Abstract
Implante (7) para establecer una unión de ajuste positivo con una pieza de tejido, para unir ésta con otra pieza de tejido o con un elemento artificial que sustituya a otra pieza de tejido o la apoye, o que sea un dispositivo terapéutico auxiliar, consistiendo el implante (7) por lo menos parcialmente de un material licuable mediante energía mecánica, estando situado el material licuable en el implante (7) de tal manera que se pueda poner en contacto con la parte del tejido, se pueda excitar mediante vibraciones mecánicas y al mismo tiempo apretar contra la parte del tejido, con el fin de licuar por lo menos una parte del material licuable e inyectarlo en orificios de la parte del tejido.
Description
Implantes y dispositivo para unir partes de
tejido.
La invención se refiere a un implante conforme
al preámbulo de la primera reivindicación independiente. El
implante sirve para establecer uniones al menos parcialmente de
ajuste positivo con partes de tejido humano o animal, en particular
con partes del esqueleto, para lo cual con ayuda de los implantes se
unen partes de tejido entre sí, o partes de tejido con medios
artificiales, por ejemplo, medios que soportan o sustituyen a
partes de tejido, u otros dispositivos terapéuticos auxiliares. La
invención se refiere también a un dispositivo conforme al preámbulo
de la correspondiente reivindicación independiente. El dispositivo y
el procedimiento sirven para implantar los implantes citados.
Implantes conocidos para establecer uniones con
partes del esqueleto (hueso) son, por ejemplo, tornillos,
pasadores, grapas, etc., mediante los cuales se unen huesos con
huesos o huesos con partes artificiales de soporte,
estabilizadoras, de apoyo o que vienen a sustituir a partes del
esqueleto (placas de estabilización o fijación, hilos, alambres,
elementos de rótula artificiales, dientes postizos, etc.). Estos
elementos de unión a implantar son, por ejemplo, de metal o de
plástico, o incluso de plástico reabsorbible. Después de la
curación, eventualmente se vuelven a retirar por vía operativa, pero
también se pueden dejar en el cuerpo, donde eventualmente se van
desintegrando paulatinamente para ser sustituidos por tejido
vital.
Para estabilizar una fractura de huesos se fija,
por ejemplo, una placa de fijación con los correspondientes
agujeros y mediante los tornillos citados en la zona de la fractura.
En este caso la placa y los tornillos son, por ejemplo, de metal
(p. ej., acero inoxidable o titanio). Los tornillos son
autocortantes y se enroscan en orificios lisos en el hueso, o se
enroscan en orificios roscados pretaladrados. Con fines similares se
clavan también pasadores y grapas en orificios previamente
preparados. Las uniones formadas por lo general son con ajuste de
fricción, eventualmente también con ajuste positivo.
Para la colocación de los elementos de unión
citados hay que aplicar en todos los casos unas fuerzas
relativamente elevadas (fuerzas de torsión y fuerzas de impacto) y
en el caso de que esta clase de implantes se tengan que volver a
extraer, también durante su extracción. Esto significa que en muchas
ocasiones, para la implantación y eventual retirada, los implantes
han de tener una estabilidad mecánica superior a la que deberían
tener para la carga que tendrían que soportar en estado implantado.
Especialmente, en el caso de implantes de plásticos reabsorbibles
con una resistencia mecánica considerablemente inferior al metal,
esto da lugar a que los implantes tengan que presentar unas
secciones relativamente grandes y que por este motivo sea también
necesario realizar en el tejido vital unos orificios indeseablemente
grandes.
La colocación mecánica de los implantes cuando,
por ejemplo, sea necesario vencer el rozamiento que da lugar al
ajuste por fricción, va unido también al desarrollo de unas
cantidades de calor considerables que perjudican al tejido
circundante, especialmente el tallado de roscas, el enroscado de
tornillos autorroscantes y el clavado de implantes sin taladro
previo. Para establecer las uniones citadas se conocen también en
cirugía materiales plásticos endurecibles (p. ej., cementos con
partículas de base hidráulica o polímera), que se introducen a
presión en estado de gran viscosidad desde el exterior entre los
implantes y el tejido vital o en los defectos del tejido y se
endurecen in situ. Con ello también pueden establecerse
uniones de ajuste positivo, si bien únicamente si los orificios en
los cuales se introducen a presión estos materiales presentan los
correspondientes destalonados.
Por la publicación US-3.919.775
se conoce además el hecho de introducir tapones de junta de
gutapercha en el canal de una raíz de diente, para sellarlo. El
tapón se introduce a presión en el canal mediante un instrumento,
en cuyo caso el instrumento eventualmente se calienta eléctricamente
o se excita con ultrasonido para ablandar adicionalmente el tapón,
con el fin de que se pueda introducir a presión lo más completamente
posible en el canal de la raíz y en los canales laterales.
El objetivo de la invención es ahora el de crear
unos implantes para establecer uniones de ajuste positivo con
partes de tejido (en particular partes de huesos, cartílagos,
bandas, tendones, pero también otras partes de tejido) cuyos
implantes se puedan implantar de forma sencilla, con un pequeño
esfuerzo y especialmente de manera rápida y que puedan suministrar
inmediatamente muy buenas uniones (estabilidad primaria). Además
sería deseable que los implantes presentasen menos problemas en
cuanto a la cantidad de calor y a la concentración de tensiones, de
lo que viene ocurriendo al menos con una parte de los implantes
conocidos y el que se pudiese reducir más el volumen del material
ajeno a autoimplantar. Además es uno de los objetivos de la
invención crear un dispositivo y un procedimiento para implantar
los implantes.
Estos objetivos se resuelven por medio de los
implantes y el dispositivo que se definen en las
reivindicaciones.
La invención aprovecha los hechos de por sí
conocidos, por ejemplo, por la publicación
WO-98/42988, de que en especial los materiales
polímeros termoplásticos se pueden licuar de forma controlada
mediante la aplicación de vibraciones mecánicas y que en este
estado se pueden introducir a presión en oquedades (p. ej., poros
de la madera) mediante presión hidrostática, con lo cual una vez que
quedan rígidos se obtienen unas uniones con ajuste positivo.
De acuerdo con la invención, los implantes que
sirven para el establecimiento de uniones de ajuste positivo con
partes de tejido constan al menos parcialmente de un material que, a
unas temperaturas relativamente bajas (< 250ºC) y mediante
energía de vibraciones mecánicas (especialmente ultrasonido), es
decir, por rozamiento interno y/o exterior, se pueden llevar a un
estado licuado tal que por el efecto de una presión exterior se
puedan introducir a presión en poros u otros orificios de la parte
del tejido, formando allí el ajuste positivo una vez que hayan
vuelto a quedar rígidos.
Como materiales licuables por energía mecánica
para los implantes conformes a la invención son adecuados, en
particular, los polímeros que pasan al estado plástico a
temperaturas relativamente bajas, en particular los termoplásticos
ya utilizados en medicina, polietileno (PE), polimetilmetacrilato
(PMME), policarbonato (PC), poliamidas, poliésteres, poliacrilatos
y los correspondientes copolímeros que no son reabsorbibles, o los
polímeros reabsorbibles a base de ácido láctico y/o ácido glicólico
(PLA, PLLA, PGA, PLGA, etc.), así como polihidroxialcanoatos (PHA),
policaprolactonas (PCL), polisacáridos, polidioxanonas (PD),
polianhídridos o los correspondientes copolímeros. También son
adecuados los cementos hidráulicos o polímeros de por sí conocidos
que tienen propiedades isotrópicas, como por ejemplo, los cementos
de fosfato de calcio, de sulfato de calcio o de metacrilato, que
también pueden contener materiales propios del cuerpo o
trasplantados preparados tixotrópicamente. Gracias a sus
propiedades tixotrópicas, estos cementos pueden pasar de un estado
altamente viscoso a un estado líquido mediante energía mecánica (en
particular ultrasonido) sin necesidad de aumentar la
temperatura.
Para llevar a cabo la implantación, se pone en
contacto un implante conforme a la invención con la parte del
tejido (sobre la superficie o en un orificio eventualmente
especialmente preparado para el implante) aplicando después, por
ejemplo, energía ultrasónica y apretando al mismo tiempo contra la
parte del tejido. Mediante la debida forma del implante y la
correspondiente dosificación de la energía se puede conseguir que
el material licuable se licúe controladamente de modo local sólo el
mínimo necesario. Una vez que se haya licuado e introducido a
presión material suficiente se corta la aportación de energía, de
manera que el material licuado se vuelve rígido en su nueva forma,
mientras preferentemente se mantiene la presión sobre el
implante.
Para llevar a cabo la implantación, los
materiales citados no se licúan mediante calor exterior sino
mediante energía mecánica (energía de oscilaciones, energía de
vibraciones), debido al rozamiento interno y/o externo. De este
modo la carga térmica del tejido circundante se mantiene baja.
