Abstract
Den foreliggende oppfinnelse vedrører adjuvansmiddelblandinger egnet for anvendelse i vaksiner. Spesielt omfatter adjuvansmiddelblandingene ifølge den foreliggende oppfinnelse et saponin og et immunstimulatorisk oligonukleotid, eventuelt med en bærer. Den foreliggende oppfinnelse tilveiebringer videre vaksiner omfattende adjuvansmidlene ifølge den foreliggende oppfinnelse og et antigen. Oppfinnelsen tilveiebringer videre fremgangsmåter for fremstilling av adjuvansmidlene og vaksinene ifølge den foreliggende oppfinnelse og deres anvendelse som legemidler. Metoder for tilførsel av vak- siner ifølge den foreliggende oppfinnelse for behandling av et individ som er mottagelig for, eller lider av en sykdom, tilveiebringes også.
Description
Foreliggende oppfinnelse vedrører nye adjuvanssammensetninger for anvendelse i vaksiner. Spesielt omfatter adjuvanssammensetningene ifølge foreliggende oppfinnelse en kombinasjon av saponin og et immunstimulerende oligonukleotid, idet kombinasjonen eventuelt ytterligere omfatter en bærer. Foreliggende oppfinnelse tilveiebringer også vaksiner omfattende adjuvanssammensetningene ifølge den foreliggende oppfinnelse og minst ett antigen. Ytterligere tilveiebringes fremgangsmåter for fremstilling av vaksinene ifølge foreliggende oppfinnelse og deres anvendelse som legemidler.
Immunstimulatoriske oligonukleotider inneholdende umetylerte CpG-dinukleotider ("CpG") er kjent innen teknikken som adjuvansmidler når de tilføres både på systemisk og mukosal måte (WO 96/02555, EP 468520, Davis et al, J. Immunol, 1998, 160(2):870-876; McCluskie og Davis, J. Immunol, 1998, 161(9):4463-6). CpG er en forkortelse for cytosin-guanosin-dinukleotidenhetene tilstede i DNA. Historisk ble det iakttatt at DNA-fraksjonen BCG kunne utøve en antitumorvirkning. I ytterligere under-søkelser ble syntetiske oligonukleotider avledet fra BCG-gensekvenser vist å være i
stand til å indusere immunstimulatoriske virkninger (både in vitro og in vivo). Forfatter-ne av disse undersøkelsene konkluderte med at visse palindromiske sekvenser, inklusive en sentral CG-enhet, bar denne aktiviteten. Den sentrale rollen av CG-enheten i immun-stimulasjon ble senere belyst i en publikasjon av Krieg, Nature 374, s. 546, 1995. Detaljert analyse har vist at CG-enheten må være i en bestemt sekvenssammenheng, og at
slike sekvenser er vanlige i bakterielt DNA, men er sjeldne i virveldyr-DNA. Den im-munostimulatoriske sekvensen er ofte: Purin, Purin, C, G, pyrimidin, pyrimidin; hvori den dinukleotide CG-enheten ikke er metylert, men andre umetylerte CpG-sekvenser er kjent å være immunstimulatoriske og kan anvendes i den foreliggende oppfinnelse.
I visse kombinasjoner av de seks nukleotidene er en palindromisk sekvens tilstede. Flere av disse enheter, enten som "repeats" av én enhet eller en kombinasjon av forskjellige enheter, kan være tilstede i det samme oligonukleotidet. Nærværet av ett eller flere av disse imunnstimulatoriske sekvensholdige oligonukleotider kan aktivere forskjellige immun-"subset", inklusive naturlige dreperceller (som produserer interferon y og har cytolytisk aktivitet) og makrofager (Wooldrige et al, vol. 89 (nr. 8), 1977). Andre urne- tylerte CpG-inneholdende sekvenser som ikke har denne konsensus-sekvens har likevel nå vist seg å være immunmodulatoriske.
Når CpG er sammensatt i vaksiner blir det generelt tilført i fri oppløsning sammen med fritt antigen (WO 96/02555; McCluskie og Davis, supra) eller kovalent konjugert til et antigen (PCT-publikasjon nr. WO 98/16247), eller sammensatt med en bærer slik som aluminiumhydroksid ((Hepatitis surface antigen) Davis et al, supra; Brazolot-Millan et al, Proc. NatlAcad. Sci., USA, 1998, 95(26), 15553-8).
Saponiner omtales i: Lacaille-Dubois, M. og Wagner H. (1996, A review of the biologi-cal and pharmacological activities of saponins. Phytomedicine vol. 2 s. 363-386). Saponiner er steroid- eller triterpenglykosider som er vidt og bredt forekommende innenfor plante- og marine dyreriker. Saponiner er kjent for å danne kolloidale oppløsninger i vann som skummer ved omrysting og for utfelling av kolesterol. Når saponiner er nær cellemembraner, skaper de poreliknende strukturer i membranet som bevirker at membranet brytes. Hemolyse av erytrocytter er et eksempel på dette fenomen som er en egenskap for visse, men ikke alle, saponiner.
Saponiner er kjent som adjuvantmidler i vaksiner for systemisk tilførsel. Adjuvanten og hemolytisk aktivitet av individuelle saponiner er blitt grundig studert innen dette feltet (Lacaille-Dubois og Wagner, supra). For eksempel er Quil A (avledet fra barken av det sydamerikanske treet Quillaja Saponaria Molina) og fraksjoner derav, beskrevet i US 5.057.540 og "Saponins as vaccine adjuvants", Kensil, C. R., Cr it Rev. Ther. Drug Carrier Syst., 1996, 12 (l-2):l-55; og i EP 0.362.279 Bl.
Spesielle strukturer, benevnt immunstimulerende komplekser ("Immune Stimulating Complexes (ISCOMs)") omfattende fraksjoner av Quil A, er hemolytiske og har vært anvendt ved fremstilling av vaksiner (Morein, B., EP 0.109.942 Bl). Disse strukturene er rapportert å ha adjuvansaktivitet (EP 0.109.942 Bl; WO 96/11711).
De hemolytiske saponinene QS21 og QS17 (HPLC-rensede fraksjoner av Quil A) er blitt beskrevet som potente systemiske adjuvansmidler, og metoden for deres fremstil ling er omhandlet i US-patent nr. 5.057.540 og EP 0.362.279 Bl. I disse referanser er også beskrevet anvendelsen av QS7 (en ikke-hemolytisk fraksjon av Quil A) som virker som et potent adjuvansmiddel for systemiske vaksiner. Spesielle adju-vansmiddelsystemer omfattende fraksjoner av Quil A, som QS21 og QS27, er beskrevet i WO 96/33739 og WO 96/11711.
Andre saponiner som har vært anvendt i systemiske vaksinasjonsstudier inkluderer dem som er avledet fra andre plantearter som Gypsophila og Saponaria (Bomford et al, Vaccine, 10(9):572-577, 1992).
Saponiner er også kjent å ha vært anvendt i mukosalt påførte vaksinestudier som har oppnådd variabel suksess i induksjonen av immunresponser. Quil A saponin er tidligere blitt vist ikke å ha noen virkning på induksjonen av en immunrespons når antigen tilfø-res intranasalt (Gizurarson et al, 1994. Vaccine Research 3, 23-29). Mens andre forfat-tere har anvendt dette adjuvanset med hell (Maharaj et al, Can. J. Microbiol, 1986, 32(5):414-20; Chavali og Campbell, Immunobiology, 174(3):347-59). ISCOMs (immunstimulerende komplekser) omfattende Quil A-saponin, har vært anvendt i intra-gastriske og intranasale vaksinesammensetninger, og fremviste adjuvansaktivitet (Mel Mowat et al, 1991, Immunology, 72, 317-322; Mel Mowat og Donachie, Immunology Today, 12,383-385).
QS21, den ikke-giftige fraksjon av Quil A, er også blitt beskrevet som et oralt eller intranasalt adjuvansmiddel (Sumino et al, J. Virol, 1998, 72(6):4931-9; WO 98/56415).
Kombinasjon av QS21 og CpG er videre vist å resultere i en synergistisk samvirkning når formulert i vandig løsning (Kensil et al; abstract 218 fra Keystone Symposia, Mole-cular and Cellular Biology, 12-17; april 1999).
Anvendelsen av andre saponiner i intranasale vaksinasjonsstudier er blitt beskrevet. For eksempel har Chenopodium^w/woa-saponiner vært anvendt i både intranasale og intra- gastriske vaksiner (Estrada et al, Comp. Immunol. Microbiol. Infect. Dis., 1998,21(3): 225-36).
Den foreliggende oppfinnelse vedrører det overraskende funn at immunstimulatoriske oligonukleotider (CpG) og saponinkombinasjoner er ekstremt potente adjuvansmidler. Det tilveiebringes følgelig en adjuvanssammensetning omfattende en kombinasjon av saponin og et immunstimulatorisk oligonukleotid. Foretrukket kan adjuvansmidlene ifølge den foreliggende oppfinnelse ytterligere omfatte en bærer. I en foretrukket form av den foreliggende oppfinnelse virker saponinet og oligonukleotidene i adjuvansmidlet og vaksineblandingene synergistisk til induksjon av antigenspesifikt antistoff og er potente i induksjonen av immunresponser som konvensjonelt assosieres med Thl-type-immunsystemet. Følgelig er adjuvanssammensetningene ikke bare egnet for immu-noprofylakse av sykdommer, men også overraskende for immunoterapi av sykdommer som hårdnakkede virale, bakterielle eller parasittiske infeksjoner, og også kroniske lidelser som cancer.
