Abstract
一種負壓傷口敷材,係包括泡棉以及具有立體網狀結構之生物纖維膜,係結合至該泡棉之表面,且在該泡棉與生物纖維膜結合的表面上之該生物纖維膜之投影面積大於或等於該泡棉之投影面積。藉由該生物纖維膜存在於泡棉與傷口之間,施行負壓傷口治療時可以避免泡棉與傷口沾黏以及新生組織長入泡棉的問題,降低更換泡棉時對傷口的二次傷害,並舒緩患者的疼痛。
Description
本創作係關於一種傷口敷材,尤指一種用於負壓傷口治療的傷口敷材。
負壓傷口治療(Negative Pressure Wound Therapy, NPWT)是一種促進傷口癒合的輔助性治療方法,適用於許多急性及慢性傷口,包括骨科、軟組織、皮膚移植、壓瘡、下肢靜脈潰瘍、糖尿病足、外科感染以及術後傷口等。NPWT係於清理過後的傷口中置入與傷口形狀大致相符的泡棉,使泡棉與傷口表面充分接觸,由於泡棉內部有許多孔洞,於是可利用真空泵浦對已填充泡棉的傷口施加負壓,使得傷口中的膿液及感染物質可藉由泡棉的孔洞排出,亦吸引組織液至泡棉內而維持了傷口潮濕的治療環境,並促進傷口周邊血液微循環,增進新生組織的生長以達到加速傷口癒合的功效。
然而,NPWT中的泡棉,尤其係使用疏水性聚氨酯發泡體者,其對於傷口具有高沾黏性,且新生組織容易生長入泡棉內的孔洞之中,另外,泡棉大約2至3天即須更換,若是傷口的感染情形較嚴重,根據醫師的診斷,泡棉的更換頻率可能會更頻繁。每次更換泡棉時,由於傷口的沾黏以及新生組織的長入,往往造成患者的劇烈疼痛,亦有對傷口二次傷害的情況產生,在臨床應用上是很大的困擾。
因此,如何避免泡棉與傷口的沾黏以及避免新生組織長入泡棉內的孔洞實為臨床及業界亟欲改善的課題。
為解決上述問題,本創作提供一種負壓傷口敷材,係包括泡棉以及生物纖維膜,所述生物纖維膜係具有立體網狀結構,且結合至所述泡棉之表面,其中,在該泡棉與生物纖維膜結合的表面上之該生物纖維膜之投影面積大於或等於該泡棉之投影面積。
所述生物纖維膜係指微生物,例如選自由葡糖醋桿菌屬(
Gluconacetobacter)、醋桿菌屬(
Acetobacter)、根瘤菌屬(
Rhizobium)、八疊球菌屬(
Sarcina)、假單胞菌屬(
Pseudomonas)、無色桿菌屬(
Achromobacter)、產鹼菌屬(
Alcaligenes)、腸桿菌屬(
Enterobacter)、固氮菌屬(
Azotobacter)及農桿菌屬(
Agrobacterium)所組成群組中之至少一種細菌,利用培養液中的碳源於細胞內合成β-1,4-糖苷鏈後經胞外分泌排出之纖維素所形成。
於一具體實施態樣中,所述生物纖維膜係使細菌於具有碳源、氮源及視需要之酵母萃取物之培養液中培養所形成。所述培養例如靜置培養,所述碳源例如葡萄糖、甘露醇、糖蜜、果糖、蔗糖等,所述氮源例如蛋白腖。進一步地,所述碳源、氮源及酵母萃取物的重量比可為60:6:6至15:3:0;或60:6:6至15:3:0.5;或60:6:6至15:3:1;或60:6:6至15:3:2;或60:6:6至15:3:3;或60:6:6至15:3:4;或60:6:6至15:3:5。
於一具體實施態樣中,所述立體網狀結構具有複數彼此平行之骨幹纖維以及交織於相鄰任二所述骨幹纖維之複數層間纖維,藉由所述複數骨幹纖維及複數層間纖維彼此交織構成奈米孔洞。
又於一具體實施態樣中,所述骨幹纖維以及層間纖維皆係生物纖維,且所述骨幹纖維之直徑大於或等於所述層間纖維之直徑。
於一具體實施態樣中,所述生物纖維之直徑為20至100 nm。
於一具體實施態樣中,所述生物纖維膜之每單位面積生物纖維量係0.0013至0.0018 g/cm
2。
