Description
Foreliggende oppfinnelse angår nye nukleosidmonofosfatderivater av lipidesterrester med generell formel (I)
hvori
R<1> representerer en rettkjedet eller forgrenet, mettet
alkylkjede med 1-20 karbonatomer;
R<2> representerer en rettkjedet eller forgrenet, mettet
alkylkjede med 1-20 karbonatomer;
R<3> representerer hydrogen eller en hydroksygruppe;
R<4> representerer en hydroksygruppe;
R<5> representerer hydrogen eller en hydroksygruppe;
X representerer et svovelatom eller en sulfonylgruppe;
Y representerer et oksygenatom;
B representerer en purin- og/eller pyrimidinbase;
med formel
hvor R6 er H eller halogen og R10 er SH eller C^-Cg-alkyltio; forutsatt at minst én av restene R<3> eller R<5> er hydrogen;
samt deres tautomerer, deres optisk aktive former og racemiske blandinger, og deres fysiologisk akseptable salter av uorganiske og organiske syrer eller baser.
Fordi disse forbindelser med generell formel I inneholder asymmetrisk karbonatomer, angår foreliggende oppfinnelse likeledes alle de optisk aktive former og racemiske blandinger av forbindelsene.
J. Bio. Chem. 256, 6112 (1990 og EP 0 350 287 beskriver fremstilling og anvendelse av liponukleotider som antiviruslegemidler. I disse publikasjoner er imidlertid kun dimyristoylfosfatidyl- og dipalmitoylfosfatidylrester koblet til velkjente nukleosider som AZT (azidotymidin) og ddC (2',3'-dideoksycytidin) undersøkt og syntetisert, inkludert deres fettsyreesterstruktur.
J. Med. Chem. 33, 1380 (1990) beskriver nukleosid-konjugater av tioeterlipider med cytidindifosfat, hvilke har antitumoraktivitet og kan finne anvendelse i onkologi. Chem. Pharm. Bull. 36, 209 (1988) beskriver 51 -(3-sn-fosfatidyl)-nukleosider med antileukemisk aktivitet, så vel som deres enzymatiske syntese fra de tilsvarende nukleosider og fosfo-choliner i nærvær av fosfolipase D med transferaseaktivitet. J. Med. Chem. 34, 1408 (1991) beskriver likeledes nukleosid-konjugater med anti-HIV-l-aktivitet som er substituert med metoksy eller etoksy i sn-2-posisjonen av lipiddelen. Patent-søknad WO 92/03462 beskriver tioeter-lipidkonjugater med anti-virusaktivitet, spesielt for behandling av HIV-infeksjoner.
Forbindelsene ifølge foreliggende oppfinnelse har verdifulle, farmakologiske egenskaper. Nærmere bestemt er de egnet ved terapi og profylakse av maligne tumorer, slik som tumorterapi av ondartede tilstander, neoplasmer, karsinomer, sarkomer eller leukemier. Forbindelsene oppviser dessuten immunundertrykkende aktivitet og de kan derfor anvendes ved terapi av organspesifikke eller generaliserte autoimmun-sykdommer, slik som reumatoid artritt, systemisk lupus erythematosus, kronisk transplantat vs. vertsykdom, multippel sklerose etc, eller ved forebyggelse av allogen eller semi-allogen transplantatavstøtning, f.eks. nyrer, lever, lunger, hjerte etc. Forbindelsene har videre antivirus-, antiretro-virus- eller antiankogenisk aktivitet og de er således egnede ved profylakse og terapi av virus- og onkogenisk indu-serte/forårsakede sykdommer (slik som AIDS etc). Sammenlignet med hittil anvendte forbindelser for behandling av maligne tumorer har forbindelsene ifølge foreliggende oppfinnelse for-høyet effektivitet eller lavere toksisitet, og har således en bredere terapeutisk rekkevidde. Av denne årsaker er de fordel-aktige ved at administreringen av legemidler inneholdende disse forbindelser kan utføres kontinuerlig over en forlenget tidsperiode, og tilbaketrekning av preparatet eller avbrudd i administreringen, som ofte har vært rutinemessig med hittil anvendte cytostatiske midler i tumorterapi, eller som på grunn av deres uønskede bivirkninger har vært nødvendig, kan unngås.
