Abstract
Un cabezal (10) para aplicar energía luminosa a una profundidad seleccionada en un medio de dispersión (18) que tiene una capa externa en contacto físico y térmico con dicho cabezal incluyendo: un conjunto conductor térmico (12) (42) que tiene una superficie emisora de energía; y una pluralidad de elementos emisores de energía óptica (11) montados en dicho conjunto (12) (42) junto a dicha superficie emisora de energía, estando cada elemento en contacto térmico con dicho conjunto y orientado para dirigir energía luminosa a través de dicha superficie, caracterizado porque incluye: una capa conductora térmica transparente (15) (31) (40) sobre dicha superficie y en contacto térmico con ella, estando dicha capa en contacto con dicha capa externa del medio (18) cuando el cabezal le está aplicando energía luminosa; y un mecanismo de enfriamiento (14) para dicho conjunto, que permite que dicho conjunto disipe calor de dichos elementos y dicha capa externa del medio.
Description
Cabezal de distribución de energía luminosa.
Esta invención se refiere a cabezales de
distribución de energía luminosa, y más en particular a un cabezal
de diodo láser u otro cabezal de distribución de energía luminosa
para suministrar energía luminosa a una profundidad seleccionada en
un medio, en particular un medio de dispersión, cabezal que
proporciona mejor administración del calor para los diodos láser (u
otro emisor de energía luminosa) y el medio y/o que utiliza más
eficientemente energía luminosa del láser/emisor.
Los emisores de energía luminosa, incluyendo
láseres, y en particular láseres de diodo semiconductor, lámparas de
destellos, lámparas halógenas y otras lámparas de filamento, etc,
están encontrando una creciente aplicación en aplicaciones médicas,
industriales, de investigación, gubernamentales y otras. Para muchas
de estas aplicaciones, la energía luminosa se ha de distribuir a una
profundidad seleccionada en un medio de dispersión de luz. A causa
de la dispersión, solamente una fracción de la energía luminosa
suministrada a la superficie del medio llega a la zona deseada,
refractándose del medio gran parte de la energía restante y
disipándose en la atmósfera circundante. Con un medio altamente
dispersante tal como la piel, se puede perder hasta
50-80 por ciento de la energía incidente debido a
este efecto de retrodispersión, que requiere emisores/láseres de
energía luminosa más potentes o un número mayor de emisores/diodos
láseres (donde se utilizan diodos láser), o que requiere que la
energía luminosa sea administrada sobre un área mucho menor, para
lograr una fluencia deseada al blanco. La utilización de un cabezal
con un emisor/láser más potente o la utilización de un mayor número
de emisores/diodos láser y/o más potentes hace que el cabezal sea
más grande y más caro y aumenta los problemas de gestión del calor
que resultan del uso del cabezal. Concentrar el haz para lograr
mayor fluencia con menor tamaño del punto o agujero afecta
adversamente a la profundidad en el medio que puede alcanzar la
energía luminosa y puede aumentar considerablemente el tiempo
requerido para efectuar un procedimiento dado.
El documento
US-A-5 820 625 describe un cabezal
para aplicar energía luminosa a una profundidad seleccionada en un
medio de dispersión que tiene una capa externa en contacto físico y
térmico con dicho cabezal incluyendo: un conjunto conductor térmico
que tiene una superficie emisora de energía; una pluralidad de
elementos emisores de energía óptica montados en dicho conjunto
junto a dicha superficie emisora de energía, estando cada elemento
en contacto térmico con dicho conjunto y orientado para dirigir
energía luminosa a través de dicha superficie; y un mecanismo de
enfriamiento para dicho conjunto.
La Patente de Estados Unidos número 5.824.023 de
Rox Anderson describe una forma de tratar el problema de reflexión
de algunos dispositivos láser u otros emisores de energía luminosa.
Sin embargo, la técnica de esta patente también da lugar a pequeños
tamaños del punto y no es fácilmente adaptable para uso en algunas
aplicaciones, tal como en cabezales de diodos láser. Por lo tanto,
se necesita una técnica mejorada para permitir la utilización óptima
de la energía luminosa de dispositivos emisores de energía luminosa
en general, y de diodos láser o barras de diodos láser de un cabezal
de diodo láser en particular, reciclando o reutilizando la luz
dispersada de la superficie del medio irradiado y dirigiéndola de
nuevo hacia una zona deseada en el medio.
Un problema relacionado implica la administración
del calor al utilizar un cabezal de diodo láser, u otro cabezal
conteniendo emisores de energía luminosa, y en particular la
capacidad de utilizar un elemento de enfriamiento común para enfriar
los diodos láser/emisores de energía luminosa y la superficie del
medio irradiado. El enfriamiento superficial puede ser necesario en
varias aplicaciones, en particular aplicaciones médicas, puesto que
la energía láser que se suministra a una profundidad en el medio,
por ejemplo la piel del paciente, debe pasar a través de la
superficie del medio, por ejemplo la epidermis de la piel del
paciente, para llegar a la zona deseada. El calentamiento de la
superficie del medio puede producir daño en la superficie si no se
prevé un enfriamiento adecuado. Los sistemas de la técnica anterior
no han previsto enfriamiento de la superficie del medio o han
requerido elementos de enfriamiento separados para los diodos y el
medio.
