Description
Die vorliegende Erfindung betrifft die Anwendung von Octreotid zur Behandlung
von Brustkrebs.
Somatostatin ist ein Tetradecapeptid mit folgender Struktur
Somatostatin besitzt GH-, Magen-, Insulin- und Glucagon- hemmende Eigen
schaften. Jedoch hat es eine kurze Wirkungszeit. Um dieses Problem zu
lösen, wurden langwirkende Somatostatinanalogon hergestellt.
Eine ganz bevorzugte Verbindung ist die Verbindung IVa
auch Octreotid genannt.
Octreotid kann in freier Form oder in
einer pharmazeutisch akzeptablen Salz- bzw. Komplexform vorliegen. Als
Säureadditionssalze kommen solche mit organischen Säuren, polymeren
Säuren oder anorganischen Säuren in Frage. Als Beispiele für Säureaddi
tionssalze seien die Hydrochloride und Acetate genannt. Unter Komplexen
sind solche an sich bekannten Verbindungen zu verstehen, die beim Zusatz
anorganischer Substanzen, wie Salze oder Hydroxide (z. B. Ca-, Zn-Salze)
und/oder beim Zusatz polymerer organischer Stoffe entstehen.
Octreotid kann nach bekannten Methoden
hergestellt werden. Sie kann zweckmässig als Polyacetat-Polyhydrate
eingesetzt werden. Typische Peptidkonzentrationen liegen in dem Bereich
85 bis 95%.
Brustkrebs ist die häufigste Krebskrankheit bei der Frau über 40
und eine Haupttodesursache. Die Erfindung kann von Bedeutung sein sowohl
bei hormonabhängigen, z. B. oestrogenabhängigen, als auch hormonunab
hängigen Tumoren. Brustkrebs ist eine Krankheit, für welche noch viele
Bemühungen benötigt werden, um jede Art von Erleichterung zu finden.
Es wurde gefunden, dass Octreotid
eine Wirkung auf Patientinnen mit Brustkrebst hat; beispielsweise
stoppt diese Verbindung den Krankheitsfortschritt wie z. B. durch eine
längere und breitere Antwortswirkung gezeigt werden kann.
Die Wirkung von Octreotid, wie oben definiert, auf Pati
entinnen mit Brustkrebs, kann in folgenden klinischen Studien gezeigt
werden:
In einem ersten klinischen Versuch wurden 5 Patientinnen mit Brust
karzinom behandelt. Diese Patientinnen hatten keine vorherige systemische
Behandlung der Metastasis (Adjuvansbehandlung wird nicht berücksichtigt)
und wiesen einen leichten Zugang zu den Venen auf. Die Patientinnen haben
PS 0 oder 1 und können nachklimakterisch sein.
Octreotid kann in kontinuierlicher
Weise parenteral verabreicht werden, beispielsweise mit Hilfe einer Pumpe
in einer Menge von beispielsweise 0.5 bis 2 mg pro 24 Stunden, mindestens
3 Tage lang.
Der IGF-Wachsumsfaktor wurde bestimmt und es wurde eine Herab
setzung der Ausscheidung gefunden.
Eine zweite kontrollierte Studie kann wie folgt durchgeführt werden:
In einer zweiten Prüfung wurden mindestens 14 Patientinnen mit
Brustkrebs mit Octreotid behandelt und die
Intensität und die Dauer der Wirkung bestimmt.
In diese Prüfung werden Patientinnen mit durch histologische Biopsie
festgestelltem Brustkrebs (Drüsenanalysis-EOA) einbezogen. Ihre Krankheit
hat die metastatische Phase mit oder ohne messbare und bewertbare örtliche
Beschränkung erreicht. Gewünschtenfalls werden Patientinnen einbezogen,
welche anderen konventionellen Therapien wie chirurgische Behandlung,
Strahlentherapie, andere Chemotherapie und/oder Hormontherapie widerstehen.
Die Patientinnen weisen mindestens ein Röntgenschussziel auf, welches
messbar und bewertbar wie ein kutaner oder sub-kutaner primitiver metasta
tischer Tumor ist. Er kann gangliär oder viszeral sein. Vorzugsweise werden
Patientinnen einbezogen, deren Läsionen sich im letzten Monat vor der
klinischen Prüfung weiterentwickelt und welche eine Ueberlebenschance von
mindestens 3 Monaten haben.
Folgende Patientinnen werden vorzugsweise ausgeschlossen:
- - Patientinnen, bei welchen das einzige Diagnosekriterium für Brustkrebs auf biologischen Modifikationen beruht
- - Patientinnen mit einer embryonalen Karzinomantigenpathologie
- - Patientinnen mit Ascites, Pleuraerguss, Lungenkarzinomlymphangi tis, oder Knochenbestimmung als einziges metastatisches Symptom,
- - Patientinnen, welche für ein einziges Schussziel mit Strahlen weniger als acht Wochen vor der Aufnahme in die Studie behandelt wurden; jedoch können diese Patientinnen einbezogen werden, wenn eine Weiterentwicklung während dieser Zeit bewiesen werden kann,
- - Patientinnen mit einer einzigen Hirnbestimmung
- - Patientinnen, welche einen anderen bösartigen Tumor, mit Ausnahme eines Karzinoms in situ im Cervix uteri oder eines Spino- oder basalen Hautzellenkrebses, aufweisen, und
- - Patientinnen, welche nicht regelmässig zur Konsultation kommen können.
Durch Ausschliessen dieser Patientinnen kann die Wirksamkeit der
Verbindung deutlicher gezeigt werden. Trotzdem können die zu dieser
Ausschlusskategorie gehörenden Patientinnen auch mit Octreotid
behandelt werden.
