mentol; alcoholes, tales
como el etanol, el propilenglicol, el glicerol y otros alcoholes
similares; dimetilformamida; dimetilacetamida; cera; dióxido de
carbono supercrítico; hielo seco y mezclas de los mismos.
Los soportes sólidos sobre los cuales se
calienta la composición adoptan diversas formas. Entre los ejemplos
de las citadas formas se incluyen, sin que ello suponga ninguna
limitación, cilindros de menos de 1,0 mm de diámetro, cajas de
menos de 1 mm de espesor y virtualmente cualquier tipo de forma
impregnada de pequeños poros (a saber, de menos de 1,0 mm de
tamaño). Preferiblemente, los soportes sólidos proporcionan una
proporción superficie a volumen elevada (por ejemplo, superior a 100
por metro) y una proporción superficie a masa elevada (por ejemplo,
superior a 1 cm^{2} por gramo).
Un soporte sólido de una determinada forma puede
ser también transformado en otra forma con diferentes propiedades.
Por ejemplo, una lámina plana de 0,25 mm de espesor tiene una
proporción superficie a volumen de aproximadamente 8.000 por metro.
Enrollando la lámina para formar un cilindro hueco de 1 cm de
diámetro produce un soporte que retiene la elevada proporción
superficie a masa de la lámina original, pero tiene una proporción
inferior de superficie a volumen (aproximadamente 400 por
metro).
Para construir los soportes sólidos se utilizan
un determinado número de diferentes materiales. Entre las diversas
clases de los citados materiales se incluyen, sin que ello suponga
ninguna limitación, metales, materiales inorgánicos, materiales
carbonosos y polímeros. Se proporcionan seguidamente ejemplos de las
clases de materiales: aluminio, plata, oro, acero inoxidable, cobre
y tungsteno; sílice, vidrio, silicio y alúmina; grafito, carbonos
porosos, fibras de carbono y fieltros de carbono;
politetrafluoroetileno y polietilenglicol. Se utilizan también
combinaciones de materiales y variantes revestidas de los
mismos.
Cuando se utiliza aluminio como soporte sólido,
la lámina de aluminio constituye un material adecuado. Entre los
ejemplos de materiales basados en sílice, alúmina y silicio, se
incluyen la sílice amorfa S-5631 (Sigma, St. Louis,
MO), la BCR171 (una alúmina de área superficial definida superior a
los 2 m^{2}/g procedente de Aldrich, St. Louis, MO) y un disco de
silicio como el utilizado en la industria de los semiconductores.
Las fibras y los fieltros de carbono están disponibles en American
Kynol, Inc, New York, NY. Resinas de cromatografía como el
octadecilsilano, unido covalentemente a sílice porosa, constituyen
ejemplos de variantes revestidas de sílice.
El calentamiento de las composiciones
sedantes-hipnóticas se lleva a cabo utilizando
cualquier procedimiento que resulte adecuado. Entre los ejemplos de
procedimientos a través de los cuales puede generarse calor se
incluyen los siguientes: paso de corriente a través de un elemento
que es una resistencia eléctrica; absorción de radiación
electromagnética, tal como microondas o luz de láser; y, reacciones
químicas exotérmicas, tales como la solvatación exotérmica, la
hidratación de materiales piróforos y la oxidación de materiales
combustibles.
Los aerosoles que contienen productos
sedantes-hipnóticos de la presente invención son
suministrados a un mamífero utilizando un dispositivo de
inhalación. Cuando el aerosol es un aerosol por condensación, el
dispositivo tiene al menos tres elementos: un elemento para el
calentamiento de una composición que contiene un producto
sedante-hipnótico para formar vapor; un elemento que
permite el enfriamiento del vapor, proporcionando con ello un
aerosol por condensación; y, un elemento que permite al mamífero
inhalar el aerosol. Anteriormente se han descrito diversos
procedimientos de calentamiento que resultan adecuados. El elemento
que permite el enfriamiento es, en su forma más simple, una
pasarela inerte que une el medio de calentamiento con el medio de
inhalación. El elemento que permite la inhalación es un portal de
salida del aerosol, que forma una conexión entre el elemento de
enfriamiento y el sistema respiratorio del mamífero.