Según lo anterior, esta invención proporciona, en
un primer aspecto, un cabezal para aplicar energía luminosa a una
profundidad seleccionada en un medio de dispersión que tiene una
capa externa en contacto físico y térmico con el cabezal. El cabezal
incluye un bloque o conjunto conductor térmico que tiene una
superficie emisora de energía; una pluralidad de diodos láser u
otros elementos emisores de energía montados en el bloque junto a la
superficie emisora de energía, estando cada uno de los elementos en
contacto térmico con el conjunto y estando orientado para dirigir
energía luminosa a través de la superficie emisora de energía. Se ha
previsto una capa conductora térmica fina transparente sobre la
superficie fotoemisora y en contacto térmico con ella, estando la
capa en contacto con la capa externa del medio cuando el cabezal le
está aplicando energía luminosa. Finalmente, se ha previsto para el
conjunto un mecanismo de enfriamiento que permite que el conjunto
disipe calor de los elementos y la capa externa del medio. En
algunas realizaciones, la capa conductora térmica es un
recubrimiento formado en la superficie fotoemisora del conjunto.
En las realizaciones preferidas, el cabezal
también incluye una capa reflectora formada en la capa conductora
térmica, capa reflectora que tiene un agujero formado debajo de cada
elemento a través del que se puede aplicar energía luminosa al
medio. La capa reflectora está preferiblemente entre la capa
conductora térmica y la superficie emisora de energía del
montaje/bloque, y tiene preferiblemente un área más grande que el
área del bloque. En particular, el área de la capa reflectora podría
ser al menos sustancialmente tan grande como el agujero de reflexión
de energía luminosa dispersada del medio. Para lograr un coeficiente
de amplificación deseado (f) como resultado de la retrorreflexión de
la capa reflectora, el agujero a través del que se aplica energía

El bloque para el cabezal de diodo láser puede
asumir varias formas. En particular, en algunas realizaciones de la
invención, el bloque tiene una depresión formada en él, siendo la
superficie emisora de energía la superficie de la depresión, y
estando montado cada uno de los elementos en algunas realizaciones
para emitir energía luminosa sustancialmente perpendicular a la
superficie de depresión al punto donde está montado el elemento. El
medio es empujado a la depresión y a contacto con su superficie. El
empuje del medio a la depresión se puede llevar a cabo presionando
meramente el cabezal contra un medio blando deformable, tal como
algunas zonas de la piel de una persona, o se puede prever
aspiración, por ejemplo una línea de vacío, para arrastrar la piel u
otro medio a la depresión. La depresión puede tener varias formas,
incluyendo sustancialmente semicilíndrica o sustancialmente
rectangular. Donde el cabezal está siendo utilizado para extracción
de vello, por ejemplo, en una persona, la depresión puede ser de un
tamaño suficiente para permitir que un solo folículo capilar entre
en la depresión en el plano de la depresión rectangular. La capa
reflectora también se puede formar y utilizar con cabezales que usan
el bloque enfriado para enfriar los diodos u otros emisores de
energía luminosa solamente y no para enfriar la superficie del

transparente más gruesa o para cabezales que usan elementos emisores
de energía luminosa distintos de barras de diodos láser, por ejemplo
lámparas de filamento o tubos de luz alimentados por un componente
emisor de luz adecuado. En tales cabezales, la capa reflectora
todavía tendría zonas del tipo antes indicado y tendría
preferiblemente un agujero emisor con una dimensión mínima D_{min}
determinada como se ha indicado antes. En estas realizaciones, la
capa transparente podría ser una guía de onda de forma seleccionada,
forma que podría ser una forma truncada que, dependiendo del tamaño
deseado del agujero, tendría su extremo más grande o más corto junto
al bloque. Lados o paredes seleccionados de la guía de onda pueden
tener una capa reflectora dependiente de ángulo para atenuar de
forma pronunciada la energía luminosa inclinada que entra en la guía
de onda.
En otro aspecto de la invención, el cabezal puede
incluir al menos un elemento emisor de energía montado para aplicar
energía luminosa al medio a través de un agujero, agujero que tiene
una dimensión mínima D_{min} definida como se ha indicado
anteriormente, y una capa reflectora montada para retrorreflejar
energía luminosa retrodispersada del medio. El agujero puede ser
circular, siendo D un diámetro del agujero, o sustancialmente
rectangular, siendo D la longitud de un lado corto del agujero.
Los anteriores y otros objetos, características y
ventajas de la invención serán evidentes por la siguiente
descripción más específica de las realizaciones preferidas de la
invención como se ilustra en los dibujos anexos.

Con referencia en primer lugar a la figura 1, se
representa un cabezal láser 10 que contiene una pluralidad de barras
de diodos láser 11, cada una de las cuales incluye una pluralidad de
emisores 11A y está montada en una ranura 13 formada en un bloque
12. El bloque 12 se puede formar de uno o varios materiales que
tienen buenas propiedades de conducción térmica, y se puede fabricar
de varias formas, todos las cuales caen dentro del alcance de esta
invención. En particular, el bloque 12 se puede formar de un solo
material que, además de tener buenas propiedades de conducción
térmica, también es un aislante eléctrico, revistiéndose o
chapándose las paredes laterales de las ranuras 13 con un material
conductor eléctrico y estando soldadas las barras diodo en las
ranuras, formándose un circuito eléctrico entre ranuras adyacentes
de manera que pueda fluir corriente a través de los diodos sin
cortocircuito a través del bloque 12. Alternativamente, la porción
del bloque 12 entre las ranuras 13 se puede fabricar de conjuntos
conductores eléctricos fijados de forma adecuada a un sustrato
conductor térmico y aislante eléctrico, proporcionando los conjuntos
conductores un recorrido eléctrico a través de los diodos y evitando
el sustrato aislante los cortocircuitos. También se pueden emplear
otras técnicas para obtener un recorrido eléctrico a través de los
diodos para permitir su energización selectiva, aunque no
proporcionen un recorrido de cortocircuito a través del bloque
12.
