Ifølge fremgangsmåden omdannes først et vandopløseligt peptid eller peptidsalt til et vanduopløseligt peptidsalt, derefter suspenderes peptidsaltet i en opløsning af bionedbrydeligt polymert materiale, hvorpå suspensionen omdannes til en emulsion af olie-i-vand-typen, og til sidst isoleres mikrosfærerne af det bionedbrydelige 15 polymere materiale efter overførsel af olie-i-vand-emulsionen til et overskud af et vandigt medium,
Hidtil er der blevet foreslået forskellige løsninger til fremstilling af præparater, der vedvarende og kontrolleret kan frigive medikamentelle stoffer ved at anvende 20 fremstilling af bionedbrydelige implantater, mikroindkapsling eller fremstilling af bionedbrydelige porøse matricer, der er vundet fx som mikrosfærer eller mikropar-tikler af forskellig størrelse. I denne henseende kan nævnes EP-A-0052510, der omhandler mikroindkapsling ved faseadskillelse af vandopløseiige lægemidler, og EP-A-0058481 eller US-A-3976071, der omhandler fremstilling af implantater eller 25 af bionedbrydelige porøse matricer, hovedsageligt baseret på polylaktid eller copo-lylaktid-glykolid. Disse teknikker anvender forudgående opløsning i et organisk opløsningsmiddel af den bionedbrydelige polymer eller copolymer, der anvendes som støtte og, om nødvendigt, opløsning af selve det medikamentelle stof.
30 Andre teknikker, ved hjælp af hvilke der også kan tilvejebringes mikrokapsler eller mikrosfærer, anvender emulgeringsmetoder, hvor det vigtigste trin i sådanne fremgangsmåder er opnåelsen af en emulsion af olie-i-vand-typen fra en organisk opløsning af polymert materiale og en vandig opløsning af peptidet - se i denne henseende US-A-4384975, 3891570, 4389330, 3737337, 4652441 eller 35 W090/13361. Under alle omstændigheder er fagfolk imidlertid nødt til at udvikle 2 DK 176218 B1 teknikker, som er komplekse og vanskelige at styre, for i si høj grad som muligt at reducere tabene af de særdeles vandopløselige aktive peptidstoffer, som fx en dobbeltemulgering.
5 Ved anvendelse af en fremgangsmåde, hvori der dannes en emulsion af olie-i-vand-typen, som derefter overføres til et vandigt medium, gør opfindelsen det muligt mod alle forventninger at overvinde manglerne ved de hidtil kendte teknikker.
Ved at starte med at omdanne et vandopiøseligt peptid eller peptidsalt til et vand-10 uopløseligt peptidsalt gør opfindelsen det muligt for fagfolk på en hidtil ukendt made at drage fordel af de anvendte bestanddeles relative opløseligheder, og især af de anvendte opløsningsmidlers og "ikke-opløsningsmidlers" relative opløseligheder.
15 Den foreliggende opfindelse angår således en fremgangsmåde, der er ejendommelig ved, at: a. et vandopiøseligt peptid eller peptidsalt omdannes til et tilsvarende vanduoplø-seligt peptidsalt, idet vandopløselighed refererer til en opløselighed lig med eller 20 over 0,1 mg/ml ved 25°C, og vanduopløselighed refererer til en opløselighed på mindre end 0,1 mg/ml ved 25°C; b. det vanduopløselige peptid eller peptidsalt suspenderes i organisk medium indeholdende det bionedbrydelige polymere materiale i opløst tilstand; 25 c. den organiske suspension dispergeres i vandigt medium dannende den kontinuerlige fase af den resulterende emulsion; d. emulsionen overføres til et overskud af vandigt medium, og til sidst skilles de 30 således vundne mikrosfærer fra den flydende fase. 1 beskrivelsen er udtrykket "medikamentelt peptidstof" anvendt til primært at betegne et naturligt eller et syntetisk polypeptid, som er fysiologisk aktivt, og som omfatter 3-45 aminosyrer. Antallet af de polypeptider, som kan behandles i over-35 ensstemmelse med fremgangsmåden ifølge opfindelsen, er ganske stort og omfat- 3 DK 176218 B1 ter især oxytocin, vasopressin, corticotrophin, calcitonin, epidermal vækstfaktor (EGF), prolactin, inhibitin, interferon, somatostatin, insulin, glucagon, aurikulær natriuretisk faktor (ANF), endorphin, en renininhibitor, luteiniserende hormonfrigi-vende hormon (LHRH), væksthormonfrigørende hormon (GHRH), peptid T eller en 5 af de syntetiske analoger eller homologer deraf.

