Description
本出願は、それぞれ、引用によって本明細書に組み込まれる、2009年8月13日に出願された米国特許仮出願第61/233794号および2009年8月13日に出願された米国特許仮出願第61/233804号の利益を主張するものである。
遠距離と近距離の両方ではっきりと見ることができるようにするための多焦点IOLおよび調節IOLを含む、固定焦点IOLに代わるいくつかのIOLが開発されている。多焦点IOLは、遠距離と近距離の両方で良好な視力を実現するが、これらのレンズは通常、中間距離では性能が不十分であり、焦点のずれた光が存在することに関連するグレア、ハロ、および夜間視力障害を伴う。いくつかの構成の調節IOLも開発されているが、天然水晶体の機能を模倣できている調節IOLはない。孔を有するIOLがVorosmarthyの特許(米国特許第4976732号)に記載されている。しかし、このようなデバイスは、焦点を遠距離から近距離に変更するものではなく、焦点ずれによる像のぼやけを老眼の正視者が文字を読めるレベルに低下させようとするものに過ぎない。特に、Vorosmarthyは、マスクを有するIOLを薄くする技術を小切開手術に応用する問題に対処していない。
インプラント本体2014上またはインプラント本体2014内に位置させることのできるマスクの様々な変形実施形態は、本明細書で論じられ、かつ引用によって全体が本明細書に組み込まれる米国特許第7628810号、米国特許出願第2006/0113054号、および米国特許出願第2006/0265058号に記載されている。図11Aは、マスク2034aの一実施形態を示している。マスク2034aは、マスク2034a上の実質的に中央に配置されたピンホール開口部または孔2038aを囲む環状領域2036aを含むことができる。ピンホール孔2038aは全体として、本明細書ではマスク2034aの光軸と呼ぶ中央軸2039aの周りに配置されてよい。ピンホール孔2038aは円形であってよい。図11Bは、図11Aに示されているマスク2034aと同様のマスク2034bの他の実施形態を示している。図11Aのマスク2034aの環状領域2036aは、環状領域2036aの外周から内周に湾曲しており、一方、図11Bのマスク2034bの環状領域2036bは実質的に平坦である。
このマスクは、非常に高い表面積対体積比を有し、移植後に大量の日光にさらされるため、紫外線(UV)または他の光波長への曝露を含む、劣化に対する耐性が高い材料を含むことが好ましい。UV放射による劣化に対する耐性を示す、本明細書に開示されたマスクを形成する際に、UV吸収添加剤を含む、またはUV吸収モノマー(コモノマーを含む)で作られたポリマーを含む、UV吸収成分を含むポリマーを使用してよい。このようなポリマーの例には、開示が引用によって本明細書に全体的に組み込まれる、米国特許第4985559号および米国特許第4528311号に記載されたポリマーが含まれるがそれらに限らない。好ましい実施形態では、マスクは、UV放射に対する耐性を示す材料を含む。一実施形態では、マスクは、UV放射を実質的に反射または透過するポリマー材料を含む。UV放射による劣化に対する耐性を示すマスクの他に、レンズ本体がUV吸収成分を含むことができ、あるいはレンズ本体中のUV吸収成分がUV放射による劣化を防止することができるため、マスクはUV放射による劣化に対する耐性を示さなくてよい。
面を冷却してもよい。ステップ3600で、カミソリの刃を使用して、レーザ加工したマスクをシリコンディスクから取り外す。任意のステップとして、レーザ加工したマスクを二次成形型に入れることを含めてもよい。この型は、平押し金型、凸状型(convex mold)、凹状型(concave mold)、またはより複雑な形状を有する型のような所望の任意の形状であってよい。ある技術では、マスクを二次成形型の下半部に入れることができる。二次成形型の上半部をマスク上に配置し、型を約160℃の炉に入れる。次に、型を加熱し焼成させてマスクを形成する。型を約160℃で約2時間にわたって焼成させる。2時間後、炉温度を約30℃に低下させ、マスクを約2時間にわたって、あるいは炉温度が約40℃よりも低くなるまで焼成させる。