Mediante la energía mecánica se alcanza un elevado efecto de
cizallamiento entre las diferentes fases del material. Esto
contribuye a que con una carga reducida del entorno, los materiales
se licúen uniformemente alcanzando una viscosidad baja. El material
así licuado se introduce entonces a presión mediante presión
hidrostática en los poros u orificios del tejido circundante. De
esta manera se tiene la posibilidad de entremezclar el tejido
circundante, reforzándolo aprovechando este efecto
hidrostático.
En caso de necesidad resulta ventajoso añadir al
material licuable sustancias adicionales para cumplir otras
funciones, por ejemplo, sustancias que refuercen mecánicamente el
material, que lo hagan esponjar en una reacción secundaria o que
formen poros dentro de él, sustancias que deberán liberarse en el
entorno vital para estimular la curación o regeneración, o
sustancias que deban asumir otras funciones. Como sustancias
destinadas a estimular la curación o la regeneración se pueden
añadir al material, por ejemplo, factores de crecimiento,
antibióticos o antiinflamatorios, de tal manera que estas sustancias
se pueden introducir de forma selectiva con el flujo de material a
un punto deseado, por ejemplo, se pueden distribuir en una zona de
tejido y en el caso de que se trate de un material reabsorbible,
puedan quedar libres de forma retardada.
Con los implantes de unión objeto de la
invención pueden efectuarse más uniones puntuales o de gran
superficie. De este modo se puede influir de forma selectiva en la
distribución de cargas y controlarla. Por ejemplo, mediante los
implantes conformes a la invención, se pueden sujetar placas de
fijación o estabilización en gran superficie sobre una superficie
ósea (véanse, por ejemplo, las figuras 15 ó 16), o de forma más
puntual y con efecto de profundidad (véanse las figuras 2 a 4). Las
uniones en la superficie se logran, por ejemplo, colocando las
placas u otros dispositivos de apoyo o fijación, que presenten zonas
integrales de licuación o superficies enteras de licuación, sobre
los puntos que se trata de unir, excitándolos a continuación para
la licuación por medio de energía mecánica (p. ej.,
vibración-ultrasonido), generalmente de forma local.
Para ello las zonas licuables están equipadas convenientemente con
direccionadores de la energía, o están en contacto con ellos. Estos
direccionadores de la energía favorecen la licuación local. Los
direccionadores de la energía pueden ser elementos que sobresalgan,
que estén realizados, por ejemplo, en forma de pirámide, icono,
semiesfera o nervio y que provoquen la concentración de la energía
mecánica aplicada.
Los anclajes con efecto de profundidad se
consiguen colocando sobre la superficie de tejido o introduciendo
en ésta y excitando a continuación, por ejemplo, implantes en forma
de espiga o de taco, que presenten una sección (o geometría de
sección) uniforme o variable a lo largo de su longitud y que
consistan total o parcialmente en un material licuable. Estos
implantes están realizados ventajosamente de tal manera que la
licuación se inicie en puntos predeterminados (punta, zonas
específicas del vástago). También aquí se puede conseguir una
licuación controlada mediante direccionadores de la energía (los
elementos de las formas definidas anteriormente). Al introducir el
implante en la superficie se logra un anclaje con efecto de
profundidad. Las condiciones hidrostáticas pueden elegirse de tal
manera que el material líquido sea inyectado con gran presión en el
entorno circundante.
El dispositivo utilizado para la implantación de
los implantes objeto de la invención, es decir, para la licuación
del material licuable de un implante que esté en contacto con la
parte del tejido y para su inyección en el tejido, puede servir
adicionalmente para el control activo de la distribución de
temperaturas en el tejido circundante, de manera que se eviten
eficazmente unas cantidades de calor y unas temperaturas
insoportables y los perjuicios para el tejido vital que pueden
derivarse de ello. El proceso de implantación se controla
eventualmente mediante un control activo de la energía aportada y
evacuada (distribución de calor y gestión), mediante el mando del
dispositivo así como de eventuales sensores y elementos conductores
de calor aplicados adecuadamente. Esto quiere decir, que se influye
mediante la dosificación de la energía aportada y la evacuación del
exceso de energía.
La energía aplicada para licuar el material se
genera preferentemente mediante estimulación piezoeléctrica o
magnetorrestrictiva. Una unidad oscilante (p. ej. piezoelemento más
sonotrodo), que se controla por medio de un generador y se excita
para que vibre, transmite ondas en la gama de frecuencia de unos 2 a
200 KHz, preferentemente ultrasonido (p. ej., 20 o 40 KHz), al
implante unido activamente con ella (apretado contra ella). Éste
está acoplado de tal manera con el hueso o con el tejido que se
trata de unir que la energía sónica da lugar a una absorción de
energía interna o superficial del material licuable, con lo cual
éste se licúa al menos localmente. El proceso de licuado se
consigue mediante un intenso efecto de cizallamiento. Mediante un
segundo componente de diferente densidad, que está empotrado
localmente en el material a licuar (p. ej., en forma de bolitas),
se puede influir en el rozamiento interno y por lo tanto en la
licuación interna, tal como sucede, por ejemplo, cuando se utilizan
como implantes o partes del implante cementos tixotrópicos
particulares.
Las uniones realizadas mediante los implantes
objeto de la invención y realizadas mediante el dispositivo objeto
de la invención se basan principalmente en un ajuste positivo, donde
los medios para el ajuste positivo por ambos lados pueden ser muy
pequeños (irregularidades de la superficie o rugosidad superficial,
poros de tejido), o también mayores (oquedades mayores en el tejido
o entre partes de tejido), u orificios u oquedades realizados
mecánicamente en el tejido. Los implantes de unión se excitan
mecánicamente por ultrasonido, de tal manera que se licúan de forma
controlada, especialmente en la zona de contacto con la parte del
tejido o en su interior. La licuación tiene lugar por lo general en
la superficie de la parte del tejido o en su interior. La licuación
tiene lugar por lo general, en la superficie de la parte del tejido
o en una correspondiente oquedad (orificio), que haya sido formada,
por ejemplo, por la penetración del implante de unión a través de
la superficie al interior del tejido, o que haya sido preparada
previamente con otros medios mecánicos.
Un efecto que se ha observado en la introducción
con efecto de profundidad del material licuable se puede describir
en la forma siguiente de manera muy simplificada y esquemática: a
semejanza de un émbolo en un cilindro hidráulico, el material del
implante de unión que todavía no ha sido licuado está situado en un
orificio y lo rellena esencialmente sellándolo. Dado que por este
motivo el material licuado no puede escapar del orificio, y debido
a la carga que actúa desde el exterior (presión sobre el implante),
se forma una presión hidrostática. Ésta y las vibraciones
ultrasónicas dan lugar a que el material licuado se inyecte en las
oquedades existentes y/o de nueva formación del material (tejido
vital) circundante que se trata de unir. La profundidad de
penetración depende entre otras cosas de la oposición de este
entorno, de los parámetros de trabajo ajustados y del material
licuable (especialmente de su viscosidad). Mediante el volumen
licuable o licuado del implante de unión se puede determinar la
cantidad de material inyectado en el tejido. En el caso de que se
necesitara mucho material, o si el tamaño y número de las oquedades
existentes en el tejido fueran desconocidos, existe la posibilidad
de utilizar implantes o componentes de implante que se puedan ir
realimentando a voluntad.
Las puntas de tensión producidas por el material
desplazado y comprimido, que pudieran dar lugar al fallo, por
ejemplo, rotura del tejido, se evitan mediante la aplicación
selectiva y ajustada entre sí de ultrasonido y presión mecánica o
hidrostática, así como por la correspondiente configuración de los
implantes y de los materiales licuables dispuestos en ellos. Las
oquedades e intersticios se rellenan con el material licuado y si
el tejido es suficientemente poroso, eventualmente sin taladrado
previo. Mediante una compresión selectiva del material por zonas,
en las zonas del tejido circundante se logra que los implantes de
unión encuentren una fuerte retención incluso en tejidos muy
porosos (p. ej., tejidos óseos osteoporóticos). De este modo se
consiguen unas fuerzas de extracción o unas capacidades de carga
mecánicas muy elevadas, que en el caso de utilizarse un material al
menos parcialmente reabsorbible, se reducen controladamente después
de una fase inicial de un proceso de curación, es decir, que son
asumidas por el tejido que vuelve a crecer (estabilización
secundaria).
La invención es, por ejemplo, adecuada para
anclar una prótesis dental en una mandíbula. Para ello la prótesis
dental presenta preferentemente una parte inferior normalizada,
realizada como implante conforme a la invención y que se puede unir
con diferentes partes superiores del diente. La parte inferior
consiste, por lo tanto, en su totalidad o en parte, de material
licuable de energía mecánica, que introducido en un determinado
orificio en la mandíbula se licúa mediante la excitación por medio
de energía mecánica y se inyecta a presión en los poros del tejido
óseo. Al hacerlo el implante se adapta al orificio y a los poros del
tejido. De este modo se produce una base inmediatamente estable
(estabilización primaria), bien anclada no solo en el orificio de
la raíz del diente de la mandíbula, sino también en el tejido óseo
contiguo, para fijar una parte superior del diente, que
eventualmente se sustituirá más adelante al menos en parte por
tejido óseo regenerado (estabilización secundaria).
