Abstract
Foreliggende oppfinnelse angår immunologi og mer spesifikt immunterapeutiske kombinasjoner anvendt for å kontrollere vekst og/eller celleproliferasj on av tumorer. Ved hjelp av foreliggende oppfinnelse er en terapeutisk effekt mot tumorer observert ved kombinasjon av idiotypiske vaksiner hvis aktive bestanddel er et antigangliosidantistoff (Abl), med idiotypiske vaksiner hvis aktive bestanddel er antiidiotypisk antistoff (Ab2) oppnådd mot et antigangliosidantistoff, eller vaksiner hvis aktive bestanddel er ett eller flere gangliosider. Kombinasjoner av disse vaksinene som forårsaker en synergistisk terapeutisk effekt mot tumorer er beskrevet. De omtalte kombinasjonene kan anvendes for pasienter i forskjellige kliniske tilstander av tumorer som uttrykker gangliosider.
Description
Foreliggende oppfinnelse angår human medisin og angår spesielt en immunterapeutisk kombinasjon for immunterapi av tumorer som overuttrykker gangliosider.
Oppfinnelsen angår også anvendelse av denne kombinasjon.
Gangliosider er komponenter av plasmamembranene til mesteparten av pattedyrcellene. Selv når disse glykosfingolipidene er uttrykt i normalt vev, er de svært attraktive mål for immunterapi på grunn av ekspresjonsmønstrene i løpet av malign transformasjon av cellene (Hakomori, S. Ann. Rev. Biochem. 50; 1981:733-764; Hakomori, Cancer Res. 45; 1985:2405-2414; Irie, R. F., et al. I: Therapeutic monoclonal antibodies. Borrebaeck, C.A.K, og Larrick, J.W. (red.). M Stockton Press, 1990, s. 75-94).
Sakkaridnaturen av gangliosidene, sammen med det faktum at immunforsvaret oppfatter dem som en del av egen kropp, gjør at de er veldig svakt immunogene molekyler. Noen strategier har blitt anvendt for å øke immunogeniteten til disse antigenene. De er basert på presentasjon av gangliosider for immunsystemet i et annet molekylært miljø. Én av disse strategiene er anvendelse av konjugerte vaksiner hvor sakkaridantigenet er kovalent bundet til et bærerprotein hvor bæreren er veldig immunogen for T-celler. Det gjør det mulig å oppnå en sterk og langvarig antistoffimmunrespons. Ved anvendelse av denne prosedyren er det mulig å oppnå IgG-antistoffer mot slike gangliosider, selv om titerne etter reimmuniseringene var lavere sammenlignet med de oppnådd i en klassisk immunrespons mot tymusuavhengige antigener (Helling, F et al. Cancer Res. 54; 1994:197-203; Helling, F et al. Cancer Res. 55; 1995:2783-2788; Livingston PO et al. Cancer Immunol Immunother 45; 1997:10-19).
Nye vaksinepreparater er rapportert å fremkalle en immunrespons mot N-acetylerte og N-glykolylerte gangliosider. Disse vaksinene er basert på hydrofob konjugering mellom gangliosider og veldig små proteoliposomer (VSSP) oppnådd fra assosieringen av det ytre membranproteinkomplekset (OMPC) fra Neisseria meningitidis, gramnegative bakterier, med gangliosider (gangliosiderA/SSP)
(Estévez F. et al. Vaccine 18; 1999:190-197; patentene US 5.788.985 og US 6.149.921). Vaksinene sammensatt av GM3-gangliosid/VSSP eller (NeuGcGM3)-gangliosid/VSSP har tilveiebrakt høye IgM- og IgG-antistofftitere spesifikke mot GM3 og NeuGcGM3.
Ellers økte vaksinasjon med GM3/VSSP overlevelse av mus som bærer melanom B16, den reduserte også tumorvolumer og den økte rejeksjonsrate med hensyn til det subkutane transplantatet av tumoren (Alonso et al. Int. J Oncology 15; 1999: 59-66; Car, A. et al. Melanoma Research, undertrykking).
