Abstract
Un procedimiento para preparar talidomida en el que N-ftaloilglutamina o N-ftaloilisoglutamina se cicla con N, N¿-carbonildiimidazol, caracterizado porque el procedimiento comprende calentar dichos N- ftaloilglutamina o N-ftaloilisoglutamina y N, N¿- carbonildiimidazol en tetrahidrofurano anhidro calentado a reflujo.
Description
Procedimiento para la preparación de
talidomida.
La presente invención se refiere a un
procedimiento para reducir la concentración de TNF_{\alpha} en un
mamífero y a los compuestos y composiciones útiles para ello.
El TNF_{\alpha}, o factor de necrosis tumoral
\alpha, es una citocina que se libera principalmente por los
fagocitos mononucleares en respuesta a diversos
inmunoestimuladores. Cuando se administra a animales o a seres
humanos produce inflamación, fiebre, efectos cardiovasculares,
hemorragia, coagulación y respuestas de fase aguda similares a las
que se ven durante infecciones agudas y estados de choque.
Se ha asociado una producción excesiva o no
regulada de TNF_{\alpha} con distintas enfermedades. Entre ellas
se incluyen endotoxemia y/o síndrome de choque séptico (Tracey y
cols., Nature 330, 662-664 (1987), y Hinshaw
y col., Circ. Shock 30, 279-292 (1990)),
caquexia (Dezube y col., Lancet 335 (8690), 662 (1990)) y
síndrome de dificultad respiratoria del adulto, en el que se han
detectado concentraciones de TNF_{\alpha} superiores a 12.000
pg/ml en aspirados pulmonares de pacientes con SDRA (Millar y col.,
Lancet 2 (8665), 712-714 (1989)). La
infusión sistémica de TNF_{\alpha} recombinante dio lugar a los
cambios que se ven de manera típica en el SDRA
(Ferrai-Baliviera y col., Arch Surg 124(12),
1400-1405 (1989)).
Parece que el TNF_{\alpha} está implicado en
las enfermedades por reabsorción ósea, incluyendo la artritis, en
la que se ha determinado que cuando se activan, los leucocitos
presentan actividad de reabsorción del hueso, y los datos indican
que el TNF_{\alpha} contribuye a esta actividad (Bertolini y col.,
Nature 319, 516-518 (1986), y Johnson y
col., Endocrinology 124(3), 1424-1427
(1989)). Se ha determinado que el TNF_{\alpha} estimula la
reabsorción ósea e inhibe la formación de hueso in vitro e
in vivo mediante la estimulación de la formación y
activación de los osteoclastos, combinada con una inhibición de la
función de los osteoblastos. Aunque el TNF_{\alpha} puede
participar en muchas enfermedades por reabsorción ósea, incluyendo
la artritis, el vínculo más evidente con enfermedades es la
asociación entre la producción de TNF_{\alpha} por los tejidos
del tumor o del huésped y la hipercalcemia asociada a la neoplasia
(Calci. Tissue Int. (US) 46(Suppl.), S3-S10
(1990)). En la reacción del injerto frente al huésped,
concentraciones séricas elevadas de TNF_{\alpha} se han asociado
a complicaciones graves después del transplante alogénico agudo de
médula ósea (Holler y col., Blood 75(4),
1011-1016 (1990)).
El paludismo cerebral es un síndrome neurológico
hiperagudo mortal que se asocia a concentraciones sanguíneas
elevadas de TNF_{\alpha} y es la complicación más grave que
aparece en los pacientes que tienen paludismo. La concentración
sérica de TNF_{\alpha} se correlacionó directamente con la
gravedad de la enfermedad y con el pronóstico en pacientes con
ataques agudos de paludismo (Grau y col., N. Eng. J. Med.

El TNF_{\alpha} también participa en el área de
las enfermedades pulmonares inflamatorias crónicas. El depósito de
partículas de sílice da lugar a silicosis, una enfermedad producida
por una reacción fibrótica que cursa con insuficiencia respiratoria
progresiva. Los anticuerpos frente al TNF_{\alpha} bloquearon
completamente la fibrosis pulmonar inducida por sílice en ratones
(Pignet y col., Nature 344, 245-247 (1990)).
Se han demostrado niveles elevados de producción de TNF_{\alpha}
(en el suero y en macrófagos aislados) en modelos animales de
fibrosis inducida por sílice y asbesto (Bissonnette y col.,
Inflammation 13(3), 329-339 (1989)). También
se ha demostrado que los macrófagos alveolares de pacientes con
sarcoidosis pulmonar liberan espontáneamente cantidades masivas de
TNF_{\alpha} en comparación con los macrófagos de donantes sanos
(Baughman y col., J. Lab. Clin. Med. 115(1),
36-42 (1990)).