De foretrukne oligonukleotider for anvendelse i adjuvanssammensetninger eller vaksiner ifølge foreliggende oppfinnelse, inneholder fortrukket to eller flere dinukleotid-CpG-enheter separert med minst tre, mer foretrukket med minst seks eller flere nukleotider. Oligonukleotidene ifølge den foreliggende oppfinnelse er typisk deoksynukleotider. I en foretrukket utførelsesform er internukleotidet i oligonukleotidet fosforoditioat, eller mer foretrukket en fosforotioatbinding, selv om fosfodiester- og andre internukleotidbindinger er innenfor rammen for oppfinnelsen inklusive oligonukleotider med blandede internukleotidbindinger. Metoder for fremstilling av fosfortioat-oligonukleotider eller fosforditioat er beskrevet i US 5.666.153, US 5.278.302 og WO95/26204.
Eksempler på foretrukne oligonukleotider har de følgende sekvenser. Sekvensene inneholder de foretukne fosfortioatrnodifiserte internukleotidbindinger.
Alternative CpG-oligonukleotider kan omfatte de foretrukne ovenstående sekvenser ved at de har inkonsekvensmessige delesjoner eller addisjoner dertil.
CpG-oligonukleotidene anvendt i den foreliggende oppfinnelse kan syntetiseres ved hjelp av en hvilken som helst metode kjent på området (f.eks. EP 468520). Passende kan slike oligonukleotider syntetiseres ved bruk av en automatisert syntetisator.
Oligonukleotidene anvendt i den foreliggende oppfinnelse er typisk deoksynukleotider. I en foretrukket utførelsesform er internukleotidbindingen i oligonukleotidet fosforditioat- eller mer foretrukket fosfortioatbinding, selv om fosfodiestere er innenfor rammen av den foreliggende oppfinnelse. Oligonukleotid omfattende forskjellige internukleotidbindinger er medomfattet, f.eks. blandede fosfortioatfosfodiestere. Andre internukleotidbindinger som stabiliserer oligonukleotidet kan anvendes.
Saponinene som kan anvendes i adjuvanskombinasjonene ifølge den foreliggende oppfinnelse, inkluderer dem som er avledet fra barken av Quillaja Saponaria Molina, benevnt Quil A, og fraksjoner derav, beskrevet i US-patent 5.057.540 og "Saponins as vaccineadjuvants", Kensil, C. R., CritRev TherDrug Carrier Syst., 1996,12 (1-2): 1-55; og EP 0.362.279 Bl. Særlig foretrukne fraksjoner av Quil A er QS21, QS7 og QS17.
P-escin er et ytterligere foretrukket hemalytisk saponin for anvendelse i adjuvansmiddelblandingene ifølge den foreliggende oppfinnelse. Escin er beskrevet i Merck index (12. utgave; stikkord 3737) som en blanding av saponiner som forekommer i barken til hestekastanjetreet, latin: Aesculus hippocastanum. Dets isolasjon er beskrevet ved hjelp av kromatografi og rensing (Fiedler, Arzneimiddet- Forsch. 4,213 (1953)), og ved hjelp av ionebytteharpikser (Erbring et al, US-patent 3.238.190). Fraksjoner av escin, a og P, er blitt renset og vist å være biologisk aktive (Yoshikawa M., et al, { Chem Pharm Bull (Tokyo) 1996, aug; 44(8): 1454-1464)). P-escin er også kjent som aescin.
Et ytterligere foretrukket hemolytisk saponin for anvendelse i den foreliggende oppfinnelse er Digitonin. Digitonin er beskrevet i Merck index (12. utgave, stikkord 3204) som et saponin avledet fra frøene av Digitalis purpurea og renset ifølge prosedyren som er beskrevet av Gisvold et al, JAm. Pharm. Assoc, 1934, 23, 664; og Ruhenstroth-Bauer, PhysiolChem., 1955,301,621. Dets anvendelse er beskrevet som et klinisk rea-gens for kolesterolbestemmelsen.
Adjuvansmiddelkombinasjonene ifølge den foreliggende oppfinnelse kan ytterligere omfatte en bærer, slik at saponinet eller CpG, eller begge, kan være assosiert med en spesiell bærerenhet for å forsterke adjuvansiteten av kombinasjonen. Særlig foretrukne systemiske vaksiner omfatter for eksempel et bærermolekyl.
CpG anvendt i adjuvanskombinasj onene ifølge den foreliggende oppfinnelse kan være i fri oppløsning eller kan være kompleksert til partikkelformede bærere som for eksempel mineralsalter (for eksempel, men ikke begrenset til, aluminium eller kalsiumsalter), liposomer, ISCOMs, emulsjoner (olje-i-vann, vann-i-olje-i vann), polymerer (som for eksempel, men ikke begrenset til) polymelkesyre, polyglykolsyre, polyfosfazin, poly-aminosyre, alginat, chitosan) eller mikropartikler. Foretrukket er de nevnte bærere kati-oniske. Vaksinene ifølge den foreliggende oppfinnelse kan ytterligere omfatte et antigen som kan være assosiert med CpG-bærerkomplekset, eller behøver ikke å være assosiert med CpG-bærerkomplekset. I dette tilfellet kan antigenet være i fri suspensjon eller assosiert med en separat bærer.
Saponinene som utgjør en del av den foreliggende oppfinnelse kan være separate i form av miceller, eller kan være i form av store ordnede strukturer som for eksempel ISCOMs (EP 0.019.942 Bl) eller liposomer (WO 96/33739) når de sammensettes med kolesterol og lipid, eller i form av en olje-i-vann-emulsjon (WO 95/17210). Saponinene kan foretrukket være assosiert med et metallsalt, som for eksempel aluminiumhydroksid eller aluminiumfosfat (WO 98/15287). Alternativt kan saponinet være assosiert med en partikkelformet bærer som for eksempel chitosan. Saponinet kan også være i en tørr tilstand som for eksempel et pulver. De endelige sammensetninger i den form hvor de tilføres til den mukosale overflaten av individet som vaksineres, er foretrukket av hemolytisk karakter. Saponinet kan eventuelt være fysisk assosiert med antigenet enten ved direkte binding eller ved kointeraksjon med det samme partikkelformede bærermoleky-let (GB 9822712.7; WO 98/16247). CpG og saponinet i adjuvansmidlene eller vaksinene ifølge den foreliggende oppfinnelse kan i seg selv være separate eller assosierte. For eksempel kan CpG og saponinet være i fri suspensjon eller kan være assosiert via en bærer, mer foretrukket en partikkelformet bærer som for eksempel aluminiumhydroksid eller ved hjelp av et kationisk liposom eller ISCOM.
En foretrukket adjuvansmiddelkombinasjon ifølge den foreliggende oppfinnelse er sammensatt av ett eller flere CpG-oligonukleotider inneholdende minst 3, foretrukket minst 6, nukleotider mellom to tilstøtende CG-enheter sammen med QS21 og en partikkelformet bærer valgt fra gruppen bestående av en olje-i-vann-emulsjon eller DQ. Mest foretrukket omfatter adjuvanskombinasjonen CpG 2006 (SEQ ID NO:4), eller CpG 1758 (SEQ ID: 2) eller CpG 1826 (SEQ ID NO: 1) blandet med QS21 og en partikkelformet bærer valgt fra gruppen bestående av en olje-i-vann-emulsjon eller DQ. Følgelig omfatter spesielt foretrukne vaksiner for eksempel slike adjuvanskombinasjoner og et antigen. Den foretrukne vaksine ifølge den foreliggende oppfinnelse anvendes for å ge-nerere systemiske immunresponser etter tilførsel til et individ via den systemiske tilfør-sel.
Adjuvanskombinasj onene ifølge den foreliggende oppfinnelse kan anvendes som både systemisk og mukosalt adjuvansmiddel. I en spesiell form av oppfinnelsen tilveiebringes en systemisk vaksine for tilførsel på systemisk eller parenteral måte som for eksempel intramuskulær, intradermal, transdermal, subkutan, intraperitoneal eller intravenøs tilførsel. En foretrukket tilførselsmåte er via transdermal tilførsel, for eksempel ved hjelp av hudplastere.
De systemiske vaksinepreparater ifølge den foreliggende oppfinnelse kan anvendes for å beskytte eller behandle et pattedyr som er mottagelig for eller lider av sykdom, ved hjelp av tilførsel av nevnte vaksine ved hjelp av intramuskulær, intraperitoneal, intradermal, transdermal, intravenøs eller subkutan tilførsel. Metoder for systemisk tilførsel av vaksinepreparatene kan inkludere konvensjonelle sprøyter og nåler eller anordninger konstruert for ballistisk tilførsel av faste vaksiner (WO 99/27961) eller nåleløse væske-trykkstråleanordninger (US-patentene 4.596.556 og 5.993.412), eller transdermale plastere (WO 97/48440; WO 98/28037). Den foreliggende oppfinnelse kan også anvendes for å forsterke immunogenisiteten av antigener påført huden (transdermal eller transkutan tilførsel WO 98/20734; WO 98/28037). Den foreliggende søknad beskriver derfor en tilførselsanordning for systemisk tilførsel, forhåndsfylt med vaksinen eller adjuvansblandingene ifølge den foreliggende oppfinnelse.
