Die Erfindung bezieht sich auf pharmazeutische Zusammensetzungen von therapeutisch wirksamen Polypeptidsalzen, welche eine kontinuierliche, gesteuerte und anhaltende Freigabe von derartigen Peptiden bewirken, wenn sie in eine physiologische Umgebung vermittels eines Implantates oder von Injektionen unter die Haut oder in den Muskel von Tieren oder Menschen gebracht werden. 5 Die Erfindung wird weiterhin durch die Verwendung von biologisch abbaubaren und biologisch verträglichen
Polymeren bzw. Copolymeren als Matrix charakterisiert, in welcher wasserunlösliche Polypeptidsalze dispergiert, eingeschlossen oder eingekapselt sind.
Die Notwendigkeit der Herbeiführung langanhaltender Freigabe von Peptiden für die parenterale Verabreichung ist vor längerer Zeit bereits erkannt worden. 10 (cf. T. M. S. Eine Anzahl derartiger Formulierungen, jedoch auf wasserlösliche Polypeptide angewendet, ist vorbeschrieben 15 worden in EP-A2-0052510"Microencapsulation of water solublepolypeptides" undinEP-Al-0058481 "Continuous release pharmaceutical compositions”.
Gegenstand der Erfindung ist eine bioerodierbare pharmazeutische Zusammensetzung für anhaltende und kontrolüertePolypeptid-Wirkstoffireigabe über eine längereZeitspanne, welche als Trägerein Polylactid,Polyglycolid, ein Copolymer von Milch- und Glykolsäure oder eine Mischung derartiger Polymerer, und als Wirkstoff mindestens 20 ein Polypeptidsalz aufweist, vorzugsweise in Form von injizierbaren Teilchen mit einer Größe von 1 bis 500 pm, gegebenenfalls durch Gammastrahlung sterilisiert und in einem pharmazeutisch akzeptablen, für die parenterale Verabreichung zweckmäßigen Träger suspendiert oder in fester, durch Gammabestrahlung sterilisierter und für subcutane Implantation geeigneter Form, welche dadurch gekennzeichnet ist, daß das Polypeptidsalz ein wasserunlösliches Salz ist, d. h. daß die Löslichkeit nach 4-stündigem Dispergieren und/oder Rühren des Peptids bei40 °C . 25 oderdarunternichtmehrals25 mg/1 beträgt, und daß die Zusammensetzung weiters die charakteristische Eigenschaft auf weist, daß bei Einbringen der pharmazeutischen Zusammensetzung in eine wässerige physidogischeUmgebung das Peptid kontinuierlich über eine Zeitspanne von wenigstens einer Woche freigesetzt wird, wobei die anfängliche Freigabe während der ersten 24 Stunden höchstens 30 % der insgesamt freigegebenen Menge an Peptid beträgt
Das gegenüber dem Stand der Technik gemäß den obenzitierten EP-Dokumenten neue, übemaschende und 30 vollkommen unerwartete Merkmal gemäß der vorliegenden Erfindung beruht auf der Tatsache, daß therapeutisch zweckmäßige, lang anhaltende und gesteuerte Freigabezusammsetzungen durch Verwendung von im wesentlichen wasserunlöslichen Polypeptidsalzen in vorteilhafter Weise erhalten werden können, wobei diese Peptidsalze eine unmeßbar kleine Löslichkeit in wässeriger Lösung bei Raum- oder Körpertemperaturen haben und dennoch eine wirksame und gesteuerte Freigabe der Polypeptidsalze ergeben, wenn sie in Form der hier beschriebenen 35 Zusammenseztung in eine physiologische, im wesentlichen wäßrige Umgebung parenteral verabreicht werden.
Es ist ein neues und überraschendes Merkmal der vorliegenden Erfindung, daß Polypeptide, welche normalmweise in der Natur oder wenn sie synthetisch hergestellt werden, wasserlöslich sind in vorteilhafter Weise durch Bildung unlöslicher Zusatzsalze wasserunlöslich gemacht werden, beispielsweise mit Embonsäure, Gerbsäure, Stearinsäure und anderen nicht-toxischen wasserunlöslichen Säuren, vor ihrer Mikroverkapselung oder Dispersion 40 in einer biologisch abbaubaren Polymermatrix.
