WERKWIJZE VOOR DE BEREIDING VAN MICROBOLLETJES VAN BIOLOGISCH AFBREEKBAAR POLYMERE STOF
GEBIED VAN DE UITVINDING
Een werkwijze voor de bereiding van een preparaat bedoeld voor de aanhoudende en gecontroleerde afgifte van medicamenteuze peptiden in de vorm van microbolletjes van een biologisch afbreekbare polymere stof die die medicamenteuze stof insluit.
SAMENVATTING VAN DE UITVINDING
De werkwijze is gericht op het verschaffen van een preparaat bedoeld voor de aanhoudende en gecontroleerde afgifte van medicamenteuze peptiden, in de vorm van de microbolletjes van een biologisch afbreekbare stof die die medicamenteuze stof insluit.
Die bestaat in de eerste plaats uit het omzetten van een in water oplosbaar peptide of zout daarvan in een in water onoplosbaar peptide of zout daarvan, waarna dat peptide of zout in een oplossing van een biologisch afbreekbare polymere stof gesuspendeerd wordt, het omzetten van die suspensie in een emulsie van het type olie-in-water, en tenslotte uit het isoleren van de microbolletjes van biologisch afbreekbaar polymeer nadat de olie-in-water-emulsie overgebracht is in een overmaat waterig medium.
STAND DER TECHNIEK
Tot op de dag van vandaag zijn diverse oplossingen voorgesteld voor het bereiden van preparaten die medicamenteuze stoffen over lange tijd gecontroleerd kunnen afgeven, waartoe men biologisch afbreekbare implantaten maakt of kleine deeltjes van diverse afmetingen van biologisch afbreekbare poreuze matrices, dus van microbolletjes of microcapsules. Wat dat betreft kan men EP-A-0052510 noemen voor de micro-inkapseling door fasenomkering van in water oplosbare middelen en EP-A-0058481 of US-A-3976071 voor het bereiden van implantaten van biologisch afbreekbare poreuze matrices, voornamelijk op basis van polylactiden of van copolymeren van melkzuur en glycolzuur. Deze technieken maken gebruik van een vooraf oplossen in een als drager te gebruiken biologisch afbreekbaar polymeer of copolymeer, en indien nodig van het oplossen van de medicamenteuze stof zelf.
Andere technieken, die ook microcapsules of microbolletjes kunnen geven, berusten op emulgering, waarbij de belangrijkste stap, uitgaande van een organische oplossing van het polymere materiaal en een waterige oplossing van het peptide het verkrijgen van een emulsie van het type olie-in-water is - zie wat dat betreft US-A-4384975, 3891570, 4389330, 3737337 en 4652441 en WO-90/13361. In al die gevallen zijn de in dit vak bekwamen echter verplicht technieken te ontwikkelen die ingewikkeld en moeilijk onder controle te houden zijn, om verliezen van de sterk in water oplosbare werkzame peptiden zo goed mogelijk te voorkomen, bijvoorbeeld door een dubbele emulgering.
DE UITVINDING
In een werkwijze waarbij men een emulsie van het type olie-in-water, gevolgd door het overbrengen daarvan naar een waterig medium toepast, maakt de uitvinding het, tegen alle verwachten in, mogelijk om op voordelige wijze de thans bekende tekortkomingen van de techniek te overwinnen.
Door een in water oplosbaar peptide of peptide-derivaat in een in water onoplosbaar peptide, respectievelijk peptidezout om te zetten stelt de uitvinding in feite de deskundigen nieuwe middelen ter hand om voordeel te trekken van de relatieve oplosbaarheden der te gebruiken uitgangsstoffen, en in het bijzonder van het gebruik van oplosmiddelen en niet-oplosmiddelen.