Alternativt kan vaksinepreparatene ifølge den foreliggende oppfinnelse anvendes for å beskytte eller behandlet et pattedyr som er mottagelig for, eller lider av sykdom, ved hjelp av tilførsel av nevnte vaksine via en mukosal måte, som for eksempel oral/nær-ingsmiddel eller nasal måte. Alternative mukosalmåter er intravaginal og intrarektal. Den foretrukne mukosale tilførselsmåte er via den nasale måte, benevnt intranasal vak sinasjon. Metoder for intranasal vaksinasjon er velkjent på området, inklusive tilførselen av en dråpe-, spray- eller tørr pulverform av vaksinen i nese-svelg av individet som skal immuniseres. Forstøvede eller aerosoliserte vaksinesammensetninger utgjør også en del av denne oppfinnelsen. Enteriske sammensetninger som for eksempel gastro-resistente kapsler og granuler for oral tilførsel, stikkpiller for rektal eller vaginal tilførsel, utgjør også en del av denne oppfinnelsen.
Adjuvanssammensetningene ifølge den foreliggende oppfinnelse representerer en klasse av mukosale adjuvansmidler egnet for tilførsel i mennesker for å erstatte systemisk vaksinasjon ved mukosal vaksinasjon. I en foretrukket form av den foreliggende oppfinnelsen kan rene saponiner som for eksempel Quil A eller derivater derav, inklusive QS21; Escin, Digitonin; eller Gypsophila eller Cehnopodium^w/woa-saponiner anvendes i kombinasjon med immunstimulatoriske oligonukleotider som adjuvansmidler for mukosal tilførsel av antigener for å oppnå en systemisk immunrespons.
Adjuvansmiddelkombinasjonene ifølge den foreliggende oppfinnelse anvendes ved sammensetningen av vaksiner, idet disse vaksiner kan tilføres via den systemiske eller mukosale måte. Foretrukket, når vaksinene anvendes for mukosal tilførsel, omfatter adjuvansmiddelkombinasjonen et hemolytisk saponin.
For mukosal tilførsel omfatter blandingen ifølge oppfinnelsen foretrukket et hemolytisk saponin. Hemolytisk saponin, eller saponinpreparat, innenfor forståelsen av den foreliggende oppfinnelse bestemmes med henvisning til det følgende forsøk. 1. Friskt blod fra marsvin vaskes med fosfatbuffret saltoppløsning (PBS) 3 ganger i en benkesentrifuge. Etter resuspensjon til det opprinnelige volum fortynnes blodet videre 10 ganger i PBS. 2. 50 ul av denne blodsuspesnjonen tilsettes i 800 ul PBS inneholdende to gangers fortynninger av overflateaktivt middel eller saponin. 3. Etter 8 timer bedømmes hemolysen visuelt eller ved å måle den optiske densite-ten av supernatanten. Nærværet av en rød supernatant, som absorberer lys ved 570 nm indikerer nærvåret av hemolyse. 4. Resultatene uttrykkes som konsentrasjonen av den første saponinfortynning hvor hemolyse ikke lenger forgår.
For denne oppfinnelsens formål er saponin-adjuvanspreparatet hemolytisk hvis det lyse-rer erytrocyttene ved en konsentrasjon på mindre enn 0,1%. Som referansemidler er hovedsakelig rene prøver av Quil A, QS21, QS7, Digitonin og p-escin, alle hemolytiske saponiner som definert for dette forsøk. Innenfor den iboende forsøksvariabiliteten av en slik biologisk analyse, har saponinene ifølge den foreliggende oppfinnelse foretrukket en hemolytisk aktivitet på omtrent mellom 0,5-0,00001%, mer foretrukket mellom 0,05-0,00001%, ennå mer foretrukket mellom 0,005-0,00001%, og mest foretrukket mellom 0,011-0,004%. Ideelt bør saponinene ha en hemolytisk aktivitet likt (dvs. innenfor en tidobbelt forskjell) som for QS21.
Vaksinene ifølge den foreliggende oppfinnelse kan også tilføres via den orale måte. I slike tilfeller kan det farmasøytisk tålbare eksipientmiddel også inkludere alkaliske buf-fere eller enteriske kaplser eller mikrogranuler. Vaksinene ifølge den foreliggende oppfinnelse kan også tilføres via den vaginale måte. I disse tilfeller kan de farmasøytisk tålbare eksipiensmidler også inkludere emulgeringsmidler, polymerer som for eksempel "CARBOPOL" og andre kjente stabiliseirngsmidler i vaginale kremer og stikkpiller. Vaksinene ifølge den foreliggende oppfinnelse kan også tilføres via den rektale måte. I slike tilfeller kan eksipiensmidlene også inkludere voksarter og polymerer kjent på området for tildannelse av rektale stikkpiller.
Preparater av mer enn ett saponin i adjuvansmiddelkombinasjonene ifølge den foreliggende oppfinnelse, er også en del av den foreliggende oppfinnelse. For eksempel kombinasjoner av minst to av den følgende gruppe omfattende QS21, QS7, Quil A, P-escin eller digitonin. Ytterligere kan blandingen ifølge den foreliggende oppfinnelse omfatte kombinasjoner av mer enn ett immunstimulatorisk oligonukleotid.
I en liknende utførelsesform av den foreliggende oppfinnelse kan CpG/saponinkom-binasjonene for både systemisk og mukosal tilførsel ytterligere kombineres med andre adjuvansmidler inklusive monofosforyl lipid A og dets ikke-giftige derivat 3-de-O-acylert monofosforyl-lipid A. Alternativt kan saponinsammensetningen kombineres med vaksinekonstituenser sammensatt av chitosan eller andre polykationiske polymerer, polylaktid- og polylaktid-ko-glykolidpartikler, poly-N-acetylglukosaminbasert poly-mermatriks, partikler sammensatt av polysakkarider eller kjemisk modifiserte polysakkarider, liposomer og lipidbaserte partikler, partikler sammensatt av glyserolmonoestere etc. Saponinene kan også sammensettes i nærvær av kolesterol for å danne partikkelformede strukturer som for eksempel liposomer eller ISCOMs. Videre kan saponinene sammensettes med en polyoksyetyleneter eller -ester, i enten en ikke-partikkelformet oppløsning eller suspensjon, eller i en partikkelformet struktur som for eksempel pauci-lamellær liposom eller ISCOM. Saponinene kan også sammensettes med eksipiensmid ler som for eksempel "Carbopol" for å øke viskositeten, eller kan sammensettes i en tørr pulverform med et pulvereksipientmiddel som for eksempel laktose. 3 De-O-acylert monofosforyl-lipid A er et velkjent adjuvansmiddel som fremstilles av Ribi Immunochem, Montana. Det kan fremstilles ved hjelp av metoder som omhandles i GB 2122204B. En foretrukket form av 3 De-O-acylert monofosforyl lipid A er i form av en emulsjon med en liten partikkelstørrelse mindre enn 0,2 nm i diameter (EP 0.689.454 Bl). Særlige foretrukne adjuvansmidler er kombinasjonsmidler av 3D-MPL og QS21 (EP 0.671.948 Bl), olje-i-vann-emulsjoner omfattende 3D-MPL og QS21 (WO 95/17210, WO 98/56414) eller 3D-MPL sammensatt med andre bærere (EP 0.689.454 Bl).
Foretrukket inneholder vaksinesammensetningene ifølge den foreliggende oppfinnelse et antigen eller en antigenisk blanding istand til å utløse en immunrespons mot et humant patogen, idet antigenet eller den antigeniske blanding er avledet fra HIV-1, (som f.eks. tat, nef, gpl20 eller gpl60), humane herpes virus, som for eksempel gD eller derivater derav eller "Immediate Early Protein" som for eksempel ICP27 fra HSV1 eller HSV2, cytomegalovirus ((særlig Human) som for eksempel gB eller derivater derav), Rotaviurs (inklusive livsvekkede virus), Epstein Barr-virus (som for eksempel gp350 eller derivater derav), Varicella Zoster-virus (som f.eks. gpl, II og IE63), eller fra et hepatitt-virus som for eksempel hepatitt B-virus (for eksempel hepatitt B-overflateantigen eller et derivat derav), hepatitt A-virus, hepatitt C-virus og hepatitt E-virus, eller fra andre virale patogener, som for eksempel paramyxovirus: Respiratorisk syncytialt virus (RSV) (som for eksempel F- og G-proteiner eller derivater derav), parainfluensa-virus, mesling-virus, kusma-virus, humane papillom-virus (for eksempel HPV6,11,16, 18,...), flavivirus (f.eks. gulfebervirus, Dengue-virus, flått-båret encefalitt-virus, ja-pansk encefalitt-virus) eller influensa-virus (helt levende eller inaktivert virus, splittin-fluensa-virus, dyrket i egg eller MDCK-celler), eller hele influensavirosomer (som beskrevet av R. Andre foretrukne spesifikke antigener for M. tuberculosis er for eksempel Tb Ral2, Tb H9, Tb Ra35, Tb38-1, Erd 14, DPV, MTI, MSL, mTTC2 og hTCCl (WO 99/51748). Proteiner for M. tuberculosis inkluderer fusjonsproteiner og varianter derav hvor minst to, foretrukket tre, polypeptider av M. tuberculosis er fusjonert til et større protein. Foretrukne fusjoner inkluderer Ral2-TbH9-Ra35, Erdl4-DPV-MTI, DPV-MTI-MSL, Erdl4-DPV-MTI-MSL-mTCC2, Erdl4-DPV-MTI-MSL, DPV-MTI-MSL, DPV-MTI-MSL-mTCC2, TbH9-DPV-MTI (WO 99/51748).
