512 609 2 biologiskt nedbrytbara implantat, mikroinkapsling eller fram- ställning av' biologiskt nedbrytbara porösa matriser~ erhållna exempelvis såsom mikrosfärer eller mikropartiklar av olika dimen- sioner. I detta hänseende kan nämnas EP-A-0052510 för mikroin- kapsling genom fasseparation av vattenlösliga läkemedel och EP-A- 005848l eller US-A-3976071 för framställningen av implantat eller av biologiskt nedbrytbara porösa.matriser, baserade huvudsakligen på polylaktid eller på sampolylaktid-glykolid. Dessa tekniker utnyttjar en föregående upplösning i ett organiskt lösningsmedel av den biologiskt nedbrytbara polymeren eller sampolymeren som användes såsom bärare och, om så önskas, upplösningen av den medicinskt verksamma substansen själv.
Andra tekniker, som också är kapabla att ge mikrokapslar eller mikrosfärer, utnyttjar emulgeringsprocedurer, varvid det vikti- gaste steget av sådana procedurer är erhållandet av en emulsion av typen olja-i-vatten från en organisk lösning av polymert ma- terial och en vattenhaltig lösning av peptiden - se i detta hän- seende US-A-4384975, 3891570, 4389330, 3737337, 4652441 eller WO- 90/13361. I vilket fall som helst är emellertid fackmännen på området tvungna att utveckla tekniker som är komplicerade och svåra att kontrollera i syfte att så mycket som möjligt reducera förlusterna av'de ytterst vatten-lösliga.aktiva.peptidsubstanser- na, såsom exempelvis en dubbel emulgering.
UPPFINNINGEN I ett förfarande utnyttjande bildningen av en emulsion av olja-i- vatten-typ följt av dess överföring till ett vattenhaltigt medium möjliggör uppfinningen, mot alla förväntningar, att på fördelak- tigt sätt övervinna bristerna i tekniker kända till dato.
I verkligheten, genom att först gå tillväga så att en vatten-lös- lig peptid eller peptidderivat.omvandlasi.en vatten-olöslig pep- tid, resp. peptidsalt, tillhandahåller uppfinningen för fackmän- nen ett helt nytt medel dragande fördel av de relativa löslighe- terna hos ingredienserna som användes och i synnerhet av de in- volverade lösningsmedlen och "icke-lösningsmedlen". ,~ niiini 'Ä än »HÉÜiI 512 609 3 FÖREDRAGNA UTFÖRINGSFORMER AV UPPFINNINGEN I synnerhet är ändamålet med uppfinningen ett förfarande som kännetecknas av att: a. en vatten-löslig peptid eller peptidsalt omvandlas i. en vatten-olöslig peptid, resp. peptidsalt; b. nämnda vatten-olösliga peptid, resp. peptidsalt, suspenderas i ett organiskt medium innehållande det biologiskt ned- brytbara polymera materialet i upplöst tillstånd; c. nämnda organiska suspension dispergeras i ett vattenhaltigt medium bildande den kontinuerliga fasen av den resulterande emulsionen; d. nämnda emulsion föres över till ett överskott av ett vatten- haltigt medium, och slutligen separeras mikrosfärerna, som sålunda erhållits, från vätskefasen.
I uppfinningen användes "medicinskt aktiv peptidsubstans" för att beteckna primärt en naturlig eller en syntetisk polypeptid, som är fysiologiskt aktiv och som omfattar från 3 till 45 aminosyror.
Kretsen av polypeptider som kan behandlas i enligthet med förfarandet enligt uppfinningen är rätt omfattande och innefattar i synnerhet oxytocin, vasopressin, corticotrofin, calcitonin, epiderm tillväxtfaktor (EGF), prolaktin, inhibitin, interferon, somatostatin, insulin, glucagon, auriculär natriuretisk faktor (ANF), endorfin, en renin-inhibitor, luteiniseringshormon- frigörande hormon (LHRH), tillväxthormon-frigörande hormon (GHRH), peptid T eller en av deras syntetiska analoger eller homologer.

