Abstract
UNA FORMULACION FARMACEUTICA DE LIBERACION CONTROLADA COMPRENDE NANOPARTICULAS FORMADAS POR UN POLIMERO DE POLICIANOACRILATO BIODEGRADABLE EN EL CUAL LA INSULINA ESTA INCLUIDA, ENCONTRANDOSE LA INSULINA COMPLEJADA CON EL POLICIANOACRILATO. ESTAS PARTICULAS SON CAPACES DE LIBERAR INSULINA BIOACTIVA IN VIVO A UNA VELOCIDAD MENOR QUE LA DE LAS NANOPARTICULAS EN LAS QUE LA INSULINA SE ENCUENTRA LIBRE. Description
Nanopartículas biodegradables de liberación
controlada que contienen insulina.
Esta invención se refiere a formulaciones de
nanopartículas biodegradables de liberación controlada y, en
particular, a nanopartículas de poli(cianoacrilato de
alquilo) biodegradables que contienen insulina o análogos de
insulina.
Las nanopartículas de poli(cianoacrilato
de alquilo) se han estudiado como potenciales vehículos de fármacos
para formulaciones de liberación sostenida, para dirigir fármacos y
para mejorar la biodisponibilidad de péptidos y proteínas tales
como insulina, calcitonina y factor de liberación de hormona de
crecimiento (Damgé, C. et al., Diabetes, 37:
246 (51) (1988); Lowe, P. et al., J. Pharma.
Pharmacol., 46: 547-552 (1994); y
Gautier, J. et al., J. Controlled Release, 20:
67-78 (1992)). Se utilizan típicamente monómeros de
cianoacrilato de cadena corta, tales como cianoacrilato de
n-butilo, isobutilo e isohexilo debido a sus cortos
tiempos de degradación y bajas toxicidades.
Se emplean habitualmente dos métodos para la
preparación de estas nanopartículas: el método de polimerización
interfacial y el método de polimerización aniónica. En el método de
polimerización interfacial, el monómero y el fármaco se disuelven
en una fase orgánica, ocurriendo la polimerización en la interfase
orgánico-acuosa tras la adición de la fase orgánica
a una fase acuosa (Al Khouri, N. et al., Int. J.
Pharm., 28: 125-132 (1986). Este método
produce nanocápsulas. En general, se han producido nanopartículas
cargadas con péptido de la técnica anterior mediante esta técnica
(véanse, por ejemplo, los documentos
EP-A-0 608 207;
EP-A-0 447 318; y
FR-A-2.515.960).
La polimerización aniónica ocurre en medios
acuosos a bajo pH y se cataliza mediante iones hidroxilo. En este
método, el fármaco puede incorporarse a la nanopartícula,
adsorberse sobre la nanopartícula o una combinación de ambos,
dependiendo del momento de adición del fármaco al medio de
polimerización (Couvreur, P. et al., J. Pharma.
Pharmacol., 31: 331-332 (1979)).
Las nanopartículas de poli(cianoacrilato
de alquilo) asociadas a vidarabina, que resultan de la interacción
química de la vidarabina con el monómero cianoacrílico durante el
proceso de polimerización, se han preparado utilizando el método de
polimerización aniónica modificado por la inclusión obligatoria de
sulfosuccinato de dioctilo en el medio acuoso. Sin embargo, la
formación del complejo poli(cianoacrilato de
alquilo)/vidarabina inactivó la actividad biológica de la
vidarabina (Guise. V., et al., Pharmaceutical Res.,
7: 736-741 (1990)).
La administración de insulina exógena puede
mejorar las anormalidades metabólicas en diabetes de tipo II
compensando la secreción reducida de insulina endógena, reduciendo
la producción excesiva de glucosa hepática y estimulando la
captación de glucosa. Adicionalmente, la administración de insulina
no sustitutiva en diabetes no insulinodependiente está indicada
cuando, sin ella, no se consigue una compensación satisfactoria de
la diabetes. Los posibles efectos secundarios de la terapia de
insulina incluyen ganancia de peso, hiperinsulinemia e
hipoglucemia.
