Aposito para heridas
Esta invention se refiere a un aposito para heridas, en particular, a la cinta o aposito de tira del tipo compuesto de fibras formadoras de gel en forma de una capa o capas de tela tejida o no tejida. En particular, la invencion se refiere a apositos que comprenden fibras formadoras de gel utilizados en el tratamiento de heridas de cavidades o de los senos nasales o heridas postoperatorias.
Se conoce el uso de materiales celulosicos carboximetilados en situaciones en las que se requiere un alto grado de absorcion de exudado. Por ejemplo, WO 93/12275 describe la production de diversos productos absorbentes capaces de absorber muchas veces su propio peso de agua. Esto hace que las fibras carboximetiladas formen un gel. WO 94/16746 y WO 00/01425 describen el uso de materiales de Liocel carboximetilado en apositos para heridas donde se debaten las ventajas de la formation de gel en la prevention de la adherencia y por lo tanto la reduction de los danos en la herida y el dolor en la elimination. US 5 807 295 A revela un material de vendaje medico, por ejemplo, para su uso como un aposito para heridas, vendaje de soporte de tejidos blandos, vendaje de aparato ortopedico o ferula ortopedica, comprende dos capas superpuestas de, por ejemplo, una tela tejida, material de punto o no tejido separados por hebras de capas mono- filamentosas o fibrosas. Los espacios intersticiales se pueden rellenar con una resina endurecible y/o el agente farmacologicamente activo. WO 93/11805 A1 revela biomateriales compuestos de membranas de material compuesto biodegradables, biocompatibles, bioabsorbibles y para uso en cirugla para la regeneration guiada de tejidos. Las membranas de material compuesto se componen de hilos embebidos en una matriz, en donde la matriz y los hilos pueden estar compuestos de esteres del acido hialuronico, utilizados solos o en combination, o esteres de acido hialuronico en combinacion con esteres de acido alglnico u otros pollmeros. US 2007042024 A1 revela un material para su uso como un aposito para heridas, siendo en forma de un rollo y que comprende fibras formadoras de gel y el material que tiene llneas de costura longitudinales.
Apositos para heridas conocidos que comprenden fibras formadoras de gel son esencialmente planos, rectangulares y bastante pequenos, por lo general de 20 cm X 15 cm. La utilidad de tales apositos se limita en relation con las heridas de los senos paranasales o de las cavidades debido a la dificultad en retirar el aposito de dicha herida. El gel de fibras formadoras de gel en la absorcion del exudado y en consecuencia pierden resistencia a la traction una vez estan en un estado gelificado. Esto presenta un problema cuando el aposito debe ser removido ya que la remocion generalmente se realiza tirando de la cinta de la herida desde un extremo de ella. La perdida de resistencia a la traccion implica que queden fragmentos de aposito de la remocion y tengan que ser removidos en muchos pedazos o por lavado.
Sin embargo, serla deseable traer las ventajas de gel que forma apositos de fibras para heridas de cavidad teniendo los apositos disponibles en una forma de tira con la suficiente resistencia a la traccion para permitir que el aposito pueda ser removido en una sola pieza de la herida una vez que se ha gelificado y que pueda ser removido en una sola pieza, independientemente de la parte que el aposito se agarre en la remocion.
Es conocida para la formacion de apositos de cinta con una malla de refuerzo con el fin de mejorar la resistencia a la traccion del aposito. Sin embargo, existen inconvenientes en hacerlo. La malla resta valor a la capacidad de absorcion del aposito y puede crear una barrera flsica para la absorcion. La malla tambien hace que el aposito sea opaco lo que significa que la herida y la piel circundante no se puedan observar una vez que el aposito esta in situ.
Es conocido que para aumentar la resistencia a la traccion de los vendajes por costura del vendaje a lo largo de su longitud con una o mas llneas de costura. Sin embargo, cuando la costura longitudinal se aplica a una tira delgada da fuerza solo en la direction de la costura y restringe la manera como el aposito se puede remover.
