Abstract

发明背景
对啮齿动物的研究表明,用抗原特异性T细胞对癌症和感染进行过继性免疫治疗是有效的,并且有证据表明该疗法对人类有治疗效果1-8。对于临床应用,需要分离具有所需抗原特异性的T细胞,或对T细胞进行改造以表达靶向感染的或转化的细胞的受体,然后在培养中扩增这些细胞9-14。T细胞克隆的转移前景诱人,因为其使得能够对特异性和功能进行控制,且便于进行体内持久性、毒性和功效的评估。此外,在同种异体的干细胞移植环境中,将源自供体的靶向病原体或恶性细胞的T细胞克隆施用于接受者,可避免因注入未经选择的供体T细胞而发生的移植物抗宿主病3,4,15。然而,临床研究明显显示培养的T细胞,尤其是克隆的CD8+T细胞的功效,由于其无法在过继转移后持续存在而频频受限16,17。
用于过继性免疫治疗的T细胞所源自的淋巴细胞库(pool)包含初始的和长寿的、抗原刺激过的记忆T细胞(TM)。TM可进一步分为表型、归巢性(homing properties)和功能不同的中心记忆(TCM)和效应记忆(TEM)细胞亚群18。CD8+TCM在细胞表面表达CD62L和CCR7,其促进迁移至淋巴结,且如果再次暴露于抗原就会快速增殖。CD8+TEM缺乏细胞表面CD62L且优选迁移至周围组织,并表现出直接的效应功能19。针对抗原刺激的应答,CD8+TCM和TEM都分化为表达高水平颗粒酶和穿孔素的溶细胞效应T细胞(TE),但其为短寿细胞20。因而,临床免疫治疗试验中T细胞存活力差可能仅源于其在体外培养中分化为注定要死亡的TE 17,21,22。需要确定提高过继转移的T细胞体内存活力的细胞群和方法。
发明概述
在一个方面中,本发明涉及,诸如通过过继转移肿瘤特异性、亚群特异性遗传修饰的CD4+T细胞,赋予和/或增强细胞免疫治疗介导的免疫应答的方法和组合物,其中所述CD4+T细胞赋予和/或增强CD8+T细胞维持抗肿瘤反应性的能力,且增加和/或最大化肿瘤特异性增殖。
在一个实施方案中,本发明提供了以下方法:通过将提供细胞免疫应答的遗传修饰的细胞毒性T淋巴细胞制剂和遗传修饰的辅助性T淋巴细胞制剂施用于个体,在患有疾病或病症的所述个体中进行细胞免疫治疗的方法,其中所述细胞毒性T淋巴细胞制剂包含具有嵌合抗原受体的CD8+T细胞,所述嵌合抗原受体具有对与所述疾病或病症相关的抗原有特异性的胞外抗体可变域和T细胞或其它受体的胞内信号传导域,如共刺激域;以及所述遗传修饰的辅助性T淋巴细胞制剂表现出明显的Th1表型并产生其它细胞因子,引发直接肿瘤识别并增强所述遗传修饰的细胞毒性T淋巴细胞制剂介导细胞免疫应答的能力,其中所述辅助性T淋巴细胞制剂包含具有嵌合抗原受体的CD4+T细胞,所述嵌合抗原受体包含对所述疾病或病症相关抗原特异的胞外抗体可变域和T细胞受体的胞内信号传导域。可对上述方法进行各种修改。例如,修饰所述CD4+T细胞和CD8+T细胞的嵌合抗原受体可以相同或不同。在可选的实施方案中,可以用重组T细胞受体(TCR)修饰所述T细胞。TCR可以对任何抗原、病原体或肿瘤具有特异性。在黑素瘤(例如,MART1、gp100)、白血病(例如,WT1、次要组织相容性抗原)、乳腺癌(例如,her2、NY-BR1)中存在针对许多肿瘤抗原的TCR。

附图说明
图1显示了在用编码慢病毒的ROR1-CAR转导的CAR中嵌合抗原受体(CAR)表达的表型和分析,未转导的CD8+T细胞系作为对照。所述ROR1-CAR盒包含作为转导标记的截短型EGFR,并可以通过用抗-EGFR单克隆抗体染色来检测。截短型Fc-ROR1融合蛋白直接结合至ROR1-CAR的抗原结合域,并选择性地染色转导的ROR1-CAR而不染色未转导的对照T细胞系。ROR1-CAR在CD8+T细胞的细胞表面上的表达通过结合至ROR1-Fc融合蛋白来直接测量,以及通过在载体中的2A序列下游编码的截短型EGFR的表达来间接测量。
图2显示了在51Cr释放试验中,表达ROR1-特异性嵌合抗原受体的CD8+T细胞针对一组人ROR1-阳性肿瘤细胞系(K562)、原代肿瘤细胞(B-CLL)和自体正常B-细胞的溶细胞活性。与ROR1在恶性而非成熟的正常B细胞均一表达一致的是,遗传修饰的CD8+ROR1-CAR T细胞仅溶解ROR1+肿瘤细胞而不溶解成熟的正常B细胞。CD8+ROR1-CAR T细胞表现出针对包括原代CLL在内的ROR1-阳性肿瘤细胞的特异性溶细胞活性,但不针对正常的B细胞。
图3显示了ROR1-CAR转导的CD4+T细胞系的表型和CAR表达,未转染的CD4+T细胞系作为对照。ROR1-CAR在CD4+T细胞的细胞表面上的表达是通过特异性结合至ROR1-Fc融合蛋白来测量的。截短型Fc ROR1融合蛋白而非单独的Fc蛋白直接结合至ROR1-CAR,并选择性地染色ROR1-CAR转导的而不是未转导的对照CD4+T细胞系,证实了ROR1-CAR在细胞表面上的表达和结合至ROR1-蛋白。ROR1-CAR在CD4+T细胞的细胞表面上的表达,通过特异性结合至ROR1-Fc融合蛋白而不结合至对照Fc融合蛋白来测量。
图4(即图4A-4B一起)显示了在51Cr释放试验中,CD4+ROR1-CAR T细胞针对的弱但具有特异性的溶细胞活性,其针对一组ROR1-阳性肿瘤细胞包括原代CLL、套细胞淋巴瘤细胞系Jeko-1、用ROR1(K562/ROR1)稳定转染的K562细胞,而非初始ROR1-阴性K562细胞。CD4+ROR1-CAR T细胞针对ROR1-阳性肿瘤细胞表现出弱的但具有特异性的溶细胞活性。
图5(即图5A-5B一起)显示了来自IFNγELISA(图5A)和多重细胞因子检测(图5B)的结果。CD4+和CD8+ROR1-CAR T细胞系的细胞因子分泌。将CD4+ROR1-CAR和CD8ROR1-CAR T细胞与ROR1+肿瘤细胞共孵育,通过ELISA(5A)测量干扰素γ(IFNg)的水平,通过Luminex测定(5B)来测量IFNg、TNFa、IL-2、IL-4、IL-10和IL-17。CD4+ROR1-CAR修饰的T细胞特异性地识别ROR1-阳性肿瘤细胞和肿瘤细胞系,并比CD8+ROR1-CAR修饰的T细胞产生更大量的Th1细胞因子包括IFN-γ、TNF-α,尤其是IL-2。这些数据表明,通过ROR1-CAR刺激后,CD4+ROR1-CAR T细胞除了介导直接抗肿瘤反应外还表现出辅助性效应功能,也可用于增强CD8+ROR1-CAR修饰的T细胞介导细胞免疫应答的能力。
图6描述了增殖研究的结果,表明在用ROR1-阳性肿瘤细胞系和原代肿瘤细胞刺激后,CD4+ROR1-CAR T细胞被诱导进行增殖(CFSE测定),并且增殖细胞的百分比和增殖的亚群细胞进行细胞分裂的次数都比CD8+ROR1-CAR修饰的T细胞明显更高。用ROR1-阳性肿瘤细胞(K562/ROR1、原代CLL和Jeko MCL)刺激后,CD4+ROR1-CAR T细胞的增殖比CD8+ROR1-CARCTL更旺盛。

