[産業上の利用分野]
本発明はヒトの一定の疾患の治療様式としての養子免疫
療法(adoptive iramunothera
py )に関する。特に、本発明はインターロイキン2
の投与と共にリンフォカイン活性化キラー細胞を使用す
るヒトの癌および他の免疫不全症または状態を治療する
方法に関する。 [従来の技術] 過去二十年の間、lI!瘍に応答する宿主免疫の促道に
基づいて癌治療の免疫療法を発達させる試みが行われて
きた。これらの方法は、一般的な免疫促進がそれに対陣
して宿主の抗腫瘍反応を高めることを期待して、特異的
なI!!瘍細胞に対してまたは非特異的な促進物質を用

主の免疫能力は必要でないことに留意しなければならな
い。従って、一般的にm瘍を有する状態に伴う免疫抑制
はこの治療に対しては問題とならない。宿主の免疫能力
は必要でなく、それが実際には免疫細胞の養子移転の効
果に対して有益であり得るので、養子免疫療法は容易に
化学療法や放射線療法等の他の治療法と組合せることが
できる。移転される試薬は免疫学的に特異的なので、こ

だろう。 第1図は移転された細胞のLAK(リンフォカイン活性
化キラー)活性を示す。第2号の患者(表2を参照)の
それぞれの白血球搬出から得た末梢血液単核細胞が、L
AKIIII胞を生じるようにIL−2(インターロイ
キン2)で活性化され、−足部分(al 1quots
)が凍結保存された。治療のプロトコルが完成したとき
、全ての試料は解凍され、同時に、4時間のクロム放出
試験で、新鮮なNK耐性腫1!細胞(上)およびNK!
!f応性に562細胞株(下)の両方を細胞溶解(ly
sis )する能力を試験された。 第2図は、30.000単位/kgまたは100゜00
0単位/kiJのいずれかを静脈投与した後の血清IL
−2水準(第11号の患者)を示す。静脈内ポーラス投
与の後、IL−2の血清水準は急激に落ち、ヒトの組換
え(recombinant ) I L −2の2成
分半減期と一致する。第1の成分は6−7分の半減期を
有し、第2の成分は約70分の半減期を有する。 第3図は、LAK細胞とIL−2を用いる治療を受ける
12人の患者のそれぞれの正確な治療計画を示す。II
L IIは白血球搬出の日を示す。IL−2投与の日
は白扱きの長方形で示されている。 長方形内部の数値は8時間毎に静脈投与された単位(x
105)/kaの数値である。注入されたLAK細胞の
数(X10−10)も示されている。 プロトコルは、治療法に対する患者の耐性およびプロト
コルが進行すると共にIL−2の投与量をより高くする
試みに従って、患者毎に異なっている。 第4図は、第1号の患者の皮下の転移黒色腫小節の生体
組織検査(左)を示す。皮下の転移の治療前の生体組織
検査(右)は、少数のリンパ球様細胞を含む繊維性の擬
被膜(pseudocapsule )と多形の腫I!
細胞の凝集体を示す。治療の終了の4−週後に行った同
じ患者の生体組織検査は、黒色腫の広範な凝固壊死およ
び繊維性被膜内に慢性の炎症性細胞が増加していること
を示している。このときは生存可能の(viable)
III!は見られなかった。(ヘマトキシリンおよび
エオシン;220)第5図は、第1号の患者の皮下の黒
色腫小節の後退(regr6ssion)の時間経過を
示す。LAK細胞およびIL−2の投与の終了後、顕著
な饅層の後退が見られた。治療終了後4週までに黒色腫
小面は大きさが減少し、治療後3月までに完全に消失し
た。 第6図は、第9号の患者の転移黒色腫の皮下沈着の続き
の生体組織検査(左)を示す。皮下の黒色腫の治S前の
生体組織検査〈右)は、微少の壊死を伴う黒色腫細胞の
シートと極く稀にリンパ球様細胞を示した。LAK細胞
とIL−2を用いる治療の2週後に切除された転移沈着
物の生体組織検査体は、顕1な慢性の炎症性湿潤物を示
した。 個々の壊死腫瘍細胞が残りの生存可能な腫瘍内に見られ

(右)x500゜ 第7図は、第2号の患者のL A、 K [1胞とIL
−2を用いる治療の前後の転移結腸癌の肺への成長を示
す。この患者は5か所の肺の転移病巣を持っていた。こ
こには1か所の転移病巣の成長曲線が示されている。こ
の病巣は2年間に亙って成長し続け、マイトマイシンC
による治療に反応しなかった。LAK細胞とIL−2を
用いる治療後1週問以内にこの病巣は後退し始めた。 第8図は、第7図に示した右肺の肺病巣(上左図の矢印
)の治療#後のX1m写真である。この病巣は1985
年1月10日の治療開始以前は段々と成長していた。1
1日後には小節の顕著な後退が見られた。大きな病巣の
真上および右に見られた2個の対陣の(Satelli
te )の小節は完全に後退した(第9図参照)。 第9図は、結腸癌を持つ患者(第2号)の肺転移の直線
(l 1near)断層X線図である。これらのX線写
真はLAK細胞とIL−2を用いる治療以前に右肺に2
個の転移小節(矢印)が存在したことを示している(左
)。これらの2個の肺転移は完全に後退した(右)。ク
リップの配置は両方のX線写真が同じ位置の断層図であ
ることを示している。これらの転移は5か月後の再検査
で観察されなかった。 第10図は、LAKi!!胞とIL−2を用いる治療の
前(上)と後(下)の転移腎細胞癌を持つ患者(第11
@の患者)の肺のコンピュータ連動断層xm図である。 この患者は多数の(multiple)肺小節を持って
いた(矢印は転移の一つを指す)が、LAK細胞とIL
−2を用いる治療の終了後2週以内にはその全てが顕著
に後退した。 第11図は、肺の先端(apical)腺、腫を持つ患
者(第12号の患者)の肺のコンピュータ連動断層X線
図である。LAK細胞とIL−2を用いる治療の前(左

