Abstract

Description
항 CD20 항원에 대한 항체의 B-세포 림프종을 위한 진단제 및/또는 치료제로서의 용도는 이미 보고되어 있다. CD20은 이 항원이 악성 B-세포, 즉, 증식이 줄지 않아 B-세포 림프종을 일으킬 수 있는 B세포의 표면에서 매우 고밀도로 발현되므로, B-세포 림프종에 대한 유용한 표지 또는 표적이다.

발명의 요약

특히, 키메라 항체 후에 방사성 동위원소로 표지된 항체를 사용하여 환자를 치료하는 방법이 행해지고, 그것에 의해서 상기 키메라 항-CD20 항체를 투여한 후 약 1주 내지 약 2년 동안 방사성 동위원소로 표지된 항-CD20 항체를 투여하여 수행한다. 더 상세하게는, 상기 키메라 항-CD20 항체를 투여한 후 약 1주 내지 약 9개월 동안 방사성 동위원소로 표지된 항-CD20 항체를 투여한다.
어떤 항-CD20 항체라도 본 발명의 방법에 사용될 수 있지만, 바람직한 키메라 항체는 C2B8(IDEC Pharmaceuticals, Rituximab)이다. 방사성 동위원소로 표지된 항체로는 이트륨-90(90Y)으로 표지된 쥐 항체인 Y2B8이 바람직하다. 그러나, 다른 방사성 동위원소로 표지된 항체, 특히 베타 또는 알파 동위원소로 표지된 것도 사용될 수 있다. 항-CD19 항체도 사용될 수 있다.

Rituximab
및 Y2B8
미국에서 비-호지킨 림프종(NHL) 환자는 거의 250,000명이다. NHL을 가진 환자들 대부분은 화학치료법, 방사선치료법, 또는 자가조직의 골수(ABMT) 또는 말초 혈액 간세포(PBSC) 지지체를 사용한 고투여량 치료법에 의해 치료되지 않는다.

Rituximab은 쥐의 경쇄 및 중쇄 가변부위 및 인간 감마 I 중쇄 및 카파 경쇄 가변부위로 유전적으로 조작된 모노클로날 항체이다. 키메라 항체는 451 아미노산의 두 중쇄 및 213 아미노산의 두 경쇄로 구성되고, 약 145kD의 분자량을 가진다. "In2B8" 접합체는 2기능성 킬레이터, 즉, 1-이소티오시아나토벤질-3-메틸-DTPA와 1-메틸-3-이소티오시아나토벤질-DTPA의 1:1 혼합물을 포함하여 이루어지는 MX-DTPA(디에틸렌트리아민펜타아세트산)를 통해 인듐[111](111In)에 부착되는, 인간 CD20 항원에 특이적인 쥐의 모노클로날 항체인 2B8을 포함하여 이루어진다. 인듐-[111]은 이것이 감마 방사선을 방출하고 이미지제로서 이전의 사용을 밝혀내기 때문에 진단용 방사성 핵종으로 선택된다.
킬레이터 및 킬레이터 접합체와 관련된 특허가 당해 기술 분야에 공지되어 있다. 예를 들면, 갠소우(Gansow)의 미국 특허 제 4,831,175호는 다치환된 디에틸렌트리아민펜타아세트산 킬레이트 및 이를 함유하는 단백질 접합체, 및 이의 제조방법에 관한 것이다. 또한, 갠소우의 미국 특허 제 5,099,069호, 제 5,246,692호, 제 5,286,850호 및 제 5,124,471호는 다치환된 DTPA 킬레이트에 관한 것이다. 이들 특허는 전체가 본 명세서에 통합되어 있다.
MX-DTPA에서 킬레이트화를 촉진시키기 위해 사용되는 특정 2기능성 킬레이터는 그것이 3가 금속에 대해 고도의 친화력을 가지고, 증가된 종양 대 비종양 비율, 감소된 뼈 섭취, 및 표적 부위, 즉, B-세포 림프종 종양 부위에서 방사성 핵종의 더 큰 생체내 보유력을 제공하기 때문에 선택되었다. 그러나, 다른 2기능성 킬레이터가 당해 기술 분야에 공지되어 있고, 종양 치료에서 유리할 수도 있다.
또한, 미국 특허 제 5,736,137호에는 B-세포 림프종 및 종양 세포의 표적화 및 파괴를 위한 방사성 동위원소로 표지된 치료적 항체들이 개시되어 있다. 특히,Y2B8 접합체는 동일한 2작용성 킬레이트를 통해 이트륨-[90](90Y)에 부착된, 동일한 항-인간 CD20 쥐 모노클로날 항체인 2B8을 포함하여 이루어진다. 이 방사성 핵종은 여러가지 이유로 치료용으로 선택된다. 예를 들어,131I와는 달리90Y의 64시간 반감기는 종양에 의해 항체가 축적되기에 충분히 길고, 이것은 100 내지 1000 세포 직경 범위를 가지는, 자연붕괴시 감마 방사선을 동반하지 않는 고에너지의 순수한 베타 방사체이다. 최소량의 침투 방사선은 외래환자에게90Y-표지된 항체 투여를 허락한다. 또한, 표지된 항체의 세포내 이입은 세포 사멸을 필요로 하지 않고, 이온화 방사선의 국부적 방출은 표적 항원이 결여된 인접한 종양 세포에 치명적이어야 한다.
90Y 방사성 핵종은 동일한 2기능성 킬레이터 분자 MX-DTPA를 사용하여 2B8 항체에 부착되므로, Y2B8 접합체는 예를 들면, 표적 부위(종양)에서 증가된 방사성 핵종의 보유력과 같은 상기 논의된 동일한 이점을 가진다. 그러나,111In과 달리 이것은 이와 관련된 감마 방사선의 결여로 인하여 이미지화 목적에는 사용될 수 없다. 따라서,111In과 같은 진단적 "이미지화" 방사성 핵종은 본 발명의 조합법에서 치료적 키메라 또는90Y-표지된 항체의 투여 전 및/또는 후에 종양의 위치 및 상대적인 크기를 결정하는데 사용될 수 있다. 또한, 인듐-표지된 항체는 수행될 선량 측정 평가를 가능하게 한다.
항체의 의도적인, 즉, 진단제 또는 치료제로서의 사용에 따라, 다른 방사성 동위원소 표지가 당해 기술 분야에 공지되어 있고, 유사한 목적을 위해 사용되어 왔다. 예를 들면, 임상 진단에서 사용되어 온 방사성 핵종에는111In 뿐만 아니라131In,125In,123In,99Tc,67Ga가 포함된다. 항체들은 또한 표적화된 면역치료법에서의 잠재적인 사용을 위해 다양한 방사성 핵종으로 표지되어 왔다(Peirersz et al.(1987) The use of monoclonal antibody conjugates for the diagnosis and treatment of cancer.Immunol. 미국 특허 제 5,736,137호에 보고된 바와 같이, 비표지 키메라 항-CD20 항체 뿐만 아니라, 방사성 동위원소로 표지된 Y2B8 접합체의 투여로 B-세포 림프아세포 종양을 가진 마우스에서 종양이 상당히 감소되었다. 또한, 인간 임상 실험은 거기서 림프종 환자에게 키메라 항-CD20 항체를 주입하면 상당한 B-세포 제거가 나타났다고 보고하였다. 사실, 키메라 2B8은 최근 Rituxan라는 이름으로 국가의 첫번째 FDA-승인된 항암 모노클로날 항체로 공지되었다. 따라서, 하나 이상의 키메라 항-CD20 항체가 B-세포 림프종 치료에서의 치료 효능을 나타내는 것으로 나타났다.