优选实施方案详述

具体实施方式
在体外培养的CD4+T淋巴细胞明显增加了肿瘤特异性CD8+T细胞在体外和体内的增殖、持久性和抗肿瘤反应性。在一些实施方案中,相比来自中心和效应记忆或大型CD4+T细胞的CD4+T细胞,初始CD4+T细胞具有使得其有优异的辅助活性的内在程序(intrinsicprogramming)。

T淋巴细胞可以根据已知技术来收集,并通过已知技术来富集或消减,如与抗体的亲和结合,如流式细胞计数和/或免疫磁珠选择。经过富集和/或消减步骤以后,可以根据已知技术(包括但不限于描述于Riddell等人的美国专利No.6,040,177中的内容)或对本领域技术人员来说显而易见的其变体,对期望的T淋巴细胞进行体外扩增。

T淋巴细胞群的修饰

在一些实施方案中,CD8+T细胞和CD4+T细胞都用特异性结合至病原体特异性细胞表面抗原的抗体重链域进行了遗传修饰。在实施方案中,CAR对与病原体、肿瘤或癌症细胞有关的细胞表面表达的抗原具有特异性。在一些实施方案中,CAR对HIV抗原、HCV抗原、HBV抗原、CMV抗原、寄生虫抗原和肿瘤抗原如酪氨酸激酶孤儿受体ROR1、tEGFR、Her2、L1-CAM、CD19、CD20、CD22、间皮素和CEA有特异性。本文描述了制备CAR的方法,也可参见Forman的US6,410,319,Jensen等人的WO 2002/077029、7,446,191、WO 2010/065818、WO2010/025177、WO 2007/059298和7,514,537,以及Berger C.等人于J.Clinical Investigation,118:1294-308(2008)中的描述,它们均以引用的方式全部并入本文中。
在实施方案中,可将相同或不同的CAR引入至每个CD4+和CD8+T淋巴细胞中。在实施方案中,在这些细胞群的每个中的CAR具有特异地结合至相同抗原的抗原结合分子。所述细胞信号转导模块可以不同。在实施方案中,CD4或CD8T淋巴细胞的每个在转导前均可分类为初始、中心记忆性、效应记忆性或效应细胞。在可选的实施方案中,所述CD4或CD8T淋巴细胞的每个在转导前均可分类为初始、中心记忆性、效应记忆性或效应细胞。
在可选的实施方案中,T细胞可用重组T细胞受体进行修饰。TCR可能对任何抗原、病原体或肿瘤具有特异性。存在针对在黑素瘤(例如,MART1、gp100)、白血病(例如,WT1、次要组织相容性抗原)、乳腺癌(例如,her2、NY-BR1)中的许多肿瘤抗原的TCR。
已经开发出利用重组的感染病毒粒子进行基因递送的各种感染技术。这代表了本发明T淋巴细胞转导的当前优选方法。已经以这种方式使用的病毒载体包括源自猿猴病毒40、腺病毒、腺相关病毒(AAV)、慢病毒载体和逆转录病毒的病毒载体。因此,基因转移和表达方法众多但本质上的功能是在哺乳动物细胞中引入和表达遗传物质。一些上述技术已经用于转导造血细胞或淋巴细胞,包括磷酸钙法转染、原生质体融合、电穿孔以及用重组腺病毒、腺相关病毒和逆转录病毒载体的感染。已经成功地通过电穿孔和逆转录病毒感染转导了原代T淋巴细胞。
逆转录病毒载体提供了将基因转移至真核细胞的高效方法。而且,逆转录病毒的整合以受控方式发生,使得每个细胞稳定地整合一个或多个拷贝的新遗传信息。
预期到刺激因子(例如,淋巴因子或细胞因子)的过表达可能对所处理的个体有毒性。因此,在本发明的范围内包括使得本发明的T细胞易于在体内进行阴性选择的基因片段。所用“阴性选择”是指作为在个体的体内条件下的变化结果,融合细胞可以被去除。所述阴性选择表型可能是由于插入赋予对所施用制剂例如化合物的敏感性的基因的结果。在本领域中已知阴性选择基因,除其它以外包括下述基因:赋予对更昔洛韦敏感性的单纯疱疹病毒I型胸苷激酶(HSV-I TK)基因(Wigler et al.,Cell 11:223,1977),细胞次黄嘌呤磷酸核糖转移酶(HPRT)基因,细胞腺嘌呤磷酸核糖转移酶(APRT)基因、细菌胞苷脱氨酶(Mullen et al.,Proc.Natl.Acad.Sci.USA.89:33(1992))。
在一些实施方案中,在T细胞中包含使得能够在体外挑选出阴性选择表型细胞的阳性标志物是有用的。所述阳性选择标志物可以是基因,当被导入宿主细胞中时,其表达出允许对携带所述基因的细胞进行阳性选择的显性表型。这种基因在本领域是已知的,除其它以外包括,赋予对潮霉素-B抗性的潮霉素-B磷酸转移酶基因(hph)、来自编码对抗生素G418抗性的Tn5的氨基糖苷磷酸转移酶基因(neo或aph)、二氢叶酸还原酶(DHFR)基因、腺苷脱氨酶基因(ADA)和多重耐药(MDR)基因。