Idiotyp nettverksteorien foreslått av Jerne 1974 (Jerne, N.K. Ann. Immunol. 125C; 1974:373-389) pekte ut immunsystemet som et antistoffnettverk med et kompleks interaksjonsmønster mellom dem og mange naturlige antigener, at immunsystemet blir regulert ved interaksjoner som skjer via de variable regionene eller idiotype (Id) og ved interaksjon idiotype-antiidiotype. Jernes teori støttet nye strategier for kreftaktiv immunterapi (Al) basert på anvendelse av antiidiotypiske vaksiner. Vaksinasjonen blir utført med et antistoff (Ab1) som gjenkjenner det tumorassosierte antigenet som induserer antiidiotypiske antistoffer (Ab2). Disse antiidiotypiske antistoffene etterligner det nominale antigenet og i sin tur genererer de anti-antiidiotypiske antistoffer (Ab3) mot det tumorassosierte antigenet (Schmolling J, et al. Hybridoma 14; 1995:183-186). På den annen side er induksjon av immunrespons mot tumorassosierte antigener rapportert etter vaksinasjon med anti-ld antistoffer (Ab2) (Raychauhuri, S. et al. J. Immunol. 137; 1986:1743-1749; Raychauhuri, S. et al. J. Immunol. 139; 1987:3902-3910; Bhattacharya-Chatterjee, M. et al. J. Immunol. 139; 1987:1354-1360; Bhattacharya-Chatterjee, M. et al. J. Et annet valg angående anvendelse av Ab2-antistoffer og for å øke immunogeniteten av sakkaridrester involverer muligheten av at slik sakkaridepitop kan representeres ved en proteinepitop på antistoffmolekylet. Faktisk er mange av disse anti-ld antistoffene oppnådd; de etterligner gangliosider sterkt uttrykt i tumorceller slik som GM3, GD3 og GD2 (Yamamoto, S et al. J. Nati. Cancer Inst. 82; 1990:1757-1760; Chapman, P.B. et al. J. Clin. Invest 88; 1991:186-192; Cheung N-K V, et al. Int J Cancer 54; 1993:499-505; Saleh MN. et al. J Immunol 151; 1993:3390-3398; Sen G. et al. J Immunotherapy 21; 1998: 75-83).
Lovende resultater er oppnådd fra kliniske studier med kreftpasienter ved anvendelse av Ab2-antistoffer samtidig med BCG eller QS21 som adjuvans (McCaffery M. et al. Clin Cancer Res 2; 1996: 679-686; Foon KA. et al. Clin Cancer Res. 4; 1998:1117-1124).
Det monoklonale antistoffet P3 (deponeringsnummer ECACC 94113026) er kjent fra tidligere, som spesifikt gjenkjenner sialinsyren i N-glykolyl-inneholdende monosialo- og disialogangliosider. Dette MAb'et Ab1 gjenkjenner antigener på humane brysttumorer og melanomer (US patent 5.788.985; Våzquez AM. et al. Hybridoma 14; 1995:551-556; Moreno E. et al. Glycobiology 8; 1998: 695-705; Marquina G. et al. Cancer Res 56; 1996:5165-5171; Carr, A. et al. Hybridoma 19 (3); 2000:241-247).
Det antiidiotypiske antistoff 1E10 (Ab2 MAb 1E10) (deponeringsnummer ECACC 97112901) oppnådd ved immunisering med MAb P3 er et gammatype antistoff (ikke indre bilde). Ab2 MAb 1E10 viste antitumoreffekt på vekst av brysttumorer og melanomer (US patent 6.063.379; Våzquez et al. Hybridoma 14; 1995:183-186; Våzquez et al. Oncology Reports 7; 2000:751-756).
Det eksisterer ikke bevis i tidligere publiserte artikler om anvendelse av kombinasjonen mellom vaksiner omfattende gangliosider og farmasøytiske preparater omfattende antigangliosidantistoffer (Ab1) eller antiidiotypiske antistoffer (Ab2), betegnet idiotypiske vaksiner; heller ikke anvendelse av kombinasjoner av idiotypisk vaksine Ab1 med idiotypisk vaksine Ab2.