Die Verwendungvon geringlöslichen oder wasserunlöslichen Derivaten ist bekannt, und zwar auch auf dem Feld der Peptide (vergl. Schally et al. US-PS 4 010 125, Spalte 7, Zeile 25), wenn langsam freigebende Depot-Dosageformen benötigt werden.
Wenn jedoch biologisch abbaubare Polymere, wie beispielsweise Polymilchsäure, Polyglykolsäure, 45 Polyhydroxybuttersäure, Polyorthoester, Polyacetale und dgl. als Arzneimittelträger eingesetzt werden, hat die Freigabe der Peptide in kontinuierlicher Art und Weise üblicherweise eine bemerkenswerte Wasserlöslichkeit erfordert Versuchsberichtehaben gezeigt, daßderbiologische Abbau vonPolymeren (beispielsweise vonPolylactiden und Polylactid-Co-Glykoliden) zur Wasseraufnahme und Erzeugung von wasserhaltigen Kanälen oder Poren führt, aus welchen die Peptide austreten, weil sie wasserlöslich sind. 50 DieerfindungsgemäßeZusammensetzung besteht im Gegensatz dazu aus Partikeln, insbesondere Mikropartikeln von Peptidsalzen wasserunlöslicher Natur, welche in einer bioabbaubaren Polymermatrix gleichmäßig dispergiert sind. Die spezifische Struktur ist der Beschreibung zu entnehmen. Sie entspricht insbesondere einer Methode, gemäß welcher das Peptidsalz vor einem Entfernen des Lösungsmittels und einem darauffolgenden Trocknen in die Polymerlösung eindispergiert wird. Die Peptidsalze sind ganz gezielt als in Wasser unlösliche Salze ausgewählt und 55 ihre verschwindende Restlöslichkeit ist eng begrenzt Die im Rahmen der vorliegenden Erfindung einzusetzenden
Peptide, also besonders LHRH-und Somatostatin-Analoga, sind aus dem Stand derTechnik als hoch wasserlösliche Peptide bekannt Die Erfindung beinhaltet nun im speziellen, solche Peptide durch Überführungin spezifische Salze ganz gezielt in wasserunlösliche Form zu bringen. -2-

Im Gegensatz zur Erfindung beschreibt die EP-A2211267Zusammensetzungen für parenterale Anwendung, die auf einem kompaktierten und teilweise überzogenen bzw. umhüllten Fettsäuresalz eines Peptids basieren, das aber dennoch eine nicht unbeachtliche Löslichkeit in Wasser aufweist Seite 3, Zeilen 23-31 geben dort eine klare Darstellung der Herstellung solch«: Mittel: das gewöhnliche Peptid-Fettsäuresalz wird zuerst durch Kompaktieren oder Extrudieren zusammengepreßt und dann mit einem nicht- oder doch bioabbaubaren Polymeren überzogen. Für den Erhalt einer brauchbaren Wirksamkeit wird dann ein Teil der Beschichtung entfernt Im großen und ganzen kann die Struktur bzw. der aufwendige Aufbau dieser Zusammensetzung nur als Vielzahl von mit einem Polymer umhüllten bzw. beschichteten "Herzstücken” aus einem kompakten Peptid-Salz-Partikel bezeichnet werden.
Es bestdien also ganz wesentlicheunterschiede zwischen diesem Stand der Technik und der Erfindung allein nur beim Vergleich von deren Aufbau, der aber schon als solcher ganz wesentlich für die Wirkung bei Verabreichung ist
Was mm die anfängliche Freigabe des Peptids während der ersten 24 h mit einem Maximalwert von 30 % von der Gesamtmenge betrifft, so ist klar festzustellen, daß auch hier ein wesentlicher Unterschied zwischen Erfindung und dem Stand der Technik besteht (siehe z. B. dazu Seite 2 der EP-Al 58 481), gemäß welchem die dortige Zusammensetzung aus einer Polymermatrix miteinemwasserlöslichenPeptid-Salz gebildet ist undeine anfängliche Freigabe von mehr als 30 % Wirkstoff, bezogen auf dessen Gesamtmenge, während der ersten 24 h auf weist
Die US-PS 4 667 014 spricht hingegen von einem pharmazeutisch annehmbaren, nicht toxischen Salz der dort angeführten LHRH-Analogen mit einem geringen Grad an Löslichkeit in Körperflüssigkeiten. Zu den das Salz bildenden Säuren wird in Zeilen 23 - 29 der Spalte 12 festgestellt: "Die Verbindungen gemäß der vorliegenden Erfindung, oder bevorzugt ein solches relativ unlösliches Salz wie eben beschrieben, kann in ein Gel, z. B. in ein Aluminium-Monostearat-Gel, z. B. mit Sesamöl formuliert werden, geeignet für Injektionen. Besonders bevorzugte Salze sind Zinksalze, Zinktannat-Salze, Pamoat-Salze u. dgl."