Haciendo referencia a la Figura 1, se describe
un dispositivo utilizado para suministrar un aerosol que contiene
un producto sedante-hipnótico. El dispositivo de
suministro 100 tiene una terminación próxima 102 y una terminación
distal 104, un módulo de calentamiento 106, una fuente de energía
108 y una pieza bucal 110. Una composición
sedante-hipnótica es depositada sobre una superficie
112 de un módulo de calentamiento 106. Tras la activación de un
interruptor 114, llevada a cabo por un usuario, la fuente de
suministro 108 inicia el calentamiento del módulo de calentamiento
106 (por ejemplo, a través de ignición de combustible fuel o paso
de corriente a través de un elemento de calentamiento resistivo). La
composición de producto sedante-hipnótico se
volatiliza debido al calentamiento del módulo de calentamiento 106 y
condensa para proporcionar un aerosol por condensación, con
anterioridad a alcanzar la pieza bucal 110 en el extremo próximo al
del dispositivo 102. Una corriente de aire que viaja desde el
extremo distal del dispositivo hasta la pieza bucal 110, transporta
el aerosol por condensación hacia la pieza bucal 110, donde el mismo
es inhalado por el mamífero.
Si se desea, los dispositivos contienen una
diversidad de componentes para facilitar el suministro de aerosoles
que contienen producto sedante-hipnótico. Por
ejemplo, el dispositivo puede incluir cualquier componente conocido
en el estado de la técnica para controlar la pauta temporal de
aerosolización del fármaco, en relación con la inhalación (por
ejemplo, actuación mediante respiración), para proporcionar
información a los pacientes en relación con la velocidad o el
volumen de inhalación, para evitar el uso excesivo (a saber, la
característica de cierre), para evitar el uso de individuos no
autorizados, y/o para registrar episodios de dosificación.
La cantidad de dosificación de los productos
sedantes-hipnóticos en forma aerosol es generalmente
no superior a dos veces la dosis estándar de un fármaco
suministrado por vía oral. Por ejemplo, el zaleplón, el zolpidem y
la zopiclona son administrados por vía oral a concentraciones de 5
mg o de 10 mg, para el tratamiento del insomnio. Como aerosoles,
entre 0,5 y 40 mg de los compuestos son generalmente proporcionados
por inspiración, para la misma indicación. Una dosificación
habitual de un aerosol con producto
sedante-hipnótico es, o bien administrada en forma
de inhalación única, o en forma de series de inhalaciones llevadas a
cabo en el espacio temporal de una hora o menos (la dosificación
equivales a la suma de las cantidades inhaladas). Cuando el fármaco
es administrado en forma de series de inhalaciones, una cantidad
diferente puede ser administrada en cada una de las
inhalacio-
nes.

La dosis apropiada de aerosoles que contienen
productos sedantes-hipnóticos para el tratamiento de
una determinada dolencia puede ser determinada utilizando
procedimientos tales como los experimentos con animales y ensayos
clínicos de determinación de dosis (Fases I/II). Un experimento con
animales conlleva la medición de las concentraciones en plasma del
fármaco en un animal, después de su exposición al aerosol. En los
citados estudios se utilizan habitualmente mamíferos tales como
perros o primates, dado que sus sistemas respiratorios son
similares a los de los humanos. Los niveles de las dosis iniciales
de comprobación en humanos son generalmente inferiores o iguales a
las dosis en el modelo animal que dieron lugar a niveles de fármaco
en plasma asociados con un efecto terapéutico en humanos. Se lleva
después a cabo el escalado de dosis en humanos, hasta que, o bien
se obtiene una respuesta terapéutica óptima o una dosis limitadora
de la toxicidad.