Aunque las inyecciones subcutáneas diarias únicas
o múltiples de insulina son la base principal de las técnicas de
suministro de insulina, están ahora disponibles o en desarrollo
otros diversos métodos de suministro de insulina, incluyendo (a)
infusión subcutánea continua de insulina mediante una bomba de
infusión portátil; (b) transplante total o segmentado de páncreas;
(c) transplante de células islote aisladas; (d) implante de una
bomba de insulina programable; (e) mecanismos de suministro oral,
nasal, rectal o transdérmico de insulina; (f) administración de
análogos de insulina; (g) implante de cápsulas poliméricas que
proporcionan una liberación continua o por pulsos de insulina; y (h)
implante de un biohíbrido páncreas artificial que utiliza islotes
encapsulados. A pesar de estos avances, el suministro ideal de
insulina a pacientes está aún por desarrollar. Por ejemplo, los

formados por monómeros de cianoacrilato de alquilo
4-6C tales como cianoacrilato de
n-butilo, cianoacrilato de isobutilo y cianoacrilato
de isohexilo o mezclas de los mismos.
Como se utiliza en la presente memoria, el
término "biodegradable" aplicado a polímeros de
poli(cianoacrilato de alquilo) significa polímeros que son
degradables in vivo enzimática o no enzimáticamente

fisiológicas normales.

típicas, la insulina se compleja con policianoacrilato durante la
etapa de polimerización, formando nanopartículas cargadas con
insulina complejada con poli(cianoacrilato de alquilo). A pH
mayor, la insulina no se compleja con el polímero sino que se
incorpora a las nanopartículas en forma de insulina libre. Más
sorprendentemente, las nanopartículas cargadas con insulina
complejada con poli(cianoacrilato de alquilo) son capaces de
liberar insulina bioactiva in vivo. La velocidad de
liberación de insulina de estas nanopartículas cargadas con insulina
complejada con poli(cianoacrilato de alquilo) es más lenta
que la de las correspondientes nanopartículas de
poli(cianoacrilato de alquilo) cargadas con insulina libre.
A un pH intermedio, las nanopartículas pueden ser una mezcla
natural de insulina libre y complejada.
Por tanto, esta invención proporciona
formulaciones orales y parenterales que contienen nanopartículas
cargadas con insulina complejada con poli(cianoacrilato de
alquilo) que son capaces de liberar cantidades farmacéuticamente
eficaces de insulina bioactiva de manera controlada para satisfacer
las necesidades fisiológicas de insulina cuando se administran por
vía oral o parenteral a un sujeto, particularmente un ser humano.
Además, la naturaleza de liberación controlada de estas
formulaciones hace posible el direccionamiento de la insulina al
intestino delgado cuando se administra por vía oral, lo que
potencia la biodisponibilidad de la insulina. Opcionalmente, estas
formulaciones oral y parenteral pueden comprender una mezcla de
nanopartículas cargadas con insulina complejada con
poli(cianoacrilato de alquilo) y nanopartículas de
poli(cianoacrilato de alquilo) cargadas con insulina
libre.
Las nanopartículas biodegradables contienen
preferiblemente de 5% a 30% p/v de contenido de insulina a
monómero, más especialmente 15-25% p/v de
insulina.
El tamaño de las nanopartículas biodegradables es
adecuadamente de 50-900 nm, preferiblemente de
100-400 nm.
Las nanopartículas cargadas con insulina
complejada con poli(cianoacrilato de alquilo) según la
invención y, opcionalmente, nanopartículas de
poli(cianoacrilato de alquilo) cargadas con insulina libre si
están presentes, se incorporan adecuadamente a formas de
dosificación oral tales como cápsulas, comprimidos, polvos,
incluyendo polvos capaces de efervescencia tras la adición de agua,
o suspensiones. Adicionalmente, puede aplicarse un recubrimiento
entérico a las nanopartículas cargadas de insulina complejada con
poli(cianoacrilato de alquilo) y, opcionalmente,
nanopartículas de poli(cianoacrilato de alquilo) cargadas
con insulina libre si están presentes, para proteger la formulación
mientras pasa a través del estómago para dirigir adicionalmente la
liberación de insulina al intestino delgado. Como alternativa, las
nanopartículas pueden administrarse por vía parenteral para liberar
insulina que imite las necesidades fisiológicas de insulina.