Forbindelsene ifølge foreliggende oppfinnelse lider ikke av disse ulemper. Deres virkninger er immunundertrykkende eller antitumoraktive, uten å være uspesifikk cytotoksiske i farmakologisk relevante doser.
Forbindelsene ifølge foreliggende oppfinnelse og deres farmasøytiske formuleringer kan likeledes anvendes i kombinasjon med andre legemidler for behandling og profylakse av de ovennevnte sykdommer. Eksempler på disse ytterligere legemidler involverer midler som f.eks. mitoseinhibitorer, slik som colchicin, mitopodizid, vinblastin, alkylerende cytostatiske midler som syklofosfamid, melfalan, myleran eller cisplatin, antimetabolitter som folsyreantagonister (meto-trexat) og antagonister av purin- og pyrimidinbaser (merkaptopurin, 5-fluoruridin, cytarabin), cytostatisk aktive antibiotika, slik som antrasykliner (f.eks. doxorubicin, daunorubicin), hormoner som fosfosterol, tamoxifen, andre cytostatisk/cytotoksisk aktive kjemoterapeutiske midler og andre immunundertrykkende legemidler (slik som syklosporiner, FK 506, rapamyciner, desoksypergulain etc.).
Mulige salter av forbindelser med generell formel I er fremfor alt alkali-, jordalkali- og ammoniumsaltene av fosfatgruppen. Foretrukne som alkalisalter er litium-, natrium-og kaliumsalter. Mulige som jordalkalisalter er spesielt magnesium- og kalsiumsalter. I overensstemmelse med foreliggende oppfinnelse er ammoniumsalter underforstått å være de som inneholder ammoniumionet som kan være substituert opp til fire ganger med alkylrester med 1-4 karbonatomer, og/eller aralkylrester, fortrinnsvis benzylrester. Substituentene kan her være like eller forskjellige.
Forbindelsene med generell formel I kan inneholde basiske grupper, spesielt aminogrupper, som kan være omdannet til syreaddisjonssalter ved hjelp av egnede uorganiske eller organiske syrer. For dette formål er mulige syrer spesielt: Saltsyre, hydrogenbromid, svovelsyre, fosforsyre, fumarsyre, ravsyre, vinsyre, sitronsyre, melkesyre, maleinsyre eller metansulfonsyre.
I den generelle formel I representerer R<1> fortrinnsvis en rettkjedet C8-C15-alkylgruppe. Mer spesifikt representerer R<1 >en nonyl-, decyl-, undecyl-, dodecyl-, tridecyl- eller tetradecylgruppe.
R<2> representerer fortrinnsvis en rettkjedet C8-C15-alkylgruppe. Mer spesifikt representerer R<2> en oktyl, nonyl-, decyl-, undecyl-, dodecyl-, tridecyl- eller tetradecylgruppe.
Spesielt foretrukne er forbindelser hvori R<5 >representerer et hydrogenatom og R3 og R<4> er hydroksy.
De følgende nukleosider er fortrinnsvis egnede som koblingskomponenter for å fremstille lipidnukleotidkonjugatene med formel (I):

Forbindelsene med generell formel (I) kan fremstilles ved

i nærvær av et aktiverende syreklorid, slik som 2,4,6-triisopropylbenzensulfonsyreklorid, og en tertiær nitro-genbase, f.eks. pyridin eller lutidin, i et inert løsnings-middel som toluen, eller umiddelbart i vannfritt pyridin, og eventuelt etter hydrolyse fjerning av de oksygenbeskyttende grupper i overensstemmelse med konvensjonelle prosedyrer i nukleosidkjemien, eller
2. omsetning av en forbindelse med generell formel

hvori R<1>, R<2>, X og Y har de ovennevnte betydninger, med en forbindelse med generell formel VI, hvori R<3>, R<4>, R5 og B har de ovennevnte betydninger, i nærvær av fosfolipase D fra Streptomyces, i et inert løsningsmiddel som kloroform, i nærvær av en egnet buffer, og eventuelt etter reaksjonen fjerning av de oksygenbeskyttende grupper i overensstemmelse med konvensjonelle prosedyrer i nukleosidkjemien.
Fremstillingen av forbindelsene med generell formel V og VII utføres på analog måte som beskrevet i Lipids 22, 947
(1987) og J. Med. Chem. 34, 1377 (1991).