Den första fasen av processen består i omvandling medelst kon- ventionella tekniker av en vatten-löslig peptid eller peptidsalt i en vatten-olöslig peptid, resp. peptidsalt. Med "vatten-löslig" menas en peptid eller peptidderivat som har en vattenlöslighet över eller lika med 0,1 mg/ml vid 25°C, företrädesvis över eller lika med 1,0 mg/ml.
Med “vatten-olöslig" menas en peptid eller ett peptidderivat som har en vattenlöslighet mindre än eller lika med 0,1 mg/ml vid °C. Bland de föredragna polymerarmaterialen bör nämnas sampolymererna av mjölk- och glykol-syror (PLGA) och i synnerhet sampolymererna av L- eller D,L-mjölksyra innehållande från 45 till 90% (molar) av mjölksyraenheter och resp. 55 till 10% (molar) av glykolsyra- enheter.
Man kan i detta sammanhang också nämna olika polyalkylenfumarater eller -succinater, och i synnerhet poly-1,4-butylen-succinatet, poly-2,3-butylen-succinatet, poly-1,4-butylen-fumaratet eller poly-2,3-butylen-fumaratet. Dessa polymerer är lätt framställda såsom beskrives i litteraturen eller kan erhållas från speciali- serade firmor.
Vad beträffar lösningsmedlet valt för det polymera materialet kan man använda ett organiskt lösningsmedel såsom exempelvis metylenklorid, men i vilket fall som helst måste lösningsmedlet vara ett "icke-lösningsmedel" för den valda peptiden eller peptidsaltet.
Enligt uppfinningen, sedan väl nämnda peptid eller salt suspende- rats i den organiska lösningen av det polymera. materialet, inkorporeras denna lösning i en förutbestämd mängd av ett vatten- haltigt medium, vanligast vatten kompletterat med ett lämpligt ytaktivt medel. Målet är att snabbt bilda en homogen emulsion av olja-i-vatten-typen, varvid nämnda vattenhaltiga medium fungerar genom att ge den kontinuerliga fasen. Olika faktorer skall beak- tas vid framställning av en sådan emulsion som i sin tur influ- erar storleken eller strukturen av mikrosfärerna som resulterare från förfarandet. En av faktorerna som måste tagas i betraktande är tillsatshastigheten av den organiska lösningen till det vatte- nhaltiga mediet; en annan kan vara temperaturen eller vidare om- MJ ä 512 609 6 röringshastighetæuaeller dispergeringsenergin (ultraljudsbehand- ling), med den sist nämnda parametern i synnerhet påverkande storleken av de slutliga mikrosfärerna. Det ligger inom fack- mannens förmåga att utvälja nætoderna och betingelserna för emulgering lämpliga för att uppnå det avsedda syftet.
Vid framställningen av nämnda emulsion kan det också visa sig fördelaktigt att modifiera volymförhållandet av faserna i kon- takt, i synnerhet att minska initialvolymen av den organiska fasen med avseende på den av den vattenhaltiga fasen. I några fall, beroende på flyktigheten av de organiska lösningsmedlen som användes - exempelvis metylenklorid - kan förångningen som äger rum spontant under omröringen redan visa sig tillräcklig; i andra fall, kan detta önskvärda fenomen accelereras genom att företaga en partiell föràngning under reducerat tryck.
Sedan den organisk-vattenhaltiga emulsionen väl har stabilise- rats, så överföres den i en överskjutande mängd av ett vatten- haltigt medium, vanligast vatten. Syftet med denna operation är att intensifiera hårdnandet av de embryonala mikrosfärerna bildade i emulsionen genom extraktion av det organiska lösnings- medlet som fortfarande blivit kvar inuti nämnda mikrosfärer.
Denna operation syftar också till eliminering på samma gång av spårmängder av ytaktivt medel som kan ha blivit kvar i massan av polymeren under dess slutliga hårdningsfas. Det bör observeras, att vatten är ett “icke-lösningsmedel" för både det biologiskt nedbrytbara polymera materialet, såsom PLGA exempelvis, och för peptidsaltet innestängt inuti nämnda mikrosfärer. Denna situation är särskilt gynnsam för den ofrånkomliga extraktionen av det återstående polymerlösningsmedlet, såsom exempelvis CH2Cl2.