따라서, 조합 면역치료와 관련하여, 쥐 항체는 특히 진단제로서 유용성을 찾을 수 있다. 또한, 미국 특허 제 5,736,137호에 나타난 바와 같이, Rituxan투여에 이어 이트륨-표지된 항-CD20 항체의 치료적으로 유효한 투여는 (a) 키메라 항-CD20 항체로는 명백해지지 않는 어떤 잔여 말초 혈액 B-세포를 명백히 하거나; (b) 림프절로부터 B-세포 제거하거나; 또는 (c) 다른 조직으로부터 B-세포 제거를 시작하기에 충분하다.
따라서, 암 치료 항체에 대한 방사성 동위원소 표지의 접합은 그러한 항체의 잠재적인 치료 효능을 평가하고, 치료 과정을 관찰하기 위한 진단제를 만들고, 키메라 항체의 초기 종양-사멸 잠재력을 향상시키는데 사용될 수 있는 부가적인 치료제를 고안하는데 사용될 수 있는 유용한 임상적 도구를 제공한다. 비-호지킨 림프종 치료에서 항-CD20 항체의 입증된 효능 및 방사성에 대한 림프구의 공지된 감도가 주어진다면, 재발 또는 난치성 비-호지킨 림프종의 빈도를 감소시키는 조합치료법에서 그러한 키메라 및 방사성 동위원소로 표지된 치료 항체의 용도를 밝히는 것은 매우 유익하다. 또한, 다른 B-세포 림프종의 치료에서의 그러한 조합치료법의 용도가 밝혀진다면 이로울 것이다.
저급 또는 여포성 NHL
재발 또는 난치성 NHL의 단일-약제 연구
Rituximab의 FDA 승인은 주로 저급 또는 여포성 NHL에 걸린 환자에서 5개의 단일-약제 연구를 기초로 하였다. 10-500 mg/m2범위의 단일 Rituximab주입의 초기 I기 연구에 의하면, 최대 면역관용 투여에 도달하지 않았지만; 최고 투여량에서 주입 시간의 길이는 외래환자 치료에 알맞게 고려되지 않았다. 15명의 환자 중에 ORR은 13%(표1)(6)이었다.

I/II 기 투여량-범위 연구 중 I 기에서, 환자에게 주당 4회 주입을 통하여 125-375 mg/㎡ 을 투여하였다. 투여량 관련 독성은 나타나지 않았고, 375mg/㎡을 II 기 투여량으로 선택하였다. 이러한 투여량을 투여한 37명의 환자(46%) 중 17명에게서 종양 퇴행이 관찰되었고, 3명(8%)에게서 완전한 반응(CR) 및 14명(38%)에게서 부분적 반응 PR(7)이 관찰되었다.
이어서, Rituximab을 주당 4회 375 mg/㎡ 주입하는 단일부문 중추 연구를 166명의 재발 또는 난치성의 저급 또는 여포성 NHL(International Working Formulation[IWF] Types A-D and REAL classification, small lymphocytic lymphoma, Follicular center, follicular Grades I, II, III(8)) 환자에게 실시하였다. 종양 크기 >10cm 또는 말초 혈액 중 > 5000 림프구/㎕인 환자를 연구에서 제외하였다. 중간 연령은 58세였고(105명의 남성 및 61명의 여성), 이전 치료의 중간 수는 3이었다. 평가된 환자 149명 중 56%에게 골수 침습이 존재하였다. 45%는 ≥2 외부결절 부위를 가졌고, 41%는 거대 질환(≥5cm)을 가졌다.
완전한 반응(CR)은 목, 가슴, 배 및 골반 CT 스캔에서, 28일 간격으로 2회에 나타난 모든 림프절의 <1x1㎠로의 퇴행, 림프종의 모든 증상 및 징후의 해소 및 골수, 간 및 비장의 정상화를 필요로 한다. 부분적 반응(PR)은 적어도 28일 동안 진행성 질환의 증거가 전혀 없이 병변의 수직 측정 생성물의 합이 ≥50% 감소하는 것을 필요로 한다. CR 또는 PR을 얻지 못한 환자는 측정가능한 질환의 순감소(>50%)가 관찰되더라도 비-반응자로 간주하였다. 진행에 대한 시간은 첫번째 주입부터 진행까지를 측정하였다.
총 반응률(OPR)은 CR 6% 및 PR 42%로 48%였다. 반응자의 진행 중간 시간(TTP)은 13.2개월이었고, 반응 중간 지속 기간(DR)은 11.6개월이었다. 반응자 80명 중 22명(28%)은 20.9+ 내지 32.9+ 개월에 완화가 진행중이었다(9).
Rituximab을 투여하면, B-세포가 신속하고 지속적으로 제거되었다. 순환성 B-세포는 처음 세번 투여로 제거되었고, 6개월 내지 9개월 이상 후-치료하는 동안 환자 중 83%에게 제거가 지속되었다. 치료 12개월 후에 중간 B-세포 수준은 정상으로 회복되었다. 중간 NK 세포 수는 변화가 없었으나, 베이스라인의 NK 세포의 더 높은 절대수와 Rituximab에 대한 반응 사이에는 명확한 상관관계가 관찰되었다(10).
베이스라인 예후 인자 몇가지를 분석하여 이들의 반응에 대한 상관관계를 결정하였다. 두드러지게, 이전의 고투여량 치료(p<0.01)를 받지 않았던 환자는 43%인 것에 비하여, ABMT 또는 PBSC 이후의 재발 환자 23명은 ORR이 78% 였다. 다변수 분석에서, ORR은 소형 림프구성 림프종 환자와 비교하여 여포성 NHL 환자에게서 더 높았고(58% 대 12%, p<0.01), 화학 내성 재발 환자와 비교하여 화학 민감성 재발 환자에게서 더 높았다(53% 대 36%, p=0.06). 반응률에 미치는 효과는 연령>60세, 외부결절 질환, 이전의 안트라사이클린 치료 또는 골수 침습과 전혀 상관이 없었다.
중간 혈청 항체 농도와, 치료 및 추적검사 동안 여러 시점에서의 반응 간에 통계적으로 큰 상관관계가 있는 것으로 밝혀졌다(11).
혈청 항체 수준은 소형 림프구성 림프종 환자에 비해 여포성 NHL 환자에게서더 높았다. 평균 혈청 항체는 또한 종양 크기 측정 및 베이스라인에서의 순환성 B-세포 수와 역의 상관관계가 있었다. 낮은 혈청 항체 농도와 다수의 순환성 NHL 세포 및 더 높은 종양 크기와의 관계는, 항체 클리어란스의 주요 모드가 종양 세포에 대한 것임을 제시한다. 더 높은 혈청 항체 농도와 반응 및 작은 종양 크기 또는 순환 세포와의 관계는, 거대 질환 환자와 같은 일부 그룹의 환자에게서 반응을 유도하기 위하여 Rituximab의 더 높은 또는 더 많은 투여가 필요하다는 것을 제시한다.