优选的阳性选择标志物源自选自下组的基因:hph、nco和gpt,优选的阴性选择标志物源自选自下组的基因:胞嘧啶脱氨酶、HSV-I TK、VZV TK、HPRT,APRT和gpt。尤其优选的标志物为双功能选择融合基因,其中所述阳性选择标志物源自hph或neo,所述阴性选择标志物源自胞嘧啶脱氨酶或TK基因或选择标志物。
可使用本领域熟知的各种方法转导T淋巴细胞。例如,逆转录病毒转导可按如下进行:使用如本文所描述的REM刺激后第1天,给细胞提供20-30单位/ml的IL-2;第3天,用按照标准方法制备的逆转录病毒上清液替换二分之一的培养基,然后向培养基中补充5ug/ml聚凝胺和20-30单位/ml IL-2;第4天,洗涤所述细胞并将其放入补充有20-30单位/ml IL-2的新鲜培养基中;第5天,重复暴露于逆转录病毒中;第6天,将所述细胞放入补充有30单位/mlIL-2的选择性培养基(例如,含有对应于所述逆转录病毒载体中提供的抗生素抗性基因的抗生素)中;第13天,使用Ficoll Hypaque密度梯度分离法将活细胞与死细胞分离,然后将所述活细胞进行亚克隆。
CD4+和CD8+细胞可以用编码CAR的表达载体进行修饰。在实施方案中,然后如上所述,通过针对这些细胞群中每一种是唯一的细胞表面抗原的分选,将这些细胞进一步分类为初始、中心记忆和效应细胞亚群。此外,CD4+或CD8+细胞群可通过其细胞因子谱或增殖活性来挑选。例如,可以挑选用抗原刺激时,相比假(sham)转导的细胞或转导的CD8+细胞,具有提高的细胞因子如IL-2、IL-4、IL-10、TNFα和IFNγ产物的CD4+T淋巴细胞。在其它实施方案中,挑选出具有提高的IL-2和/或TNFα产物的初始CD4+T细胞。同样,相比假转导的CD8+细胞,将具有提高的IFNγ产物的CD8+细胞挑选出来。
在实施方案中,将响应抗原而增殖的CD4+和CD8+细胞挑选出来。例如,挑选出用抗原刺激时,相比假转导的细胞或转导的CD8+细胞,增殖旺盛的CD4+细胞。
在一些实施方案中,挑选出对于带有抗原的细胞具有细胞毒性的CD4+和CD8+细胞。在实施方案中,预期相比CD8+细胞,CD4+具有弱的细胞毒性。
本申请预期,CD4+和CD8+T细胞的组合可用于组合物中。在一个实施方案中,组合CAR转导的CD4+细胞可与对CAR具有同样的抗原特异性的CD8+抗原反应性细胞组合。在其它实施方案中,将CAR转导的CD8+细胞与抗原反应性CD4+细胞组合。在又一个实施方案中,将CAR修饰的CD4+和CD8+细胞组合。
如本文所描述,本申请预期,可将CD4+和CD8+细胞进一步分开为亚群如初始、中心记忆和效应细胞群。如本文所描述,在一些实施方案中,初始CD4+细胞为CD45RO-、CD45RA+、CD62L+CD4+T细胞。在一些实施方案中,中心记忆性CD4+细胞为CD62L阳性和CD45RO阳性。在一些实施方案中,效应CD4+细胞为CD62L阴性和CD45RO阳性。这些细胞群的每一种均可独立地用CAR修饰。

方法
本申请提供了制备过继性细胞免疫治疗组合物的方法,以及使用这些组合物在患有疾病或病症的个体中进行细胞免疫治疗的用途和方法。
在实施方案中,制备所述组合物的方法包括获得修饰的初始CD4+T辅助细胞,其中所述修饰的辅助性T淋巴细胞制剂包含具有嵌合抗原受体的CD4+T细胞,所述嵌合抗原受体包含对疾病或病症相关抗原特异的胞外抗体可变域和胞内信号传导域。
在另一个实施方案中,方法还包括获得修饰的CD8+细胞毒性T细胞,其中所述修饰的细胞毒性T淋巴细胞制剂包含具有嵌合抗原受体的CD8+细胞,所述嵌合抗原受体包含对疾病或病症相关抗原特异的胞外抗体可变域和T细胞受体的胞内信号传导域。