BEVOORKEÜRDE UITVOERINGSVORMEN VAN DE UITVINDING
Meer in het bijzonder beoogt de uitvinding een werkwijze die gekenmerkt wordt doordat a) een in water oplosbaar peptide of zout daarvan in een in water onoplosbare peptide of zout daarvan omgezet wordt, b) dat in water onoplosbare peptide of zout gesuspendeerd wordt in een organisch medium dat de biologisch afbreekbare polymere stof in opgeloste toestand bevat, c) die organische suspensie gesuspendeerd wordt in een waterig medium dat dan de continue fase van een emulsie wordt, en d) die emulsie in een overmaat waterig medium gebracht wordt en tenslotte de aldus verkregen microbolle-tjes van de vloeistoffase gescheiden worden.
In de beschrijving van deze uitvinding wordt "medicamenteus peptide" gebruikt om ieder natuurlijk of synthetisch peptide aan te duiden dat fysiologisch werkzaam is en dat 3 tot 45 aminozuren omvat. De verscheidenheid van polypeptiden die met de werkwijze volgens de uitvinding behandeld kan worden is behoorlijk uitgebreid en daartoe behoren in het bijzonder oxytocine, vasopressine, cortico-tropine, calcitonine, epidermale groeifactor (EGF), prolac-tine, inhibitine, interferon, somatostatine, insuline, glucagon, de auriculaire natriuretische factor (ANF), endorphine, een renine-remmer, het hormoon dat het luteïni-zerende hormoon of het groeihormoon vrijmaakt (LHRH en GHRH), peptide T en ieder synthetisch analogen of homoloog daarvan.
Met voorkeur kan men polypeptiden zoals LHRH en somatostatine noemen, en al hun synthetische homologen en analoga zoals 1---, D-Phe-Cys-Phe-D-Trp-Lys-Thr-Cys-Thr-OH, D-Phe-Cys-Phe-D-Trp-Lys-TKr-Cys-Thr-NH2, ι--1 D-Trp-Cys-Phe-D-Trp-Lys-Thr-Cys-Thr-NH2, 1----1 D-Phe-Cys-Tyr-D-Trp-Lys-Val-Cys-Thr-NH2 D-Phe-Cys-Tyr-D-Trp-Lys-Val-Cys-Trp-NH2

Deze opsomming is echter niet uitputtend.
De eerste fase van de werkwijze bestaat uit het omzetten, met gebruikelijke technieken, van een in water oplosbaar peptide of zout daarvan in een in water onoplosbaar peptide, respectievelijk zout daarvan. Met "in water oplosbaar" wordt een peptide of peptide-derivaat bedoeld met een oplosbaarheid in water boven 0,1 mg/ml bij 25°C, en bij voorkeur boven 1,0 mg/ml.
Met "in water onoplosbaar" wordt een peptide of peptide-derivaat bedoeld met een oplosbaarheid in water van 0,1 mg/ml of minder bij 25°C. Peptide-zouten zoals het pamoaat, tannaat, stearaat en palmitaat voldoen aan die definitie.
Wat de biologisch afbreekbare polymere stof betreft, de meest gebruikelijke daarvan zijn polymeren zoals polylactiden en polyglycoliden en de copolymeren van melkzuur en glycolzuur, polyesters zoals polyalkyleenfuma-raat en -succinaat, en verder polyesters, polyacetalen en polyanhydriden.
Onder de bevoorkeurde polymere stoffen moet men de copolymeren van melkzuur en glycolzuur (PLGA) noemen, en in het bijzonder de copolymeren die voor 45 tot 90 mol% uit L-of D,L-melkzuur en voor 55 tot 10 mol% uit glycolzuur bestaan.
Wat dat betreft kan men ook diverse polyalkyleen-fumaraten en -succinaten noemen, en in het bijzonder het polybutyleen-1,4-succinaat, het polybutyleen-2,3-succinaat, het polybutyleen-1,4-fumaraat en het polybutyleen-2,3-fumaraat. Deze polymeren zijn gemakkelijk te bereiden zoals in de literatuur beschreven is en zijn ook bij gespecialiseerde firma's verkrijgbaar.