Otro campo de aplicación de la invención aquí
descrita se encuentra en el campo de la medicina de implantación,
por ejemplo, de elementos de articulaciones artificiales. Tanto una
cavidad cotiloidea como un cóndilo o su caña artificiales se pueden
unir mediante los implantes objeto de la invención con el tejido
óseo vital, o se pueden anclar en éste. Además, para la transmisión
cuidadosa de cargas de la implantación, se eligen los materiales
que intervienen de tal manera que se eviten en gran medida saltos de
rigidez, todo lo cual contribuye positivamente a la vida útil del
implante.
El dispositivo utilizado para la implantación de
los implantes objeto de la invención se puede describir de forma
simplificada en la forma siguiente: un generador sirve para generar
vibraciones eléctricas, que se transmiten a una unidad vibratoria a
través de un medio de transmisión, por ejemplo, un cable. La unidad
vibratoria se compone normalmente de un elemento oscilante (unidad
de accionamiento) y de un resonador, que están unidos entre sí con
unión activa. La unidad de accionamiento (p. ej., un piezoelemento)
excita al resonador para que oscile. Uno de estos dispositivos se
conoce por la patente GB-A-2277448,
en cuya descripción se basa el preámbulo de la reivindicación 17.
Las oscilaciones del resonador se transmiten al implante, bien de
forma directa o por medio de un elemento de transmisión. Entonces
el implante se licúa al menos parcialmente debido a la licuación
interna o debido al contacto con un entorno que todavía no oscila
(parte de tejido u otra parte del implante). Para ello, el implante
se puede sujetar mediante una sujeción correspondiente y se conduce
mediante una guía. Especialmente con vistas a una cirugía invasiva
mínima resulta adecuada la fijación del implante directamente en la
unidad oscilante. Se trata especialmente de los dispositivos de
sujeción y/o de conducción, que no solamente sirven para la
sujeción o fijación temporal del implante sino también para la
gestión de la temperatura (especialmente la evacuación del
calor).
Debido a la forma como se licúa el material del
implante, de manera selectiva y local, no se generan grandes
cantidades de calor. Además se puede controlar activamente la
temperatura de las zonas de tejido contiguas al implante mediante
una gestión de la temperatura, por ejemplo, mediante elementos
conductores del calor que actúan selectivamente para la evacuación
de calor, o mediante líquidos de refrigeración (p. ej., solución
estéril de Ringer), que actúan controlando la temperatura.
El procedimiento para la implantación de un
implante conforme a la invención, por ejemplo, en el esqueleto
humano o animal se desarrolla en la forma siguiente: el implante se
pone en contacto con la parte del esqueleto, a continuación se
generan oscilaciones mecánicas que se transmiten al material
licuable del implante por medio de energía mecánica, mientras se
aprieta el implante contra la parte del esqueleto. Se aporta
energía mecánica hasta que el material licuable esté suficientemente
líquido y penetre en la zona de contacto en el tejido óseo o por lo
menos en las irregularidades superficiales de la parte del
esqueleto. A continuación se vuelven a detener las oscilaciones
mecánicas, con lo cual el material licuado solidifica
(preferentemente manteniendo la presión hidrostática) y el implante
queda anclado con ajuste positivo en la parte del esqueleto.
Los implantes de unión conformes a la invención
tienen, por ejemplo, forma de pasadores, tacos o placas/láminas.
Sirven para unir partes de tejido entre sí o partes de tejido con
elementos artificiales.
Para las implantaciones de la clase antes
descrita se emplea convenientemente un kit o conjunto que
comprende, por lo menos, un tipo de los implantes conformes a la
invención, preferentemente una selección de implantes de diferentes
dimensiones, adecuadas para un campo de aplicación, así como un
dispositivo para realizar la implantación. El kit comprende
preferentemente también medios para el empleo estéril del
dispositivo (envueltas estériles para el dispositivo) y
eventualmente, piezas de sustitución de componentes (en particular
un resonador, la parte distal del resonador o la parte de
transmisión) que se puedan esterilizar. Estas partes del resonador
también pueden estar adaptadas por sus diferentes formas a diversos
implantes y/o aplicaciones. El kit comprende además,
preferentemente, instrucciones relativas a la implantación,
indicaciones sobre parámetros de implantación para tipos
específicos de implantes y otros medios auxiliares para la
preparación del tejido (p. ej., brocas preparadas de acuerdo con
los implantes), instrumentos de posicionamiento, instrumentos de
control o conducciones de implante adaptadas a los implantes y/o
resonadores.
El kit o conjunto se mantiene siempre disponible
mediante la reposición de implantes. La selección depende de las
necesidades, que pueden variar con el tiempo. Los suministros de
reposición (kit de repuesto y adicional) comprenden, por una parte,
la sustitución de los implantes consumidos y a su vez los nuevos
tipos de implantes, por ejemplo, con los correspondientes medios
para la preparación del tejido, instrumentos de posicionamiento,
instrumentos de control, resonadores o partes del resonador,
adaptados, conducciones de los implantes y en particular las
correspondientes instrucciones para la implantación.

La figura 1 muestra esquemáticamente y de forma
muy simplificada un ejemplo con una forma de realización de un
dispositivo de implantación 1, que puede utilizarse para implantar
los implantes conformes a la invención.
El dispositivo 1 consta de un generador 2 y de
una unidad oscilante 3, que están unidas entre sí por medio de un
cable 4. La unidad oscilante 3, alojada parcialmente en una carcasa
5, está realizada como equipo de mano, que se emplea, en cuanto a
su manejo, de forma semejante a un taladro de mano. La unidad
oscilante 3 lleva un elemento oscilante (no representado con mayor
detalle) integrado en la carcasa 5, que tiene unión activa con un
resonador (sonotrodo) 6. Por lo menos una parte distal de resonador
sobresale de la carcasa 5. El generador 2 suministra energía al
elemento oscilante. Excitado por el elemento oscilante, el resonador
vibra a una frecuencia predeterminada o eventualmente con un
conjunto de frecuencias predeterminado. Son especialmente adecuadas
las frecuencias de 2 a 200 KHz y las amplitudes del resonador de 1 a
100 \mum, en la dirección (dirección z) indicada por la doble
flecha. Las frecuencias se pueden ajustar en función del campo de
aplicación, de los materiales que se trata de licuar y de la forma
del resonador y del implante. También cabe la posibilidad de
superponer a las vibraciones, en la gama de ultrasonidos, unas
oscilaciones mecánicas adicionales, por ejemplo, de menor
frecuencia y mayor amplitud. Pero en muchos casos será suficiente
con que el dispositivo esté preparado para una única frecuencia de
oscilación, por ejemplo, para 20 ó 40 KHz y por ejemplo, para una
amplitud del resonador de unas 20 a 30 \mum en la dirección z
(dirección en la que se comprime un implante 7 contra una parte del
tejido por medio del resonador 6). Para regular la potencia (energía
aportada por unidad de tiempo), la excitación puede ser pulsante,
pudiendo ajustarse las separaciones entre impulsos y eventualmente
la longitud de los impulsos.
Ventajosamente, la frecuencia de oscilación y la
forma del resonador se ajustan de forma de por sí conocida de tal
manera que el resonador vibre en una onda estacionaria y su extremo
distal, mediante el cual se aprieta sobre el implante, presente una
amplitud máxima en dirección z. También es conveniente que los
implantes en forma de espiga tengan una longitud que esté ajustada a
una frecuencia de excitación predeterminada y a un material de
implante predeterminado.
Tal como está representado en la figura 1, el
extremo distal del resonador 6 puede estar configurado para sujetar
un implante 7, lo cual facilita el posicionamiento del implante en
una parte del tejido o en un hueco de una parte del tejido, en este
caso de un hueso de la pierna 10. Para un posicionamiento y una
implantación sin orificio se deberá prever una conducción del
implante que se apoye en la carcasa 5. Pero también cabe
perfectamente que en una realización, que no es parte de la
invención, el resonador 6 esté provisto de una superficie frontal
plana, a modo de un martillo y que simplemente apriete contra un
implante sujeto en un hueco del tejido o por medio de una sujeción
o guía adecuada independiente. En el caso de un resonador de esta
clase es preciso vigilar para que su superficie frontal distal no se
pegue al implante durante la implantación. Esto se consigue, por
ejemplo, mediante una superficie frontal correspondiente, no
adherente, del resonador, o mediante una parte del implante
contigua al resonador que sea de un material no licuable.
Para que el dispositivo pueda utilizarse en una
zona estéril del quirófano, se utiliza, por ejemplo, dentro de una
envuelta estéril. La envuelta estéril presenta ventajosamente un
orificio para la parte distal del resonador y el resonador o la
parte distal de resonador es desmontable, intercambiable y
esterilizable.