Foreliggende oppfinnelse angår en vaksinekombinasjon og dens anvendelse som beskrevet, for å forsterke effekten som hver av dem produserer separat.
Foreliggende oppfinnelse vedrører en immunterapeutisk kombinasjon for immunterapi av tumorer som overuttrykker gangliosider, som er kjennetegnet ved at den omfatter de følgende komponentene: (B) en idiotypisk vaksine omfattende et spesifikt murint antiidiotypisk monoklonalt antistoff (Ab2) mot et monoklonalt antigangliosidantistoff; og
(C) en gangliosidvaksine.
Oppfinnelsen vedrører også anvendelse av den immunterapeutiske kombinasjon
for fremstilling av et medikament for behandling av tumorer som overuttrykker gangliosider.
I kombinasjonen kan B være en idiotypisk vaksine omfattende det murine antiidiotypiske antistoffet 1E10 med deponeringsnummer ECACC 97112901 og C kan være en vaksine omfattende N-glykolyl GM3 (NeuGcGM3) eller N-acetyl GM3 (NeuAcGM3) gangliosider.
I foreliggende oppfinnelse kan B pluss C administreres samtidig eller alternerende.
Kombinasjonen ifølge foreliggende oppfinnelse kan være anvendelig ved behandling av kreft, spesielt de som overuttrykker gangliosider, så vel som lunge, bryst, fordøyelsessystem, urogenitalsystem, melanomer, sarkomer og de utgått fra nevroektodermalt vev.
Skjemaet for administrering av de farmasøytiske preparatene eller kombinasjon av farmasøytiske preparater for å behandle pattedyr er også beskrevet.
En fremgangsmåte som omfatter administrering til pattedyr av det farmasøytiske preparatet eller kombinasjon av farmasøytiske preparater beskrevet tidligere for forebygging eller behandling av tumorer i bryst, lunge, fordøyelsessystem, urogenitalsystem, melanomer, sarkomer og av nevroektodermal opprinnelse er også beskrevet.
1. Fremstilling av vaksine omfattende gangliosider:
Vaksinene omfattende gangliosider blir oppnådd i henhold til spesifikasjonen ved Estevez et al. i Vaccine 18, 1999: 190-197 og i patentene US 5.788.985 og US 6.149.921. Veldig små proteoliposomer (VSSP) blir oppnådd fra assosieringen av det ytre membranproteinkomplekset (OMPC) fra den gramnegative bakteriestammen, Neisseria meningitidis, med syntetiske eller naturlige gangliosider inkorporert deri (VSSP-G). Gangliosidene kan være (NeuGcGM3), GD3 eller (NeuAcGM3).
2. Fremstilling av idiotypiske vaksiner:
De farmasøytiske preparatene blir oppnådd i henhold til spesifikasjonene i patentene US 5.817.513 og US 6.063.379. Vaksinen omfatter murine monoklonale antigangliosidantistoffer slik som MAb P3 eller murine antiidiotypiske monoklonale antistoffer slik som MAb 1E10 som gjenkjenner monoklonale antigangliosidantistoffer. 3. Immunterapeutiske kombinasjoner som forsterker immunresponsen og antitumoreffekten av gangliosidvaksiner og idiotypiske vaksiner i dyremodeller: De omtalte prosedyrene kan anvendes i mus eller hvilken som helst annen pattedyrart. Komponentene i kombinasjonen er gangliosidvaksinen og den idiotypiske vaksinen; de kan kombineres på forskjellige måter.
I én kombinasjon kan dyrene immuniseres med 3 til 10 doser i et område fra 25 fag til 1 mg av det murine monoklonale antigangliosidantistoffet; intervallet mellom doser kan være 7 og 14 dager. I løpet av denne perioden mottar dyrene mellom 3 til 10 doser i et område mellom 60 til 1000 fag av gangliosidvaksinen, intervallet mellom doser kan være 7 til 14 dager.