通常可由本发明治疗的个体为人和其它灵长类个体,如用于兽医药目的的猴和猿。所述个体可为雄性或雌性,且可为任何合适的年龄,包括婴幼儿、少年、青年、成年和老年个体。
所述方法可用于治疗例如,实体瘤、恶性血液病、黑素瘤,或者病毒或其它病原体感染。病原体感染包括HIV、HCV、HBV、CMV和寄生虫病。在一些实施方案中,与所述疾病或病症相关的抗原选自酪氨酸激酶孤儿受体ROR1、tEGFR、Her2、L1-CAM、CD19、CD20、CD22、间皮素、CEA和乙型肝炎表面抗原。
可治疗的个体包括患有癌症的个体,所述癌症包括但不限于结肠、肺、肝脏、乳腺、前列腺、卵巢、皮肤(包括黑素瘤)、骨骼和脑部的癌症等。在一些实施方案中,肿瘤相关的抗原是已知的,如黑素瘤、乳腺癌、鳞状细胞癌、结肠癌、白血病、骨髓瘤、前列腺癌等(在这些实施方案中,可以分离记忆T细胞或通过引入所述T细胞受体基因对其进行改造)。在其它实施方案中,可以用表达改造的免疫受体的遗传修饰的T细胞来靶向所述肿瘤相关蛋白。实例包括但不限于B细胞淋巴瘤、乳腺癌、前列腺癌和白血病。
可被治疗的个体还包括患有传染病或具有发展为传染病风险的个体,所述传染病包括但不限于病毒、逆转录病毒、细菌和原生动物感染等。可被治疗的个体包括患有病毒感染的免疫缺陷型患者,包括但不限于移植患者中的巨细胞病毒(CMV)、爱泼斯坦-巴尔(Epstein-Barr)病毒(EBV)、腺病毒、BK多瘤病毒感染等。
根据已知技术或基于本申请公开而对本领域技术人员显而易见的其变体,按照以上描述制备的细胞可用于进行过继性免疫治疗的方法和组合物中。参见,例如,Gruenberg等人的美国专利申请公开No.2003/0170238,也参见Rosenberg的美国专利No.4,690,915。
在一些实施方案中,所述细胞配制如下:首先从其培养基收获它们,然后在适于以治疗有效量施用的培养基和容器系统(“药学上可接受的”载体)中洗涤和浓缩所述细胞。合适的输注培养基可为任何等渗培养基制剂,通常为生理盐水、Normosol R(Abbott)或Plasma-Lyte A(Baxter)注射液,也可使用5%葡萄糖水或林格氏乳酸盐溶液。所述输注培养基可以补充有人血清白蛋白。
组合物中的治疗有效量的细胞为至少2细胞(例如,1个CD8+中心记忆性T细胞和1个CD4+辅助性T细胞亚群),或者更通常为大于102个细胞,多达106,多达并包括108或109个细胞,并且可以大于1010个细胞。细胞的数量取决于组合物预期的最终应用及其所包括的细胞类型。例如,如果期望细胞对于特定抗原具有特异性,则细胞群应含有大于70%,通常大于80%、85%和90-95%的这类细胞。对于本文提供的应用,通常细胞在1升或更少的体积中,可以为500ml或更少,甚至250ml或100ml或更少。因此,所期望的细胞的密度通常大于106细胞/m1,通常大于107细胞/ml,通常108细胞/ml或更大。临床上相关的免疫细胞数目可分配到多个输注,累积等于或大于109、1010或1011个细胞。
在一些实施方案中,本发明的淋巴细胞可用于赋予个体免疫力。所用“免疫力”是指减轻与对病原体感染或肿瘤的应答有关的一个或多个身体症状,淋巴细胞的应答针对所述病原体感染或肿瘤。所施用细胞的量通常为在具有针对病原体的免疫力的正常个体中存在的范围。因此,所述细胞通常通过输注施用,每次输注的用量范围从2个细胞到至少106-1010个细胞/m2,优选范围为至少107-109细胞/m2。克隆可通过单次输注施用,或通过在一段时间中进行多次输注。然而,由于预期不同个体的响应性也不同,输注细胞的类型、数量以及输注次数和进行多次输注的时间范围由主治医生确定,这可以通过常规检查来确定。如本文所示例,足够水平的T淋巴细胞(包括细胞毒性T淋巴细胞和/或辅助性T淋巴细胞)的产生可使用本发明的快速扩增方法容易地获得。参见,例如,Riddell等人的美国专利No.6,040,177,第17栏。
在下述实施例中进一步说明本发明。
实验
实施例1-T细胞转导和CAR表达分析
ROR1-特异性CAR可在人CD8+T细胞中表达,并赋予特异性识别ROR1+B-细胞肿瘤而非成熟的正常B细胞的能力。我们构建了ROR1-特异性嵌合抗原受体,当其在来自健康供体或CLL患者的T细胞中表达时,赋予特异性识别原代B-CLL和套细胞淋巴瘤的能力。
材料和方法
细胞系
如所描述的(25),制备爱泼斯坦-巴尔(Epstein-Barr)病毒(EBV)转化的B细胞(EBV-LCL)。肿瘤细胞系Jeko-1和BALL-1由Oliver Press和Jerald Radich(弗雷德哈钦森癌症研究中心)博士提供。所有细胞系均维持于RPMI中,10%胎牛血清、0.8mM L-谷氨酰胺和1%青霉素-链霉素(LCL培养基)。K562细胞获自美国典型培养物中心(American TypeCulture Collection)。
用ROR1转染K562细胞
对于聚合酶链式反应(PCR)–扩增所述ROR1-基因,由B-CLL细胞获得总RNA(RNeasyPlusKit;QIAGEN),并用M-MLV逆转录酶(Invitrogen)逆转录至cDNA中。使用Herculase-II DNA聚合酶(Stratagene),用特定的引物进行PCR(ROR1-F:5-XhoIAGAGGAGGAATGCACCGGCC-3,以及ROR1-R:5-XhoI-CACAGAAGGTACTTGTTGCGATGT-3)。将所述PCR产物克隆至MIGR-1逆转录病毒载体(23)中,并验证其序列。将Effectene转染试剂(QIAGEN)用于转染具有MIGR-1/ROR1的铂(Platinum)-A细胞(Cell Biolabs),并产生ROR1-编码的逆转录病毒。将K562细胞于32℃以2500rpm离心60min进行逆转录病毒转导,扩增,然后将所述ROR1-阳性细胞亚群分选纯化。
实时定量PCR
按照前述段落的描述制备B-CLL、正常的静止和活化B细胞和EBV-LCL的第一链cDNA。来自正常组织(人组织板(Human Tissue panel)I/II,Blood Fractions)的第一链cDNA获自Clontech。一式两份分析ROR1mRNA的表达,并相对于GAPDH进行标准化。在ABIPrism 7900(Applied Biosystems)上于50μL反应物中进行扩增,所述反应物由25μL PowerSYBR Green PCR Master Mix(Applied Biosystems)、2.5ng的cDNA和300nM基因-特异性正向和反向引物组成:

使用SDS软件v2.2.2(Applied Biosystems)确定所述循环阈值(Ct),用比较性Ct法(2-(ΔΔCt))计算基因表达水平。
载体构建和慢病毒的制备
CD20-CAR(CD20R-epHIV7)和绿色荧光蛋白(GFP)-编码的慢病毒载体(GFP-epHIV7)如前所述(24)。ROR1-CAR被编码于相同的载体中。在以前的研究中制备、克隆并表征了小鼠mAb(克隆2A2),其显示了对在原代B-CLL和MCL肿瘤细胞系表达的人ROR1的特异性结合。合成了编码包含mAb 2A2的VL和VH链的scFv的密码子优化核苷酸序列(GENEART),并用NheI和RsrII限制位点克隆至CD20R-epHIV7以替换CD20-特异性scFv。在293T细胞中制备慢病毒,所述细胞利用Effectene(Qiagen)共转染了慢病毒载体与包装载体pCHGP-2、pCMVRev2和pCMV-G。转染后16小时更换培养基,48小时后收集慢病毒。
慢病毒转导和CAR-转导的T细胞克隆的分离