Ferner wird erwähnt daß"die Verbindungen oder bevorzugt deren relativunlösliche Salze ...auchin "Cholesterol-Pellets", z. B. für die Verabreichung an Tiere, formuliert werden können."
Beide Fälle repräsentieren aber bloß den bisher üblichen trivialen Weg, wie diese Salze angewandt werden.
Wenn dort jedoch von anderen Formulierungen auf Polymer-Basis, wie Polymilchsäure-Basis, die Rede ist so sind diese verknüpft mit wasserlöslichen LHRH-Analogen oder deren wasserlöslichen Salzen, wie z. B. Acetaten, und zwar aus den gleichen Gründen wie bei der vorher schon kurz erwähnten EP 58 481, insbesondere, was den Freisetzungsmechanismus betrifft
Es ist einzuräumen, daß die EP 58 481 nicht ausdrücklich wasserunlösliche Peptidsalze ausschließt aber eine eingehende Analyse dieses Dokuments führt zu einem diesbezüglich ganz eindeutigen Schluß:
Zum Beispiel ist auf Seite 4 dieser EP geoffenbart, daß die dortige Erfindung "am meisten brauchbar ist für Polypeptide, welche relativ hydrophil sind."
Die vorliegende Erfindung sieht ganz im Gegensatz dazu wasserunlösliche Salze vor.
Zur Freigabe des Peptids wird dort ferner festgestellt: "Die dort geoffenbarte Materie unterscheidet sich von unserer Erfindung darin, daß die Freisetzung der Wirksubstanz aus der porösen Polymermatrix im wesentlichen auf der wasserlöslichen Natur des Wirkstoffes beruht" (siehe besonders S 6 - 8 der EP-Al 58 481).
Damit bestätigt sich aber eindeutig, daß Salze, wie sie gemäß der vorliegenden Erfindung Einsatz finden, vom Stand der Technik nicht ins Auge gefaßt worden sind.
Auch die EP 145 240 ist deshalb nicht relevant, weil sie sich ausschließlich auf "wasserlösliche Medikamente" bezieht
Der sonstige Stand der Technik bezieht sich zwar auch teilweise auf Peptid-Salze, wie Pamoate oder Tannate, davon daß diese Salze für eine V erwendung zum Dispergieren in einer bioabbaubaren Polymermatrix eingesetzt oder auch nur vorgesehen wären, ist jedoch nirgends die Rede. Es erfolgt nur eine Verwendung der Salze selbst in einem Öl-Suspensionsmedium.