La pureza de un aerosol que contiene un producto
sedante-hipnótico es determinada utilizando una
diversidad de procedimientos, ejemplos de los cuales se describen
en Sekine et al., Journal of Forensic Science
32:1271-1280 (1987) y Martin et al., Journal
of Analytic Toxicology 13: 158-162 81989). Un
procedimiento conlleva la formación de aerosol en un dispositivo a
través del cual se mantiene el flujo de un gas (por ejemplo, flujo
de aire), generalmente a una velocidad de entre 0,4 y 60 L/min. El
flujo de gas transporta el aerosol hacia una o más trampas. Una vez
aislado de la trampa, el aerosol es sometido a una técnica
analítica, tal como cromatografía de gas o líquida, que permite la
determinación de la pureza de la composición.
Para la recogida del aerosol se utilizan
diferentes trampas. La siguiente lista contiene ejemplos de las
citadas trampas: filtros; lana de vidrio; impactadores; trampas de
disolventes, tales como trampas con etanol enfriado con hielo seco,
metanol, acetona y trampas co diclorometano a diversos valores de
pH; jeringas que toman muestras del aerosol; recipientes a baja
presión (por ejemplo, vacío), vacíos, hacia cuyo interior es
arrastrado el aerosol; y, recipientes vacíos que rodean por completo
e incluyen el dispositivo que genera el aerosol. Cuando se utiliza
un sólido, tal como lana de vidrio, el mismo es habitualmente
extraído con un disolvente, tal como etanol. El extracto de
disolvente es sometido a análisis, además del propio sólido (por
ejemplo, lana de vidrio). Cuando se utiliza una jeringa o un
recipiente, el recipiente es extraído de forma similar con un
disolvente.
El cromatógrafo de gas o de líquido discutido
anteriormente contiene un sistema de detección (a saber, un
detector). Los citados sistemas de detección son bien conocidos en
el estado de la técnica e incluyen, por ejemplo, detectores de
ionización en llama, de absorción fotónica y de espectrometría de
masas. Una ventaja de un detector de espectrometría de masas reside
en el hecho de que el mismo puede ser utilizado para determinar la
estructura de los productos de degradación
sedantes-hipnóticos.
La distribución por tamaño de partícula de un
aerosol que contiene un producto sedante-hipnótico
se determina utilizando un procedimiento adecuado en el estado de
la técnica (por ejemplo, un impacto en casada). Un sistema
utilizado en estudios de impacto en cascada es el Andersen Eight
Stage Non-viable Cascade Impactor (Andersen
Instruments, Smyrna, GA), conectado a un horno a través de una falsa
entrada (USP throat, Andersen Instruments, Smyrna,GA).
La densidad de masa de aerosol inhalable es
determinada, por ejemplo, suministrando un aerosol que contiene un
fármaco en el interior de una cámara confinada, a través de un
dispositivo de inhalación y midiendo la masa recogida en la cámara.
Habitualmente, el aerosol es arrastrado hacia el interior de la
cámara por medio de un gradiente de presión entre el dispositivo y
la cámara, en donde la cámara tiene una presión más baja que la del
dispositivo. El volumen de la cámara debería ser aproximadamente el
volumen corriente de un paciente que inhale.
La densidad de masa del fármaco de aerosol
inhalable de determina, por ejemplo, suministrando un aerosol que
contiene un fármaco en el interior de una cámara confinada, a través
de un dispositivo de inhalación y midiendo la masa recogida en la
cámara. Habitualmente, el aerosol es arrastrado hacia el interior de
la cámara por medio de un gradiente de presión entre el dispositivo
y la cámara, en donde la cámara tiene una presión más baja que la
del dispositivo. El volumen de la cámara debería ser aproximadamente
igual al volumen corriente de un paciente que inhale. La cantidad
de compuesto fármaco activo recogida en la cámara se determina
mediante la extracción de la cámara, el desarrollo de análisis de
cromatografía del extracto y la comparación de los resultados de
los análisis cromatográficos con los de un estándar que contiene
cantidades conocidas de fármaco.
