Claims (1)
- 【特許請求の範囲】 1. 哺乳動物の体内に挿入し、これを通じて挿入されたカテーテルの末端の 回転位置を決定するために使用することが可能なトルク伝達シースであって、 可撓性の管状シース体であって、基端と、末端と、該管状シース体を通 じて延びる中空の管腔とを備え、管状シース体の前記基端から前記末端にトルク を伝達するだけの充分なトルク強度を有することによりシース体の末端はシース 体の基端の回転にほぼ伴って回転する管状シース体と、 前記管状シース体の管腔内に形成されるカテーテル係合面であって、カ テーテルが前記シースに挿通される際にカテーテルに対して係合して、i)カテ ーテルが前記導入シースと独立して回転することを防止し、ii)少なくともカ テーテルの末端がシースと共に回転するように機能するカテーテル係合面とを備 えるトルク伝達シース。 2. 前記管状シース体は強化部材が入れられた高分子材料にて形成され該高 分子材料のトルク強度が大きくなっている請求項1に記載のトルク伝達シース。 3. 前記強化部材は編み目に形成される請求項2に記載のトルク伝達シース 。 4. 前記編み目は細長部材の第1の群と第2の群とからなり、細長部材の前 記各群は互いにほぼ平行に並んで配置された多数の細長部材からなり、前記第1 の群は前記シース体の管腔の周囲に時計回りに巻回され、前記第2の群は前記シ ース体の管腔の周囲に反時計周りに螺旋状に巻回され、第1の群の細長部材は多 数の交差点において第2の群の細長部材と交差し、第1の群の細長部材のそれぞ れは前記交差点において第2の群の細長部材の上下に交互に通されることにより 前記シース体の管腔の周囲に管状の編み目を形成する請求項3に記載のトルク伝 達シース。 5. 解剖学的管腔構造内に挿入可能な固定式ガイドカテーテルであって、 長手方向に延びる1以上の管腔を有する細長のカテーテル本体と、 前記カテーテル本体の第1の位置に形成され、前記1以上の管腔に連通 する開口と、 前記カテーテル本体に設けられる圧力作用部材であって、前記解剖学的 管腔構造に対して係合可能であることによりカテーテル本体の前記第1の位置が 該解剖学的管腔構造内で動くことを防止する圧力作用部材とを備える固定式ガイ ドカテーテル。 6. 前記圧力作用部材はバルーンであり、該バルーンはカテーテルの前記第 1の位置が前記解剖学的管腔構造内で動くことを防止するために解剖学的管腔構 造に対して係合するように膨張させることが可能である請求項5に記載の固定式 ガイドカテーテル。 7. 前記バルーンは解剖学的管腔構造に対して係合するバルーンの表面に施 される摩擦増大処理を含む請求項6に記載の固定式ガイドカテーテル。 8. 前記バルーン上の前記摩擦増大処理は、テクスチャリング、接着剤、及 び織られた繊維からなる摩擦増大処理の群から選択される請求項7に記載の固定 式ガイドカテーテル。 9. 前記第1の管腔に設けられる1以上の係合面であって、第1の管腔に挿 通された第2のカテーテルに対して係合することにより第2のカテーテルが前記 バルーン固定式ガイドカテーテルと独立して回転することを防止するように機能 する係合面を更に備える請求項5に記載の固定式ガイドカテーテル。 7. 少なくとも、a)イメージングカテーテルと、b)前記バルーン固定式 カテーテルが置かれた解剖学的管腔構造の壁を穿通することが可能な組織穿通要 素を有する通路形成カテーテルと組み合わされ、前記固定式ガイドカテーテルは 前記イメージングカテーテル及び前記通路形成カテーテルと組み合わせて使用し て、 i) 前記ガイドカテーテルの前記開口が前記解剖学的管腔構造の通 路が形成されるべき位置の近傍に配置されるまで解剖学的管腔構造内にガイドカ テーテルを経管的に進める工程と、 ii) 前記イメージングカテーテルによりガイドカテーテルの前記開 口に対して同一直線上に並んだ解剖学的構造の像が得られるようにガイドカテー テルの管腔にイメージングカテーテルを挿入する工程と、 