Por consiguiente, la presente invencion pretende proporcionar un mejor aposito para heridas que mitigue los problemas asociados con los apositos de cinta en las heridas de cavidades o senos paranasales.
Se ha encontrado ahora que es posible mejorar la resistencia a la traccion de los apositos de la tira en un estado (gelificado) seco o humedo.
De acuerdo con lo anterior, la invencion proporciona un aposito para heridas que comprende una capa en forma de una tira caracterizado porque la tira comprende fibras formadoras de gel, la tira tiene llneas longitudinales de costura formadas a partir de un hilo y llneas transversales de costura formadas a partir de un hilo, donde el hilo es de nylon, Tencel, poliolefina, poliuretano, poliester o celulosico y el aposito para heridas es para su uso en heridas de cavidades o heridas de los senos paranasales.

El hilo puede ser un solo filamento o hilo de filamento multiple o un hilo de fibra cortada. El hilo es de nylon, poliolefina, poliuretano, poliester o celulosa. El hilo se puede impregnar con un agente activo, por ejemplo, agente antimicrobiano.
Tales apositos son adecuados para el tratamiento de heridas de cavidades o de los senos nasales, postoperatorias o quirurgicas o cualquier herida que necesite ser cubierta.
La costura longitudinal pasa preferiblemente a traves de todo el espesor de la tira y puede ser visible en ambos lados de la tira. La costura transversal tambien puede pasar a traves de todo el espesor de la tira o puede estar presente en un solo lado de la tira o en ambos.
Por fibras formadoras de gel se entiende fibras higroscopicas que, despues de la absorcion de exudado de la herida se hacen humedas resbaladizas o gelatinosas y por lo tanto reducen la tendencia de las fibras circundantes a adherirse a la herida. Las fibras formadoras de gel pueden ser del tipo que retiene su integridad estructural en la absorcion de exudado o pueden ser del tipo que pierde su forma fibrosa y se convierten en un gel sin estructura. Las fibras formadoras de gel se hilan preferiblemente fibras de carboximetilcelulosa sodica, fibras celulosicas modificadas qulmicamente, fibras de pectina, fibras de alginato, fibras de quitosano, fibras de acido hialuronico u otras fibras de polisacaridos o fibras derivadas de gomas. Las fibras celulosicas tienen preferiblemente un grado de sustitucion de al menos 0.05 grupos de carboximetilo por unidad de glucosa. Las fibras formadoras de gel preferiblemente tienen una absorbancia de al menos 2 gramos 0.9% de solucion salina por gramo de fibra (tal como se mide por el metodo de hinchamiento libre).
Preferiblemente, las fibras formadoras de gel tienen una absorbancia de al menos 10 g/g como se mide en el metodo de absorbancia de libre eleccion, mas preferiblemente entre 15 g/g y 25 g/g.
El aposito puede, por ejemplo, comprender fibras no formadoras de gel y, en particular, puede comprender lycra u otra fibra elastica.
El aposito puede tener una forma de 0.5, 1, 2 o mas metros de longitud y ser de aproximadamente 0.5 cm a 10 cm de ancho, preferiblemente de 0.5 cm a 5 cm de ancho. Las llneas longitudinales de costura pueden ser de 1 mm a 10 mm de distancia y ser paralelas a un borde longitudinal de la tira. Preferiblemente, las llneas longitudinales de costura son de 2 mm a 5 mm de separacion. La carboximetilacion se puede alcanzar, por ejemplo, por el tratamiento secuencial o simultaneo del material celulosico con un alcali fuerte, tal como hidroxido de sodio acuoso, y acido monocloroacetico o una sal del mismo. Las condiciones de reaccion apropiadas dependeran de la composicion del tejido y el grado de carboximetilacion requerido y resultaran facilmente evidentes para el experto en el arte. El grado de carboximetilacion es de manera deseable de tal manera que tras la absorcion de exudado de las fibras en la superficie de contacto con la piel del vendaje formen un gel.
