1 194576 . Werkwijze voor de bereiding van microbolletjes van een biologisch afbreekbare polymere stof die een medicamenteus peptide bevatten
De onderhavige uitvinding heeft betrekking op een werkwijze voor de bereiding van een preparaat bedoeld 5 voor de aanhoudende en gecontroleerde afgifte van een medicamenteus peptide in de vorm van microbolletjes van een biologisch afbreekbare polymere stof die de medicamenteuze stof insluit, waarbij men een suspensie van het peptide in een oplossing van de biologisch afbreekbare polymere stof in een organisch medium dispergeert in een waterig medium ter vorming van een olie-in-wateremulsie, het oplosmiddel uit de oliefase verwijdert, zodat microbolletjes worden gevormd en de microbolletjes isoleert.
10 Een dergelijke werkwijze is bekend uit het Britse octrooischrift GB-A-2.211.091. Dit octrooigeschrift beschrijft een preparaat op basis van polyesters voor de gereguleerde afgifte van geneesmiddelen. Het beschreven preparaat bevat als drager voor het geneesmiddel een biologisch afbreekbaar polymeer, copolymeer of mengsel van polymeren en/of copolymeren van een dicarbonzuur uit Krebs-cyclus en een alifatisch diol met 4 koolstofatomen of 1,4-cyclohexaandimethanol. GB-A-2.211.091 beschrijft de bereiding 4 15 van een farmaceutisch preparaat op polyesterbasis door micro-inkapsulering door het suspenderen van het decapeptide LHR-D-Trp6 in een oplossing van poly-1,4-butyleensuccinaat in methyleenchloride, en de verkregen suspensie te emulgeren in een oplossing van 0,27% methylcellulose in gedestilleerd water. Na verwijdering van het organische oplosmiddel door middel van roterende verdamping gedurende 2 uur bij 40°C onder verlaagde druk, worden microcapsules verkregen die werden gefiltreerd, met koud water 20 gewassen en onder vacuüm gedroogd.
Gevonden is nu dat door de vorming van microbolletjes in twee stappen uit te voeren een betere regeling mogelijk wordt van de factoren die de kenmerken en eigenschappen van de in de eerste stap bereide emulsie bepalen die op hun beurt de grootte en de structuur van de met de werkwijze bereide microbolletjes beïnvloeden. Aldus voorziet de uitvinding een werkwijze als in de aanhef omschreven met het kenmerk, dat 25 men a) een in water oplosbaar natuurlijk of synthetisch peptide met 3 tot 45 aminozuren met uitzondering van Ac-D-Nal- D-pCIPhe-D-Pal-Set-Tyr-D-Cit-Leu-Arg-D-Ala-NH2en Ac-D-Nal-D-pCIPhe-D-Tryp-Ser-Tyr-D-Cit-Leu-Arg-Pro-D-Ala-NH2 of een zout van dat peptide omzet in een in water onoplosbaar pamoaat-, tannaat-, stearaat- of palmitaatzout van dat peptide, 30 b) dat in water onoplosbare zout suspendeert in een organisch medium dat de biologisch afbreekbare polymere stof in opgeloste vorm bevat, c) die organische suspensie suspendeert in water dat dan de continue fase van een stabiele olie-in-wateremulsie wordt, en d) die emulsie in een overmaat water, waaraan een geschikte oppervlakteactieve stof is toegevoegd, brengt 35 en men de aldus verkregen microbolletjes van de vloeistoffase scheidt.