Otros ejemplos de la forma de realización del
dispositivo de implantación 1 conforme a la invención están
realizados, por ejemplo, como equipos manuales, que llevan todos los
componentes (incluida la alimentación de energía, p. ej., en forma
de acumulador) o también como equipos fijos.
La figura 2 muestra una placa de fijación o
estabilización fijada en una parte de hueso 20 mediante implantes 7
conformes a la invención, en la zona de una rotura o fisura del
hueso, mediante la cual se estabiliza la fisura o fractura. En este
caso, la parte del hueso 20 presenta una capa cortical exterior 22,
que no es muy gruesa pero relativamente compacta y debajo un tejido
esponjoso 23, que es poroso. A diferencia de lo representado en la
figura 2, en el tejido natural la transición de cortical a esponjosa
es una transición paulatina, donde el tejido se va haciendo cada
vez más poroso. Los implantes 7 pasan a través de la placa 21 que
presenta para ello los orificios correspondientes, pasan por la
cortical 22 y la esponjosa 23 y van anclados como mínimo en la
esponjosa 23.
Las figuras 3 y 4 muestran en sección y a mayor
escala dos ejemplos de implantes 7 conformes a la invención, que
pueden utilizarse, por ejemplo, para la aplicación representada en
la figura 2. La figura 3 muestra un implante después de la
implantación, la figura 4 otro implante posicionado en un hueco 24
de la placa 21 de la cortical 22 para ser sometido a energía
vibratoria.
Para la implantación es necesario abrir por lo
menos la capa cortical 22, por ejemplo, mediante taladrado. El
correspondiente orificio puede prolongarse eventualmente como
agujero ciego en la esponjosa 23. Dado que la cortical no presenta
poros adecuados para la inyección de material licuado, se pueden
crear en ella orificios o rugosidades superficiales, tallando por
ejemplo, una rosca 25, o escarificando las paredes interiores del
orificio, donde se pueda inyectar a presión el material licuado que
solidifique formando una unión con ajuste positivo. En la esponjosa
23 se inyecta el material licuado del implante en los poros y de
esta manera se ancla en profundidad el implante 7. Se ha comprobado
que la inyección a presión hidrostática del material líquido en los
poros del tejido resulta notablemente más delicado para el tejido
que la introducción mecánica de un material sólido. Por este motivo
se pueden establecer también uniones estables con tejidos que no
presenten gran resistencia mecánica, tales como, por ejemplo, con
tejidos óseos osteoporóticos.
Para unir el implante 7 con la placa 21 se puede
prever una cabeza igual que para los tornillos mecánicos, tal como
está representado en la figura 2. Por otra parte se puede prever
también en la placa 21, que es, por ejemplo, metálica, según la
figura 3, una rosca como en la cortical 22, donde penetre el
material licuado y solidifique formando un ajuste positivo. En este
caso se puede renunciar a una cabeza. El implante 7 se alinea a
haces con la placa 21, no mediante el recorte indeseable de una
parte que sobresalga sino introduciendo el implante debidamente
dimensionado hasta la posición deseada.
Para una placa de material termoplástico 21, tal
como está representada en la figura 4, se puede realizar la unión
entre la placa y el implante (seguro contra aflojamiento) por medio
de una unión de ajuste de material (soldadura o pegado)
simultáneamente con el anclaje en el tejido. Esa unión de ajuste de
material comienza a formarse en el punto de unión 26 al introducir
el implante. También en este caso se introduce el implante 7
ventajosamente de tal manera que al final quede a haces con la cara
exterior de la placa 21.
Puesto que no es necesario enroscar los
implantes 7 en el tejido, éstos no necesitan estar provistos, como
los tornillos de colocación mecánica, con medios preparados para
acoplar una fuerza de torsión relativamente grande. El dimensionado
de los implantes de puede efectuar exclusivamente de acuerdo con su
función en estado implantado, es decir, que los implantes pueden
ser más esbeltos y los orificios a realizar en el tejido pueden
diseñarse menores de lo necesario para tornillos convencionales del
mismo material, dado que las formas para el ajuste positivo se
obtienen mediante la licuación y nueva solidificación del material,
tienen menores tensiones y entalladuras, lo que vuelve a
incrementar su resistencia y les confiere menor propensión a la
fatiga del material.
Los implantes conformes a la invención que se
vayan a anclar con efecto de profundidad en la parte del tejido
como en las figuras 2 a 4, tienen preferentemente forma de espiga o
taco y llevan el material licuable, por ejemplo, en su extremo
distal así como en otras zonas de la superficie donde sea deseable
el anclaje (p. ej., en roscas en la placa 21 y la cortical 22).
Para ello, tal como está representado en las figuras 2 a 4, pueden
consistir enteramente del material licuable, en cuyo caso el extremo
distal y las zonas superficiales en las que se vaya a licuar
especialmente el material, están equipados preferentemente con
direccionadores de la energía, o donde en las superficies que están
en contacto con estas zonas se prevean esta clase de direccionadores
de energía. Esta clase de direccionadores de energía son, por
ejemplo, extremos distales del implante que van adelgazando en
punta o de otra manera para dar lugar a una o varias zonas de la
punta, esencialmente de forma puntual o lineal. Otras zonas de la
superficie que se vayan a licuar presentan, por ejemplo, resaltes,
puntas o nervios, cuyas alturas y anchuras se deberán adaptar al
anclaje que se vaya a establecer. Los direccionadores de energía
sobresalen por lo menos 10 \mum de la superficie, pero también
pueden ser considerablemente mayores, dando lugar como nervios
axiales a unas secciones de espiga con resaltes o aristas, tal como
están representados a título de ejemplo en las figuras 5a a 5d. Los
implantes en forma de espiga presentan esta clase de secciones en
toda su longitud o sólo en una parte de su longitud.
Para implantes en forma de espiga que deban
anclarse, además del anclaje en la zona de su extremo distal o como
anclaje único en la zona de su superficie envolvente, se deberán
prever huecos del tejido (p. ej., taladros), que al introducir el
implante den lugar, al menos por zonas, a un ajuste de fricción
entre el tejido y el implante o los direccionadores de energía, que
por lo tanto son un poquito más estrechos que la sección de los
implantes.
Para otras funciones, el material licuable puede
contener fases extrañas u otras sustancias. En particular, el
material licuable puede estar reforzado mecánicamente mediante una
mezcla con fibras o Whisker (p. ej., cerámicas o vidrios de fosfato
de calcio), el material licuable puede presentar componentes que
esponjen o sean solubles in situ y que por lo tanto formen
poros (p. ej., poliésteres, polisacáridos, hidrogeles, fosfato
sódico), o se le pueden añadir sustancias que se liberen in
situ tales como factores de crecimiento, antibióticos,
antiinflamatorios o tampones (p. ej., fosfato sódico) para evitar
los efectos nocivos de una desintegración ácida. También cabe
imaginar mezclas para hacerlo visible en imágenes de rayos x y
funciones similares.
Se ha visto que para anclar en espongiosa los
implantes según las figuras 2 a 4, que sean de polímeros, como, por
ejemplo, PC o PLLA y que tengan un diámetro de 3 a 4 mm, se aplican
ventajosamente para la introducción a presión unas fuerzas del
orden de 0,5 a 5 N por mm^{2} de sección del implante. Con unas
fuerzas, del orden de magnitud citadas, los implantes se pueden
introducir en el tejido con una velocidad superior a 5 mm/s.
Las Figuras 6 a 8 muestran a título de ejemplo
otros tres implantes en forma de espiga 7, que además de las zonas
de material licuable presentan un núcleo 11 (figuras 6 y 7) o una
vaina 13 (figura 8) de un material no licuable, por ejemplo, de
metal, cerámica, vidrio o un material compuesto.
Los implantes según las figuras 6 y 7 presentan
en su extremo distal una caperuza 12 del material licuable, que
acaba más o menos en punta (figura 6) o que presentan varias zonas
extremas (figura 7) que acaban más o menos en punta o en forma
lineal. La superficie envolvente del núcleo 11 va rodeada
enteramente del material licuable (figura 6) o sólo por zonas, en
cuyo caso estas zonas se extienden axialmente y pueden ser de forma
anular (figura 7), o pueden estar distribuidas de manera regular o
irregular sobre la superficie envolvente. También en estas zonas de
la superficie envolvente se deberán prever ventajosamente
direccionadores de energía, tal como se han descrito anteriormente
para implantes que sean enteramente de material licuable. Según la
profundidad de anclaje deseada en la zona de la envolvente, el
material licuable tiene mayor o menor espesor, pero no debería
tener menos de aprox. 10 \mum.
Como direccionadores de energía resultan también
adecuadas unas reducciones de sección escalonadas, tal como están
representadas en la figura 6. Los implantes que presenten esta clase
de escalones se implantan ventajosamente en orificios del tejido
debidamente escalonados o que se vayan estrechando.
El revestimiento de un implante en forma de
espiga o taco con núcleo 11 no licuable puede afectar o bien a todo
el extremo proximal del implante o sólo a la zona exterior anular
compuesta por el material licuable.