Fremstillingen av forbindelsene med generell formel VI er f.eks. beskrevet i EP-A-0 286 028 og WO 90/08147. Noen av de inkluderte nukleosider er kommersielt tilgjengelige forbindelser.
Forbindelser lignende formel I er beskrevet i EP-A-
0 350 287. De tilsvarende 1,2-diestere av glyserol er beskrevet deri.
Legemidlene inneholdende forbindelser med formel I for behandling av virusinfeksjoner kan appliseres i flytende eller fast form intestinalt eller parenteralt. De vanlige appliseringsformer er her mulige, slik som tabletter, kapsler, belagte tabletter, siruper, løsninger eller suspensjoner. Vann anvendes fortrinnsvis som injeksjonsmedium inneholdende additiver, slik som stabilisatorer, løsningsmidler og buffere som er vanlige i injeksjonsløsninger. Slike additiver er f.eks. tartrat- og sitratbuffere, etanol, kompleksdannende midler som etylendiamintetraeddiksyre og dens ikke-toksiske salter, høymolekylære polymerer som flytende polyetylenoksid for viskositetskontroll. Flytende vehikler for injeksjonsløsninger må være sterile og fylles fortrinnsvis i ampuller. Faste bærere er f.eks. stivelse, laktose, mannitol, metylcellulose, talkum, høydispergerte kiselsyrer, høymolekylære fettsyrer som stearinsyre, gelatin, agar-agar, kalsiumfosfat, magnesiumstearat, dyre- og plantefett, faste høymolekylære polymerer som polyetylenglykol etc. Hvis ønskelig kan formuleringer egnede for oral applisering inkludere smaksmidler eller søtningsmidler.
Doseringen kan avhenge av forskjellige faktorer, slik som appliseringsmåte, art, alder eller individuell tilstand. Forbindelsene ifølge foreliggende oppfinnelse appliseres egnet i mengder på 0,1-100 mg, fortrinnsvis 0,2-80 mg pr. dag pr. kg kroppsvekt. Det er foretrukket å dele den daglige dose i 2-5 appliseringer, idet tabletter med et innhold av aktiv bestanddel på 0,5-500 mg administreres med hver applisering. Tablettene kan likeledes ha vedvarende frigivelse, hvilket reduserer antallet appliseringer til 1-3 pr. dag. Innholdet av aktiv bestanddel i tablettene med vedvarende frigivelse kan være 2-1 000 mg. Den aktive bestanddel kan også administreres ved kontinuerlige infusjoner, hvor mengder på 5-1 000 mg pr. dag vanligvis er tilstrekkelig.
Eksempel 1
( 5- fluoruridin)- 5'- fosforsvre-( 3- dodecvlmerkapto- 2- decyloksy)-propylester
3,6 g (6,1 mmol) fosforsyre-(3-dodecylmerkapto-2-decyloksy)propylester ble behandlet to ganger med 3 0 ml vannfri pyridin og konsentrert ved fordamping. Resten ble oppløst i 30 ml vannfri pyridin, behandlet med 2,76 g (9,1 mmol) 2,4,6-triisopropylbenzensulfonsyreklorid under nitrogen og omrørt ved romtemperatur i 3 0 minutter. Deretter ble 1,60 g (6,1 mmol) 5-fluoruridin (Fluka) tilsatt, og blandingen ble hensatt under N2 i 24 timer.
Hydrolyse ble utført ved anvendelse av 15 ml vann, blandingen ble omrørt i ytterligere 2 timer ved romtemperatur, løsningsmiddel ble fjernet under vakuum og resten ble bort-skrapet to ganger ved anvendelse av en liten mengde toluen. Resten ble renset ved kolonnekromatografi på "LiChroprep" RP-18 med en lineær gradient av metanol/vann 7/1 til metanol som elueringsmiddel. Utbyttet ble 3,1 g (69 % av den teoretiske mengde); olje. Rf = 0,24 (CH2Cl2/MeOH 8/2); R2 = 0,55 (CH2Cl2/MeOH/H20 6,5/2,5/0,4) på Merck 5715 TLC-plater, kiselgel 60 F.
Fosforsyre-(3-dodecylmerkapto-2-decyloksy)propyl-esteren ble fremstilt som beskrevet i WO 92/03462.