FR-A-2.649.319 beschrijft een werkwijze voor het bereiden van microdeeltjes die een peptide bevatten, waarbij eerst een primaire water-in-olie-emulsie wordt gevormd door een waterige suspensie van een peptide of peptidezout in een oplossing van een biologisch afbreekbaar polymeer, zoals een copolymeer van melkzuur en glycolzuur (PLGA), in bijvoorbeeld methyleenchloride te dispergeren en vervolgens een 40 secundaire (water-in-olie)-in-wateremulsie, dat wil zeggen een drietasenemulsie, te vormen door de primaire emulsie te dispergeren in bijvoorbeeld een groot volume gelatine bevattende waterige oplossing, waarna men de oliedruppeltjes die peptide bevatten, laat harden tot microbolletjes door geleidelijke verdamping van het methyleenchloride door het langzaam roeren van de drietasenemulsie. Na filtratie worden de gewonnen deeltjes gewassen met water om gelatine te verwijderen. FR-A-2.649.319 beschrijft in voorbeeld 10 de 45 bereiding van octreotidepamoaat door octreotide in vrije basevorm en 1,1'-methyleenbis(2-hydroxy-3- naftoëzuur) op te lossen in een waterig oplosmiddel, deze oplossing te filtreren en te droogvriezen. Dit zout wordt gedispergeerd in de oplossing van een biologisch afbreekbaar polymeer zoals PLGA, waarna de resulterende suspensie met een overmaat water met gelatine wordt gemengd. Vermeld is dat de therapeutische duur van de vrijmaking van het peptide kan worden verlengd door het harden-wassen uit te voeren 50 met een olie-in-wateremulsie van een buffer-heptaan of door een oplosmiddel met emulgator gevolgd door een afzonderlijke wastrap met water.
FR-A-2.620.621 heeft betrekking op de bereiding van microbolletjes door het dispergeren van bijvoorbeeld een peptidepamoaat in een oplossing van een polymeer in methyleenchloride, het grotendeels verdampen van het methyleenchloride, het uitgieten van de overblijvende geconcentreerde suspensie om 55 een film van het polymeer met het peptidepamoaat te vormen en vervolgens het malen van de verkregen film. De bereiding van een olie-in-wateremulsie wordt in FR-A-2.620.621 niet beschreven.
In de beschrijving van deze uitvinding wordt ’’medicamenteus peptide” gebruikt om ieder natuurlijk of 194576 2 synthetisch peptide aan te duiden dat fysiologisch werkzaam is en dat 3 tot 45 aminozuren (behoudens de twee peptiden die onderwerp van de NL-A 9201310 zijn) omvat.

De eerste fase van de werkwijze bestaat uit het omzetten, met gebruikelijke technieken, van een in water oplosbaar peptide of zout daarvan in een in water onoplosbaar peptidepamoaat, tannaat, stearaat en palmitaat, respectievelijk zout daarvan. Met ”in water oplosbaar” wordt een peptide of peptide-derivaat bedoeld met een oplosbaarheid in water boven 0,1 mg/ml bij 25°C, en bij voorkeur boven 1,0 mg/ml.· 30 Met ”in water onoplosbaar” wordt bedoeld een oplosbaarheid in water van 0,1 mg/ml of minder bij 25°C.
Wat de biologisch afbreekbare polymeren betreft, de meest gebruikelijke daarvan zijn polymeren zoals polylactiden en polyglycoliden en de copolymeren van melkzuur en glycolzuur, polyesters zoals polyalky-leenfumaraat en -succinaat, en verder polyesters, polyacetalen en polyanhydriden.
Onder de bij voorkeur toegepaste polymeren moet men overeenkomstig FR-A-2.649.319, NL-A 9001646 35 en FR-A-2.620.621, de copolymeren van melkzuur en glycolzuur (PLGA) noemen, en in het bijzonder de copolymeren die voor 45 tot 90 mol% uit L- of D,L-melkzuur en voor 55 tot 10 mol% uit glycolzuur bestaan.
Wat dat betreft kan men ook diverse polyalkyleenfumaraten en -succinaten noemen, en in het bijzonder het polybutyleen-1,4-fumaraat en het polybutyleen-2,3-fumaraat. Deze polymeren zijn gemakkelijk te bereiden zoals in de literatuur (vergelijk NL-A 8802490) beschreven is en zijn ook bij gespecialiseerde 40 firma’s verkrijgbaar.