Wat het voor de polymere stof te gebruiken oplosmiddel betreft kan men een organisch oplosmiddel gebruiken, zoals methyleenchloride, maar in ieder geval moet het oplosmiddel een niet-oplosmiddel voor het gekozen peptide of peptidezout zijn.
Als dat peptide of zout daarvan eenmaal in de organische oplossing van de polymere stof gesuspendeerd is, moet die suspensie/oplossing in een bepaalde hoeveelheid in een waterig medium opgenomen worden, in het algemeen in water waaraan de geëigende oppervlak-actieve stof toegevoegd is. Het doel daarvan is snel tot een homogene emulsie van het type olie-in-water te komen, waarvan dat waterige medium de continue fase is. Diverse factoren moeten bij het bereiden van zo'n emulsie in acht genomen worden, en die emulsie heeft op zijn beurt invloed op de structuur van de bij deze werkwijze ontstane microbolletjes. Eén van de factoren die men in acht moet nemen is het tempo waarin de organische oplossing aan het waterige medium toegevoegd wordt; een andere kan de temperatuur zijn op de roersnel-heid of energie bij het dispergeren (ultrasone tellingen) en vooral die laatste parameter beïnvloedt de grootte van de uiteindelijke microbolletjes. Het ligt binnen de capaciteit van de in dit vak bekwamen de werkwijzen voor en omstandigheden bij het emulgeren uit te zoeken om tot het beoogde doel te komen.
Bij het bereiden van die emulsie kan het ook voordelig blijken de volumeverhouding van de met elkaar in contact te brengen fasen bij te stellen, en in het bijzonder om het beginvolume van de organische fase ten opzichte van die van de waterfase te verlagen. In sommige gevallen kan daarvoor, dankzij de vluchtigheid van het gebruikte oplosmiddel - bijvoorbeeld methyleenchloride - de spontaan bij het roeren optredende verdamping daarvoor voldoende blijken; in sommige gevallen kan dit wenselijke verschijnsel nog versterkt worden door een gedeeltelijke verdamping onder verlaagde druk.
Als de organische/waterige emulsie eenmaal gestabiliseerd is, wordt die in een overmaat waterig medium gebracht, gewoon in water. Het doel van die bewerking is het verharden van de in de emulsie embryonisch gevormde microbolletjes, door het organische oplosmiddel dat nog in die microbolletjes zit er uit te extraheren. Deze bewerking heeft ook de bedoeling tegelijkertijd sporen oppervlak-actieve stof die bij het uiteindelijke uitharden in het polymeer zijn blijven zitten te verwijderen. Opgemerkt moet worden dat water een "niet-oplosmiddel" is, zowel voor de biologisch afbreekbare polymere stof zoals PLGA, als voor het peptide-zout dat in die microbolletjes ingesloten zit. Deze situatie is vooral gunstig voor de onontbeerlijke extractie van de rest aan polymeer-oplosmiddel, zoals bijvoorbeeld het CH2Cl2.
Als die emulsie in overmaat waterig medium overgebracht is, worden de uitgeharde microbolletjes met gebruikelijke technieken verzameld, bijvoorbeeld door centrifugeren, filtreren of door bezinken onder invloed van de zwaartekracht. Hetzelfde geldt voor het uitwassen, het zuiveren en het drogen.
Eén van de voordelen van de werkwijze volgens de uitvinding is dat die het mogelijk maakt microbolletjes te krijgen waarvan de grootte nauwkeurig geregeld kan worden, welke regeling voornamelijk plaatsvindt bij het bereiden van de emulsie (bijvoorbeeld door de roersnelheid). Een ander voordeel is dat een bijzonder hoge belading met peptide bereikt kan worden, zo tot 5, 10 of 20 gew.%, of zelfs nog meer, afhankelijk van de omstandigheden. Verder is de mate van opname van het peptide of het peptide-zout bijzonder hoog; dit is voornamelijk toe te schrijven aan de voorafgaande omzetting van het gekozen peptide vanuit een in water oplosbaar derivaat in een in water onoplosbaar derivaat, zoals bijvoorbeeld een zout.
