Description
本發明係有關於一種人工腦膜醫療裝置。
操作腦神經外科手術時,須將頭骨切開並且破壞腦膜才可進行,手術結束後,外科醫師會使用腦膜替代品修復受損之腦膜。在臨床上,已有多種自然或合成的腦膜替代物,例如自體移植物(autografts)、異體移植物(allografts)、異種移植物(xenografts)以及不可吸收以及可吸收的高分子(absorbable or unabsorbable polymers)等。而這些腦膜替代物最主要的功能包括避免引發感染及不良的組織反應、有效防止腦脊液滲漏、促進硬腦膜組織再生、防止腦組織沾黏。
腦膜修復方法包括需縫合方法以及免縫合方法,自體移植物(autografts)、異體移植物(allografts)、異種移植物(xenografts)以及不可吸收的高分子(例如Neuro-Patch)屬於傳統需縫合的方法。在臨床上常遇到的缺點包括手術時間長以及操作便利性差,以至於在操作上增加了風險,為解決腦膜修補的臨床需求,因此目前已有不需縫合的簡易操作,例如DuraGen(Integra),以提供較短的操作時間以及較佳的操作便利性,因此可降低手術風險,故免縫合方法使用率較高。
最常使用的人工腦膜為以膠原(collagen)為材料的DuraGen,手術之步驟僅需將DuraGen剪裁成較受損腦膜
為大的面積,並且將DuraGen人工腦膜覆蓋於受損腦膜上,即完成手術,然而,免縫合方法的固定性差,因此容易造成人工腦膜之位移且腦脊髓液容易滲漏。
本發明提供一種人工腦膜醫療裝置,人工腦膜醫療裝置用來與由患者之整個頭骨上裁切下來之部份頭骨,包括人工腦膜以及連接元件,連接元件與具有人工腦膜之部份頭骨固定其中,該部份頭骨包括側表面以及內表面,且該人工腦膜覆蓋該部份頭骨之整個該內表面,當該部份頭骨以及該人工腦膜固定於該整個頭骨之上時,該部份頭骨之該內表面朝向於該患者的腦。
應注意的是,連接元件於內表面將人工腦膜固定。

請參閱第1、2圖,操作腦神經外科手術時,醫生由整個頭骨10上裁切部份頭骨11,而整個頭骨10底下之腦膜同時遭到破壞,並且,於整個頭骨10上形成一裁切孔12。在醫生修復腦神經後,受損的腦膜也必須被修復,接著,部份頭骨11才能覆蓋於裁切孔12上,因此,本發明提供一種人工腦膜醫療裝置。人工腦膜醫療裝置20包括人工腦膜21以及連接元件22a,連接元件22a與具有人工腦膜21之部分頭骨11固定,部份頭骨11包括外表面111、內表面以及側表面112,請參閱第2圖,本實施例之人工腦膜21覆蓋部份頭骨11之內表面、側表面112以及部分之外表面111,且連接元件22a將人工腦膜21固定於側表面112上,於本實施例中,連接元件22a為綑綁件,以綑綁人工腦膜21以及部份頭骨11。連接元件22a更可為螺絲、鉤體、鉚釘、膠體或固定釘,但不限於以上揭露之實施態樣。
請參閱第3、4圖,第3、4圖之結構大致同於第2圖之實施例,相同元件省略不再贅述。而主要差異處在於連接元件22a於外表面111固定人工腦膜21,在第3、4圖之實施例中,連接元件22a更可為螺絲、鉤體、鉚釘、膠體或固定釘,但不限於以上揭露之實施態樣。
請參閱第5圖,第5圖之結構大致同於第3圖之實施例,相同元件省略不再贅述。而主要差異處在於人工腦膜醫療裝置20更包括襯墊23,襯墊23設於連接元件22a以及人工腦膜21之間以增加摩擦力。

請參閱第7圖,人工腦膜醫療裝置20包括人工腦膜21以及連接元件22a,部份頭骨11更包括複數孔洞113,使醫生更為輕易地由整個頭骨10上取下部份頭骨11(如第1圖所示)。人工腦膜21包括四個延伸部212,且人工腦膜21覆蓋部份頭骨11並且藉由連接元件22a以及延伸部212將人工腦膜21固定於側表面112上。於本實施例中,連接元件22a為螺絲,而連接元件22a更可為膠體(未圖示),但不限於以上揭露之實施態樣。