结果
利用生物素化的抗-EGFR mAb和链霉亲和素偶联的染料来分选纯化转导的CD8+T细胞。通过用直接结合至ROR1-CAR的scFv的生物素化重组Fc-ROR1胞外域融合蛋白来染色所述细胞以及与链霉亲和素-偶联物共染色,评估了分选纯化的T细胞表面上的ROR1-CAR表达。Fc-ROR1-蛋白特异性染色用ROR1-CAR慢病毒载体转导的CD8+T细胞,而不染色用编码GFP的对照慢病毒载体转导的CD8+T细胞(图1)。
我们通过有限稀释法建立了ROR1-CAR转导的(n=10)和对照GFP-转导的CD8+T-细胞克隆(n=4),并证实了在多轮体外扩增后,CAR稳定的细胞表面表达。相比未转导的或GFP转导的T-细胞克隆,ROR1-CAR转导的T-细胞克隆的生长并没有明显差异(数据未示出)。
ROR1-CAR转导的T-细胞克隆,有效地裂解了用ROR1-基因稳定转染的原代B-CLL和K562细胞,而非初始、ROR1-阴性K562细胞,这证明了对ROR1的特异性识别(图2).
讨论
在临床试验上针对B-细胞恶性肿瘤研究了使用CAR-修饰的T细胞的过继性免疫治疗。被靶向的表面分子为B-细胞谱系-特异性的,且包括从祖-B-细胞阶段至浆细胞的正常B-谱系细胞上表达的CD19,以及从前-B-细胞阶段至记忆B细胞的正常B细胞上表达的CD20。因此,靶向这些分子的预期的有效治疗结果是正常B细胞和B-细胞前体的消减。在2个独立的分析中,基因表达谱研究已经鉴定了优先或仅由恶性而非正常B细胞表达的基因,以及作为CLL标签基因出现的ROR1(27,28)。在用已经修饰为表达CD154的自体肿瘤细胞进行接种后,在CLL患者中产生了针对ROR1的特异性抗体,用来那多胺(lenalidomide)治疗对正常组织无明显毒性,表明该肿瘤抗原可以是用于免疫治疗的适合靶标(29,30)。
我们的研究说明了用改造的表达ROR1-CAR的T细胞靶向ROR1-阳性肿瘤细胞的可能性。在慢病毒转导大型PBMC或分选纯化的TCM后,CD8+ROR1-CAR T细胞可源自正常供体和CLL患者,其在过继转移后于动物模型中长时间持续存在(31)。ROR1-CAR转导的T细胞有效地裂解了原代B-CLL而非正常静止或激活的B-细胞。这些T细胞产生了效应细胞因子包括TNF-α、IFNγ和IL-2,并且能够响应表达ROR1的肿瘤细胞而增殖。
实施例2–制备CD4+CAR T细胞系以及效应功能分析
CD4+ROR1-CAR T细胞可由健康供体/CLL-患者的PBMC产生。ROR1-特异性CAR可在人CD4+T细胞中表达,并赋予特异性识别ROR1+B-细胞肿瘤而非成熟的正常B细胞的能力。
材料和方法
细胞系
如所描述的(25)制备爱泼斯坦-巴尔(Epstein-Barr)病毒(EBV)转化的B细胞(EBV-LCL)。肿瘤细胞系Jeko-1和BALL-1由Oliver Press和Jerald Radich(弗雷德哈钦森癌症研究中心)博士提供。所有细胞系均维持于RPMI中,含有10%胎牛血清、0.8mM L-谷氨酰胺和1%青霉素-链霉素(LCL培养基)。K562细胞和293T细胞获自美国典型培养物中心(American Type Culture Collection)并根据其指导进行培养。
用ROR1转染K562细胞
对于ROR1-基因的聚合酶链式反应(PCR)–扩增,由B-CLL细胞获得总RNA(RNeasyPlusKit;QIAGEN),并用M-MLV逆转录酶(Invitrogen)逆转录至cDNA中。使用Herculase-II DNA聚合酶(Stratagene),用特异性引物进行PCR(ROR1-F:5-XhoIAGAGGAGGAATGCACCGGCC-3,以及ROR1-R:5-XhoI-CACAGAAGGTACTTGTTGCGATGT-3)。将所述PCR产物克隆至MIGR-1逆转录病毒载体(23)中,并验证其序列。将Effectene转染试剂(QIAGEN)用于转染具有MIGR-1/ROR1的铂(Platinum)-A细胞(Cell Biolabs),并产生ROR1-编码的逆转录病毒。将K562细胞于32℃以2500rpm离心60min进行逆转录病毒转导,扩增,然后分选纯化ROR1-阳性细胞亚群。
载体构建和慢病毒的制备
CD20-CAR(CD20R-epHIV7)和绿色荧光蛋白(GFP)-编码的慢病毒载体(GFP-epHIV7)如先前的描述(24)。ROR1-CAR被编码于相同的载体中。在此前的研究中已制备、克隆并表征了小鼠mAb(克隆2A2),其显示了对原代B-CLL和MCL肿瘤细胞系表达的人ROR1的特异性结合。合成了编码包含mAb 2A2的VL和VH链的scFv的密码子优化核苷酸序列(GENEART),并用NheI和RsrII限制位点克隆至CD20R-epHIV7以替换CD20-特异性scFv。在293T细胞中使用Effectene(Qiagen)将慢病毒载体与包装载体pCHGP-2、pCMVRev2和pCMV-G共转染来制备慢病毒。转染后16h更换培养基,48h后收集慢病毒。
慢病毒转导和CD4+ROR1-CAR T细胞系的分离