に見られた。 第12図は、第3号の患者におけるLAK細胞とIL−
2の投与後の循環している白血球分画の変動を示す。白

減少した。 [発明の詳細な説明] 本発明の上述の目的と利点は、リンフォカイン活性化ヒ
ト末梢血液リンパ球を包含する組成物によって、これが
、免疫不全状態または癌に罹患しているヒトに対して適
当な薬剤的に許容できる(滅菌済みで無毒)担体中で投
与されるときに巣だされる。上記のリンパ球の活性化に
対する好ましいリンフォカインはインターロイキン2(
1m−2)である。IL−2は組換えIL−2を含む任
意の適当な源から得られる。抗腫瘍活性を有するそのよ
うなリンフォカイン活性化細胞を、ここでば″リンフォ
カイン活性化キラー(LAK)細胞″と定義する。 LAK細胞と同様に、フィトヘムアグルチニン活性化キ
ラー細胞(PAK細胞)も使用される。 LAKまたはPAK細胞では最小限の副作用が観測され
た。養子免疫療法の成功と発達を妨げてきた主要な障害
は、養子移転に使用する適当な細胞を手に入れることで
あった。これらの細胞は犬山に手に入ることが好ましい
。多くの動物モデルから、明らかなヒトの悪性疾患を臨
床的に治療するためには約10″の免疫細胞が必要とさ
れることが予測される。これらの細胞は腫瘍に対して免
疫学的に特異的であり、且つ、養子移転に十分に耐える
ものでなければならない。細胞は自系であることが好ま
しいが、これは絶対的な条件ではない。 免疫学的に活性な細胞は他の通常の細胞より大きい傾向
があるので、これらの細胞の″通行(tragic )
”が重要である。投与された細胞は体中で腫瘍の部位
へ通行できなければならない。 更に、免疫性のリンパ球様細胞は増殖できる。養子移転
に対する理想的な細胞は、l!瘍による直接の抗原性刺
激または追加の成長因子の存在のいずれかによって腫瘍
の部位で数を殖やすことができる必要がある。移転され
た細胞の効果を拡大する能力はこの方法の主要な財産と
なり得る。 末梢血液単核i胞は、当業界で公知の標準法に従って多
数の(Illu1口ple)白血球搬出で得られ、1L
−2と共に培養される。容易に入手できるので組換えI
L−2が好まれた。IL−2中での数日、通常3日ない
し4日の体外培養の後、生成の細胞は正常細胞に影響を
与えることなく新鮮な腫瘍細胞を細胞溶解することがで
きる。これらの活性化細胞は好ましくは全身性(sys
tem : c )投与により自系の患者に再注入され
、その後、適当な薬量の組換えI L−2が頻繁な間隔
、通常約8時間毎に投与される。患者は、IL−2を9
0薬今まで、および、14回までの一連の白血球搬出で
得られた活性細胞(LAKまたはPAK)を約2.8X
1010から約12.6x1010まで投与された。当
然、患者の状態に従って1シー2およびLAK細胞の投
与量は加減された。日に多数回投与する借は、体重1キ
ログラム当り、リンパ球の数は105−10sz細胞(
cell) 、および、I L −2の旦は103−1
06単位の範囲に亙ることかできる。 本発明による活性化細胞は、癌、ウィルス性および他の
感染性疾患、自己免疫疾患の治療、免疫欠乏疾患の矯正
等に使用できる。 LAK細胞の航駆体は、Tおよび8L胞表面標識を持た
ないパナル(null)”リンパ球のサブ集団に属する
ことが見出された。ヒトにおいては、これらの前駆体細
胞は末梢血液、リンパ部、骨髄および胸管に広く見出さ
れる。 し具体例」 本発明の実施および/または試験において任意の類似ま
たは同等の方法が使用できるが、好ましい具体例を次に
説明する。以下の記述で言及される全ての文献はここに
参照として挿入される。特に断わらない限り、ここで使
用される種々の語は本発明が属する当業界で十分に理解
されているものと同一の意味を有する。 患者集団 本研究では標準的な治療法では処置できなかった転移癌
を持つ12人の患者の治療が行われたく表1)。5人の
患者が悪性の黒色腫、3人が結腸直腸の癌、2人が肉腫
、1人が腎細胞癌、1人が肺の腺癌を持っていた。全て
の患者は、理学的検査または標準的な放射線写真検査の
いずれかにより臨床的に評価できる疾患を持っていた。 患者は皆、1次疾患の外科切除を実際に受けるかまたは
試みており、その後、転移が起き、可能であれば標準的
な治療が行われたが、効果がなかった。 これらの患者の治療に使用された臨床的プロトコルは、
国立癌研究所の臨床研究委員会並びに食品医薬品局(い
ずれも米国)に認可されたものである。治療の試みの開
始前に、全ての患者の署名した通知された( info
rmed)承諾が得られている。 治療の試みに入る前に、全ての患者について、1類によ
る完全な医学的評価および転移疾患の全ての部位の測定
が行われた。治療の終了時、および、治療の機種々の期
間、一般に、2週、6週およびその後3月毎の期間をお