I en annen kombinasjon kan dyrene immuniseres med 3 til 10 doser i et område fra 25 ug til 1 mg av det murine antiidiotypiske antistoffet spesifikt mot et antigangliosidantistoff; intervallet mellom doser kan være 7 til 14 dager. I løpet av denne perioden mottar dyrene mellom 3 til 10 doser i et område mellom 60 til 1000 ug av gangliosidvaksinen, intervallet mellom doser kan være 7 til 14 dager.
Administreringen av begge vaksinetypene kan være samtidig eller alternerende. Vaksinene kan formuleres som separate produkter eller som et vaksinepreparat når de blir administrert samtidig. Når vaksiner blir administrert på en alternerende måte, kan intervallene mellom hver vaksinetype være 3-7 dager. Vaksinene blir administrert i et adjuvans som kan være aluminiumhydroksyd (62,5 ug - 2,5 mg), Montanid ISA 51 (0,1-1,2 ml/dose) eller hvilket som helst annet passende adjuvans. Totalvolumet for hver dose kan være 10 uJ - 2 ml.
Vaksiner omfattende gangliosider kan administreres intradermalt, subkutant, intramuskulært, intraperitonealt, intramukosalt eller kombinasjoner derav. Samme administreringsmetoder kan anvendes for vaksiner omfattende antistoffer.
De immunterapeutiske kombinasjonene øker antistoffimmunresponsen, så vel som den cellulære immunresponsen i behandlede dyr.
Varighet for tilstedeværelse av lokale tumorer, tumorvolumet og overlevelse av behandlede individer blir sammenlignet med samme parametre for kontrollgruppen, for å vurdere effektiviteten av immunterapeutiske kombinasjoner. Ved sammenligning av disse parametrene blant behandlede og kontrollgrupper kan det observeres en kortere tilstedeværelse av lokale tumorer, en reduksjon av tumorvolumet og en økning i overlevelse for gruppen behandlet med terapeutiske kombinasjoner ifølge foreliggende oppfinnelse. Kontrollgruppene er ikke bare de dyrene som er behandlet med adjuvanset, men også de gruppene bare behandlet med én vaksine istedenfor med kombinasjoner derav.
4. Immunterapeutiske kombinasjoner som forsterker immunresponsen og antitumoreffekten av gangliosidvaksiner og idiotypiske vaksiner i humane pasienter: Fremgangsmåten henvist til tidligere kan også anvendes på kreftpasienter i forskjellige kliniske stadier. Spesielt de tumorene som overuttrykker gangliosider, så vel som lunge, bryst, fordøyelsessystem, urogenitalsystem, melanomer, sarkomer og de utgått fra nevroektodermalt vev.
I én kombinasjon kan pasientene immuniseres med 3 til 10 doser i et område fra 0,1 til 5 mg av det murine monoklonale antigangliosidantistoffet; intervallet mellom doser kan være 7 og 14 dager. I løpet av denne perioden mottar pasientene blant 3 til 10 doser i et område mellom 60 til 1000 ug av gangliosidvaksinen, intervallet mellom doser kan være 7 til 14 dager.
I en andre kombinasjon kan pasientene immuniseres med 3 til 10 doser i et område fra 0,1 til 5 mg av det murine antiidiotypiske antistoffet spesifikt mot et antigangliosidantistoff; intervallet mellom doser kan være 7 til 14 dager. I løpet av denne perioden mottar pasientene blant 4 til 6 doser i et område mellom 60 til 1000 ug av gangliosidvaksinen, intervallet mellom doser kan være 7 til 14 dager.
I en tredje kombinasjon kan pasientene immuniseres med det murine monoklonale antigangliosidantistoffet; doser, hyppighet og intervaller kan være som beskrevet tidligere. I løpet av denne perioden mottar pasientene blant 3 til 10 doser i et område mellom 0,1 til 2 mg av det murine antiidiotypiske antistoffet spesifikt mot et antigangliosidantistoff.
