<Desc/Clms Page number 1> Die Erfindung betrifft eine Zubereitung zur stetigen und kontrollierten Freisetzung von medikamentösen Peptidsubstanzen in Form von Mikrokugeln aus einem biologisch abbaubaren polymeren Material, welches die medikamentöse Substanz enthält, sowie ein Verfahren zu deren Herstellung. Die Substanz hat die Formel (I) EMI1.1 (I) Aus PROCEEDINGS OF THE NATIONAL ACADEMY OF SCIENCES OF THE UNITED STATES OF AMERICA, Vol. 88, Februar 1991, Seiten 844 - 848, ist es bekannt, wasserlösliches Trifluoracetatsalz eines LHRH-Analagons (LHRH = Releasing-Hormon des luteinisierenden Hormons) in Mikrokapseln einzubringen, wobei auch "SB-75-pamoate", also Pamoate dieses LHRHAnologons, eingebettet in einer Polymermatrix erwähnt sind. Das Verfahren zur Herstellung der vorstehend angegebenen Zubereitung besteht darin, dass man zunächst ein wasserlösliches Peptidsalz in ein wasserunlösliches Peptidsalz überführt, worauf man das Peptidsalz in einer Lösung eines biologisch abbaubaren polymeren Materials suspendiert, die Suspension in eine Emulsion vom Typ ÖI-in-Wasser überführt und die Mikrokugeln aus dem biologisch abbaubaren Polymer isoliert, nachdem man die ÖI-in-Wasser-Emulsion in einen Überschuss eines wässrigen Mediums übergeführt hat. Nach dem Stand der Technik wurden bisher verschiedene Lösungen zur Herstellung von Zubereitungen zur stetigen und kontrollierten Freisetzung von medikamentösen Substanzen vorgeschlagen, wobei biologisch abbaubare Implantate, Mikrokapseln oder biologisch abbaubare poröse Matrices hergestellt wurden, die beispielsweise als Mikrokugeln oder Mikroteilchen mit unterschiedlichen Abmessungen erhalten wurden. In diesem Zusammenhang ist auf die EP 0052510 A2 (Mikroverkapselung durch Phasentrennung von wasserlöslichen Medikamenten) und auf die EP 0058481 A1 oder die US 3976071 A (Herstellung von Implantaten oder von biologisch abbaubaren porösen Matrices, hauptsächlich auf der Basis von Polylactiden oder CopolylactidGlycoliden) hinzuweisen. Bei diesen Verfahren wird das als Träger verwendete biologisch abbaubare Polymer oder Copolymer zunächst in einem organischen Lösungsmittel gelöst, worauf die medikamentöse Substanz selbst gelöst wird, wenn sie benötigt wird Andere Verfahren, nach denen ebenfalls Mikrokapseln oder Mikrokugeln erhalten werden, machen von Emulsionsmethoden Gebrauch, deren wichtigste Stufe darin besteht, dass eine Emulsion vom Typ 01-in-Wasser aus einer organischen Lösung des polymeren Materials und einer wässrigen Lösung des Peptids hergestellt wird (vgl. die US 4384975 A, US 3891570 A, US 4389330 A, US 3737337 A, US 4652441 A oder die WO 90 13361 A1. In jedem Fall war man gezwungen, komplizierte und schwer beherrschbare Methoden zu entwickeln, um Verluste an den leicht wasserlöslichen aktiven Peptidsubstanzen möglichst klein zu halten, z. B. durch eine doppelte Emulgierung. Gemäss FR 2 649 319 A1 werden in einem 5-stufigen Verfahren Mikropartikel hergestellt, wobei ein "agent inducteur de phase ä la dispersion" hinzugefügt werden muss, um Mikropartikel zu bilden. Die Mikropartikel werden schliesslich aus einer ÖI-in-Wasser-Emulsion gewonnen, was mit der vorliegenden Erfindung nichts zu tun hat. Ferner ist zu sagen, dass dieses Patent nur Embonatsalze von Peptiden des Somatostatin-Typs beschreibt. Bei einem Verfahren, bei dem die Bildung