出が繰返し行われた。1回当り約5X109−5X10
10の単核細胞を集めるよう試みられた。 約60−60−7O/分の流速で、1O−12nの全面
が約4時間内に処理され、この細胞収率で実行すること
ができた。標準酸シトレートブドウ糖(acid c
itrare dextrose) (ACD、
NIH処方A)が抗凝固剤として使用された。 血管へのアクセスは、可能であるときは二重(doub
le)の反射(anticubital )静脈穿刺に
より行ったが、多くの患者は−g (single)ま
たは二重の管腔中心静脈カテーテルを必要とした。 15mffのACD−Aと3.000単位のへプリン(
ブタ、保存薬を含まない。O″Ne1l。 Jones and l:eldman、
5t 、 1outs、 MO)がフエレーシ
ス時に収集バッグに加えられた。それぞれの白血球パッ
クの最終容量は300−400mgであり、フエンワル
(Fenwal ) トランスファバッグ(Trave
nol 、 [)eerrield、 I l )中
に収集された。 リンパ球の収穫と培養 単核細胞はフィコルハイパーグ (F ico!lhypague)密度勾配を用いて分
離された。 カルシウムおよびマグネシウムを含まないバンク()−
lank ’ s )平衡塩溶液(HBSS)の2ない
し3部が、血しようトランスファセット(Fenwal
、4C2240)を用いて1部の白血球搬出細胞懸濁
と混合された。40mfiの希釈した細胞試料が50m
2の円錐遠心管に注がれ、10m2のリンパ球分離媒質
(n+edia ) (LSM ;Litton
Bionetics、Rockville、MD)でア
ンダライ(underlay)された。勾配は900X
gで15分間遠心分離され、分離されたリンパ球は収穫
され、HBSSで2度洗浄され、LAK活性化媒質中に
再懸濁された。この媒質は、10単位/m2のペニシリ
ン、10μQ/ml、のストレプトマイシンサルフェイ
ト、2mMのグルタミン、5μQ/mf)、のグルタミ
シンサルフエイト、および、2%の熱不活性化ヒトAS
血清 (KCBiologicals、 Inc、、 1e
nexa 、 KA)を含有するRPMI 1640
(低いエンドトキシン。M icrobiologic

−107柵胞/m2を含有する1℃の細胞懸濁が2.5
りのローラーボトル(Cornino 25140)
に加えられ、組換えIL−2が約1,000単位/m2
の最終濃度で加えられた。ローラーボトルは37℃で培
養され、3ないし4日間1分当り0.5−1回転で連続
的に回転された。生成のLAK細胞は1℃のボトル中で
約510xaで15分間遠心分離され、ベレットは25
0rr12の遠心管にプールされ、カルシウム、マグネ
シウムまたはフェノールレッドを含まないHBBSで2

ら2mnの一定部分が取られ、微生物の存在を調べるた
めにグラム染色が行われた。更に、最終のLAK注入懸
濁の一定部分が菌類および好気性細菌の存在を調べるた
めに培養された。最終の注入された細胞懸濁の一定部分
が、後の免疫学的試験のために液体窒素中に凍結保存さ
れた。 必要ならば、活性化した注入可能のLAK細胞を標準の
工程の後に凍結保存できる。これらの貯蔵された細胞は
後に解凍して患者に投与するために使用できる。 インターロイキン2 この治療の試みで使用された組換えIL−2はセタス社
(Cetus CorporaNon、 Emery
ville 。 CA)から入手した。このIL−2は、ジャーカット(
J urkat ) l)l胞株から単離されたlシー
2に対する遺伝子に感染させた大腸菌(ニー性■)中で
生産された。IL−2は精製して同質(homogen
eity )にされ、SDSポリアクリルアミド ゲル
電気泳動では単−帯として移動した。 この組換えIL−2の生物学的特性は、文献(Rose
nberg、 et at、 、 1984 5ci
ence;223:1412−1415)に広く記述さ
れている。IL−2は凍結乾燥粉末で得られ、バイアル
当り1.2mgの滅菌水で再形成された。それぞれのバ
イアルは約0.3111!11のIL−2(比活性、3
−5X106単位/Il1g)を含有した。リムルスア
メボサイト(カブトガニ アメーバ様細胞)試験で測定
したところ、バイアル当り0.04ナノグラム以下のエ
ンドトキシンが存在した。それぞれのバイアルは、また
、5%のマンニトール、おJ:ヒIL−2(7)11g
当り約130μq(7)ドデシ)1.を硫酸ナトリウム
を含有した。前述したように、組換えに由来しない源か
らのIL−2も当然使用できる。 しAKI および組 え■ −2の 与LAK細胞は
第8号の患者(表1参照)を除く全ての患者に、中心静
脈カテーテルを通して静脈内にまたは大きな末梢静脈内
に投与された。第8号の患者には、経皮カテーテルを通
して肝動脈への直接の注入により投与された。約106
1!t1の最初の注入に続いて5分後に、約20分以上
に亙り残りのi胞が注入された。注入ラインにはろ紙は
使用されなかまた。注入バッグは注入の間、5分毎に静
かに混合された。 組換え10−2は、5%のヒト血清アルブミンを含有す
る50mff1の通常の食塩水中に希釈され、8時間毎
に15分に亙り静脈内に注入された。 111江」1 これらの研究で使用したIし−2の力価は、文献(Ro

用して、試料がIL−2依存細胞株の増殖を維持する能
力を測定することによって、決定した。これらの実験で

IL−2標準に沿って、この物質について行った試験は
、ここで使用したような1単位は、およそ0.4国際単
位に対応することを示した。 全ての注入された細胞の一定部分は凍結保存され、各患
者治療の最侵において、これらの細胞は解凍され、そし
て文献(L」ユze et工a!。 1981、Cancer Res、;41 :4420
−4425>に記載されたような、新鮮なヒト腫瘍標的
(teraet )細胞に対する標準4時間クロム放出
(release )試験を使用して、LAK活性のた
めに試験された。加えて、血清および抹消白血球を免疫
学的試験のために一定明間で凍結保存した。 LAK細胞および組換えIL−2の投与LAK1B胞は
、新鮮な天然キラー耐性腫瘍標的細胞および天然キラー
感応性に562細胞株の細胞溶解のために慣例的に試験
された。第2号の患者からの細胞についてなされたしA
K試験の特徴的な結果を第1図に示す。最大LAKIB
胞溶解は、エフェクタ;標的の比が20:1又はこれ以
下において見られ、時々、最大溶解はエフェクタ:標的
の比が5:1において見られた。K562腫瘍胞株は新
鮮な腫瘍細胞試料よりも、LAK細胞溶解に対して一貫
してより敏感だった。しAKII[l胞世代は、各治療
サイクルで白血球搬出を通して比較的一定だった。 患者は、各患者に使用したプロトコルによって、10.
000単位/kg、30.000単位/kg。 または100.000単位/kaの量で8時間ごとにI
L−2のポーラスな注入を受けた。第11号の患者のI
L−2注入後の血清IL−2レベルの特徴的パターンを
第2図に示す。血清IL−2レベルは、文献(Dono