El TNF_{\alpha} también está implicado en la
respuesta inflamatoria que sigue a la reperfusión, denominada
lesión por reperfusión, y es una causa importante de lesión tisular
después de la pérdida del flujo sanguíneo (Vedder y col., PNAS
87, 2643-2646 (1990)). El TNF_{\alpha}
también altera las propiedades de las células endoteliales y tiene
diversas actividades procoagulantes, como la producción de un
aumento de la actividad del factor tisular procoagulante y la
supresión de la ruta anticoagulante de la proteína C, así como la
reducción de la expresión de trombomodulina (Sherry y col., J. Cell
Biol. 107, 1269-1277 (1988)). El
TNF_{\alpha} tiene actividades proinflamatorias que, junto a su
producción temprana (durante la etapa inicial de un episodio
inflamatorio), hacen que sea un probable mediador de la lesión
tisular en varios trastornos importantes que incluyen, aunque no se
limitan a ellos, infarto de miocardio, accidente cerebrovascular y
choque circulatorio. Puede tener importancia específica la
expresión inducida por TNF_{\alpha} de moléculas de adhesión en
las células endoteliales, como la molécula de adhesión intercelular
(ICAM) o la molécula de adhesión leucocítica endotelial (ELAM)
(Munro y col., Am. J. Path. 135(1), 121-132
(1989)).
Además, ahora se sabe que el TNF_{\alpha} es un
potente activador de la replicación de los retrovirus, incluyendo
la activación del VIH-1 (Duh y col., Proc. Natl.
Acad. Sci. 86, 5974-5978 (1989); Poll y col.,
Proc. Natl. Acad. Sci. 87, 782-785 (1990);
Monto y col., Blood 79, 2670 (1990); Clouse y col., J.
Immunol. 142, 431-438 (1989); Poll y col.,
AIDS Res. Hum. Retrovirus 191-197 (1992)). El sida
se debe a la infección de los linfocitos T por el virus de la
inmunodeficiencia humana (VIH). Se han identificado al menos tres
tipos de cepas de VIH, VIH-1, VIH-2
y VIH-3. Como consecuencia de la infección por el
VIH está alterada la inmunidad mediada por los linfocitos T, y los
pacientes infectados manifiestan infecciones oportunistas graves y/o
neoplasias poco habituales. La entrada del VIH en el interior del
linfocito T precisa la activación del linfocito T. Otros virus,
como VIH-1 y VIH-2, infectan los
linfocitos T después del activación de los linfocitos T, y esa
expresión de las proteínas del virus y/o replicación del virus está
mediada o mantenida por esa activación de los linfocitos T. Una vez
que un linfocito T activado ha sido infectado por el VIH, el
linfocito T debe seguir manteniéndose en un estado activado para
permitir la expresión génica del VIH y/o la replicación del VIH.
Las citocinas, específicamente el TNF_{\alpha}, están implicadas
en la expresión proteica y/o replicación vírica del VIH mediadas
por linfocitos T activados al participar en el mantenimiento de la
activación de los linfocitos T. Por lo tanto, la interferencia con
la actividad de las citocinas, como mediante la prevención o
inhibición de la producción de citocinas, de manera notable el
TNF_{\alpha}, en un paciente infectado por el VIH ayuda a limitar
el mantenimiento de los linfocitos T provocados por la infección por
VIH.
Se ha implicado a monocitos, macrófagos y células
relacionadas, como las células de Kupffer y las células de la glía,
en el mantenimiento de la infección por el VIH. Estas células, al
igual que los linfocitos T, son dianas de la replicación vírica, y
el nivel de replicación vírica depende del estado de activación de
las células (Rosenberg y col., The Immunopathogenesis of HIV
Infection. Advances in Immunology 57 (1989)). Se ha mostrado que
las citocinas, como el TNF_{\alpha}, activan la replicación del
VIH en monocitos y/o en macrófagos (Poli y col., Proc. Natl. Acad.
Sci. 87, 782-784 (1990)). Por lo tanto, la
prevención o la inhibición de la producción o de la actividad de
las citocinas ayuda a limitar la progresión del VIH, como se ha
reseñado más arriba para los linfocitos T. Otros estudios han
identificado que el TNF_{\alpha} es un factor común en la
activación del VIH in vitro y han proporcionado un mecanismo
de acción claro a través de una proteína reguladora nuclear que se
encuentra en el citoplasma de las células (Osborn y col., PNAS
86, 2336-2340 (1989)). Estos datos indican
que una reducción de la síntesis del TNF_{\alpha} puede tener un
efecto antivírico en las infecciones por el VIH, mediante la
reducción de la transcripción y, por lo tanto, de la producción de
virus.