El bloque 12 sirve como un disipador térmico para
barras diodo 11 y se puede utilizar varias técnicas para eliminar
calor del bloque 12. Estas incluyen disponer uno o varios canales a
través del bloque 12 y hacer fluir un fluido, que es generalmente
agua, pero que puede ser algún otro líquido o gas, a través del
canal para eliminar calor del bloque 12. Alternativamente, se puede
unir al bloque 12 uno o varios componentes termoeléctricos 14, por
ejemplo elementos Peltier, y utilizar para eliminar calor de
ellos.
Un elemento transparente 15 al que se ha unido
una máscara de alta reflectividad 16, está montado en la parte
inferior del bloque 12, estando la máscara 16 preferiblemente entre
el bloque 12 y el elemento 15. Para una realización preferida donde
el cabezal 10 se utiliza para tratamiento dermatológico, siendo la
piel del paciente el medio de dispersión 18, el elemento
transparente se forma preferiblemente de zafiro o algún otro
material con buen índice de coincidencia con la piel, y es
preferiblemente un recubrimiento de zafiro, por ejemplo, de 1 a 2
micras de grosor, o una placa o pastilla de zafiro de, por ejemplo,
1 a 2 mm de grosor. Si el componente 15 es una placa o pastilla, la
máscara 16 puede ser un recubrimiento de un material altamente
reflector tal como Ag, Cu, Au o un recubrimiento dieléctrico de
capas múltiples que se forma usando una tecnología de recubrimiento
apropiada conocida en la técnica, tal como litografía, sobre la
placa/pastilla 15. Se han formado agujeros 20 (figura 2A) en el
recubrimiento 16 debajo de cada emisor de barras de diodos 11A,
siendo los agujeros 20 solamente suficientemente grandes para que la
luz de las barras diodo pueda pasar sin obstáculos a su través. Es
deseable mantener las hendiduras o los agujeros 20 en capa
reflectora o máscara 16 lo más pequeños que sea posible porque
maximiza la reflectividad de la máscara y por lo tanto, como se
explicará más adelante, optimiza la retrorreflexión de energía
dispersada de la piel u otros medios 18. Por razones que se
explicarán con más detalle más adelante, la capa reflectora 16
deberá tener una huella más grande que el bloque 12 para mejorar más
la retrorreflexión de la luz dispersada o energía emitida al medio
18. Puesto que en la realización ilustrativa, la máscara 16 se
soporta en una placa o pastilla transparente 15, este componente
también tiene un perfil más grande. Alternativamente, la máscara 16
puede ser una chapa fina o pastilla en la que se ha formado
hendiduras 20, y el componente transparente 15 puede ser una capa,
por ejemplo, de zafiro que tiene un grosor en el rango de 1 a 2
micras que se reviste encima. En este caso, el recubrimiento no
tiene que extenderse más allá de las dimensiones de bloque 12; sin
embargo, es preferible que este recubrimiento se extienda por toda
la dimensión de la máscara 16 para proporcionar una buena adaptación
del índice óptico para luz retrorreflejada.
Finalmente, el aparato de la figura 1 incluye una
caja 23 conectada al cabezal 10 por líneas eléctricas adecuadas y,
donde sea apropiado, tubos (para el agua de refrigeración) 24. La
caja 23 puede contener fuentes de alimentación apropiadas para las
barras diodos 11, circuitería de control, bombas de fluido donde se
utiliza enfriamiento de fluido y otros componentes apropiados. Los
componentes específicos contenidos en la caja 23 no forman parte de
la presente invención.
El aparato de la figura 1 tiene varias
características ventajosas sobre la técnica anterior. En primer
lugar, donde el medio 18 es la piel de un paciente que se somete a
un procedimiento dermatológico, tal como por ejemplo la extracción
de un tatuaje, una mancha de Oporto, vaso sanguíneo, u otro lesión
vascular, o extracción de vello, es deseable enfriar la epidermis,
la capa superficial de la piel, para evitar que se dañe por calor
durante el procedimiento. En la técnica anterior se ha previsto un
mecanismo de enfriamiento de la epidermis en particular, y del área
superficial de la piel del paciente en general, mecanismo de
enfriamiento que está separado y es independiente del mecanismo de
enfriamiento utilizado para disipar calor de las barras diodo 11.
Estos mecanismos de enfriamiento separados aumentan el tamaño, el
peso, la complejidad y el costo del sistema en general, y del
cabezal de distribución 10 en particular. La realización de la
figura 1 supera estos problemas teniendo a lo sumo unos pocos
milímetros de material entre el bloque 12, que es enfriado por
componentes termoeléctricos 14, por un fluido circulante, y/o por
otros medios adecuados, y la piel del paciente. Además, el zafiro
usado típicamente para el componente transparente 15 tiene buenas
propiedades de transferencia térmica de manera que el calor de la
piel del paciente puede fluir fácilmente al bloque enfriado 12, y
este bloque puede servir como un disipador térmico para las barras
diodo 11 y la epidermis de la piel del paciente u otra zona
superficie de un medio 18 al que se está aplicando energía luminosa.
Esta disposición es más compacta, más simple y menos cara que los
cabezales de la técnica anterior que realizan la misma función.