に、ヒトにおける組換えIL−2の初めの6乃至7分の
半減期と一致して、各IL−2ポーラス注入の完了後に
急激に落ちた。ioo、oo。 単位/kQの投与後、血清レベルは、一般に8時間後の
次の注入の時までに、1乃至5単位、/1まで落ちた。 血清IL−2のより低いレベルは、30゜OOo単位/
kgが投与された場合(第2図)に見られ、そして10
,000単位/kgの投与の後には少しのみのくもしあ
るとしたら)血清におけるIL−2レベルがH察できた
。マウスの腫瘍モデルが、IL−2の持続された血清レ
ベルが最適の治療学的効果を得るために必要であること
を示唆したので、治療を進めるとき、組換えIL−2の
増加した量が静脈に使用された。 治療の結果 12人の患者に使用した正確な治療計画を第3図に表わ
す。第10号、第11号および第12号の患者を除いて
、全ての患者は少なくとも2サイクルの白血球搬出を受
け、数人の患者は3サイクル受けた。全ての場合、LA
K細胞投与のときにIL−2投与を開始し、モしてLA
KII胞投与の後少なくとも数日間IL−2を続けるこ
とが試みられたくもし患者によって許された場合には)
。 各患者におけるLAK細胞および組換えIL−2の薬量
を表2に示す。転移性黒色腫をもつ患者での1つの腫瘍
の完全な後退を含めて、これらの12人の患者のうちの
6人は測定可能な腫瘍後退を経験した。 第1号の患者は、LAKIII胞およびIL−2での治
療後に、全ての転移黒色腫の皮下沈着の完全な後退をし
た。これらの病変は治療の間安定し、そして治療の完了
の後およそ4週間から、腫瘍小節はゆっくり後退し始め
た。治療の終わりの後4週でなされた生体組織検査は、
全ての腫瘍、慢性炎症、および線維症の凝固性の壊死を
示した(第4図)。これらの全ての病変は完全に後退し
、この患者は治療の終結後6週間病気がない。LAK細
胞とIL−2に関する、これらのいくつかの皮膚病変の
後退の時間変化を第5図に示す。 治療の前、および治療の機種々の間隔で切取った、別の
患者(第9号)の皮下黒色腫の連続生態組織検査は、L
AKII胞およびIL−2の投与に続く“活性化された
°゛リンパ様細胞を伴う広い浸潤を示した(第6図)。 もとの腫瘍は最小の壊死を示し、リンパ球をほとんど含
まなかったが、治療の終わりまでに、リンパ様の湿潤は
腫瘍縁で顕著で、生存可能で壊死性の腫瘍細胞の間に散
乱された。この患者において、数個の小節が大きさでの
後退を示したが、他の小節は拡大した。この患者(第9
号)は、こうして臨床的な反応はなかったと考えられた
。 第2号の患者は、5個の肺転移を伴う直腸癌であった。 これらの転移の3個は、LAIII胞およびIL−2で
の治療の後に、従来の胸部X線および直線断1i11!
影法によって示されたように完全に消失した。いく分よ
り大きな残りの2つは、太きさは小さくなったが消えな
かった。これらの後者の転移病変の1つのil瘍後退の
時間変化は第7図に見られる。部分的に後退した1つの
病変の、および完全に後退した2つの病変の治療前およ
び治療後のX線は、それぞれ第8および9図に示されて
いる。治療の最後までに消えた3つの病変は、治療後5
月間再出現していない。部分的に消失した2つの小節は
6週間後に再び成長し初め、この患者は治療の第2のコ
ースで、これらの小節の大きさの少しの減少をみた。こ
の患者は治療の第3のコースを受けている。興味深いの
はこの患者のCEA水準で、これは治療の初めは15っ
であり、治療の第1のコースの後は91で、治療の第2
コースの後は33であった。 第8号の患者は、超音波検査によって示されたように結
腸癌からの肝転移の部分的後退を経験し、第9号の信者
は、黒色腫からの肺転移の部分的後退をした。これらの
患者は、転移疾患のかさ高い部位を有し、そして病変の
体積の50%の減少が見られたけれども完全に消えた病
変はなかった。 第11号の患者は、治療の完了後、2週間内に腎臓細胞
からの肺転移の部分的後退を示しく第10図)、第12
号の患者は、治療後2週間内に、大きい先端肺の腺癌の
治療に対する部分的応答を示したく第11図)、これら
の患者は、注意深く追跡され続けられており、完全な寛
解が見られないなら再治療されるだろう。 プロトコルが進むときになされた治療法の変更において
大きな因子が含まれた。これらは(1)IL−2投与の
間、末梢血液からのLAK前駆体の急速な消失があり、
そしてこうして白血球搬出は、IL−2が与えられた間
、許容しうる細胞収量を生じなかった。このプロトコル
において報告された12人の患者の治療の前に治療した
4人の患者から、IL−2注入の間、リンパ球を収穫す
る試みがなされた。最小数の細胞が得られた。これはI
L−2投与の間、循環系においてLAK前駆体が存在し
ない理由による。そしてこれら4人の患者のいずれも総
量で2X101’より多い細胞を受けなかった。この理
由により、白血球搬出はIL−2投与が中止された後に
行った。(2)IL−2投与の中止後、循環系のリンパ
球の数ならびにLAK前駆体の数において、目立ったは
ね返りがあった。したがって、プロトコルを進めるとき
IL−2は数日間投与され、それから白血球搬出の開始
のおよそ36時間前にIL−2投与を中止した。第3号
の患者におけるIL−2投与の関数としてのリンパ球分
画における変動の例を第12図に示す。こうしてIL−
2の投与およびリンパ球搬出を繰返すことにより、収穫
されたLAKII胞の数を非常に増加することが可能で
あった。1L−2の毒性は、与えることのできるIL−
2の量を制限した。動物モデルは、治療的効果が、注入
されたLAKの数および投与されたIL−2の量に直接
関係することを示した。IL−2投与に関係した毒性副
作用の理由により、(下記)、この臨床的プロトコルの
早い段階において治療されたほとんどの患者は、8時間
ごとに10.000単位/kQのIL−2を受けた。後
続の患者は30.000m位/kgを受け、第11号お
よび第12号の患者は、ioo、ooo単位/lagを
受けた。一般に、より多いtL−2薬量は、長期間良好
には耐えられないので、より少ない投与量で治療サイク
ルが行われた。 治療の毒性 これらの患者における治療の毒性を表3に示、す。 先の研究において、活性化されたキラー細胞だけの注入
は一時的な熱および寒けおよび肺拡散容最の一時的な減
少を伴うことが証明された( Rosenberg、
1984. J、 Biol 、 Re5p 、 M