前述のマスクの多くは、患者の焦点深度を改善するのに使用することができる。様々な追加的なマスク実施形態について以下に論じる。後述の実施形態のいくつかは、環状領域によって遮断される光の量を変化させる、マスクの環状領域の内部に光透過穴を含む。マスクの内部の光透過穴は、患者の弱光視力を改善することができる。光透過穴のある実施形態では、光透過穴は、本明細書で説明するマスクの視力改善効果に干渉する回折パターンを生成することができる。したがって、本明細書では、回折パターンを生成せず、また他の点でもマスク実施形態の視力改善効果に干渉しないある種のマスクについて説明する。
マスクは、上述の特徴を含む構成を含む様々な他の構成を有してよい。たとえば、光透過穴の密度(たとえば、マスクの面積当たりの穴の面積)は、マスクのそれぞれの領域で異なってよい。ある実施形態では、穴の密度がマスクの内周から外周に半径方向外側に高くなっていく。ある実施形態では、穴の密度が半径方向外側にマスクの内周から外周に低くなっていく。他の変形例も考えられる。たとえば、マスク4000の中央環状領域は、図24Aに示されているように、内側環状領域および外側環状領域よりも高い密度の穴を有してよい。他の例では、マスクの中央環状領域は、内側環状領域および外側環状領域よりも低い密度の穴を有してよい。穴の密度は、マスクの、穴を有する表面積の割合である。穴の密度は、たとえば、比較的大きな面積を有する比較的少数の穴または比較的小さな面積を有する比較的多数の穴によって形成することができる。上述のように、各穴は、可視回折パターンを低減させるように配置されてよい。
眼内インプラント(または眼内レンズ)はいくつかの異なる方法で製造することができる。ある実施形態では、ロッドの長さに沿った光学的に透明の内側領域と、ロッドの長さに沿った光学的に透明の外側領域と、内側領域と外側領域との間のロッドの長さに沿った実質的に光学的に不透明な中央領域とを含むロッドを形成してよい。ロッドの長さに平行な軸に実質的に垂直な平面に沿った断面部分を切開して、内部にマスクを含むインプラント本体(たとえば、レンズ本体)を形成することができる。ある実施形態では、光学的に透明なロッドを形成することによってロッドを形成することができる。光学的に透明なロッドの周りに不透明なシリンダを形成してもよい。その場合、不透明なシリンダの周りに光学的に透明なシリンダを形成してよい。ある実施形態では、各シリンダは、鋳造または成形によって形成される。
前房眼内レンズは一般に、比較的硬い熱可塑性物質であるポリメチルメタクリレート(PMMA)で作られている。前房内のインプラントの安定性を維持するにはある程度の剛性が必要であると考えられていた。たとえば、眼内レンズの所望の安定性を実現するためにハプティックに補強部材を付加してもよい(たとえば、米国特許第6228115号(Hoffmannら)を参照されたい)。しかし、PMMA眼内レンズの圧縮力は安定性に必要な圧縮力よりもかなり強い。シリコーン、ハイドロゲル、およびソフトアクリルのような軟質材料で眼内レンズを構成することも可能である。これらのより柔らかい材料を用いた場合、前房内のインプラントの安定性にはいくらか疑問があるが、ある圧縮力および接触領域を使用すると軟質材料で作られた眼内インプラントは安定する。
患者の眼の焦点深度を増大することなどによって、患者の視力を改善する眼内インプラントは、様々な種類の構造を含むことができる。図49A〜49Cは、インプラント本体6002を含む眼内インプラント6000の実施形態を示している。インプラント本体6002は、マスク6006と、マスク6006に囲まれた孔6008と、マスク6006の周りの外側穴領域6010とを含むことができる。外側穴領域6010は、その周りにインプラント本体6002の外側部分6012を有してよい。