Sedan nämnda emulsion överförts i ett överskott av ett vatten- haltigt medium uppsamlas de hårdnade mikrosfärerna med användande av' konventionella tekniker, exempelvis genom centrifugering, filtrering eller genom tyngdkraftssedimentering. Detsamma gäller tvättnings-, renings- och torknings-operationerna. 512 609 7 En av fördelarna med förfarandet enligt uppfinningen är att det gör det möjligt att erhålla mikrosfärer, vilkas storlek kan nog- grant regleras, varvid denna reglering äger rum huvudsakligen under framställningen av emulsionen (exempelvis omröringshastig- heten). En annan fördel är att särskilt hög peptidfyllning kan uppnås, uppgående till 5, 10 eller 20_vikt% eller ännu högre, beroende på betingelserna. Vidare är utbytet av peptid- eller peptidsaltinkorporeringen särskilt högt; detta beror huvud- sakligen på den omvandlingen överföringen av den valda peptiden från ett vatten-lösligt derivat i ett vatten-olösligt derivat såsom ett salt t.ex.
Mikrosfärerna som erhållits enligt förfarandet enligt upp- finningen från de ovan nämnda ingredienserna användes sedan efter att ha vederbörligen steriliserats för framställningen av suspensioner för användning i parenteral administrering, exempelvis genom intramuskulär eller subkutan injektion.
Uppfinningen illustreras medelst följande exempel. Substanserna och arbetsbetingelserna som användes i dessa exempel begränsar inte uppfinningen på något sätt.
Exempel 1 3 g av acetatet av D-Trp-6-LHRH av formeln: (pyro)Glu-His-Trp-Ser-Tyr-D-Trp-Leu-Arg-Pro-GlY"NH2 omvandlades i det motsvarande pamoatet med användande av konven- tionella tekniker. Efter pulvrisering förefaller detta salt vara bildat av mikropartiklar av amorf struktur med en genomsnittlig partikelstorlek av approximativt 10 mikron. Löslighet: mindre än 0,025 mg/ml i H20 vid 40°C. 0,280 g av pamoatet av D-Trpö-LHRH suspenderades sedan i 20 ml CH2Cl2, och nämnda suspension sattes till 20 ml CH2Cl2, innehål- lande 1,720 g av upplöst sampolymer av D,L-mjölk- och glykolsyror (PLGA) 75:25 (mol%/logaritmiskt viskositetstal av 0,82 i HFIP).
,~ | HH H HH l H I 512 609 8 Blandningen framställdes vid rumstemperatur under omröring för att erhålla en suspension som var perfekt homogen.
Den resulterande suspensionen hälldes sedan, i en portion, i 500 ml vatten innehållande 0,075% upplöst metoxicellulosa, och omrö- ring av blandningen fortsattes i ungefär 90 min vid rumstempera- tur (omröringshastighet: 900 varv per minut). Utvecklingen av emulsionen övervakades med regelbundna tidsintervall, i genom- snitt varje 30 minut, genom tagande av ett prov och undersökning av de erhållna mikrosfärerna i mikroskop.
Sedan väl omröringen avslutats (stabilisering av storleksreduk- tionen hos mikrosfärerna), överfördes nämnda. emulsion, i en portion, i 2 liter vatten som hölls vid approximativt l0°C, under omröring av blandningen till homogenisering.
Mikrosfärerna av PLGA isolerades från reaktionsblandningen och renades genom successiva centrifugeringar omväxlande med tvätt- ningar med H20 och filtrerades och torkades slutligen under redu- cerat tryck. 1,25 g av PLGA-mikrosfärer uppsamlades sålunda (ut- byte: 63%), som innefattade mer än 96% partiklar med en diameter mindre än 100 mikron (maximum vid 55-85 mikron).
Analysen (upplösning av den fasta PLGA, extraktionen och bestäm- ningen av peptiden med HPLC) visar att innehållet av pamoatet av D-Trpó-LHRH av mikrosfärerna uppgår till 9,05 vikt% (teoretiskt: %).
