Description
Preparaat voor de lang aanhoudende en gecontroleerde afgifte van medicamenteuze stoffen en een werkwijze voor het bereiden daarvan
Gebied van de uitvinding
Een preparaat bedoeld voor de lang aanhoudende en gecontroleerde afgifte van medicamenteuze peptiden, verkregen in de vorm van microbolletjes van een biologisch afbreekbare polymere stof die die medicamenteuze stof insluit. Een werkwijze voor de bereiding daarvan.
Samenvatting van de uitvinding
Een preparaat bedoeld voor de lang aanhoudende en gecontroleerde afgifte van een medicamenteus peptide volgens de formule (I)
Ac-D-Nal-D-pClPhe-R3-Ser-Tyr-D-Cit-Leu-Arg-pro-D-Ala-NH2 (I) waarin R3 D-Pal of D-Trp is, in de vorm van microbolletjes van een biologisch afbreekbare polymere stof die een water onoplosbaar zout van het peptide volgens formule I insluit.
Een werkwijze voor de bereiding van het bovengenoemde preparaat bestaat om te beginnen uit het omzetten van een in water oplosbaar peptide-zout in een in water onoplosbaar peptide-zout, dan uit het suspenderen van dat peptide-zout in een oplossing van een biologisch afbreekbare polymere stof, het omzetten van die suspensie in een emulsie van het type olie-in-water, en tenslotte uit het isoleren van de microbolletjes van biologisch afbreekbaar polymeer nadat de olie-in-water-emulsie in een overmaat waterig medium gebracht is.
Stand der techniek
Tot op heden zijn er diverse oplossingen voorgesteld voor het bereiden van preparaten waaruit een lang aanhoudende en beheerste afgifte van medicamenteuze stoffen mogelijk is, waarbij men gebruik maakt van biologisch afbreekbare inplantaten of van microcapsules of van de bereiding van biologisch afbreekbare poreuze matrices in de vorm van bijvoorbeeld microbolletjes of microdeeltjes van diverse afmetingen. Wat dat betreft kan men EP-A-0052510 voor de bereiding van microcapsules door fasenscheiding van in water onoplosbare middelen noemen en EP-A-0058481 of us-A-3976071 voor de bereiding van inplantaten en van biologisch afbreekbare poreuze matrices, voornamelijk op basis van polylactiden of van copolymeren van melkzuur en glycol-zuur. Deze technieken maken gebruik van het voorafgaand oplossen van het als drager te gebruiken biologisch afbreekbare polymeer of copolymeer, en zonodig van het oplossen van de medicamenteuze stof zelf.
Andere technieken, die tot microbolletjes of microcapsules leiden, maken gebruik van emulgering, waarbij de belangrijkste stap van zulke werkwijzen het verkrijgen van een emulsie van het type olie-in-water uit een organische oplossing van de polymere stof en een waterige oplossing van het peptide is - zie wat dat betreft US-A-4384975, 3891570, 4389330, 3737337, en 4652441 en ook WO-90/13361. In ieder geval zijn de in dit vakbekwamen verplicht technieken te ontwikkelen die ingewikkeld en moeilijk in de hand te houden zijn om zoveel mogelijk de verliezen van de goed in water oplosbare, werkzame peptiden te beperken, bijvoorbeeld een dubbele emulgering.
De uitvinding
In een werkwijze waarbij men de vorming van een emulsie van het olie-in-water-type, gevolgd door het overbrengen daarvan naar een waterig medium toepast, maakt de uitvinding het, tegen alle verwachtingen in, mogelijk op voordelige wijze de thans bekende tekortkomingen van de techniek te overwinnen.
Door eerst een in water oplosbaar zout een peptide volgens formule I om te zetten in een in water onoplosbaar zout van dat peptide stelt de uitvinding in feite de deskundigen nieuwe middelen ter hand om voordelen te trekken van de relatieve oplosbaarheden van de gebruikte bestanddelen, in het bijzonder in de oplosmiddelen en niet-oplosmiddelen die daarbij betrokken zijn.
Bevoorkeurde uitvoeringsvormen van de uitvinding
Peptiden volgens formule I worden gedefinieerd als LHRH-analoga; ze kunnen met voordeel gebruikt worden bij de therapeutische behandeling van van hormonen afhankelijke storingen. Tenzij anders gespecificeerd hebben de aminozuren van formule I de L-configuratie. Daarbij staat "D-Nal" voor D-3-(/J-naftyl)alanine en "D-Pal" voor D-3-(/?-pyri-dyl)alanine.