einer Emulsion vom Typ ÖI-in-Wasser und die anschliessende Überführung in ein wässriges Medium angewendet wird, ermöglicht die Erfindung überraschenderweise eine erfolgreiche Überwindung der Nachteile des Standes der Technik. Indem zunächst ein wasserlösliches Peptidsalz in ein wasserunlösliches Peptidsalz der Formel (I) überführt wird, zeigt die Erfindung der Fachwelt einen neuartigen Weg, um die relativen Löslichkeiten der verwendeten Substanzen, insbesondere der Lösungsmittel und "Nicht-Lösungsmittel", mit Vorteil auszunutzen. Die Peptide mit der Formel (I) werden als LHRH-Analoge definiert ; sie können mit Vorteil bei der therapeutischen Behandlung von hormonbedingten Beschwerden verwendet werden. Falls nichts anderes angegeben ist, haben die Aminosäuren in der Formel (I) die L-Konfiguration. D-Nal bedeutet D-3-(2-Naphthyl)-alanin und D-Pal bedeutet D-3-( 3-Pyridyl)-alanin. Die Zubereitung gemäss der Erfindung liegt in Form von Mikrokugeln aus einem biologisch <Desc/Clms Page number 2> abbaubaren polymeren Material vor, das ein wasserunlösliches Salz des Peptids der Formel (I) einschliesst. Diese Zubereitung, die z. B. 5 Gew.-% wasserunlösliches Salz einschliesst, ermöglicht die stetige Freisetzung des Peptids über mehrere Tage nach der parenteralen Verabreichung an Menschen oder Tiere. Gegenstand der Erfindung ist ferner ein Verfahren zur Herstellung der vorstehend definierten Zubereitung, das dadurch gekennzeichnet ist, dass a) ein wasserlösliches Peptidsalz der Formel (I) in ein wasserunlösliches Peptidsalz überführt wird ; b) das wasserunlösliche Peptidsalz in einem organischen Medium, welches das biologisch abbaubare polymere Material in gelöstem Zustand enthält, suspendiert wird; c) die organische Suspension in einem wässrigen Medium, welches.die zusammenhängende Phase der erhaltenen Emulsion bildet, dispergiert wird; d) die Emulsion in einen Überschuss eines wässrigen Mediums überführt wird und die hierbei erhaltenen Mikrokugeln schliesslich von der flüssigen Phase abgetrennt werden. Die erste Stufe des Verfahrens besteht darin, dass man in an sich bekannter Weise ein wasserlösliches Peptidsalz in ein wasserunlösliches Peptidsalz überführt. Unter "wasserlöslich" versteht man ein Peptidsalz mit einer Wasserlöslichkeit von > 0,1 mg/mi bei 25 C, vorzugsweise von # 1,0 mg/ml. Unter "wasserunlöslich" versteht man ein Peptidsalz mit einer Wasserlöslichkeit von # 0,1 mg/mi bei 25 C. Peptidsalze, wie das Pamoat (Embonat), Tannat, Stearat oder Palmitat fallen unter diese Definition. Als biologisch abbaubares polymeres Material werden am häufigsten Polymere, wie z.B. Polylactide, Polyglycolide oder Copolymere aus Milchsäure und Glycolsäure verwendet. Unter den bevorzugten polymeren Materialien sind zur erwähnen die Copolymeren aus Milchsäure und Glycolsäure (PLGA), insbesondere die Copolymeren aus L- oder D,L-Milchsäure, die 45 bis 90 Mol-% Milchsäureeinheiten und 55 bis 10 Mol-% Glycolsäureeinheiten enthalten. Als Lösungsmittel für das polymere Material kann man ein organisches Lösungsmittel, wie Methylenchlorid, verwenden, wobei aber das Lösungsmittel in jedem Fall ein "Nichtlösungsmittel" für das gewählte Peptidsalz sein muss. Sobald das Peptidsalz in der organischen Lösung des polymeren Materials suspendiert ist, wird diese Lösung erfindungsgemäss in eine vorbestimmte