Mikrosfärerna som sålunda.erhållits underkastades därefter steri- lisering med gamma-strålar och suspenderades i en lämplig steril vehikel. In-vivo-test (bestämning av blodtestosteron-nivån i han- ràttor) bekräftade den regelbundna frigöringen av den aktiva sub- stansen över minst 21 dagar, producerande från D4 (injektion vid DO) ett fall i testosteronnivån till värden typiska för kastrera- de djur. 512 609 TID (dagar) TESTOSTERON (ng/ml) 0 1 2 v 4 v 7 r 11 0,8 14 1,2 18 1,9 21 2,0 210 Exempel 2 Exakt samma procedur användes som i exempel 1, utom att 0,560 g av pamoatet av D-Trp6-LHRH användes för 1,440 g av PLGA 75:25 (mol%). 1,49 g av PLG-mikrosfärer uppsamlades (utbyte: 75%) och de omfat- tade mer än 90% partiklar med en diameter mindre än 100 mikron.
Fyllningsnivà: 16,3 vikt% (teoretiskt: 20%).
Exemgel 3 Samma procedur användes som i exempel 1, startande med 0,140 g av pamoatet av D-Trpö-LHRH och 0,860 g poly-l,4-butylensuccinat (logaritmiskt viskositetstal i HFIP approximativt 0,35).
Den resulterande organisk-vattenhaltiga emulsionen överfördes, i en portion, i 500 ml vatten och den resulterande blandningen ut- sattes för successiva centrifugerings- och H20-tvättningsbehand- lingar, filtrering och slutligen torkning under reducerat tryck till att ge 0,52 g (utbyte: 52%) av polysuccinat-mikrosfärer.
Fyllningsnivà: 2,87 vikt% (teoretiskt: 10%).
\Il|l|\ HIIIHVHïH H I \' 512 609 Exempel 4 Först omvandlades acetatet av en analog av somatostatin av formeln: D-Phe-Cvs-Tyr-D-Trp-Lys-Va1-Cvs-Trp-NH2 i det motsvarande pamoatet med användning av konventionella tek- niker för att erhålla partiklar med en genomsnittlig storlek av approximativt 10 mikron. 0,266 g av nämnda pamoat och 1,734 g av PLGA 75:25 (mol%) använ- des sedan i överensstämmelse med proceduren i exempel 1. Efter hällning av den organisk-vattenhaltiga emulsionen i 2 liter H20 vid l0°C, homogenisering, centrifugering och samma efterföljande behandlingar som beskrivits ovan, uppsamlades 1,23 g (utbyte: 62%) av PLGA-mikrosfärer, vilka omfattade mer än 98% partiklar med en diameter mindre än 100 mikron (maximum vid 40-65 mikron).
Fyllningsnivà: 8,7 vikt% (teoretiskt: 10%).
Exempel 5 Exakt samma procedur användes som i exempel 4, med 0,532 g av pamoatet av somatostatinanalogen för 1,468 g av PLGA 75:25.
Mikrosfärer av PLGA: 1,21 g (utbyte: 60%).
Fyllningsnivå: 17,5 vikt% (teoretiskt: 20%).
De sålunda erhållna mikrosfärerna steriliserades medelst gamma- strålar, suspenderades slutligen i en lämplig steril vehikel. In- vivo-test (bestämning av blodserumnivån av somatostatinanalogen i råttor som fått en injektion vid DO) bekräftar en reglerad fri- göring av en bestämbar mängd av aktiv substans över en period av dagar (injektion: IM). 512 609 ll TID (dagar) BESTÄMNING Av PEPTID (ng/ml) O + 3 timmar 64,0 1 15,0 2 10,0 3 5,0 6 310 e 2,0 210 14 1,5 16 1,5 1,5 Exempel 6 3 g av acetatet av en LHRH-analog av formeln: omvandlades motsvarande pamoatet genom konventionella tekniker och behandlades på sådant sätt att partiklar erhölls med en ge- nomsnittlig storlek av approximativt 10 mikron. 0,317 g av nämnda pamoat och 1,683 g av PLGA 75:25 (mol%) be- handlades därefter enligt proceduren i exempel 1 till att slut- ligen producera 1,61 g (ubyte: 80%) av PLGA-mikrosfärer inne- fattande mer än 94% partiklar med en diameter mindre än 100 mikron (maximum vid 55-85 mikron).