그럼에도 불구하고, Rituximab를 사용하여, >5cm 종양을 가진 환자 중 43%, >7cm의 종양을 가진 환자 중 35%에서 반응이 나타났으며, 이로써 Rituximab를 사용하여 거대 질환을 갖는 환자를 치료할 수 있는 것으로 제시되었다. 항체 치료가 종양의 밀집특성으로 인해 거대 질환 치료에 도움이 되지 않는다는 오랜 관념을 고려할 때, 이는 놀랄만하다.
일본에서 실시된 연구(12)에서, 재발 B-세포 림프종 환자를 Rituximab250mg/㎡(N=4) 또는 375mg/㎡(N=8)을 주당 4회 사용하여 치료하였다. 11명의 평가가능 환자 중 8명은 여포성 NHL을 가졌고, 2명은 광범성 대형세포 NHL을 가졌으며, 1명은 외피세포 림프종을 가졌다. 64%의 ORR 에 대해, 11명 중 2명은 CR, 5명은 PR을 가졌다; 모든 반응자는 여포성 조직학을 가졌다.
Rituximab혈청 수준 및 반응자는 이전 연구를 통하여 명확하게 상관관계를 가지므로, Rituximab375mg/㎡를 주당 8회 투여하는 II기 연구를 저급 또는여포성 NHL 환자에게 실시하였다. ORR 은 평가가능 환자 중 60%였고, CR률 14%, PR률 46%였다. 반응자 및 DR의 TTP 중간값은 각각 13.4+개월 및 19.4+개월이었다(13). 연구를 통하여 비교는 어렵지만, TTP 및 DR은 더많은 투여량을 사용하여 개선 가능한 것으로 보인다.
항체 치료가 미소 전이 (micrometastatic) 질환에만 유용하다는 초기 가정과는 달리, Rituximab는 고도의 거대 질환에 상당히 유효하다. 별도의 연구에서, 재발 또는 난치성의, 거대 저급 NHL 환자 31명(직경>10cm의 단일 병변)에게 375mg/㎡ Rituximab을 주당 4회 주입으로 투여하였다. 28명의 평가가능 환자(43%) 중 12명은 CR(1,4%) 또는 PR(11, 39%)을 나타내었다(14).
월덴스트롬의 마크로글로불린혈증(Waldenstrom's Macroglobulinemia)
월덴스트롬의 마크로글로불린혈증(WM)은, B-림프구가 과량의 IgM 항체를 분비하는 악성종양이다. WM은 일반적으로 60세 이상의 사람에게 발생하나, 30대 초의 성인에서도 발견되었다. 오늘날 WM은 희귀한 불치의 악성종양으로 간주되며, 과거에는 혈청 점도를 감소시키기 위하여 혈장분리반출법으로 치료되었다. 알킬화제 및 코르티코스테로이드와 같은 화학치료 약물이 종종 처방되었다. WM에 가장 추천되는 약물은 류스타틴(Leustatin, 2CdA)였다.
환자들이 Rituximab(375mg/㎡ 주당 4회)로 치료된 경우인 (15)왈덴스트롬 마크로글로불린혈증 환자 7명에 대한 보고서에서 환자 중 4명(57%)에서의 반응이 주목되었다. 중간 진행이 없는 생존 기간은 8개월이었다(3-27+ 개월의 범위). 따라서 Rituximab은 특히 2CdA와 같은 화학치료제를 사용한, 조합치료 프로토콜에 유용하다.
만성 림프구성 백혈병(CLL)
CLL은 작은 림프구성 림프종(SLL)의 체액(백혈병성) 등가증이다. SLL 환자는, Rituximab표준 투여량으로 치료시 다른 저급 NHL서브타입 환자보다 혈청 수준이 낮고 반응률이 낮다. 이는 아마도 CLL 환자의 순환성 종양 세포 수준이 매우 높고, CLL에 관련된 악성 세포가 세포 표면에서 감소된 수준의 CD20 발현을 갖는 것으로 생각되기 때문으로 보인다.
그럼에도 불구하고, 본 발명자들은 CLL과 같은 혈액학적 악성종양이 Rituximab로 치료될 수 있다는 것을 발견하였다. 최근 임상 연구를 통해 CLL환자를 더 높은 투여량의 Rituximab로 치료하여 평가하였다(16). 모든 환자에는 375mg/㎥을 1차 투여하여 주입-재발 부작용을 최소화하였다. 이어서, 주당 투여(3)는 동일하였으나, 증가된 투여량 수준으로 실시하였다. 16명의 환자는 500 내지 1500mg/㎥의 투여량으로 치료하였다. 중간 연령은 66세(범위, 25-78)였다. 81%는 말기 III-IV 질환이었다. 중간 백혈구 수는 40x109/L(범위, 4-200), Hgb 11.6g/dl(범위, 7.7-14.7), 혈소판 75x109/L (범위, 16-160), 중간 β2면역글로불린은 4.5mg/L(범위, 3.1-9.2)였다. 이전 치료법의 중간 수는 2.5(범위 1-9)였다. 60%의 환자는 난치성이었다. 두명의 환자는 1차 투여(375mg/㎥)시 심각한 고혈압이 발생하였고, 다른 한명은 추가 치료를 받았다. 1500mg/㎥ 투여 수준의 환자를 완전히 평가하지 않았으나, 연이은 단계확대 투여시 독성은 약하였다. 8명의 환자는 완치되었다(400mg/㎥에서 4명, 650mg/㎥에서 3명, 825mg/㎥에서 1명). 560mg/㎥으로 치료한 환자 한명은 완전히 완화되었다. 한명의 환자는 치료시 진행성 림포시토시스를 가지며, 모든 다른 환자는 말초 혈액 림포시토시스가 감소하였으나, 림프절에는 효과가 적었다. 투여량 단계적 확대 연구는 진행중이다.
CLL 환자의 반응을 개선하기 위한 또다른 접근방식은 사이토카인을 사용하여 CD20 항원을 상향조절하는 것이다. 시험관 내 연구에서, CLL 환자로부터의 단핵 세포를 24시간동안 다양한 사이토카인을 사용하여 배양하였다. 유세포분석 결과는 IL-4, GM-CSF 및 TNF-알파를 사용하여 상당히 상향-조절되는 것으로 나타났다(17). 사실상, 최근 데이터에 따르면, CLL 세포에서 관찰되는 CD20의 상향 조절은 종양 세포로 한정될 것이라고 제시되었다(Venogopal et al. Poster-Panpacific Lymphoma meeting, June 1999. 만성 림프구성 백혈병(CLL)에서 CD20 항원 발현의 사이토카인-유도된 상향 조절은 종양 세포로 한정될 수 있다.). 예비 데이터 또한, 인터페론 알파도 500 내지 1000U/ml의 농도로 적용시 24시간만에 CLL 세포 상의 CD20을 상향조절할 수 있는 것으로 제시한다.
따라서, 특정 사이토카인을 CLL 환자에게 Rituximab투여 이전 또는 동시에 투여함으로써, 악성 B-세포의 표면상 CD20 발현은 상향조절될 수 있으며, 이로써 CD19와 같은 다른 세포 표면 표지뿐 아니라 더욱 효과적인 면역치료 표적인 CD20을 만들 수 있다. 생체 내 CD20 상향조절을 위한 사이토카인 최적 투여량을시험하기 위하여, 공동 연구를 시작하였다.
