本发明涉及医用装置的技术领域,具体地说是一种用于创面负压治疗的可降解敷料。
背景技术
1993年Fleischmann发现通过负压吸引密闭于创面上的海绵敷料,可加速创面水肿消退,促进肉芽组织增生。目前,创面负压治疗技术已证实可促进急性、亚急性和慢性创面的愈合。在长期临床应用过程中,创面负压治疗不断改进和发展。与创面负压治疗技术相比,创面灌注-负压治疗技术借助外源性创面灌注液对创面的物理和化学作用,更有利于减轻创面细菌负荷,提高创面组织活性。但无论创面负压治疗还是创面灌注-负压治疗都存在创面肉芽组织生长至创面敷料孔隙中的现象。首先,创面治疗过程中,肉芽组织生长填充创面敷料孔隙,可堵塞创面引流通道,阻碍创面引流效果,导致创面分泌物和坏死产物堆积,增加创面水肿程度和细菌负荷。其次,敷料更换过程中,生长至创面敷料孔隙中的肉芽组织被外力撕裂,可出现创面出血及患者疼痛剧烈的隐患。再次,创面敷料被肉芽组织包裹,当创面敷料更换后,存在部分敷料组织残留于创面的可能。
目前,创面负压治疗所用敷料均无有效措施解决上述问题。因此,急需研发一种创面负压治疗用可降解的敷料,以便有利于创面治疗工作的发展。

本发明的目的在于提供一种改进的可降解敷料,由于对敷料的结构、材料进行了突破和改进,使得该种可降解敷料不仅可以自行降解,更可以随时调节降解的速度,即通过灌注管道对底层敷料进行灌注特定液体,通过负压管道层将降解的水溶性液体和创面产生的分泌物进行抽离,从而在一个相对封闭的环境中通过对可降解敷料的控制实现创面的有效治疗。
为了实现上述目的,本发明的技术方案是:一种用于创面负压治疗的多层敷料,其特征在于:所述的多层敷料包括底层敷料和顶层敷料,底层敷料由可降解材料构成,用于贴附在创面上,底层敷料与顶层敷料之间设有灌注管道层,顶层敷料由不可降解材料构成,上部设有用于固定的贴膜,顶层敷料与贴膜之间设有负压管道层;负压管道层的中心处设有一与负压管道层相垂直的负压进出通道,所述的负压进出通道穿过贴膜并与外部的负压装置相连,灌注管道层的中心处设有一与灌注管道层相垂直的灌注进入通道,所述的灌注进入通道穿越负压进出通道并与外部的灌注装置相连。
优选的,所述的底层敷料由可降解的壳聚糖类材料制成,底层敷料为亲水结构,内部呈互相连通的三维网状结构,所述的三维网状的网孔大小为600-800μm。
优选的,所述的顶层敷料由聚氨酯海绵类材料制成,顶层敷料为疏水结构,顶层敷料布设有孔隙,所述的孔隙大小为600-800μm。
进一步,灌注管道层包括呈放射状均布的灌注副管道,所述的灌注副管道的一端与灌注进入通道相连,灌注副管道和灌注进入通道均采用硅胶制成;负压管道层包括呈放射状均布的负压副管道,所述的负压副管道的一端与负压进出通道相连,负压副管道和负压进出通道均采用硅胶制成。
进一步,所述的灌注副管道与底层敷料之间呈0-15度的夹角,灌注副管道的一端大,另一端小,该大端与灌注进入通道相连。
进一步,底层敷料在潮湿环境中自行降解,底层敷料在催化剂或者消化酶的作用下自行降解速度加快。
更进一步,所述的灌注进入通道可通入催化剂或者消化酶,使催化剂或者消化酶沿灌注副管道到达底层敷料并加速底层敷料的降解;所述的负压进出通道通过外接的负压装置将降解的底层敷料、创面分泌物和坏死组织抽离创面。
更进一步,所述的贴膜由不可降解材料制成贴膜将顶层敷料和底层敷料封闭在贴膜范围内。
相对于现有技术,本发明的技术方案除了整体技术方案的改进,还包括很多细节方面的改进,具体而言,具有以下有益效果:
1、本发明所述的改进方案,采用两种不同材质的底层敷料和顶层敷料,其中底层敷料在特定的环境下会自动降解,而顶层敷料是采用不可降解的材料构成的,顶层敷料和底层敷料之间设有灌注管道层,通过灌注管道层可以进一步调节底层敷料的降解速度并为创面输送必要的止血、抗感染和促进创面愈合的液剂,帮助创面愈合。
2、本发明所述的创新方案中,在顶层敷料上设有贴膜,贴膜与顶层敷料之间设有负压管道层,负压管道层不仅能够增加负压,使得创面快速生长和愈合,更能进一步抽取底层敷料降解后形成的多余液体以及创面在生长过程中产生的废液和分泌物,保证创面能够快速生长、防止感染。
3、本发明所述的负压管道与灌注管道是配合使用的,两者有效结合,负压管道与外接的负压装置相连,灌注管道与外接的灌注装置相连,一个灌注液体到达底层敷料,继而和创面接触产生效果,一个用于抽取多余的液体,保证创面生长的有利条件,不用一直进行敷料更换,可以减少因此产生的疼痛、出血以及创面异物残留的问题。
4、底层敷料降解的速度与创面肉芽组织长入敷料孔隙的速度相匹配,始终保持敷料孔隙为600-800μm,底层敷料降解过程中,降解产物可具有止血、抗感染以及促进创面愈合等作用。
5、负压管道及灌注管道在敷料中呈放射状分布,治疗全程均可保证创面敷料引流的通畅。

下面将结合附图对本发明的技术方案进行清楚、完整地描述,显然,所描述的实施例是本发明一部分实施例,而不是全部的实施例。基于本发明中的实施例,本领域普通技术人员在没有做出创造性劳动前提下所获得的所有其他实施例,都属于本发明保护的范围。
在本发明的描述中,需要说明的是,除非另有明确的规定和限定,术语“安装”、“相连”、“连接”应做广义理解,例如,可以是固定连接,也可以是可拆卸连接,或一体地连接;可以是直接相连,也可以通过中间媒介间接相连,可以是两个元件内部的连通。对于本领域的普通技术人员而言,可以具体情况理解上述术语在本发明中的具体含义。
本发明所述的是一种用于创面负压治疗的可降解敷料,具体参见图1中的结构,其与现有技术的区别在于:所述的可降解敷料包括底层敷料和顶层敷料,底层敷料用于贴附在创面上,底层敷料与顶层敷料之间设有灌注管道层,灌注管道层可以通过外源性灌注装置将可以帮助创面治疗的液体灌注到底层敷料表面,一方面可以帮助创面更好的生长和愈合,另一方面可以通过灌注的液体来调节底层敷料的降解速度,达到降解可控的目的。
进一步的,顶层敷料上设有用于固定可降解敷料的贴膜,贴膜将整个底层敷料、顶层敷料相对封闭固定在创面的表面,形成一个有利于创面生长、愈合的小环境。顶层敷料与贴膜之间设有负压管道层,负压管道层的中心处设有一与负压管道层相垂直的负压进出通道,所述的负压进出通道穿过贴膜并与外部的负压装置相连,负压装置与负压管道层相连,一方面是为了抽取底层敷料降解所产生的液体以及创面产生的分泌物等,保持创面的相对湿度,有利于创面的生长和愈合,同时,通过负压装置使得创面表面产生一定的负压,促进肉芽的生长。
所述的亲水壳聚糖系甲壳素通过H2O2降解获得,壳聚糖的脱乙酰度为20%,利用冷冻干燥法制成内部互相贯通的蜂窝状结构的底层敷料,平均孔径600-800μm的海绵状结构,该材料安全无毒,结构柔软,孔隙率高,弹性好,生物相容性好,无刺激性,不致敏,无异物反应。
进一步,灌注管道层的中心处设有一与灌注管道层相垂直的灌注进入通道,所述的灌注进入通道设置在负压进出通道的内部,即灌注进入通道穿越负压进出通道并与外部的灌注装置相连,这样不仅对灌注进入通道起到更好的保护,同时两个通道互相套叠也减少了顶层敷料上的开口面积,有利于小创面的使用和治疗。