Die erfinderische Erkenntnis, daß Peptide aus Matrizen und Mikrokapseln in einem in höchstem Maße erwünschten Freigabemuster freigegeben werden können, wenn deren Wasserlöslichkeit praktisch auf das Null-Niveau verringert worden ist ist vollkommen überraschend und widerspricht also den Lehren des Standes der Technik. Insbesondere ist gemäß der Erfindung gefunden worden, daß die Freigabe gewisser Polypeptide, beispielsweise von D-Trp^-LHRH, aus polymeren Matrizen hinsichtlich der Gleichförmigkeit und der Dauer umso besser ist, je mehr das zugesetzte Salz des Polypeptides wasserunlöslich ist. "Wasserunlöslichkeit" ist hierbei als derjenige Betrag an Polypeptid definiert worden, welcher in Lösung gemessen werden kann, wenn das Salz 4 Stunden in destilliertem Wasser bei Temperaturen von 40 °C oder darunter dispergiert oder verrührt wird, wobei ein solcher Betrag bei 25 mg/1 oder darunter (0 bis 25 ppm) liegt
Es ist in höchstem Maße erwünscht, biologisch aktive Polypeptide kontinuierlich und für eine langandauemde Zeitspanne, von einer Woche bis zu mehreren Monaten, zu verabreichen. Es ist auch in höchstem Maße erwünscht, daß das Freigabemuster gesteuert wird, beispielsweise um ungleichmäßige Freigaben des Polypeptides zu Beginn, -3-
AT 397 035 B in der Mitte oder am Ende des therapeutischen Behandlungszyklusses zu vermeiden. Es ist oft gefunden worden, daß Polypeptide aus biologisch abbaubaren Matrizen stoßförmig freigegeben werden (sog. Burst-Effekt), und zwar entweder zu Beginn des Zyldusses oder an dessen Ende, wenn die Polymermatrix durch Hydrolyse zerstört worden ist 5 Ein bemerkenswertes Merkmal der vorliegenden Erfindung ist die Steuerung des Freigabemusters, und insbesondere eine Verringerung des anfänglichen Stoßeffektes. Das wasserunlösliche Polypeptidsalz wird in einem geringeren Ausmaß als die entsprechenden wasserlöslichen Derivate freigegeben, wodurch auf diese Art und Weise eine verlängerte Freigabezeit entstehtund eine Überdosierungdes Patienten vermieden wird. Durch Umformen eines normalerweise wasserlöslichen Polypeptids in ein unlösliches Polypeptidsalz, kann gemäß der Erfindung der 10 anfängliche Stoßeffekt (d. h. die Menge des innerhalb derersten 24 Stunden freigegebenen Peptides) auf weniger als 30 % der Gesamtdosis begrenzt werden.
Beispiel I 50 g eines Copolymers aus D, L-Lactid und Glykolid mit einem 50/50 Molar-Verhältnis von DJL-Lactid zu IS Glykolid und einem mittleren Molekulargewicht von 50 000 wird in 950 g Methylenchlorid gelöst
Die Lösung wird durch ein Milliporen-Filter hindurchgeführt, um teilchenförmige Stoffe und Pyrogene zu entfernen. 1 g D-Trp^-LHRH-Embonsäuresalz wird dieser Lösung zugegeben und mittels eines hochwirksamen Scherwiikungsmischers dispergiert
Die erzeugte Mischung wird in einen rotierenden Verdampfer gebracht und der größte Teil des Methylenchlorids 20 wird unter Vakuum entfernt Die gebildete dicke Dispersion wird auf eine Glasplatte gegossen und mittels eines einstellbaren Schiebers verteilt der auf 0,7 mm eingestellt ist
Nach Lufttrocknen wird der gebildete Film während einer Zeitspanne von 48 Stunden unter Vakuum getrocknet und sodann durch eine 0,8 mm große Öffnung bei 70 °C unter Druck extrudiert. Die sich ergebenden Stangen werden mit Hilfe einer Tieftemperatureinrichtung bei -40 °C zermahlen. 25 Das gebildete granulatfömige Material wirddurchein 180 μ-Sieb hindurchgesiebt die unterseitige Fraktion wird gesammelt und durch Aussetzen einer Gammastrahlung zwischen 2,5 und 2,8 Mrad sterilisiert.

Das gleiche Verfahren, wiebeim Beispiel I angegeben, wird durchgeführt indem D-Tip^-LHRH-Embonsäuiesalz 30 durch D-Trp^-LHRH-Stearinsäuresalz ersetzt wird.

Das Verfahren nach den Beispielen I bis IV wird mit D, L-Lactid-Co-Polyglykolid-Copolymeren bzw. D,L-Lactid-Polymeren angewendet bei welchen das molare Verhältnis 67 % D, L Lactid, 33 % Glykolid, 75 % D, L 50 Lactid 25 % Glykolid oder 100 % D, L Lactid betrug.
