Description
- Hintergrund der Erfindung
- Die vorliegende Erfindung betrifft ein Verfahren zum Reduzieren der TNFα-Spiegel bei einem Säuger und dafür verwendbare Verbindungen und Zusammensetzungen.
- TNFα oder Tumornekrosefaktorα ist ein Cytokin, das hauptsächlich durch einkernige Phagozyten in Reaktion auf verschiedene Immunostimulatoren freigesetzt wird. Wenn an Säuger oder Menschen verabreicht, verursacht es Entzündung, Fieber, cardiovaskuläre Wirkungen, Hämorrhagie, Koagulation bzw. Gerinnung und akute Phasenreaktionen, ähnlich zu jenen, die während akuten Infektionen und Schockzuständen beobachtet werden.
- Zu starke oder unregulierte TNFα-Produktion erwies sich als in eine Vielzahl von Erkrankungszuständen einbezogen. Diese schließen Endotoxämie und/oder toxisches Schocksyndrom (Tracey et al., Nature 330, 662–664 (1987) und Hinshaw et al., Circ. Shock 30, 279–292 (1990)); Kachexie (Dezube et al., Lancet, 335 (8690), 662 (1990)); und adultes Atmungs-Distress-Syndrom, worin TNFα-Konzentration oberhalb 12 000 pg/ml in pulmonalen Aspiraten von ARDS-Patienten nachgewiesen wurde (Millar et al., Lancet 2(8665), 712–714 (1989)), ein. Systemische Infusion von rekombinantem TNFα ergab auch Änderungen, die man typischerweise bei ARDS beobachtet (Ferrai-Baliviera et al., Arch. Surg. 124(12), 1400–1405 (1989)).
- TNFα scheint in Knochenresorptionserkrankungen, einschließlich Arthritis, einbezogen zu sein, wobei ermittelt wurde, dass, wenn aktiviert, Leukozyten eine Knochenresorbierende Wirksamkeit erzeugen werden, und Daten lassen vermuten, dass TNFα zu dieser Wirksamkeit beiträgt (Bertolini et al., Nature 319, 516518 (1986) und Johnson et al., Endocrinology 124(3), 1424–1427 (1989).) Es wurde bestimmt, dass TNFα Knochenresorption stimuliert und Knochenbildung in vitro und in vivo durch Stimulierung von Osteoclastenbildung und Aktivierung, kombiniert mit Hemmung der Osteoblastenfunktion, inhibiert. Obwohl TNFα in viele Knochenresorptionserkrankungen, einschließlich Arthritis, einbezogen sein kann, ist die zwingendste Verbindung mit einer Erkrankung die Assoziation zwischen Produktion von TNFα durch Tumor oder Wirtsgewebe und Bösartigkeit, verbunden mit Hypercalcämie (Calci. Tissue Int. (US) 46(Suppl.), Seiten 3–10 (1990)). Bei Implantat-gegen-Wirts-Reaktion waren erhöhte Serum-TNFα-Spiegel mit starken Komplikationen nach akuten allogenen Knochenmarktransplantationen verbunden (Holler et al., Blood, 75(4), 1011–1016 (1990)).
- Cerebrale Malaria ist ein lethales, hyperakutes neurologisches Syndrom, das mit hohen Blutspiegeln an TNFα verbunden ist, und schwersten Komplikation, die bei Malariapatienten auftreten. Serumspiegel von TNFα korrelierten direkt mit der Schwere der Erkrankung und der Prognose bei Patienten mit akuten Malariaanfällen (Grau et al., N. Engl. J. Med. 320(24), 1586–1591 (1989)).
- TNFα spielt auch eine Rolle auf dem Gebiet von chronischen pulmonaren Entzündungserkrankungen. Die Abscheidung von Siliziumdioxidteilchen führt zu Silikose, eine Erkrankung von zunehmendem Atmungsversagen, verursacht durch eine fibrotische Reaktion. Antikörper zu TNFα blockierten vollständig die Silica-induzierte Lungenfibrose bei Mäusen (Pignet et al., Nature, 344: 245–247 (1990)). Hohe Spiegel von TNFα-Produktion (im Serum und in isolierten Makrophagen) wurden in Tiermodellen von Silica- und Asbest-induzierter Fibrose nachgewiesen (Bissonnette et al., Inflammation 13(3), 329–339 (1989)). Es wurde auch gefunden, dass alveolare Makrophagen von pulmonalen Sarcoidosepatienten spontan massive Mengen an TNFα freisetzten, verglichen mit Makrophagen von normalen Donoren (Baughman et al., J. Lab. Clin. Med. 115(1), 36–42 (1990)).