Menge eines wässrigen Mediums, im allgemeinen Wasser, das mit einem geeigneten Tensid versetzt ist, eingebracht. Dadurch soll schnell eine homogene Emulsion vom Typ 01-in-Wasser gebildet werden, wobei das wässrige Medium die zusammenhängende Phase bildet. Bei der Herstellung einer solchen Emulsion sind verschiedene Faktoren zu berücksichtigen, die die Grösse oder die Struktur der erhaltenen Mikrokugeln bestimmen. Einer dieser zu berücksichtigenden Faktoren ist die Zugabegeschwindigkeit der organischen Lösung zum wässrigen Medium ; anderer kann die Temperatur, die Rührgeschwindigkeit oder die Dispergierenergie (Ultraschallbehandlung) sein, wobei besonders der zuletzt erwähnte Parameter die Grösse der erhaltenen Mikrokugeln beeinflusst. Es liegt im Ermessen des Durchschnittsfachmanns, geeignete Verfahren und Bedingungen für die Emulgierung auszuwählen. Bei der Herstellung der Emulsion kann es vorteilhaft sein, das Volumenverhältnis zwischen den miteinander in Berührung stehenden Phasen zu modifizieren, insbesondere das Ausgangsvolumen der organischen Phase im Vergleich zu dem der wässrigen Phase zu vermindern. In einigen Fällen kann aufgrund der Flüchtigkeit der verwendeten organischen Lösungsmittel (z.B. Methylenchlorid) bereits die beim Rühren spontan auftretende Verdampfung ausreichend sein; in anderen Fällen kann diese erwünschte Erscheinung dadurch beschleunigt werden, dass man eine partielle Verdampfung unter vermindertem Druck vornimmt. Sobald sich die organisch-wässrige Emulsion stabilisiert hat, wird sie in einen Überschuss eines wässrigen Mediums, im allgemeinen Wasser, übergeführt. Durch diese Massnahme soll die Härtung der in der Emulsion "embryonal" gebildeten Mikrokugeln intensiviert werden, indem das noch innerhalb der Mikrokugeln befindliche organische Lösungsmittel extrahiert wird. Mit dieser Massnahme wird gleichzeitig auch bezweckt, dass Spurenmengen des Tensids, die etwa während der Aushärtungsphase in der Masse des Materials verblieben sind, entfernt werden. Es ist auch darauf hinzuweisen, dass Wasser sowohl für das biologisch abbaubare polymere Material, z. B. für PLGA, als auch für das in den Mikrokugeln festgehaltene Peptidsalz ein "Nicht-Lösungsmittel" ist. Diese <Desc/Clms Page number 3> Situation ist besonders günstig für die erforderliche Extraktion des restlichen Lösungsmittels für das Polymer, z. B. von CH2CI2. Nachdem die Emulsion in einen Überschuss des wässrigen Mediums übergeführt worden ist, werden die gehärteten Mikrokugeln in an sich bekannter Weise gesammelt, beispielsweise durch Zentrifu- gieren, Filtrieren oder Absitzen durch Schwerkraft. Das gleiche gilt für die Wasch-, Reinigungs- und Trocknungsschritte. Einer der Vorteile des Verfahrens gemäss der Erfindung besteht darin, dass man Mikrokugeln mit genau einstellbarer Grösse erhält, wobei diese Einstellung hauptsächlich während der Herstel- lung der Emulsion erfolgt. (z.B. durch die Rührgeschwindigkeit). Ein weiterer Vorteil besteht dann, dass eine besonders hohe Beladung mit Peptid erreicht werden kann, die 5,10 oder 20 Gew.