て、より有効な治療法が開発されれば、さらに有用であろう。

D20抗体を用いて再発性または抵抗性B細胞リンパ腫を治療する利点を報告す
る。特に抗CD20抗体を用いる治療は、サイトカイン、放射線治療法、骨髄機
能廃絶療法、または化学療法と併用する時、有効な相乗作用を提供することがわ
かっている。驚くべきことに、かって骨髄移植または幹細胞移植を受けた患者は
、かって治療を受けたことのない患者に比較して、全体の応答速度が予想外に上
昇した。

の治療法の前または後に投与されてよい。患者が最初の治療法に応答して緩解ま
たは再発の可能性が低下するかどうかに関係なく、連続投与を行ってよい。

アメリカリンパ腫(the Revised European American Lymphoma)(REAL)分

REAL分類項目のいくつかを、臨床的に有用な「無痛性」または「侵攻性」リ
ンパ腫に分類した。無痛性リンパ腫には、細胞診断により「悪性度」に分離され
た濾胞細胞性リンパ腫、びまん性リンパ球性リンパ腫/慢性リンパ球性白血病(
CLL)、リンパ形質細胞様/ワルデンストレームマクログロブリン血症、周縁
リンパ腫、およびヘアリーセル白血病がある。侵攻性リンパ腫には、びまん性混
合および巨大細胞リンパ腫、バーキットリンパ腫/びまん性小非切れ込み細胞リ

めに開発された新しい世代のモノクローナル抗体の1つである(2,3)。
マI重鎖およびカッパ軽鎖定常領域を有する、遺伝子工学的に作成されるモノク
ローナル抗体である。このキメラ抗体は、451アミノ酸の2つの重鎖と213アミノ
酸の2つの軽鎖からなり、分子量が約145kDである。リツクシマブ(登録商標)
は、補体結合およびADCC介在においてそのマウスのものより有効であり、ヒ
ト補体の存在下でCDCを仲介する(4)。この抗体は、B細胞株Fl−18、
ラモス(Ramos)、およびラジ(Raji)の細胞増殖を阻害し、ジフテキラ毒素、リ

からB細胞の枯渇を開始するのに充分であり;または(c)他の組織からB細胞
の枯渇を開始するのに充分であることが証明された。

濾胞中心、濾胞悪性度I、II、III(8))を有する166人の患者で行った。末梢
血中の腫瘍塊>10cmまたは>5000リンパ球/μlを有する患者は、試験から除外
した。年齢の平均値は58才(男性105名、女性61名)で、先行治療の平均数は3
であった。評価した149名のうちの56%で骨髄の関与が存在した。45%は≧2節
外部位を有し、41%は巨大病変(≧5cm)を有した。

、30歳代初期の成人でも検出されている。今日WMは、まれな不治の無痛性の悪
性腫瘍であり、従来は、血清の粘度を低下させるプラズマフェレシスにより治療
されている。しばしば、アルキル化剤のような化学療法薬およびコルチコステロ
イドが処方される。WMの最も推奨される薬剤は、ロイスタチン(Leustatin)
(2CdA)である。

b 11.6g/dl(範囲、7.7〜14.7)、血小板75×109/L(範囲、16〜160)、平均
β2免疫グロブリンは4.5mg/l(範囲、3.1〜9.2)であった。先行治療の平均回数
は2.5(範囲1〜9)であった。60%の患者は、治療に抵抗性であった。最初の
投与(375mg/m3)で2名の患者は、重傷の高血圧を発症し、他の1人はさらに治
療を受けた。以後の増加した投与量では毒性は弱かったが、1500mg/m3の用量レ
ベルではいずれの患者も完全には評価していない。8名の患者は治療を終了した
(500mg/m3で4名、650mg/m3で3名、825mg/m3で1名)。560mg/m3で治療した1
人の患者は、完全に緩解した。1人の患者は、治療でリンパ球増加を示し、他の
すべての患者は、末梢血リンパ球増加が低下したが、リンパ節に対する作用は小
さかった。用量を上げる実験が進行中である。