Por tanto, para una administración oral
conveniente y eficaz, pueden formarse comprimidos con cantidades
farmacéuticamente eficaces de las nanopartículas de esta invención
con uno o más excipientes, englobarse en cápsulas tales como
cápsulas de gel, formularse con ingredientes que tras la adición de
agua proporcionan una solución efervescente, y suspenderse en una
solución líquida y similares. Las nanopartículas pueden suspenderse
en una solución salina o similar para administración
parenteral.
Cuando las nanopartículas forman comprimidos, los
comprimidos comprenden opcionalmente un recubrimiento entérico sobre
el comprimido para dirigir la liberación de insulina al intestino
delgado cuando se administran por vía oral.
Se apreciará que las formulaciones farmacéuticas
según la invención pueden utilizarse entre otras cosas para
proporcionar insulina exógena a pacientes con diabetes de tipo II o
para tratar diabetes no insulinodependiente. Adecuadamente, las
formulaciones de esta invención se administran a seres humanos a
una dosis de insulina, que varía de individuo a individuo, tal que
los niveles de glucosa en sangre se ajustan para permanecer dentro
de los intervalos normales. Las formulaciones orales se diseñan
preferiblemente como productos de 4 veces al día, mientras que las
formulaciones parenterales que tienen típicamente \geq 100 UI de
insulina biodisponible por ml pueden administrarse una o dos veces
al día hasta una vez a la semana.
La Figura 1 muestra el mecanismo de la ruta para
el método de polimerización aniónica de formación de partículas de
poli(cianoacrilato de alquilo);
la Figura 2 muestra los niveles de glucosa en
sangre durante 6 horas expresados como porcentaje de los valores de
línea base para cuatro grupos de ratas Wistar: control (n= 4; ratas
no tratadas); solución de insulina (n= 4; solución que contiene 2,5
UI de insulina administrada mediante inyección IM a cada rata a
tiempo 0); pH= 2 (n= 4; suspensión acuosa de nanopartículas de
poli(cianoacrilato de alquilo) cargadas con insulina (16 UI)
producidas a pH= 2, administrada mediante inyección IM a cada rata
a tiempo 0); y pH= 4,8 (n= 3; suspensión acuosa de nanopartículas
de poli(cianoacrilato de alquilo) cargadas con insulina (16
UI) producidas a pH 4,8, administrada mediante inyección IM a cada
rata a tiempo 0); y
la Figura 3 muestra los niveles de glucosa en
sangre durante 4 horas expresados como porcentaje de los valores de
línea base para dos grupos de ratas Wistar macho: control (n= 8;
ratas no tratadas) y estudio (n= 5; suspensión acuosa de
nanopartículas de poli(cianoacrilato de alquilo) cargadas con
insulina (204 UI) producidas a pH= 2, administrada por vía oral a
cada rata a tiempo 0).
En los siguientes ejemplos, se donó cianoacrilato
de n-butilo por Loctite Ltd. (Irlanda). Se
obtuvieron cianoacrilato de isobutilo, dextrano 10, 40 y 70 e
insulina (bovina) de Sigma. La morfología de superficie de las
nanopartículas producidas se estudió mediante microscopía de
barrido electrónico (MBE) (Leica Cambridge S360). La carga de
fármaco se monitorizó mediante el método de residuos utilizando
espectroscopía UV y mediante análisis de HPLC con gradiente según
la USP XXII. Se utilizaron ratas macho Wistar en los estudios in
vitro y los niveles de glucosa en sangre se monitorizaron
mediante el sistema Ames™ Glucostix.