연구 프로토콜은 초기에 GM-CSF 250mcg/㎡ SQ QD X 3 를 사용하여 10명의 환자를 치료하고, IL-4mcg/kg SQ QD X 3로 10명, G-CSF 5mcg/kg SQ QD X 3으로 10명의 환자를 치료하는 것을 포함한다. 세포자멸사 연구용 피콘 하이파크(Ficon Hypaque) 원심분리를 통하여 단핵 세포를 분리하여, CD20의 상향조절로 Rituximab에 의한 종양 세포 사멸이 개선되는지를 확인한다.
CLL 항체 치료법을 CLL 치료에 유용한 것으로 알려진 다른 통상적인 화학 치료법과 결합할 수 있다. CLL에 가장 자주 사용되는 단일 용제는 클로람부실(류케란)시약으로, 일일 0.1mg/kg 또는 매 4주마다 0.4 내지 1.0mg/kg 로 투여한다. 클로람부실은 경구 프레드니손(30 내지 100mg/㎡/d)과 종종 결합되며, 자가 면역 세포감소증 치료에 유용하다. 시클로포스파미드를 클로람부실 대신 사용가능하며, 일반적인 투여량은 빈크리스틴 및 스테로이드(예를 들어, COP법)와 함께 매 3-4주에 1-2g/㎡를 투여한다.
다양한 약물 조합물이 CLL에 사용되었으며, COP(시클로포스파미드, 온코빈 및 프레드니손) 및 CHOP(이러한 세 약물 + 독소루비신)가 포함된다. 플루다라빈은 CLL 치료에 효과를 보였고, 매 3-4주마다 25-30mg/㎡/d로 치료한 환자의 그룹에서 50%의 ORR이 나타났다. 환자중 일부는 플루다라빈으로 치료되기 어려운 것으로 나타났다. 플루다라빈에 불치인 환자는 2-CDA에도 치료되기 어려운 경우가 많으므로, 이러한 환자는 또한 2-CdA 에 내성이 있을 수 있다(O'Brien et al. N. Engl. J. Med. 330:319-322(1994)).
따라서, 항-CD20 항체 치료는 화학치료 약물로 치료한 후 재발되거나 난치인 환자에게 특히 유용한 것이다. 이러한 환자에게 Rituximab치료법은 방사선치료법과 결합될 수도 있다. 15 cGy의 단편 크기가 작은 TBI를 총투여량 75 내지 150cGy로 하면 환자 중 약 3분의 1에 효과적인 것으로 나타났다.
II기 임상시험이 CLL 환자에게 CALGB로 현재 실시되고 있다. Rituximab및 플루다라빈은 동시에 투여되고, 그 결과 Rituximab강화 대 Rituximab에 의한 플루다라빈 유도가 일어난다.
Rituximab
와 골수제거 치료
골수제거 치료법으로 무통성 림프종에 대한 반응을 얻었다; 그러나, 고-투여량 치료법으로도 잔류 종양 세포가 잔류할 수 있으며, 재주입된 PBSC가 종양 세포를 함유할 수 있다. Rituximab은 간 세포 수동술 전 및 이식 후에 사용되어, 잔류 CD20+종양 세포를 감소시키고, 수집된 골수 또는 간 세포의 오염을 감소시킨다. 중간 결과에 따르면, CD20+세포는 수집된 세포에서 검출되지 않는 것으로 증명되었다. 24명의 환자 중 18명에서는 융합이 되었고, 치료는 잘 면역관용되었다. 잔류 종양 세포를 평가하기 위하여 PCR 시험이 진행중이다(18).
Rituximab
를 사용한 재발 저급 NHL의 재치료
Rituximab에 반응한 후 재발된 53명의 환자에게, 재치료를 평가하기 위해 실시한 실험이 보고되었다(19). 42%의 ORR에 대해, 56명의 평가가능 환자중 7명(13%)은 CR, 16명(29%)은 PR을 얻었다. 2차 반응을 나타낸 4명의 환자는 3차치료를 행하였다. 이중 3명이 반응하였다.
Rituximab로 두 과정 치료 후, 여포성의 작게 분할된 세포 NHL로 초기 분류된 한 환자의 종양은 더이상 CD20 항원을 발현하지 않았고, 광범성 대형세포 NHL로 형질전환시에 Rituximab에 반응하지 않았다(20).
따라서, Rituximab를 사용한 재치료는, 이전에 Rituximab로 치료한 후 재발된 환자의 치료에 효과적이나, 2차 치료후 CD20- 종양 세포의 발생빈도가 증가될 수 있다. 이러한 관찰결과로, 본 명세서 기재의 조합치료법의 유용성이 확인된다.
저급 NHL에 대한 Rituximab
및 CHOP 화학 치료법의 조합
사이클로포스파미드, 독소루비신, 빈크리스틴 및 프레드니손(CHOP) 화학치료법은 저급 또는 여포성 NHL의 효과적인 최우량 치료법이다. 초기 반응률은 높으나, 결국에는 재발이 일어나고, 수반되는 화학치료법은 더 짧은 기간동안 완화를 일으킨다. 이들의 작용 메카니즘이 교차-내성이 아니기 때문에 새로 진단받은 재발 저급 또는 여포성 NHL에서의 CHOP 및 Rituximab(21)의 조합을 평가하기 위하여, II기 임상실험을 시작하였는데, Rituximab은 독소루비신(5)을 포함하는 특정 세포독성 약물과 상승작용이 있기 때문이다.
38명의 환자 중 29명은 전에 항암 치료를 받은 적이 없었다. CHOP를 표준 투여량으로 매 32주마다 6 주기로, Rituximab(375 mg/㎡)의 6회 주입과 함께 투여하였다. Rituximab1회 및 2회 주입은 1차 CHOP 주기 전 1일째 및 6일째에 투여하였고, 1차 CHOP 주기는 8일째에 시작하였다. Rituximab3회 및 4회 주입은 3차 및 5차 CHOP 주기 전 2일째에 실시하였고, 5회 및 6회 주입은 6차 CHOP 주기 후 134일째 및 141일째에 각각 주입하였다.
이러한 조합 연구에서, 38명의 치료 환자의 100%가 반응하였다(CR 58%; PR 42%). 35명의 완전히 치료된 것으로 평가된 환자 중, 63%는 CR, 37%는 PR이었다(21). 36.7+ 개월의 중간 관찰 시간 후 도달되지 않은 중간 진행이 없는 생존으로 중간 DR은 35.3+ 개월이다. 36+ 개월 내지 53.4+ 개월 후에, 20명의 환자는 여전히 완화상태이다(22). 이 DR은 최우량 치료로서도 인상적이고, 이러한 실험 집단 중 24%는 화학치료 후 재발하였다.
CALGB로 실시하는 연구에서, 저급 NHL 환자 40명에 Rituximab을 주당 8회 투여, 시클로포스파미드는 8일째부터 매일 경구투여할 것이다. 20명의 환자에게는 Rituximab만을 주당 8회 투여할 것이다.
저급 NHL 환자에 ECOG로 실시하는 III기 연구에서, 시클로포스파미드 및 플루다라빈(Arm A)을 표준 CVP 치료법(Arm B)과 조합하여 비교한다. Arm A 또는 Arm B 무작위화에서, 환자를 연령, 종양 정도, 조직학, 및 B 증상으로 나누었다. 두 암(arm)의 반응자는 Rituximab유지 치료(2년간 매 6개월마다 주당 4회 375mg/㎡)(Arm C)에 대하여, 또는 관찰(Arm D)에 대하여 2차 무작위화를 실시할 것이다.