於一具體實施態樣中,所述負壓傷口敷材復包括藥物,例如具有抗菌、抗發炎、促進細胞生長等之促進傷口癒合之藥物,包括但不限於由抗生素、抗菌劑、生長因子、纖維蛋白、血管擴張劑、血管新生促進劑、抗氧化劑、抗發炎劑所組成群組中之至少一種。更具體而言,該藥物係包含於該泡棉中、生物纖維膜中或該泡棉及生物纖維膜二者之中。
於一具體實施態樣中,所述泡棉係包括選自由聚氨酯發泡體、聚乙烯醇發泡體、聚醚發泡體、聚酯發泡體、聚乳酸發泡體、聚烯烴發泡體、甲殼素發泡體、纖維素發泡體、海藻酸發泡體、明膠發泡體、膠原蛋白發泡體所組成群組中之至少一種材質。
又一具體實施態樣中,所述泡棉具有多層結構,例如接觸層、吸收層、引流層等,其中,該接觸層係接觸生物纖維膜。
本創作之負壓傷口敷材透過生物纖維膜結合於泡棉之表面,於施行負壓傷口治療(NPWT)時,使填入傷口之泡棉與傷口表面之間存在有生物纖維膜,以於更換敷材時,避免傷口沾黏敷材,減輕患者的劇烈疼痛。進一步地,由於生物纖維膜具有立體網狀結構,立體網狀結構中有骨幹纖維及層間纖維交織形成奈米孔洞,具有極佳的透氣性,不會破壞負壓環境,另一方面,生物纖維膜的奈米孔洞以及泡棉的孔洞亦可以承載藥物,得以於抽出膿液及感染物質的同時釋放藥物於傷口表面,促進傷口的癒合。
以下係藉由特定的具體實施例說明本創作之實施方式,熟習此技藝之人士可由本說明書所揭示之內容輕易地瞭解本創作之優點及功效。本創作亦可藉由其它不同之實施方式加以施行或應用,本說明書中的各項細節亦可基於不同觀點與應用,在不悖離本創作所揭示之精神下賦予不同之修飾與變更。此外,本文所有範圍和值都係包含及可合併的。落在本文中所述的範圍內之任何數值或點,例如任何整數都可以作為最小值或最大值以導出下位範圍等。
本創作之負壓傷口敷材的製備上,係將泡棉置於裝有培養液的容器之底面或固定於裝有培養液的容器中,並令培養液之液面略高於泡棉,使泡棉完全沉浸於培養液,再於培養液中靜置培養包括選自由葡糖醋桿菌屬(
Gluconacetobacter)、醋桿菌屬(
Acetobacter)、根瘤菌屬(
Rhizobium)、八疊球菌屬(
Sarcina)、假單胞菌屬(
Pseudomonas)、無色桿菌屬(
Achromobacter)、產鹼菌屬(
Alcaligenes)、腸桿菌屬(
Enterobacter)、固氮菌屬(
Azotobacter)及農桿菌屬(
Agrobacterium)所組成群組中之至少一種細菌,較佳地,例如葡糖醋桿菌屬或醋桿菌屬之單一種細菌。培養時間較佳為24至96小時,培養後,生物纖維膜形成於泡棉上,且生物纖維膜結合至泡棉之表面,其中,培養液包括有碳源、氮源以及酵母萃取物。
所述泡棉於水平面的投影面積需小於等於培養液之液面面積,以令由氣液介面所形成之生物纖維膜於在該泡棉與生物纖維膜結合的表面上之投影面積大於或等於泡棉的投影面積。
如第1圖所示,本創作的負壓傷口敷材1包括泡棉10;以及設於所述泡棉10上的生物纖維膜12,以使生物纖維膜12結合至泡棉10之表面,且生物纖維膜12在該泡棉10與生物纖維膜12結合的表面上之投影面積大於泡棉10的投影面積。於另一實施態樣中,生物纖維膜12在該泡棉10與生物纖維膜12結合的表面上之投影面積可以等於泡棉10的投影面積。
於一具體實施態樣中,培養液包括作為碳源之甘露醇、作為氮源之蛋白腖及視需要之酵母萃取物,其重量份比例為60:6:6至15:3:0,且培養液被控制為酸性,例如pH值為0.5至6.5之間,培養液中微生物濃度的吸光值(波長設定620nm)範圍被控制在0.006至0.01,培養溫度為25至28℃,培養時間為24至96小時。經檢測,所述生物纖維膜的厚度為至少0.3至0.6mm,生物纖維膜中之生物纖維的直徑約20至100nm。又,所述生物纖維膜的每單位面積生物纖維量為0.0013至0.0018 g/cm