Se produjeron nanopartículas de
poli(cianoacrilato de alquilo) biodegradables a partir de
cianoacrilato de n- e isobutilo utilizando el método de
polimerización aniónica en condiciones estándar. Por ejemplo, para
una carga de fármaco inicial de 20%, se disolvieron 100 mg de
insulina en 49,5 ml de una solución de HCl 0,01 M que contenía 0,5%
de dextrano 70 (como estabilizante) antes de la adición del
monómero. Después de añadir el monómero, por ejemplo 0,5 ml de
cianoacrilato de isobutilo, la solución se agitó durante 4 horas, se
filtró o se centrifugó y se secó durante una noche. No se observó
diferencia en la morfología de las partículas formadas a partir de
cianoacrilato de n- o isobutilo en estas condiciones. La superficie
de las partículas producidas era lisa y libre de fármaco cristalino
y las partículas eran de 100 nm - 400 nm de diámetro. Para una
carga inicial de 20% p/p, se obtuvieron valores de carga de
insulina de 17,82% para cianoacrilato de n-butilo y
17,97% para cianoacrilato de isobutilo. Por tanto, la formación de
nanopartículas cargadas de insulina no se influenció por la
elección de estos monómeros.
Se produjeron nanopartículas utilizando el método
descrito en el ejemplo 1 para estudiar el efecto de variar el peso
molecular del estabilizador dextrano (utilizando dextrano 10,
dextrano 40 y dextrano 70, M_{w}= 10.000, 40.000 y 70.000,
respectivamente) a una concentración 0,5% p/v sobre la formación de
nanopartículas. Adicionalmente, utilizando dextrano 70, la
concentración de dextrano se varió también de 0,05 a 1% p/v. Como
se muestra en la tabla 1, no se observó diferencia en el tamaño y
morfología de las nanopartículas ni en la eficacia de retención de
insulina cuando se variaron el peso molecular o la concentración
del dextrano. Este resultado para nanopartículas de
poli(cianoacrilato de alquilo) cargadas con insulina es
contrario al reseñado para nanopartículas de
poli(cianoacrilato de alquilo) vacías (Douglas, S. et
al., J. Colloid Interface Science, 103:
154-163 (1985).

Se produjeron nanopartículas de cianoacrilato de
n-butilo cargadas con insulina utilizando el método
descrito en el ejemplo 1 para estudiar el efecto de variar el pH
del medio de polimerización de 1 a 4,8 sobre la formación de
nanopartículas. A medida que el pH del medio de polimerización
aumentó por encima de 3, se produjeron distintas partículas
definidas que tenían superficies lisas exentas de fármaco
cristalino. Sin embargo, por debajo de pH 3, la formación de
nanopartículas distintas parecía estar limitada, mostrando sólo
pequeñas cantidades de nanopartículas bien formadas. A este bajo pH,
la formación de partículas ocurre rápidamente, dando como resultado
que la mayoría de las nanopartículas no están bien formadas.
La experimentación adicional muestra que resulta
un precipitado blanco espumoso cuando se disuelve en primer lugar
la insulina, a 10% y 20% p/v de contenido de monómero, en una
solución de HCl 0,1 M que contiene 0,5% p/v de dextrano 70 a la que
se añaden 0,25 ml de monómero cianoacrilato de isobutilo. El
precipitado puede recogerse mediante filtración y secarse,
proporcionando un polvo blanco fino (100-400 nm).
Esta reacción no ocurre en ausencia de insulina en la solución de
HCl o si la insulina se añade después del monómero
cianoacrilato.