+ 인터페론 알파
인터페론은 면역계 조절에 관계하는 사이토카인이다(23). 인터페론이 항체 효과를 증가시킬 수 있는 메카니즘에는, 항원 발현의 가능성(24), 항체의 종양으로의 표적화 증가(25, 26) 및 면역독소의 세포독성 개선(27)이 포함된다.
조합실험에서, 인터페론 알파(Roferon-A), NHL에 단일-약제 임상 활성을 갖는 사이토카인, 및 Rituximab을 재발된 저급 또는 여포성 NHL 환자에게 투여하였다. 인터페론-알파(2.5 또는 5 MIU)를 피하에 12주동안 주당 3회 투여하였다. Rituximab를, 치료 5번째 주에 시작하여 주당 4 투여량(375mg/㎡)으로 IV 주입을 통해 투여하였다. ORR은 45%였다(17/38 환자); 11%는 CR, 34%는 PR이었다. 반응자의 중간 DR 및 TTP의 캐플란-마이어 추정(Kaplan-Meier estimates)은 각각 22.3+ 및 25.2+ 개월이었다(29). 이전의 인터페론-알파 및 안트라사이클린을 함유하는 화학치료법의 결합 연구는 진행 시간이 연장되었으나, 반응 또는 생존율은 지속적으로 증가되지 않았다(30-32). 이러한 초기 결과는, Rituximab및 인터페론-알파의 조합이 Rituximab단독에 비하여 진행 시간을 연장할 수 있음을 제시한다.
Rituximab
+ G-CSF
별도의 연구에서, 재발된 저급 NHL에서 Rituximab과 G-CSF를 평가하고 있다. 건강한 지원자 뿐만 아니라 시험관 내에서도, G-CSF가, 골수전구체세포에 대한 그것의 효과를 통해, ADCC에서 작용체세포로서 기능을 할 수 있는 FcRI-양성 호중구를 유도한다는 것이 밝혀졌다. 그 때문에, 조합치료의 독성과 효능을 평가하기 위하여 I/II기 연구를 시작하였다.
I기와 II기 모두에서, Rituximab를 투여하기 2일 전에 시작하여, 3일 동안 투여된 표준투여량의 G-CSF(5㎍/㎏/day)를 환자에게 투여하였다. I기는 Rituximab투여량을 단계적 확대(매주 125, 250, 또는 375㎎/㎡ X4)하는 것으로 구성하였다. 지금까지 평가된 9명의 환자에서 초기결과는 9명의 환자 중 8명의 환자에게서 사소한 독성을 가진 67%(44% CR, 22% PR)의 ORR이 야기되었다(33). 가장 흔한 부작용은 열(4/8환자), 비염(4/8), 오한(3/8) 및 두통(3/8)이었는데, 이는 Rituximab를 단독으로 투여한 경우에서 이전에 관찰된 부작용에 비교할만 하다. 본 연구의 II기 부분이 시작되었는데, 이는 G-CGF와 375㎎/㎡의 RituximabX4의 조합의 효능을 조사할 것이다.
Rituximab
+ IL-2
자가조직의 말초혈액간세포(PBSC) 또는 골수(BM)구조를 사용한 고투여량 치료가 NHL을 치료하는데 사용되었지만, 50-80%로 높은 재발의 위험때문에 성공이 제한되고 있다. 이식후 지속적인 완화를 향상시키기 위한 노력으로, IL-2의 고투여량 및 저투여량 치료를 포함하는 면역치료가 많은 치료센터에서 연구되어 왔다. 그러한 연구는 IL-2 치료가 초기에 이식 후 항종양활성을 나타낸다는 것을 제시하고 있다.
자가조직이식 후 처음에, 환자들은 잠재적으로 감소된 면역매개 종양근절을 일으키는 지연된 면역 재구성을 나타낸다(43, 44). 실제로, CD$+ T세포와 세포독성 CD8+ T세포 모두 억압된 것으로 나타났다(45-49). 시험관내 분석은미토겐(mitogen)과 용해성 항원에 반응하여 IL-2의 생산이 감소될 뿐 아니라 T 세포용해 및 증식반응이 크게 억제된다는 것을 나타내었다. 그러나 용해성 IL-2는 자가조직 이식 후 환자의 면역세포들이 외인성 IL-2에 반응할 수 있다는 것을 제시하는 이들 면역반응을 회복할 수 있다(47). 말초혈액 NK 활성도 대조 값보다 BMT 후에 더 낮게 유지되고, NK 활성도 외인성 IL-2를 첨가함으로써 증대된다(49). 이들 데이터는 간세포이식 직후 환자에게 IL-2를 투여하면 종양정도가 최소일 때 및 IL-2가 없어서 면역반응성이 결여된 때인 결정적인 기간에 면역반응성을 증진시킬 수 있다는 것을 제시한다.
예를 들면, 칼리쥬루 등(Caligiuru et al.)은 12주 동안 24시간 CIV에 의해 0.45 X 106U/M2/day로 투여된 IL-2(Hoffman-LaRoche)가 CD56 밝은 NK 세포의 절대수를 확대할 수 있다는 것을 보여주고 있다(50-52). 이 방법은 독성이 거의 없는 외래환자실에서 비이식환자들에게 투여되었다.
동물모델은 종양특이적 T 작용체세포가 투여되었을 때 저 투여량의 IL-2를 유도하는 비 LAK가 항종양활성을 극적으로 증진시킨다는 것을 보여준다(53). 또한, 소이퍼 등(Soiffer et al.)(54)은 재발된 백혈병 또는 림프종에 대한 치료를 받고 있는 13명의 자가조직 BMT 또는 T세포 제거된 동종 BMT 수혜자에게 저투여량의 IL-2를 투여하였다. 실험실에서, 순환성 CD56 밝은 CD16+ CD3- NK세포가 5 내지 40배 증가한 증진된 면역학적 반응성이 나타났다. 게다가 IL-2의 이 저투여량법은 NK 표적 K562의 시험관 내 사멸을 증대시켰다. 소이퍼 등(55)이 저투여량 IL-2를 받은29명의 동종 BMT 환자들의 결과를 갱신했을 때, 그들은 조직학적 대조구(30%, p=0.41)에 비해 이들 환자들에 대한 생존률(70%)이 우수하다는 것을 발견하였다.
로리아 등(Lauria et al.)(56)은 1년 동안 2주 간 2 X 106IU/㎡ qod 및 그 다음에 일주일에 2회 3 X 106IU/㎡ 투여량의 IL-2로 ABMT 후 평균 42일에 고급의 NHL로 11명의 환자를 치료하였다. 표현형분석은 치료 6개월 후 총림프구의 비율과 절대수 및 특히 CD16 및 CD56 NK 세포 모두의 비율과 절대수가 지속적으로 상당히(p=0.001) 증가했다는 것을 보여주었다. 치료개시 후 22개월(10-42개월) 간 중간 추적조사를 한 결과 환자들 중 누구도 더 이상 진행하지 않았다. 또한, ABMT 후 잔여질병을 가진 2명의 환자들(한사람은 간, 두번째 사람은 림프절)은 IL-2 치료를 한지 7 내지 10개월 후 완전한 반응성을 획득했다.