Den første fase i fremgangsmåden består i, at et vandopløseligt peptid eller peptidsalt ved hjælp af sædvanlige teknikker omdannes til et vanduopløseligt peptidsalt.
Med "vandopløselig" menes et peptid eller peptidderivat med en vandopløselighed, der er større eller lig med 0,1 mg/ml ved 25°C, fortrinsvis større eller lig med 1,0 30 mg/ml.
Med "vanduopløselig" menes et peptidsalt med en vandopløselighed, der er mindre end 0,1 mg/ml ved 25°C. Peptidsalte såsom pamoat, tannat, stearat eller paimitat opfylder denne betingelse.

Med hensyn til det bionedbrydelige polymere materiale er de mest almindeligt anvendte materialer polymerer såsom polylaktid, polyglykolid, copolymer af mælke-og glykolsyrer, polyester såsom polyalkylenfumarat eller -succinat eller yderligere polyorthoester, polyacetat eller polyanhydrid.

Blandt de foretrukne polymere materialer kan nævnes copolymererne af mælke- og glykolsyrer (PLGA) og især copolymererne af L- eller D,L-mælkesyre indeholdende henholdsvis 45-90 molprocent mælkesyreenheder og 55-10 molprocent glykolsyre-enheder.
10 I denne forbindelse kan også nævnes forskellige polyalkylenfumarater eller -succi-nater, især poly-l,4-butylensuccinat, poly-2,3-butylensuccinat, poly-l,4-butylen-fumarat eller poly-2,3-butylenfumarat. Hvad angår det opløsningsmiddel, der er valgt til det polymere materiale, kan der anvendes et organisk opløsningsmiddel som fx methylenchlorid, men under alle omstændigheder skal opløsningsmidlet være et "ikke-opløsningsmiddel" for det valgte peptidsalt.
20 Når først saltet er suspenderet i den organiske opløsning af det polymere materiale, inkorporeres ifølge opfindelsen denne opløsning i en forudbestemt mængde af et vandigt medium, sædvanligvis i vand suppleret med et egnet overfladeaktivt middel. Formålet er hurtigt at danne en homogen emulsion af olie-i-vand typen, 25 idet det vandige medium tilvejebringer den kontinuerlige fase. Forskellige faktorer skal tages i betragtning, når der fremstilles en sådan emulsion, idet disse faktorer successivt påvirker størrelsen eller strukturen af de mikrosfærer, der fremkommer ved fremgangsmåden. En af de faktorer, der skal tages i betragtning, er tilsætningshastigheden for den organiske opløsning til det vandige medium; en anden 30 faktor kan være temperaturen eller yderligere omrøringshastigheden eller disper-geringsenergien {ultralydsbehandling), idet sidstnævnte parameter især påvirker størrelsen på de endelige mikrosfærer. Det er muligt for fagfolk at vælge de fremgangsmåder og emulgeringstilstande, der er egnede til at opnå det ønskede mål.

Ved fremstillingen af emulsionen kan det også være en fordel at modificere volumenforholdet for de faser, der er i kontakt med hinanden, især at sænke den organiske fases oprindelige volumen i forhold til den vandige fases volumen. På grund de anvendte organiske opløsningsmidler flygtighed - fx methylenchlorid - kan den 5 fordampning, der sker spontant under omrøring, i nogle tilfælde vise sig at være tilstrækkelig; i andre tilfælde kan dette ønskelige fænomen fremskyndes ved at anvende delvis fordampning under reduceret tryk.
Nar først den organisk-vandige emulsion er blevet stabiliseret, overføres den til en 10 overskydende mængde af et vandigt medium, sædvanligvis vand. Formålet med denne proces er at intensivere hærdningen af de embryoniske mikrosfærer, der dannes i emulsionen, ved at ekstrahere det organiske opløsningsmiddel, der stadig er tilbage inden i mi krosfærerne. Denne proces har også til formål samtidig at fjerne de spormængder af det overfladeaktive stof, som kan være tilbage i selve 15 polymeren under den sidste hærdende fase. Det skal bemærkes, at vand er et "ikke-opløsningsmiddel" både for det bionedbrydelige polymere materiale såsom PLGA fx og for det peptidsalt, der er fanget inden i mikrosfærerne. Denne situation er særdeles gunstig for den uundværlige ekstraktion af det tilbageværende polymeropløsningsmiddel som fx CH2C12.
20 Når emulsionen er overført til et overskud af et vandigt medium, opsamles de hærdede mikrosfærer under anvendelse af sædvanlige teknikker, fx centrifugering, filtrering eller tyngdebundfældning. Det samme gælder vaskningen, oprensningen og tørreprocesserne.

En af fordelene ved fremgangsmåden ifølge opfindelsen er, at den gør det muligt at opnå mikrosfærer, hvis størrelse kan styres nøjagtigt, idet denne styring hovedsageligt finder sted under fremstillingen af emulsionen (fx omrøringshastighed). En anden fordel er, at der kan opnås et særligt højt peptidindhold, som udgør 5, 10 30 eller 20 vægtprocent eller endnu højere afhængig af forholdene. Desuden er udbyttet af peptidsaltinkorporeringen særligt højt; dette skyldes hovedsageligt den forudgående omdannelse af det valgte peptid fra et vandopløseligt derivat til et vanduopløseligt salt.