回(375mg/m2)投与した。ORRは45%(17/38患者)であり、11%がCRを、

れはG−CSFと375mg/m2リツクシマブ(登録商標)×4の併用の有効性を試験
するであろう。
スキューによる高用量療法が使用されているが、再発のリスクが高い(50〜80%
)ため、成功率が限定されている。移植後の持続性のある緩解を改善するために
、IL−2による高用量および低用量を含む免疫療法が、多くの治療センターで
試験されている。このような試験は、IL−2療法が早期の移植後抗腫瘍活性を
示すことを示唆している。

性CD8+ T細胞が低下していることが証明されている(45〜49)。インビト
ロ測定法は、分裂促進物質と可溶性抗原に応答して、T細胞細胞溶解性および増
殖性応答の大きな抑制ならびにIL−2産生の低下を証明している。しかし可溶
性IL−2は、これらの免疫応答を回復することができ、自己移植後の患者の免

T後は末梢血NK活性も対照値より低いままであり、NK活性もまた、外因性I

くの患者へのIL−2投与は、腫瘍負荷量が最小であり、IL−2の非存在下で
免疫応答性が欠如している決定的に重要な時期に、免疫応答性を増強することを

D56の明白なCD16+CD3−NK細胞が5〜40倍増加した。さらにこのI
L−2の低用量法により、NK標的K562のインビトロ死滅が増強された。ソ
イファー(Soiffer)ら(55)は、低用量IL−2を投与された29人の同種異系
BMT患者の結果を更新し、これらの患者の生存率(70%)が組織検査対照(30
%、p=0.41)と比較して優れていることを見いだした。

を有する2人の患者(1人は肝臓で2人目はリンパ節)は、IL−2治療の7お
よび10ヶ月後に完全緩解を示した。

CD8+細胞;CD16+CD69+細胞;CD16+CD56+細胞;CD1
6+CD122+細胞;CD16+Dr+細胞;およびCD8+CD56+細胞
を含む)。腫瘍標的K562とダウジ(Daudi)に対する溶解活性の発現も増加
し、それぞれ平均5.9倍と6.5倍の増加を示した。再発は(起きた時は)、移植後
平均17.8ヶ月で発生し、従って緩解は、IL−2治療の無い移植受容者で歴史的
に見られたものより特徴的に長いことが報告された。
考慮すると、リツクシマブ(登録商標)の生物活性は、ADCCと補体介在溶解
活性を介するようであるため、移植後にIL−2治療法をリツクシマブ(登録商
標)と併用することが妥当と思われた。従って、併用療法の安全性と可能な薬効
を評価するためにFHCRCと共同して第I相試験を開始した。
CA形成の頻度を評価するために、別の第II相試験も行われている。この試験の
具体的な目的は、IL−2へのインビボ暴露によりADCCが増強されるかどう
か、かつADCC活性が臨床応答と相関するかどうかを評価することである。患
者の試験対象基準は、組織学的に確認されたステージII〜IV低悪性度濾胞性B細
胞またはマントル細胞リンパ腫である。この臨床試験の目的において、マントル
細胞リンパ腫は、免疫組織化学によるCD5+、CD23−(利用できるなら)
および/またはbcl−1+であると定義される。標準的治療法(すなわち、化
学療法、放射線治療法、ABMTおよび/または免疫治療法)による初期の治療
に応答しないかまたは再発した患者は、適合する。
(登録商標)+GM−CSF リツクシマブ(登録商標)とGM−CSFによる併用療法の有効性を試験する
ために、2つの別の第II相試験も開始している。1つの試験は、再発性低悪性度
B細胞リンパ腫を有する40人の患者を含み、375mg/m2のリツクシマブ(登録商標
)を週1回×4(d.1、8、15、22)およびリツクシマブ(登録商標)の
最初の投与の1時間前に開始して、240μgのGM−CSF(ロイキン(Leukine
)、イムネックス(Immunex))を皮下に週3回で8週間投与する。この試験は
、併用療法の臨床的有効性(全体的応答率(ORR)、全体的完全応答率、悪化
までの時間と疾患の無い生存)を評価するために、併用療法の安全性(有害事象
の定性、定量、持続と可逆性)を解析するために、そして関係のあるリンパ球亜
集団とサイトカインに対する併用療法の作用を測定するために、使用される。第