El TNF_{\alpha} puede inducir la replicación
del virus del SIDA del VIH latente en líneas de linfocitos T y de
macrófagos (Folk y col., PNAS 86, 2365-2368
(1989)). Un mecanismo molecular para la actividad inductora del
virus viene indicado por la capacidad del TNF_{\alpha} de activar
una proteína reguladora génica (NFKB) que se encuentra en el
citoplasma de las células y que promueve la replicación del VIH
mediante la unión a una secuencia génica reguladora del virus (LTR)
(Osborn y col., PNAS 86, 2336-2340 (1989)).
La participación del TNF_{\alpha} en la caquexia asociada al sida
viene indicada por la elevación de la concentración sérica de
TNF_{\alpha} y por los altos niveles de producción espontánea de
TNF_{\alpha} en los monocitos de sangre periférica de los
pacientes (Wright y col., J. Immunol. 141(1),
99-104 (1988)).
Se ha implicado al TNF_{\alpha} en diversas
funciones en otras infecciones víricas, como el virus de la
citomegalia (CMV), el virus de la gripe, el adenovirus y la familia
de los virus herpes, por razones similares a las que ya se han
presentado.
Por lo tanto, se puede predecir que la prevención
o la inhibición de la producción o de la acción del TNF_{\alpha}
es una potente estrategia terapéutica en muchas enfermedades
inflamatorias, infecciosas, inmunológicas o neoplásicas. Estas
enfermedades incluyen, aunque no están limitadas a ellas, choque
séptico, sepsis, choque endotóxico, choque hemodinámico y síndrome
séptico, lesión por reperfusión postisquémica, paludismo,
infecciones por micobacterias, meningitis, psoriasis, insuficiencia
cardiaca congestiva, enfermedades fibróticas, caquexia, rechazo del
injerto, cáncer, enfermedades autoinmunitarias, infecciones
oportunistas en el sida, artritis reumatoidea, espondilitis
reumatoidea, artrosis, otras enfermedades artríticas, enfermedad de
Crohn, colitis ulcerosa, esclerosis múltiple, lupus eritematoso
sistémico, ENL en la lepra, lesión por radiación y lesión alveolar
hiperóxica. Los esfuerzos dirigidos a la supresión de los efectos
del TNF_{\alpha} han variado desde la utilización de corticoides
como dexametasona y prednisolona hasta el uso de anticuerpos, tanto
policlonales como monoclonales (Beutler y col., Science 234,
470-474 (1985); documento WO 92/11383).
El factor nuclear kB (NFKB) es un activador
pleiotrópico de la transcripción (Lenardo y col., Cell 1989,
58, 227-229). Se ha implicado al NFKB como
activador de la transcripción en diversas enfermedades y estados
inflamatorios, y se piensa que regula la concentración de citocinas
que incluyen, aunque no están limitadas a ella, TNF_{\alpha}, y
también puede ser un activador de la transcripción del VIH (Dbaibo
y col., J. Biol. Chem. 1993, 268,
17762-17766; Duh y col., Proc. Natl. Acad. Sci.
1989, 86, 5974-5978; Bachelerie y col.,
Nature 1991, 350, 709-712; Boswas y col., J.
Acquired Immune Deficiency Syndrome 1993, 6,
778-786; Suzuki y col., Biochem. And Biophys. Res.
Comm. 1993, 193, 277-283; Suzuki y col.,
Biochem. And Biophys. Res. Comm. 1992, 189,
1709-1715; Suzuki y col., Biochem. Mol. Bio. Int.
1993, 31(4), 693-700; Shakno y col., 1990,
171, 35-47; y Staal y col., Proc. Natl. Acad.
Sci. USA 1990, 87, 9943-9947). De esta
manera, la inhibición de la unión a NFKB puede regular la
transcripción de los genes de las citocinas, y mediante esta
modulación y otros mecanismos puede ser útil en la inhibición de
multitud de enfermedades. Los compuestos que se reivindican en esta
patente pueden inhibir la acción del NFKB en el núcleo y de esta
manera son útiles en el tratamiento de diversas enfermedades que
incluyen, aunque no se limitan a ellas, artritis reumatoidea,
espondilitis reumatoidea, artrosis, otras enfermedades artríticas,
choque séptico, sepsis, choque endotóxico, enfermedad del injerto
frente al huésped, consunción enfermedad de Crohn, colitis ulcerosa,
esclerosis múltiple, lupus eritematoso sistémico, ENL en la lepra,
infección por VIH, sida e infecciones oportunistas en el sida.