Sin limitarse a explicación teórica alguna, se
sugiere que estas observaciones pueden explicarse,
retrospectivamente, por referencia al mecanismo mediante el que
ocurre la polimerización aniónica y las propiedades fisicoquímicas
de la insulina. Como se muestra en la Figura 1, los iones hidroxilo
presentes en el medio acuoso de polimerización inician la
polimerización. Debido a que la insulina es un péptido con un punto
isoeléctrico de 5,3, a un pH inferior a 5,3 está cargada
positivamente, mientras que a un pH superior a 5,3 la insulina
puede estar cargada negativamente. A pH bajo (pH \leq 2). La
molécula de insulina tiene una carga neta de +6, y puede complejar
con las cadenas cortas de cianoacrilato cargadas negativamente,
dando como resultado la terminación del proceso de polimerización.
A medida que el pH del medio de polimerización aumenta a 5, la
carga neta de la insulina se aproxima a cero, y por tanto, la
complejación con las cadenas cortas de cianoacrilato es improbable.
A este pH, se producen nanopartículas cargadas de insulina bien
formadas.
El análisis por HPLC del contenido de insulina de
las nanopartículas producidas a pH 2 no proporcionó picos de
insulina al tiempo de retención de insulina esperado. Sin embargo,
el análisis UV (mediante el método de residuos) indicó \geq 80%
de retención de fármaco. El análisis por HPLC de las nanopartículas
producidas a pH 4,8 mostró la presencia de insulina al tiempo de
retención esperado de 19 minutos. Estas observaciones apoyan la
conclusión de que, a pH bajo, la insulina se compleja con el
polímero y no se eluye al tiempo de retención esperado para la
insulina libre, mientras que a pH 4,8 la insulina libre se
incorpora a las nanopartículas y se eluye al tiempo de retención
esperado.
Para apoyar esta conclusión, se investigó la
formación de nanopartículas vacías en el mismo intervalo de pH para
establecer si la presencia de insulina inhibe la formación de
nanopartículas a pH bajo. Se produjeron nanopartículas a pH 2, 3 y
4 en las mismas condiciones que las empleadas para las
nanopartículas cargadas con insulina. En todos los casos, las
nanopartículas producidas fueron lisas, esféricas y de tamaño
uniforme. Por tanto, a pH bajo, la presencia de insulina en el
medio de polimerización interfiere con la formación de
nanopartículas.
Se examinaron la bioactividad y las propiedades
de liberación de las nanopartículas de cianoacrilato de
n-butilo cargadas con insulina producidas según el
ejemplo 3 mediante administración intramuscular (IM) a ratas Wistar
macho. Las nanopartículas cargadas con insulina producidas a pH 2
(16 UI) y a pH 4,8 (16 UI) se administraron en suspensión acuosa
mediante inyección IM a dos grupos de ratas, y se monitorizaron los
niveles de glucosa en sangre durante un periodo de 6 horas. Se
administró una solución de insulina pura (2,5 UI) mediante
inyección IM a un tercer grupo de ratas para proporcionar una
comparación entre el suministro mediante las formulaciones de
nanopartículas y la insulina no encapsulada. Se monitorizaron
también los niveles de glucosa en sangre de ratas no tratadas
(controles) durante el mismo periodo de tiempo.
La Figura 2 muestra los niveles de glucosa en
sangre para los grupos a pH 2, pH 4,8, control y no tratado
expresados como porcentaje de los valores de línea base. Los
niveles de glucosa en sangre en el grupo no tratado fueron de 87,6
a 108,9% del valor de línea base en el periodo de estudio. La
administración de las nanopartículas dio como resultado una
reducción constante de los niveles de glucosa en sangre del 20% del
valor de línea base, indicando que la insulina de ambos tipos de
nanopartículas (complejadas a pH 2; insulina libre a pH 4,8) es
bioactiva y se libera de manera sostenida controlada. La reducción
del nivel de glucosa en sangre fue más rápida para las
nanopartículas producidas a pH 4,8 que para las producidas a pH 2,
presumiblemente debido a la liberación más rápida de insulina libre
de la formulación a pH 4,8 en contraposición con la liberación más
lenta de insulina complejada con poli(cianoacrilato de
alquilo) en la formulación a pH 2.