896−905) 、最近の研究における患者は、しば
しばIll胞注入の直後に寒けをもった。この寒けは、
メペリジン(25乃至50mg)又はモルフォリン(4
乃至1ng)の静脈投与によって、一般によく!IIl
[Iされる。 このプロトコルにおいて、IL−2投与に関係する主な
副作用は、おそらく毛管透過性の全身的増加から生ずる
体液保持による体重増加であった。 12人の患者の8人は開始時の体重の10%以上増加し
た。この体液保持は、柔らかい組織において最も深く、
目立った末梢水腫を生じたが、早期には肺を分けるよう
に見えた。IL−2の量を30、OOOおよびIOo、
000m位/kfll与、’した選択された患者の肺水
測定は、普通のυ1限内であった。しかしIL−2投与
のコースの遅くにおいて、体液保持は、しばしば、12
人の患者の9人において、放射性写真が呼吸困難に関係
する肺における間隙の水腫のように見える肋膜の流出お
よび腹水を生じた。これらの患者の2人(第1号および
第12号の患者)は、それぞれ1および4日の潜伏を要
するひどい呼吸窮迫を発生した。加えて、IL−2注入
は熱および倦怠に関係し、12人の患者の7人は全身の
紅斑性の皮膚発疹を発生した。熱、寒けおよび倦怠は、
アセトアミノフェノン(6時間毎に650m(1)およ
びインドメチシン(6時間毎に25mg)の使用によっ
て除去することができ、患者の多くはこれらの治療を受
けた。多くの患者は、それらの発疹の治療のための抗ヒ
スタミン剤であるヒドロキシジンヒドロクロリドをも受
けた。はとんどの患者に使用した睡眠薬はドキセビンで
あった。はとんどの患者は胃腸の出血の予防にラニチジ
ン(1日150ma経口)を続けた。低尿排出を生ずる
顕著な腎臓機能障害は、共通でない副作用で、12人の
患者の3人のみが2111(+/%以上の血清クレアチ
ン水準を発生した。12人の患者の5人は2 mQ/%
以上の一時的高ビリルリン血を発生した。好酸球増加は
共通で、12人の患者の11人が循環系好酸球を発生し
、これは全白血球(white cell)分画の5%
以上であり、数人の患者においては全循環系白細胞の8
0%以上はIL−2投与の高さで好酸球であった。IL
−2投与は造血抑制にも関係するようであった。全ての
患者は治療の間、貧血を直すために輸血を必要とし、1
2人の患者の4人は50゜OOO血小板/rAI11
以下の血小板減少症を発生した。 全ての患者において、不利となる効果はIL−2投与停
止後敏速に消失し、12人の患者は全て結局、追跡調査
のために退院を許された。一般に利尿は、1m−2の中
止後24時間以内に始まり、利尿剤の使用により促進す
ることができた。他の副作用は一般にIL−2を止めた
後4乃至5日以内に消失した。この報告において考慮し
ていない別の1人の患者は、細胞注入の第1のサイクル
の間に胸の痛みを発生し、治療は1サイクルの完了の前
に中止された。続きの研究は、心筋の虚血の診断を確認
しなかった。この患者は退院し再治療されなかった。 この研究は、癌患者の治療において、インターロイキン
−2(IL−2)と結合してのリンフォカイン活性化キ
ラー(LAK)細胞の使用の最初の報告である。この臨
床的試みに先立つものとして、相乗的しAKII胞およ
び組換え1m−2の投与が、2つの異なる系統のマウス
の免疫原的及び非免疫原的肉腫、メラノーマ性及び非メ
ラノーマ性黒色腫、マウス結腸腺癌、及びマウスぼうこ
う癌を含めて種々の移植可能マウスll11!における
樹立された肺及び肝臓の転移の後退を行うことが可能で
あるという証明がある(Mule 、 et at。 1984、5cience; 225 : 1487−

2人の患者のうち、6人は彼らの転移路の客観的な寛解
を経験した。全ての患者は転移性感が進展し、外科、化
学療法及び放射線治療を含めて標準の治療に失敗してい
た。 試験管中の研究は、LAK細胞が広い範囲のヒトのl1
ll!標的を溶解することができるが、通常の細胞を溶
解しないことを証明してきた。ヒト腫瘍の41の連続単
細胞懸濁物の研究において、胃、卵巣、すい臓、及び結
腸並びに種々の肉腫及び黒色腫の腺癌を含めて36がL
AK細胞によって著しく溶解された(Raner e