El implante según la figura 8 lleva el material
licuable en el interior de la vaina 13, que no es licuable. La
vaina 13 está dotada de unos orificios que están dispuestos allí
donde se desee realizar el anclaje. Una forma de realización de
este tipo del implante conforme a la invención resulta especialmente
adecuada para la utilización de materiales isótropos de alta
viscosidad como material licuable, ya que un material de esta clase
no está en condiciones de soportar la carga mecánica del resonador
que aprieta sobre el implante. Los orificios en la vaina se deberán
dimensionar de tal manera que el material de alta viscosidad no
pueda salir, pero sí en su forma licuada. Son especialmente
adecuadas vainas 13 de materiales sinterizados porosos. Un implante
con vaina 13 se ha de posicionar en un orificio del tejido y el
resonador solamente se colocará encima del material licuable, es
decir, que su sección se deberá adaptar a la sección interior de la
vaina.
En el extremo próximo de los implantes en forma
de espiga o de taco se puede prever, por ejemplo, un regruesamiento
a modo de cabeza, donde puede ir fijada una pieza artificial que
sustituya o fije una parte del tejido o se puede fijar otro
dispositivo auxiliar terapéutico, o puede estar dispuesto un medio
de fijación para un hilo de sutura o un alambre de Cerclage. En el
extremo próximo de un implante en forma de espiga o taco también
puede estar situado un medio de sujeción, que colabore con el
correspondiente medio de sujeción en el resonador (véanse las
figuras 9 a 11).
Un núcleo 11, por ejemplo, metálico, en un
implante en forma de espiga o taco que se vaya a implantar mediante
ultrasonido, sirve generalmente de refuerzo mecánico del implante y
está debidamente dimensionado para ello. Pero el núcleo puede ser
también considerablemente más delgado para que se pueda retirar con
facilidad fuera del implante. En este caso sirve, como ejemplo,
para hacer visible en una radiografía, por ejemplo, para una
implantación invasiva mínima o como hilo de guiado y se vuelve a
retirar inmediatamente después de la implantación.
A un implante que tenga, por ejemplo, un núcleo
metálico y que de acuerdo con la invención se ancle en el tejido
mediante un material licuable y reabsorbible, este material le
confiere una estabilidad primaria inmediata. Debido a la
desintegración del material de anclaje se va aflojando el anclaje y
la estabilización obtenida por medio del implante se dinamiza, de
manera que sucesivamente son mayores los esfuerzos que han de ser
soportados por el mismo tejido, lo cual fomenta un proceso natural
de regeneración e impide en muchos casos los procesos de
desintegración. Después de la desintegración del material licuable,
el núcleo se puede retirar muy fácilmente, si su superficie está
realizada de tal manera que el tejido vital no se encarne con ella.
Pero si su superficie está formada de tal manera que se fomente el
encarnamiento (superficie bioactiva) se obtiene una estabilidad
secundaria ideal para un implante que vaya a permanecer en el
tejido, o para un núcleo de implante que vaya a quedar en el tejido
(véase también la figura 28).
Los núcleos de implantes, tales como los
representados en las figuras 6 y 7, no pueden ser solamente de
metal (p. ej., aceros, titanio o aleaciones de
cobalto-cromo) sino también, según la aplicación, de
materiales plásticos (p. ej., polieterarilcetonas,
polifluoroetilenos y/o policloroetilenos, polieterimidas,
polietersulfonas, cloruro de polivinilo, poliuretanos,
polisulfonas, poliéster) o de materiales cerámicos o vítreos (p. ej.
óxido de aluminio, óxido de zirconio, silicatos, cerámicas o
vidrios de fosfato de calcio) o de materiales compuestos (p. ej.,
termoplásticos de alta temperatura reforzados con fibra de
carbono).
Las figuras 9 a 13 muestran a título de ejemplo
diversas aplicaciones para sujetar un implante 7 conforme a la
invención en forma de espiga o taco en o junto al extremo distal del
resonador 6 (sonotrodo) del dispositivo de implantación 1 (figura
1). Esta sujeción puede ser, por ejemplo, una sujeción de ajuste
positivo, tal como la representada en las figuras 9 y 10. El ajuste
positivo está realizado, por ejemplo, como cierre a presión (figura
9) de una prolongación próxima 14 de carácter elástico de un núcleo
de implante 11 o de un implante 7, que se puede introducir en el
correspondiente orificio 15 en el extremo distal del resonador 6 y
allí encaja a presión. Pero el ajuste positivo también se puede
realizar mediante un pasador 16 que atraviese el resonador 6 y la
prolongación próxima 14 de un núcleo de implante 11 o de un implante
7 y que esté debidamente asegurado. Para ello se procura
ventajosamente que el ajuste positivo esté situado a una distancia
d del extremo distal del resonador, donde está situado un nodo para
las oscilaciones en la dirección z, es decir, que la amplitud en
dirección z sea esencialmente igual a 0.
La figura 11 muestra una unión con ajuste por
fricción entre el resonador 6 y el implante 7 mediante el
atornillamiento 17. Cuando este atornillamiento está pretensado de
tal manera que la vibraciones se propaguen sin romper a través del
interfaz, entonces el implante 7 constituye una parte del resonador
6 y se deberá diseñar en consecuencia. Es decir, que el extremo
distal del resonador 6 no tiene por qué presentar ninguna amplitud
máxima en la dirección z y aquí también puede haber, por ejemplo, un
nodo.
Las figuras 12 y 13 muestran sujeciones
ventajosas de implantes 7 en el resonador 6, para implantes cuyo
extremo próximo sea del material licuable. En ambos casos, se trata
de un conformado y pegado condicionado por el efecto del
ultrasonido, del extremo próximo del implante 7 en el extremo distal
del resonador 6, que se inician a través de los correspondientes
direccionadores de energía en el direccionador 6. La figura 12
muestra un resonador 6 con una superficie distal, que está realizada
como la superficie activa de impacto de un martillo de picapedrero.
La figura 12 muestra un resonador con un direccionador de energía
central. En ambos casos, el extremo próximo del implante 7 se pone
en contacto con los direccionadores de energía del resonador 6 y se
pone en oscilación el resonador. El material licuable situado en la
zona de energía del resonador es el primero que se licúa y se pega
al resonador, adoptando la forma de la superficie distal de éste y
formando una cabeza 18 en el caso que está representado en la
figura 12.
La sujeción de los implantes, tal como está
representada en las figuras 9 a 13, se efectúa ventajosamente antes
de posicionar el implante sobre o en la parte del tejido y una vez
realizada la implantación se vuelve a soltar, en los casos según
las figuras 12 y 13 aplicando una fuerza con lo cual el separador 6
se separa del implante 7, por ejemplo, se quiebra o se
desenrosca.
La figura 14 muestra cómo en otros campos de
aplicación para los implantes conformes a la invención la fijación
de una placa de recubrimiento 30, de hueso o de un material
artificial, encajada en un hueco de la cubierta del cráneo 29, así
como la fijación de una placa de fijación 31, por ejemplo,
artificial, sobre un hueso de la mandíbula 32 roto o fisurado. Cabe
imaginar aplicaciones similares en la cirugía de reconstrucción en
el campo facial. Las uniones a establecer entre la placa de cubierta
30 y el tejido óseo circundante se limitan convenientemente a
puntos seleccionados del intersticio 33 entre la placa y el hueso
nativo. Del mismo modo se une la placa de fijación 31 a unos puntos
seleccionados 31 de la placa, con el hueso de la mandíbula. Las
uniones en los puntos seleccionados se realizan mediante pasos de
implantación consecutivos sirviéndose del dispositivo de
implantación 1.
Las figuras 15 a 17 muestran en sección y a
mayor escala uniones que pueden realizarse con implantes 7
conformes a la invención y que son, por ejemplo, adecuados para las
aplicaciones representadas en la figura 14.
La figura 15 muestra un implante 7 conforme a la
invención, que para establecer una unión al menos local entre el
hueso del cráneo 29 y una placa de cubierta para colocar en un hueco
del hueso del cráneo, que es, por ejemplo, de un material poroso
(p. ej., también de hueso de cráneo), se posiciona (arriba) y
después se implanta mediante energía ultrasónica (flecha doble)
para unir el hueso del cráneo 29 y la placa de cubierta 30 a través
del intersticio 33 (abajo).
El intersticio 33 está realizado preferentemente
oblicuo, de tal manera que las fuerzas de presión que actúan desde
el interior sobre la zona del intersticio sean absorbidas por la
cubierta del cráneo 29 y por la cara exterior el intersticio 33
está ensanchado para el posicionamiento del implante 7. El implante
que tiene, por ejemplo, forma esférica o de salchicha y que es de un
material termoplástico o tixotrópico, se posiciona en el
ensanchamiento interior del intersticio y se le aplica la energía de
vibración. De este modo se licúa el material del implante y se
inyecta por uno de los lados en los poros de la cubierta del cráneo
29 y por el otro lado, en los correspondientes poros de la placa de
cubierta 30, que es de hueso, o en los correspondientes orificios
establecidos artificialmente (p. ej., línea de trazos y puntos) en
una placa artificial. De este modo se obtiene en ambos lados una
unión con anclaje de ajuste positivo entre la cubierta del cráneo
29 y la placa de cubierta 30.