Fyllningsnivâ: 9,05 vikt% (teoretiskt: 10%).
Exempel 7 Exakt samma procedur användes som i exempel 6, med 0,634 g av pamoatet av LHRH-analogen för 1,366 g av PLGA 75:25.
PLGA mikrosfärer: 1,70 g (utbyte: 85%).
Fyllningsnivà: 18,3 vikt% (teoretiskt: 20%). | 1 512 609 12 Mikrosfärerna som sàlunda.erhållits underkastades därefter steri- lisering med gamma-strålar och suspenderades i en lämplig steril vehikel. In-vivo-test (bestämning av blodserumnivån av analogen i hanràttor) bekräftade den regelbundna frigöringen av en biolo- giskt signifikativ mängd av aktiv substans över åtminstone 24 dagar.

De konventionella in-vivo-testen bekräftar en reglerad frigöring av den aktiva substansen över en period av approximativt 8 dagar.

Claims (9)
1. Förfarande för framställning av en komposition utformad för fördröjd och reglerad frigöring av medicinskt aktiva substanser erhållen i form av mikrosfärer av ett biologiskt nedbrytbart polymert material inkorporerande nämnda medicinskt aktiva sub- stans, k ä n n e t e c k n a t av att a) en vatten-löslig peptid eller peptidsalt omvandlas i en vatten-olöslig peptid, resp. peptidsalt med en högsta löslighet av 0,1 mg/ml vid 25°C; b) nämnda'vatten-olösligapeptidfresp.peptidsalt,suspenderas i ett organiskt medium innehållande det biologiskt nedbryt- bara polymera materialet i upplöst tillstånd; c) nämnda organiska suspension dispergeras i ett vattenhaltigt medium bildande den kontinuerliga fasen av den resulterande emulsionen; d) nämnda emulsion föres över i ett överskott av ett vatten- haltigt medium, och slutligen separeras de sålunda erhållna mikrosfärerna från vätskefasen.
2. Förfarande enligt krav 1, k ä n n e t e c k n a t av att före överföringen av olja-i-vatten-emulsionen i ett överskott av vattenhaltigt medium utföres en partiell föràngning av det orga- niska lösningsmedlet bildande oljefasen.
3. Förfarande enligt krav l eller 2, k ä n n e t e c k n a t av att den medicinskt verksamma peptidsubstansen är en naturlig eller syntetisk polypeptid omfattande 3 till 45 aminosyror.
4. Förfarande enligt något av kraven 1 till 3, k ä n n e t e c k- n a t av att den medicinskt verksamma substansen är vald från oxytocin, vasopressin, corticotrofin, calcitoniñ, epiderm tiIl- växtfaktor (EGF), prolaktin, inhibitin, interferon, somatostatin, insulin, glukagon, aurikulär natriuretisk faktor (ANF), endorfin, enrenininhibitor,luteiniseringshormon-frigörandehormon(LHRH), tillväxthormon-reglerande hormon (GHRH), peptid T eller en av deras syntetiska analoger eller homologer. w * | ' W! ! 8. Förfarande enligt krav 7, k ä n n e t e c k n a t av att 512 609 15 sampolymeren av mjölk- och glykolsyror är en sampolymer av L- eller D,L-mjölksyra innehållande från 45 till 90% (molar) av mjölksyraenheter och resp. 55 till 10% (molar) av glykolsyraen~ heïer.
9. Förfarande enligt krav 8, k ä n n e t e c k n a t av att polyalkylenfumaratet eller -succinatet är ett poly-l,4-butylen- succinat, ett poly-2,3-butylen-succinat, ett poly-1,4-butylen- fumarat eller ett poly-2,3-butylen-fumarat.