- TNFα ist auch in die entzündliche Reaktion einbezogen, die nach Reperfusion folg, genannt Reperfusionsschädigung, und ist eine Hauptursache von Gewebsschädigung nach dem Aussetzen des Blutstroms (Vedder et al., PNAS. 87, 2643–2646 (1990)). TNFα verändert auch die Eigenschaften von endothelialen Zellen und hat verschiedene gerinnungsfördernde Wirksamkeiten, wie das Erzeugen einer Erhöhung in der Gewebsfaktor-Gerinnungsförderungswirksamkeit und Unterdrückung des Metabolismus vom gerinnungshemmenden Protein-C sowie Herunterregulieren der Expression von Thrombomodulin (Sherry et al., J. Cell Biol. 107, 1269–1277 (1988)). TNFα hat entzündungsfördernde Wirksamkeiten, die es zusammen mit seiner frühen Herstellung (während der Anfangsstufe eines Entzündungsereignis) zu einem wahrscheinlichen Mediator von Gewebsschädigung bei verschiedenen wichtigen Störungen, einschließlich Herzinfarkt, Schlaganfall und Kreislaufschock, jedoch nicht darauf begrenzt, macht. Von besonderer Bedeutung kann die TNFα-induzierte Expression von Anhaftungsmolekülen, wie intrazelluläres Anhaftungsmolekül (ICAM) und endotheliales Leukozyten-Anhaftungsmolekül (ELAM), an endothelialen Zellen (Munro et al., Am. J. Path. 135(1), 121–132 (1989)), sein.
- Darüber hinaus wurde nun bekannt, dass TNFα ein starker Aktivator von Retrovirusreplikation, einschließlich Aktivierung von HIV-1, ist. (Duh et al., Proc. Nat. Acad. Sci. 86, 5974–5978 (1989); Poll et al., Proc. Nat. Acad. Sci. 87, 782–785 (1990); Monto et al., Blood 79, 2670 (1990); Clouse et al., J. Immunol. 142, 431–438 (1989); Poll et al., AIDS Res. Hum. Retrovirus, 191–197 (1992)). AIDS ergibt sich aus der Infektion von T-Lymphozyten mit Human-Immunmangel-Virus (HIV). Mindestens drei Arten oder Stämme von HIV wurden identifiziert; das heißt, HIV-1, HIV-2 und HIV-3. Infolge einer HIV-Infektion wird T-Zellen-vermittelte Immunität beeinträchtigt und infizierte Individuen zeigen schwere opportunistische Infektionen und/oder ungewöhnliche Neoplasmen. Das Eindringen von HIV in die T-Lymphozyten erfordert T-Lymphozytenaktivierung. Andere Viren, wie HIV-1, HIV-2, infizieren T-Lymphozyten nach T-Zellenaktivierung und solche Virusproteinexpression und/oder -replikation wird durch T-Zellenaktivierung vermittelt oder aufrecht erhalten. Ist einmal ein aktivierter T-Lymphozyt mit HIV infiziert, muss der T-Lymphozyt fortgesetzt, in einem aktivierten Zustand gehalten zu werden, damit HIV-Gen-Expression und/oder HIV- Replikation stattfinden kann. Cytokine, speziell TNFα, sind in durch aktivierte T-Zellen vermittelte HIV-Proteinexpression und/oder Virusreplikation einbezogen, indem sie beim Aufrechterhalten der T-Lymphozytenaktivierung eine Rolle spielen. Deshalb unterstützt die Störung der Cytokinaktivität, wie durch Verhinderung oder Inhibierung der Cytokinproduktion, insbesondere TNFα, bei einer HIV-infizierten Person beim Begrenzen des durch HIV-Infektion verursachten Aufrechterhaltens von T-Lymphozyt.