Het preparaat volgens de uitvinding is in de vorm van microbolletjes van een biologisch afbreekbare polymere stof, die een in water onoplosbaar zout van het peptide volgens formule I insluit. Dit preparaat, met bijvoorbeeld 5 gew.% in water onoplosbaar zout, maakt na parenterale toediening aan mens of dier de meerdere dagen aanhoudende afgifte van dat peptide mogelijk.
De uitvinding betreft ook een werkwijze voor het bereiden van het hierboven gedefinieerde preparaat.
Meer in het bijzonder is één van de oogmerken van de uitvinding een werkwijze, die gekenmerkt wordt doordat a) een in water oplosbaar zout van een peptide volgens formule I omgezet wordt in een in water onoplosbaar peptide-zout, b) dat in water onoplosbare peptide-zout gesuspendeerd wordt in een organisch medium dat de biologisch afbreekbare polymere stof in opgeloste toestand bevat, c) die organische suspensie gedispergeerd wordt in een waterig medium dat dan de continue fase van de emulsie wordt, en d) die emulsie in een overmaat waterig medium gebracht wordt en tenslotte de aldus verkregen microbolletjes van de vloeistoffase gescheiden worden.
De eerste stap van de werkwijze bestaat uit het met gebruikelijke technieken omzetten van een in water oplosbaar zout van een peptide in een in water onoplosbaar zout van dat peptide. Met "in water oplosbaar" wordt bedoeld dat het zout in water van 25°C een oplosbaarheid boven 0,1 mg/ml heeft, bij voorkeur van wel 1,0 mg/ml. Met "in water onoplosbaar" wordt bedoeld dat het peptide-zout bij 25°C een oplosbaarheid in water minder dan 0,1 mg/ml heeft. Peptide-zouten zoals pamoaat, tannaat, stearaat en palmitaat voldoen aan deze definitie.
Wat het biologisch afbreekbare polymere materiaal betreft, daarvoor wordt het meest een polylactide, een polyglycolide of een copolymeer van melkzuur en glycolzuur gebruikt.
Onder de bevoorkeurde polymere stoffen moet men de copolymeren van melkzuur en glycolzuur (PLGA) noemen, en in het bijzonder de copolymeren die voor 45 tot 90 mol.% uit L- of D,L-melkzuur en voor 55 tot 10 mol.% uit glycolzuur bestaan.
Wat de oplosmiddelen voor het polymere materiaal betreft kan men een organisch oplosmiddel zoals bijvoorbeeld methyleenchloride gebruiken, maar in ieder geval moet het een niet-oplosmiddel voor het gekozen peptide of peptide-zout zijn.
Als het peptide of zout daarvan eenmaal in de organische oplossing van de polymere stof opgelost is wordt die oplossing in een bepaalde hoeveelheid waterig medium opgenomen, meestal water met daarin een passende hoeveelheid van de geëigende oppervlakte actieve stof. Het doel is om snel tot een homogene emulsie van het type olie-in-water te komen, waarin het waterige medium de continue fase vormt. Diverse factoren hoeft men bij het aanmaken van zo'n emulsie in acht nemen, welke op hun beurt ook de grootte of de structuur van de microbolletjes bepalen. Eén van de factoren waarop men moet letten is het tempo waarin men de organische oplossing in het waterige medium schenkt; een andere kan de temperatuur zijn en de roersnelheid of energie bij het dispergeren (behandeling met ultrasone trillingen) en vooral die laatste parameter beïnvloedt vooral de grootte van de uiteindelijke microbolletjes. Het ligt binnen de capaciteit van de in dit vak bekwamen om de methoden en de omstandigheden te kiezen waarmee men tot het gestelde doel komt.
Bij het aanmaken van die emulsie, is ook voordelige blijken de volumeverhouding van de met elkaar in contact komende fasen te regelen, in het bijzonder om het beginvo-lume van de organische fase ten opzichte van de hoeveelheid waterfase te verlagen. In sommige gevallen wordt dat, door de vluchtigheid van de gebruikte organische oplosmiddelen -bijvoorbeeld methyleenchloride - bereikt door spontane verdamping tijdens het roeren; in sommige gevallen wordt dit wenselijke verschijnsel versneld door onder verlaagde druk een gedeeltelijke verdamping te realiseren.