베이(Vey)등(57)은 저투여량 IL-2로 난치 또는 재발된 HD(11명의 환자) 및 NHL(14명의 환자)에 걸린 25명의 환자를 치료했다. 환자들 중 48%는 이식시 저항성 질병에 걸렸고 84%는 ABMT 후 CR을 획득했다. IL-2는 이식 후 평균 54일에 시작되었고, 5일의 제1주기와 그 후 매 다른 주마다 2일의 4주기로 구성된다. 환자들은 평균 160 X 106IU/㎡의 IL-2를 받았다. 5년간 추적조사를 한 후, 생존률 및 DFS는 72%(HD 73%와 NHL 70%) 및 45%(HD 36%와 NHL 48%)이다.
FHCRC(Fred Hutchinson Cancer Research)의 한 그룹은 최근 저투여량 IL-2 치료가 외래환자실에서 잘 면역관용되고, 저투여량 IL-2로 치료된 환자들에서 병의 완화가 IL-2 치료를 하지 않은 경우보다 더 긴 경향이 있다는 것을 발견했다. IL-2치료는 CD8+ CD69+ 세포; CD16+ CD8+ 세포; CD16+ CD69+ 세포; CD16+ CD56+ 세포; CD16+ CD122+ 세포; CD16+ Dr+ 세포; 및 CD8+ CD56+ 세포를 포함하는 면역세포들의 어떤 군집의 수에서의 증가와 관련되었다. 종양표적 K562와 다우디(Daudi)에 대한 세포용해활성의 발현도 각각 평균 5.9배 및 6.5배 증가하였다. 그들이 발생했을 때, 재발은 이식후 평균 17.8개월에서 일어났고, 따라서 완화효과는 IL-2 치료를 하지 않은 이식수혜자에서 조직학적으로 나타나는 것보다 특징적으로 더 길다고 보고되었다.
ABMT 이식수혜자에서 IL-2를 사용한 단일치료연구로부터 모아진 고무적인 데이터가 주어지면, Rituximab의 생물학적 활성이 ADCC와 보체매개 세포용해활성을 통해 매개되는 것으로 가정한다면, 이식후 IL-2 치료를 Rituximab과 조합하는 것이 적당해 보인다. 따라서, 조합치료법의 안전성과 잠재적 효능을 평가하기 위하여 FHCRC와 공동으로 I기 임상시험을 시작하였다.
저투여량 IL-2 와 Rituxan을 받은 환자에서 HACA 형성의 발병율과 효능을 평가하기 위하여 별도의 II기 연구도 수행되고 있다. 본 연구의 특정 목표는 ADCC가 IL-2에 대해 생체내 노출됨으로써 증진되는지 여부와 ADCC활성이 임상적 반응과 상호관련이 있는지 여부를 평가하는 것이다. 환자들에 대한 산입기준은 II-IV 저급, 여포성 B 세포 또는 외피세포 림프종으로 조직학적으로 확인된다. 본 임상연구의 목적을 위하여, 외피세포 림프종은 면역조직화학에 의하여 CD5+, CD23+(입수할 수 있다면) 및/또는 bcl-1+로 한정된다. 표준치료법, 즉 화학요법, 방사선요법,ABMT 및/또는 면역요법을 사용한 최초치료에 반응하지 않았거나 재발된 환자들이 적합하다.
재발된 저급 또는 여포성의 B 세포 림프종의 치료를 위한 Rituximab
+ GM-CSF
Rituximab와 GM-CSF의 조합치료의 효능을 시험하기 위하여 2개의 독립된 II기 임상시험도 시작되었다. 한 연구는 재발된 저급 B세포 림프종을 가진 환자들을 포함하고, 매주 375 ㎎/㎡ X4(d.1,8,15,22)에서 Rituximab및 Rituximab의 제1투여 1시간 전에 시작하여 8주 동안 매주 3회 250 mcg sc에서의 GM-CSF(Leukin, Immunex)를 투여하는 것을 포함한다. 본 연구는 조합치료법의 임상적 효능{총반응률(ORR), 총체적인 완전한 반응률, 진행 및 실패없는 생존에 대한 시간}을 평가하는데, 조합치료의 안전성(부작용의 질적, 양적, 지속시간 및 역전가능성)을 특성화하는데, 및 관련된 림프구의 소집합과 사이토카인에 대한 조합치료의 효과를 확인하는데 사용될 것이다. 제2연구는 사멸의 메카니즘을 평가하기 위하여 면역학적 파라미터(보체 C3와 C4, CH50, CD3 CD4, CD8, CD16, CD19 ALC CD56에 대한 유세포분석기 및 ADCC분석)도 관찰할 계획이다.
Rituximab
+ 감마-인터페론
감마-인터페론은 저급 또는 고급 림프종에 걸린 환자들을 치료하기 위하여 Rituximab과 조합하여 치료하는데 유용할 수 있다. 최근 감마-인터페론이 정상적인 공여자 B 세포에 대해서뿐만 아니라, 환자의 B세포인 다중골수종(MM) 환자 형질세포에서 CD20 발현을 상향조절한다는 것을 발견하였다. 사실, 트레온(Treon)과 동료들은 감마-인터페론이 이들 세포가 Rituximab에 결합하는 것을 증진시킨다는 것을 보여준다. 형질세포에 대한 CD20 발현의 유도는 인터페론 감마의 1U/㎖정도로 적게 보이는 상향조절로, 투여량 의존 방식으로 일어났다. 정체상태는 48시간에서 100 U/㎖에서 일어났다. 따라서 감마-인터페론은 Rituximab과 함께 투여될 때 유용할 수 있다.
중급 및 고급 NHL
단일약제연구
유럽 및 호주에서 행해진 연구에서, 54명의 재발된 또는 난치성 중급 또는 고급 NHL 환자에게서 선택적인 투약계획을 평가하였다(34). Rituximab는 8회 투여하는 동안 매주 375 ㎎/㎡ 또는 1회 375 ㎎/㎡로 주입한 후 7회 투여하는 동안 매주 500 ㎎/㎡으로 주입하였다. ORR은 31%였고; (CR 9%, PR 22%) 투여방법간 상당한 차이는 관찰되지 않았다. 광범성 대형세포 림프종에 걸린 환자(N=30)는 37%의 ORR을 나타내었고, 외피세포 림프종에 걸린 환자(N=12)는 33%의 ORR을 나타내었다.
Rituximab
와 CHOP 화학요법의 조합
다른 연구에서, 중급 또는 고급 NHL에 걸린 31명의 환자(여자 19명, 남자 12명, 중간연령 49세)는 CHOP의 6번의 21일 주기의 각각의 1일째에 Rituximab을 받았다. 30명의 평가가능한 환자 중, 96%의 ORR에 대해, 19 CR(63%)과 10 PR(33%)이 있었다. 이 방법은 면역관용이 잘 된 것으로 생각되었고, Rituximab또는 CHOP단독으로 사용한 경우보다 더 높은 반응율을 가져올 수 있다.
NCI 암치료 및 진단부는 다른 증상들에서의 Rituximab치료를 조사하기 위하여 IDEC 제약회사와 공동연구하고 있다. CHOP 대 CHOP과 Rituximab의 II기 임상시험은 혼합, 광범성 대형세포, 및 면역아세포 대형세포 조직학 NHL에 걸린 더 나이든 환자들(>60세)(N=630 계획)에게서 ECOG, CALGB, 및 SEOG로 수행하고 있다. 본 연구는 Rituximab로의 유지 대 비유지에 대한 2차 무작위화를 포함한다.