用51Cr(PerkinElmer)标记靶细胞过夜,洗涤并按一式三份以1-2×103细胞/孔和不同的效应细胞与靶细胞(E:T)比例与效应T细胞一起孵育。孵育4小时后,收集上清液用于γ计数,用标准公式计算特异性细胞溶解(25)。对于细胞因子分泌分析,将靶细胞和效应细胞以2:1的E/T比例接种到三个重复的孔中,孵育24小时后,在移出的上清液中通过多重细胞因子免疫测定(Luminex)测量干扰素INFγ、肿瘤坏死因子(TNF-α)和IL-2。
CFSE增殖测定
用0.2μM羧基荧光素琥珀酰亚胺酯(CFSE;Invitrogen)标记T细胞,洗涤T细胞,并将其与刺激细胞(stimulator cells)以2:1的比例接种于含有10U/mL重组人IL-2的CTL培养基中。孵育72小时后,用抗-CD4mAb和碘化丙啶(PI)标记细胞以将死细胞排除在分析之外。样本通过流式细胞术分析,通过CFSE稀释来评估活CD4+T细胞的细胞分裂。
共培养检测
用CFSE标记ROR1-CAR转染的CD4+T细胞和ROR1-CAR转导的CD8+细胞毒性T淋巴细胞,以2:1、1:1和1:2的比例共培养。然后,用K562/ROR1细胞和对照K562细胞刺激所述共培养物,孵育5天后通过CFSE染色稀释测定来测量细胞增殖。对于流式分析,用偶联的抗-CD8和抗-CD4mAb染色染色样本以区分CD8+和CD4+亚群。
结果
从健康供体和CLL患者的PBMC制备CD4+ROR1-CAR T细胞
我们已经表明,癌胚酪氨酸激酶受体ROR1在CLL和MCL上均一表达,并由抗-ROR1mAb产生ROR1-CAR,当在CD8+T细胞中表达时其赋予特异性识别恶性而非成熟正常B细胞的能力(32)。在本发明中,我们制备了CD4+ROR1-CAR T细胞以分析直接的肿瘤识别和它们增强CD8+ROR1-CAR CTL的能力。使用编码ROR1-CAR的慢病毒载体,可以很容易地由来自健康供体(n=4)和CLL患者(n=4)的大型外周CD4+T细胞制备CAR-修饰的CD4+T细胞。在该载体中,我们编码了在ROR1-CAR和可自我剪切的2A元件(element)下游的截短型EGFR(表皮生长因子受体,tEGFR)域,以作为转导标记和用于利用抗-EGFR mAb富集表达转基因的T细胞(图3)。利用tEGFR标志物,我们在用编码ROR1-CAR的慢病毒(MOI=3)进行单次转导后第12天,确定了CAR-修饰的T细胞的频数,并发现相比来自相同个体的CD8+CAR T细胞系,在CD4+中一致具有更高的转导效率。为了证明ROR1-CAR在CD4+T细胞表面上的表达,我们利用了生物素化的重组Fc-ROR1胞外域融合蛋白,其直接结合至ROR1-CAR的scFv并特异性染色用ROR1-CAR慢病毒转导的CD4+T细胞而非未转导的对照CD4+T细胞(图3)。我们利用tEGFR标志物富集了表达转基因的CD4+T细胞,并且通过用抗-CD3mAb刺激扩增了CAR-阳性T细胞亚群。在14-天刺激周期结束时可获得大于3-log扩增的CD4+CAR T细胞,其与在CD8+CAR CTL中观察到的扩增相当。扩增后,我们证实了ROR1-CAR在CD4+CAR T细胞表面上的稳定表达(数据未示出),并分析了ROR1-阳性肿瘤细胞的识别。
CD4+ROR1-CAR T细胞特异性识别ROR1-阳性肿瘤
我们分析了CD4+ROR1-CAR T细胞针对ROR1-阳性原代肿瘤细胞和肿瘤细胞系的效应功能。我们通过铬释放试验(CRA)分析了CD4+CAR T细胞赋予直接的细胞毒性的能力,并在标准的4-小时孵育结束时检测到ROR1-阳性靶细胞的较弱但具有特异性的细胞溶解(图4)。我们将CRA延长至10小时,并观察到进一步增加的特异性细胞溶解,然而,CD4+CAR T细胞的总体溶细胞活性还是低于CD8+ROR1-CAR CTL(图2,4)。通过IFN-γELISA,来自健康供体和CLL患者的CD4+ROR1-CAR T细胞特异性识别了原代CLL细胞、ROR1-阳性肿瘤细胞系Jeko-1(MCL)和BALL-1(B-ALL),以及用ROR1-基因(K562/ROR1)稳定转染的K562细胞,而不识别初始ROR1-阴性K562细胞,证明了对靶细胞的细胞表面上的ROR1的特异性识别(图5A)。多重细胞因子分析揭示产生了相比CD8+CAR CTL明显更高水平的其它Th1细胞因子如TNF-α和IL-2,并产生了IL-4、IL-10和IL-17(图5B)。
接着,我们通过CFSE染色评估了用ROR1-阳性肿瘤细胞刺激后的CD4+CAR T细胞的增殖,并且使用了不添加外源细胞因子的严格培养条件以去除任何潜在的非特异性刺激。CD4+CAR T细胞显示出响应ROR1-阳性肿瘤细胞的明显且具有特异性的增殖。经诱导进行增殖的T细胞百分比和增殖亚群进行细胞分裂的次数在CD4+中均明显高于CD8+CAR T细胞(图6)。总之,我们的数据表明,来自健康供体和CLL患者的CD4+T细胞,用ROR1-特异性CAR进行遗传修饰后,获得了抗肿瘤反应性。而且,在无外源细胞因子条件下增殖和产生高水平Th1细胞因子的能力,意味着通过CAR刺激后,CD4+CAR T细胞表现出典型的辅助性功能,除了赋予直接的抗-肿瘤效应,其还可以用于增强CD8+CAR CTL。
CAR-修饰的但未转导的CD4+T细胞为CD8+CAR CTL提供帮助
为了分析CD4+CAR T细胞是否能够为CD8+CAR CTL提供帮助,我们用从健康供体和CLL患者建立的CAR-转导的和对照未转导的多克隆CD4+和CD8+T细胞系进行共培养实验。作为提供了帮助的数据读出,我们定义了相比单独培养的CD8+T细胞,在CD4+T细胞存在下肿瘤特异性CD8+效应功能的改善。我们将CAR-转导的或未转导的对照CD4+T细胞与CD8+CAR CTL以不同的CD4:CD8比例(2:1,1:1,1:2)组合,用ROR1-阳性肿瘤细胞刺激它们,并通过CFSE染料稀释法来测量增殖。我们发现,将CAR-转导的而非未转导CD4+T细胞添加至CD8+CAR CTL,相比单独的CD8+CAR CTL,明显增加了CD8+亚群的特异性增殖(图7)。当把至少等量的CD4+CAR T细胞(CD4:CD8比例为2:1或1:1)加入至共培养物中时,增殖的增加最显著。将未转导的CD4+和未转导的CD8+T细胞组合作为另外的对照,在CD8+亚群中没有诱导出非特异性的增殖(数据未示出)。
讨论
在两个独立的分析中,基因表达谱研究已经确定了优先或仅由恶性而非正常B细胞表达的基因,以及作为CLL标签基因出现的ROR1(27,28)。我们的研究说明了用改造的表达ROR1-CAR的T细胞靶向ROR1-阳性恶性细胞的可能性。CD8和CD4+ROR1-CAR T细胞可以源自大型PBMC或分选纯化的T细胞慢病毒转导后的正常供体。CD8+ROR1-CAR转导的T细胞有效地溶解了原代B-CLL而不是正常的静止或激活的B-细胞。CD4+ROR1-CAR转导的T细胞较弱地溶解了原代B-CLL而不是正常的静止或激活的B-细胞。这些T细胞产生了效应细胞因子包括TNF-α、IFNγ、IL-2、IL-4和IL-10。CAR-转导的CD4+T细胞产生的细胞因子的量明显高于转导的CD8+细胞。两种细胞类型均能够响应表达ROR1的肿瘤细胞而增殖。而且,CD4+ROR1-CART细胞的增殖比CD8+ROR1-CAR CTL高2-3倍。这些结果表明,转导的CD4+辅助性T细胞表现出典型的辅助性功能,意味着它们可用于增强CD8+CAR CTL。
实施例3-源自初始、中心和效应记忆性细胞亚群的CD4+ROR1-CAR T细胞的效应功能
比较了源自初始、中心和效应记忆性细胞亚群、然后用ROR1CAR修饰的CD4T细胞的效应功能。
材料和方法
分选纯化初始、中心和效应记忆性CD4细胞
使用阴性磁珠选择(Miltenyi CD4分选试剂盒)从健康供体的PBMC中分离CD4+T细胞,其产生未触碰的(untouched)CD4+T细胞。所述CD4+部分用偶联的抗-CD45RA、抗-CD45RO和抗-CD62L mAb标记,用FACS Aria流式细胞分选仪(BD Biosciences)进行流式分选纯化,并基于这些定义的标志物的表达纯化初始(CD45RA+CD45RO-CD62L+)、中心记忆(CD45RA-CD45RO+CD62L+)和效应记忆性(CD45RA-CD45RO+CD62L-)CD4+T细胞。
CFSE增殖检测