Als de organisch/waterige emulsie eenmaal gestabiliseerd is wordt die in een overmaat van het waterige medium gebracht, gewoonlijk in water. Het doel van die bewerking is het uitharden van de in de emulsie gevormde embryonische microbolletjes te intensiveren door er het organische oplosmiddel uit te extraheren dat nog in die microbolletjes zit. Met de bewerking verwijdert men ook nog de sporen oppervlakte-actieve stof die bij dat uiteindelijke uitharden nog in het polymeer kunnen zitten. Opgemerkt moet worden dat water een "niet oplosmiddel" is, zowel voor de biologisch afbreekbare polymere stof, zoals PLGA, als voor het peptide-zout dat in de microbolletjes ingesloten zit. Deze situatie is bijzonder gunstig voor de onvermijdelijke extractie van de rest aan polymeer-oplosmiddel CH2C12.
Nadat die emulsie in een overmaat waterig medium overgebracht is worden de uitgeharde microbolletjes met gebruikelijke technieken verzameld, bijvoorbeeld door centrifugeren, door filtreren of door bezinken onder invloed van de zwaartekracht. Hetzelfde geldt voor uitwassen, zuiveren en het drogen.
Eén van de voordelen van de werkwijzen van onze uitvinding is dat het daarmee mogelijk is microbolletjes van afmetingen te maken die men nauwkeurig onder controle kan houden, welke controle voornamelijk plaatsvindt bij het bereiden van de emulsie (bijvoorbeeld met de roersnelheid). Een ander voordeel is dat een bijzonder hoge belading met peptide bereikt kan worden, zo van 5, 10 en 20 gew.% of zelfs nog hoger, zulks afhankelijk van de omstandigheden. Verder is de mate van opname van peptide of peptide-zout bijzonder hoog, wat voornamelijk toe te schrijven is aan de voorafgaande omzetting van het gekozen peptide in een in water onoplosbaar zout.
De met de werkwijze volgens de uitvinding uit de bovengenoemde bestanddelen gemaakte microbolletjes worden dan gebruikt nadat ze op geëigende wijze gesteriliseerd zijn, bijvoorbeeld voor het aanmaken van bij parenterale toediening te gebruiken suspensies, bijvoorbeeld te gebruiken voor intramusculaire of subcutane injectie.
De uitvinding wordt door de volgende voorbeelden toegelicht. De in deze voorbeelden toegepaste omstandigheden beperken de uitvinding op geen enkele wijze.
Voorbeeld 1 3 g acetaat van het LHRH-analogen volgens de formule:
Ac-D-Nal-D-pCl-Phe-D-Pal-Ser-Tyr-D-Cit-Leu-Arg-Pro-D-AlaNH2 werd met gebruikelijke technieken in het overeenkomstige pamoaat omgezet en op een zodanige wijze behandeld dat men deeltjes met een gemiddelde grootte van 10 μια kreeg.
Van het pamoaat van werd 0,317 g gesuspendeerd in 20 ml CH2C12 en die suspensie werd toegevoegd aan 20 ml CH2C12 waarin al 1,683 g copolymeer van D,L-melkzuur en glycolzuur (mol.verhouding 75:25) met een inherente viscositeit van 0,82 in HFIP opgelost zat. Het mengsel werd bij kamertemperatuur onder roeren aangemaakt, wat een suspensie had die perfect homogeen was.
De ontstane suspensie werd toen ineens uitgeschonken in 500 ml water dat 0,075% methylcellulose bevatte, en het roeren werd ongeveer 90 minuten bij kamertemperatuur voortgezet (roersnelheid 900 rpm). Op geregelde tijden, gemiddeld iedere 30 minuten, werd de vorming van de emulsie gevolgd door er een monster van te nemen en de ontstane microbolletjes onder de microscoop te bekijken.
Toen het roeren eenmaal beëindigd was (de verkleining van die microdeeltjes was tot een stabiel einde gekomen) werd de emulsie ineens onder roeren overgebracht in twee liter water van ongeveer 10°C.
De PGLA-microbolletjes werden uit het reactie-mengsel geïsoleerd en gezuiverd door achtereenvolgens uitwassen met water, afgewisseld door centrifugeren, en tenslotte werden ze gefiltreerd en onder verlaagde druk gedroogd. Zo werd 1,61 g aan PLGA-microbolletjes verkregen (opbrengst 80%), en meer dan 94% der deeltjes had een doorsnede kleiner dan 100 /m (een top bij 55-85 μια).
Analyse (oplossen van vast PLGA, extractie en bepaling van het peptide met HPLC) liet zien dat de belading der microdeeltjes met pamoaat 9,05 gew.% was (theoretisch 10%).
De aldus verkregen microbolletjes ondergingen daarna steriliseren door gamma-straling terwijl ze in een geëigende steriele drager zaten. Bij proeven in vivo (bepaling van het bloedgehalte van mannelijke ratten en testosteron) bleek een regelmatige afgifte van de werkzame stof.