La densidad de partícula de aerosol inhalable se
determina, por ejemplo, mediante el suministro de un fármaco en
fase aerosol en el interior de una cámara confinada, a través de un
dispositivo de inhalación, y la medición del número de partículas
de un tamaño determinado recogidas en la cámara. El número de
partículas de un determinado tamaño puede ser medido directamente
en base a las propiedades fotodispersoras de las partículas.
Alternativamente, el número de partículas de un determinado tamaño
se determina midiendo la masa de las partículas dentro del tamaño
de partícula considerado y calculando el número de partículas
basadas en la masa, del siguiente modo: Número total de partículas
= suma (desde el tamaño de partícula 1 al tamaño de partícula N) del
número de partículas en cada uno de los tamaños de partícula. El
número de partículas en una banda de tamaño determinado = masa en
la banda de tamaño/masa de una partícula habitual en la banda de
tamaño. La masa de una partícula habitual en una banda de tamaño
determinada= \pi^{\text{*}}D^{3\text{*}}\varphi/6, en donde
D es un diámetro de partícula habitual en la banda de tamaños
(generalmente, la media marginal de MMADs que define la banda de
tamaños) en micras, \varphi es la densidad de partícula (en g/mL)
y la masa se proporciona en unidades de picogramos
(gr^{-12}).
La velocidad de formación de partícula de
aerosol inhalable se determina, por ejemplo, mediante el suministro
del fármaco en fase aerosol en el interior de una cámara confinada,
a través de un dispositivo de inhalación. El suministro se programa
durante un período de tiempo (por ejemplo, 3 segundos) y se
determina el número de partículas de un determinado tamaño
recogidas en la cámara, tal y como se ha indicado anteriormente. La
velocidad de formación de partícula es igual al número de partículas
recogidas de entre 100 nm y 5 micras, dividido por el tiempo que ha
durado la recolección.
La velocidad de formación de aerosol se
determina, por ejemplo, mediante el suministro del fármaco en fase
aerosol en el interior de una cámara confinada, a través de un
dispositivo de inhalación. El suministro se programa durante un
período de tiempo determinado (por ejemplo, 3 segundos) y la masa
del material en forma de partículas se determina pesando la cámara
confinada antes y después del suministro del material en forma de
partícula. La velocidad de formación de aerosol es igual al
incremento en la masa en la cámara dividido por la duración del
tiempo de recogida. Alternativamente, cuando un cambio en la masa
del dispositivo de suministro o de uno de sus componentes puede
únicamente tener lugar a través de la liberación del material en
forma de partícula en fase aerosol, la masa del material en forma
de partícula puede ser igualada con la masa perdida desde el
dispositivo o componente, durante el suministro del aerosol. En este
caso, la velocidad de formación de aerosol es igual al descenso en
la masa del dispositivo o componentes durante la fase de suministro,
dividido por la duración de la fase de sumi-
nistro.

el aerosol es producto sedante-hipnótico puro, la
cantidad de fármaco recogida en la cámara se mide tal y como se ha
descrito anteriormente. La velocidad de formación de aerosol de
fármaco es igual a la cantidad de producto
sedante-hipnótico recogida en la cámara, dividida
por la duración del tiempo de recogida. Cuando el aerosol que
contiene producto sedante-hipnótico comprende un
excipiente farmacéuticamente aceptable, la multiplicación de la
velocidad de formación de aerosol por el porcentaje de producto
sedante-hipnótico proporciona la velocidad de
formación de aerosol.

Los aerosoles que contienen producto
sedante-hipnótico de la presente invención son
utilizados habitualmente para el tratamiento del insomnio. Otros
usos incluyen, sin que ello represente ninguna limitación, los
siguientes: como anticonvulsivos; ansiolíticos; y como
miorrelajantes.
Los siguientes ejemplos intentan ilustrar, más
que limitar, la presente invención.