Administreringen av begge vaksinetyper kan skje samtidig eller alternerende. Vaksinene kan formuleres som separate produkter eller som et vaksinepreparat når de blir administrert samtidig. Når vaksiner blir administrert på en alternerende måte, kan intervallene mellom hver vaksinetype være 3-7 dager. Vaksinene blir administrert i et adjuvans som kan være aluminiumhydroksyd (1-5 mg/dose), Montanid ISA 51 (0,6-1,2 ml/dose) eller hvilket som helst annet passende adjuvans. Totalvolumet for hver dose kan være 10 uJ - 2 ml.
Vaksiner omfattende gangliosider kan administreres intradermalt, subkutant, intramuskulært, intraperitonealt, intramukosalt eller kombinasjoner derav. Samme administreringsvei kan anvendes for vaksiner omfattende antistoffer.
Under vaksinasjon blir noen biokjemiske parametre og antistofftitere kontrollert ved målinger i blod. Hyppigheten kan være 1 uke til 3 måneder. Cellulær immunitet blir studert ved anvendelse av lymfocytter fra pasientene. Ekstraksjonene blir utført med en frekvens som svinger fra én uke til tre måneder.
Til slutt blir pasientene reimmunisert med begge vaksiner ved de før nevnte konsentrasjonene; intervallene mellom doser kan være 1-6 måneder. De kan administreres samtidig eller ei og i løpet av 1 til 2 år.
Foreliggende vaksinasjonsskjema fremkaller en økt antistoff- og cellulær immunrespons hos pasienter for å redusere tumormengde.
EKSEMPLER
Eksempel 1: Aktivering av antiidiotypiske T2- og anti-antiidiotypiske T3-celler i syngen modell, indusert ved immunisering med murint MAb P3.
Balb/c-hunnmus og nakne mus med samme genetiske bakgrunn, 6-8 uker gamle, ble immunisert subkutant med 100ug murint MAb P3 (anti-N-glykolylert gangliosid, deponeringsnummer ECACC 94113026; Våzquez et al., Hybridoma 14; 1995: 551-558; Moreno et al. Glycobiology 8; 1998: 695-705) og fullstendig Freunds adjuvans (CFA). Syv dager senere ble mus reimmunisert med 50ug av antistoffet i ufullstendig Freunds adjuvans (IFA). På dag 10 ble væske fra lymfeknuter samlet og lymfocyttene ble oppnådd ved å presse med en sprøyte. Cellesuspensjonen ble anvendt i forsøk med cellulær proliferasjon, som ble målt ved<3>H-tymidininkorporering. In v/fro-lymfocyttproliferasjon ble målt ved å dyrke lymfocyttsuspensjon i nærvær av økte konsentrasjoner (25 til 150 ug/ml) av murint MAb P3 og dets Ab2 MAb 1E10 (deponeringsnummer ECACC 97112901). Isotype-matchede murine MAb'er ble anvendt som kontroller. Stimuleringsindekser lik eller høyere enn 3 ble betraktet som positive. Lymfocytter oppnådd etter immuniseringen av Balb/c mus med P3 MAb prolifererte spesifikt i nærvær av dette MAb'et, og denne proliferasjonen ble ikke observert når cellene ble dyrket i nærvær av murint kontroll MAb A3 (IgM) (Alfonso M. et al.: Hybridoma 14; 1995: 209-216). Denne proliferasjonen var avhengig av nærvær av T-celler, fordi den ble ikke oppnådd med lymfeknuteceller fra P3-immuniserte atymiske nu/nu mus. Immunisering med murint MAb P3 induserte ikke bare proliferasjon av T-celler mot idiotypen av dette Ab1 MAb'et (T2), men induserte også proliferasjon av anti-antiidiotypiske T-celler (T3) spesifikk for murint Ab2 MAb 1E10 (lgG1) og ikke mot MAb C5 av samme isotype (lgG1) (Figur 1).
Eksempel 2: Aktivering av antiidiotypisk (T2) og anti-antiidiotypisk (T3) indusert ved immunisering med det kimæriske antistoffet P3.