Eksempel 2
( 6- merkaptopurin- 9- p- D- ribofuranosid) - 5 ' - fosforsvre- ( 3-dodecvlmerkapto- 2- decvloksy) propylester
6,2 g (12,5 mmol) fosforsyre-(3-dodecylmerkapto-2-decyloksy)propylester ble behandlet med 5,7 g (18,75 mmol) 2,4,6-triisopropylbenzensulfonsyreklorid som beskrevet i eksempel 1, og deretter med 3,55 g (11,25 mmol) 6-merkaptopurin-9-p-D-ribofuranosid, og etter 24 timer ble denne blanding hydrolysert med vann.
2,85 g kalsiumacetat i 15 ml vann ble deretter lang-somt tildryppet blandingen, hvilket utfelte råkalsiumsaltet av konjugatet. Etter langvarig omrøring av bunnfallet med aceton (1/10), ble det erholdt 6 g av et amorft råprodukt med
72 areal% ifølge HPLC.
Kalsiumsaltet ble suspendert i 350 ml metanol, behandlet med 150 g Amberlite IR 120 i Na<+->form og omrørt i 2 dager.
Ionebytteren ble deretter fjernet, filtratet ble inndampet og resten ble renset ved kolonnekromatografi på "LiChroprep" RP-18 med en lineær gradient av metanol/vann 5/1 til 9/1. Fraksjonene inneholdende produktet ble inndampet i vakuum og resten ble omrørt med aceton og tørket. Utbytte: 3,52 g (41 % av teoretisk mengde).
DC: Rf = 0,45 (isopropanol/butylacetat/konsentrert ammoniakk/vann 50/30/5/15.
Eksempel 3
( 6- merkaptopurin- 9-( 3- D- ribofuranosid) - 5 ' - fosforsvre- ( 3-dodecvlmerkapto- 2- decvloksy) propylester- natriumsalt
Analogt med eksempel 2 ble 41,4 g fosforsyre-(3-dodecylmerkapto-2-decyloksy)propylester i 400 ml vannfri pyridin omsatt med 42,9 g 2, 4, 6-triisopropylbenzensulfonsyreklorid og deretter med 23,7 g 6-merkaptopurin-9-p-D-ribofuranosid. Råkalsiumsaltet som ble filtrert ved avsuging etter hydrolyse og utfelling ved 25 g kalsiumacetat i 160 ml vann, ble fordelt mellom 500 ml MTB og 250 ml 2 N HC1, og omrørt inntil det var fullstendig oppløst i den organiske fase. Den organiske fase ble separert, vasket med mettet natriumklorid-løsning og konsentrert i en rotasjonsfordamper. Resten ble applisert på 80 g "LiChroprep" RP-18 (MTB-løsning av råprodukt behandles med RP-18, inndampes og tørkes), og separert porsjonsvis i en prekolonne på RP-18. En blanding av 3,7 1 metanol, 400 ml vann, 3 ml iseddik og 2 g natriumacetat tjente hver gang som elueringsmiddel. Fraksjonene inneholdende produktet ble kombinert, den ønskede forbindelse ble utfelt ved tilsetning av 20 g kalsiumacetat i 100 ml vann og filtrert ved avsuging. Utbytte: 32 g (43 % av teoretisk mengde).
Kalsiumsaltet ble suspendert i 250 ml MTB, ekstrahert med 80 ml 2 N HC1 under omrøring og den organiske fase ble vasket to ganger med mettet natriumkloridløsning. Etter fjerning av løsningsmidlet ble resten oppløst i 200 ml toluen og justert til pH 7 mot en Friskolyt-elektrode med 30 % natriummetylatløsning. Natriumsaltet ble utfelt ved omrøring i 200 ml aceton, filtrert ved avsuging og tørket i en vakuum-tørkeovn. Utbytte: 29 g (37 % av teoretisk mengde).
Rf = verdi: 0,18 (kiselgel; elueringsmiddel: iso-propanol/butylacetat/vann/konsentrert ammoniakk 50/30/15/5).