-% oder sogar noch höher sein kann, was von den Bedingungen abhängt. Weiterhin ist die Ausbeute beim Einbau des Peptidsalzes besonders hoch ; beruht hauptsächlich auf der vorhergehenden Umwandlung des Peptids aus einem wasserlöslichen Salz in ein wasserunlösliches Salz. Die nach dem Verfahren gemäss der Erfindung aus den vorstehend angegebenen Bestandtei- len erhaltenen Mikrokugeln werden, nachdem sie in geeigneter Weise sterilisiert wurden, zur Herstellung von Suspensionen für die parenterale Verabreichung, z.B. durch intramuskuläre oder subcutane Injektion, verwendet. Die Erfindung ist durch die nachstehenden Beispiele erläutert. Die in diesen Beispielen angewendeten Betriebsbedingungen sollen die Erfindung nicht beschränken. Beispiel 1 3 g des Acetats eines LHRH-Analogen der Formel: Ac-D-Nal-D-pCI-Phe-D-Pal-Ser-Tyr-D-Cit-Leu-Arg-Pro-D-AlaNH2 wurden nach den üblichen Methoden in das entsprechende Pamoat umgewandelt und so behandelt, dass Teilchen mit einer Durchschnittsgrösse von etwa 10 m erhalten wurden. 0,317 g dieses Pamoats wurden dann in 20 ml CH2CI2 suspendiert, und die Suspension wurde zu 20 ml CH2CI2 gegeben, die 1,683 g gelöstes Copolymer aus D,L-Milchsäure und Glycolsäure (PLGA) im Molverhältnis 75 :25 enthielt (Grenzviskosität 0,82 in (HFIP (Hexafluoroisopropanol)). Das Gemisch wurde bei Raumtemperaturen unter Rühren hergestellt, wobei eine vollkommen homogene Suspension erhalten wurde. Die erhaltene Suspension wurde dann auf einmal in 500 ml Wasser gegossen, in dem 0,075 % Methoxycellulose gelöst waren, und das Gemisch wurde etwa 90 Minuten bei Raumtemperatur weitergerührt (Rührgeschwindigkeit: 900 U/min). Die Entwicklung der Emulsion wurde in regelmä- #igen Zeitabständen, durchschnittlich alle 30 Minuten, überwacht, indem eine Probe entnommen wurde und die erhaltenen Mikrokugeln unter einem Mikroskop betrachtet wurden. Nach Beendigung des Rührens (Stabilisierung der Zerkleinerung der Mikrokugeln) wird die Emulsion auf einmal in zwei Liter Wasser übergeführt, das auf etwa 10 C gehalten wird, während das Gemisch bis zur Homogenisierung gerührt wird. Die PLGA-Mikrokugeln wurden aus dem Reaktionsgemisch isoliert und abwechselnd durch Zentrifugieren und Waschen mit H20 gereinigt, schliesslich filtriert und unter vermindertem Druck getrocknet. Es wurden auf diese Weise 1,61 g PLGA-Mikrokugeln gesammelt (Ausbeute: 80 %), wobei mehr als 94 % der Teilchen einen Durchmesser von weniger als 100)im hatten (Maximum bei 55-85m). Die Analyse (Auflösung der festen PLGA, Extraktion und Bestimmung des Peptids durch HPLC) zeigt, dass die Mikrokugeln mit 9,05 Gew. -% Pamoat beladen sind (theoretische Menge: 10 %). Die so erhaltenen Mikrokugeln wurden anschliessend mit Gammastrahlen sterilisiert und in einem geeigneten sterilen Träger suspendiert. Versuche in vivo (Bestimmung des Blut-Testosteron-Spiegels in männlichen Ratten) zeigten, dass der Wirkstoff gleichmässig freigegeben wurde. Beispiel 2 <Desc/Clms Page number 4> Es wurde genau wie nach Beispiel 1 mit 0,634 g des Pamoats des LHRH-Analogen auf 1,366 g PLGA (Molverhältnis 75 :25) gearbeitet. Es wurden 1,70 g PLGA-Mikrokugeln erhalten (Ausbeute: 85 %). Die Beladung betrug 18,3 % (theoretischer Wert : %). Die so erhaltenen Mikrokugeln wurden anschliessend mit Gammastrahlen sterilisiert und in einem geeigneten sterilen Träger suspendiert. Versuche in vivo (Bestimmung des Blutserumspiegels des Analogen in männlichen Ratten) bestätigten, dass eine biologisch signifikante Menge Wirkstoff über mindestens 24 Tage gleichmässig freigesetzt wurde. Der Gewichtsverlust der Hoden betrug mindestens 80 %, der Gewichtsverlust der Samenbläschen betrug mindestens 90 %. Beispiel 3 Es wurde genau wie nach Beispiel 1 mit dem Peptid der Formel: Ac-D-Nal-D-pCIPhe-D-Trp-Ser-Tyr-D-Cit-Leu-Arg-Pro-D-Ala-NH2 gearbeitet. Bei der Umwandlung des Acetatsalzes in das entsprechende Pamoat und Weiterbehandlung desselben wie nach Beispiel 1 wurden ähnliche Ergebnisse erhalten. PATENTANSPRÜCHE : 1. Zubereitung zur stetigen und kontrollierten Freisetzung einer medikamentösen Peptidsub- stanz der Formel (I): Ac-D-Nal-D-pClPhe-R3-Ser-Tyr-D-Cit-Leu-Arg-Pro-D-Ala-NH2 (I) worin R3D-Pal oder D-Trp bedeutet, dadurch gekennzeichnet, dass sie in Form von Mikro- kugeln aus einem biologisch abbaubaren polymeren Material vorliegt, welches ein wasser- unlösliches Salz des Peptids der Formel (I) enthält.
Claims (1)
- 2. Zubereitung nach Anspruch 1, dadurch gekennzeichnet, dass das wasserunlösliche Peptid ein Pamoat, Tannat, Stearat oder Palmitat darstellt.3. Zubereitung nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass das biologisch abbau- bare polymere Material ein Polylactid, ein Polyglycolid oder ein Copolymer aus Milchsäure <Desc/Clms Page number 5> und Glycolsäure darstellt.4. Zubereitung nach Anspruch 3, dadurch gekennzeichnet, dass das Copolymer aus Milch- säure und Glycolsäure ein Copolymer aus L- oder D,L-Milchsäure mit 45 bis 90 Mol-% Milchsäureeinheiten und 55 bis 10 Mol-% Glycolsäureeinheiten darstellt.5. Zubereitung nach einem der Ansprüche 1 bis 4 in Form von Mikrokugeln aus einem Copo- lymer aus Milchsäure und Glycolsäure im Molverhältnis 75 :25, welches mindestens 5 %, bezogen auf das Gewicht des Pamoatsalzes, eines Peptids der Formel (I) enthält.6. Verwendung der Zubereitung nach Anspruch 1 in einer pharmazeutischen Zubereitung zur parenteralen Verabreichung.7. Verfahren zur Herstellung einer Zubereitung nach Anspruch 1, dadurch gekennzeichnet, dass a) ein wasserlösliches Peptidsalz der Formel (I) in ein wasserunlösliches Peptidsalz über- führt wird, b) das wasserunlösliche Peptidsalz in einem organischen Medium, welches das biolo- gisch abbaubare polymere Material im gelösten Zustand enthält, suspendiert wird; c) die organische Suspension in einem wässrigen Medium, welches die zusammenhän- gende Phase der erhaltenden Emulsion bildet, dispergiert wird; d) die Emulsion in einen Überschuss eines wässrigen Mediums überführt wird und die hierbei erhaltenen Mikrokugeln schliesslich von der flüssigen Phase abgetrennt werden.8. Verfahren nach Anspruch 7, dadurch gekennzeichnet, dass vor der Überführung der 0)-in- Wasser-Emulsion in einen Überschuss des wässrigen Mediums eine partielle Verdampfung des die ölige Phase bildenden organischen Lösungsmittels durchgeführt wird.9. Verfahren nach Anspruch 7, dadurch gekennzeichnet, dass das wasserunlösliche Peptid- salz ein Pamoat, ein Tannat, ein Stearat oder ein Palmitat darstellt.10. Verfahren nach einem der Ansprüche 7 bis 9, dadurch gekennzeichnet, dass das biologisch abbaubare polymere Material ein Polylactid, ein Polyglycolid oder ein Copolymer aus Milchsäure und Glycolsäure darstellt.11. Verfahren nach Anspruch 10, dadurch gekennzeichnet, dass das Copolymer aus Milch- säure und Glycolsäure ein Copolymer aus L- oder D,L-Milchsäure darstellt, das 45 bis 90 Mol-% Milchsäureeinheiten und 55 bis 10 Mol-% Glycolsäureeinheiten enthält.KEINE ZEICHNUNG