La figura 16 muestra una lámina de fijación 35,
que puede tener también la forma de una estructura textil plana y
que puede utilizarse, por ejemplo, para la fijación local de la
placa de cubierta 30 en el hueco del hueso del cráneo 29. La lámina
35 tiene, por ejemplo, forma de banda y es ventajosamente flexible.
Consta enteramente de un termoplástico licuable o está reforzado,
por ejemplo, con una estera de fibras o una estructura similar. Se
coloca encima de la fisura 33 y se excita por ambas caras (dobles
flechas) mediante el dispositivo de implantación (figura 1), de tal
manera que se pegue a la superficie de la cubierta del cráneo 29 y
de la placa de cubierta 30 (unión de gran superficie, de menor
efecto en profundidad, que también se puede limitar a una multitud
o a un dibujo de distintos puntos de fijación individuales). En caso
de necesidad, las zonas de la superficie en las cuales el implante
se ha de unir con el material situado debajo, se someten al
correspondiente tratamiento previo, por ejemplo, se escarifican, o
en la placa artificial 30 se prevén las correspondientes
estructuras superficiales (irregularidades de la superficie,
rebajes, ranuras, etc.). Para unir la lámina 35 con una superficie
ósea basta con una presión del orden de magnitud de 0,5 a 3 N por
mm^{2} de superficie frontal del resonador.
La figura 17 muestra una placa de fijación 36
que va fijada por medio de una lámina de fijación 35 o de una
correspondiente estructura textil plana sobre el intersticio 33 y
que para poder soportar fuerzas correspondientemente grandes es,
por ejemplo, de metal y por eso se puede emplear no sólo en la
aplicación craneal sino también en la aplicación de mandíbula
también representada en la figura 13, o en la aplicación según la
figura 2. La placa de fijación 36 es por lo tanto de un material no
licuable, en el sentido de la invención y por su superficie
orientada hacia el tejido que se trata de fijar presenta una
estructura superficial adecuada para un ajuste positivo. La lámina
35 se posiciona entre la placa 36 y el tejido o material que se
trata de fijar y se excita a través de la placa 36, por lo menos
localmente, con energía vibratoria y con ello se une con la
superficie de la cubierta del cráneo 29 o la placa de cubierta 30
respectivamente. La unión positiva entre la lámina 35 y la placa de
fijación 36 puede establecerse en el curso de la implantación, o la
placa 36 puede emplearse como implante prefabricado junto con la
lámina 35 unida a la misma. En un implante de esta clase de dos
capas, la unión entre las capas también puede tener lugar mediante
un ajuste de material (pegado o soldadura). En un implante de esta
clase de dos capas, la lámina 35 también se puede limitar al
recubrimiento de la placa, cuyo recubrimiento ventajosamente no
tendrá un espesor constante, sino que en el sentido de los
direccionadores de energía consiste en un dibujo a base de resaltes,
puntas o nervios, que presentan una altura mínima (altura de
recubrimiento) de aprox. 10 \mum.
La placa de fijación 31 representada en la
figura 14 presenta, por ejemplo, unas zonas de lámina 31' situadas
en los correspondientes rebajes, con una superficie exterior dotada
de direccionadores de energía y en estos puntos se une con el hueso
de la mandíbula situado debajo.
Especialmente para la aplicación representada en
la figura 16 puede ser ventajoso que el resonador que se vaya a
utilizar esté realizado de tal manera que las vibraciones
transmitidas al implante no estén orientadas perpendicularmente a
la superficie de unión que se trata de establecer (dirección z) tal
como se indica mediante dobles flechas en las figuras, sino en
paralelo a ésta (dirección x/y). Para esto resulta eventualmente
adecuado un elemento de transmisión 37 tal como está representado en
la figura 18. Este elemento de transmisión 37 está unido con ajuste
de fricción con el resonador 6, concretamente en un punto en el que
la onda representa en dirección z un nodo (amplitud = 0) y por lo
tanto, la onda tiene máxima amplitud en la dirección x/y. Esta
vibración en dirección x/y se transmite a la lámina 35 por medio del
elemento de transmisión 37.
La figura 19 muestra esquemáticamente y de
manera muy simplificada, como otra aplicación de los implantes
conformes a la invención, un elemento de apoyo 40 para una zona de
la columna vertebral humana. El elemento de apoyo 40 es elástico y
soporta la zona de la columna vertebral de forma estacionaria o
eventualmente transitoria. El elemento de apoyo 40 se fija a las
vértebras en el sentido de la invención, por el hecho de que él
mismo es de un material correspondientemente licuable y se fija sin
efecto de profundidad (análogo a la unión según la figura 16), por
el hecho de ser de un material no licuable y se une a las vértebras
mediante lámina y sin efecto de profundidad (análogo a la figura
17) o con pretaladrado y efecto de profundidad (análogo a las
figuras 2 a 4). En la figura están representados unos implantes 7 en
forma de espiga, que presentan, por ejemplo, una cabeza que
sobresale del elemento de apoyo y que está realizada de acuerdo con
la figura 12. Para un apoyo estacionario se emplean implantes de
unión y elementos de apoyo de materiales no reabsorbibles, mientras
que para un apoyo transitorio pueden imaginarse piezas de materiales
reabsorbibles.
La figura 20 muestra la aplicación de un
implante 7 conforme a la invención, en forma de taco, como base
para una prótesis dental 40 en un hueso de mandíbula 32. El implante
7 consiste al menos parcialmente en un material termoplástico o
tixótropo. En la superficie frontal, lleva medios para la fijación
del diente artificial 40, de un puente o de una prótesis. El
implante se coloca en el correspondiente orificio 41, con o sin el
diente artificial y se sigue introduciendo a presión sometido a la
vibración ultrasónica. Comoquiera que de este modo se licúa por lo
menos una parte del implante, no solamente rellenará sin huecos todo
el orificio sino que también quedará inyectado en los poros del
hueso de la mandíbula, de modo que se forma una unión con efecto de
profundidad tal como está representada, por ejemplo, en sección en
la figura 21.
La figura 22 muestra en sección otro ejemplo de
una forma de realización del implante objeto de la invención, que
es especialmente adecuado para la aplicación representada en la
figura 20. En este caso se trata de un implante cuya zona de
material licuable no está situada en su superficie, sino dentro de
una vaina 13 permeable para el material licuable en su estado
líquido, tal como ya se había descrito con relación a la figura 8.
El implante está representado seccionado longitudinalmente, que a la
izquierda del eje vertical está dibujado antes de la aplicación del
ultrasonido y a la derecha del eje central, después de la aplicación
del ultrasonido. La vaina 13 es, por ejemplo, de un material
sinterizado metálico o cerámico, con una porosidad abierta y asume
la función portante del implante. En el caso representado lleva un
orificio 42, dotado, por ejemplo, de una rosca interior y en el
cual se puede fijar un diente artificial, un puente o una prótesis
dental. Presenta otro orificio anular 43 donde va posicionado el
material licuable, por ejemplo, un trozo de forma cilíndrica 44 del
material licuable. Para una licuación selectiva están dispuestos en
el interior del orificio de forma anular 43 unos direccionadores de
energía, con los cuales está en contacto el material licuable.
El implante según la figura 22 se posiciona, por
ejemplo, en un orificio de un hueso de la mandíbula (41, figura
20), y a continuación se aplica energía mecánica al material
licuable, por medio de un resonador 60 que tiene su extremo distal
en forma anular. De este modo se licúa el material y se inyecta a
presión a través del material poroso de la vaina al tejido óseo
circundante, con lo cual el implante queda anclado en este
tejido.
Para la aplicación representada en las figuras
19 a 20 resulta especialmente ventajoso elegir para el material
licuable un material reabsorbible, mientras que la parte portante es
de un material no licuable ni reabsorbible, que presenta suficiente
resistencia mecánica para la fijación del diente, del puente o de
la prótesis. Para esto, por lo menos la superficie de la parte
central es bioactiva (p. ej., porosa como se ha descrito para la
vaina 13), lo que quiere decir que fomenta la soldadura con el
tejido óseo. Un implante de esta clase presenta estabilidad
primaria inmediatamente después de la implantación, que es
suficiente para la fijación y uso normal del diente, puente o
prótesis. Después el material reabsorbible es sustituido
sucesivamente por material vital y se va adhiriendo creciendo con
la parte central del implante, lo cual es estimulado por su
superficie bioactiva. El implante conforme a la invención ofrece por
lo tanto estabilidad primaria inmediata sin empleo de cemento y
después de la fase de reabsorción y regeneración una estabilidad
secundaria duradera, que no tiene nada que envidiar a la
estabilidad de los implantes conocidos. En comparación con los
procedimientos de implantación conocidos, desaparece sin embargo
una fase de transición en la que conforme al estado de la técnica
se cierra el orificio y se espera a la regeneración del tejido óseo,
antes de fijar directamente en el hueso regenerado el diente, el
puente o la prótesis.