Además, como se ilustra en la figura, la energía
luminosa emitida de una barra diodo 11 en forma de rayos 26 se
dispersa en el medio 18, por ejemplo en la piel del paciente, y al
menos parte de esta energía, tal vez el 70 por ciento, dependiendo
de la pigmentación de la piel del paciente, se refleja de nuevo y
sale de la piel del paciente en cierto ángulo. Sustancialmente toda
esta luz choca en la superficie reflectora o máscara 16, y, puesto
que esta máscara tiene una reflectividad que se aproxima al 100 por
ciento, sustancialmente toda esta energía luminosa es
retrorreflejada de nuevo a la piel. Esta retrorreflexión da lugar a
aproximadamente 300 por ciento de aumento de la energía luminosa o
fluencia que alcanza un blanco a una profundidad seleccionada en la
piel del paciente para una fluencia dada emitida por las barras
diodo 11. Esto significa que se puede lograr los mismos resultados
terapéuticos usando menos barras diodo 11 o barras diodo de energía
más baja 11 o que se puede lograr energía más alta, y por lo tanto
tratamiento más eficaz, usando el mismo número de barras diodo y de
la misma potencia. Así se puede lograr resultados más eficaces para
un tamaño, costo y complejidad dados del cabezal de diodo láser.
Además, como se ilustra en la figura 10B, la
energía luminosa que entra en el medio de dispersión 18 sobre un
agujero de tamaño D, a causa de la divergencia y dispersión del haz,
saldrá del medio por un agujero D + d, donde d es un incremento del
agujero de retrodispersión y es sustancialmente constante para una
longitud de onda del haz y medio dados, independientemente del
agujero de entrada D. Esto se ilustra con las figuras 10A y 10B,
donde d es sustancialmente el mismo para un haz fino que entra
sustancialmente en el medio 18 en un único punto y para un haz ancho
que tiene un agujero D. Así, a medida que aumenta el tamaño del
agujero D, d es un menor porcentaje del agujero de reflexión D + d.

La reflexión por la máscara reflectora 16 puede
aumentar varias veces la cantidad de energía que llega a una zona
deseada. Este aumento del uso efectivo de energía luminosa se puede
describir cuantitativamente por el aumento de iluminación dentro de
medio de dispersión 18, siendo este aumento la relación (f) entre la
iluminación en un punto deseado arbitrario dentro del medio de
dispersión cuando la luz reflejada se hace volver de nuevo al medio
(IR) y cuando no (I_{O}) (es decir, f = I_{R}/I_{O}). El valor
de f depende del coeficiente de reflectancia R del medio de
dispersión 18 y el coeficiente de reflexión de la máscara
reflectante 16 (r) que devuelve la luz dispersa de nuevo al medio
(es decir, f = 1/1-Rr). Sin embargo, esta
dependencia conocida no tiene en cuenta la influencia del agujero
del haz D; puesto que el agujero del haz aumenta d como resultado de
la dispersión, el coeficiente de amplificación f tiene una fuerte
dependencia del agujero D del haz incidente. En particular, según
las ideas de esta invención, se ha determinado que cuando se toma en
cuenta el agujero del haz, el coeficiente de amplificación f se
puede expresar aproximadamente por la ecuación siguiente:

incidente en la red región del espectro, los valores en la ecuación
anterior serían R = 0,7 y d = 3 mm. Suponiendo un reflector con r =
0,95 daría lugar a D_{min} = 19,5 mm. Esto sugiere que para la
mayoría de las aplicaciones en dermatología por láser, el diámetro
del haz u otra dimensión apropiada (D) deberá ser mayor que 20 mm
para que la retrorreflexión proporcione amplificación de iluminación
deseada. Esto se ilustra en la figura 11, donde (f) se representa
como una función de la relación D/d para tres entornos de reflexión,
siendo r 0,95 en cada caso, y siendo R igual a 0,2, 0,5 y 0,8,
respectivamente. Se ve que, en particular para el medio altamente
dispersante que tiene R=0,8, f continúa aumentando con el tamaño
creciente del agujero de entrada y puede, con retrorreflexión,
proporcionar hasta 3,8 veces la intensidad lograda sin
retrorreflexión. Suponiendo que d es igual a 3 mm, un agujero de
entrada de 20 mm daría lugar a más de dos veces la iluminación en el
blanco que si no se utilizase retrorreflexión, y un agujero más
pequeño, por ejemplo D=15 mm, todavía proporcionaría una
amplificación significativa. Así, aunque cada barra diodo individual
11 produce un haz que tiene una dimensión en el rango micrométrico,
el cabezal 10 se puede diseñar para proporcionar un haz con una
dimensión D suficiente para proporcionar una amplificación de
iluminación deseada. La superficie reflectora 16 se hace
preferiblemente suficientemente grande para cubrir completamente el
agujero de reflexión que consta de D + d, pero puede requerir poca o
nula extensión más allá del extremo de bloque 12 donde D es grande
con relación a d.
La realización representada en la figura 1
proporciona así al menos tres ventajas significativas sobre los
cabezales de diodos láser de la técnica anterior. Primera:
proporciona un mecanismo muy eficiente para enfriar los diodos láser
y la superficie del medio 18 mediante el uso del mismo mecanismo de
enfriamiento. Segunda: proporciona un mecanismo simple y efectivo
para retrorreflejar luz dispersada del medio 18 sobre todo el
agujero de dispersión de tal medio; y tercera: proporciona un
agujero de haz que es suficientemente grande para la eficiente
amplificación de iluminación como resultado de retrorreflexión a la
vez que utiliza fuentes de radiación, por ejemplo barras de diodos
láser, que proporcionan individualmente pequeños agujeros de haz en
el rango micrométrico.