Wat het voor de polymere stof te gebruiken oplosmiddel betreft kan men een organisch oplosmiddel gebruiken, zoals methyleenchloride, maar in ieder geval moet het oplosmiddel een niet-oplosmiddel voor het peptide pamoaat, tannaat, stearaat of palmitaat.
Als dat peptidezout eenmaal in de organische oplossing van de polymere stof gesuspendeerd is, moet 45 die suspensie in een bepaalde hoeveelheid in een waterig medium opgenomen worden, in hei algemeen in water waaraan de geëigende oppervlak-actieve stof toegevoegd is. Het doel daarvan is snel tot een homogene emulsie van het type olie-in-water te komen, waarvan dat waterige medium de continue fase is. Diverse factoren moeten bij het bereiden van zo’n emulsie in acht genomen worden, en die emulsie heeft op zijn beurt invloed op de structuur van de bij deze werkwijze ontstane microbolletjes. Eén van de factoren 50 die men in acht moet nemen is het tempo waarin de organische oplossing aan het waterige medium toegevoegd wordt; een andere kan de temperatuur zijn of de roersnelheid of energie bij het dispergeren (ultrasone trillingen) en vooral die laatste parameter beïnvloedt de grootte van de uiteindelijke microbolletjes. Het ligt binnen de capaciteit van de in dit vak bekwamen de werkwijzen voor en omstandigheden bij het emulgeren uit te zoeken om tot het beoogde doel te komen.
55 Bij het bereiden van die emulsie kan het ook voordelig blijken de volumeverhouding van de met elkaar in contact te brengen fasen bij te stellen, en in het bijzonder om het beginvolume van de organische fase ten opzichte van dat van de waterfase te verlagen. In sommige gevallen kan daarvoor, dankzij de vluchtigheid 3 194576 ' van het gebruikte oplosmiddel - bijvoorbeeld methyleenchloride - de spontaan bij het roeren optredende verdamping voldoende blijken; in sommige gevallen kan dit wenselijk verschijnsel nog versterkt worden door een gedeeltelijke verdamping onder verlaagde druk.
Als de organische/waterige emulsie eenmaal gestabiliseerd is, wordt dit in een overmaat water gebracht.
5 Het doel van die bewerking is het verharden van de in de emulsie embryonisch gevormde microbolletjes, door het organische oplosmiddel dat nog in die microbolletjes zit er uit te extraheren. Deze bewerking heeft ook de bedoeling tegelijkertijd sporen oppervlak-actieve stof uit de microbolletjes te verwijderen. Opgemerkt moet worden dat water een ”niet-oplosmiddel” is, zowel voor de biologisch afbreekbare polymere stof zoals PLGA, als voor de peptide-zouten. Deze situatie is vooral gunstig voor de onontbeerlijke extractie van de 10 rest aan polymeer-oplosmiddel, zoals bijvoorbeeld het CH2CI.
Nadat die emulsie in overmaat waterig medium overgebracht is, worden de uitgeharde microbolletjes met gebruikelijke technieken verzameld, bijvoorbeeld door centrifugeren, filtreren of door bezinken onder invloed van de zwaartekracht. Het uitwassen, het zuiveren en het drogen wordt eveneens volgens gebruikelijke technieken uitgevoerd.
15 Eén van de voordelen van de werkwijze volgens de uitvinding is dat die het mogelijk maakt microbolletjes te krijgen waarvan de grootte nauwkeurig geregeld kan worden, welke regeling voornamelijk plaatsvindt bij het bereiden van de olie-in-wateremulsie (bijvoorbeeld door de roersnelheid).
Verder kan een hoge belading met peptide bereikt worden, bijvoorbeeld 5, 10 of 20 gew.%, afhankelijk van de omstandigheden. Verder is de mate van opname van het peptide-zout in vivo bijzonder hoog.
20 De met de werkwijze volgens de uitvinding uit de bovengenoemde uitgangsstoffen bereide microbolletjes worden dan, nadat ze op geëigende wijze gesteriliseerd zijn, gebruikt voor de bereiding van suspensies die men voor parenterale toediening wil gebruiken, bijvoorbeeld bij intramusculaire of subcutane injectie.