- Monozyten, Makrophagen und verwandte Zellen, wie Kupfer- und Glialzellen, sind auch in das Aufrechterhalten von HIV-Infektion einbezogen. Diese Zellen, wie T-Zellen, sind Ziele zur viralen Replikation und der Spiegel von viraler Replikation ist vom Aktivierungszustand der Zellen abhängig. (Rosenberg et al., The Immunopathogenesis of HIV Infection, Advances in Immunology, 57 (1989)). Von Cytokinen, wie TNFα, wurde gezeigt, dass sie die HIV-Replikation in Monozyten und/oder Makrophagen aktivieren (Poli et al., Proc. Natl. Acad. Sci., 87, 782–784 (1990)). Deshalb unterstützt die Verhinderung oder Inhibierung von Cytokinproduktion oder -aktivität beim Begrenzen des Fortschritts von HIV, wie vorstehend für T-Zellen ausgewiesen. Zusätzliche Studien haben TNFα als üblichen Faktor bei der in vitro-Aktivierung von HIV identifiziert und haben einen deutlichen Wirkmechanismus gegenüber einem kernregulativen Protein in dem Zytoplasma von Zellen gefunden (Osborn, et al., PNAS 86, 2336–2340). Dieser Nachweis lässt vermuten, dass eine Verminderung der TNFα-Synthese eine antivirale Wirkung auf HIV-Infektionen durch Verminderung der Transkription und somit Virusproduktion ausüben kann.
- AIDS-virale Replikation von latentem HIV in T-Zellen und Makrophagenlinien kann in TNFα eingeführt werden (Folks et al., PNAS 86, 2365–2368 (1989)). Es wird ein molekularer Mechanismus für die Virus-induzierende Aktivität vermutet, aufgrund der Fähigkeit von TNFα, ein Gen-regulierendes Protein (NFKB), das in dem Zytoplasma von Zellen gefunden wird, welches die HIV-Replikation durch Binden an eine virale regulatorische Gensequenz (LTR) fördert (Osborn et al., PNAS 86, 2336–2340 (1989)), zu aktivieren. TNFα in mit AIDS verbundener Kachexie wird durch erhöhte Serum-TNFα und hohe Spiegel von spontaner TNFα-Erzeugung in peripheren Blut monozyten von Patienten vermutet (Wright et al., J. Immunol. 141(1), 99–104 (1988)).
- TNFα war in verschiedene Fällen mit anderen viralen Infektionen verwickelt, wie dem Cytomegaliavirus (CMV), Influenzavirus, Adenovirus und der Herpesfamilie von Viren, aus ähnlichen, wie jenen, angeführten Gründen.
- Verhindern oder Inhibieren der Erzeugung oder Wirkung von TNFα wird deshalb als eine wirksame therapeutische Strategie für viele Entzündungs-, Infektions-, immunlogischen oder malignen Erkrankungen vorhergesagt. Diese schließen ein, sind jedoch nicht darauf begrenzt, septischen Schock, Sepsis, endotoxischen Schock, hämodynamischen Schock und Sepsissyndrom, post-ischämische Reperfusionsschädigung, Malaria, mycobakterielle Infektion, Meningitis, Psoriasis, Herzstauungsinsuffizienz, fibrotische Erkrankung, Kachexie, Transplantatwiederabstoßung, Krebs, Autoimmunerkrankung, opportunistische Infektionen bei AIDS, rheumatische Arthritis, rheumatische Spondylitis, Osteoarthritis, andere arthritische Zustände, Crohn'sche Krankheit, ulcerative Colitis, multiple Sklerose, systemischen Lupus erythrematosis, ENL in Leprosie, Strahlungsschädigung und hyperoxische alveoläre Schädigung. Die auf die Unterdrückung der Effekte von TNFα gerichteten Bemühungen lagen im Bereich von der Anwendung von Steroiden, wie Dexamethason und Prednisolon, bis zu der Anwendung von sowohl polyklonalen als auch monoklonalen Antikörpern (Beutler et al., Science 234, 470–474 (1985); WO 92/11383).