Claims (51)
- 本体材料を備えるインプラント本体と、
前記インプラント本体内に位置する孔を有するマスクとを備え、前記マスクは、前記マスクの後面と前面の間を延びる複数の穴を備え、
前記本体材料は、前記マスクの複数の穴を貫通して延び、さらに、前記複数の穴は、可視回折パターンを生成する前記穴の傾向を低減させるように前記複数の穴の穴サイズ、形状、向き、および間隔のうちの少なくとも1つが変更されることを特徴とする眼内インプラント。 - 前記複数の穴は不規則な位置に位置する、請求項1に記載の眼内インプラント。
- 複数の前記穴は、第1の穴サイズ、形状、または間隔を有し、少なくとも他の複数の穴は、前記第1の穴サイズ、形状、または間隔と異なる第2の穴サイズ、形状、または間隔を有する、請求項1に記載の眼内インプラント。
- 第1の複数の前記穴は、第1の穴サイズを有し、第2の複数の前記穴は、前記第1の穴サイズと異なる第2の穴サイズを有し、第3の複数の穴は、前記第1の穴サイズおよび前記第2の穴サイズと異なる第3の穴サイズを有する、請求項1に記載の眼内インプラント。
- 前記孔は、直径が約0.85mmから約1.8mmである、請求項1に記載の眼内インプラント。
- 前記マスクは、外径が約3mmから約5mmである、請求項1に記載の眼内インプラント。
- 前記マスクは患者の焦点深度を増大する、請求項1に記載の眼内インプラント。
- 前記前面は実質的に平坦であり、かつ前記後面は実質的に平坦である、請求項1に記載の眼内インプラント。
- 前記眼内インプラントが眼の内部に移植された後で前記眼内インプラントを支持するように前記インプラント本体に取り付けられた少なくとも1つのハプティックをさらに備える、請求項1に記載の眼内デバイス。
- レンズ本体の光軸に沿って前記レンズ本体の厚さを薄くするように構成された移行ゾーンを含む表面を備えるレンズ本体と、
前記移行ゾーンを通過する光によって生じる光学収差の実質的な部分を遮断するように構成されたマスクとを備える眼内デバイス。 - 前記表面は、前記レンズ本体の後面である、請求項10に記載の眼内デバイス。
- 前記表面は、前記レンズ本体の前面である、請求項10に記載の眼内デバイス。
- 前記表面の内側部分は凸状であり、前記移行ゾーンは凹状であり、前記表面の外側部分は凸状であり、前記移行ゾーンは前記内側部分および前記外側部分に隣接する、請求項10に記載の眼内デバイス。
- 前記表面の内側部分および外側部分は正の光学パワーを有し、前記移行ゾーンは前記内側部分および前記外側部分に隣接する、請求項10に記載の眼内デバイス。
- 前記表面の内側部分および外側部分は負の光学パワーを有し、前記移行ゾーンは前記内側部分および前記外側部分に隣接する、請求項10に記載の眼内デバイス。
- 前記表面は、内側部分と外側部分とを備え、前記内側部分の外周は前記移行ゾーンの内周に囲まれる、請求項10に記載の眼内デバイス。
- 前記移行ゾーンの外周は前記外側部分の内周に囲まれる、請求項16に記載の眼内デバイス。
- 前記表面は、内側部分と外側部分とを備え、内側部分の領域における前記レンズ本体の最大厚さは、前記移行ゾーンの領域における前記レンズ本体の最大厚さよりも薄い、請求項10に記載の眼内デバイス。
- 前記マスクは前記表面に取り付けられる、請求項10に記載の眼内デバイス。
- 前記マスクは前記レンズ本体の前記表面と第2の表面との間に位置する、請求項10に記載の眼内デバイス。
- 前記マスクは、孔と前記孔に隣接する不透明領域とを備える、請求項10に記載の眼内デバイス。
- 前記孔は、直径が約0.85mmから約1.8mmである、請求項21に記載の眼内デバイス。
- 前記マスクは、外径が約3mmから約5mmである、請求項22に記載の眼内デバイス。