De uitvinding wordt nu door de volgende voorbeelden toegelicht.
25 Voorbeeld 1 3 g acetaat van D-Trp6-LHRH volgens de formule: (pyto)Glu-His-Trp-Ser-Tyr-D-Trp-Leu-Arg-Pro-Gly-NH2 werd met gebruikelijke technieken in het overeenkomstige pamoaat omgezet. Nadat het vermalen is, verschijnt dit zout als microdeeltjes met amorfe structuur, met een gemiddelde deeltjesgrootte van 10 pm. Oplosbaarheid in water bij 40°C minder dan 0,025 mg/ml.
30 Van het pamoaat van D-Trp6-LHRH werd 0,280 g toen in 20 ml CH2CI2 gesuspendeerd en die suspensie werd toegevoegd aan 20 ml CH2CI2 waarin 1,720 g copolymeer van D,L-melkzuur en glycolzuur opgelost was (melkzuur/glycolzuur 75:25, inherente viscositeit in HFIP 0,82 dl/g). Het mengsel werd onder roeren bij kamertemperatuur aangemaakt, zodat men een suspensie kreeg die perfect homogeen was.
De ontstane suspensie werd toen ineens uitgeschonken in 500 ml water dat 0,075% methylcellulose 35 bevatte, en het roeren bij kamertemperatuur werd 90 minuten voortgezet (roersnelheid 900 rpm). Met regelmatige tussenpozen, gemiddeld om de 30 minuten, werd de ontwikkeling van de emulsie gevolgd door een monster te nemen en dat onder de microscoop op de ontstane microbolletjes te bekijken.
Toen het roeren eenmaal beëindigd was (stabilisering van de grootte der microbolletjes) werd die emulsie ineens overgebracht naar twee liter water dat op 10°C gehouden werd; dit gebeurde onder roeren 40 totdat het mengsel homogeen was.
De microbolletjes van PGLA werden uit het reactiemengsel geïsoleerd en gezuiverd door achtereenvolgens uitwassen met water en centrifugeren, en tenslotte werden ze afgefiltreerd en onder verlaagde druk gedroogd. Zo werd 1,25 g aan PLGA-microbolletjes opgevangen (opbrengst 63%), waarvan meer dan 96% der deeltjes een doorsnede beneden 100 pm had (de top zat bij 55-85 pm).
45 Analyse (oplossen van het vaste PLGA, extractie en bepaling van het peptide met HPLG) liet zien dat de belading met pamoaat van D-Trp6-LHRH op 9,05 gew.% kwam (berekend 10%).
De aldus verkregen microbolletjes ondergingen vervolgens steriliseren met gamma-straling terwijl ze in een geëigende steriele drager zaten. Proeven in vivo (bepaling van het bloedgehalte en testosteron van mannelijke ratten) bevestigden een regelmatige afgifte van de werkzame stof over ten minste 21 dagen, wat 50 het testosterongehalte vanaf dag nr. 4 (injectie op dag 0) deed dalen tot waarden die typerend voor gecastreerde dieren zijn.

Voorbeeld 2 15 Precies dezelfde procedure als in voorbeeld 1 werd gevolgd behalve dat op 1,440 g PLGA (75:25) nu 0,560 g pamoaat van D-Trp6-LHRH gebruikt werd.
Er werd 1,49 g aan PLGA-microbolietjes opgevangen (opbrengst 75%) en meer dan 90% der deeltjes daarvan had een doorsnede kleiner dan 100 pm. De belading was 16,3 gew.% (berekend 20%).

Precies dezelfde procedure werd gevolgd als in voorbeeld 1, beginnende met 0,140 g pamoaat van D-Trpe-LHRH en 0,860 g polybutyleen-l,4-succinaat (inherente viscositeit in HFIP ongeveer 0,35 dl/g).