第14圖為使用本發明之人工腦膜醫療裝置之腦部手術方法流程圖之一實施例。請搭配參閱第1、2、14圖,其步驟包括:a.由整個頭骨10上裁切部份頭骨11;b.將人工腦膜21與部份頭骨11固定;c.將與人工腦膜21固定之部份頭骨11固定於整個頭骨10上之裁切孔12中。應注意的是,人工腦膜21預先被裁切成適當的尺寸以及厚度,在步驟b.中,人工腦膜21藉由連接元件22a、22b、22c、22e而與部份頭骨11固定(第2-8、10、11、13圖)。或者,人工腦膜21藉由由液態人工腦膜22d製成之凸出部211a嵌合溝槽114,達成與部份頭骨11之固定(請參閱第12圖)。
第15圖為使用本發明之人工腦膜醫療裝置之腦部手術方法流程圖之另一實施例。請搭配參閱第1、9、15圖,其步驟包括:a1.由整個頭骨10上裁切部份頭骨11;b1.在部份頭骨11上鑽設一開孔113;c1.使人工腦膜21穿過開孔113以覆蓋部份頭骨11;d1.藉由一固定元件22使人工腦膜21與部份頭骨11固定;e1.將與人工腦膜21固定之部份頭
骨11固定於整個頭骨10上之裁切孔12中。
綜上所述,本發明可避免將人工腦膜21在腦部組織上與缺損之腦膜直接縫合,可降低腦部手術之風險以及減少手術時間,再者,固定於部份頭骨11上之人工腦膜21可藉由與整個頭骨10嵌合之方式,穩定地覆蓋缺損之腦膜,以避免腦脊髓液滲漏。換言之,在本發明之實施例中,該人工腦膜與缺損之腦膜組織在空間上不共面,且該人工腦膜穩定地覆蓋缺損之腦膜。
惟以上所述者,僅為本發明之較佳實施例而已,當不能以此限定本發明實施之範圍,即大凡依本發明申請專利範圍及發明說明內容所作之簡單的等效變化與修飾,皆仍屬本發明專利涵蓋之範圍內。另外本發明的任一實施例或申請專利範圍不須達成本發明所揭露之全部目的或優點或特點。此外,摘要部分和標題僅是用來輔助專利文件搜尋之用,並非用來限制本發明之權利範圍。

Claims (13)
- 一種人工腦膜醫療裝置,用於修補因為從一患者之一整個頭骨上裁切下來一部份頭骨而受損的腦膜組織,該部份頭骨被取下後,一裁切孔形成於該整個頭骨,包括:一人工腦膜;以及一連接元件,將該人工腦膜固定於該部份頭骨,其中,該部份頭骨包括一側表面以及一內表面,且該人工腦膜覆蓋該部份頭骨之整個該內表面,其中,當該部份頭骨以及該人工腦膜置入該裁切孔並固定於該整個頭骨之上時,該部份頭骨之該內表面朝向於該患者的腦,該人工腦膜與該受損的腦膜組織不共面,且該人工腦膜覆蓋該受損的腦膜組織,藉此,該受損的腦膜組織被該人工腦膜所修復,而毋須將該人工腦膜縫合於該受損的腦膜組織。
- 如申請專利範圍第1項所述之人工腦膜醫療裝置,其中,該連接元件於該側表面將該人工腦膜固定。
- 如申請專利範圍第2項所述之人工腦膜醫療裝置,其中該連接元件包括一綑綁件、一螺絲、一鉤體、一膠體或一固定釘。
- 如申請專利範圍第1項所述之人工腦膜醫療裝置,其中,該連接元件於該外表面將該人工腦膜固定。
- 如申請專利範圍第4項所述之人工腦膜醫療裝置,其中該連接元件包括一綑綁件、一螺絲、一鉤體、一膠體或一固定釘。
- 如申請專利範圍第4項所述之人工腦膜醫療裝置,其中該部份頭骨包括複數孔洞,該人工腦膜覆蓋該部份頭骨之該內表面,該連接元件通過該等孔洞以於該外表面將該人工腦膜固定。
- 如申請專利範圍第6項所述之人工腦膜醫療裝置,其中該連接元件包括一縫合線。
- 如申請專利範圍第1項所述之人工腦膜醫療裝置,其中該連接元件設置於該部份頭骨以及該人工腦膜之間。
- 如申請專利範圍第8項所述之人工腦膜醫療裝置,其中該連接元件包括二勾持部,分別插入該部份頭骨以及該人工腦膜。
- 如申請專利範圍第8項所述之人工腦膜醫療裝置,其中該連接元件包括一基部、一第一嵌合部以及一第二嵌合部,該第一嵌合部以及該第二嵌合部設於該基部之二對應表面,該第一嵌合部被插入該部份頭骨中,且該第二嵌合部被插入該人工腦膜中。
- 如申請專利範圍第8項所述之人工腦膜醫療裝置,其中該連接元件包括一膠體。
- 如申請專利範圍第1項所述之人工腦膜醫療裝置,其中該連接元件包括複數凸出部以及複數溝槽,該等凸出部設於該人工腦膜上,且該等溝槽設於該部份頭骨上,該等凸出部與該等溝槽嵌合。
- 如申請專利範圍第1項所述之人工腦膜醫療裝置,其更包括複數襯墊以及一環體,其中該等襯墊設於該環體 上,該環體固定該人工腦膜以及該部份頭骨,該等襯墊設於該連接元件以及該人工腦膜之間以增加摩擦力。