- 前記マスクは、患者の焦点深度を増大する、請求項10に記載の眼内デバイス。
- 前記眼内デバイスが眼に移植された後で移動するのを妨げるように前記レンズ本体に取り付けられたハプティックをさらに備える、請求項10に記載の眼内デバイス。
- 前記マスクは、可視回折パターンを生成する穴の傾向を低減させるように複数の穴の穴サイズ、形状、向き、および間隔の少なくとも1つが変更されることを特徴とする複数の穴を備える、請求項10に記載の眼内デバイス。
- 前記マスクに結合され、前記マスクから前記レンズ本体の外面まで延びる支持部材をさらに備える、請求項10に記載の眼内デバイス。
- 前記マスクは、可視回折パターンを生成する穴の傾向を低減させるように複数の穴の穴サイズ、形状、向き、および間隔の少なくとも1つが変更されることを特徴とする複数の穴を備える、請求項27に記載の眼内デバイス。
- 前記支持部材は、可視回折パターンを生成する穴の傾向を低減させるように複数の穴の穴サイズ、形状、向き、および間隔の少なくとも1つが変更されることを特徴とする複数の穴を備える、請求項28に記載の眼内デバイス。
- 前記マスクに結合されたハプティックをさらに備える、請求項10に記載の眼内デバイス。
- 第1の表面および第2の表面を備え、前記第1の表面が、第1の部分、第2の部分、および第3の部分を備え、レンズ本体の光軸が前記第1の部分を通過し、前記第2の部分が前記第1の部分と前記第3の部分との間に位置するレンズ本体と、
前記第2の表面と前記第1の表面の前記第2の部分との間に位置するマスクとを備え、
前記第2の部分に隣接する前記第1の部分から前記光軸に垂直でありかつ前記第2の表面に接する平面までの距離が第1の距離を含み、前記第2の部分に隣接する前記第3の部分から前記光軸に垂直でありかつ前記第2の表面に接する平面までの距離が第1の距離よりも長い第2の距離を含む眼内デバイス。 - 後面および前面を含む外面を備えるインプラント本体と、
前記インプラント本体の前記後面と前記前面との間に位置する不透明マスクであって、前記マスクは孔を備える、不透明マスクと、
前記マスクに結合され、前記マスクから前記インプラント本体の前記外面まで延びる支持部材とを備える眼内インプラント。 - 前記支持部材は、前記マスクから前記インプラント本体の前記後面まで延びる、請求項32に記載の眼内インプラント。
- 前記マスクに隣接する前記支持部材の第1の部分は、前記マスクに平行な第1の断面積を有し、前記後面に隣接する前記支持部材の第2の部分は、前記マスクに平行であり、前記第1の断面積よりも小さい第2の断面積を有する、請求項33の眼内インプラント。
- 前記支持部材は、前記眼内インプラントから除去可能に構成される、請求項32に記載の眼内インプラント。
- 前記支持部材は、可視回折パターンを生成する穴の傾向を低減させるように複数の穴の穴サイズ、形状、向き、および間隔の少なくとも1つが変更されることを特徴とする複数の穴を備える、請求項32に記載の眼内インプラント。
- 眼内インプラントを製造する方法であって、
孔および前記マスクに結合された少なくとも1つの支持部材を備える不透明マスクを用意するステップと、
前記少なくとも1つの支持部材が型チャンバに結合され、それによって前記マスクが移動に抗するように前記マスクを前記型チャンバ内に位置させるステップと、
レンズ材料を前記型チャンバに流し込み、それによって前記マスクの少なくとも一部を前記レンズ材料内に埋め込むステップとを含む方法。 - 前記レンズ材料を注入した後で前記少なくとも1つの支持部材の少なくとも一部を除去するステップをさらに含む、請求項37に記載の方法。
- 眼内インプラントを製造する方法であって、
孔を備える不透明マスクを型チャンバの表面に結合するステップと、
レンズ材料を前記型チャンバに流し込んで前記マスクに結合された光学部材を形成するステップとを含む方法。 - 眼内インプラントを製造する方法であって、