La figura 2B ilustra una realización alternativa
de la invención que puede ser útil en algunas aplicaciones. La
realización de la figura 2B difiere de la de la figura 2A en que la
capa transparente 15 ha sido sustituida por una lente cilíndrica 31
montada debajo de cada una de las barras de diodo láser 11. Las
lentes cilíndricas 31 se pueden soportar en la matriz de formas
conocidas en la técnica incluyendo una ménsula de soporte u otra
estructura de soporte, que se monta o forma parte del bloque 12 en
extremos opuestos de cada lente cilíndrica 31. El bloque 12 también
se extiende algo por debajo de la parte inferior de las barras diodo
11 para proporcionar soporte estructural a las lentes 31 y para que
el bloque 12 pueda contactar la superficie superior del medio 18
cuando se aplique ligera presión al bloque 12 de manera que el
bloque todavía pueda enfriar la superficie del medio. Se forma un
recubrimiento reflector 16 en la pared inferior del bloque 12 en
todas sus zonas a excepción de las zonas directamente debajo de los
emisores de barras diodo 11A, extendiéndose por lo demás el
recubrimiento reflector sustancialmente alrededor de toda la pared
del rebaje en la que está colocada la lente 31. Dependiendo de su
diámetro, una lente 31 puede funcionar para colimar el haz 26 que
emana de la barra diodo correspondiente 11 en rayos paralelos, en
contraposición a rayos divergentes como se representa en la figura
2A, o para converger tales haces hacia un punto focal que está
preferiblemente a la profundidad deseada. Dicha colimación o
convergencia del haz 26 reduce los efectos nocivos de la dispersión
en el haz, pero no elimina dicha dispersión o reduce
considerablemente la necesidad de la superficie reflectora 16.
La figura 3 muestra una realización que difiere
de la de la figura 1 en que se requiere fluencia más alta que la
proporcionada por las barras diodo solo, incluso con
retrorreflexión. Por lo tanto, la energía emitida por la capa
transparente 15 se aplica a un concentrador estándar 34A, que puede
ser un cono truncado o prisma hueco, pero es preferiblemente un
bloque o losa de material que tiene buena coincidencia de índice y
buenas propiedades de transferencia de calor al medio 18, para
ejemplo, zafiro cuando el medio es piel humana. El concentrador 34A
sacrifica el tamaño del agujero para lograr fluencia más alta de
manera conocida en la técnica. Sin embargo, el tamaño del agujero se
mantiene suficiente para adaptarse a los requisitos especificados en
la ecuación (2) anterior para mantener los efectos de amplificación
de energía de la retrorreflexión.
La realización de la figura 3 también resuelve un
segundo problema porque la luz dispersada se emite de la piel a
varios ángulos y vuelve a la piel en general al mismo ángulo en que
se recibió. Esto da lugar a una concentración más alta de energía
óptica en la superficie de la piel donde se reciben todos los haces
y menor energía a profundidad, donde está situado en general el
blanco deseado, apareciendo solamente en la superficie haces
inclinados de forma pronunciada. Dado que la energía concentrada en
la superficie de la piel no cumple ninguna finalidad terapéutica
útil y puede producir daño térmico o incomodidad si no se enfría
adecuadamente, es deseable reducir o eliminar dichos haces
reflejados inclinados de forma pronunciada, aunque sin interferir
con haces reflejados a ángulos sustancialmente perpendiculares a la
superficie del medio y devueltos a la piel a estos ángulos. Este
objetivo se lleva a cabo para la realización de la figura 3
previendo un recubrimiento 32 en las paredes laterales de
concentrador 34A, recubrimiento que tiene características de
reflexión dependientes de ángulo y puede tener una reflectividad
considerablemente más baja que la superficie reflectora 16. Esto
significa que los haces inclinados de forma pronunciada que chocan
en la superficie 32 se atenúan o eliminan, reduciendo por ello los
haces que entran en el medio 18 en un ángulo pronunciado, siendo
estos haces solamente nocivos y no produciendo ningún efecto
terapéutico útil.
Aunque la realización de la figura 3 tiene las
ventajas explicadas anteriormente, también tiene dos desventajas
potenciales. Primera: el agujero para recibir radiación reflejada es
menor que el agujero (es decir, D + d) de radiación reflejada, de
manera que esta realización no recoge toda la radiación reflejada ni
la retrorrefleja al medio 18. Esto da lugar a una ligera disminución
de la relación de amplificación de intensidad (f) en esta
realización; sin embargo, esta desventaja es mitigada por el hecho
de que gran parte de la energía perdida en esta realización es
energía a ángulos que, aunque retrorreflejados, solamente
contribuyen a calentar la superficie del medio 18 y no tienen un
efecto terapéutico ni realizan ningún otro trabajo útil en una zona
blanco situada a una profundidad seleccionada en el medio. Al ser D
mayor que (d) también minimiza esta pérdida. Si se desea, también se
podría prever extensiones reflectoras en esta realización para
retrorreflejar toda la energía reflejada.
La segunda desventaja es que, dependiendo del
grosor del concentrador 34A, el bloque enfriado 12 puede no ser
efectivo para enfriar la superficie del medio 18. En particular, el
tiempo (t) que tarda en eliminar calor de una losa de material que
tiene un lado en buen contacto térmico con la superficie a enfriar,
un lado opuesto en buen contacto térmico con el medio de
enfriamiento, en este caso el bloque 12, y una distancia o grosor
(/) entremedio viene dado por:

Donde la capa dieléctrica a través de la que se
transmite energía óptica y a través de la que se desea efectuar
enfriamiento es de zafiro con \alpha máximo = 15 \cdot
10-6 m^{2}/s, y para un intervalo típico entre
pulsos de 0,25 s, esto daría lugar al grosor combinado de la capa
transparente 15 y el concentrador 34A de menos de 1,9 mm. Por lo
tanto, el bloque 12 que se utiliza para enfriar las barras diodo 11
y la superficie de medio 18 no sería normalmente factible cuando se
utilice un concentrador 34A y, si se requiere enfriamiento, se
lograría normalmente previendo un mecanismo de enfriamiento
separado, por ejemplo uno o varios elementos de enfriamiento
termoeléctricos 36, en contacto con el concentrador 34A, y
preferiblemente cerca de su superficie inferior. Aunque solamente se
representa un solo elemento de enfriamiento en la figura 3, se
proporcionaría típicamente cuatro o más elementos, espaciados
sustancialmente uniformemente alrededor de la periferia del
concentrador 34A, para realizar su enfriamiento uniforme.