De met de werkwijze volgens de uitvinding uit de bovengenoemde uitgangsstoffen bereide microbolletjes worden dan, nadat ze op geëigende wijze gesteriliseerd zijn, gebruikt voor de bereiding van suspensies die men voor parenterale toediening wil gebruiken, bijvoorbeeld bij intramusculaire of subcutane injectie.
De uitvinding wordt nu door de volgende voorbeelden toegelicht. De in deze voorbeelden optredende stoffen en werkomstandigheden beperken de uitvinding op geen enkele wijze.
VOORBEELD 1 3 g acetaat van D-Trp6-LHRH volgens de formule: (pyro)Glu-His-Trp-Ser-Tyr-D-Trp-Leu-Arg-Pro-Gly-NH2 werd met gebruikelijke technieken in het overeenkomstige pamoaat omgezet. Nadat het vermalen is, verschijnt dit zout als microdeeltjes met amorfe structuur, met een gemiddelde deeltjesgrootte van ongeveer 10 μιη. Oplosbaarheid in water bij 40°C minder dan 0,025 mg/ml.
Van het pamoaat van D-Trp6-LHRH werd 0,280 g toen in 20 ml CH2C12 gesuspendeerd en die suspensie werd toegevoegd aan 20 ml CH2C12 waarin 1,720 g copolymeer van D,L-melkzuur en glycolzuur opgelast zat (melkzuur/glycolzuur 75:25, inherente viscositeit in HFIP 0,82). Het mengsel werd onder roeren bij kamertemperatuur aangemaakt, zodat men een suspensie kreeg die perfect homogeen was.
De ontstane suspensie werd toen ineens uitgeschonken in 500 ml water dat 0,075% methylcellulose bevatte, en het roeren bij kamertemperatuur werd ongeveer 90 minuten voortgezet (roersnelheid 900 rpm). Met regelmatige tussen- pozen, gemiddeld om de 30 minuten, werd de ontwikkeling van de emulsie gevolgd door een monster te nemen en dat onder de microscoop op de ontstane microbolletjes te bekijken.
Toen het roeren eenmaal beëindigd was (stabilisering van de grootte der microbolletjes) werd die emulsie ineens overgebracht naar twee liter water dat op ongeveer 10°C gehouden werd; dit gebeurde onder roeren totdat het mengsel homogeen was.
De microbolletjes van PGLA werden uit het reactie-mengsel geïsoleerd en door achtereenvolgens uitwassen met water en centrifugeren gezuiverd, en tenslotte werden ze afgefiltreerd en onder verlaagde druk gedroogd. Zo werd 1,25 g aan PLGA-microbolletjes opgevangen (opbrengst 63%), waarvan meer dan 96% der deeltjes een doorsnede beneden 100 jLim had (de top zat bij ^5-85 /m) .
Analyse (oplossen van het vaste PLGA, extractie en bepaling van het peptide met HPLG) liet zien dat de belading met pamoaat van D-Trp6-LHRH op 9,05 gew.% kwam (berekend 10%).
De aldus verkregen microbolletjes ondergingen vervolgens steriliseren met gamma-straling terwijl ze in een geëigende steriele drager zaten. Proeven in vivo (bepaling van het bloedgehalte en testosteron van mannelijk ke ratten) bevestigden een regelmatige afgifte van de werkzame stof over tenminste 21 dagen, wat het testosterongehalte vanaf dag nr. 4 (injectie op dag 0) deed dalen tot waarden die typerend voor gecastreerde dieren zijn.