El aposito puede ser usado como parte de un aposito de material compuesto.
En un aspecto adicional la invencion proporciona un metodo de fabricacion de un aposito para heridas para su uso en heridas en cavidades o senos paranasales, caracterizado porque el metodo comprende las etapas de:
(i) formar un rollo de tela que comprende fibras formadoras de gel;
(ii) coser el rollo con las llneas de costura longitudinal;
(iii) coser el rollo con la costura transversal; y
(iv) cortar el rollo en una direccion longitudinal para formar tiras.
El rollo de tela puede formarse haciendo una red no tejida de fibras formadoras de gel. El rollo de tela puede estar formado por tejido de un rollo de fibras formadoras de gel. La red no tejida puede estar hecha por hidroenmaranado de una red de fibras de Liocel y carboximetilado de la res as! formada. El metodo comprende la etapa adicional de la superposicion de una segunda tira en la primera tira antes de que las tiras se cosan juntas.
Preferiblemente, la costura transversal se realiza en columnas uniendo las llneas longitudinales de costura de modo que se crean huecos libres de puntada entre las columnas. De esta manera una cinta puede ser cortada del rollo por los huecos de modo que se producen cabos sueltos mlnimos en los bordes de la tira que de otro modo se podrlan perder en la herida. Preferiblemente, las columnas de puntos de sutura transversales estan aseguradas de manera que no hay hilos sueltos en los espacios entre las columnas y los bordes de la cinta o banda no tienen ningun cabo suelto. Preferiblemente, las columnas de puntos de sutura transversales son una llnea continua de la costura en zig zag entre las llneas longitudinales de puntos de sutura. De esta forma las columnas tienen huecos libres de costura en el espacio entre las columnas que permiten que el rollo se corte en tiras sin extremos sueltos en los bordes.
Las llneas de costura transversal se pueden hacer en columnas menores que la anchura del rollo. El rollo puede estar hendido entre las columnas de la costura transversal, de modo que la tira no tiene ningun hilo suelto en sus bordes. El aposito puede comprender al menos dos llneas longitudinales de costura. Preferiblemente, el aposito tiene varios pares de llneas de costura longitudinal con las llneas en cada par unido por costuras transversales en un patron acanalado para crear huecos libres de costura entre pares adyacentes de llneas longitudinales unidas de puntos de sutura. Esto permite que el aposito se corte en cintas mas delgadas por el usuario.
El uso de una tela de tira en la fabricacion de un aposito para heridas, la tira que comprende fibras formadoras de gel y que tiene llneas de costura longitudinal y llneas de costura transversal para su uso en el tratamiento de heridas de las cavidades o de los senos paranasales.
Las realizaciones preferidas de la invencion se describiran ahora con referencia a los dibujos adjuntos en los que:
La figura 1 es una vista de una capa de fibras formadoras de gel en forma de un rollo con llneas longitudinales de costura unidas por llneas transversales de costura en zig zag en forma angular antes del corte.
La figura 2 es una vista de una capa de fibras formadoras de gel en forma de un rollo con las llneas longitudinales de la costura y llneas transversales de costura en forma de un patron acanalado antes del corte.
La figura 1 muestra un rollo no tejido de fibras formadoras de gel hechas por una tecnica de aguja de fieltro de cardado para formar una red. Opcionalmente, el rollo puede tener un material antimicrobiano incorporado en el y, en particular plata por el metodo descrito en WO 02/43743. El rollo se cose en la direccion longitudinal con las llneas de costura en hilo Tencel. Las llneas longitudinales de puntos de sutura se complementan con llneas transversales de costura en forma de zig zag continuos angulares que se extienden entre las llneas longitudinales adyacentes de puntos de sutura. De esta manera huecos libres de puntada se dejan entre las columnas de costura longitudinal. El rollo se corta en la direccion longitudinal en los huecos libres de puntada para formar las cintas.

