Description
2013年3月8日に出願された米国特許出願第13/791185号、2013年11月8日に出願された米国仮特許出願第61/901753号、2008年1月3日に出願された米国特許出願第11/662128号、2013年12月27日に出願された米国特許出願第14/141913号、及び、2007年4月20に出願されて2013年5月14日に米国特許出願第8,439,963号として発行されたPCT/GB2007/001430号の国際段階として、2009年2月25日に出願された米国特許出願第12/297498号が、引用することによりその全体が本明細書の一部をなす。
本明細書に記載される方法及びシステムは、いくつかの実施形態に従って心臓血管手術において特定の有用性を示す。いくつかの態様が以下の非限定的な例によって更に示され、そこでは、システムは、冠動脈の閉塞後の心臓組織の逆行性灌流を可能にするように動脈と静脈との接続処置(PICVA)を行う臨床医によって用いられる。
米国合衆国における肢喪失者は200万人近くである。
肢喪失者のうち、主な原因は以下である:
血管疾患(54%)(糖尿病及び末梢動脈疾患(PAD))、
外傷(45%)、及び、
癌(2%未満)。
毎年、合衆国において約185000件の切断が行われている。
肢切断に伴う入院費は2007年において総計65億ドルを超えた。
切断後の生存率は多様な要因に基づき様々である。血管疾患(PAD及び糖尿病を含む)に起因する切断を受けた者は、30日での死亡率が9%〜15%と報告されており、長期生存率は1年が60%、3年が42%及び5年が35%〜45%である。
血行障害(dysvascular disease)で肢を喪失した者の半数近くが5年以内に死亡する。これは、結腸直腸癌、乳癌及び前立腺癌を患った者が被る5年死亡率よりも高い。
糖尿病を患った者のうち下肢切断を受けた者は、最大55%まで、2年〜3年以内にもう一本の脚の切断が必要となる。
結果:
包括的な検討のために総計56の試験が選択された。特定されたランダム化比較試験(RCT)はなかった。228人の患者を含む7つの患者群が選択基準と合致した。1年全体の肢温存は71%(95% CI:64%〜77%)であり、1年の二次開存率は46%(95% CI:39%〜53%)であった。大切断術を回避した患者の大部分は首尾よく創傷治癒し、休息痛が現れず、深刻な合併症がなかった。
結論:
限定された証拠に基づき、静脈の動脈化は、「手術不能な」慢性重症虚血肢を患った患者に大切断術を行う前の実行可能な代案とみなすことができる。
Claims (15)
- 血管閉塞を処置する埋め込み型装置であって、
最小直径と最大直径とからなる円錐台形の形状を有し、第1の通路内に少なくとも部分的に滞留するように構成されている、第1の端部分と、
前記最大直径よりも小さい第2の端径を有する第2の端部分であって、実質的に円筒形であって、第2の通路内に少なくとも部分的に滞留するように構成されており、前記第1の通路及び前記第2の通路のうちの一方が前記血管閉塞を含む、第2の端部分と、
前記第1の端部分と前記第2の端部分との間の中間部分であって、前記第1の端部分の最小直径と前記第2の端部分の第2の端径との間でテーパーが付いており、前記第1の通路と前記第2の通路との間の通路内に少なくとも部分的に滞留するように構成されている、中間部分と、
少なくとも前記中間部分に結合されたグラフト材であって、該装置の側壁を通る灌流を阻止するように構成されている、グラフト材と、
を備え、
前記第1の端部分の前記最大直径は4mm〜10mmであり、前記第1の端部分と前記中間部分とを合わせた長さは、30mm〜50mmであり、前記第2の端部分の第2の端径は2mm〜8mmであり、前記第2の端部分の長さは、15mm〜25mmであり、前記第1の端部分は、前記中間部分のテーパー角度と概ね同じテーパー角度を有する、

- 前記第1の端部分及び前記第2の端部分のうちの少なくとも一つは、ニチノールを含む、請求項1又は2に記載の装置。
- 前記中間部分はニチノールを含む、請求項1〜3のいずれか1項に記載の装置。
- 前記第1の端部分はカットストラットを含み、前記第2の端部分はカットストラットを含む、請求項1〜4のいずれか1項に記載の装置。
- 前記グラフト材の少なくとも一部は前記中間部分の外側にあるか、前記グラフト材の少なくとも一部は前記中間部分の内側にあるか、又は、前記グラフト材の少なくとも一部は前記中間部分内に埋め込まれる、請求項1〜5のいずれか1項に記載の装置。
- 前記第1の端部分は、前記第2の端部分より長い、請求項1〜6のいずれか1項に記載の装置。
- 構造構成部材と、一の構造構成部材と他の構造構成部材とを連結する連結ストラットとをさらに備える、請求項1〜7のいずれか1項に記載の装置。
- 前記構造構成部材の各々は、前記構造構成部材を巻回する個々のストラットを含む、請求項8に記載の装置。
- 前記個々のストラットの長さは、25mm〜100mmである、請求項9に記載の装置。
- 前記連結ストラットの厚さは、0.05mm〜0.5mmである、請求8〜10のいずれか1項に記載の装置。
- 前記第1の端部分は、0.02〜0.03°のテーパー角度を有する、請求項1〜11のいずれか1項に記載の装置。
- 前記第1の端部分及び前記第2の端部分の少なくとも1つは、リングを含む、請求項1〜12のいずれか1項に記載の装置。
- 前記リングの周方向幅は、0.25mm〜1mmである、請求項13に記載の装置。
- 前記リングの長手方向長さは、0.25mm〜1mmである、請求項13又は14に記載の装置。