Eksempel 4
( 6- merkaptopurin- 9-[ 3- D- ribofuranosid) - 5' - fosforsvre- ( 3-dodecylmerkapto- 2- decvloksv) propylester- natriumsalt
Analogt med eksempel 3 ble det urensede konjugat fremstilt fra 40 g 6-merkaptopurin-9-p-D-ribofuranosid. Råproduktet ble renset ved kolonnekromatografi ved applisering av 8 g hver gang på en kolonne med diol-fase (diameter 4 cm; lengde 25 cm) (påvisning ved 254 nm; elueringsmiddel: metanol/MTB 10/4) . Den appliserte prøve var helt klart oppløst i elueringsmidlet. De produktinneholdende fraksjoner fra de forskjellige separeringer ble kombinert, fordampet og utfelt som natriumsaltet fra toluen og aceton, som beskrevet i eksempel 3. Utbytte: 64,5 g (51 % av teoretisk mengde).
Rf-verdi: 0,85 (DIOL-fase; elueringsmiddel: metanol).
Eksempel 5
( 6- metvlmerkaptopurin- 9- p- D- ribofuranosid)- 5'- fosforsvre-( 3-dodecylmerkapto- 2- decvloksv) propylester- natriumsalt
Analogt med eksempel 1 ble 14,9 g 6-metylmerkapto-purin-9-p-D-ribofuranosid (50 mmol) omsatt med det blandede anhydrid fremstilt fra 27,3 g fosforsyre-(3-dodecylmerkapto-2-decyloksy)propylester og 25 g 2,4,6-triisopropylbenzensulfonsyreklorid i 2 50 ml vannfri pyridin, hydrolysert og konsentrert ved fordamping. Analogt med eksempel 3 ble råproduktet (HPLC: 67 areal%) renset ved kromatografi på RP-18, utfelt som kalsiumsalt og omdannet til natriumsaltet. Utbytte: 15,2 g
(38 % av den teoretiske mengde).
Rf-verdi: 0,22 (kiselgel; elueringsmiddel: iso-propanol/butylacetat/vann/konsentrert ammoniakk 50/3 0/15/5).
Eksempel 6
( 5- fluoruridin)- 5'- fosforsvre-( 3- dodecvlmerkapto- 2- decvloksy) - propylester- natriumsalt
Analogt med eksempel 1 ble 50 g 5-fluoruridin omdannet til råkonjugatet, utfelt som kalsiumsalt som beskrevet i eksempel 3, og etter omdannelse til den frie syre ble det som råprodukt renset ved kromatografi, analogt med eksempel 4, på en DIOL-fase ved anvendelse av metanol/MTB 10/4 som elueringsmiddel. Natriumsaltet fremstilt som i eksempel 3 ble isolert med et utbytte på 69 %.
Rf-verdi: 0,3 5 (DIOL-plater; elueringsmiddel: metanol/MTB 10/4).
Eksempel 7
ICcn- verdier ( fxq/ ml) for azatioprin, 6- merkaptopurin ( 6- MP) , 6-merkaptopurinribosid, BM 92 072 9 og doxorubicin i CFU- E- og CFU- GM- analyser
Den etterfølgende tabell viser IC50-verdiene (ug/ml) for azatioprin, 6-merkaptopurin (6-MP) og 6-merkaptopurinribosid, sammenlignet med 6-merkaptopurinribosineter-lipid-konjugatet BM 92 0729, for in vitro-cytotoksisitet overfor musebrystbenmargceller, inkludert kolonidannende enheter/erytrocytter (CFU-E) og kolonidannende enheter/granul-ocytter-makrofager (CFU-GM). Den cytostatiske/cytotoksiske forbindelse doxorubicin ble også inkludert som referanseforbindelse. Alle forbindelser ble testet i 3-6 forskjellige eksperimenter konsentrasjonsavhengige av i det minste dobbelte eller tredobbelte inkubasjoner pr. testede konsentrasjon.
Som det fremgår fra resultatene blir BM 92 0700 mye bedre tolerert av brystbenmargcellene sammenlignet med alle andre testede forbindelser, spesielt sammenlignet med 6-merkaptopurinribosid.