VOORBEELD 2
Precies dezelfde procedure als in voorbeeld 1 werd gevolgd, behalve dat op 1,440 g PLGA (75:25) nu 0,560 g pamoaat van D-Trp6-LHRH gebruikt werd.
Er werd 1,49 g aan PLGA-xnicrobolletj es opgevangen (opbrengst 75%) en meer dan 90% der deeltjes daarvan had een doorsnede kleiner dan 100 /zm. De belading was 16,3 gew.% (berekend 20%).
VOORBEELD 3
Precies dezelfde procedure werd gevolgd als in voorbeeld 1, beginnende met 0,140 g pamoaat van D-Trp6-LHRH en 0,860 g polybutyleen-l,4-succinaat (inherente viscositeit in HFIP ongeveer 0,35).
De ontstane organisch/waterige emulsie werd ineens overgebracht in 500 ml water en het ontstane mengsel onderging achtereenvolgens centrifugeren en uitwassen met water, affiltreren en tenslotte dragen onder verlaagde druk; dit gaf 0,52 g (52% opbrengst) aan polysuccinaat-microbolletjes. Belading: 2,87 gew.% (berekend 10%).
VOORBEELD 4
Om te beginnen werd het acetaat van een analoga van somatostatine volgens de formule i- D-Phe-Cys-Tyr-D-Trp-Lys-Val-Cys-Trp-NH2 met gebruikelijke technieken in het overeenkomstige pamoaat omgezet, wat deeltjes met een gemiddelde grootte van ongeveer 10 μια gaf.
Van dit pamoaat werd 0,266 g in de procedure van voorbeeld 1 gebruikt met 1,734 g PLGA (75:25). Na het uitschenken van de organisch/waterige emulsie in twee liter water van 10°c, homogeniseren, centrifugeren en dezelfde behandelingen als hierboven beschreven werd 1,23 g (62% opbrengst) aan PLGA-microbolletjes opgevangen, met meer dan 98% deeltjes met een doorsnede kleiner dan 100 μπι (een top bij 40-65 μιη) . De lading 8,7 gew.% (in theorie 10%).
VOORBEELD 5
Precies dezelfde procedure werd toegepast als in voorbeeld 4, met 0,532 g pamoaat van het somatostatine-analoga op 1,468 g PLGA (75:25). Men kreeg 1,21 g PLGA-microbolletjes (60% opbrengst) met een belading van 17,5 gew.% (in theorie 20%).
De aldus verkregen microbolletjes, met gamma-straling gesteriliseerd, werden tenslotte in een geëigende steriele drager gesuspendeerd. Bij proeven in vivo (bepaling van het bloedgehalte aan het somatostatine-analoga in ratten die 1 injectie op dag 0 gekregen hadden) bevestigde een beheerste afgifte van een aantoonbare hoeveelheid werkzame stof over 20 dagen (injectie op de eerste dag).
VOORBEELD 6
Van het acetaat van een LHRH-analoga volgens de formule
Ac-D-Nal-D-pCl-Phe-D-Pal-Ser-Tyr-D-Cit-Leu-Arg-Pro-D-AlaNH2 met de gebruikelijke technieken omgezet in het overeenkomstige pamoaat, en op een zodanige wijze behandeld dat men er deeltjes uitkreeg met een gemiddelde grootte van ongeveer 10 μιη.
Van dit pamoaat werd 0,317 g met de procedure van voorbeeld 1 gecombineerd met 1,683 g PLGA (75:25), waardoor men 1,61 g (80% opbrengst) aan PLGA-microbolletjes kreeg, waarvan meer dan 94% der deeltjes een doorsnede beneden 100 jtim had (top bij 55-85 μια). Belading: 9,05 gew.% (berekend 10%).
VOORBEELD 7
Precies dezelfde procedure als die van voorbeeld 6 werd toegepast, nu met 0,634 g pamoaat van het LHRH-analoga op 1,366 g PLGA (75:25). Men verkreeg 1,70 g PLGA-microbol-letjes (opbrengst 85%) met een belading van 18,3% (berekend 20%).