Se examinaron la bioactividad y las propiedades
de liberación de las nanopartículas de cianoacrilato de
n-butilo cargadas con insulina producidas según el
ejemplo 3 mediante administración oral a ratas Wistar macho. Las
nanopartículas cargadas con insulina producidas a pH 2 se
administraron por vía oral en una suspensión acuosa a un grupo de
ratas, y se monitorizaron los niveles de glucosa en sangre durante
un periodo de cuatro horas. La dosis administrada por rata era de
204 UI de insulina. Los niveles de glucosa en sangre de las ratas no
tratadas se monitorizaron también durante el mismo periodo de
tiempo.
La Figura 3 muestra los niveles de glucosa en
sangre para estos dos grupos expresados como porcentaje de los
valores de línea base. Los niveles de glucosa en sangre en ratas no
tratadas fueron entre 85 y 100% del valor de línea base durante el
periodo de estudio. En las ratas tratadas, se observó una reducción
de los niveles de glucosa en sangre del 35% del valor de línea base
durante un periodo de cuatro horas, proporcionando una
biodisponibilidad oral de \geq 7% para estas nanopartículas
cargadas con insulina. Estos resultados indican que la insulina de
nanopartículas producidas a pH 2 (complejadas con el
poli(cianoacrilato de alquilo) es bioactiva y se libera de
modo sostenido controlado.
Claims (10)
1. Una formulación farmacéutica de liberación
controlada, que comprende insulina retenida en un polímero de
poli(cianoacrilato de alquilo) biodegradable formando
nanopartículas, en la que la insulina está complejada con el
poli(cianoacrilato de alquilo).
2. Una formulación farmacéutica de liberación
controlada según la reivindicación 1, en la que el
poli(cianoacrilato de alquilo) se forma a partir de
monómeros de cianoacrilato de n-butilo,
cianoacrilato de isobutilo o cianoacrilato de isohexilo o mezclas
de los mismos.
3. Una formulación farmacéutica de liberación
controlada según una cualquiera de las reivindicaciones
precedentes, en la que la carga de insulina está en el intervalo de
aproximadamente 5% a 30% p/v del contenido de monómero.
4. Una formulación farmacéutica de liberación
controlada según una cualquiera de las reivindicaciones
precedentes, que comprende además nanopartículas de
poli(cianoacrilato de alquilo) cargadas con insulina libre
formando una mezcla de nanopartículas cargadas con insulina
complejada con poli(cianoacrilato de alquilo) y
nanopartículas de poli(cianoacrilato de alquilo) cargadas
con insulina libre.
5. Una formulación farmacéutica de liberación
controlada según una cualquiera de las reivindicaciones
precedentes, que comprende además un recubrimiento entérico sobre
las nanopartículas cargadas con insulina complejada con
poli(cianocrilato de alquilo) para dirigir la liberación de
insulina al intestino delgado cuando se administra por vía
oral.
6. Una formulación farmacéutica de liberación
controlada según la reivindicación 4 ó 5, que comprende además un
recubrimiento entérico sobre las nanopartículas de
poli(cianoacrilato de alquilo) cargadas con insulina libre
para dirigir la liberación de insulina al intestino delgado cuando
se administra por vía oral.
7. Una formulación farmacéutica de liberación
controlada según una cualquiera de las reivindicaciones
precedentes, en la que la formulación se formula en forma de
cápsulas, comprimidos, polvos, polvos capaces de efervescencia
después de la adición de agua, o suspensiones.
8. Una formulación farmacéutica de liberación
controlada según la reivindicación 7, en la que la formulación se
formula en forma de comprimidos y comprende además un recubrimiento
entérico sobre el comprimido para dirigir la liberación de insulina
al intestino delgado cuando se administra por vía oral.
9. Una formulación farmacéutica de liberación
controlada según una cualquiera de las reivindicaciones precedentes
y adaptada para administración oral.
10. Una formulación farmacéutica de liberación
controlada según una cualquiera de las reivindicaciones
1-4 y 7, y adaptada para administración
parenteral.