Beispiel VI
Das Verfahren nach den Beispielen I bis V wurde mit den wasserunlöslichen Embonsäuresalzen, Gerbsäure- oder Stearinsämesalzen von einem oder mehreren der folgenden Peptide durchgeführt: Oxytocin, Vasopressin, ACTH, 55 Calcitonin, epidermaler Wachstumsfaktor, Prolactin, Inhibin, Interferon, LHRH. Somatostathin, Insulin, Glucagon, atrialer Natriuretikfaktor, Endorphin, Renininhibitor, GHRH, Peptid-T oder synthetischen Analogen und Modifikationen der vorgenannten Peptide. -4-

Bioerodierbare pharmazeutische Zusammensetzung für anhaltende und kontrollierte Polypeptid-Wirkstofffrei-gabe über eine längere Zeitspanne, welche als Träger ein Polylactid, Polyglycolid, ein Copolymer von Milch- und Glykolsäure oder eine Mischung derartiger Polymeren, und als Wirkstoff mindestens ein Polypeptidsalz auf weist, vorzugsweise in Form von injizierbaren Teilchen mit einer Größe von 1 bis 500 pm, gegebenenfalls durch 15 Gammastrahlung sterilisiert und in einem pharmazeutisch akzeptablen, für die parenterale Verabreichung zweck mäßigen Träger suspendiert oder in fester, durch Gammastrahlung sterilisierter und für subcutane Implantation geeigneter Form, dadurch gekennzeichnet, daß das Polypeptidsalz ein wasserunlösliches Salz ist, d. h., daß die Löslichkeit nach 4-stündigem Dispergieren und/oder Rühren des Peptids bei 40 °C oder darunter nicht mehr als 25mg/1 beträgt und daß dieZusammensetzungweitersdiecharakteristischeEigenschaftaufweist,daß beiEinbringen 20 der pharmazeutischen Zusammensetzung in eine wässerige physiologische Umgebung das Peptid kontinuierlich über eine Zeitspanne von wenigstens einer Woche freigesetzt wird, wobei die anfängliche Freigabe während der eisten 24 Stunden höchstens 30 % der insgesamt freigegebenen Menge an Peptid beträgt.
- 2. Pharmazeutische Zusammensetzung nach Anspruch 1, dadurch gekennzeichnet, daß das wasserunlösliche 25 Polypeptidsalz ein pharmazeutisch akzeptables Salz von LHRH oder eines synthetisch hergestellten Analogen darstellt.
- 3. Pharmazeutische Zusammensetzung nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß das wasserunlösliche Polypeptidsalz ein pharmazeutisch akzeptables Salzaus der Gruppe derEmbonsäure-, Gerbsäure- und Stearinsäuresalze 30 darstellt.
- 4. Pharmazeutische Zusammensetzung nach Anspruch 1, dadurch gekennzeichnet, daß das wasserunlösliche Polypeptidsalz ein pharmazeutisch akzeptables Salz von Oxytocin, Vasopressin, ACTH,Calcitonin, dem epidermalen Wachstumsfaktor, Prolactin, Inhibin, Interferon, Somatostatin, Insulin, Glucagon, dem atrialen Natriuretikfaktor, 35 Endorphin, einem Renininhibitor, Wachstumshormon-Freigabehormon, Peptid Tbzw. von synthetischen Analogen und Modifikationen derselben darstellt.
- 5. Pharmazeutische Zusammensetzung nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß sie als wasserunlösliches Polypeptidsalz das Embonsäuresalz von D-Tip^-LHRH enthält 40
- 6. Pharmazeutische Zusammensetzung nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß sie als wasserunlösliches Polypeptidsalz das Embonsäuresalz von i-1 D-Phe-Cys-Tyr-D-Tip-Lys-Val-Cys-Trp-NH2 45 enthält.
- 7. Verfahren zur Bereitung einer pharmazeutischen Zusammensetzung nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, daß das wasserunlösliche Salz eines Polypeptids in einer Lösung von Polylactid, Polyglykolid, einem Copolymeren aus Milch- und Glykolsäuie oder einer Mischung derartiger Polymere suspendiert wird, das 50 Lösungsmittel durch Trocknen entfernt wird und die sich ergebende Mischung in feste, für die parenterale Injektion oder subcutane Implantation geeignete Partikel umgeformt wird.
- 8. Verfahren zur Bereitung einer pharmazeutischen Zusammensetzung nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, daß das wasserunlösliche Salz eines Polypeptids in einer Lösung von Polylactid, Polyglycolid, 55 einem Copolymeren aus Milch- und Glykolsäure oder einer Mischung derartiger Polymere suspendiert wird, ein Koazervationsmittel zugemengt wird und die resultierenden Mikrokapseln in eine pharmazeutisch akzeptable, härtende Flüssigkeit eingegossen werden und aus der Suspension gesammelt werden. -5-