- Der Kernfaktor kB (NFKB) ist ein pleiotroper transkriptionaler Aktivator (Lenardo et al. Cell 1989, 58, 227–29). NFKB ist als ein transkriptionaler Aktivator in einer Vielzahl von Erkrankungs- und Entzündungszuständen einbezogen, und es wird angenommen, dass er Zytokinspiegel reguliert, einschließlich TNFα, jedoch nicht darauf begrenzt, und auch ein Aktivator von HIV-Transkription ist (Dbaibo et al. J. Biol. Chem. 1993, 17762–66; Duh et al. Proc. Natl. Acad. Sci. 1989, 86, 5974–78; Bachelerie et al. Nature 1991, 350, 709–12; Boswas et al. J. Acquired Immune Deficiency Syndrome 1993, 6, 778–786; Suzuki et al. Biochem. And Biophys. Res. Comm. 1993, 193, 277–83; Suzuki et al. Biochem. And Biophys. Res. Comm. 1992, 189, 1709–15; Suzuki et al. Biochem. Mol. Bio. Int. 1993, 31(4), 693–700, Shakhov et al., 1990, 171, 35–47; und Staal et al. Proc. Natl. Acad. Sci. USA 1990, 87, 9943–47). Somit kann Inhibierung von NFKB-Binden Transkription von zytokinem(n) Gen(en) regulieren und durch diese Modulation und andere Mechanismen bei der Inhibierung einer Vielzahl von Erkrankungszuständen verwendbar sein. In diesem Patent beanspruchte Verbindungen können die Wirkung von NFKB in dem Kern inhibieren und somit bei der Behandlung einer Vielzahl von Erkrankungen brauchbar sein, einschließlich, jedoch nicht darauf begrenzt, rheumatische Arthritis, rheumatische Spondylitis, Osteoarthritis, andere arthritische Zustände, septischen Schock, Sepsis, endotoschen Schock, Transplantat gegen Wirtserkrankung, Abnahme, Crohn'sche Krankheit, ulcerative Colitis, multiple Sklerose, systemischen Lupus erythrematosis, ENL bei Leprosie, HIV, AIDS und opportunistischen Infektionen bei AIDS.
- TNFα- und NFKB-Spiegel werden durch eine reziproke Rückführungsschleife beeinflusst. Wie vorstehend ausgewiesen, beeinflussen die erfindungsgemäßen Verbindungen die Spiegel von sowohl TNFα als auch NFKB. Es ist jedoch zur Zeit nicht bekannt, wie die erfindungsgemäßen Verbindungen die Spiegel von TNFα, NFKB oder beiden regulieren.
- Beschreibung im Einzelnen
- Die vorliegende Erfindung betrifft eine Verbesserung der Bildung der erfindungsgemäßen Verbindungen, wie nachstehend genauer erörtert.
- Die vorliegende Erfindung stellt ein Verfahren zur Herstellung von Thalidomid bereit, in welchem N-Phthaloylglutamin oder N-Phthaloylisoglutamin mit N,N'-Carbonyldiimidazol cyclisiert wird, dadurch gekennzeichnet, dass das Verfahren das Erwärmen des N-Phthaloylglutamins oder N-Phthaloylisoglutamins und N,N'-Carbonyldiimidazols in unter Rückfluss erhitztem, wasserfreiem Tetrahydrofuran umfasst.
- Vorzugsweise wird der Schritt des Erhitzens unter Rückfluss in Gegenwart einer Base ausgeführt.
- Einem N-Alkoxycarbonylimid und einem Amin wird somit erlaubt, in Gegenwart einer Base, wie Natriumcarbonat oder Natriumbicarbonat, im Wesentlichen wie von Shealy et al., Chem. & Ind., (1965) 1030–1031) und Shealy et al., J. Pharm. Sci. 57, 757–764 (1968) beschrieben, zu reagieren, um das N-substituierte Imid zu ergeben. Alternativ kann ein cyclisches Säureanhydrid mit einem geeigneten Amin zur Bildung eines Imids umgesetzt werden. Die Bildung eines cyclischen Imids kann auch durch Erhitzen einer Lösung eines geeignet substituierten Dicarbonsäuremonoamids in wasserfreiem Tetrahydrofuran unter Rückfluss mit N,N'-Carbonyldiimidazol ausgeführt werden. Im Gegensatz zu Verfahren des Standes der Technik, die eine Ausbeute von weniger als 50% erzeugten, erzeugt diese Reaktion Ausbeuten oberhalb 60%, in einigen Fällen mehr als 90%. Diese Reaktion hat auch eine breitere Anwendbarkeit, wobei sie nicht nur bei der Herstellung von den erfindungsgemäßen Verbindungen, sondern auch bei der Herstellung von bekannten Verbindungen, wie Thalidomid, anwendbar ist.