La figura 2 muestra un rollo no tejido similar al mostrado en la figura 1, excepto que el zig zag continuo de puntos de sutura transversales se realiza en un patron acanalado entre las llneas longitudinales de puntos de sutura y uniendolos juntos. El rollo se corta en la direction longitudinal en los huecos libres de puntada para formar las cintas.
Las realizaciones preferidas de la invention se describiran ahora con referencia a los siguientes ejemplos:
Ejemplo 1
Aposito A
Un aposito para heridas se hace de un rollo de fibras formadoras de gel como se describe para el aposito de la figura 1. El rollo tenia llneas de costura longitudinales separadas 5 mm. El ancho de la columna era de 2.5 cm. Las cintas fueron cortadas de cada rollo por un corte longitudinal en una direccion longitudinal en los huecos entre las columnas de la costura transversal.
Aposito B
Un aposito para heridas alternativo fue hecho mediante la superposition de dos rollos de fibras formadoras de gel como se describe para el Aposito A y una costura como se ha descrito para el aposito A.
Aposito C
Un aposito para heridas alternativo se hizo mediante la elimination de la costura transversal del Aposito A.
Aposito D
Se formo a partir 100gsm de Aquacel un aposito no tejido hecho de fibras de carboxi metil celulosa ex ConvaTec.
Las muestras de ensayo se cortaron de los rollos cosidos para tener las dimensiones de 25 mm de ancho por 100 mm de largo para las muestras humedas y 25 mm de ancho por 75 mm de largo para las muestras secas. La resistencia a la traction de las muestras gelificadas secas se midio en la direccion longitudinal y transversal de la siguiente manera.
Ensayo de traccion en seco
Las muestras se acondicionaron a 20°C ± 2°C y 65% 4RH durante un perlodo mlnimo de 24 horas. Las muestras fueron fijadas en las mordazas neumaticas de un Zwick U.T.M. equipado con una celda de carga de 100N. La muestra se alarga a una velocidad de 100 mm/min hasta que se midio una reduction del 75% de la fuerza maxima de las muestras.
Ensayo de traccion en humedo
Las muestras se acondicionaron a 20°C ± 2°C y 65% ± 4RH durante un perlodo mlnimo de 24 horas. 2 ml de una solution de sodio y cloruro de calcio BP se dispenso a traves de una pipeta en el centro de la muestra y se dejo durante un perlodo de 1 minuto. La muestra se sujeta dentro de las mordazas neumaticas de un Zwick U.T.M. equipado con una celda de carga de 100N. La muestra se alargo a una velocidad de 100 mm/min hasta que se midio una reduccion del 75% de la fuerza maxima de las muestras.
Los resultados se dan a continuacion.

Estos resultados muestran la mejora en la resistencia a la traccion en muestras cosidas transversales. Ejemplo 2
El aposito A se utilizo para cubrir una herida de seguimiento. Al remover el vendaje de la herida, el aposito de cinta estaba completamente hidratado con fluido de la herida, sin embargo, habia mantenido su estructura. El aposito se retiro facilmente de la herida en una sola pieza.

    Reivindicaciones
    1. Un aposito para heridas que comprende una capa en forma de tira, caracterizado porque la tira comprende fibras formadoras de gel, la tira que tiene llneas longitudinales de costura formadas a partir de un hilo y llneas transversales de costura formadas a partir de un hilo, donde el hilo es nylon, Tencel, poliolefina, poliuretano, poliester o celulosico y el aposito para heridas es para su uso en heridas de cavidades o heridas de los senos paranasales.
  2. 2. El aposito para heridas como se reivindica en la reivindicacion 1, caracterizado porque las llneas longitudinales de la costura son de 1 mm a 10 mm de distancia y son paralelas a un borde largo de la tira.