De mi krosfærer, der er vundet i overensstemmelse med fremgangsmåden ifølge opfindelsen ud fra de ovennævnte bestanddele, anvendes derefter, efter at være blevet steriliseret på passende måde, til fremstilling af suspensioner, der anvendes til parenteral administration, fx ved intramuskulær eller subkutan injektion.

Opfindelsen illustreres af de følgende eksempler.
EKSEMPEL 1 10 3 g af acetatet af D-Trp6-LHRH med formlen: (pyro)Glu-His-Trp-Ser-Tyr-D-Trp-Leu-Arg-Pro-Gly-l\IH2 15 blev omdannet til det tilsvarende pamoat under anvendelse af sædvanlige teknikker. Efter findeling fremkommer dette salt som dannet af mikropartikler med en amorf struktur med en gennemsnitlig partikelstørrelse på ca. 10 pm. Opløselighed: Mindre end 0,025 mg/mt i H20 ved 40°C.
20 0,280 g af pamoatet af D-Trp6-LHRH blev derefter suspenderet i 20 ml CH2C12, og suspensionen blev sat til 20 ml CH2CI2 indeholdende 1,720 g opløst copolymer af D,L-mælke og glykolsyrer (PLGA) 75:25 (molprocent/indre viskositet ("inherent viscosity") på 0,82 i HFIP). Blandingen blev fremstillet ved stuetemperatur under omrøring, hvilket gav en suspension, som var fuldstændig homogen.

Den resulterende suspension blev derefter på én gang hældt i 500 ml vand indeholdende 0,075% opløst methoxycellulose, og omrøringen af blandingen blev fortsat i ca. 90 min. ved stuetemperatur (omrøringshastighed: 900 omdr. i min.).
30 Udviklingen i emulsionen blev kontrolleret med regelmæssige mellemrum, i gennemsnit hvert 30. minut ved at tage en prøve og undersøge de vundne mikrosfæ-rer under et mikroskop.

Da omrøringen var tilendebragt (stabilisering af reduktionen i mikrosfærernes størrelse), blev emulsionen på én gang overført til 2 I vand, der blev holdt ved ca.
10°C under omrøring, indtil blandingen var homogen.
5 PLGA-m i krosfærerne blev isoleret fra reaktionsblandingen og oprenset ved successive centrifugeringer vekslende med vaskefaser med H20 og blev til sidst filtreret og tørret under reduceret tryk. 1,25 g PLGA-m i krosfærer blev således opsamlet (udbytte: 63%), som indeholdt mere end 96% partikler med en diameter på mindre end 100 pm (maximum ved 55-85 μίτι).

Analysen (opløsning af den faste PLGA, ekstraktion og bestemmelse af peptidet ved HPLC) viser, at indholdet af pamoatet af D-Trp6-LHRH i mikrosfærerne udgør 9,05 vægtprocent (teoretisk 10%).
15 De således vundne mikrosfærer blev derefter underkastet sterilisation med gammastråler og suspenderet i en egnet steril bærer. In vivo forsøg (bestemmelse af testosteron-niveauet i blodet hos hanrotter) bekræftede regelmæssig frigivelse af det aktive stof over mindst 21 dage, hvilket fra D4 (injektion ved DO) gav et fald i testosteronniveauet til værdier, der er typiske for kastrerede dyr.

1,49 g PLGA-mikrosfærer blev opsamlet (udbytte: 75%), og de indeholdt mere end 90% partikler med en diameter på mindre end 100 pm.

EKSEMPEL 3
Den samme fremgangsmåde blev anvendt som i eksempel 1 under anvendelse af 15 0,140 g af pamoatet af D-Trp6-LHRH og 0,860 g poly-l,4-butylensuccinat (indre viskositet i HFIP, ca. 0,35).
Den resulterende organisk-vandige emulsion blev overført på én gang til 500 ml vand, og den resulterende blanding blev underkastet successiv centrifugering og 20 H20-vaskebehandlinger, filtrering og til sidst tørring under reduceret tryk, hvilket gav 0,52 g (udbytte: 52%) polysuccinatmikrosfærer.
Indholdsniveau: 2,87 vægtprocent (teoretisk 10%).
25 EKSEMPEL 4 Først blev acetatet af en somatostatinanalog med formlen: D-Phe-Cys-Tyr-D-Trp-Lys-Va!-Cys-Trp-NFl2 30 omdannet til det tilsvarende pamoat under anvendelse af sædvanlige teknikker, hvilket gav partikler med en gennemsnitlig størrelse på ca. 10 pm.
0,266 g af pamoatet og 1,734 g PLGA 75:25 (molprocent) blev derefter anvendt i 35 overensstemmelse med fremgangsmåden i eksempel 1. Efter at den organisk-van- 9 DK 176218 B1 dige emulsion var blevet hældt i 2 I H20 ved 10°C, homogeniseret, centrifugeret og underkastet de samme efterfølgende behandlinger som beskrevet ovenfor, blev 1,23 g (udbytte: 62%) PLGA mikrosfærer opsamlet, som indeholdt mere end 98% partikler med en diameter på mindre end 100 pm (maximum ved 40-65 μίτι).