结果
我们从3个健康供体的外周血中,基于CD45RA、CD45RO和CD62L的表达,用流式分选纯化CD4+N、中心(CM)和效应记忆性(EM)CD4+T细胞(图8A),并比较用ROR1-CAR修饰后它们的效应功能。在源自所述3个细胞亚群中每一个的CAR T细胞系中,我们获得了类似的高转导效率。富集表达转基因的T细胞后,多参数流式细胞仪显示了在CD4+N CAR T细胞系中的CD45RO表达和CD45RA缺失,与慢病毒转导后活化的表型一致。所述CD4+N、CM和EM CAR T细胞系保持了差异化的CD62L表达,这证实了起初的流式分选纯化已经高纯度地完成。
然后,我们分析了源自N、CM和EM细胞亚群的CD4+CAR T细胞的肿瘤识别、细胞因子分泌和增殖,并将它们与由大型CD4+T细胞制备的CAR T细胞系比较。通过IFN-γELISA我们在每个细胞系中都观察到了ROR1-阳性肿瘤细胞的特异性识别。多重细胞因子分析揭示了,源自N细胞亚群的CD4+CAR T细胞产生了目前最高水平的Th1细胞因子尤其是IL-2(图8C),且CFSE染料稀释法显示,它们响应ROR1-阳性肿瘤细胞的刺激进行了最旺盛的增殖(图8B)。
讨论
我们的研究说明了用表达ROR1-CAR的改造的T细胞靶向ROR1-阳性恶性细胞的可能。CD8和CD4+ROR1-CAR T细胞可以源自慢病毒转导任一种大型PBMC后的正常供体,以及来自定义的初始或记忆T细胞亚群的分选纯化的T细胞。CD4+初始、中心记忆和效应T细胞产生的效应细胞因子包括TNFα、IFNγ、IL-2、IL-4和IL-10。通过CAR的信号转导后,源自初始细胞亚群的CAR-转导的CD4+细胞产生的TNFα和IL-2的量,明显高于源自中心和效应记忆细胞亚群的CD4+CAR T细胞。所有CD4细胞类型均能响应ROR1/K562而增殖,然而,在源自初始细胞亚群的CAR-转导的CD4+细胞中,经诱导增殖的T细胞的百分比和增殖的细胞亚群进行分裂的次数明显更高。细胞因子谱和增殖能力均显示,初始CD4+ROR1-CAR T细胞可能最适用于增强CD8+ROR1-CAR CTL。
实施例4-初始CD4+T细胞比记忆CD4+T细胞更有助益
将初始、中心记忆和效应的转导的CD4+T细胞与转导的CD8+细胞毒性T淋巴细胞共培养,并测量所述细胞响应K562/ROR1细胞刺激的增殖反应。
材料和方法
共培养
用CFSE标记源自初始、中心和效应记忆性的ROR1-CAR转导的CD4+T细胞和源自初始和中心记忆性CD8+T细胞的ROR1-CAR转导的CD8+细胞毒性T淋巴细胞,且CD4+和CD8+CART细胞系以1:1的比例进行共培养。然后,用K562/ROR1细胞和对照K562细胞刺激所述共培养物,孵育5天后通过CFSE染料稀释法检测细胞增殖。为了进行流式分析,用偶联的抗-CD8和抗-CD4mAb染色样本以区分CD8+和CD4+亚群。
结果
CD4+初始CAR T细胞在增强CD8+CAR CTL的效应功能方面具有优异能力
我们比较了CD4+N、CM和EM CAR T细胞系的辅助性功能,以确定是否CD4+N CAR T细胞的良好细胞因子谱和增殖潜力也能转化为对于CD8+CAR CTL的最强辅助性效应。此前的工作已经证明,N、CM和EM CD8+T细胞之间存在影响其用于过继性免疫治疗的潜在效用的内在差异。我们的小组最近揭示,过继转移后源自CM而非EM的CD8+T细胞能够长时间持续存在,这使得它们成为用于免疫治疗的优选CD8+T细胞亚群(33,34)。其它小组提出,CD8+N T细胞也可能具有用于T细胞治疗的良好特质(35,36)。因此,我们从分选纯化的N和CM T细胞制备CD8+CAR CTL,以确定CD8+和CD4+CAR T细胞亚群的最佳组合。在慢病毒转导和用tEGFR标志物富集CAR-转导的CD8+T细胞后,我们证实了CD8+N和CM CAR CTL的肿瘤反应性(数据未示出),并和之前一样与CD4+CAR T细胞进行共培养实验。如所预期的,相比与CD4+CM或EM CART细胞的共培养,或单独的CD8+CAR CTL,CD8+N和CM CAR CTL与CD4+N CAR T细胞的共培养导致CD8+亚群显著升高的肿瘤特异性增殖(图9)。在所有组合中,CD4+N CAR T细胞与CD8+CMCAR CTL共培养后,观察到响应ROR1-阳性肿瘤细胞的刺激所产生的最大CD8+CAR CTL增殖(图9)。总之,我们的数据表明,N、CM和EM CD4+T细胞在细胞因子谱和增殖潜力方面存在内在差异,CD4+N T细胞产生更高的IL-2并具备更优的增殖。我们的数据显示,分选纯化的N而非CM、EM或大型CD4+T细胞可能最适合于增强CD8+CTL的效应功能,补充了此前在CD8+T细胞中的工作,即源自CM的CD8+T细胞具有用于过继性免疫治疗的良好特性。
讨论
总之,这些数据表明,ROR1-CAR修饰的CD4+和CD8+T细胞的过继转移在侵袭性全身性淋巴瘤体内模型中赋予了有效的抗-肿瘤反应,提供了CD4+CAR T细胞对于CD8+CAR CTL抗肿瘤功效的有益和协同作用的证据。我们的数据说明,通过细胞内在特性分析如何能够得知对包含肿瘤特异性CD8+和CD4+T细胞的细胞产物合理设计,从而提高癌症免疫治疗的效果。
实施例5-全身性套细胞淋巴瘤(NSG/Jeko-1-ffLuc)的小鼠肿瘤模型
我们在侵袭性全身性套细胞淋巴瘤的体内模型中检测了对ROR1-CAR修饰的CD8+CTL抗肿瘤功效提供CD4辅助的效果。

结果
T-细胞转移后第6天,我们进行生物发光成像以评估肿瘤负荷。在接受CD8+和CD4+ROR1-CAR T细胞的组合的小鼠中观察到最强的抗-肿瘤效果,相比对照组其生物发光信号具有>2log的减少(图10)。我们还在接受CD8+或CD4+ROR1-CAR修饰的T细胞的小鼠中观察到强的抗-肿瘤效果,相比对照组其生物发光信号具有>1log的减少(图10)。重要的是,施用CD8+/CD4+CAR T细胞组合后的肿瘤负荷减少,大于施用CD8+CAR CTL和CD4+CAR T细胞的肿瘤负荷减少总和,这表明CD4+CAR T细胞和CD8+CAR CTL产生了协同作用。
讨论
总之,这些数据表明,ROR1-CAR修饰的CD4+和CD8+T细胞的过继转移在侵袭性全身性淋巴瘤体内模型中赋予了有效的抗-肿瘤反应,并提供了CD4+CAR T细胞对于CD8+CAR CTL抗肿瘤功效具有有益和协同作用的证据。我们的数据说明,通过细胞内在特性分析如何能够得知对包含肿瘤特异性CD8+和CD4+T细胞的细胞产物合理设计,从而提高癌症免疫治疗的效果。
实施例6-CD19CAR T细胞表现出同样的协同作用
在侵袭性全身性套细胞淋巴瘤的体外和体内模型中的共培养物中,我们检测了提供CD4对CD19修饰的CD8+CTL抗肿瘤功效的助益的效果。
材料和方法
CD19CAR T细胞的制备可如US 2008/0131415所描述,其通过引用并入本文中。
共培养检测
用CFSE标记CD19-CAR转导的CD4+T细胞和CD19-CAR转导的CD8+细胞毒性T淋巴细胞,并以2:1、1:1和1:2的比例共培养。然后用K562/ROR1细胞和对照K562细胞刺激所述共培养物,孵育5天后通过CFSE染料稀释测定法来测量细胞增殖。对于流式分析,用偶联的抗-CD8和抗-CD4mAb染色样品,以区分CD8+和CD4+亚群。
体内模型