333)、LAK細胞によって認識された抗原について
はほとんど知られていないが、特定の理論に限定される
ことなく、それは形質変換された細胞上に遍在して分布
し新鮮な正常細胞上には分布しないと考えられる。マウ
ス及びヒトの両者におけるLAKwJ胞の前駆体は、非
T1非B゛ヌル″リンパ球であるらしい。マウスにおい
てLAK前駆体細胞はThy−1−1Ia−であり、ヒ
トでは前駆体細胞は1eu−1−1OKT−3−11e
u −7−7−1OK 1である。これらののLAKI
胞前駆体はしたがって細胞表面表現型においても及びそ
れらの溶解特異性においても天然キラー細胞と異なる(
Q rimmet at、 1982 J、 EX

)。天然キラー細胞は培養された標的細胞を溶解し、新
鮮な!!瘍標的への影響はもしあるとしても非常に少な
い。しかししAKエフェクタ細胞は、前駆体1胞と異な
り、Tm胞であるらしい。マウスにおいてThy−1抗
原を有するからである。こうしてLAK前駆体細胞は、
IL−2の影響下でT細胞に分化することができるT細
胞系統における原始細胞であるらしい。これらの細胞は
、形質転換された細胞に対する天然の免疫監視における
役割のための良い候補をつくったが、2正常なヒトにお
けるこれらLAKIII胞の生理的機能は知られていな
い。 マウスモデルは、LAK細胞及び組換えIL−2の両者
が抗腫瘍効果を行うのに必要であることを予言した(M
ule、etal、1984 5cienCe:225

6−652)。非常に大愚のIL−2がマウスにおいて
抗腫瘍効果を媒介したが(Rosenberg et

−1188)、IL−2投与に関係する毒性の理由によ
りIL−2のそれらの薬量を達成することは不可能であ
る。こうして、マウスにおいて、しAK細胞のみの投与
及び組換えIL−2のみの少量の投与は、抗腫瘍効果が
なかった。これは先のヒトの研究で確証された。そこで
は、26人がLAK細胞だけを受け、39人はIL−2
だけを受けた(16人は天然ヒトIL−2を受け、23
人は組換えIL−2を受けた)。抗腫瘍効果は見られな

7−166)、マウスモデルが体内ではIL−2への連
続的接触が必要であることを予言したので、ヒトでは8
時間毎の静脈注射を含む投与方法が選ばれた。IL−2
の連続的注入を伴う早期の研究は、許された最大の投与
層においても循環系には検出できるレベルのIL−2が
見出されないことを示した。しかしポーラスな投与が8
時間毎に与えられた場合、高い血清レベルが数時間、見
られたく第2図〉。 ココテ示したように、LAKIlfm及ヒI L−2を
同時に与えた場合、患者に著しい抗腫瘍効果が見られた
。ここで報告した6つの客観的応答は、黒色腫、結腸直
腸癌、腎臓細胞癌及び肺の腺癌を含む4つの異なる組織
的タイプの腫瘍において生じた。体外におけるLAKI
胞の広い細胞溶解特性の理由により、この治療様式が多
数の組織的サブタイプの癌への適用可能性を有すること
は明らかである。6人の患者の5人は客観的部分的寛解
を達成しく腫瘍の体積が少なくとも50%減少)、1人
の患者は病気の完全な寛解を達成し、そして散在した黒
色腫の3つの治療の後6週間M瘍がない。5つの肺の黒
色腫の3つが完全に後退した第2号の患者において、こ
の消失した3つの転移も戻っていないが、部分的にしか
後退しなかった2つの転移は、およそ1月後再び成長を
始めた。これらの結果は、1IWA沈積の消失は長期の
II!瘍制開制御ずるが、部分的な腫瘍消失は、その後
腫瘍がすぐに成長することを示している。マウスモデル
において、拡張された調査が、LAK細胞溶解に抵抗す
るI!瘍の存在のためになされ、そしてそのような細胞
が存在することを証明することはできなかった。マウス
腫瘍モデルにおいて、LAK及びIL−2治療にもかか
わらず生き残った転移は、管内及び体内の両者で元のI
!IIIのようにLAK細胞溶解を受けやすい。 この治療を改良する他の方向がマウスモデルによって示
唆された。最少の騰瘍縁はバルクの疾患(bulk d
isease)よりも治療的LAK治療を受けやすく、
外科処置の直後のアジュバントとしてのこの治療の使用
−が示される。LAKIl[l胞及びIL−2での治療
は、宿主免疫適格に依存しないので(Mule et
al、1985. J、 l01iuno1.。 135:646−652)、本治療様式は化学治療及び
放射線治療との組合わせのために理想的であろう。加え
て、マウス実験は、同腫のLAKil[I胞が同系の細
胞のように治療的に効果的であること(Mule et
al、1985. J 、 l a+muno1.。 135 :646−652>、及びLAK細胞の直接経
口注入がLAK細胞の全身投与よりも効果的であるかも
しれないことを示している。さらに、本発明はLAK細
胞に限定されないことに注意すべきである。もちろん任
意の他の適当な免疫細胞も同様にすることができた。加
えて、これらのLAK又は同様な性質を有する免疫細胞
もまた、組織培養に広げることができ、そして本発明に
したがって使用することができた。さらに、単クローン
抗体は、LAK[l胞及びIL−2の両者を腫瘍部位に
向けるためにも使用することができた。 上記のような変更は限定的ではなく当業者の1人には容
易に明らかで提案される。 ここで説明したようなIL−2と結合した゛LAK細胞
の投与は、広い種類の腫瘍又は免疫感応条件への将来適
用可能性を伴う、癌治療への新しいアプローチである。 ヒトにおいて得られた結果のマウスとの類似性は、癌及
び他の免疫関連機能障害又は疾患の制御のための有効な
治療様式として、採用される免疫治療の利用性を高める
。 ここで説明した例及び態様は、説明の目的だけであるこ
と、及びこれを考慮すると種々の変形及び変化が当業者
には示唆され、それは本出願の精神及び範囲、及び特許
請求の範囲の範囲(SCOpe )に含まれることが理
解される。