La figura 23 muestra a título de ejemplo un
dispositivo de fijación externo fijado a un hueso tubular 50 de un
brazo humano mediante implantes conformes a la invención, con apoyos
52 y un soporte 53 fijado en los apoyos. Los apoyos 52 están
fijados como implantes conformes a la invención. La parte medial de
un hueso tubular presenta múltiples corticales y muy pocas zonas de
tejido poroso en el sentido de la invención. Por este motivo se
aprovecha, tal como está representado con mayor detalle en las
figuras 24 y 25, el conducto medular 54 en el interior del hueso
largo 50 para inyectar allí el material licuado. Pero como la médula
solamente está en condiciones de oponer a la presión hidrostática
una resistencia insuficiente, los apoyos están dotados, por
ejemplo, de placas base.
Para la fijación del dispositivo de fijación se
taladran por lo tanto en el hueso largo 50 unos orificios 56
(eventualmente dotados de rosca 25), que penetran hasta el conducto
medular 54, correspondiéndose el diámetro del orificio con el
diámetro del implante 7, en particular de su placa base 55. El
implante 7 presenta centrado un apoyo 52, en cuyo extremo distal va
fijada la placa base 55 y alrededor de la cual y recubriendo
esencialmente la placa base 55 está situada una zona anular o
tubular 57 del material licuable. El implante se introduce en el
orificio 56 y se sujeta a una profundidad predeterminada con medios
adecuados que se aplicarán por el exterior. A continuación se
empuja alrededor del apoyo 52 el material licuable 56 por el efecto
del ultrasonido contra la placa base 55, de manera que se inyecta
entre el hueso 50 y la placa base 55 en el conducto medular 54,
formando de este modo una unión de ajuste positivo que mantiene al
apoyo 52 en el orificio 56. Este anclaje permite la fijación
unicortical asegurada contra el vuelco de los apoyos 52, lo que
conforme al estado de la técnica solamente es posible por medio de
una fijación bicortical.
La figura 26 muestra otra forma de realización
del implante 7 conforme a la invención, que resulta especialmente
adecuado para la aplicación representada en la figura 23. En ese
caso, el material licuable, que es, por ejemplo, un elemento
tixotrópico, está situado en el interior del apoyo 52 y encima de la
placa del fondo 55 están previstos unos orificios 58, cuyas
dimensiones son tales que el cemento no pueda salir en su forma de
alta viscosidad, pero sí en su forma licuada obtenida por efecto del
resonador 6. El extremo del apoyo 52 está realizado por tanto en
este caso como vaina en el sentido de la figura 8. El cemento
expulsado con ayuda del resonador 6 a través de los orificios 58
asegura el apoyo dentro del conducto medular 54 y en el tejido óseo
eventualmente contiguo.
La figura 27 muestra un tornillo de tracción 60,
equipado como implante conforme a la invención, que se utiliza, por
ejemplo, junto con una placa de trocanter 61 para la fijación de un
hueso de tibia roto. El tornillo de tracción 60 es hueco (en el
sentido de una vaina de implante 13, figura 8) y presenta por lo
menos en su zona distal unos orificios a través de los cuales pueda
salir un material licuado, para poder anclar esta zona distal, por
ejemplo, en un tejido óseo osteoporótico, mejor de lo que sería
posible únicamente con la rosca del tornillo de tracción. En este
caso, la rosca del tornillo sirve especialmente para juntar el
tejido en la zona de la fractura, hasta que el extremo distal del
tornillo quede anclado en el tejido mediante el material
licuable.
La figura 28 muestra en una representación en
sección, muy esquemática, un hueso largo 50 al cual va fijado un
elemento de rótula artificial 62 mediante el implante 7 conforme a
la invención. La caña 63 del elemento de rótula 62 y el material
licuable dispuesto alrededor de la caña representan el implante
conforme a la invención 7, que por efecto del ultrasonido se
inyecta en el hueso largo 50, para lo cual el material 57 se licúa
y se inyecta a presión en los poros de la esponjosa 23 y en las
irregularidades de la superficie interior de la cortical 22. La
caña 63 presenta ventajosamente una estructura superficial adecuada
para obtener una unión con ajuste positivo con el material licuable
57, en el sentido de la placa 36 representada en la figura 17.
Una forma de realización especialmente
conveniente de la caña 63 es, por ejemplo, de titanio y presenta
una superficie porosa y por lo tanto bioactiva y va rodeada de un
material licuable, reabsorbible. Un implante de esta clase presenta
inmediatamente después de la implantación una estabilidad primaria
que permite por lo menos una carga parcial. La estabilidad primaria
queda después relevada por una estabilidad secundaria, condicionada
por la encarnadura del tejido óseo vital con la superficie porosa de
la caña de titanio 63. Dicho de otra manera, esto quiere decir que
el elemento de rótula artificial se puede cargar esencialmente
inmediatamente después de la implantación, también sin emplear
cemento, lo cual favorece la regeneración del tejido vital y
previene la degradación (osteoporosis). A pesar de ello, el tejido
vital se encarna en otra fase con la caña de titanio.
La figura 29 muestra también muy
esquemáticamente una rótula 70, en cuya zona una banda 71 une entre
sí los huesos 72 y 73. La banda 71 está encarnada con el hueso, pero
esta unión se puede romper en caso de sobrecarga. Para la
reparación se pueden aplicar implantes 7 conformes a la invención,
pudiendo utilizarse ventajosamente, por ejemplo, formas de
realización conforme a las figuras 2 a 4. Para ello se abre la
cortical del hueso de la rótula y a través de la banda 77 se pasan
implantes 7 en forma de espiga y se aseguran por el exterior
mediante una cabeza (p. ej., según la figura 13).
También cabe imaginar formas de realización con
menor efecto de profundidad según las figuras 16 y 17.
La figura 30 muestra que una unión que se puede
establecer con un implante 7 semejante pero no conforme a la
invención, no tiene por qué servir necesariamente para la unión
entre dos elementos (dos partes de tejido o una parte del tejido y
una parte artificial). También cabe imaginar el servirse de un
implante para rellenar un hueco del tejido 80, causado, por
ejemplo, por un tumor. Para ello se emplea conveniente un implante
7 de un material 81 tixotrópico y de alta viscosidad, que
eventualmente se introduce en el orificio 80 mediante una guía 82
posicionada alrededor del orificio, sobresaliendo de éste. El
resonador 6 empleado para esta aplicación se corresponde en su
sección con la sección interior de la vía 82, de manera que con él
se puede introducir a presión material 81 en el orificio 80, igual
que con un cilindro. Con ello no solamente se rellena el orificio
80, esencialmente sin huecos, sino que el material 81 que se va
licuando por efecto del ultrasonido, se inyecta también en los
poros del tejido que desembocan en el hueco 80 formando allí después
de solidificar una unión con ajuste positivo, tal como está
representado abajo en la figura 30. Esta unión de ajuste positivo
mantiene al implante 7 con seguridad en el hueco 80, sin que éste
tenga que presentar para ello destalonados y sin que tengan que
emplearse otros medios auxiliares (p. ej., periostio cosido
encima).
Al material licuable 81 se le puede añadir
también material óseo del paciente, preparado con la debida
finura.
Si en un caso tal como el representado en la
figura 30 se emplea en lugar de un cemento tixotrópico un material
termoplástico, el orificio 80 también puede ser un orificio
especialmente preparado al efecto y en su interior se puede fijar
mediante el plástico un elemento de fijación 83 para un alambre 84 o
un hilo, tal como está representado con línea de trazos y puntos en
la figura 30 (sólo abajo). También se puede fijar de esta manera un
dispositivo terapéutico auxiliar tal como, por ejemplo, un
estimulador.
Ejemplo
1
Espigas de PLLA y policarbonato fabricadas por
fundición inyectada, con una sección redonda de diámetros entre 3,5
y 4,25 mm, con longitudes de 26 a 40 mm (longitud ideal para 20 KHz:
35 mm) y con su extremo distal con punta roma, así como cuatro
ranuras que se extienden axialmente a lo largo de 10 mm a lo largo
del extremo distal, se anclaron en espongiosa (cabeza de fémur) de
reses recién sacrificadas, aplicando una frecuencia de excitación
de 20 KHz. Para ello se abrió la delgada capa cortical situada
encima de la espongiosa, pero no se pretaladró la espongiosa. Los
implantes se comprimieron contra el tejido con presiones de 60 a
130 N y se excitaron con la frecuencia de excitación (amplitud del
sonotrodo aprox. 20 a 25 \mum). El avance se limitó a 10 mm. En
todos los casos se alcanzó en menos de 2 s. A continuación se
mantuvieron los implantes durante 5 s sin excitación.