El zolpidem y la zopiclona se encuentran
disponibles comercialmente en Sigma
(www.sigma-aldrich.com). El zaleplón se encuentra
disponible en forma de cápsulas (SONATA®) y puede ser aislado
utilizando procedimientos estándar del estado de la técnica.

1
Una solución de 5,5 mg de zaleplón en
aproximadamente 120 \muL de diclorometano fue revestida sobre una
pieza de hoja de aluminio de 3 cm x 8 cm. El diclorometano se dejó
evaporar. La hoja revestida fue envuelta alrededor de un tubo
halógeno de 300 vatios (Feit Electric Company, Pico Rivera, CA), el
cual fue insertado en el interior de un tubo de vidrio sellado en
uno de sus extremos con un tapón de caucho. Se hizo circular una
corriente alterna de 60 V (controlada a través de una conducción
eléctrica controlada a su vez por un autotransformador) a través
del bulbo durante un período de 7 segundos, obteniéndose vapor
térmico de zaleplón (incluyendo aerosol de zaleplón), el cual fue
recogido sobre las paredes del tubo de vidrio. El análisis mediante
cromatografía HPLC de fase inversa con detección por absorción de
luz de 225 nm demostró que el material recogido resultaba ser

Una solución de 10,7 mg de zolpidem en 100
\muL de diclorometano fue extendida en forma de capa delgada
sobre la parte central de una lámina de hoja de aluminio de 3,5 cm
x 7 cm. El diclorometano fue dejado evaporar. La hoja de aluminio
fue envuelta alrededor de un tubo halógeno de 300 vatios, el cual
fue insertado en el interior de un tubo de vidrio en forma de T.
Las oberturas del tubo fueron selladas con parafilm, el cual fue
pinchado con quince agujas para dejar pasar el aire. La tercera
obertura fue conectada a un matraz de vidrio de 3 cuellos, de 1L de
capacidad. El matraz de vidrio fue conectado adicionalmente a un
pistón grande, capaz de arrastrar 1,1 litros de aire a través del
matraz. Se hizo circular una corriente alterna a través del bulbo
de halógeno, mediante la aplicación de 90 V, utilizando una línea de
energía de 110 V conectada a un autotransformador. Transcurrido 1
segundo apareció un aerosol y fue arrastrado hacia el interior de un
matraz de 1 L de capacidad por medio de la utilización de un
pistón, terminando la recogida del aerosol una vez transcurridos 6
segundos. El aerosol fue analizado mediante la conexión del matraz
de 1 L de capacidad con un impactador en cascada no viable Andersen
de 8 niveles. Los resultados se muestran en la tabla 1. El MMAD del
aerosol recogido era de 2,9 micras, con una desviación estándar
geométrica de 2,1. También se muestra en la tabla 1 el número de
partículas recogidas en los diversos niveles del impactador en
cascada, proporcionado por la masa recogida sobre el nivel dividida
por la masa de una partícula habitual atrapada sobre este nivel. La
masa de una partícula única de diámetro D es proporcionada por el
volumen de la partícula, \piD^{3}/6, multiplicado por la
densidad del fármaco (considerada como 1 g/cm^{3}). La densidad de
partícula de aerosol inhalable es la suma de los números de las
partículas recogidas sobre los estadios del impactador 3 a 8,
dividido por el volumen de recogida de 1 L, proporcionando una
densidad de partícula de aerosol inhalable de 3,9 x 10^{6}
partículas/mL. La velocidad de formación de partícula de aerosol
inhalable es la suma de los números de las partículas recogidas
sobre los niveles 3 a 8 del impactador, dividida por el tiempo de
formación de 6 segundos, proporcionando una velocidad de formación
de partícula de aerosol inhalable de 6,4 x 10^{8}

Una solución de 8,3 mg de zolpidem en 100 \muL
de diclorometano fue extendida en forma de capa fina sobre la parte
central de una lámina de hoja de aluminio de 3,5 x 7 cm. Se dejó
evaporar el diclorometano. La hoja de aluminio fue envuelta
alrededor de un tubo halógeno de 300 vatios, el cual fue insertado
en el interior de un tubo de vidrio en forma de T. Las oberturas
del tubo fueron selladas con parafilm, el cual fue pinchado con
quince agujas para dejar pasar el aire. La tercera obertura fue
conectada a un matraz de vidrio de 3 cuellos, de 1 L de capacidad.