マインターフェロンもリツクシマブ(登録商標)との併用療法が有用かも知れな
い。ガンマインターフェロンは、多発性骨髄腫(MM)患者の形質細胞、患者B
細胞ならびに正常ドナーB細胞上でのCD20発現をアップレギュレーションす
ることが最近見いだされた(トレオン(Treon)ら、ルガノ(Lugano)、1999)
。実際トレオン(Treon)と共同研究者たちは、ガンマインターフェロンがこれ
らの細胞のリツクシマブ(登録商標)への結合を増強することを証明した。形質
細胞上でのCD20発現の誘導は、用量依存性に起き、1U/mlのインターフェロ
ンガンマという少ない量でアップレギュレーションが見られた。48時間で100U/m
lでプラトーになった。すなわちガンマインターフェロンもまた、リツクシマブ
(登録商標)と組合せて投与することが有用なようである。
中悪性度または高悪性度NHL患者で、代替投与スケジュールを評価した(34)
。リツクシマブ(登録商標)を375mg/m2を週1回で8回、または375mg/m2を1回
と次に500mg/m2を週1回で7回注入した。ORRは31%(CR 9%、PR 22
%)で、投与法の間に有意な差は観察されなかった。びまん性巨大細胞リンパ腫
を有する患者(N=30)は、ORRが37%であり、マントル細胞リンパ腫を有す
る患者(N=12)はORRが33%であった。