iii)前記イメージングカテーテルによって得られる像によりガイド カテーテルの前記開口が前記血管外通路が形成されるべき部位に対して同一直線 上に並んだことが示されるまでガイドカテーテルを動かす工程と、 iv) 前記圧力作用部材を解剖学的管腔構造10に係合させてガイド カテーテルの前記第1の位置をほぼ固定された位置に保持する工程と、 v) ガイドカテーテルの第1の管腔からイメージングカテーテルを 抜去する工程と、 vi) 前記通路形成カテーテルがガイドカテーテルの前記1以上の係 合面に対して係合するようにガイドカテーテルの管腔に通路形成カテーテルを挿 入する工程と、 vii)前記組織穿通要素を、ガイドカテーテルの前記開口を通じ、解 剖学的管腔構造の壁を穿通するように前記通路形成カテーテルから送出すること により前記通路を形成する工程とを行うことにより、解剖学的管腔構造の所定の 位置に通路を形成することが可能な請求項9に記載の固定式ガイドカテーテル。 8. 前記圧力作用部材は膨張可能なバルーンであり、前記iv)工程は解剖 学的管腔構造に対してバルーンが係合するようにバルーンを膨張させることを含 む請求項7に記載の固定式ガイドカテーテル。 9. 通路形成カテーテルであって、i)細長のカテーテル本体と、ii)該 細長のカテーテル本体の第1の位置から送出して解剖学的管腔構造の壁を通じ、 前記カテーテルが置かれる標的部位に進めることが可能な組織穿通要素と、ii i)標的部位の像を得るために使用することが可能なイメージング手段とを有す る種類の通路形成カテーテルにおいて、 前記カテーテルの第2の位置に配設されるマーカであって、マーカが前 記標的部位の方向に向けられるようにカテーテルの位置及び回転方向を調節した 場合に、前記組織穿通要素が管腔構造の壁を通じ前記標的部位にまで穿通するよ うに前記イメージング手段及び前記第1の位置に対して前記第2の位置は位置決 めされるマーカを含む改良。 10. 前記マーカは前記カテーテルに配設されるU字形部材を含む請求項9に 記載のカテーテル。 11. 前記マーカは前記カテーテルに配設される概ね長方形の部材を含む請求 項9に記載のカテーテル。 12. 前記マーカは前記カテーテルに配設される細長のワイアを含む請求項9 に記載のカテーテル。 13. カテーテル本体の前記末端に取り付けられるとともに該末端から末端側 に延びる円弧状部材であって、組織穿通要素の径路にほぼ直交する平面内に配置 される円弧状部材を前記マーカは含む請求項9に記載のカテーテル。 14. 前記カテーテルの末端に配設される三脚部材であって、前記カテーテル 及び互いに対して取り付けられる第1、第2、及び第3の脚部を有し、該脚部の 内の1以上は組織穿通要素の径路に対して同一直線上に位置する三脚部材を前記 マーカは含む請求項9に記載のカテーテル。 15. 前記イメージング手段は、前記カテーテル本体を通じて長手方向に延び る細長のイメージング管腔であって、前記マーカ及び前記解剖学的標的構造の像 を得るためにイメージング装置を挿入して位置決めすることが可能なイメージン グ管腔を有する請求項9に記載のカテーテル。 16. 前記イメージング管腔はカテーテルを通じて長手方向に延びてカテーテ ルの前記末端に設けられた送出孔として開口し、イメージング装置がカテーテル の末端から突出して該末端を越えて延びるように前記管腔を通じてイメージング 装置を進めることが可能である請求項15に記載のカテーテル。 17. 前記イメージング管腔はカテーテルを通じて長手方向に延び、イメージ ング窓がカテーテルの第2の位置に形成されることにより、イメージング管腔に イメージング装置を挿通し、該イメージング装置を利用して前記イメージング窓 を介して前記マーカ及び解剖学的標的構造の像を得ることが可能である請求項1 5に記載のカテーテル。 18. 