Mest foretrukne antigener for Chlamydia inkluderer for eksemepel høymolekylvekts-proteinet (HWMP) (WO 99/17741), ORF3 (EP 366.412) og antatte membranproteiner ("Putative Membrane Proteins" (Pmps). Andre Chlamydia-antigener i vaksinesammensetningen kan selekteres fra gruppen beskrevet i WO 99/28475.
Foretrukne bakterielle vaksiner omfatter antigener avledet fra Streptococcus spp, inklusive S. pneumoniae (for eksempel kapsulære polysakkarider og konjugater derav, PsaA, PspA, streptolysin, cholinbindende proteiner) og proteinantigenet Pneumolysin (Bio-chem Biophys Acta, 1989, 67,1007; Rubins et al, Microbial Pathogenesis, 25, 337-342), og mutant detoksifiserte derivater derav (WO 90/06951; WO 99/03884). Andre foretrukne bakterielle vaksiner omfatter antigener avledet fra Haemophilus spp., inklusive H. influenzae type B (for eksempel PRP og konjugater derav), ikke-typbar H. influenzae, for eksempel OMP26, høymolekylvektadhesiner, P5, P6, protein D og lipopro-tein D, og fimbrin og fimbrinavledede peptider (US-patent 5.843.464) eller multiple kopivarianter eller fusjonsproteiner derav.
Derivater av hepatitt B-overflateantigen er velkjent på området og inkluderer bl.a. de PreSl, PreS2 S-antigener som er anført beskrevet i europeiske patentsøknader EP-A-414.374; EP-A-0304.578 og EP 198-474.1 ett foretrukket aspekt omfatter vaksinesammensetningen ifølge oppfinnelsen HIV-1-antigenet, gpl20, særlig når det uttrykkes i CHO-celler. I en ytterligere utførelsesform omfatter vaksinesammensetningen ifølge oppfinnelsen gD2t som definert i det foregående.
I en foretrukket utførelsesform av den foreliggende oppfinnelse omfatter vaksiner inneholdende det angitte adjuvansmiddel antigen avledet fra human papillomavirus (HPV), som er ansett å være ansvarlig for kjønnsvorter (HPV 6 eller HPV 11 og andre), og HPV-virus ansvarlige for cervikal cancer (HPV 16, HPV 18 og andre).
Spesielt foretrukne former av kjønnsvorte-profelaktisk eller terapeutisk vaksine, omfatter LI-partikler eller kapsomerer, og fusjonsproteiner omfattende ett eller flere antigener valgt fra HPV 6-og HPV 11-proteinene E6, E7, LI og L2.
De mest foretrukne former av fusjonsprotein er: L2E7 som omskrevet i WO 96/26277 og protein D (l/3)-E7 beskrevet i GB 9717953.5 (PCT/EP98/05285).
En foretrukket HPV cervikal infeksjons- eller cancervaksineblanding for profylakse eller terapi, omfatter HPV 16 eller -18 antigener. For eksempel, LI eller L2 antigenmo-nomerer, eller LI eller L2-antigener forekommende sammen som en virusliknende partikkel (VLP) eller LI proteinet tilstede alene i en VLP eller kapsomerstruktur. Slike antigener, virusliknende partikler og kapsomerer er i og for seg kjent. Se for eksempel WO 94/00152, WO 94/20137, WO 94/05792 og WO 93/02184.
Ytterligere tidlige proteiner kan inkluderes alene eller som fusjonsproteiner som for eksempel E7, E2 eller foretrukket E5; særlig foretrukne utførelsesformer av dette inkluderer et VLP omfattende LlE7-fusjonsproteiner (WO 96/11272).
Særlig foretrukne HPV 16-antigener omfatter de tidlige proteiner E6 eller E7 i fusjon med en protein D-bærer for å danne protein D - E6- eller E7-fusjoner fra HPV 16 eller kombinasjoner derav; eller kombinasjoner av E6 eller E7 med L2 (WO 96/26277).
Alternativt kan HPV 16 eller 18 tidlige proteiner E6 og E7 forekomme i et enkelt molekyl, foretrukket en protein D- E6/E7-fusjon. Slik vaksine kan eventuelt inneholde enten den ene eller begge av E6- og E7-proteiner fra HPV 18, foretrukket i form av et protein D - E6 eller protein D - E7-fusjonsprotein eller protein D E6/E7-fusjonsprotein.
Vaksinen ifølge den foreliggende oppfinnelsen kan ytterligere omfatte antigener fra andre HPV-stammer, foretrukket fra stammene HPV 31 eller 33.
Vaksiner ifølge den foreliggende oppfinnelse omfatter videre antigener avledet fra parasitter som bevirker malaria. For eksempel inkluderer foretrukne antigener fra Plasmodia falciparum RTS,S og TRAP. RTS er et hybrid protein omfattende hovedsakelig hele den C-terminale delen av circumsporozoitt (CS) proteinet fra P. falciparum knyttet via fire aminosyrer av preS2-delen av hepatitt B-overflateantigenet til overflate (S) antigenet av hepatitt B-virus. Dets fulle struktur er beskrevet i den internasjonale patentsøkna-den nr. PCT/EP92/02591, publisert under nummer WO 93/10152, som krever prioritet fra UK-patentsøknad nr. 9124390.7. Når det uttrykkes i gjær, produseres RTS som en lipoproteinpartikkel, og når det ko-uttrykkes med S-antigenet fra HBV, produserer det en blandet partikkel kjent som RTS,S. TRAP-antigener er beskrevet i den internasjonale patentsøknaden nr. PCT/GB89/00895, publisert under WO 90/01496. En foretrukket utførelsesform av den foreliggende oppfinnelse er en malaria-vaksine hvori det antigeniske preparat omfatter en kombinasjon av RTS,S og TRAP-antigener. Andre plasmodia-antigener som sannsynlige kandidater til å være komponenter av en flertrinns malaria-vaksine, er P. falciparum MSP1, AMA1, MSP3, EBA, GLURP, RAPI, RAP2, Se-guestrin, PfEMPl, Pf332, LSA1, LSA3, STARP, SALSA, PfEXPl, Pfs25, Pfs28, PFS27/25, Pfsl6, Pfs48/45, Pfs230 og deres analoger i Plasmodium spp.
Sammensetningene kan også inneholde et antitumorantigen og være nyttige for immun-terapeutisk behandling av cancere. For eksempel finner adjuvantsmiddelsammenset-ningen anvendelse med tumoravstøtningsantigener som for eksempel slike for prostata-, bryst-, kolorektal-, lunge-, pankreas-, renal- eller melanomcancere. Eksempelvise antigener inkluderer MAGE 1 og MAGE 3 eller andre MAGE-antigener (for behandlingen av melanom), PRAME, BAGE eller GAGE (Robbins og Kawakami, 1996, Current opi-nions in Immunology 8, s. 628-636; Van den Eynde et al, International Journal of Cli-nical & Laboratory Researc (avgitt 1997); Correale et al. (1997), Journal of the National Cancer Institute 89, s. 293. Disse antigener uttrykkes faktisk i et bredt område av tumor-typer som melanom, lungekarcinom, sarkom og blærekarcinom. Andre tumorspesifikke antigener er egnet for anvendelse med adjduvantsmidlene ifølge den foreliggende oppfinnelse og inkluderer, men er ikke begrenset til, tumorspesifikke gangliosider. Prostataspesifikt antigen (PSA) eller Her-2/neu, KSA (GA733), PAP, mammaglobin, MUC-1, karcinoembryonisk antigen (CEA). I ett aspekt av den foreliggende oppfinnelsen tilveiebringes følgelig en vaksine omfattende en adjuvansmiddelblanding ifølge oppfinnelsen og et tumoravstøtningsantigen.
I et spesielt foretrukket aspekt av den foreliggende oppfinnelse omfatter vaksinene et tumorantigen; slike vaksiner er overraskende potente i terapien av cancer, som for eksempel prostata-, bryst-, kolorektal-, lunge-, pankreas-, renal- ovarie- eller melanomcancere. Følgelig kan sammensetningene inneholde tumorassosiert antigen så vel som antigener assosiert med tumorunderstøttende mekanismer (f.eks. angiogenese, tumorin-vasjon). Følgelig omfatter antigener særlig relevante for vaksiner i terapi av cancer også prostataspesifikt membranantigen (PSMA), prostata-stamcelle-antigen (PSCA), tyrosinase, survivin, NY-ESOl, prostase, PS108 (WO 98/50567), RAGE, LAGE, HAGE. Ytterligere kan det nevnte antigen være et selv-peptidhormon slik som full-lengde- gonadotrofinhormonfrigivende hormon (GnRH, WO 95/20600), et kort 10 aminosyrers langt peptid, nyttig ved behandling av mange cancere, eller i immunkastrasjon.
Det er forutsett at blandinger ifølge den foreliggende oppfinnelse vil bli anvendt for å sammensette vaksiner inneholdende antigener avledet fra Borrelia sp.. For eksempel kan antigener inkludere nukleinsyre, patogenavledet antigen eller antigeniske preparater, rekombinant produsert protein eller peptider og kimæriske fusjonsproteiner. Spesielt er antigenet OspA. OspA kan være et fullt utviklet protein i en lipidert form basert på vertcellen ( E. coli) betegnet (Lipo-OspA) eller et ikke-lipidert derivat. Slike ikke-lipiderte derivater inkluderer det ikke-lipiderte NS 1-Osp A-fusjonsproteinet som har de første 81 N-terminale aminosyrer av det ikke-strukturelle proteinet (NS1) av influensaviruset, og det fullstendige OspA-proteinet, og et ytterligere MDP-OspA er en ikke-lipidert form av OspA som bærer 3 ytterligere N-terminale aminosyrer.