다나 파버 인스티튜트(Dana Farber Institute)에서 이미 미치료 외피세포 림프종에 걸린 40명의 환자에서의 Rituximab와 CHOP의 III기 임상시험을 하고 있다. Rituximab은 첫번째 날에 투여되고 CHOP는 6주기동안 매 21일의 1-3일에 주어졌다. 본 연구의 자연증가가 완료되었다. SWOG에 의해 행해진 새로 진단된 여포성림프종에서의 CHOP 후 Rituximab의 II기 임상시험도 완료되었다. 이들 두 임상시험의 결과는 분석중이다.
악성AIDS협회에 의해 수행된 HIV관련 CHOP와 Rituximab대 CHOP 단독의 II기 임상시험이 진행중이고; 예정된 환자는 120명이다.
골수제거치료 재발 후 Rituximab
Rituximab는 자가조직 PBSC 지지체로 고투여량 치료를 한 후 재발된 중급 NHL에 걸린 환자들에서 유망한 초기결과를 나타내었다. 7명의 환자 중 6명이 반응했고(1 CR과 5 PR) 1명의 환자는 안정한 질환을 나타내었으며; 치료는 면역관용이 잘 되었다(36).
안전성실험
저급 또는 여포성 NHL에 걸린 환자에서 Rituximab의 안전성윤곽을 제공하기 위하여 5개의 단일약제 미국연구에서 315명의 환자의 부작용과 임상적 실험데이터를 조합하였다. 부작용의 대부분은 주입과 관련된 것이었고 제1주입 후 빈도가 감소하여 발생했다. 가장 흔한 주입관련부작용은 열(49%), 오한(32%), 메스꺼움(18%), 피로(16%), 두통(14%), 혈관부종(13%), 가려움증(10%), 및 때로 저혈압(10%)과 기관지경련(8%)이었다. (최후 투여 후 30일 이상의) 치료기간 동안, 환자 중 10%가 3급 또는 4급 부작용을 경험하였는데, 주로 주입과 관련되거나 혈액학적인 것이었다. 혈소판감소증(< 50,000 혈소판/㎣)은 환자들 중 1.3%에서 발생하였고, 호중구감소증(< 1000/㎣)는 1.9%에서 발생했고, 빈혈증(< 8gm/dL)은 1.0%에서 발생하였다. Rituximab가 70 - 80%의 환자들에게서 B 세포제거를 유도했지만, 소수의 환자들에게서 비정상적으로 감소된 혈장 면역글로불린이 관찰되었고 감염의 경우가 증가되는 것으로 나타나지 않았다.
Rituximab주입의 중단이 필요한 저혈압은 환자들중 10%에서 발생했고, 1%에서는 3급 또는 4급이었다. 혈관부종은 환자들중 13%에서 보고되었고 1명의 환자에서 심각한 것으로 생각되었다. 기관지경련은 환자들 중 8%에서 발생했고; 2%는 기관지확장제로 치료되었다. 세기관지염 말소의 경우도 단 한 건 보고되었다. 대부분의 환자들은 2차 및 이후 주입에 의해 더 이상의 주입관련독성을 겪지 않았다. 재치료에 대한 부작용을 보고하는 환자의 백분율은 첫번째 코스 후에 보고된 것과유사하였다(14).
4명의 환자들은 Rituximab를 주입하는 동안 부정맥을 나타내었다. 4명 중 1명은 심실의 심계항진증과 심실상의 심계항진증 때문에 치료를 중단하였다. 다른 3명의 환자들은 삼단맥(N=1)와 불규칙적인 맥박(N=2)을 겪었지만 치료의 중단은 필요하지 않았다. 이전에 심근경색의 병력을 가진 한 환자에게서 주입하는 동안 협심증이 보고되었고, 주입 4일 후 심근경색이 발생하였다.
부작용 및 3급과 4급 부작용의 총발생률은 비거대 질병을 가진 환자들에서보다 거대 질병을 가진 환자들에게서 더 높았다. 현기증, 호중구감소증, 혈소판감소증, 근육통, 빈혈증, 및 가슴통증의 발병률은 > 10 ㎝ 병발을 가진 환자들에서 더 높았다. 3급 또는 4급 호중구감소증, 빈혈증, 저혈압, 및 호흡곤란의 발병률도 < 10 cm 손상을 가진 환자들에 비해서 거대 질병을 가진 환자들에서 더 높았다.
1997년 FDA가 Rituximab를 재발 또는 난치성 저급 또는 여포성 NHL의 치료용으로 승인한 이래, 약 17,000 환자들이 치료되어 왔다. 1998년 5월에, 치명적인 결과를 일으키는 Rituximab의 사용과 관련된 심각한 주입관련 부작용에 대한 8개의 매매 후 보고서의 기재내용이 요약되었다. 8명의 사망자 중 7명에서, Rituximab를 1차 주입하는 동안 심각한 증상이 발생했다. 8명 중 2명에 대해서는 죽음의 원인이 보고되지 않아 미상으로 남아있다. 8명의 보고된 사망자 중 6명은 저산소증, 폐의 침윤, 성인의 호흡장애증후군을 포함한 심각한 호흡기 부작용때문이었다. 한 환자는 600,000/㎣의 전처리 림프구수를 나타내었고; 다른 환자는 8의크레아티닌; 세번째 환자는 40의 호흡율; 4번째 환자는 범혈구감소증을 나타내었다. 높은 종양부담을 가지거나 많은 수의 순환성 악성세포를 가진 환자들은 더 높을 위험성을 가질 수 있고, 이들 환자는 각 주입 내내 면밀히 체크해야 한다.
최근에 기술된 부작용의 대부분은 Rituximab임상연구에서 이미 밝혀졌다. 한가지 주목할만한 예외는 급속 종양세포용해와 관련된 주입관련증후군인데, 이는 많은 수의 순환성 종양세포를 가진 6명의 환자에게서 보고되었다(37, 38). 이 증후군은 열, 오한, 저산소혈증과 관련된 기관지경련, 말초 림프구의 급속한 감소, 종양파괴의 실험적 흔적, 및 일시적이고 심각한 혈소판감소증으로 특징지워진다. 이들 환자는 B-프로림프구성 백혈병(N=2), 만성 림프구성 백혈병(N=2), 외피세포 림프종(N=1), 또는 형질전환된 NHL(N=1)의 진단을 받았고; 모두 높은 순환성 림프구, 거대 선증, 및 장기종대를 나타내었다. 이들 6명의 환자중 5명은 입원가료가 필요했지만, 증상은 해소되고 이후의 Rituximab치료는 잘 면역관용되었다; 마지막 환자는 더 이상의 치료를 거부하고 2주 후 병이 진행되어 사망했다.
CLL에 걸린 7명의 환자 및 외피세포 림프종에 걸린 1명의 환자에 대한 별도의 보고서에서, 림프구 수가 > 10 X 109L인 환자들에서 Rituximab1차주입 후 종양세포용해증후군이 관찰되었다(39).

이트륨표지된 항-CD20 항체를 사용한 방사선면역치료
평가되고 있는 NHL에 대한 다른 치료적 접근방식은 Rituximab과 조합된 방사선표지된 항-CD20 항체(IDEC-Y2B8)이다. IDEC-Y2B8(90Y-ibritumomab tiuxetan)은 항체에 공유결합된 킬레이터인 MX-DTPA를 통하여90Y에 결합된 쥐의 IgG1카파 항-CD-20이다. 말초 B림프구를 제거하고 방사선표지된 항체의 생체분포를 향상시키기 위하여 IDEC-Y2B8 전에 Rituximab(250 ㎎/㎡)을 투여한다.