Kimærisk antistoff P3 ble anvendt for å immunisere BALB/c hunnmus, 6-8 uker gamle. For den første subkutane dosen ble 100ug av antistoffet emulgert i ACF anvendt. Syv dager senere ble dyr reimmunisert med 50ug av antistoffet emulgert i Al F. På dag 10 ble væske fra lymfeknuter (LN) samlet og LN cellelymfoproliferativ kapasitet ble analysert ved inkubering i nærvær av de murine og kimæriske variantene av antistoffene P3 og Ab2 1E10. C5 murint MAb og dets kimæriske variant ble anvendt som isotype-matchede kontroller (patentsøknad WO 97/33916 A1). Lymfocytter fra mus immunisert med det kimæriske antistoffet P3 prolifererte spesifikt når de ble dyrket i nærvær av dette antistoffet eller kimærisk 1E10 MAb og også når de ble dyrket i nærvær av de murine variantene av disse antistoffene. Spesifisitet av proliferasjon ble demonstrert ved anvendelse av isotype-matchede kontroll MAb'er. Derfor opprettholder kimærisk P3 MAb evnen den murine varianten har til å indusere spesifikk proliferasjon av antiidiotypiske (T2) og anti-antiidiotypiske (T3) T-celler (Figur 2).
Eksempel 3: Forsterkning av antitumoreffekten av GM3-VSSP/Montanid ISA 51 vaksinen ved dens kombinasjon med en vaksine sammensatt av murint MAb 1E10 adsorbert i aluminiumhydroksyd.
En gruppe 6-8 uker gamle C57BL/6 hunnmus ble immunisert subkutant på dag 0, 14, 28 og 42 med 50ug murint 1E10 MAb adsorbert i aluminiumhydroksyd. På dag 7, 21, 35 og 49 mottok mus intramuskulære doser med 120ug GM3-VSSP/Montanid ISA 51. En annen gruppe med mus ble immunisert på en lignende måte, men begynte immuniseringene med vaksinefremstillingen inneholdende GM3-gangliosid. Kontrollgrupper immunisert separat på de samme tidsintervallene og med samme antall doser av hver vaksine eller bare med fosfatbufret saltvannsløsning ble anvendt i disse forsøkene. På dag 63 ble mus i alle grupper inokulert subkutant med 10.000 celler av den murine tumoren B16. Tumorvekst ble evaluert i alle grupper.
"Mixed Lineal Pattern" statistisk dataanalyse ble anvendt for sammenligning av tumorvekst mellom alle grupper og gruppekontrollen som bare mottok PBS. Vaksinasjon med 1E10 MAb alene beskyttet ikke mus mot en eksponering for 10.000 B16-melanomceller. VSSP-GM3 vaksine forårsaket en bremsing av tumorvekst (p <0,05). Kombinasjonene av GM3-VSSP og 1E10 MAb vaksiner førte til en økning i den beskyttende effekten observert med GM3-VSSP vaksine alene (p <0,05) (Figur 3).
Eksempel 4: Immuniseringsplan for kreftpasienter med NeuGc-GM3/VSSP gangliosidvaksine ved anvendelse av Montanid ISA 51 som adjuvans.
Med formål om å demonstrere sikkerheten og immunogeniteten av immunisering med NeuGc-GM3/VSSP vaksinen ved anvendelse av Montanid ISA 51 som adjuvans (patent US 5.788.985 og US 6.149.921), ble en klinisk studie utført hvor 20 pasienter med fremskredet ondartet melanom, som ikke var kvalifisert til noen annen onkospesifikk behandling, ble immunisert med vaksinen.
Pasientene mottok 9 doser av vaksinefremstillingen inneholdende 200 ug GM3-gangliosid. Doser ble administrert på dag 0, 14, 28, 42, 56, 84, 112, 140 og 168.1 henhold til legens kriterier mottok pasienter ytterligere doser hver 28. dag etter den sjette måneden inntil de fullførte ett år med behandling.
Blodprøver ble tatt for rutinebiokjemibestemmelser og for å evaluere serumtitere av anti-NeuGcGM3-gangliosidantistoffer på dag 0, 14, 28, 56, 112, 168, 224, 280 og 332.