El matraz de vidrio fue conectado adicionalmente a un pistón grande,
capaz de arrastrar 1,1 litros de aire a través del matraz. Se hizo
circular una corriente alterna a través del bulbo de halógeno,
mediante la aplicación de 90 V, utilizando una línea de energía de
110 V conectada a un autotransformador. Transcurridos unos
segundos, apareció un aerosol y fue arrastrado hacia el interior de
un matraz de 1 L de capacidad, por medio de la utilización de un
pistón, terminando la recogida del aerosol una vez transcurridos 6
segundos. Se dejó sedimentar el aerosol sobre las paredes del matraz
de 1L durante un período de aproximadamente 30 minutos. El matraz
fue entonces extraído con acetonitrilo y se analizó el extracto a
través de HPLC con detección a través de absorción de luz a 225 nm.
La comparación con estándares que contenían cantidades conocidas de
zolpidem reveló que 3,7 mg de zolpidem, de pureza superior al 97%,
habían siso recogidos en el matraz, dando lugar a una densidad de
masa de fármaco aerosol de 3,7 mg/L. La hoja de aluminio, la cual
había sido previamente revestida con zolpidem, fue pesada como
continuación del experimento. De los 8,3 mg revestidos inicialmente
sobre el aluminio, se averiguó que 7,4 mg del material se habían
aerosol izado en un periodo de tiempo de 6 segundos, implicando una
velocidad de formación de aerosol de fármaco de 1,2 mg/segundo.

Claims (25)
1. Aerosol por condensación que contiene un
producto sedante-hipnótico seleccionado de entre el
grupo que comprende zaleplón, zolpidem y zopiclona, en donde:
- a)
- el citado aerosol por condensación comprende partículas que comprenden menos del 10 por ciento en peso de productos de degradación sedantes-hipnóticos; y
- b)
- el citado aerosol por condensación tiene un MMAD de menos de 5 \mum.
2. Aerosol por condensación según la
reivindicación 1, formado por volatilización del producto
sedante-hipnótico en condiciones eficaces para
producir un vapor del producto sedante-hipnótico y
condensación del vapor para formar las citadas partículas.
3. Aerosol por condensación según la
reivindicación 2, en donde la citada volatilización incluye el
calentamiento de un soporte sólido revestido con una composición
que comprende el producto sedante-hipnótico, para
volatilizar el producto sedante-hipnótico a partir
de la composición revestida.
4. Aerosol por condensación según la
reivindicación 2 o la reivindicación 3, en donde la citada
condensación incluye dejar enfriar el vapor.
5. Aerosol por condensación según cualquiera de
las reivindicaciones 1 a 4, en donde las citadas partículas
comprenden menos del 5 por ciento en peso de productos de
degradación sedantes-hipnóticos.
6. Aerosol por condensación según cualquiera de
las reivindicaciones 1 a 4, en donde las citadas partículas
comprenden menos del 2,5 por ciento en peso de productos de
degradación sedantes-hipnóticos.
7. Aerosol por condensación según cualquiera de
las reivindicaciones 1 a 6, en donde el citado aerosol por
condensación comprende al menos el 5 por ciento en peso del producto
sedante-hipnótico.
8. Aerosol por condensación según cualquiera de
las reivindicaciones 1 a 6, en donde el citado aerosol por
condensación comprende al menos el 90 por ciento en peso del
producto sedante-hipnótico.
9. Aerosol por condensación según cualquiera de
las reivindicaciones 1 a 8, en donde el citado aerosol por
condensación tiene un MMAD de menos de 3 \mum.