现有技术中敷料都为不可降解的物质组成,治疗过程完成后需要将敷料去除。本发明的敷料底层与创面接触的部分是可降解的,治疗结束后不需要将敷料去除。
现有技术的敷料弊端在于治疗过程中创面肉芽组织会长入敷料的孔隙中,造成三方面问题:(1)治疗时,肉芽组织堵塞敷料孔隙,造成创面引流效率下降;(2)更换敷料时,肉芽组织与敷料发生撕扯,造成不必要的创面出血和疼痛;(3)敷料更换后,可能有部分敷料组织残存在肉芽组织内部,成为后期感染的隐患。
本发明的可降解材料制作的敷料优势在于随着敷料降解,敷料孔径可逐渐扩大。(1)治疗时,肉芽组织长入敷料孔隙中,敷料孔隙也在不断扩大,肉芽组织生长速度与敷料孔隙增大速度相适应,始终保持两者相对间隙,确保创面引流的效率。(2)更换敷料前灌注加速降解的溶液使创面敷料发生迅速降解,降解产物在负压作用下抽离创面。更换敷料时,敷料完全降解,没有敷料和肉芽组织发生撕扯的可能,创面出血和疼痛的问题也得以解决。(3)敷料更换后,即使极少量敷料被肉芽组织包裹,残留在创面上,由于敷料可以降解,也不存在长期残留成为感染源的危险。此外,使用可降解材料制作底层敷料的优势还在于,敷料降解产物可具有抗感染,促进局部血管生成等多种作用,有利于创面生长。
具体来说本发明有如下优点:
1、可配合创面负压治疗或创面灌注-负压治疗完成创面治疗。
2、治疗全程均可保证创面敷料引流的通畅。
3、治疗过程中可通过降解产物对创面产生止血、抗感染以及促进创面愈合等作用。
4、敷料更换过程可减少疼痛、出血以及创面异物残留的问题。
在一个实施例中,所述的底层敷料由可降解的壳聚糖类材料制成,底层敷料为亲水结构,内部呈互相连通的三维网状结构,所述的三维网状的网孔大小为600-800μm。底层敷料在创面潮湿的环境下,发生缓慢的降解,海绵孔隙不断增大,降解的速度与肉芽组织的生长相匹配,始终保持海绵敷料有效孔隙为600-800μm。具体可参见图3-图5,在底层敷料降解的过程中,降解产物可具有止血、抗感染以及促进创面愈合等作用。由于海绵敷料孔隙呈三维网状结构,故肉芽组织沿海绵敷料孔隙生长过程中可能将部分敷料包裹。用于底层敷料快速降解的特定溶解物质对人体无害,且不会影响顶层敷料、负压管道、灌注管道及贴膜的理化性能。
所述的底层敷料具体制备工艺为:采用冷冻干燥法制备,最佳工艺参数为醋酸浓度1%,壳聚糖-醋酸溶液浓度为0.1%,壳聚糖凝胶浓度为0.5%,添加0.001w%戊二醛,0.1w%的甘油,在真空度为相应温度下饱和蒸气压的1/3,真空度为150kPa, -10℃酒精浴中冷冻成形。获得的壳聚糖海绵表面细腻平整,孔隙分布均匀,柔软性好,可进行大角度弯折,孔隙率90% 保水率700w%,水蒸气透过率为15kg/48h.m2,平均孔径600-800μm。
底层敷料在体内自行降解主要依靠人体内的溶菌酶完成,通过调整壳聚糖敷料的脱乙酰度可以改变降解速度。本案中壳聚糖敷料脱乙酰度为20%,可以保证敷料在体内自行降解速度不少于14天。
底层敷料也可以在外源性催化酶的作用下发生降解。常用的催化酶包括脂肪酶或α-淀粉酶或菠萝蛋白酶。通过调整酶的浓度和环境温度可以改变壳聚糖敷料降解的速度。因为考虑到过高浓度催化酶和过高温度对对人体的损害,本案最佳条件为脂肪酶溶液酶量1500U/g,温度40℃;α-淀粉酶酶量2000U/g,温度40℃;菠萝蛋白酶酶量1g/L,温度40℃。