Vaksiner ifølge den foreliggende oppfinnelse kan anvendes for forebygging eller terapi av allergi. Slike vaksiner ville omfatte allergenspesifikke (for eksempel Der pl) og al-lergen-ikke-spesifikke antigener (for eksempel peptider avledet fra human IgE, inklusive men ikke begrenset til, stanworth dekapeptidet (EP 0.477.231 Bl)).
Vaksiner ifølge den foreliggende oppfinnelse kan også anvendes for forebygging eller terapi av andre kroniske lidelser enn allergi, cancer eller smittsomme sykdommer. Slike kroniske lidelser er sykdommer som for eksempel aterosklerose og Alzheimer's sykdom.
Antigener som er relevante for forebyggingen og terapien av pasienter som er mottage-lige for, eller lider av, Alzheimer neurodegenerativ sykdom, er spesielt det N-terminale 39-43 aminosyrefragmentet (Ap*) av amyloidforløperproteinet og mindre fragmenter. Dette antigenet er omhandlet i den internasjonale patentsøknaden W099/27944 (Athena Neurosciences).
Mengden av protein i hver vaksinedose er valgt som en mengde som induserer en im-munprotektiv respons uten signifikante, skadelige bivirkninger i typiske vaksinerte individer. Denne mengden vil variere avhengig av hvilket spesifikt immunogen som anvendes og hvorledes dette frembys. Generelt er det ventet at hver dose vil omfatte 1-1000 ug protein, foretrukket 1-500 ug, foretrukket 1-100 ug, mest foretrukket 1 til 50 ug. En optimal mengde for en spesiell vaksine kan fastslås ved hjelp av standardstudier som innebærer observasjon av passende immunresponser i vaksinerte individer. Etter en initiell vaksinasjon kan individer motta én eller flere vedlikeholdsimmunisasjoner med tilstrekkelig avstand fra hverandre. En slik vaksinesammensetning kan påføres en mukosal overflate i et pattedyr i enten et initierende eller vedlikeholdende vaksinasjonsre-gime; eller kan alternativt tilføres systemisk, for eksempel via de transdermale, subku-tane eller intramuskulære måter.
Mengden av CpG eller immunstimulatoriske oligonukleotider i tilsetningsmidlene eller vaksinene ifølge den foreliggende oppfinnelse er generelt liten, men avhengig av vaksinesammensetningen kan mengden være i området fra 1-100 ug pr. dose, foretrukket 1-500 ug pr. dose, og mer foretrukket mellom 1 til 100 ug pr. dose.
Mengden av saponin for anvendelse i tilsetningsmidlene ifølge den foreliggende oppfinnelse kan være i området fra 1-1000 ug pr. dose, foretrukket 1 til 500 ug pr. dose, mer foretrukket 1-250 fig pr. dose, og mest foretrukket mellom 1 til 100 fig. Forholdet mellom CpG:saponin (vekt/vekt) vil derfor være i området 1:1000 til 1000:1, og vil typisk være i området fra 1:100 til 100:1, og foretrukket i området fra 1:10 til 1:1 til 10:1, og mest foretrukket 1:1,4:1 eller 10:1.
Sammensetningene ifølge den foreliggende oppfinnelsen kan anvendes for både fore-byggende og terapeutiske formål. Følgelig tilveiebringes anvendelsen av en kombinasjon av et saponin og et CpG-moleyl ved produksjon av en vaksine for forebygging og behandling av virale, bakterielle, parasittiske infeksjoner, allergi, cancer og andre ikke-kroniske lidelser. I et ytterligere aspekt av den foreliggende oppfinnelse tilveiebringes en vaksine eller adjuvansmiddelkombinasjon, omfattende et saponin og CpG, som heri beskrevet for anvendelse som et legemiddel. Vaksinefremstilling er generelt beskrevet i New Trends and Developments in Vaccines, utgitt av Voller et., University Park Press, Baltimore, Maryland, USA, 1978.
Det er forutsatt at blandinger ifølge foreliggende oppfinnelse vil bli anvendt for å sammensette vaksiner inneholdende antigener avledet fra en rekke forskjellige kilder. For eksempel kan antigener inkludere human, bakterielle eller viral nukleinsyre, patogent avledede antigen eller antigeniske preparater, tumoravledet antigen eller antigeniske preparater, vertavledete antigener, inklusive peptider avledet fra IgE, som for eksempel det histaminfrigivende dekapeptid av IgE (kjent som Stanworth dekapeptid), rekombinant fremstilt protein eller peptider, og kimæriske fusjonsproteiner.
Ved den foreliggende oppfinnelse tilveiebringes en systemisk vaksineblanding omfattende et antigen, et saponin og en immunstimaltorisk oligonukleotid.
Alternativt tilveiebringes ved den foreliggende oppfinnelse en mukosal vaksineblanding omfattende et antigen og et hemolytisk saponin.
Videre tilveiebringes en fremgangsmåte for fremstilling av en vaksine omfattende at et saponin og et CpG-molekyl blandes med et antigen.
Eksempler på egnede farmasøytisk egnede tålbare eksipiensmidler for anvendelse i kombinasjonene ifølge den foreliggende oppfinnelse inkluderer vann, fosfatbuffret salt-oppløsning, isotoniske bufferoppløsninger.

Den foreliggende oppfinnelse illustreres ved hjelp av de følgende eksempler.
EKSEMPEL 1 Anvendelse av QS21 og CpG for intranasal vedlikeholdsdosering av systemiske antistoffer til Lip- OspA
I dette eksempel ble det undersøkt hvorvidt lytiske saponiner som for eksempel QS21 og immunstimulanter som for eksempel CpG var i stand til på en synergistisk måte å forsterke systemiske immunologiske responser til en intranasal vedlikeholds vaksinasjon i mus. Balb/c hunnmus (5 dyr pr. gruppe), 8 uker gamle, ble intramuskulært immunisert med lipo-OspA (1 ug) sammensatt på alum (50 ug). Etter 3 måneder ble musene intranasalt vedlikeholdsvaksinert (under bedøvelse) med 10 ul av oppløsning (5 ul pr. nesebor, tilført som dråper ved hjelp av pipette) inneholdende 5 ug lipo-OspA i enten A: PBS; B:20 fig CpG 1001 (TCC ATG AGC TTC CTG ACG TT, Krieg 1826); C: 5 fig QS21 (oppnådd fra Cambridge Biotech, USA); D: 20 fig CpG 1001 + 5 fig QS21; eller E: ved intramuskulær injeksjon av 1 fig lipo-OspA adsorbert på alum (50fig). Figurene 1 og 2 viser OspA-spesifikke IgG-titere og LA2-titere 14 dager etter den nasale vedlikeholdsdosering.
Metoder
ELISA for måling av OspA - sepsifikt serum - IgG i mus:
Maxisorp "Nunc"-immunplater ble belagt over natten ved 4°C med 50 (il/brønn av 1 ug/ml OspA fortynnet i PBS (i rader B til H på plate), eller med 50 ni av 5 ug/ml renset geite-antimuse-Ig (Boehringer) i PBS (rad A). Frie seter på platene blokkeres (1 time, 37°C) ved bruk av metningsbuffer: PBS inneholdende 1% BSA, 0,1% polyoksyetylen-sorbitan-monolaurat ("TWEEN") 20), og 4% normalt bovint serum (NBS). Deretter ble serier 2 gangers fortynninger av IgG-isotypeblanding fortynnet i metningsbuffer (50 ni pr. brønn) og tilsatt som en standard kurve (blanding av muse-monoklonale antistoffer IgGl, IgG2a og IgG2b fra Sigma, begynnende med 200 ng/ml og anbragt i rad A), og serumprøver (begynnende med en 1/100 fortynning og anbragt i rader B til H) inkubert i 1 time og 30 min. ved 37°C. Platene blir så vasket (x3) med vaskebuffer (PBS, 0,1% polyoksyetylen-sorbitan-monolaurat ("TWEEN" 20)). Deretter inkuberes biotinylert geite-antimuse-IgG (Amersham) fortynnet 1/5000 i metningsbuffer inkubert (50 ul/brønn) i 1 time time og 30 min. ved 37°C. Etter 3 vaskinger og påfølgende tilsetning av streptavidin-pepperrotperoksidasekonjugat (Amersham), vaskes platene 5 ganger og inkuberes i 20 min. ved romtemperatur med 50fil/brønn av påvisningsbuffer (OPDA 0,4 mg/ml (Sigma) og H2O20,03% i 50 mM pH 4,5 sitratbuffer). Revelasjon stanses ved å tilsette 50 ul/brønn H2SO42N. Optiske densiteter avleses ved 492 og 630 nm ved å anvende Biorad 3550 immunleser. Antistofftitere beregnes ved hjelp av den 4 parameters matematiske metoden ved bruk av "SoftMaxPro" programvare.
Inhibis fons forsøk for målingen av serum LA2 - liknende antistofftitere til lipo - OspA Antistofftitere i de vaksinerte individer ble undersøkt med hensyn til deres LA2-liknende spesifisitet. LA2 er et murint monoklonalt antistoff som gjenkjenner en konfor-masjonell OspA-epitop ved overflaten av bakterien og er blitt vist å være i stand til å drepe B. burgdorferi in vitro, så vel som å beskytte mus mot en eksponering for labora-toriedyrket spirochæte (Schaible UE et al. 1990. Proe. Nati. Acad Sei USA 87:3768-3772). Videre er LA-2-mab blitt vist å korrelere med bakteriedrepende antistoffer, og undersøkelser på humansera viste også en god korrelasjon mellom de totale anti-OspA IgG-titere og LA-2-titerne (som målt ved hjelp av ELISA).