  3. 3. El aposito para heridas como se reivindica en cualquier reivindicacion precedente, caracterizado porque el aposito comprende una segunda tira, superpuesta sobre la primera tira en donde las llneas longitudinales de costura unen las dos tiras juntas.
  4. 4. El aposito para heridas como se reivindica en cualquier reivindicacion precedente, caracterizado porque las llneas transversales de costura se cosen a traves de la tira.
  5. 5. El aposito para heridas como se reivindica en cualquier reivindicacion precedente, caracterizado porque las llneas transversales de costura se extienden entre las llneas longitudinales de costura y se unen juntas.
  6. 6. El aposito para heridas como se reivindica en cualquier reivindicacion precedente, caracterizado porque las fibras formadoras de gel se seleccionan del grupo de: fibras hiladas de celulosa, fibras celulosicas modificadas qulmicamente, fibras de pectina, fibras de alginato, fibras de quitosano, fibras de acido hialuronico, otras fibras de polisacaridos y fibras derivadas de gomas.
  7. 7. Un aposito para heridas como se reivindica en cualquier reivindicacion precedente, caracterizado porque las llneas transversales de costura se terminan en los bordes de la tira para reducir el deshilachado.
  8. 8. Un aposito para heridas como se reivindica en cualquier reivindicacion precedente, caracterizado porque las llneas de costura transversal estan en la forma de un zig-zag continuo que se extiende en las columnas entre las llneas longitudinales de la costura.
  9. 9. Un aposito para heridas como se reivindica en cualquier reivindicacion precedente, caracterizado porque el aposito se usa como parte de un aposito de material compuesto.
  10. 10. Un aposito para heridas como se reivindica en cualquier reivindicacion precedente, caracterizado porque el aposito comprende al menos dos llneas longitudinales de costura.
  11. 11. Un metodo de fabrication de un aposito para heridas segun la reivindicacion 1, para uso en heridas de cavidades o de los senos paranasales, caracterizado porque el metodo comprende las etapas de:
    (i) formar un rollo de tela que comprende fibras formadoras de gel;
    (ii) coser el rollo con llneas longitudinales de costura;
    (iii) coser el rollo con llneas transversales de la costura; y
    (iv) cortar longitudinalmente el rollo en una direction longitudinal para formar tiras.
  12. 12. Un metodo como el reivindicado en la reivindicacion 11, caracterizado porque el rollo de tela se forma haciendo una red no tejida de fibras formadoras de gel.
  13. 13. Un metodo como el reivindicado en la reivindicacion 11, caracterizado porque el rollo de tela se forma tejiendo un rollo de fibras formadoras de gel.
  14. 14. Un metodo como el reivindicado en la reivindicacion 12, caracterizado porque la red no tejida se hace por hidroenmaranado de una red de fibras de Liocel y carboximetilando la red as! formada.
  15. 15. Un metodo como se reivindica en cualquier reivindicacion precedente, caracterizado porque el metodo comprende la etapa adicional de tratar el aposito con una fuente de plata para dar propiedades antimicrobianas al aposito.
  16. 16. Un metodo como se reivindica en cualquier reivindicacion precedente, caracterizado porque las llneas de costura transversales se realizan en columnas menores que el ancho del rollo.
  17. 17. Un metodo como el reivindicado en la reivindicacion 16, caracterizado porque el rollo se corta entre las columnas de la costura transversal, de modo que las tiras no dejan hilos sueltos en sus bordes.
  18. 18. Un metodo como el reivindicado en la reivindicacion 13, caracterizado porque el metodo comprende la etapa adicional de:
    superponer una segunda tira a la primera tira antes de que las tiras se cosan juntas.
  19. 19. Uso de una tela de tira en la fabricacion de un aposito para heridas segun la reivindicacion 1, la tira que comprende 5 fibras formadoras de gel y que tiene llneas de costura longitudinal y llneas de costura transversal para uso en el
    tratamiento de heridas en cavidad o de los senos paranasales.