VOORBEELD 2
Precies dezelfde procedure werd gevolgd als in voorbeeld 1, nu met 0,634 g pamoaat van het LHRH-analogon op 1,366 g PLGA (75:25).
Opbrengst aan PLGA-microbolletjes : 1,70 g (85%).
Belading daarvan : 18,3% (in theorie 20%)
De aldus verkregen microbolletjes ondergingen toen steriliseren met gamma-straling terwijl ze in een geëigende steriele drager gesuspendeerd zaten. Bij proeven in vivo (bepaling van de bloodserumgehalten van mannelijke ratten aan analogon) bleek de regelmatige afgifte van een biologisch significante hoeveelheid van de werkzame stof over tenminste 24 dagen.
Deze uitkomsten werden verder bevestigd door analyse aan op D30 opgeofferde proefdieren: een gewichtsverlies van de testes van tenminste 80% en van de zaadleiders van tenminste 90%.
VOORBEELD 3
Precies dezelfde procedure als in voorbeeld 1 werd toegepast, maar nu onder toepassing van het peptide volgens de formule
Ac-D-Nal-D-pClPhe-D-Trp-Ser-Tyr-D-Cit-Leu-Arg-Pro-D-Ala-NH2 Door het acetaat daarvan om te zetten in het overeenkomstige pamoaat en dat laatste aan dezelfde behandeling te onderwerpen werden net zulke resultaten bereikt.
Claims (11)
1. Preparaat voor de lang aanhoudende en gecontroleerde afgifte van een medicamenteus peptide volgens formule I Ac-D-Nal-D-pClPhe-R3-Ser-Tyr-D-Cit-Leu-Arg-Pro-DAla-NH2/ (I) waaraan R3 D-Pal of D-Trp voorstelt, met het kenmerk dat het in de vorm van microbolletjes van biologisch afbreekbare polymere stof is die een in water onoplosbaar zout van een peptide volgens formule I insluit.
2. Preparaat volgens conclusie lf waarin het in water onoplosbare peptide een pamoaat, tannaat, stearaat of palmitaat is.
3. Preparaat volgens conclusie 1 of 2, waarvan de biologisch afbreekbare polymere stof een polylactide, een polyglycolide of een copolymeer van melkzuur en glycolzuur is.
4. Preparaat volgens conclusie 3 waarvan het copolymeer van melkzuur en glycolzuur een copolymeer is dat voor 45 tot 90 mol% L- of D, L-melkzuur en voor 55 tot 10 mol% glycolzuur is.
5. Preparaat volgens één der conclusies 1 tot en met 4 in de vorm van microbolletjes van een copolymeer dat voor 75% melkzuur en voor 25 mol% glycolzuur is en waarin in tenminste 56 gew.% pamoaat van een peptide volgens formule I ingesloten zit.
6. Gebruik van een preparaat volgens conclusie 1 in een farmaceutisch preparaat voor de parenterale toediening.
7. Werkwijze voor het bereiden van een preparaat volgens conclusie 1, met het kenmerk dat: a) een in water oplosbaar zout van een peptide volgens formule I omgezet wordt in een water onoplosbaar peptide-zout, b) dat in water onoplosbare peptide-zout gesus- pendeerd wordt in een organisch medium dat de biologisch afbreekbare polymere stof in opgeloste toestand bevat, c) die organische suspensies gedispergeerd wordt in een waterig medium dat dan de continue fase van de emulsie wordt, en d) de emulsie in een overmaat waterig medium gebracht wordt en tenslotte de aldus verkregen microbolle-tjes van de vloeistoffase gescheiden worden.
8. Werkwijze volgens conclusie 7, met het kenmerk dat, voorafgaande aan het overbrengen van de olie-in-water-emulsie naar een overmaat waterig medium, een gedeeltelijke verdamping van het organische oplosmiddel van de oliefase uitgevoerd wordt.
9. Werkwijze volgens conclusie 7, met het kenmerk dat het in water onoplosbare peptide-zout een pamoaat, een tannaat, een stearaat of een palmitaat is.
10. Werkwijze volgens één der conclusies 7 tot en met 9, met het kenmerk dat de biologisch afbreekbare polymere stof een polylactide, een polyglycolide of een copolymeer van melkzuur en glycolzuur is.
11. Werkwijze volgens conclusie 10, met het kenmerk dat het copolymeer van melkzuur en glycolzuur een copolymeer is dat voor 45 tot 90 mol% uit L- of D,L-melk-zuur en voor 55 tot 10 mol% uit glycolzuur bestaat.