Se obtuvieron profundidades de anclaje del orden
de magnitud de 15 mm, que al arrancarlos eran más fuertes que los
implantes (fuerzas de arranque máximas superiores a 500 N). En
sensores que se colocaron en la cortical a 1 mm fuera del taladro
previo (1,5 mm debajo de la superficie cortical) se midieron unos
10 s después del inicio de la implantación temperaturas máximas de
44ºC (aprox. 22º por encima de la temperatura ambiente). Los
excesos de temperatura descendieron en unos 30 s a la mitad de su
valor.
Para el material PLLA no se comprobó ninguna
disminución de peso molecular en el material implantado en
comparación con el material del implante todavía sin implantar.
Claims (35)
1. Implante (7) para establecer una unión de
ajuste positivo con una pieza de tejido, para unir ésta con otra
pieza de tejido o con un elemento artificial que sustituya a otra
pieza de tejido o la apoye, o que sea un dispositivo terapéutico
auxiliar, consistiendo el implante (7) por lo menos parcialmente de
un material licuable mediante energía mecánica, estando situado el
material licuable en el implante (7) de tal manera que se pueda
poner en contacto con la parte del tejido, se pueda excitar mediante
vibraciones mecánicas y al mismo tiempo apretar contra la parte del
tejido, con el fin de licuar por lo menos una parte del material
licuable e inyectarlo en orificios de la parte del tejido.
2. Implante según la reivindicación 1, siendo
el material licuable termoplástico o tixótropo.
3. Implante según una de las reivindicaciones 1
ó 2, presentando el material licuable por lo menos un componente
reabsorbible.
4. Implante según una de las reivindicaciones 1
a 3, conteniendo el material licuable sustancias adicionales para
otras funciones.
5. Implante según una de las reivindicaciones 1
a 4, teniendo forma de espiga o de taco.
6. Implante según la reivindicación 5,
presentando en una zona próxima material licuable mediante energía
mecánica.
7. Implante según la reivindicación 5 ó 6,
presentando una zona próxima realizada en forma de cabeza.
8. Implante según la reivindicación 5, estando
situado el material licuable en la superficie del implante de un
extremo distal del implante y/o en la superficie envolvente del
implante, presentando el material licuable unos escalones que
funcionan como direccionadores de energía.
9. Implante según la reivindicación 5, estando
situado el material licuable en la superficie del implante de un
extremo distal del implante y/o en la superficie envolvente del
implante, presentando un núcleo (11) de un material no
licuable.
10. Implante según la reivindicación 5,
presentando una vaina (13) de un material no licuable, estando
situado el material licuable en el interior de la vaina y
presentando la vaina unos orificios (58) para la salida del
material licuado.
11. Implante según la reivindicación 9 ó 10,
siendo el material licuable absorbible y presentando el núcleo (11)
o la vaina (13) al menos parcialmente una superficie que estimula la
encarnadura con la parte del tejido.
12. Implante según una de las reivindicaciones 4
a 11, presentando medios de soporte (14, 15) en su extremo
próximo.
13. Implante según una de las reivindicaciones 1
a 4, teniendo la forma de una placa (35 y 36) o lámina (35).
14. Implante según la reivindicación 13, siendo
la placa o lámina (35) de una sola capa.
15. Implante según la reivindicación 13,
presentando la placa (35 ó 36) o lámina una capa del material
licuable y una capa de un material no licuable, cubriendo el
material licuable por lo menos una parte de una de las superficies
del material no licuable, estando unidas las dos capas entre sí con
ajuste positivo o ajuste de material.
16. Implante según una de las reivindicaciones 1
a 15, donde éste es un medio de anclaje para un elemento artificial
de apoyo o fijación, para un elemento de rótula artificial, para un
diente artificial, para un puente, para una prótesis dental, para
un hilo de sutura, para un alambre de cerclage o para un dispositivo
auxiliar terapéutico.
17. Dispositivo de implantación (1) para
implantes (7) según una de las reivindicaciones 1 a 16, con un
generador (2) un elemento oscilante y un resonador (6), estando
equipado el generador (2) para excitar al elemento vibratorio con
oscilaciones mecánicas, estando el resonador (6) unido activamente
con el elemento oscilante formando una unidad oscilante (3),
pudiendo unirse activamente el resonador (6) con el implante (7)
para la transmisión de las oscilaciones mecánicas y estando equipado
para presionar el implante (7) contra una parte del tejido,
caracterizado porque el dispositivo presenta adicionalmente
una guía de implante que va apoyada en una carcasa (5).
18. Dispositivo de implantación según la
reivindicación 17, caracterizado porque el dispositivo está
preparado para oscilaciones mecánicas del oscilador (6) con una
frecuencia de 2 a 200 KHz.
19. Dispositivo de implantación según una de las
reivindicaciones 17 a 18, caracterizado porque se pueden
ajustar diferentes frecuencias de las oscilaciones mecánicas,
diferentes modelos de frecuencia y/o diferentes pulsaciones de
energía.
20. Dispositivo de implantación según una de las
reivindicaciones 17 a 19, caracterizado porque el resonador
(6) o una parte distal del resonador (6) es sustituible y
esterilizable.
21. Dispositivo de implantación según una de las
reivindicaciones 17 a 20, caracterizado porque el resonador
(6) está equipado para sujetar un implante (7) en forma de espiga o
de taco.
22. Kit para realizar uniones con partes de
tejido, cuyo kit lleva un dispositivo de implantación (1) con un
generador (2), un elemento oscilante y un resonador (6), estando
equipado el generador (2) para excitar al elemento oscilante para
efectuar oscilaciones mecánicas, pudiendo unirse el resonador (6)
activamente con el implante (7) para la transmisión de las
oscilaciones mecánicas y estando equipado para presionar el implante
(7) contra una parte del tejido, presentando una multitud de
implantes (7) según una de las reivindicaciones 1 a 13.
23. Kit según la reivindicación 22,
caracterizado porque el kit lleva una multitud de implantes
en forma de espiga y por lo menos una placa (21) con orificios
adaptados a los implantes (7) en forma de espiga.
24. Kit según la reivindicación 23,
caracterizado porque la placa (21) es metálica, porque los
implantes (7) en forma de espiga presentan en una zona próxima
material licuable mediante energía mecánica para la unión con la
placa (21) y porque los orificios están equipados para un ajuste
positivo con el material licuable.
25. Kit según la reivindicación 24,
caracterizado porque los orificios llevan una rosca
interior.
26. Kit según la reivindicación 23,
caracterizado porque la placa (21) y/o un extremo próximo del
implante lleva material licuable, de tal manera que la placa (21) y
el extremo próximo del implante se pueden soldar o pegar entre sí
mediante oscilaciones mecánicas.
27. Kit según la reivindicación 22,
caracterizado porque el resonador (6) del dispositivo de
implantación (1) presenta en su extremo distal unos medios de
sujeción y porque los implantes (7) tienen forma de espiga o de
taco y su extremo próximo presenta medios de sujeción adaptados a
los medios de sujeción del resonador (6).
28. Kit según la reivindicación 27,
caracterizado porque el medio de sujeción del implante y el
medio de sujeción del resonador están diseñados para una sujeción
con ajuste por fricción.
29. Kit según una de las reivindicaciones 22 a
28, caracterizado por llevar diversos implantes en forma de
espiga o de taco así como resonadores (6) o partes distales del
resonador, cuyas superficies distales se corresponden con las
secciones próximas de los implantes.
30. Kit según una de las reivindicaciones 22 a
29, caracterizado porque por lo menos una parte de los
implantes (7) tiene forma de espiga y porque el kit incluye además
brocas cuyo diámetro está adaptado al diámetro mayor de los
implantes (7) para un ajuste por fricción.
31. Kit según una de las reivindicaciones 22 a
30, caracterizado porque por lo menos una parte de los
implantes (7) llevan el material licuable en una vaina (13) con
orificios (58) y porque el kit lleva además por lo menos un
resonador (6) o una parte distal de resonador, adaptada en cuanto a
su sección a la sección interior próxima de la vaina (13).
32. Kit según una de las reivindicaciones 22 a
31, caracterizado porque el kit lleva además guías (82) con
un orificio y por lo menos un resonador (6) o parte distal de
resonador, adaptado en cuanto a su sección a este orificio.
33. Kit según una de las reivindicaciones 22 a
32, caracterizado por contener además envueltas estériles
para el dispositivo de implantación (1).
34. Kit según una de las reivindicaciones 22 a
30, caracterizado porque se pueden ajustar diferentes
frecuencias de oscilación, diferentes modelos de frecuencias y/o
diferentes pulsaciones de energía en el dispositivo de implantación
(1), y porque el kit lleva además unas instrucciones de ajuste para
los diferentes implantes.
35. Kit adicional con uno o varios implantes (7)
según una de las reivindicaciones 1 a 16, cuyo kit adicional lleva
además unas instrucciones para la implantación de los implantes
sirviéndose de energía de oscilación mecánica.