前記カテーテルは、該カテーテルの末端に配設されるとともに長手方向 に延びる中空の通路を有する柔軟性先端部材を更に備え、前記マーカは、細長部 材であって前記カテーテル本体に取り付けられるとともに前記細長部材に形成さ れた中空の通路の少なくとも一部を通じて延びる細長部材を含む請求項9に記載 のカテーテル。 19. 前記中空通路は第1の径を有し、前記細長部材は前記第1の径よりも小 さい第2の径を有することにより前記中空通路内において細長部材を間隙が取り 囲む請求項18に記載のカテーテル。 20. 前記細長部材は前記先端部材の末端を越えて突出する請求項18に記載 のカテーテル。 21. 前記カテーテル内に形成され、複数の支柱部によって囲まれた切り欠き を前記マーカは含み、前記イメージング手段を前記切り欠き内に配置することが 可能であり、前記支柱部の1以上を前記マーカとして使用することが可能である 請求項9に記載のカテーテル。 22. 前記支柱部は前記カテーテル本体に取り付けられるとともに前記切り欠 きに亙って延びる細長のワイアを含む請求項21に記載のカテーテル。 23. 前記切り欠きは、前記カテーテル本体から切り取られる領域を含み、前 記切り欠きの基端側に位置する基端側カテーテル本体部分と、前記切り欠きの末 端側に位置する末端側カテーテル本体部分とが形成される請求項21に記載のカ テーテル。 24. 前記カテーテル本体を通じて長手方向にイメージングカテーテル管腔が 延び、前記イメージング手段は、前記イメージングカテーテル管腔を通じて前記 切り欠き内へと進めることが可能なイメージングカテーテルを含み、該イメージ ングカテーテルによって得られた像はマーカとして使用可能な前記1以上の支柱 部の像を含む請求項23に記載のカテーテル。 25. 前記マーカは前記イメージング手段によって検出可能な信号を放射する 発信要素である請求項9に記載の通路形成カテーテル。 26.前記発信要素は圧電結晶である請求項25に記載の通路形成カテーテル。 27.前記カテーテルの前記第1の位置から前記解剖学的標的構造までの距離が 既知であり、前記マーカは更に、それぞれが前記カテーテルの前記第1の位置か ら前記解剖学的標的構造までの距離に関連付けられた複数の距離相関マーカ位置 を含み、これにより前記イメージング手段を用いて前記距離に関連付けられたマ ーキングの内の選択された1つを前記解剖学的標的構造の像に対して同一直線上 に並ぶように位置決めすることによりカテーテルを最適な位置及び方向に配置し て前記組織穿通要素が前記解剖学的標的組織を越えて延びることなく解剖学的標 的組織へと所望の通路が形成される請求項9に記載のカテーテル。 28. 細長のカテーテル本体であって、該カテーテル本体から横方向に送出さ れることにより、解剖学的管腔構造の壁を穿通して解剖学的標的部位に向かって カテーテルが挿入される組織間通路を形成する組織穿通要素を有するカテーテル 本体を備える通路形成カテーテルの位置決定/方向付けを行うためのシステムで あって、 前記解剖学的標的部位から信号を放射させる発信要素と、 前記解剖学的部位からの前記信号を受信する受信要素とを備え、 前記発信及び受信要素の一方が前記組織穿通要素が前記カテーテルから 送出される際に辿る経路に対して固定位置に配置されることにより、これを用い て前記組織穿通要素が前記解剖学的標的部位へと所望の組織間通路を形成するよ うに前記カテーテルの位置及び方向を決定することが可能であるシステム。 29. 前記発信要素及び前記受信要素はいずれも前記通路形成カテーテルに配 設される請求項28に記載のシステム。 30. 前記発信要素及び前記受信要素の一方は前記解剖学的標的部位に配置さ れ、他方は前記通路形成カテーテルに配置される請求項28に記載のシステム。 31. 前記発信要素は、 音波エネルギーと、 超音波エネルギーと、 光エネルギーと、 レーザ光エネルギーと、 無線周波エネルギーと、 電磁信号とからなるエネルギー形態の群から選択されるエネルギーの一 形態を放射するエネルギー放射部を含み、前記受信要素は前記エネルギー形態を 受信するように構成されたセンサを含む請求項28に記載のシステム。 