poration))と共同して、他の適応症でのリツクシマブ(登録商標)治療を探索
している。混合性のびまん性巨大細胞および免疫芽球性巨大細胞組織学的的NH
Lを有する高齢患者(>60才)で、ECOG、CALGBおよびSWOGにより
CHOP対CHOPとリツクシマブ(登録商標)の第II相試験が行われている(
N=630が計画されている)。この試験は、リツクシマブ(登録商標)による維
持対非維持への2次ランダム化を含む。
HOPの第III相試験も、ダナファーバー研究所(Dana Farber Institute)で行
われている。21日毎の6サイクルで、リツクシマブ(登録商標)は1日目に投与
され、CHOPは1〜3日目に投与される。この試験の発生項目は完了している
。SWOGにより行われた新に診断された濾胞性リンパ腫でCHOPの後にリツ
クシマブ(登録商標)を使用する第II相試験もまた、完了している。これらの2
つの試験の結果は現在解析されている。
LにおけるCHOPとリツクシマブ(登録商標)対CHOPの第II相試験が行わ
れている。120人の患者が計画されている。
クシマブ(登録商標)は有望な初期結果を示した。応答した7人の患者のうちの
6人(CR1人、PR5人)と1人の患者は、安定な疾患を有し、治療は充分許
容された(36)。
せて、低悪性度または濾胞性NHL患者のリツクシマブ(登録商標)の安全性プ
ロフィールを提供する。大半の有害事象は注入関連であり、最初の注入後には頻
度が低下して起きた。最も一般的な注入関連事象は、発熱(49%)、寒気(32%
)、吐き気(18%)、疲労(16%)、頭痛(14%)、血管浮腫(13%)、かゆみ
(10%)、および時々低血圧(10%)と気管支けいれん(8%)であった。治療
期間中(最後の投与後30日まで)、10%の患者はグレード3または4の有害事象
を示し、これは主に注入関連または血液学関連であった。血小板減少症(<50,0
00血小板/mm3)が1.3%の患者に、好中球減少症(<1000/mm3)が1.9%に、そ
して貧血(<8gm/dl)が1.0%に起きた。リツクシマブ(登録商標)は70%〜80
%の患者でB細胞枯渇を誘発したが、異常に低い血清免疫グロブリンが少数の患
者で観察され、感染頻度は上昇していないようであった。
、1%ではグレード3または4であった。13%の患者で血管浮腫が報告され、1
人の患者では重症と思われた。気管支けいれんは8%の患者で発生し、2%は気
管支拡張薬で治療された。閉鎖性気管支梢炎の1件の報告も注目された。2回目
および以後の注入までに、多くの患者はさらなる注入関連の毒性は示さなかった
。再治療により有害事象を報告する患者の割合は、第1回目のコース後に報告さ
れたものに類似していた(14)。
うちの1人は、心室性頻脈と上室性頻拍のために治療を中止した。他の3人の患
者は、三連脈(N=1)と不整脈(N=2)を示したが、治療の中止は必要では
なかった。心筋梗塞の病歴を有する1人の患者では、注入の間に狭心症が報告さ
れ、注入の4日後に心筋梗塞を起こした。
を有する患者より巨大病変を有する患者でより高かった。めまい、好中球減少症
、血小板減少症、筋肉痛、貧血および胸痛は、>10cmの病変を有する患者でより
高かった。グレード3または4の好中球減少症、貧血、低血圧、および呼吸困難
の発生率も、<10cmの病変を有する患者に比較して巨大病変を有する患者で高か
った(19)。
ツクシマブ(登録商標)がFDAにより認可されて以来、推定17,000人の患者が
治療されている。1998年5月に、リツクシマブ(登録商標)の使用に関連する重
症の注入関連有害事象が致死的結果を引き起こした8つの市販後報告がまとめら
れた。8人の死亡のうちの7人では、最初のリツクシマブ(登録商標)注入中に
重症の症状が発生した。8症例のうちの2例については、死因は報告されないか
または不明である。重症の呼吸事象(低酸素症、肺浸潤、または成人型呼吸窮迫
症候群)が8人の報告された死亡のうちの6人の死亡を引き起こした。1人の患
者は、治療前のリンパ球数が600,000/mm3であり、二人目はクレアチニンが8,
3人目は呼吸速度が40、4人目は汎血球減少症を有した。高腫瘍負荷または循環
悪性腫瘍細胞の多い患者はよりリスクが高く、これらの患者は、各注入中注意深
く追跡する必要がある。
床試験で以前観察されたものであった。1つの目立つ例外は、急速な腫瘍溶解を
伴う注入関連症状であり、これは循環腫瘍細胞の多い6人の患者で報告された(
37、38)。この症状は、発熱、硬直、低酸素血症を伴う気管支けいれん、末梢血
リンパ球の急速な低下、腫瘍破壊の実験室的証拠、および一過性の重症血小板減
少症が特徴である。これらの患者は、B前リンパ球性白血病(N=2)、慢性リ
ンパ性白血病(N=2)、マントル細胞リンパ腫(N=1)、または形質転換N
HL(N=1)を診断された。これらすべての患者は、循環リンパ球が高く、巨
大腺症と臓器巨大症があった。これらの6人の患者のうち5人は入院が必要であ
ったが、症状は解消し、以後のリツクシマブ(登録商標)治療は充分許容された
。最後の患者は、さらに治療を拒否し、2週間後に疾患の悪化により死亡した。
報告では、リンパ球数が>10×109Lの患者で最初のリツクシマブ(登録商標)
注入後に腫瘍溶解症状が観察された(39)。
放射線免疫治療法 評価中のNHLへの別の治療的アプローチは、放射能標識抗CD20抗体(I
DEC−Y2B8)のリツクシマブ(登録商標)との併用である。IDEC−Y
2B8(90Y−イブリツモマブチウキセタン(90Y-ibritumomab tiuxetan))
は、キレート剤MX−DTPA(これは抗体に共有結合している)を介して90Y
に結合したマウスIgG1 カッパ抗CD20抗体である。リツクシマブ(登録商
標)(250mg/m2)は、末梢Bリンパ球を枯渇させかつ放射能標識抗体の生体分布
を改善するためにIDEC−Y2B8の前に投与される。
中悪性度NHL(N=14)、またはマントル細胞リンパ腫(N=3)の患者が、
IDEC−Y2B8で治療された。平均年齢は60であり、71%は男性であり、96
%はコーカサス人種であった。再発性または抵抗性NHLの51人の患者のうち、
34人(67%)は、単回投与の0.2、0.3、または0.4mCi/kgのIDEC−Y2B8
に応答した。低悪性度または濾胞性NHLの患者ではORRは82%(28/34)で
あり、中悪性度リンパ腫の患者では43%(6/14)であった。マントル細胞疾患
の患者は応答しなかった。
とリツクシマブ(登録商標)(375mg/m2を週に1回で4回)を比較する第III相
ランダム化試験が行われている。リツクシマブ(登録商標)に対して抵抗性であ
る再発性NHLの患者で、別の第III相試験も行われている。
の抑制を達成し、過度の毒性無しで腫瘍関連症状の緩解を提供することである。
リツクシマブ(登録商標)による治療は、ほとんどの患者で有害事象が限定され
た短期の22日間の通院治療法である。臨床試験では、評価可能な再発または化学
療法抵抗性低悪性度または濾胞性NHL患者の50%は、完全応答または部分的応
答を達成した。これらの応答は、維持治療法無しで永続性があり、重要な試験で
応答者の平均TTPは13.2ヶ月で、平均DRは11.6ヶ月であった。