Maxisorp "Nunc"-immunplater belegges over natten ved 4°C med 50 ul/brønn av
0,5 ng/ml lipo-OspA-fortynnet i PBS. Frie seter ble blokkert med metningsbuffer i 1 time ved 37°C med (100 nl/brønn metningsbuffer: PBS/BSA 1% /"Tween" 10 0,1%/- NBS 4%). Serier 2 gangers fortynninger av LA2-monoklonalt Ab (mAb) begynnende
ved 4 ng/ml ble fortynnet i metningsbuffer (50 ni pr. brønn) for å danne en standardkurve. Fortynninger av serumprøver fra de vaksinerte individer (begynnende ved en 1/10 fortynning) ble også tilsatt og platene inkubert i 2 timer ved 37°C. Platene ble vasket etter inkubasjon 3 ganger med PBS/"Tween" 20 (0,1%). LA2 mAb-peroksidase-konjugat (1/10.000) fortynnet i metningsbuffer ble tilsatt til hver brønn (50 nl/brønn) og inkubert i 1 time ved 37°C. Etter 5 vaskinger ble platene inkubert i 20 min. ved romtemperatur (i mørke) med 50 nl/brønn revelasjonsbuffer (OPDA 0,4 mg/ml og H2O20,03% i 50 mM pH 4,5 sitratbuffer). Reaksjonen og fargedannelse ble stanset med H2SO42N. Optiske densiteter avleses ved 492 og 630 nm ved bruk av Biorad 3550 immunleser. LA2-liknende Ab-titere beregnes ved hjelp av den 4 parameters matemtiske metode ved bruk av "SoftMaxPro"-programvare. LA2-liknende antistofftitere ble bestemt ved sammenlikning med standardkurven.
Resultater
CpG så vel som QS21 forbedrer signifikant den intranasale vedlikeholdsdosering av systemiske antistoffer til Lip-OspA. Videre, når begge adjuvansmidler kombineres, vises klart en synergistisk virkning på disse responser, spesielt overfor LA2-antistoffer. Humorale responser utløst i nærvær av QS21 og CpG er signifikant høyere enn dem som induseres av den parenterale vedlikeholdsdosering. Samlet viser disse resultater klart potensialet med intranasale sammensetninger som kombinerer et lytisk saponin og et immunstimulerende middel.
EKSEMPEL 2. Synergistisk kombinasjon av QS21 og CpG for å forsterke den intranasale vedlikeholdsdosering av systemiske antistoffer til influensavirus I dette eksempel ble det undersøkt hvorvidt hemolytiske saponiner som for eksempel QS21 (se eksempel) og immunostimulerende midler som CpG, var istand til på en synergistisk måte å forsterke den intranasale vedlikeholdsdosering av systemiske antistoffer i mus som var primet intranasalt initiert med inaktivert helt influensavirus.
Balb/c-hunnmus (10 dyr pr. gruppe), 8 uker gamle, ble intranasalt initiert med P-propiolakton inaktivert trivalent helt influensavirus (A/Beijing/262/95; A/Johannesburg/33/94; B/Panama/45/90; 5 ng HA/stamme) for å etterlikne den naturlige initiering som opptrer i mennesker. Etter 28 dager ble musene intranasalt vedlikeholdsdosert (under bedøv-else) med 20 ni av en oppløsning (10 ni pr. nesebor, tilført som dråper ved hjelp av pipette) inneholdende 1,5 ng HA/stamme P-propiolakton inaktivert trivalent helt influensavirus (samme stammer som i den initierende immunisasjon) i enten A: PBS; B: 50 fig CpG (TCG TCG TTT TGT CGT TTT GTC GTT, Kneg 2006); C: 4,5 fig QS21 (oppnådd fra Cambridge Biotech, USA); D: 50 fig CpG + 4,5 fig QS21; eller E: Ved intramuskulær injeksjon av 1,5 ug HA/stamme av trivalent splittinfluensavirus (samme stammer som i den initierende immunisasjon). Influensa-antigener ble supplert av produsenten SSD GmbH (Dresden, Tyskland).
Figurene 3 og 4 viser serum-influensastammespesifikke IgG-titerne og HemAgglutina-sjoninhibering ("HemAgglutionation Inhibition") (HAI) titere 14 dager etter den nasale vedlikeholdsdosering.
Metoder
ELISA for måling av anti - influensa - IsG - titere i mus:
Maxisorp "Nunc"-immunplater blekes over natten ved 4°C med 50fil/brønn av 1fig/ml helt influensavirusantigen fortynnet i PBS (i rader B til H på platen), eller med 50 (il av 5 ng/ml renset geite-antimus-Ig (Boehringer), i PBS (rad A). Frie seter på platene blokkeres (1 time, 37°C) ved bruk av metningsbuffer: PBS inneholdende 1% BSA, 0,1% polyoksyetylensorbitanmonolaurat ("TWEEN") 20), og 4% normalt bovint serum (NBS). Deretter tilsettes serier 2 gangers fortynninger av IgG-isotypeblanding, fortynnet i metningsbuffer (50 mC) ved bruk av metningsbuffer: PBS inneholdende 1% BSA, 0,1% polyoksyetylensorbitanmonolaurat ("TWEEN" 20), og 4% normalt bovint serum (NBS). Deretter tilsettes serier 2 gangers fortynninger av IgG-isotypeblanding, fortynnet i metningsbuffer (50 ml pr. brønn) satt som en standardkurve (blanding av musemono-klonale antistoffer IgGl, IgG2b fra Sigma, begynnende ved 200 ng/ml og anbragt i rad A), og serumprøver (begynnende ved en 1/100 fortynning og anbragt i rader B til H) inkuberes i 1 time og 30 min. ved 37°C. Platene blir så vasket (x3) med vaskebuffer (PBS, 0,1% polyoksyetylensorbitan-monolaurat ("TWEEN" 20). Deretter inkuberes biotinylert geite-anti-muse-IgG (Amersham) fortynnet 1/5000 i metningsbuffer (50fil/brønn) i 1 time og 30 min. ved 37°C. Etter 3 vaskinger og etterfølgende tilsetning av streptavidin-pepperrotperoksidasekonjugat (Amersham), vaskes platene 5 ganger og inkuberes i 20 min. ved romtemperatur med 50 nl/brønn av revelasjonsbuffer (OPDA 0,4 mg/ml (Sigma) og H2O20,03% i 50 mM pH 4,5 sitratbuffer). Revelasjon stanses ved å tilsette 50 nl/brønn H2SO42N. Optiske densiteter avleses ved 492 og 630 nm ved bruk av Biorad 3550 immunleser. Antistofftitere beregnes ved hjelp av den 4 parameters matematiske metode ved bruk av "SoftMaxPro"-programvare. Det hele influensaviruset anvendt for beleggingen (stamme A/Beijing/262/95), inaktivert med P-propiolakton (BPL), leveres av produsenten SSD GmBH (Dresden, Tyskland). HemAsslutinasjonsinhibasfon ( HAI ) aktivitet av influensaspesifikt serum Abs i mus Sera (25 ni) behandles først i 20 minutter ved romtemperatur (RT) med 100 ni borat 0,5 M buffer (pH 9) og 125 ni Dade Behring-levert kaolin. Etter sentrifugering (30 minutter, 3000 RPM eller 860 g), tas 100 ni supernatant (tilsvarende en 1/10 fortynning av serumet) og inkuberes i 1 time ved 4°C med 0,5% røde blodceller fra kylling. Supernatant samles etter sentrifugering i 10 minutter ved 3200 RPM (970 g). Begge operasjoner foretas for å eliminere de naturlige hemagglutinerende faktorene inneholdt i seraet. Deretter fortynnes 25 ni behandlet sera i 25 ni PBS (serie 2 gangers fortynninger begynnende ved 1/20) i 96 brønners Greiner-plater. PBL-inaktivert helt virus tilsettes (25 nl/brønn) ved en konsentrasjon på 4 hemagglutinasjonsenheter (dvs. ved en fortynning som er 4 ganger lavere enn den siste fortynning som frembringer en agglutinasjon av røde blodceller) i 30 minutter ved RT under omrøring. Røde blodceller fra kylling tilsettes deretter (25 nl/brønn) i 1 time ved RT. Platene holdes til slutt over natten ved 4°C før de avleses. HAI-titerne tilsvarer den siste serumfortynning som inhiberer den virus-induserte hemagglutinasjon.
Resultater
CpG såvel som QS21 forbedrer ikke den intranasale vedlikeholdsdosering av IgG eller HAI-antistoffer til influensastammer. Når begge adjuvansmidler kombineres, vises imidlertid klart en synergistisk virkning på disse responser. HAI-responsene utløst i nærvær av QS21 og CpG er endog liknende dem som induseres av den parenterale vedlikeholdsdosering. Disse resultater bekrefter potensialet av intranasale sammensetninger som kombinerer et hemalytisk saponin og et immunstimulerende middel. De viser også at flere CpG-sekvenser kan være effektive i denne sammenheng (Krieg 2006 i det foreliggende eksempel og Krieg 1826 i eksemplene 3 og 5).