Antistofftitere ble målt ved ELISA. Serumfortynninger ble betraktet som positive når antigangliosid OD-verdier var lik eller høyere enn 0,1 (referert til anti-metanol
OD).
Toksisiteten til vaksinen NeuGcGM3-VSSP/Montanid ISA 51 bestod av erytem, lokal smerte, indurasjon i injeksjonsstedet og feber, og dette muliggjorde klassifisering av toksisiteten som mild, grad I/Il i henhold til OMS-kriteriene.
Pasienter utviklet spesifikke antistofftitere mot NeuGcGM3 (mellom 1:80 og 1:2560). Detektert antistoffisotype omfattet IgG og IgM (alle pasienter) og IgG (75 % av pasientene; Tabell I).
Hos pasient 01, som ble med i studien med en diagnose av fremskredet ondartet melanom (utviklet metastatisk sykdom, EMD), ble regresjon og stabilisering av noen hudlesjoner observert etter to måneder med behandling, med ufarget ring rundt disse lesjonene hvor tilstedeværelsen av inflammatorisk infiltrat og nekrose ble demonstrert ved anatomisk patologiske undersøkelser i biopsier (Figur 4).
Hos pasient 02, med diagnosen fremskredet ondartet melanom, ble en stabilisering av lungemetastaselesjon observert i høyre vertex 18-20 mm i løpet av minst 4 måneder (Figur 5).
Eksempel 5: Immuniseringsplan for kreftpasienter med en idiotypisk vaksine inneholdende 1E10 Ab2 MAb aluminiumhydroksydpresipitat.
Med formål om å demonstrere sikkerheten og immunogeniteten til en idiotypisk vaksine inneholdende murint antiidiotypisk MAb 1E10 (patent US 5.817.513 og US 6.063.379) og aluminiumhydroksyd som adjuvans, ble en klinisk studie utført med 20 pasienter med fremskredet ondartet melanom, som ikke var kvalifisert til noen annen onkospesifikk behandling.
Pasienter mottok 4 doser av vaksinen bestående av 2 mg 1E10 MAb. Blodprøver ble tatt før behandlingen og 14 dager etter hver immunisering for rutinebiokjemibestemmelser og for å evaluere serumtitere av antistoffer mot 1E10 MAb idiotype og NeuGc-GM3-gangliosid. Antistofftitere ble målt ved en ELISA-test, der positive verdier ble betraktet som de lik eller høyere enn 0,15.
Vaksineadministrering til pasientene produserte mild toksisitet, klassifisert som grad I og II, i henhold til OMS.
116 av de 17 evaluerbare pasientene ble sterke IgG Ab3-antistoffresponser
indusert som var detekterbare etter å ha mottatt 2 eller 3 doser av vaksinen. Analysen av spesifisiteten til dette Ab3 viste en preferensiell gjenkjennelse av pasientens serum av 1E10 MAb, når sammenlignet med andre isotype-matchede kontroll MAb'er, som antydet induksjon av idiotype-spesifikk komponent i antistoffresponsen til 1E10 MAb. Dette ble bekreftet av den sterke serumreaktiviteten mot F(ab')2-fragmentene av dette MAb'et, med en median titer på 1:15000 (titerefra 1:10000 til mer enn 1:100000), med liten eller ingen gjenkjennelse av F(ab')2-fragmentene av kontroll-MAb'ene anvendt som kontroller (Figur 6).
Antistoffene dannet mot NeuGcGM3 var, i de fleste tilfellene, både IgM og IgG, med titere henholdsvis på 1:4000 og 1:800 (Figur 7).
En pasient med diagnosen ondartet melanom og levermetastaser ble inkludert i den kliniske studien; etter behandlingen ble det vist stabilisering av sykdommen i 8 måneder og pasientens overlevelse var 15 måneder.
Eksempel 6: Kombinert immunisering med antiidiotypisk vaksine omfattende Ab2 MAb1E10 og NeuGcGM3-VSSP gangliosidvaksine.