顶层敷料由聚氨酯海绵类材料制成,顶层敷料为疏水结构,顶层敷料布设有孔隙,所述的孔隙大小为600-800μm。该材料安全无毒,结构柔软,孔隙率高,弹性好,生物相容性好,无刺激性,不致敏,无异物反应。创面负压治疗过程中,外源性负压装置通过负压管道将创面分泌物和坏死组织通过底层敷料和顶层敷料的海绵孔隙抽离创面,减轻创面细菌负荷,促进创面肉芽组织生长。创面敷料更换时,将特定催化剂和消化酶溶液沿灌注管道输注至创面表面,底层敷料短时间内发生彻底降解,降解产物沿负压管道抽离创面。随后即可将贴膜、灌注管道、负压管道、顶层敷料去除。
在另一个实施例中,所述创面负压治疗用可降解敷料由底层敷料、顶层敷料、贴膜、灌注管道和负压管道组成,这里底层敷料、灌注管道层、顶层敷料和负压管道层的厚度之比为1:0.8:1.2:0.5。所述底层敷料由可降解材料构成,内部呈相互连通的三维网状结构,不同条件下降解速度不同;所述顶层敷料由不可降解材料构成,内部呈相互连通的三维网状结构;所述贴膜所述的贴膜由具有透湿、透气、防水、隔菌的聚氨酯薄膜组成,可将顶层敷料与底层敷料封闭在创面中;所述灌注管道层预制在底层敷料与顶层敷料之间;所述负压管道层预制在顶层敷料与贴膜之间。创面治疗过程中,将底层敷料、灌注管道层、顶层敷料和负压管道层置于创面中,以贴膜将敷料封闭于创面上。创面治疗过程中,底层敷料发生缓慢降解,保证创面肉芽组织不会堵塞敷料孔隙,同时,外源性创面清洁剂或药物可经灌注管道输注至创面,外源性负压将创面分泌物、坏死组织、灌注液及底层敷料降解产物通过负压管道层抽离创面。敷料更换前,特定溶剂经灌注管道输入,底层敷料发生迅速降解,敷料孔隙迅速扩大,有利于敷料的更换。
灌注管道层包括呈放射状均布的灌注副管道,所述的灌注副管道的一端与灌注进入通道相连,灌注副管道和灌注进入通道均采用硅胶制成;所述的灌注副管道与底层敷料之间呈0-15度的夹角,优选一定斜度的角度,比如5-8度,这样更加有利于灌注的快速、有效,同时,在灌注副管道上需要布设有若干孔洞,以保证灌注的均匀性,有效性。灌注副管道的一端大,另一端小,该大端与灌注进入通道相连。
负压管道层包括呈放射状均布的负压副管道,所述的负压副管道的一端与负压进出通道相连,负压副管道和负压进出通道均采用硅胶制成。这里所述的每一根负压副管道的底部都设有一虹吸槽,帮助吸取液体,保证负压的有效性和持续性。负压管道一旦吸附力下降到45%以下,则说明负压管道层被淤堵了,这时就需要及时更换可降解敷料,以保证伤口的有效愈合。
在一个具体的实施例中,首先准备好本发明说需要的底层敷料、顶层敷料、灌注管道层和负压管道层的材料,将创面清洁后,底层敷料覆盖在创面上,将顶层敷料放置在底层敷料上,将灌注管道层放置在底层敷料和顶层敷料之间,使用贴膜将顶层敷料、底层敷料以及灌注管道层和负压管道层封闭至创面上,灌注管道层和负压管道层的灌注进入通道和负压进出通道均穿透贴膜分别与相应的灌注装置和负压装置相连接。
接着,创面灌注-负压治疗过程中,根据治疗计划安排,外源性负压装置与灌注装置协调运作,创面交替处于负压与灌注两种治疗状态下。外源性负压装置运作时,外源性负压装置通过负压管道呈将创面分泌物、坏死组织和外源性灌注液通过底层敷料和顶层敷料的海绵孔隙抽离创面,减轻创面细菌负荷,促进创面肉芽组织生长。外源性灌注装置运作时,外源性灌注液通过灌注管道层浸润创面,创面灌注液(如创面清洁剂、创面消毒剂和相关药物)通过灌注管道层进入创面表面,具有创面清洁、抗感染和促进创面愈合等作用。创面分泌物、创面坏死组织、底层敷料降解产物以及灌注液可通过负压管道层抽离创面表面。