2。
所述靜置培養係指使細菌在培養液的表面以非織造方式形成生物纖維,該生物纖維由液面向容器底部累積、延伸,並逐漸觸及泡棉之表面,最終該生物纖維長入泡棉之表面,令生物纖維膜與泡棉結合。
此外,靜置培養使用的容器較佳為扁平狀的容器,其係指相較於容器長度和寬度而言,容器的高度較低,以透過較低的容器高度,較寬廣的生長面積,控制細菌的耗氧量,進而達到生物纖維直徑的調控。
於一具體實施態樣中,所述生物纖維膜材係由葡糖醋桿菌屬之細菌於具有甘露醇、蛋白腖、酵母萃取物之培養液中靜置醱酵而成。
所述葡糖醋桿菌屬之細菌係以選擇木質醋酸菌(
Gluconacetobacter xylinum)為佳,其特色在於容易控制所得之生物纖維粗細程度。
於一具體實施態樣中,本創作的負壓傷口敷材中之具有立體網狀結構之生物纖維膜係如第2A圖所示,立體網狀結構121由複數生物纖維所構成,具體而言,該立體網狀結構121具有複數彼此平行、或於該生物纖維膜長度方向或寬度方向延伸的骨幹纖維121a及與該骨幹纖維121a交織的層間纖維121b,如此,層間纖維121b在水平方向及垂直方向上連接相鄰任二該骨幹纖維121a以形成三維立體網狀結構121。而該骨幹纖維121a及層間纖維121b皆為生物纖維,且如圖所示,該骨幹纖維121a的直徑大於或等於該層間纖維121b的直徑。根據本創作之具體實施態樣,所述生物纖維膜長度方向或寬度方向所構成的表面係該泡棉與生物纖維膜結合的表面。
再參閱第2B圖,其顯示生物纖維膜的側視照片,其中該生物纖維膜12亦具有複數彼此平行、或於該生物纖維膜12長度方向或寬度方向延伸的骨幹纖維121a及與該骨幹纖維121a交織的層間纖維121b。
所述泡棉於本創作中並未限定,只要是開放式(open cell)且具有生物相容性的泡棉即可,例如聚氨酯發泡體、聚乙烯醇發泡體、聚醚發泡體、聚酯發泡體、聚乳酸發泡體、聚烯烴發泡體、甲殼素發泡體、纖維素發泡體、海藻酸發泡體、明膠發泡體及膠原蛋白發泡體等。
所述泡棉可使用由單一材料所製成者,亦可使用由複數材料所製成者,形成一具有複合結構之泡棉。進一步,所述泡棉亦可具有多層結構,該多層結構可指各層為連續不斷的相同材質,但具有不同的物理特性,如密度、孔隙度、滲液傳輸比、壓縮比等,該些特性可以於製造過程中調整各種參數而得;該多層結構亦可指各層為不同的材質,如聚氨酯與聚乙烯醇的分層結構。多層結構中的各層可依需求具有不同的功能,例如接觸層、吸收層、引流層等。
本創作的負壓傷口敷材之泡棉除了可為單層結構外,於如第3圖所示之一具體實施態樣中,本創作之負壓傷口敷材3包括具有多層結構之泡棉30以及生物纖維膜32,多層結構依序包括接觸層301、吸收層302以及引流層303。所述接觸層301係接觸生物纖維膜32,接觸層301具有較大的孔隙,使生物纖維膜32之部分生物纖維可以長入接觸層301之孔隙中,以增進生物纖維膜32與泡棉30之結合性,並可令傷口滲液快速進入泡棉之中;所述吸收層302具有較佳的吸液特性,並可暫時容受傷口滲液;所述引流層303具有較佳的滲液傳輸特性,使泡棉所吸收之滲液可被引流至負壓傷口裝置之液體傳輸管。
於泡棉上形成並結合生物纖維膜後形成本創作之負壓傷口敷材,將所得之負壓傷口敷材進行清洗,使負壓傷口敷材為中性。所述清洗方式例如以80℃以上、1%之NaOH溶液鹼煮15分鐘,再以清水進行清洗。
對清洗後之負壓傷口敷材實施滅菌處理,滅菌處理包括但不限於高壓蒸氣滅菌、γ輻射滅菌、環氧乙烷(EO)滅菌等。較佳係使用高壓蒸氣滅菌法,例如1.2 atm,121℃,20分鐘。