Las figuras 4 y 5 ilustran dos realizaciones
adicionales de la invención que difieren de la representada en la
figura 3 solamente en que, en la figura 4, la losa 34B es un
expansor en vez de un concentrador, mientras que en la figura 5, la
losa 34C tiene paredes paralelas para no funcionar como un
concentrador o un expansor. Por lo tanto, las losas 34A, 34B y 34C
permiten usar un bloque único 12 con barras diodo 11, capa
transparente 15 y capa reflectora 16 para lograr varios niveles de
fluencia programables. La realización de la figura 4 es ventajosa
porque permite recoger y reciclar más luz dispersa emitida por la
superficie de medio 18 que las otras realizaciones, teniendo la
realización de la figura 5 capacidades intermedias de recogida de
luz dispersa. Las tres realizaciones pueden tener paredes laterales
reflectoras dependientes de ángulo 32 para reducir o eliminar
sustancialmente la luz emitida a ángulos pronunciados. Aunque la
reflexión reducida de las superficies 32 puede ser uniforme, es
preferible que la reflectancia de estas superficies sea dependiente
del ángulo de manera que la luz que choque en estas superficies a
ángulos más pronunciados se atenúe más intensamente, mientras que la
luz que choque en estas superficies a ángulos más pequeños, y por lo
tanto la luz casi más emitida de la superficie en una dirección
perpendicular, se atenúe mucho menos, o en otros términos se refleje
más plenamente. Además, la superficie reflectora 16 en todas las
realizaciones también puede ser dependiente de ángulo, reflejando
más fuertemente luz que entra a ángulos sustancialmente
perpendiculares que luz que entra a ángulos más pronunciados. Aunque
esto se puede lograr con un recubrimiento de capa única, se logra
preferiblemente con un recubrimiento de capas múltiples.
Las figuras 6-8 ilustran varias
especies de otra realización de la invención donde el bloque 12 es
sustituido por un bloque 42 en el que se ha formado un rebaje 44. Se
han formado ranuras 13 en una configuración seleccionada alrededor
del perímetro del rebaje 44. En particular, con referencia a la
figura 6, en el bloque 42A se ha formado un rebaje semicilíndrico
44A, estando dispuestas ranuras 13 en las que se han montado barras
diodo 11 en una configuración semicircular alrededor de la periferia
del rebaje 44A, siendo cada ranura 13 y barra diodo 11
sustancialmente perpendiculares a la circunferencia del rebaje 44A
(es decir, sustancialmente paralelas a radios) en el punto de la
circunferencia donde están situadas. Parte del medio 18 adyacente al
rebaje 44A se sube y pone en contacto con la superficie transparente
15 formada dentro del rebaje. El medio se puede poner en el rebaje
44A presionando el bloque 42A contra un medio relativamente blando
18, por ejemplo piel en algunas zonas, para empujar la piel al
rebaje, o se puede prever una fuente de vacío, junto a las barras
cerca del medio del rebaje o entre tales barras, para aspirar la
piel al rebaje. También se puede emplear otras técnicas para empujar
la piel u otros medios 18 al rebaje 44A, además de o en lugar de una
o varias de las dos técnicas mencionadas anteriormente. Finalmente,
en la porción inferior del bloque 42A fuera del rebaje 44A se ha
formado un recubrimiento reflector dependiente de ángulo 32,
reflejando esta superficie parte de la luz de nuevo a la piel en una
zona donde se puede dispersar al rebaje
44A.

Para la realización de la figura 6, la zona
deseada para la energía luminosa estaría aproximadamente en los
focos de las barras diodo, que en general sería un punto cerca del
centro inferior del rebaje 44A. La luz reflejada por la piel antes
de llegar a dicha zona deseada sería reflejada típicamente de nuevo
al rebaje y en último término devuelta al blanco dando lugar a una
relación muy alta de aumento de iluminación (f) con esta
realización.
La figura 7 ilustra una realización que difiere
de la de la figura 6 en la que el rebaje 44A en el bloque 42A, en
lugar de tener meramente una capa transparente fina 15, tiene un
bloque transparente o lente 40 con un rebaje rectangular estrecho 48
formado en la lente cilíndrica 40. Las ranuras 13 y las barras diodo
11 montadas en ellas están a un ángulo ligeramente mayor para tener
un enfoque cerca de la mitad superior del rebaje 48. La realización
de la figura 7 está especialmente adaptada para aplicaciones de
extracción de vello donde un pliegue de piel que tiene un solo
folículo capilar en el plano de la figura (puede haber varios
folículos del cabello en el rebaje 48 a lo largo de la longitud del
rebaje) está en el rebaje 48 en un tiempo dado. Normalmente se
necesitará vacío para arrastrar dicho pliegue de piel al rebaje 48.
En cuanto a la realización de la figura 6, esta realización da lugar
a una alta concentración de luz, incluyendo luz reflejada de la
superficie reflectora 16 que llega al punto deseado en el rebaje 48.
Este efecto se mejora más previendo en la superficie inferior de la
lente cilíndrica 40 un recubrimiento altamente reflector 49 que
evita que salga luz de la lente al medio 18. Así, se aplica a la
zona deseada sustancialmente 100 por ciento de la luz producida por
barras diodo 11 en esta realización de la invención, no perdiéndose
virtualmente nada de energía a dispersión.