Eksempel 8
Benmargstoksisitet for BM 92 0729, azatioprin, 6- merkaptopurin og 6- merkaptopurinribosid hos Balb/ c- hunnmus: dag + 4 ( eksperiment 93 074 0)
Eksperiment 93 074 0 viser benmargstoksisiteten for BM 92 0792, azatioprin, 6-merkaptopurin og 6-merkaptopurinribosid in vivo hos Balb/c-hunnmus som ble behandlet én gang pr. dag p.o. i fire etterfølgende dager (dag 0-dag +3). Dyrene ble avlivet på dag +4 og benmargcellularitet (celler/femur) ble bestemt. Resultatene viser ingen benmargstoksisitet for 6-merkaptopurinribosid-eter-lipidkonjugatet BM 92 0729 opp til den høyeste testede dose, dvs. 100 mg kg"<1> dag'<1>, hvilket til-svarer på en molar basis 3 0 mg kg"<1> dag"<1> 6-merkaptopurinribosid. Denne sistnevnte forbindelse viser, i motsetning til eterlipidkonjugatet BM 92 0792, en klar doseavhengig reduksjon av benmargcellularitet. Det samme funn ble erholdt for de andre forbindelser, inkludert azatioprin og 6-merkaptopurin.

Eksperiment 94 0026 er et eksperiment som hadde som mål å reprodusere resultatene erholdt i eksperiment 93 074 0 (eksempel 8). I dette eksperiment ble syklosporin A også inkludert som en referanseforbindelse. Utfallet av eksperiment 940026 bekreftet resultatene erholdt i eksperiment 930740 in vivo.

Tabellen vist nedenfor gir de gjennomsnittlige IC50-verdier for 5-fluoruridin (5-FU) og 5-FU-eterlipidkonjugatet BM 92 0700 for benmargstoksisitet in vitro i CFU-E- og CFU-GM-analyser. Analysebetingelser er beskrevet i eksempel 7.
Dataene viser at eterlipidkonjugatet av 5-fluoruridin BM 92 0700 er hhv. 610 og 238 ganger mindre toksisk overfor erytrocytt- og granulocytt/makrofag-benmargsstamceller sammenlignet med 5-FU.

Eksempel 11
Påvirkning av 5- FU- eterlipidkonjugatet BM 92 07 00 ( figur 1) og av 5- FU ( figur 2) på L 1210- leukemi in vivo: Overlevelsestid
Mus ble inokulert med L 1210-leukemiceller på dag 0 (n = 10 dyr/gruppe) og ble deretter behandlet én gang daglig i.p. fra dag 0 (+lh) - dag +41 (6 uker), med de ukentlige sykluser angitt på hhv. figurene 1 og 2.
Fra overlevelseskurvene fra kontroll- og behandlings-gruppene vist på figur 2 fremgår det at 5-FU har, som rapportert i litteraturen, en meget smal dose-virkningsprofil, dvs. at økning av dosen fra f. eks. 2 x 10/5 x 0,1 mg kg"<1> dag"<1 >til 2 x 10/5 x 0,3 mg kg"<1> dag"<1>, eller til enda høyere doser, fører til reduserte overlevelsesverdier.
I motsetning til dette ble det med 5-FU-eterlipidkonjugatet BM 92 0700 erholdt en klar doseavhengig økning i overlevelsestiden sammenlignet med kontroll I og II (figur 1), hvilket indikerer at ekvimolare doser av BM 92 0700 er åpenbart mer effektive i denne leukemimodell sammenlignet med standardforbindelsen 5-FU.
Tatt i betraktning at BM 92 0700 er mer effektiv (figurene 1 og 2) og mye mindre toksisk overfor benmargsceller, kan det konkluderes med at BM 92 0700 har en mye høyere terapeutisk indeks/forhold sammenlignet med det cytostatiske standardmiddel 5-FU.

1. Nukleosidmonofosfatderivater, karakterisert ved at de har formel (I)
hvori

R<2> representerer en rettkjedet eller forgrenet, mettet
alkylkjede med 1-20 karbonatomer;
R<3> representerer hydrogen eller en hydroksygruppe;
R<4> representerer en hydroksygruppe;
R<5> representerer hydrogen eller en hydroksygruppe;
X representerer et svovelatom eller en sulfonylgruppe;
Y representerer et oksygenatom;
B representerer en purin- og/eller pyrimidinbase;
med formel
hvor R6 er H eller halogen og R<10> er SH eller C^-Cg-alkyltio; forutsatt at minst én av restene R<3> eller R<5> er hydrogen;
samt deres tautomerer, deres optisk aktive former og racemiske blandinger, og deres fysiologisk akseptable salter av uorganiske og organiske syrer eller baser.