De aldus verkregen microbolletjes ondergingen vervolgens steriliseren door gamma-straling terwijl ze in een geëigend steriele drager gesuspendeerd waren. Proeven in vivo (bepaling van het bloedserumgehalte aan het analoga in mannelijke ratten) bevestigden de regelmatige afgifte van een biologisch significante hoeveelheid werkzame stof over tenminste 24 dagen.
Deze uitkomsten werden verder bevestigd met analyses die op dag nr. 30 aan opgeofferde proefdieren uitgevoerd werden: het gewichtsverlies van de testes was tenminste 80% en van de zaadleiders tenminste 90%.
VOORBEELD 8
Precies dezelfde procedure als in voorbeeld 1 werd gevolgd, uitgaande van 0,05 g pamoaat van zalmcalcitonine en 1,0 g van het copolymeer uit D,L-melkzuur en glycolzuur (50:50).
De gebruikelijke proeven in vivo bevestigden een beheerste afgifte van de werkzame stof over ongeveer 8 dagen.
Claims (9)
1. Werkwijze voor de bereiding van een preparaat bedoeld voor de aanhoudende en gecontroleerde afgifte van een medicamenteus peptide in de vorm van microbolletjes van een biologisch afbreekbare polymere stof die die medicamenteuze stof insluit, met het kenmerk, dat a) een in waterige oplossing peptide of zout daarvan in een in water oplosbaar peptide of zout daarvan omgezet wordt, b) dat in water onoplosbare peptide of zout gesuspendeerd wordt in een organisch medium dat de biologisch afbreekbare polymere stof in opgeloste stof bevat, c) die organische dispersie gedispergeerd wordt in een waterig medium dat dan de continue fase van een emulsie wordt, en d) die emulsie in een overmaat waterig medium gebracht wordt en tenslotte de aldus verkregen microbolletjes van de vloeistoffase gescheiden worden.
2. Werkwijze volgens conclusie 1, met het kenmerk dat er voor het overbrengen van de olie-in-water-emulsie in een overmaat waterig medium een gedeeltelijke verdamping van het organische oplosmiddel dat de oliefase vormt uitgevoerd wordt.
3. Werkwijze volgens conclusie 1 of , met het kenmerk dat het medicamenteuze peptide een natuurlijk of synthetisch polypeptide van 3 tot 45 aminozuren is.
4. Werkwijze volgens één der conclusies 1 tot 3, met het kenmerk dat de medicamenteuze stof gekozen is onder oxytocine, vasopressine, corticotropine, calcitonine, epidermale groeifactor (EGF), prolactine, inhibitine, interferon, somatostatine, insuline, glucagon, auriculair natriurethische factor (ANF), endorfine, een renine-remmer, het hormoon dat het luteiniserende hormoon of het groeihormoon vrijmaakt, peptide T en één van hun synthetische analoga of homologen.
5. Werkwijze volgens één der conclusies 1 tot en met 6, met het kenmerk dat de biologisch afbreekbare polymere stof een polylactide, een polyglycolide, een copolymeer van melkzuur en glycolzuur, een polyester zoals polyalkyleen fumaraat of -succinaat of een polyorthoester, een polyacetal of een polyanhydride is.
8. Werkwijze volgens conclusie 7, met het kenmerk dat het copolymeer van melkzuur en glycolzuur een copolymeer van 45 tot 90% L- of D,L-melkzuur met 55 tot 10 mol.% glycolzuur is.
9. Werkwijze volgens conclusie 8, met het kenmerk dat het polyalkyleenfumaraat of -succinaat een polybuty-leen-1,4-succinaat, een polybutyleen-2,3-succinaat, een polybutyleen-1,4-fumaraat of een polybutyleen-2,3-fumaraat is.