検査(右)は、少数のリンパ球様細胞を含む繊維性の擬
被膜(pseudocapsule )と多形の腫I!
細胞の凝集体を示す。治療の終了の4−週後に行った同
じ患者の生体組織検査は、黒色腫の広範な凝固壊死およ
び繊維性被膜内に慢性の炎症性細胞が増加していること
を示している。このときは生存可能の(viable)
III!は見られなかった。(ヘマトキシリンおよび
エオシン;220)第5図は、第1号の患者の皮下の黒
色腫小節の後退(regr6ssion)の時間経過を
示す。LAK細胞およびIL−2の投与の終了後、顕著
な饅層の後退が見られた。治療終了後4週までに黒色腫
小面は大きさが減少し、治療後3月までに完全に消失し
た。 第6図は、第9号の患者の転移黒色腫の皮下沈着の続き
の生体組織検査(左)を示す。皮下の黒色腫の治S前の
生体組織検査〈右)は、微少の壊死を伴う黒色腫細胞の
シートと極く稀にリンパ球様細胞を示した。LAK細胞
とIL−2を用いる治療の2週後に切除された転移沈着
物の生体組織検査体は、顕1な慢性の炎症性湿潤物を示
した。 個々の壊死腫瘍細胞が残りの生存可能な腫瘍内に見られ

(右)x500゜ 第7図は、第2号の患者のL A、 K [1胞とIL
−2を用いる治療の前後の転移結腸癌の肺への成長を示
す。この患者は5か所の肺の転移病巣を持っていた。こ
こには1か所の転移病巣の成長曲線が示されている。こ
の病巣は2年間に亙って成長し続け、マイトマイシンC
による治療に反応しなかった。LAK細胞とIL−2を
用いる治療後1週問以内にこの病巣は後退し始めた。 第8図は、第7図に示した右肺の肺病巣(上左図の矢印
)の治療#後のX1m写真である。この病巣は1985
年1月10日の治療開始以前は段々と成長していた。1
1日後には小節の顕著な後退が見られた。大きな病巣の
真上および右に見られた2個の対陣の(Satelli
te )の小節は完全に後退した(第9図参照)。 第9図は、結腸癌を持つ患者(第2号)の肺転移の直線
(l 1near)断層X線図である。これらのX線写
真はLAK細胞とIL−2を用いる治療以前に右肺に2
個の転移小節(矢印)が存在したことを示している(左
)。これらの2個の肺転移は完全に後退した(右)。ク

減少した。 [発明の詳細な説明] 本発明の上述の目的と利点は、リンフォカイン活性化ヒ
ト末梢血液リンパ球を包含する組成物によって、これが
、免疫不全状態または癌に罹患しているヒトに対して適
当な薬剤的に許容できる(滅菌済みで無毒)担体中で投
与されるときに巣だされる。上記のリンパ球の活性化に
対する好ましいリンフォカインはインターロイキン2(
1m−2)である。IL−2は組換えIL−2を含む任
意の適当な源から得られる。抗腫瘍活性を有するそのよ
うなリンフォカイン活性化細胞を、ここでば″リンフォ
カイン活性化キラー(LAK)細胞″と定義する。 LAK細胞と同様に、フィトヘムアグルチニン活性化キ
ラー細胞(PAK細胞)も使用される。 LAKまたはPAK細胞では最小限の副作用が観測され
た。養子免疫療法の成功と発達を妨げてきた主要な障害
は、養子移転に使用する適当な細胞を手に入れることで
あった。これらの細胞は犬山に手に入ることが好ましい
。多くの動物モデルから、明らかなヒトの悪性疾患を臨
床的に治療するためには約10″の免疫細胞が必要とさ
れることが予測される。これらの細胞は腫瘍に対して免
疫学的に特異的であり、且つ、養子移転に十分に耐える
ものでなければならない。細胞は自系であることが好ま
しいが、これは絶対的な条件ではない。 免疫学的に活性な細胞は他の通常の細胞より大きい傾向
があるので、これらの細胞の″通行(tragic )
”が重要である。投与された細胞は体中で腫瘍の部位
へ通行できなければならない。 更に、免疫性のリンパ球様細胞は増殖できる。養子移転
に対する理想的な細胞は、l!瘍による直接の抗原性刺
激または追加の成長因子の存在のいずれかによって腫瘍
の部位で数を殖やすことができる必要がある。移転され
た細胞の効果を拡大する能力はこの方法の主要な財産と
なり得る。 末梢血液単核i胞は、当業界で公知の標準法に従って多
数の(Illu1口ple)白血球搬出で得られ、1L
−2と共に培養される。容易に入手できるので組換えI
L−2が好まれた。IL−2中での数日、通常3日ない
し4日の体外培養の後、生成の細胞は正常細胞に影響を
与えることなく新鮮な腫瘍細胞を細胞溶解することがで
きる。これらの活性化細胞は好ましくは全身性(sys
tem : c )投与により自系の患者に再注入され
、その後、適当な薬量の組換えI L−2が頻繁な間隔
、通常約8時間毎に投与される。患者は、IL−2を9
0薬今まで、および、14回までの一連の白血球搬出で
得られた活性細胞(LAKまたはPAK)を約2.8X
1010から約12.6x1010まで投与された。当
然、患者の状態に従って1シー2およびLAK細胞の投
与量は加減された。日に多数回投与する借は、体重1キ
ログラム当り、リンパ球の数は105−10sz細胞(
cell) 、および、I L −2の旦は103−1
06単位の範囲に亙ることかできる。 本発明による活性化細胞は、癌、ウィルス性および他の
感染性疾患、自己免疫疾患の治療、免疫欠乏疾患の矯正
等に使用できる。 LAK細胞の航駆体は、Tおよび8L胞表面標識を持た
ないパナル(null)”リンパ球のサブ集団に属する
ことが見出された。ヒトにおいては、これらの前駆体細
胞は末梢血液、リンパ部、骨髄および胸管に広く見出さ
れる。 し具体例」 本発明の実施および/または試験において任意の類似ま
たは同等の方法が使用できるが、好ましい具体例を次に
説明する。以下の記述で言及される全ての文献はここに
参照として挿入される。特に断わらない限り、ここで使
用される種々の語は本発明が属する当業界で十分に理解
されているものと同一の意味を有する。 患者集団 本研究では標準的な治療法では処置できなかった転移癌
を持つ12人の患者の治療が行われたく表1)。5人の
患者が悪性の黒色腫、3人が結腸直腸の癌、2人が肉腫
、1人が腎細胞癌、1人が肺の腺癌を持っていた。全て
の患者は、理学的検査または標準的な放射線写真検査の
いずれかにより臨床的に評価できる疾患を持っていた。 患者は皆、1次疾患の外科切除を実際に受けるかまたは
試みており、その後、転移が起き、可能であれば標準的
な治療が行われたが、効果がなかった。 これらの患者の治療に使用された臨床的プロトコルは、
国立癌研究所の臨床研究委員会並びに食品医薬品局(い
ずれも米国)に認可されたものである。治療の試みの開
始前に、全ての患者の署名した通知された( info
rmed)承諾が得られている。 治療の試みに入る前に、全ての患者について、1類によ
る完全な医学的評価および転移疾患の全ての部位の測定
が行われた。治療の終了時、および、治療の機種々の期
間、一般に、2週、6週およびその後3月毎の期間をお