EKSEMPEL 3. Synergistisk kombinasjon av / 3- Escin og CpG for å forsterke den intranasale vedlikeholdsdosering av systemiske antistoffer til Lipo- OspA
I det foreliggende eksempel vurderes muligheten for at en synergi lik den som iakttas mellom QS21 og CpG kunne kombineres med andre hemolytiske saponiner (se eksempel) som for eksempel P-Escin. Det ikke-hemolytiske saponin, glycyrrhizinsyre testes også.
Balb/c-hunnmus (6 dyr pr. gruppe), 8 uker gamle, ble intramuskulært initiert med lipo-OspA (1 ng) sammensatt på alum (50 ng)-Etter 3 måneder ble musene intranasalt vedlikeholdsdosert (under bedøvelse) med 10 ni oppløsning (5 ni pr. nesebor, tilført som dråper ved hjelp av en pipette) inneholdende 5 ng lipo-OspA i enten A: PBS; B: 50 ng CpG 1001 (TCC ATG AGC TTC CTG ACG TT, Krieg 1826); C: 5 ng P-Escin (levert fra Sigma); D: 50 ng CpG 1001 + 5 ng P-Escin; E: 5 ng glycyrrhizinsyre (levert fra Sigma); F: 50 ng CpG 1001 + 5 ng glycyrrhizinsyre eller G: Ved intramuskulær injeksjon av 1 ng lipo-OspA adsorbert på alum (50 ng)- Figur 5 viser de OspA-spesifikke-LA2-titere 14 dager etter den nasale vedlikeholdsdosering.
Metoder
Metodene er de samme som dem som er detaljert i eksempel 1.
Resultater
P-Escin og CpG virker synergistisk for å forsterke den intranasale vedlikeholdsdosering av systemisk LA2 Abs. Denne kombinasjon utløser mer forhøyede Ab-responser enn den parenterale vedlikeholdsdosering. På den annen side oppnås en slik synergi ikke ved å kombinere CpG med glycyrrhizinsyre.
Dissse resultater og de tidligere resultater i denne fremstillingen, viser samlet evnen til CpG og forskjellige hemolytiske saponiner til å fremkalle immunresponser på en synergistisk måte.
EKSEMPEL 4. Immunogenisitetsstudier ved bruk av P. falciparum RTS, S og HIV- 1 gp 120 sammensatt med CpG og/ eller DQ21
1 . Forsøksutkast
To muse-immunogenisitetsstudier ble gjennomført for å evaluere potensielle additive
eller synergistiske effekter av CpG-olignukleotider (CpG) og QS21. Grupper av mus ble immunisert med RTS,S og gpl20 sammensatt med CpG og QS21 alene eller i kombinasjon. Disse adjuvansmiddelkombinasjoner ble også testet i nærvær av bæreren Al(OH)3eller en olj e-i-vann (olje/vekt) emulsjon.
Immunogenisiteten av sammensetningene ble undersøkt etter to parenterale immunisa-sjoner. Sera ble analysert for nærvær av antigenspesifikke antistoffer, og for fordelingen av antistoffisotyper. Miltceller ble anvendt for å evaluere cellemedierte immunresponser. Disse celler ble testet for nærvær av cytotoksiske T-lymfocytter (CTL) og lymfofor-merende (lymfoformering) celler.
2. Sammensetning
2. 1 Forsøk 1
Sammensetningsprosess:
Sammensetninger ble fremstilt tre dager før hver injeksjon. Når det var nødvendig, ble RTS,S (10 fig) og gpl20 (10 fig) adsorbert på 100 fig AL(OH)3. Når det var nødvendig ble MPL (5 ng) tilsatt og inkubert 30 min. før buffertilsetning som en blanding av 10 ganger konsentrert PBS pH 7,4 og H2O bortsett fra gruppen uten DQ for hvilken buffe-ren var P04, NaCl 10/150 pH 6,8. Etter 30 min. ble om nødvendig, QS21 (5 ng) blandet med liposomer i et vektforhold QS21/kolesterol på 1/5 (referert til som DQ) tilsatt til blandingen. 30 minutter senere, for sammensetningene med oligo, ble 100 ng CpG tilsatt 30 min. før tilsetning av 50 ng/ml tiomersal som konserveringsmiddel.
Alle inkubasjoner ble gjennomført ved romtemperatur med omrøring.
2. 2 Forsøk 2
Sammensettingsprosess:
Sammensettingsprosessene utføres samtidig for begge injeksjoner. Volumet av injek-sjonen for én mus er 100 ul. 50 fil/ml tiomersal blir tilsatt som konserveringsmiddel.
Gruppe 1: RTS,S (10fig) og gpl20 (10fig) fortynnes med H20 og PBS 6,8 for isotoni-sitet. Etter 5 minutter adsorberes blandingen på CpG 1856 (100 fig).
Gruppe 2: RTS,S (10 fig) og gpl20 (10fig) fortynnes med H20 og PBS pH 7,4 for iso-tonisitet. Etter 30 minutter adsorberes RTS,S og gpl20 på DQ (5fig). Etter 30 minutters adsorpsjon, adsorberes sammensetningen på CpG 1856 (100fig).
Gruppe 3: RTS,S (10fig) og gpl20 (10fig) fortynnes med H20 og PBS pH 6,8 for iso-tonisitet. Etter 5 min. adsorberes blandingen på en olje/vekt-emulsjon. Etter 5 min. adsorpsjon, adsorberes blandingen på QS21 (5 fig) før tilsettingen av CpG (100fig).
3 . Immunologiske metoder
9 (Balb/C x C57B1/6) Fl-mus pr. gruppe mottok i bakre fotsåle 2 x 50 fil vaksine to ganger med et to ukers intervall. To uker sener ble sera oppnådd for å bedømme antistoffresponser, og miltceller ble høstet for å bestemme cellemedierte immunresponser.
For lymfoformeringsanalyse ble celler sådd i kvadruplikater i 96 brønners rundbunnede mikrotiterplater med en konsentrasjon på 2 x IO<6>pr. ml. Celler ble dyrket i 72 eller 96 timer i RPMI-1640 supplert med antibiotika, glutamin og 1% (volum/volum) normalt museserum i nærvær av forskjellige konsentrasjoner av RTS,S eller gpl20 antigen. Kontrollceller ble dyrket uten antigen. Cellene ble deretter pulsert over natten med 1 fiCi/brønn [<3>H]-tymidin, høstet, og den innlemmede radioaktiviteten ble bestemt i en beta-teller. Resultater er uttrykt som midlere impulstall pr. minutt (cpm).
For CTL-analyse ble celler dyrket i 7 dager i 6-brønners plater i nærvær av 10 fig pr. ml syntetisk peptid pCMI003 (IPQSLDSWWTSL) tilsvarende en HbsAg CTL-epitop (Schirmbeck et al., 1995) eller peptid pCMI007 (GIHIGPGRAFYAARK) som representerer en gpl20 CTL-epitop (Casement et al., 1995). Ved slutten av dyrkingsperioden ble effektorceller bedømt i duplikat for HbsAg-spesifikk cytolytisk aktivitet i standard
[<51>Cr]-frigivelsesforsøk ved bruk av kontroll og S-transfekterte P815-celler. Gpl20-spesifikk cytotoksisitet ble bestemt ved bruk av P815 målceller som enten var etterlatt ubehandlet eller pulsert i 1 time med peptid pCMI007. Minimum og maksimum frigivelse ble bestemt med målceller uten effektorceller og henholdsvis ved tilsetting av 3%
(volum/volum) Triton X-100. Resultater er uttrykt som % [5 lCr]-frigivelse (cpm av forsøkskultur - cpm for spontan frigivelse/cpm av maksimum frigivelse - cpm av spontan frigivelse).
Titrasjon og isotyping av samlet sera ble utført i et standard enzymtilknyttet immunsor-bentanalyse format (ELISA) ved bruk av plater belagt med Hbs.Ag. Sera ble fortynnet i PBS/BSA begynnende ved 1:400. Biotinylerte sekundære antistoffer spesifikke for lg eller isotypene IgGl, IgG2a og IgG2b etterfulgt av et pepperrot-peroksydase-streptavidinkonjugat ble anvendt for deteksjon av bundne antistoffer. ELISA-titere ble beregnet fra en referanse ved hjelp av "SoftmaxPro" og uttrykt i ELISA-enheter (EU/ml). Gp 120-spesifikke antistofftitere ble bestemt ved en standard ELISA ved bruk av plater belagt med gpl20-protein. Sera ble fortynnet i PBS/"Tween"20/BSA begynnende ved 1:100. Biotinylerte sekundære antistoffer spesifikke for lg eller isotypene IgGl, IgG2a og IgG2b etterfulgt av et pepperrot-peroksydase-streptavidinkonjugat ble anvendt for deteksjon av bundne antistoffer. Titere ble beregnet i relasjon til et standard muse-Ig og uttrykt som ug/ml.
4 . Resultater
Forsøk 1
Analyse av lymfoformeringsresponser viste ikke noen signifikante forskjeller i reaktivi-tet overfor RTS,S mellom gruppene. I motsetning til dette viste gruppene 1 og 3, begge inneholdende CpG og DQ21, bedre gpl20-spesifikk lymfoformeringsresponser enn gruppene inneholdende CpG eller DQS21 alene (figur 6).