La figura 8 es similar a la figura 6 a excepción
de que el rebaje 44B en el bloque 42B tiene una sección transversal
rectangular en vez de una sección transversal semicircular, y las
ranuras 13 son perpendiculares a las paredes del rebaje 44B en los
puntos donde están situadas. Aunque esta realización no da lugar a
un enfoque de la luz en un único punto como en las realizaciones de
las figuras 6 y 7, da lugar a una alta concentración de energía
luminosa en el rebaje 44B que se aplica al medio movido al rebaje
por presión, vacío, u otros medios adecuados.
La realización de la figura 9 es similar a la de
la figura 1 a excepción de que en vez de extenderse porciones para
las capas 15 y 16, hay extensiones flexibles 52 en cada extremo del
bloque, formándose en las extensiones un recubrimiento reflector
dependiente de ángulo 32. Se puede usar vacío para arrastrar parte
del medio 18 a la zona debajo del bloque 12 y las extensiones 52
para proporcionar mejor radiación de una zona deseada en esta región
o debajo de ella. Las secciones laterales 52 con recubrimiento
reflector dependiente de ángulo son más eficaces al dirigir energía
luminosa en la región (d) (figura 10B) a la zona deseada que las
pestañas de la figura 1.
Aunque no se ha mencionado específicamente
anteriormente, las realizaciones de las figuras 6-9
también pueden utilizar la técnica de enfriamiento de la figura 1
donde se utiliza el bloque 12 y/o el bloque 42 para enfriar barras
diodo 11 y para enfriar la superficie de la piel u otros medios 18.
La realización de la figura 7 no es tan efectiva para lograr este
objeto como algunas de las otras realizaciones.
Aunque en las realizaciones antes descritas, las
barras diodo se han montado en el bloque 12 del cabezal 10, en
algunas aplicaciones se podría montar adecuadamente otros emisores
de luz, por ejemplo lámparas de filamento tal como lámparas
halógenas, en el bloque 12 en lugar de las barras diodo. También se
podrían lograr muchas de las ventajas de esta invención si un tubo
de luz que recibe luz de un láser u otra fuente emisora de luz
sustituye a cada barra diodo 11 en las varias realizaciones. Por
ejemplo, la figura 12 muestra un cabezal 10' que difiere del
representado en la figura 1 en el que se pasa luz de un láser u otro
emisor de energía luminosa de potencia y longitud de onda adecuadas
a través de un tubo de luz en las líneas 24 a una red de tubos de
luz 60 en el bloque 12, habiendo una pluralidad de tubos de luz 60
detrás de cada tubo de luz mostrado para proporcionar
sustancialmente la misma configuración de emisión de luz que para la
pluralidad de emisores 11A de cada barra diodo 11. El tamaño mínimo
de agujero D para lograr una amplificación seleccionada (f) de
retrorreflexión también es aplicable sustancialmente a cualquier
láser u otro cabezal emisor de energía luminosa usado en un medio de
dispersión, incluyendo los representados en las varias patentes y
solicitudes anteriores incluyendo 5.595.568; 5.735.844; 5.824.023, y
la solicitud número de serie 09/078055, cuya figura 4, por ejemplo,
muestra un cabezal que se puede usar al llevar a la práctica las
ideas de esta invención, pero que difiere de la figura 12 en que los
tubos de luz están inclinados para enfocar la energía luminosa.
Donde se utilizan tubos de luz, la capa transparente o el elemento
pueden no ser necesarios, y el recubrimiento reflector 16 se puede
aplicar directamente a la superficie inferior del bloque 12,
disponiéndose agujeros en el recubrimiento debajo de cada tubo de
luz.
Además, la capa transparente 15 está espaciada
preferiblemente al menos varias micras, por ejemplo
50-100 micras, de las barras diodo como protección
contra cortocircuitos de las barras láser, y este espacio se puede
llenar con aire u otro gas, o con un líquido o material sólido
aislante que sea transparente al menos en las zonas debajo de los
agujeros o hendiduras en la capa reflectora 16. En esta realización,
la separación puede ser tal que el enfriamiento del medio del bloque
12 ya no sea posible.
Se ha descrito así una invención, incluyendo
varias realizaciones y varias especies de cada realización, que en
algunas realizaciones proporciona un mecanismo de enfriamiento más
simple de la superficie de un medio que experimenta un láser u otro
procedimiento de energía óptica y que también proporciona un uso más
óptimo, y en algunos casos sustancialmente óptimo, de la energía
luminosa producida por barras de diodo láser, u otra fuente de
energía óptica, incluso cuando la luz está siendo suministrada a un
medio altamente dispersante, diseñando el dispositivo para
proporcionar un agujero de entrada adecuado y mecanismos adecuados
para retrorreflejar dicha luz. Además, aunque se han descrito varias
realizaciones y sus especies, es evidente que éstas se ofrecen a
efectos ilustrativos solamente y que también se podría emplear otros
mecanismos similares o equivalentes. Así, aunque la invención se ha
mostrado y descrito con detalle anteriormente con referencia a las
realizaciones preferidas y especies, los expertos en la técnica
pueden hacer cambios en lo anterior y otros cambios en la forma y
detalle sin apartarse del alcance de la invención, que se ha de
definir solamente por las reivindicaciones anexas.