底层敷料在潮湿环境下,发生缓慢的降解,海绵孔隙不断增大,降解的速度与肉芽组织的生长相匹配,始终保持海绵敷料的有效孔隙为600-800μm。由于海绵敷料孔隙呈三维网状结构,故肉芽组织沿海绵敷料孔隙生长过程,不可避免的将部分敷料包裹至肉芽组织内。底层敷料在催化剂或者消化酶的作用下自行降解速度加快。如需更换创面敷料,通过创面灌注管道层向创面灌注特定溶解物质(例如壳聚糖酶,淀粉酶、蛋白酶等,对人体安全无害,不影响创面愈合正常进程),短时间内底层敷料发生降解,降解产物通过负压管道层自创面抽离。随后,将贴膜自创面表面剥离,连同顶层敷料、灌注管道层和负压管道层进行更换。
以上内容是结合具体的优选实施方式对本发明所作的进一步详细说明,不能认定本发明具体实施只局限于上述这些说明。对于本发明所属技术领域的普通技术人员来说,在不脱离本发明构思的前提下,还可以做出若干简单推演或替换,都应当视为属于本发明的保护范围。
Claims (3)
1.一种用于创面负压治疗的可降解敷料,其特征在于:所述的可降解敷料包括底层敷料和顶层敷料,底层敷料用于贴附在创面上,底层敷料与顶层敷料之间设有灌注管道层,顶层敷料上设有用于固定可降解敷料的贴膜,顶层敷料与贴膜之间设有负压管道层;
负压管道层的中心处设有一与负压管道层相垂直的负压进出通道,所述的负压进出通道穿过贴膜并与外部的负压装置相连,灌注管道层的中心处设有一与灌注管道层相垂直的灌注进入通道,所述的灌注进入通道穿越负压进出通道并与外部的灌注装置相连;
所述的底层敷料由可降解的亲水的壳聚糖制成,所述的亲水的壳聚糖系甲壳素通过H202降解获得,获得的壳聚糖脱乙酰度为20%,利用冷冻干燥的方法制成内部互相贯通的蜂窝状底层敷料,所述的底层敷料平均孔径600-800μm的海绵状结构;
在冷冻干燥法的制备方法中,采用的醋酸浓度1%,壳聚糖-醋酸溶液浓度为0.1%,壳聚糖凝胶浓度为0.5%,添加0.001w%戊二醛,0.1w%的甘油,在真空度为相应温度下饱和蒸气压的1/3,真空度为150kPa,-10℃酒精浴中冷冻成形,获得可进行大角度弯折,孔隙率90% 保水率700w%,水蒸气透过率为15kg/48h.m2,平均孔径600-800μm的壳聚糖海绵;
底层辅料在治疗时,肉芽组织长入敷料孔隙中,敷料孔隙也不断扩大,肉芽组织生长速度与敷料孔隙增大速度相适应,始终保持两者相对间隙,确保创面引流的效率;
底层敷料在创面环境下自行缓慢降解,完全降解时间大于等于14天或者底层敷料在催化剂或者消化酶的作用下自行降解速度加快,采用的催化剂或者消化酶为一下的任意一种:脂肪酶溶液酶量1500U/g,温度40℃;α-淀粉酶酶量2000U/g,温度40℃;菠萝蛋白酶酶量1g/L,温度40℃;
所述创面负压治疗的可降解敷料由底层敷料、顶层敷料、贴膜、灌注管道和负压管道组成,这里底层敷料、灌注管道层、顶层敷料和负压管道层的厚度之比为1:0.8:1.2:0.5;
灌注管道层包括呈放射状均布的灌注副管道,所述的灌注副管道的一端与灌注进入通道相连,灌注副管道和灌注进入通道均采用硅胶制成;
负压管道层包括呈放射状均布的负压副管道,所述的负压副管道的一端与负压进出通道相连,负压副管道和负压进出通道均采用硅胶制成;
所述的灌注副管道与底层敷料之间呈0-15度的夹角,灌注副管道的一端大,该端与灌注进入通道相连,灌注副管道的另一端小;
所述的灌注进入通道可通入催化剂或者消化酶,使催化剂或者消化酶沿灌注副管道到达底层敷料并加速底层敷料的降解;
所述的负压进出通道通过外接的负压装置将降解的底层敷料、创面分泌物和坏死组织抽离创面。
2.根据权利要求1所述的一种用于创面负压治疗的可降解敷料,其特征在于:所述的顶层敷料由聚氨酯海绵制成,顶层敷料的内部为互相贯通呈蜂窝状的网状结构,平均孔隙600-800μm。