En melanompasient med metastaser, som hadde blitt underkastet månedlig kirurgi, etter diagnosen, mottok 6 doser av den idiotypiske vaksinen inneholdende Ab2 MAb1E10 og aluminiumhydroksydgel (2 mg av MAb pr. dose), dagene 0, 14, 28, 42 og 56.1 løpet av denne perioden ble også gangliosidvaksinen inneholdende 200 ug NeuGcGM3-VSSP gangliosid og Montanid ISA 51 som adjuvans administrert til pasienten på dag 7, 21, 35,49 og 63. Etter at pasienten mottok dette immuniseringsskjemaet ble han reimmunisert månedlig med begge vaksiner samtidig i løpet av to måneder. For vaksinasjonsperioden forekom ingen nye lesjoner og pasienttilstanden var god.
Eksempel 7: Gjenkjennelse av tumorvev ved MAb 14F7.
NeuGcGM3-gangliosid blir gjenkjent av MAb 14F7 (patentsøknad WO 99/40119).
Gjenkjennelse av tumorvev ved antistoffet er vist.
Formalinfiksert vev ble innesluttet i parafin. Histologien ble evaluert i hematoksylin-eosin-fargede snitt.
Disse snittene ble immunmerket med komplekset biotin-estreptavidin-peroksydase (Hsu, S. M. og Raine, L. 1981, J. Histochem Cytochem., 29:1349-1353). Kort fortalt ble parafinen fjernet og fuktige snitt ble behandlet med H2O23 % (i metanolløsning) i 30 minutter for å redusere endogen peroksydaseaktivitet. Etter inkubering med MAb 14F7 (rent) i 1 time ved romtemperatur ble biotinylerte anti-mus immunglobuliner og streptavidin-peroksydase kompleks tilsatt (Dakopatts) ved romtemperatur; mellom inkuberingene ble snittene vasket med en Tris-HCI bufferløsning. Reaksjonen (POD) ble utviklet med 5 ml Tris-HCI, 0,005 ml bufferløsning inneholdende H2O2til 30 % og 3-3 diaminobencidin 3 mg.
Etter vasking med hematoksylin-kontrastvann (Mayer) ble snittene dekket med balsam og dekket. Enzymreaksjonen gir en brun farge.
De friske biopsiene fra patologiske vev ble skaffet til veie én time etter kirurgi. Alle vevene ble vasket med saltvannsløsning og frosset i flytende nitrogen umiddelbart og de ble konservert frosset ved -80°C.
De frosne fragmentene ble kuttet i en Leica kryostat ved -25°C. Seriesnitt med 5 um ble oppnådd, lufttørket og anvendt umiddelbart eller konservert ved -20°C pakket inn i aluminiumspapir; etterpå ble snittene fiksert med paraformaldehyd 4 % i 20 minutter.
I Tabell II er det vist immunfarging med MAb 14F7 i mange humane tumorer. En sterk membran- og cytoplasmafarging er observert i mer enn 50 % av tumorcellene. Fargen var veldig intens i kolonkarsinomer, livmor, eggstokk, sarkomer, lymfeknuter og hjernemetastaser av brystkreft, så vel som melanommetastaser.

Claims (9)
1. Immunterapeutisk kombinasjon for immunterapi av tumorer som overuttrykker gangliosider,
karakterisert vedat den omfatter de følgende komponentene: (B) en idiotypisk vaksine omfattende et spesifikt murint antiidiotypisk monoklonalt antistoff (Ab2) mot et monoklonalt antigangliosidantistoff; og (C) en gangliosidvaksine.
2. Immunterapeutisk kombinasjon ifølge krav 1,karakterisert vedat nevnte vaksine B omfatter det murine antiidiotypiske antistoffet 1E10 (deponeringsnummer ECACC 97112901).
3. Immunterapeutisk kombinasjon ifølge krav 1,karakterisert vedat nevnte vaksine C omfatter et NeuGcGM3-gangliosid.
4. Immunterapeutisk kombinasjon ifølge krav 1,karakterisert vedat nevnte vaksine C omfatter et NeuAcGM3-gangliosid.
5. Immunterapeutisk kombinasjon ifølge krav 1 til 4,karakterisertved at B og C blir administrert samtidig eller alternerende.