La concentración de TNF_{\alpha} y de NFKB está
influida por un bucle de retroalimentación recíproca. Como se ha
señalado más arriba, los compuestos de la presente invención
afectan a la concentración tanto de TNF_{\alpha} como de NFKB.
Sin embargo, en este momento no se sabe cómo los compuestos de la
presente invención regulan la concentración del TNF_{\alpha}, del
NFKB o de ambos.
La presente invención se relaciona con una mejora
de la formación de los compuestos de la presente invención, como se
analiza más adelante con mayor detalle.
La presente invención proporciona un
procedimiento para preparar talidomida en el que
N-ftaloilglutamina o
N-ftaloilisoglutamina se cicla con
N,N'-carbonildiimidazol, que se caracteriza porque
el procedimiento comprende calentar dichos
N-ftaloilglutamina o
N-ftaloilisoglutamina y
N,N'-carbonildiimidazol en tetrahidrofurano anhidro
calentado a reflujo.
Preferentemente, la etapa de calentamiento hasta
reflujo se realiza en presencia de una base.
De esta manera se permite que una
N-alcoxicarbonilimida reaccione con una amina en
presencia de una base como carbonato sódico o bicarbonato sódico
sustancialmente como lo describen Shealy y col., Chem. & Ind.,
(1965) 1030-1031, y Shealy y col., J. Pharm. Sci.
57, 757-764 (1968), para obtener la imida
N-sustituida. De manera alternativa se puede hacer
reaccionar un anhídrido de ácido cíclico con una amina adecuada
para formar una imida. También se puede conseguir la formación de
una imida cíclica calentando hasta reflujo una solución de una
monoamida de un ácido dicarboxílico adecuadamente sustituida en
tetrahidrofurano anhidro con
N,N'-carbonildiimidazol. Al contrario que los
procedimientos de la técnica previa que tenían un rendimiento de
menos de 50%, esta reacción tiene un rendimiento superior a 60%, y
en algunos casos superior a 90%. Esta reacción también tiene una
mayor aplicabilidad, y es útil no sólo en la preparación de los
compuestos de la presente invención sino también en la preparación
de compuestos conocidos como talidomida.
Los siguientes ejemplos ayudarán a tipificar más
la naturaleza de esta invención, aunque no se deben considerar como
una limitación del alcance de la misma, que está definido
únicamente por las reivindicaciones adjuntas.
A una solución agitada de
L-glutamina (43,8 g, 300 mmol) y carbonato sódico
(33,4 g, 315 mmol) en 750 ml de agua se añade rápidamente
N-carbetoxiftalimida (65,8 g [pureza de 97%, 67,8
g], 300 mmol) en forma de sólido. Después de una hora se filtra la
mezcla de la reacción para eliminar la
N-carbetoxiftalimida que no ha reaccionado. El pH
del filtrado agitado se ajusta a 3-4 con ácido
clorhídrico 4 N. Posteriormente la mezcla se inocula con
N-ftaloil-L-glutamina
y el pH se ajusta a 1-2 con ácido clorhídrico 4 N.
La suspensión resultante se agita durante una hora. La suspensión
se filtra y se lava el sólido con grandes cantidades de agua. El
sólido se seca al aire libre y posteriormente se seca al vacío
(60ºC, < 1 mm <133,3 Pa) durante una noche para obtener 49,07
g (59%) de
N-ftaloil-L-glutamina,
que también se puede denominar ácido
2-ftalimidoglutarámico, en forma de polvo
blanco.
Se calienta a reflujo durante 16 horas una mezcla
agitada de
N-ftaloil-L-glutamina
(48, g, 174 mmol), carbonildiimidazol (30,43 g, 188 mmol) y
4-dimetilaminopiridina (0,105 g, 0,861 mmol) en
tetrahidrofurano anhidro (300 ml). La suspensión de la reacción se
filtra y el sólido se lava con cloruro de metileno (200 ml). El
sólido se seca al aire y posteriormente se seca al vacío (60ºC,
< 1 mm <133,3 Pa) para obtener 40,40 g 90% de talidomida en
forma de polvo blanco. RMN ^{1}H (DMSO-d_{6})

Claims (2)
1. Un procedimiento para preparar talidomida en
el que N-ftaloilglutamina o
N-ftaloilisoglutamina se cicla con
N,N'-carbonildiimidazol, caracterizado
porque el procedimiento comprende calentar dichos
N-ftaloilglutamina o
N-ftaloilisoglutamina y
N,N'-carbonildiimidazol en tetrahidrofurano anhidro
calentado a reflujo.
2. El procedimiento de la reivindicación 1 en el
que el calentamiento a reflujo en tetrahidrofurano anhidro se
realiza en presencia de una base.