の作用機序を有し、応答率と応答の持続期間で代替治療法に充分匹敵する。進行
中の試験の完了により、他のCD20+Bリンパ球悪性腫瘍の治療における代替
リツクシマブ(登録商標)治療法とリツクシマブ(登録商標)の役割が証明され
るであろう。
Claims (31)
- 【請求項1】 再発性B細胞リンパ腫を有する患者に治療上有効量の抗CD
20抗体を投与することを含んでなる、再発性B細胞リンパ腫の治療方法。 - 【請求項2】 患者は以前抗CD20抗体で治療された、請求項1に記載の
方法。 - 【請求項3】 患者は以前骨髄移植または幹細胞移植を受けた、請求項1に
記載の方法。 - 【請求項4】 患者は以前放射線療法を受けた、請求項1に記載の方法。
- 【請求項5】 患者は以前B細胞リンパ腫の化学療法を受けた、請求項1に
記載の方法。 - 【請求項6】 化学療法は、CHOP、ICE、ミトザントロン、シタラビ
ン、DVP、ATRA、イダルビシン、ヘルツァー(hoelzer)化学療法、ララ
(LaLa)化学療法、ABVD、CEOP、2−CdA、FLAG&IDA(以後
のG−CSF治療有りまたは無し)、VAD、M&P、C−Weekly、AB
CM、MOPP、およびDHAPよりなる群から選択される、請求項5に記載の
方法。 - 【請求項7】 キメラ抗CD20抗体の投与後に、顕著な腫瘍の緩解または
退行を示さない被験体に、放射能標識抗CD20抗体を投与することを含んでな
る、B細胞リンパ腫を有する被験体の治療方法。 - 【請求項8】 放射能標識抗CD20抗体は、キメラ抗CD20抗体の投与
後に約1週間〜約2年間投与される。請求項7に記載の方法。 - 【請求項9】 放射能標識抗CD20抗体は、キメラ抗CD20抗体の投与
後に約1週間〜約9ヶ月投与される。請求項8に記載の方法。 - 【請求項10】 抗CD20抗体は、キメラ抗CD20抗体である、請求項

ロン、ガンマインターフェロン、IL−2、GM−CSFまたはG−CSFより

ターフェロン、IL−2、GM−CSFまたはG−CSFは、連続的に、任意の