本創作之負壓傷口敷材於一具體實施態樣中可以進一步包括藥物,藥物可以於清洗程序後添加,由於生物纖維膜及泡棉皆具有孔洞,可使藥物留置於生物纖維膜、泡棉或兩者之中。藥物可選用如具有抗菌、抗發炎、促進細胞生長等之促進傷口癒合之藥物,包括但不限於由抗生素、抗菌劑、生長因子、纖維蛋白、血管擴張劑、血管新生促進劑、抗氧化劑、抗發炎劑所組成群組中之至少一種。
本創作之負壓傷口敷材可應用於負壓傷口治療(NPWT),於一具體實施態樣中,負壓傷口敷材與封膜、液體傳輸管、液體收集罐及負壓源組成負壓傷口治療裝置。具體而言,將本創作之負壓傷口敷材填入傷口後(生物纖維膜側朝向傷口表面),貼附封膜完整覆蓋負壓傷口敷材,且封膜之面積需大於傷口,使封膜邊緣可以黏附至傷口以外之正常皮膚。於封膜之中央有一開口,液體傳輸管之一端可以經該開口延伸入負壓傷口敷材之泡棉,而液體傳輸管之另一端連接至液體收集罐,液體收集罐再經另一管線連接至負壓源。經負壓源抽吸,傷口中的膿液及感染物質經生物纖維膜、泡棉及液體傳輸管流至液體收集罐。
由於泡棉與傷口表面之間有生物纖維膜阻隔,生物纖維膜具有優異的抗沾黏性,因此,本創作之負壓傷口敷材不會與傷口產生沾黏,且傷口的新生組織亦不會長入泡棉內,可以降低患者於替換負壓傷口敷材時的疼痛感,亦大大減少對傷口的二次傷害,且透過使藥物留置於泡棉及/或生物纖維膜之孔洞中,可以進一步於治療時釋放藥物於傷口,促進傷口的癒合。

Claims (11)
- 一種負壓傷口敷材,係包括:
泡棉;以及
具有立體網狀結構之生物纖維膜,係結合至該泡棉之表面,且在該泡棉與生物纖維膜結合的表面上之該生物纖維膜之投影面積大於或等於該泡棉之投影面積。 - 如申請專利範圍第1項所述之負壓傷口敷材,其中,該生物纖維膜係包括選自由葡糖醋桿菌屬( Gluconacetobacter)、醋桿菌屬( Acetobacter)、根瘤菌屬( Rhizobium)、八疊球菌屬( Sarcina)、假單胞菌屬( Pseudomonas)、無色桿菌屬( Achromobacter)、產鹼菌屬( Alcaligenes)、腸桿菌屬( Enterobacter)、固氮菌屬( Azotobacter)及農桿菌屬( Agrobacterium)所組成群組中之至少一種細菌所形成。
- 如申請專利範圍第1項所述之負壓傷口敷材,其中,該立體網狀結構具有複數彼此平行之骨幹纖維以及交織於相鄰任二該骨幹纖維之複數層間纖維。
- 如申請專利範圍第3項所述之負壓傷口敷材,其中,該骨幹纖維及該層間纖維皆係生物纖維,且該骨幹纖維之直徑大於或等於該層間纖維之直徑。
- 如申請專利範圍第4項所述之負壓傷口敷材,其中,該生物纖維之直徑為20至100 nm。
- 如申請專利範圍第1項所述之負壓傷口敷材,其中,該生物纖維膜之每單位面積生物纖維量係0.0013至0.0018 g/cm 2。
- 如申請專利範圍第1項所述之負壓傷口敷材,其中,該負壓傷口敷材復包括藥物,係包含於該泡棉中、該生物纖維膜中或該泡棉及生物纖維膜二者之中。
- 如申請專利範圍第7項所述之負壓傷口敷材,其中,該藥物係選自由抗生素、抗菌劑、生長因子、纖維蛋白、血管擴張劑、血管新生促進劑、抗氧化劑、抗發炎劑所組成群組中之至少一種。
- 如申請專利範圍第1項所述之負壓傷口敷材,其中,該泡棉係包括選自由聚氨酯發泡體、聚乙烯醇發泡體、聚醚發泡體、聚酯發泡體、聚乳酸發泡體、聚烯烴發泡體、甲殼素發泡體、纖維素發泡體、海藻酸發泡體、明膠發泡體及膠原蛋白發泡體所組成群組中之至少一種材質。
- 如申請專利範圍第1項所述之負壓傷口敷材,其中,該泡棉係具有單層結構或多層結構。
- 如申請專利範圍第10項所述之負壓傷口敷材,其中,該多層結構依序包括接觸層、吸收層以及引流層,其中,該接觸層係接觸生物纖維膜。