光学部材の表面の一部を除去して孔領域の周りに環状のキャビティを形成するステップと、
前記キャビティに不透明材料を少なくとも部分的に充填するステップと、
前記孔領域の少なくとも一部および前記光学部材の中央領域を除去して前記レンズの前記孔領域の厚さを薄くするステップとを含み、
前記不透明材料の少なくとも一部が前記光学部材の前記表面上に残って不透明マスクを形成する方法。 - 眼内インプラントを製造する方法であって、
孔領域の周りに環状のキャビティを含む光学部材を用意するステップと、
前記キャビティに不透明材料を少なくとも部分的に充填するステップと、
前記孔領域の少なくとも一部および前記光学部材の中央領域を除去して前記光学部材の前記孔領域の厚さを薄くするステップとを含み、
前記不透明材料の少なくとも一部は、前記光学部材の表面上に残って不透明マスクを形成する方法。 - 眼内インプラントを製造する方法であって、
孔を含む不透明マスクを、型穴と物理的に接触しないように前記型穴内に位置させるステップと、
インプラント本体材料を前記型穴に注入して前記マスクの周りにインプラント本体を形成するステップとを含む方法。 - 前記マスクは、磁界によって位置決めされる、請求項42に記載の方法。
- 前記マスクは、前記マスクから前記型穴の外側のフレームまで延びるワイヤによって位置決めされる、請求項42に記載の方法。
- 患者の眼の溝領域に移植されるように構成されたインプラント本体を備え、前記インプラント本体が、
マスクを形成する不透明領域に少なくとも部分的に囲まれた孔と、
前記マスクの外周の実質的に外側に位置し、少なくとも1つの外側穴および少なくとも1つの連結部を備え、前記外側穴領域の入射可視光線透過率が少なくとも90%である外側穴領域と、
前記少なくとも1つの連結部によって前記マスクに取り付けられた外側領域とを備える、眼内インプラント。 - 眼内インプラントを製造する方法であって、
患者の眼の溝領域に移植されるように構成されたインプラント本体を用意するステップと、
前記インプラント本体の一部を除去することによって前記インプラント本体の孔を形成するステップと、
前記インプラント本体の外縁部と前記孔に隣接する不透明マスク領域との間に少なくとも1つの開口部を形成するステップとを含む方法。 - 眼内インプラントを製造する方法であって、
ロッドを用意するステップであって、前記ロッドが、
第1の端部および第2の端部、
長手方向において前記第1の端部と前記第2の端部との間に位置する光学的に透明な内側領域、
長手方向において前記第1の端部と前記第2の端部との間に位置する実質的に透明な外側領域、ならびに
長手方向において前記第1の端部と前記第2の端部との間に位置し、前記光学的に透明な内側領域と前記実質的に透明な外側領域との間に配置された実質的に不透明な領域を備える、ステップと、
第1の表面および第2の表面を有する前記眼内インプラントを形成するように前記ロッドを横方向に切断するステップとを含む方法。 - 前記第1の表面が実質的に平坦でありかつ前記第2の表面が実質的に平坦である前記眼内インプラントを形成するように前記ロッドを横方向に切断するステップをさらに含む、請求項47に記載の方法。
- 前記第1の表面が凸状でありかつ前記第2の表面が凹状である前記眼内インプラントを形成するように前記ロッドを横方向に切断するステップをさらに含む、請求項47に記載の方法。
- 前記第1の表面の第1の部分が、凸状であり、前記光学的に透明な内側領域に対応し、前記第1の表面の第2の部分が、凹状であり、前記実質的に不透明な領域に対応する前記眼内インプラントを形成するように前記ロッドを横方向に切断するステップをさらに含む、請求項47に記載の方法。
- 前記第1の表面の第3の部分が、凸状であり、前記実質的に透明な外側領域に対応する眼内インプラントを形成するように前記ロッドを横方向に切断するステップをさらに含む、請求項50に記載の方法。