De ontstane organisch/waterige emulsie werd ineens overgebracht in 500 ml water en het ontstane mengsel onderging achtereenvolgens centrifugeren en uitwassen met water, affiltreren en tenslotte dragen 25 onder verlaagde druk; dit gaf 0,52 g (52% opbrengst) aan polysuccinaat-microbolletjes. Belading: 2,87 gew.% (berekend 10%).

Om te beginnen werd het acetaat van een analoga van somatostanine volgens de formule 30 I I
D-Phe-Cys-Tyr-D-T rp-Lys-Val-Cys-T rp-NH2 met gebruikelijke technieken in het overeenkomstige pamoaat omgezet, wat deeltjes met een gemiddelde grootte van ongeveer 10 pm gaf.
Van dit pamoaat werd 0,266 g in de procedure van voorbeeld 1 gebruikt met 1,734 g PLGA (75:25). Na 35 het uitschenken van de organisch/waterige emulsie in twee liter water van 10°C, homogeniseren, centrifugeren en dezelfde behandelingen als hierboven beschreven werd 1,23 g (62% opbrengst) aan PLGA-microbolletjes opgevangen, met meer dan 98% deeltjes met een doorsnede kleiner dan 100 pm (een top bij 40-65 pm). De lading 8,7 gew.% (in theorie 10%).

Precies dezelfde procedure werd toegepast als in voorbeeld 4, met 0,532 g pamoaat van het somatostatine-analoga op 1,468 g PLGA (75:25). Men kreeg 1,21 g PLGA-microbolletjes (60% opbrengst) met een belading van 17,5 gew.% (in theorie 10%).
De aldus verkregen microbolletjes, met gammastraling gesteriliseerd, werden tenslotte in een geëigende 45 steriele drager gesuspendeerd. Bij proeven in vivo (bepaling van het bloedgehalte aan het somatostatine-analoga in ratten die 1 injectie op dag 0 gekregen hadden) bevestigde een beheerste afgifte van een aantoonbare hoeveelheid werkzame stof over 20 dagen (injectie op de eerste dag).

Claims (2)
1. Werkwijze voor de bereiding van een preparaat bedoeld voor de aanhoudende en gecontroleerde afgifte van een medicamenteus peptide in de vorm van microbolletjes van een biologisch afbreekbare polymere 20 stof die de medicamenteuze stof insluit, waarbij men een suspensie van het peptide in een oplossing van de biologisch afbreekbare polymere stof in een organisch medium dispergeert in een waterig medium ter vorming van een olie-in-wateremulsie, het oplosmiddel uit de oliefase verwijdert, zodat microbolletjes worden gevormd en de microbolletjes isoleert, met het kenmerk, dat men a) een in water oplosbaar natuurlijk of synthetisch peptide met 3 tot 45 aminozuren met uitzondering van 25 Ac-D-Nal-D-p-CIPhe-D-Pal-Ser-Tyr-D-Cit-Leu-Arg-D-Ala-NH2, en Ac-D-Nal-D-pCIPhe-D-Trp-Ser-Tyr-D-Cit- Leu-Arg-Pro-D-Ala-NH2 of een zout van dat peptide omzet in een in water onoplosbaar pamoaat-, tannaat-, stearaat- of palmitaatzout van dat peptide, b) dat in water onoplosbare zout suspendeert in een organisch medium dat de biologisch afbreekbare polymere stof in opgeloste vorm bevat, 30 c) die organische suspensie dispergeert in water waaraan een geschikte oppervlakte-actieve stof is toegevoegd dat dan de continue fase van een stabiele olie-in-wateremulsie wordt, en d) die emulsie in een overmaat water brengt en men de aldus verkregen microbolletjes van de vloeistoffase scheidt.
2. Werkwijze volgens conclusie 1, met het kenmerk dat er voor het overbrengen van de olie-in- 35 wateremulsie in een overmaat water een gedeeltelijke verdamping van het organische oplosmiddel dat de oliefase vormt, uitgevoerd wordt.