최근 보고된 I/II기 연구에서(40-42), 저급 NHL(N=34), 중급 NHL(N=14), 또는 외피세포 림프종 (N=3)에 걸린 환자들을 IDEC-Y2B8으로 치료하였다. 중간연령은 60세였고, 71%가 남자이고 96%가 백인이었다. 재발 또는 난치성 NHL에 걸린 51명의 환자 둥, 34명(67%)이 0.2, 0.3, 0.4 mCi/kg의 IDEC-Y2B8의 단일투여량에 반응하였다. ORR은 저급 또는 여포성 NHL에 걸린 환자들에 대해 82%(28/34)였고, 중급 림프종에 걸린 환자들에 대해서 43%(6/14)였다. 외피세포질병을 가진 환자들은 반응하지 않았다.
저급 여포성 또는 형질전환된 NHL 환자를 치료하기 위하여 IDEC-Y2B8을 Rituximab와 비교하는 III기 무작위연구(매주 4회 375 ㎎/㎡)가 진행되고 있다. 다른 III기 임상시험도 Rituximab에 난치성인 재발된 NHL에 걸린 환자들에서 행해지고 있다.
요약
NHL에 대한 치료법이 없을 때, 치료의 목적은 의미있는 지속시간동안 질병을 제어하고 과도한 독성없이 종양관련증상의 완화를 제공하는 것이다. Rituximab를사용한 치료는 대부분의 환자에서 제한된 부작용을 가진 짧은 22일 외래환자치료이다. 임상연구에서, 평가할만한 재발된 또는 화학요법 난치성 저급 또는 여포성 NHL 환자의 50%가 완전한 또는 부분적인 반응을 이루었다. 이들 반응은 유지치료없이 지속될 수 있고; 중추연구에서 반응자들에 대한 평균 TTP는 13.2개월이고 평균 DR은 11.6개월이었다.
Rituximab는 재발된 저급 또는 여포성 B 세포 NHL에 걸린 환자들에 대한 안전하고 효과적인 치료방법으로 입증된다. 그것은 상당한 임상적 활성, 신규한 작용메카니즘을 나타내고 반응률과 반응지속시간에서 선택적인 치료법들과 필적한다. 진행중인 연구가 완료되면 다른 CD20+ B 림프구 악성종양의 치료에서 선택적인 Rituximab법과 Rituximab의 역할이 입증될 것이다.
Claims (31)
- B-세포 림프종이 재발한 환자에게 치료적 유효량의 항-CD20 항체를 투여하는 것을 포함하는 재발된 B-세포 림프종 치료법.
- 제 1항에 있어서, 상기 환자는 이전에 항-CD20 항체로 치료되었던 것을 특징으로 하는 치료법.
- 제 1항에 있어서, 상기 환자는 이전에 골수 또는 간세포 이식을 받았던 것을 특징으로 하는 치료법.
- 제 1항에 있어서, 상기 환자는 이전에 방사선 치료를 받았던 것을 특징으로 하는 치료법.
- 제 1항에 있어서, 상기 환자는 이전에 상기 B-세포 림프종에 대해 화학요법을 받았던 것을 특징으로 하는 치료법.
- 제 5항에 있어서, 상기 화학요법이 CHOP, ICE, 미토잔트론(Mitozantrone), 사이타라빈(Cytarabine), DVP, ATRA, 이다루비신(Idarubicin), 호울저(hoelzer) 화학요법체제, 라라(La La) 화학요법체제, ABVD, CEOP, 2-CdA, 이어서 G-CSF 처리를하거나 또는 하지 않는 FLAG & IDA, VAD, M & P, C-Weekly, ABCM, MOPP 및 DHAP로 구성된 그룹에서 선택되는 것을 특징으로 하는 치료법.
- 방사성 동위원소로 표지된 항-CD20 항체를 B-세포 림프종을 가진 환자에게 투여하는 것을 포함하는, 키메라 항-CD20 항체의 투여 후에 분명한 종양 완화 또는 퇴화를 나타내지 않는 B-세포 림프종 환자의 치료법.
- 제 7항에 있어서, 상기 방사성 동위원소로 표지된 항-CD20 항체가 상기 키메라 항-CD20 항체의 투여 후에 약 1주 내지 약 2년 동안 투여되는 것을 특징으로 하는 치료법.
- 제 8항에 있어서, 상기 방사성 동위원소로 표지된 항-CD20 항체가 상기 키메라 항-CD20 항체의 투여 후에 약 1주 내지 약 9개월 동안 투여되는 것을 특징으로 하는 치료법.
- 제 1항에 있어서, 상기 항-CD20 항체가 키메라 항-CD20 항체인 것을 특징으로 하는 치료법.
- 제 10항에 있어서, 상기 키메라 항-CD20 항체가 C2B8(Rituximab)인 것을 특징으로 하는 치료법.
- 하나 이상의 항-CD20 항체 및 하나 이상의 사이토카인을 포함하는 공동 치료 조합을 투여하는 것을 포함하고, 치료 효과가 단독 투여 치료의 부가적인 효과보다 더 좋은 것을 특징으로 하는, B-세포 림프종 치료법.
- 제 12항에 있어서, 상기 하나 이상의 사이토카인이 알파 인터페론, 감마 안터페론, IL-2, GM-CSF 및 G-CSF로 이루어진 그룹으로부터 선택되는 것을 특징으로 하는 치료법.
- 제 13항에 있어서, 상기 항-CD20 항체 및 상기 알파 인터페론, 감마 인터페론, IL-2, GM-CSF 또는 G-CSF가 연속적으로, 순서대로 또는 조합되어 투여되는 것을 특징으로 하는 치료법.

- 환자에게 치료적 유효량의 항-CD20 항체를 화학요법 전, 사이 또는 후에 투여하는 것을 포함하는 B-세포 림프종 치료법.

- 제 21항에 있어서, 상기 항-CD20 항체가 키메라 항-CD20 항체인 것을 특징으로 하는 치료법.
- 제 22항에 있어서, 상기 키메라 항-CD20 항체가 C2B8(Rituximab)인 것을 특징으로 하는 치료법.
- 환자에게 항-CD20 항체를 투여함으로써 골수제거 치료 전 또는 후에 골수 또는 간세포 내의 잔류 CD20+ 종양 세포를 감소시키는 방법.
- 제 24항에 있어서, 상기 항-CD20 항체가 키메라 항-CD20 항체인 것을 특징으로 하는 치료법.
- 제 25항에 있어서, 상기 키메라 항-CD20 항체가 C2B8(Rituximab)인 것을 특징으로 하는 치료법.
- 제 1항에 있어서, 상기 B-세포 림프종이 저급/여포성 비-호지킨 림프종(NHL), 소형 림프구성 (SL) NHL, 중급/여포성 NHL, 중급 광범성 NHL, 만성 림프구성 백혈병(CLL), 고급 면역아세포 NHL, 고급 림프아세포 NHL, 고급 소형 미분열 세포 NHL, 거대 질환 NHL, 외피세포 림프종, AIDS-관련 림프종 및 월덴스트롬 마크로글로불린혈증으로 구성된 그룹으로부터 선택되는 것을 특징으로 하는 치료법.