I dette forsøket ble bare HbsAg-spesifikk CTL målt. Der var ingen uttalt forskjell i CTL-induksjon mellom gruppene 1 og 3 som hadde mottatt CpG og DQS21 i kombinasjon og gruppene 2 og 4 immunisert med bare én av de to adjuvansmiddelkomponenter, mens nærværet av Al(OH)3nedsatte den iakttatte CTL-aktivitet for kombinasjonen av CpG og DQS21 i gruppe 1 (figur 7). En tendens var imidlertid tilstede ved at CpG og DQS21 var bedre enn DQS21 alene, og kombiasjonen induserte mer CTL i nærvær av Al(OH)3enn CpG alene (figur 7).
Den humorale immunresponsen av musene ble undersøkt bare for nærvær av HbsAg-spesifikke antistoffer. Titere var liknende i alle grupper, bortsett fra for grupper 3, som viste en omtrent tredobbel økning, som viste at i nærvær av Al(OH)3er kombinasjonen av DQS21 og CpG mer immunogenisk enn CpG alene (figur 8). Isotypefordelingen var liknende for de Al(OH)3-holdige gruppene 3 og 4, mens i fravær av Al(OH)3induserte kombinasjonen av CpG og DQS21 et sterkere THi-liknende isotypemønster DQS21 alene (figur 8).
Forsøk 2
Lymfoformeringsresponser spesifikke for RTS,S og gpl20 var meget like i dette forsø-ket. Dataene indikerer at tilsetningen av DQS21 (enten alene eller med en olje/vann-emulsjon) øker lymfoformeringsresponser til begge antigener (figur 9).
CTL-responser ble evaluert ved å anvende både et HbsAg og et gpl20 CTL-epitop-peptid. I begge tilfeller kunne CTL detekteres etter immunisasjon av gruppe 1 med CpG alene (figur 10). Tilsetning av DQS21 resulterte imidlertid i betraktelig økning i CTL for begge antigener (figur 10). Nærværet av en olje/vann-emulsjon enten nøytraliserte den positive effekten av DQS21 (gpl20) eller økte bakgrunnen for in v/Yro-analysen (HbsAG).
Antistoffresponser til HbsAg og gpl20 økte ved tilsetning av DQS21 til CpG-adjuvansmidlet (figur 1 IA). En ytterligere økning ble iakttatt når en olje/vann-emulsjon ble ink-ludert i sammensetningen (figur 1 IA). Tilsetning av DQS21 til CpG forskjøv gpl20 iso-typeprofilene mot en mer uttalt THi-tendens (figur 1 IB), mens innvirkningen på HbsAg-isotypeprofilene var mindre uttalt i dette forsøket.
5. Konklusjoner
Immunisasjon med RTS,S og gpl20 sammensatt med kombinasjonen av CpG og DQS21, resulterer i sterke antigenspesifikke immunresponser. Kombinasjonen av adju-vansmiddelkomponentene CpG og DQS21:
forsterker lymfoformeringsresponser
øker CTL-aktivitet
forbedrer antistofftitere og THi-isotypemønstre
i sammenlikning med de enkelte komponenter.
EKSEMPEL 5. Terapeutisk potensiale av CpG og/ eller DQS21- sammensetninger i TCl- tumormodell
1 . Forsøksopple<gg>
Fire grupper av 10 mus C57bl/6 mottok 10e6 (200 fil) TC 1-celler (E7-uttrykkende tu-morceller) subkutant ved dag 0 i siden.
Mus ble så vaksinert to ganger ved dag 14 og 21 etter tumoreksponeringen, med 5 ng av sammensatt PD1/3E7 HPV16 injisert i fotpoten. Tumorvekst ble målt individuelt to ganger i uken.
Grupper av mus:
1. Ingen vaksine
2. PD1/3E7 + CPG (10 ng ODN 2006)
3. PD1/3E7 + DQS21 (0,5 ng)

Tumorveksten ble overvåket ved å måle de enkelte tumorer to ganger i uken.
2 . Sammensetninger
Sammensetninger ble utført på injeksjonsdagene. Volumet av injeksjon for én mus var 100 (il. Ved behov ble PD1/3E7 (5 ng) fortynnet med H20 og PBS pH 7,4 for isotoni- sitet. Etter 5 minutter ble QS21 (0,5 ng) blandet med liposomer i et vektforhold QS21/kolesterol på 1/5 (referert til som DQ) tilsatt til sammensetningen. 30 minutter senere, for sammensetningen med oligo, ble 10 fig CpG (ODN 2006) tilsatt 30 min. før tilsetting av 1 ng/ml tiomersal som konserveringsmiddel.
3 . Resultater
Utviklingen av den gjennomsnittlige tumorvekst pr. grupper på 10 dyr over tid er vist i figur 12. 100% av dyrene som mottok en tumoreksponering på 10e6 TC 1-celler, utviklet progressivt voksende tumor.
70-80% av de ikke-vaksinerte dyr eller av dyrene vaksinert med E7-proteinet i DQS21døde ved dag 35.
To vaksinasjoner med E7-proteinet, sammensatt i DQS21, hadde nesten ingen virkning på tumorvekst. I motsetning til dette, induserte 2 vaksinasjoner, IFP (dag 14, 21) med 5 ng ProtD 1/3 E7 HPV16 i CPG-adjuvansmiddel regresjonen av disse forutetablerte tumorer, og beskytter musen mot å dø: 70-80% av musene var fremdeles i live på dag 35. Kombinasjonen av de 2 immunostimulanter CPG og DQS21 viste en liten forbedrende effekt i forhold til CpG anvendt alene.
Claims (20)
1.
Adjuvanssammensetning,karakterisert vedat den omfatter et saponin og et immunstimulerende oligonukleotid, der saponinet er på formen av et liposom formulert med koleresterol og lipid, eller en olje-i-vann-emulsjon.

Adjuvanssammensetning ifølge krav 1,karakterisertv e d at det immunstimulerende oligonukleotidet omfatter et purin, purin, C, G, pyrimidin, pyrimidinsekvens.
3.
Adjuvanssammensetning ifølge krav 1,karakterisertv e d at det immunstimulerende oligonukleotidet er valgt fra gruppen omfattende: SEQ ID NO:l; SEQ ID NO:2; SEQ IDNO:3; SEQ ID NO:4; SEQ ID NO:5.

Adjuvanssammensetning ifølge krav 1,karakterisertv e d at det immunostimulerende oligonukleotidet inneholder minst to umetylerte CG-repeats som er skilt ved minst 3 nukleotider.

Adjuvanssammensetning ifølge krav 1,karakterisertv e d at den omfatter et saponin og et immunstimulerende oligonukleotid inneholdende umetylert CpG-dinukleotid, hvori saponinet er på formen av et liposom formulert med kolesterol og lipid.

Adjuvanssammensetning ifølge krav 1,karakterisertv e d at den omfatter et saponin og et immunstimulerende oligonukleotid inneholdende umetylert CpG-dinukleotid, hvori saponinet er på formen av en olje-i-vann-emulsjon.

Vaksinesammensetning,karakterisert vedat den omfatter en adjuvanssammensetning ifølge et hvilket som helst av kravene 1 til 9, og ytterligere et antigen.

Vaksinesammensetning ifølge krav 10,karakterisertv e d at antigenet er avledet fra en organisme valgt fra gruppen omfattende: Humant immundefisiensvirus (HIV), Varicella Zoster-virus, Herpes Simplex Virus type 1, Herpes Simplexs Virus type 2, human cytomegalovirus, Dengue-virus, hepatitt A, B, C eller E, respiratorisk syncytialvirus, human papillomvirus, influensavirus, Hib, meningittvi-rus, Salmonella, Neisseria, Borrelia, Chlamydia, Bordetella, Streptococcus, My-coplasma, Mycobacteria, Haemophilus, Plasmodium eller Toxoplasma, Stanworth de-kapepetid; eller tumorassosierte antigener (TAA), MAGE, BAGE, GAGE, MUC-1, Her-2 neu, CEA, PSA, KSA eller PRAME; eller et selvpeptidhormon, GnRH.

Vaksinesammensetning ifølge krav 11,karakterisertv e d at antigenet er avledet fra gruppen omfattende (a) tumorassosierte antigener PSMA, PSCA, tyrosinase, survivin, NY-ESOl, prostase, PS 108, RAGE, LAGE, HAGE; (b) eller det N-terminale 39-43 aminosyrefragmentet (Abeta) av amyloidforlø-perproteinet; (c) eller antigener assosiert med atherosklerose.

Vaksinesammensetning ifølge hvilket som helst av kravene 10 til 12,karakterisert vedat vaksinen tilføres systemisk.

Vaksinesammensetning ifølge hvilket som helst av kravene 10 til 12,karakterisert vedat vaksinen tilføres mukosalt.
15.
Vaksinesammensetning ifølge krav 14,karakterisertv e d at saponinet i adjuvanssammensetningen er hemolytisk.

Vaksinesammensetning ifølge hvilket som helst av kravene 10 til 12, forbruk som et medikament.
17.
Anvendelse av en kombinasjon av et saponin og et CpG-molekyl ifølge hvilket som helst av kravene 10 til 16, for fremstilling av en vaksine for profylakse og behandling av virale-, bakterielle-, parasittiske infeksjoner, allergi, cancer eller andre kroniske lidelser.

Fremgangsmåte for fremstilling av en vaksinesammensetning ifølge krav 18,karakterisert vedå blande det følgende (a) et saponin, (b) et immunstimulerende oligonukleotid og (c) et antigen, hvori saponinet er på formen av en olje-i-vann-emulsjon og hvori oligonukleotidet inneholder umetylerte CpG-dinukleotider.