10. Aerosol por condensación según cualquiera de
las reivindicaciones 1 a 9, para su utilización en terapia de
inhalación.
11. Composición para el suministro de un
producto sedante-hipnótico, comprendiendo dicha
composición un aerosol por condensación según cualquiera de las
reivindicaciones 1 a 10.
12. Procedimiento para producir un producto
sedante-hipnótico en forma de aerosol, que
comprende:
- a)
- volatilizar un producto sedante-hipnótico seleccionado del grupo que comprende zaleplón, zolpidem y zopiclona, en condiciones eficaces para producir un vapor del producto sedante-hipnótico, en donde la citada volatilización comprende el calentamiento de una composición que comprende al menos el 5 por ciento en peso del producto sedante-hipnótico; y
- b)
- condensar el vapor, proporcionando con ello un aerosol por condensación que comprende partículas que comprenden menos del 10 por ciento en peso de productos de degradación sedantes-hipnóticos y que tienen un MMAD de menos de 5 \mum.
13. Procedimiento según la reivindicación 12, en
donde el paso a) comprende el calentamiento de un soporte sólido
revestido con una composición que comprende el producto
sedante-hipnótico para volatilizar el producto
sedante-hipnótico a partir de la composición
revestida.
14. Procedimiento según la reivindicación 12, en
donde el paso a) comprende el calentamiento de un soporte sólido
sobre el cual se ha depositado una composición que comprende el
producto sedante-hipnótico, para volatilizar el
producto sedante-hipnótico a partir de la
composición depositada.
15. Procedimiento según cualquiera de las
reivindicaciones 12 a 14, en donde el paso b) comprende dejar
enfriar el vapor, proporcionando de este modo el aerosol por
condensación.
16. Procedimiento según cualquiera de las
reivindicaciones 12 a 15, en donde las citadas partículas comprenden
menos del 5 por ciento en peso de productos de degradación
sedantes-hipnóticos.
17. Procedimiento según cualquiera de las
reivindicaciones 12 a 15, en donde las citadas partículas comprenden
menos del 2,5 por ciento en peso de productos de degradación
sedantes-hipnóticos.
18. Procedimiento según cualquiera de las
reivindicaciones 12 a 17, en donde las citadas partículas comprenden
al menos el 90 por ciento en peso de producto
sedante-hipnótico.
19. Procedimiento según cualquiera de las
reivindicaciones 12 a 18, en donde las citadas partículas se forman
a una velocidad superior a 0,5 mg/segundo.
20. Procedimiento según cualquiera de las
reivindicaciones 12 a 18, en donde las citadas partículas se forman
a una velocidad superior a 1 mg/segundo.
21. Procedimiento según cualquiera de las
reivindicaciones 12 a 20, en donde el citado aerosol por
condensación tiene un MMAD inferior a 3 \mum.
22. Procedimiento según cualquiera de las
reivindicaciones 12 a 21, en donde el aerosol por condensación se
utiliza en terapia de inhalación.
23. Kit para el suministro de un aerosol por
condensación de un producto sedante-hipnótico, en
donde el kit comprende:
- a)
- una composición que comprende un producto sedante-hipnótico seleccionado de entre el grupo que comprende zaleplón, zolpidem y zopiclona, en donde la composición comprende al menos el 5 por ciento en peso del producto sedante-hipnótico; y
- b)
- un dispositivo que forma un aerosol por condensación que contiene el producto sedante-hipnótico a partir de la composición;
en donde el dispositivo comprende:
- i)
- un elemento para calentar la composición para formar un vapor;
- ii)
- un elemento que permite enfriar el vapor para formar un aerosol por condensación; y
- iii)
- un elemento que permite la inhalación del aerosol.
24. Kit según la reivindicación 23, en donde el
aerosol por condensación es para uso en terapia de inhalación.
25. Kit según la reivindicación 23 o 24, en
donde el aerosol por condensación es tal como se ha definido en
cualquiera de las reivindicaciones 1 a 10.