- Die nachstehenden Beispiele dienen zur weiteren Beschreibung der Beschaffenheit dieser Erfindung, sollten jedoch nicht als Begrenzung von deren Umfang aufgefasst werden, wobei der Umfang nur durch die beigefügten Ansprüche definiert wird.
- BEISPIEL 1
- Zu einer gerührten Lösung von L-Glutamin (43,8 g, 300 mMol) und Natriumcarbonat (33,4 g, 315 mMol) in 750 ml Wasser wird schnell N-Carbethoxyphthalimid [65,8 (97% rein, 67,8 g), 300 mMol] als ein Feststoff gegeben. Nach 1 Stunde wird das Reaktionsgemisch zum Entfernen von nichtumgesetztem N-Carbethoxyphthalimid filtriert. Der pH-Wert des gerührten Filtrats wird mit 4 N Salzsäure auf 3–4 eingestellt. Das Gemisch wird dann mit N-Phthaloyl-L-glutamin beimpft und der pH-Wert wird mit 4 N Salzsäure auf 1–2 eingestellt. Die erhaltene Aufschlämmung wird für 1 Stunde gerührt. Die Aufschlämmung wird filtriert und der Feststoff mit reichlichen Mengen Wasser gewaschen. Der Feststoff wird luftgetrocknet, dann im Vakuum (60°C, < 1 mm) über Nacht getrocknet, unter Bereitstellung von 49,07 g (59%) N-Phthaloyl-L- glutamin, das alternativ 2-Phthalimidoglutaramidsäure genannt werden kann, als ein weißes Pulver.
- BEISPIEL 2
- Ein gerührtes Gemisch von N-Phthaloyl-L-glutamin (48,0 g, 174 mMol), Carbonyldiimidazol (30,43 g, 188 mMol) und 4-Dimethylaminopyridin (0,105 g, 0,861 mMol) in wasserfreiem Tetrahydrofuran (300 ml) wird 16 Stunden unter Rückfluss erhitzt. Die Reaktionsaufschlämmung wird filtriert und der Feststoff mit Methylenchlorid (200 ml) gewaschen. Der Feststoff wird luftgetrocknet und dann im Vakuum (60°C, < 1 mm) getrocknet, unter Bereitstellung von 40,40 g (90%) Thalidomid als ein weißes Pulver.
1H NMR (DMSO-d6) δ 11,16 (s, 1H, NH), 8,05–7,80 (br s, 4H, Ar), 5,18 (dd, 1H, J = 12,5 Hz, CHCO), 3,05–2,85 (m, 1H, CH2CO), 2,70–2,45 (m, 2H, CH2CH2), 2,15–2,00 (m, 1H, CH2). 13C NMR (DMSO-d6) δ 172,8, 169,8, 167,1, 134,9, 131,2, 123,4, 49,0, 30,9, 22,0.
Claims (2)
- Verfahren zur Herstellung von Thalidomid, in welchem N-Phthaloylglutamin oder N-Phthaloylisoglutamin mit N,N'-Carbonyldiimidazol cyclisiert wird, dadurch gekennzeichnet, daß das Verfahren das Erwärmen des N-Phthaloylglutamins oder N-Phthaloylisoglutamins und N,N'-Carbonyldiimidazols in unter Rückfluß erhitztem wasserfreien Tetrahydrofuran umfaßt.
- Verfahren nach Anspruch 1, worin das Erwärmen in unter Rückfluß erhitztem wasserfreien Tetrahydrofuran in der Gegenwart einer Base durchgeführt wird.