Claims (24)
1. Un cabezal (10) para aplicar energía luminosa
a una profundidad seleccionada en un medio de dispersión (18) que
tiene una capa externa en contacto físico y térmico con dicho
cabezal incluyendo:
un conjunto conductor térmico (12) (42) que tiene
una superficie emisora de energía; y
una pluralidad de elementos emisores de energía
óptica (11) montados en dicho conjunto (12) (42) junto a dicha
superficie emisora de energía, estando cada elemento en contacto
térmico con dicho conjunto y orientado para dirigir energía luminosa
a través de dicha superficie, caracterizado porque
incluye:
una capa conductora térmica transparente (15)
(31) (40) sobre dicha superficie y en contacto térmico con ella,

incluyendo una capa reflectora (16) en dicha capa conductora térmica
(15), teniendo dicha capa reflectora (16) un agujero (20) formado en
la misma debajo de cada elemento (11) a través del que se puede
aplicar energía luminosa a dicho medio (18).
3. Un cabezal (10) según la reivindicación 2,
donde dicha capa reflectora (16) tiene un área más grande que el
área de dicho conjunto (12) (42).
4. Un cabezal (10) según la reivindicación 2 o 3,
donde el área de dicha capa reflectora (16) es al menos
sustancialmente tan grande como un agujero de reflexión para energía
luminosa dispersada de dicho medio (18).
5. Un cabezal (10) según cualquier reivindicación
anterior, donde se aplica energía luminosa a dicho medio (18) a
través de un agujero, donde hay un coeficiente de amplificación
deseado f como resultado de la retrorreflexión de dicha capa
reflectora (16), donde el medio (18) y la capa reflectora (16)
tienen coeficientes de reflexión R y r respectivamente, y donde el
valor mínimo D_{min} para una dimensión D del agujero es

donde dicho agujero es sustancialmente circular, y donde D es el
diámetro del agujero.

donde dicho agujero es sustancialmente rectangular y D es la
longitud de un lado corto de dicho agujero.
8. Un cabezal (10) según cualquier reivindicación
anterior, donde dicho conjunto (42) tiene una depresión (44A)
formada en el mismo, siendo dicha superficie emisora de energía la
superficie de dicha depresión, estando montado cada uno de dichos
elementos (11) para emitir energía luminosa sustancialmente
perpendicular a la superficie de la depresión en el punto de ella
donde está montado el elemento, pudiendo ser empujado dicho medio
(18) a dicha depresión (44A) y a contacto con su superficie.
9. Un cabezal (10) según la reivindicación 8,
donde dicha depresión es de forma sustancialmente semiesférica.
10. Un cabezal (10) según la reivindicación 8,
donde dicha depresión es de forma sustancialmente rectangular.
11. Un cabezal (10) según cualquier
reivindicación anterior, donde dicha capa conductora térmica (15) es
un recubrimiento formado en dicha superficie fotoemisora.
12. Un cabezal (10) según cualquier
reivindicación anterior, donde cada elemento (11) es una barra de
diodo láser.
13. Un cabezal (10) según cualquier
reivindicación anterior incluyendo una capa reflectora (16) que
cubre todas las porciones de dicha superficie a excepción de cada
porción seleccionada de dicha superficie a través de la que se
dirige energía luminosa, retrorreflejando dicha capa reflectora (16)
energía luminosa dispersada de dicho medio.
14. Un cabezal (10) según la reivindicación 13,
donde cada elemento emisor de energía óptica (11) incluye una guía
de luz que se extiende al menos en parte a través de dicha
estructura de dicha superficie y conectada ópticamente para recibir
energía luminosa de un componente emisor de luz.
15. Un cabezal (10) según la reivindicación 14,
donde dicho componente emisor de luz es uno de un láser, una lámpara
de destellos y una lámpara de filamento.
16. Un cabezal (10) según la reivindicación 13,
14 o 15, donde cada elemento emisor de energía (11) incluye un
emisor de energía luminosa montado en dicha capa conductora térmica
(15) (31) (40) para emitir su energía luminosa mediante la porción
correspondiente seleccionada de dicha superficie.
17. Un cabezal (10) según la reivindicación 16,
donde cada emisor (11) está montado junto a, pero espaciado de,
dicha superficie, estando dicha capa reflectora (16) en al menos un
lado de dicha capa conductora térmica transparente (15) (31)
(40).
18. Un cabezal (10) según la reivindicación 16 o
17, donde dicha capa reflectora (16) tiene un área más grande que el
área de dicha superficie.
19. Un cabezal (10) según cualquier
reivindicación anterior, donde dicha capa conductora térmica (15) es
una guía de onda (34) de una forma seleccionada.
20. Un cabezal (10) según la reivindicación 19,
incluyendo una capa reflectora dependiente de ángulo (32) en paredes
seleccionadas de dicha guía de onda para atenuar de forma
pronunciada la energía luminosa inclinada que entra en la guía de
onda.
21. Un cabezal (10) según la reivindicación 19 o
20, donde dicha forma seleccionada es una pirámide truncada, estando
adyacente a dicho conjunto (12) uno de un extremo más grande y un
extremo más corto de una pirámide.
22. Un cabezal (10) según cualquiera de las
reivindicaciones 16 a 21, donde dicha capa conductora térmica (15)
es un recubrimiento formado en dicha superficie fotoemisora.
23. Un cabezal (10) según cualquiera de las
reivindicaciones 16 a 22, donde dicho emisor (11) es uno de una
barra de diodo láser y una lámpara de filamento.
24. Un cabezal (10) según cualquiera de las
reivindicaciones 12 a 23, donde dicho conjunto (42A') tiene una
depresión (44A) formada en el mismo, siendo dicha superficie emisora
de energía la superficie de la depresión (44A), estando adaptada la
capa conductora térmica transparente (40) para llenar dicha
depresión, estando formado un rebaje estrecho (48) en dicha capa
conductora térmica, dirigiendo dichos elementos emisores de energía
óptica (11) energía luminosa a dicha superficie para enfocar luz a
un punto en dicho rebaje (48).