EKSEMPEL 5 10 Nøjagtig den samme fremgangsmåde blev anvendt som i eksempel 4 under anvendelse af 0,532 g af pamoatet af somatostatinanalogen sammen med 1,468 g PLGA 75:25.
PLGA-mikrosfærer: 1,21 g (udbytte: 60%).

De således vundne mikrosfærer, der blev steriliseret ved hjælp af gammastråler, blev til sidst suspenderet i en egnet steril bærer. In vivo forsøg (bestemmelse af 20 somatostatinanalogniveauet i blodserum hos rotter, som havde fået én injektion ved JO) bekræfter en kontrolleret frigivelse af en detekterbar mængde aktivt stof over en periode på 20 dage (injektion: IM).

EKSEMPEL 7 20 Nøjagtig den samme fremgangsmåde blev anvendt som i eksempel 6 under anvendelse af 0,634 g af pamoatet af LHRH-analogen sammen med 1,366 g PLGA 75:25.
PLGA-mikrosfærer: 1,70 g (udbytte: 85%).

De således vundne mikrosfærer blev derefter underkastet gammastrålesterilisation og suspenderet i en egnet steril bærer. In vivo forsøg (bestemmelse af analogniveauet i blodserum hos hanrotter) bekræfter regelmæssig frigivelse afen biologisk 30 signifikant mængde af aktivt stof over mindst 24 dage.

Disse resultater blev yderligere bekræftet af analyser udført på dyr, der blev aflivet på D30: vægttab for testiklerne pa mindst 80%, vægttab for sædblærer på mindst 5 90%.
EKSEMPEL 8 Nøjagtig den samme fremgangsmåde blev anvendt som i eksempel 1 under anven-10 delse af 0,05 g af pamoatet af laksecalcitonin og 1,0 g copolymer af D,L-mælke-/glykolsyrer 50:50 (molprocent).
De sædvanlige in vivo forsøg bekræfter en kontrolleret frigivelse af det aktive stof over en periode på ca. 8 dage.

Claims (9)
1. Fremgangsmåde til fremstilling af et præparat, der er beregnet til vedvarende og kontrolleret frigivelse af medikamentelle peptidstoffer, der er vundet i form af mi- 5 krosfærer af bionedbrydeligt polymert materiale indeholdende det medikamentelle stof, kendetegnet ved, at a) et vandopløseligt peptid eller peptidsalt omdannes til et tilsvarende vanduoplø-10 seligt peptidsalt idet vandopløselighed refererer til en opløselighed på lig med eller over 0,1 mg/m! ved 25°C, og vanduopløselighed refererer til en opløselighed der er mindre end 0,1 mg/ml ved 25°C; b) det vanduopløselige peptidsalt suspenderes i organisk medium indeholdende det 15 bionedbrydelige polymere materiale i opløst tilstand; c) den organiske suspension dispergeres i vandigt medium dannende den kontinuerlige fase af den resulterende emulsion; 20 d) emulsionen overføres til et overskud af vandigt medium, og til sidst skilles de således vundne mikrosfærer fra den flydende fase.
2. Fremgangsmåde ifølge krav 1, kendetegnet ved, at der udføres en delvis afdampning af det organiske opløs-25 ningsmiddel, der danner oliefasen, inden overførslen af olie-i-vand-emulsionen til et overskud af vandigt medium.
3. Fremgangsmåde ifølge krav 1 eller 2, kendetegnet ved, at det medikamentelle peptid-stof er et naturligt eller et synte-30 tisk polypeptid omfattende 3-45 aminosyrer.
4. Fremgangsmåde ifølge ét hvilket som helst af kravene 1 til 3, kendetegnet ved, at det medikamentelle stof er valgt blandt oxytocin, vasopressin, corticotrophin, calcitonin, epidermal vækstfaktor (EGF), prolactin, inhibitin, inter-35 feron, somatostatin, insulin, glucagon, aurikulær natriuretisk faktor (ANF), en- DK 176218 B1 dorphin, en renininhibitor, luteiniserende hormonfrigivende hormon (LHRH), væksthormon-frigørende hormon (GHRH), peptid T eller en af deres syntetiske analoger eller homologer.