32. 前記受信及び発信要素は、該受信及び発信要素が互いに同一直線上に並 べられた場合に前記組織穿通要素が前記通路形成カテーテルから前記解剖学的標 的部位へと送出されるよう前記通路形成カテーテルが適当に配置される位置にそ れぞれ配置され、前記システムは更に、 受信要素によって受信される信号の強度を監視するための装置であって 、受信要素によって受信された信号がピーク値に達したことを確認することによ り発信要素と受信要素とが互いに同一直線上に並んだこと、ひいてはカテーテル が正確な位置及び方向に配置されたことが示される装置を備える請求項28に記 載のシステム。 33. 前記受信要素によって受信される信号の強度を監視するための前記装置 は、 信号調節及び濾過要素と、 整流器と、 漏出積分器と、 アナログデジタル変換器と、 受信要素によって受信された信号の強度を表示するように構成されたデ ィスプレイとを同順に含む請求項32に記載のシステム。 34. 細長の可撓性ワイアであって、該ワイアの周囲に形成された放射防止シ ールドを有し、ワイアの末端が前記シールドを越えて延出することによりワイア の末端のみからエネルギーが放射される細長の可撓性ワイアを前記発信要素は含 む請求項28に記載のシステム。 35. 第1の血管の壁を穿通して前記解剖学的標的部位に延びる通路を形成す るために前記通路形成カテーテルが前記第1の血管内に経管的に進められ、前記 発信及び受信要素の一方を前記解剖学的標的部位に挿入し、他方を前記通路形成 カテーテルに配設する術式における使用に適合されたシステム。 36. 解剖学的管腔構造の壁を穿通して延びる組織間通路を形成するためのシ ステムであって、 a)細長の可撓性カテーテル本体と、末端と、前記カテーテル本体を通 じて長手方向に延びる1以上の管腔と、カテーテル本体の前記末端において前記 管腔が開口する末端管腔とを有する屈折カテーテルであって、カテーテル本体の 末端付近の部分を、 i) 直線形状と、 ii)屈曲形状との間で交互に動かすことが可能である屈折カ テーテルと、 b)前記屈折カテーテルの前記1以上の管腔の少なくとも一部に挿通さ れ、前記屈折カテーテルが前記解剖学的管腔構造内に挿入される際に少なくとも 解剖学的管腔構造の像を得ることが可能なイメージング装置と、 c)前記カテーテルの前記1以上の管腔を通じて進め、前記末端開口か ら送出することが可能であり、カテーテルの末端部が前記解剖学的管腔構造内で 屈曲形状をとる場合に前記末端開口から送出されて解剖学的管腔構造の壁を穿通 する組織穿通要素とを備えるシステム。 37. 前記カテーテルは更に、 第1の位置において前記カテーテルに配設されるマーカであって、前記 カテーテルの既知の位置に配置され、前記カテーテルの前記1以上の管腔内に前 記イメージング装置が挿入される際に該イメージング装置によって像視可能であ ることにより、前記解剖学的管腔構造内における前記カテーテルの選択された回 転方向決定を容易とし、カテーテルの前記末端部が屈曲形状に動かされる場合に カテーテルの前記末端開口を解剖学的管腔構造の所望の部位に向けるために使用 することが可能であるマーカを備える請求項36に記載のシステム。 38. 前記システムは更に、 d)前記カテーテルの前記1以上の管腔に挿通して前記末端から送出す ることが可能な通路改変機構であって、始めに前記組織穿通要素によって形成さ れた通路を改変するために使用することが可能な通路改変機構を更に備える請求 項36に記載のシステム。 39.請求項38に記載のシステムであって、以下の通路改変機構の群から選択 されるシステム、 前記通路を閉鎖するための機構と、 前記通路をステントするための機構と、 前記通路を拡張するための機構と、 前記通路を焼灼するための機構と、 前記通路内にチャンネルコネクタを留置するための機構と、