结果
图10显示了,在用CD19+套细胞淋巴瘤肿瘤系Jeko-1刺激的CD8+CD19-CAR CTL和CD4+CD19-CAR T-细胞系共培养实验中,源自初始亚群的CD4+CAR T-细胞系在增强中心记忆-衍生的CD8+CAR CTL的肿瘤特异性增殖方面的优异能力。尽管源自中心或效应记忆细胞亚群的CD4+CAR T-细胞系增强中心记忆-衍生的CD8+CAR CTL的肿瘤特异性增殖的程度小得多。
图11显示了,CD8+CAR T细胞和CD4+CAR T细胞在免疫缺陷型小鼠(NOD/SCID-Raji)的淋巴瘤模型中独立地赋予了直接的抗肿瘤功效。小鼠接受CD19-CAR转导的或对照模拟转导的(mock-transduced)CD8+中心记忆-衍生的(A),或者CD19-CAR转导的或对照模拟转导的CD4+初始-衍生的T细胞(B)。
图12显示了,在全身性套细胞淋巴瘤(NSG/Jeko-1-ffLuc)的小鼠肿瘤模型中,CD4+ROR1-CAR修饰的T细胞对CD8+ROR1-CAR CTL的抗肿瘤功效的增强和协同作用。在全身侵袭性套细胞淋巴瘤(NSG/Jeko-1)的小鼠肿瘤模型中,相比单独的细胞群或相比未转导的细胞,ROR1-CAR修饰的CD8+和CD4+T细胞的抗肿瘤功效得到增强。
图13显示了,在全身性淋巴瘤(NSG/Raji)的小鼠模型中,CD8+和CD4+CD19-CAR T细胞的协同作用。在肿瘤接种后(处理前)第6天通过生物发光成像证实了Raji肿瘤的移植(处理方案如图A中所示,通过生物发光的肿瘤如图B所示)。在用CD8+CD19-CAR T细胞处理的同批次小鼠,以及在用组合的CD8+和CD4+CD19-CAR T-细胞产物(处理后中间的黑色和灰色条)图B)处理的小鼠中,使用生物发光成像分析肿瘤负荷显示,完全根除了所述Raji肿瘤。然后将所述小鼠第二次接种Raji肿瘤细胞进行激发(challenge),并分析外周血中的CD4+和CD8+CAR T细胞频数和肿瘤移植。在用组合的CD8+和CD4+CAR T-细胞产物处理的小鼠中,肿瘤激发后有显著升高水平的CD8+CART细胞(图C的下部图面),且完全排斥Raji接种物(肿瘤激发后,右边的灰色条,图B)。相反,在接受单独的CD8+CD19-CAR CTL的小鼠中,我们未检测到肿瘤激发后的CAR T细胞增加(图C),并且Raji肿瘤细胞能够进行移植(肿瘤激发后,右边黑色条,图面B)。
讨论
总之,这些数据表明,用另外的CAR构建体,CD19转导细胞、CD19-CAR修饰的CD4+和CD8+T细胞在侵袭性全身性淋巴瘤体内模型中赋予了有效的抗-肿瘤反应,并提供了CD4+CART细胞对于CD8+CAR CTL抗肿瘤功效的有益和协同作用的证据。

Claims (10)
1.过继性细胞免疫治疗组合物,包含:
a)抗原反应性嵌合抗原受体修饰的CD4+辅助性T细胞,其引发直接的肿瘤识别并增强CD8+对病原体的免疫应答,其中所述辅助性T淋巴细胞制剂包含具有嵌合抗原受体的CD4+T细胞,所述嵌合抗原受体包含对疾病或病症相关抗原特异的胞外抗体可变域和T细胞受体的胞内信号传导域;以及
b)生理学上可接受的赋形剂。
2.如权利要求1所述的过继性细胞免疫治疗组合物,还包含:
c)遗传修饰的CD8+细胞毒性T淋巴细胞制剂,其包含具有嵌合抗原受体的CD8+ T细胞,所述嵌合抗原受体包含对疾病或病症相关抗原特异的胞外抗体可变域和T细胞受体的胞内信号传导域。
3.如权利要求1所述的过继性细胞免疫治疗组合物,还包含:
c)引发细胞免疫应答的抗原特异性CD8+细胞毒性T淋巴细胞制剂,其包含源自患者的CD8+ T细胞。
4.如权利要求1-3中任一项所述的过继性细胞免疫治疗组合物,其中所述抗原反应性嵌合抗原受体修饰的CD4+辅助性T细胞源自CD45RO阴性、CD62L阳性CD4阳性T细胞。
5.如权利要求1-4中任一项所述的过继性细胞免疫治疗组合物,其中所述CD4+辅助性T淋巴细胞选自初始CD4+ T细胞、中心记忆性CD4+ T细胞、效应记忆性CD4+ T细胞或大型CD4+ T细胞。
6.如权利要求5所述的过继性细胞免疫治疗组合物,其中所述CD4+辅助性淋巴细胞为初始CD4+T细胞,其中所述初始CD4+T细胞包括CD45RO-、CD45RA+、CD62L+CD4+ T细胞。
7.如权利要求1-6中任一项所述的过继性细胞免疫治疗组合物,其中所述CD8+细胞毒性T淋巴细胞选自初始CD8+ T细胞、中心记忆性CD8+ T细胞、效应记忆性CD8+ T细胞或大型CD8+ T细胞。
8.如权利要求7所述的过继性细胞免疫治疗组合物,其中所述CD8+细胞毒性T淋巴细胞为中心记忆性T细胞,其中所述中心记忆性T细胞包括CD45RO+、CD62L+、CD8+ T细胞。
9.如权利要求1-8中任一项所述的过继性细胞免疫治疗组合物,其中所述CD8+细胞毒性T淋巴细胞为中心记忆性T细胞,且所述CD4+辅助性T淋巴细胞为初始CD4+ T细胞。
10.如权利要求1-9中任一项所述的过继性细胞免疫治疗组合物,其中所述疾病或病症为实体瘤、恶性血液病、黑素瘤或病毒感染。