Octreotid kann in einer gleichen oder
in kleinerer Menge als oben in der ersten klinischen Prüfung angegeben,
vorzugsweise in zwei Dosierungen, eine morgens und die zweite abends,
verabreicht werden. Die Behandlung dauert mindestens 3 Monate oder bis
zur vollkommenen Remission. Die Wirkung kann nach herkömmlichen Methoden
verfolgt werden, beispielsweise nach den IUCC-Kriterien, wie zum Bei
spiel Weiterentwicklung, Stabilisierung, teilweise oder vollständige
Remission. Die Bewertung wird an den Tagen 0, 15, 45, 60 und 90 durch
geführt.
Eine dritte klinische Studie kann wie folgt durchgeführt werden:
Patientinnen mit Brustkrebs im fortgeschrittenen Stadium werden
behandelt. Zusätzlich wird ihr Brustkrebs, durch Autoradiographie von
benachbarten Gewebsschnitten, mit einem Radioligand, z. B. 125I-[Leu8,
D-Trp22, Tyr25]-Somatostatin-28 oder 125I-Tyr3-Analogon der Verbindung
IVa, auf den Somatostatinrezeptorengehalt untersucht. Die Patientinnen
leiden unter fortgeschrittener Krankheit mit Parametern, welche nach
IUCC-Kriterien (z. B. Erscheinung von neuen Läsionen, Entwicklung der
bestehenden metastatischen Läsionen) messbar und bewertbar sind und auf
primäre hormonale und/oder zytotoxische Therapie nicht reagieren. Wie
schon für die 2. klinische Studie angegeben, werden auch hier für diese
Prüfung Patientinnen ausgeschlossen, welche eine frühere oder gleich
zeitige Malignität an anderen Stellen besitzen, mit Ausnahme von durch
Konusbiopsie festgestelltem Karzinom in situ am Cervix uteri und ent
sprechend behandeltem basalem oder squamösem Hautzellenkrebs.
Octreotid kann in einer gleichen oder
in einer kleineren Menge als in der 2. Studie, verabreicht werden. Vor-
wird Octreotid parenteral verab
reicht, bespielsweise sub-kutan, besonders in kontinuierlicher Weise mit
einer tragbaren Spritzpumpe (Infusionspumpe). Die Behandlung dauert
mindestens 2 Monate oder bis zur vollkommenen Remission. Die Wirkung
kann nach bekannten Methoden verfolgt werden, beispielsweise gemäss den
IUCC-Kriterien. Die Bewertung wird an den Tagen 0, 30 und 60 durchge
führt. Alle Läsionen werden bei jeder Untersuchung gemessen und falls
Multipleläsionen vorhanden sind, wird eine Anzahl von 5 repräsentativen
Läsionen für die Bestimmung ausgewählt. Wenn die Produktsumme der Dia
meter von jeder einzelnen Läsion oder von den 5 augewählten Läsionen um
50% oder mehr abnimmt, wird dieser Zustand als rückbildend bezeichnet.
Octreotid wird z. B. parenteral, beispielsweise sub-kutan verab
reicht. Die Dosis kann im selben Verhältnis stehen wie für die Behand
lung von gastro-pankreatischen endokrinen Tumoren (GI) wie Vipomas, oder
von Acromegalie, bis zu 10-mal diese Menge. Bevorzugter Bereich ent
spricht ca. 4 bis 10-mal der für die Behandlung von GI-Tumoren oder
Acromegalie eingesetzten Menge.
Für Octreotid, werden die GI-Tumore am
Anfang mit einer Dosis von 0.05 mg einmal oder 2-mal täglich durch sub-
kutane Injektionen behandelt. Die Dosis kann dann bis 0.2 mg 3-mal täg
lich erhöht werden. Für die Behandlung von Acromegalie kann täglich eine
Dosis von 100 bis 300 µg s. c. verabreicht werden. Octreotid ist bis
mindestens 1 mg verträglich.
Geeignete Tagesdosen für Octreotid in der erfindungsgemässen Be
handlung liegen im Bereich von 0.1 bis 1 mg s. c., vorzugsweise 0.2
bis 1 mg s. c.. Octreotid wird vorzugsweise parenteral in Form eines
Präparates mit Milchsäure, wie in der Deutschen Patentanmeldung
P 38 22 557.3 definiert, verabreicht.
In Uebereinstimmung mit dem Vorstehenden sieht die vorliegende
Erfindung ferner Folgendes vor:
- a) die Anwendung von Octreotid in freier Form oder in einer pharmazeutisch akzeptablen Salz- bzw. Komplexform, zur Behandlung des Brustkrebs, beispielsweise von hormonabhängigen oder -unabhängigen Brustkarzinomen und/oder Brusttumoren mit Somatostatinrezeptoren; und/oder
- b) die Anwendung von Octreotid in freier Form oder in einer pharmazeutisch akzeptablen Salz- bzw. Komplexform, zur Herstellung eines Arzneimittels für die Brustkrebsbehandlung, besonders eines parenteralen Präparates, beispielsweise ein Präparat für sub-kutane Injektion.
Präparate, welche Octreotid und Milchsäure enthalten, sind besonders für die
Behandlung von Brustkrebs geeignet, besonders für eine kontinuierliche
Infusion. Die Verabreichung kann kontinuierlich während 24 Stunden, mit
guter, lokaler Verträglichkeit für die Patientinnen, durchgeführt wer
den.
Claims (2)
1. Anwendung von Octreotid in freier Form oder in pharmazeutisch akzeptablen Salz-
bzw. Komplexform zur Behandlung von Brustkrebs.
2. Anwendung gemäss Anspruch 1 für die Behandlung von Brusttumoren mit
Somatostatinrezeptoren.