出が繰返し行われた。1回当り約5X109−5X10
10の単核細胞を集めるよう試みられた。 約60−60−7O/分の流速で、1O−12nの全面
が約4時間内に処理され、この細胞収率で実行すること
ができた。標準酸シトレートブドウ糖(acid c
itrare dextrose) (ACD、
NIH処方A)が抗凝固剤として使用された。 血管へのアクセスは、可能であるときは二重(doub
le)の反射(anticubital )静脈穿刺に
より行ったが、多くの患者は−g (single)ま
たは二重の管腔中心静脈カテーテルを必要とした。 15mffのACD−Aと3.000単位のへプリン(
ブタ、保存薬を含まない。O″Ne1l。 Jones and l:eldman、

ス時に収集バッグに加えられた。それぞれの白血球パッ
クの最終容量は300−400mgであり、フエンワル
(Fenwal ) トランスファバッグ(Trave
nol 、 [)eerrield、 I l )中
に収集された。 リンパ球の収穫と培養 単核細胞はフィコルハイパーグ (F ico!lhypague)密度勾配を用いて分
離された。 カルシウムおよびマグネシウムを含まないバンク()−
lank ’ s )平衡塩溶液(HBSS)の2ない
し3部が、血しようトランスファセット(Fenwal
、4C2240)を用いて1部の白血球搬出細胞懸濁
と混合された。40mfiの希釈した細胞試料が50m
2の円錐遠心管に注がれ、10m2のリンパ球分離媒質
(n+edia ) (LSM ;Litton
Bionetics、Rockville、MD)でア
ンダライ(underlay)された。勾配は900X
gで15分間遠心分離され、分離されたリンパ球は収穫
され、HBSSで2度洗浄され、LAK活性化媒質中に
再懸濁された。この媒質は、10単位/m2のペニシリ
ン、10μQ/ml、のストレプトマイシンサルフェイ
ト、2mMのグルタミン、5μQ/mf)、のグルタミ
シンサルフエイト、および、2%の熱不活性化ヒトAS
血清 (KCBiologicals、 Inc、、 1e
nexa 、 KA)を含有するRPMI 1640
(低いエンドトキシン。M icrobiologic
al A 5sociates 。 Rockville、 M D )から成る。約105
−107柵胞/m2を含有する1℃の細胞懸濁が2.5

の最終濃度で加えられた。ローラーボトルは37℃で培
養され、3ないし4日間1分当り0.5−1回転で連続
的に回転された。生成のLAK細胞は1℃のボトル中で
約510xaで15分間遠心分離され、ベレットは25
0rr12の遠心管にプールされ、カルシウム、マグネ
シウムまたはフェノールレッドを含まないHBBSで2
度以上洗浄され、細胞は、5%正常ヒト血清アルブミン

た。 収穫の工程が始められたとき、一つのローラーボトルか
ら2mnの一定部分が取られ、微生物の存在を調べるた
めにグラム染色が行われた。更に、最終のLAK注入懸
濁の一定部分が菌類および好気性細菌の存在を調べるた
めに培養された。最終の注入された細胞懸濁の一定部分
が、後の免疫学的試験のために液体窒素中に凍結保存さ
れた。 必要ならば、活性化した注入可能のLAK細胞を標準の
工程の後に凍結保存できる。これらの貯蔵された細胞は
後に解凍して患者に投与するために使用できる。 インターロイキン2 この治療の試みで使用された組換えIL−2はセタス社
(Cetus CorporaNon、 Emery
ville 。 CA)から入手した。このIL−2は、ジャーカット(
J urkat ) l)l胞株から単離されたlシー
2に対する遺伝子に感染させた大腸菌(ニー性■)中で
生産された。IL−2は精製して同質(homogen
eity )にされ、SDSポリアクリルアミド ゲル
電気泳動では単−帯として移動した。 この組換えIL−2の生物学的特性は、文献(Rose
nberg、 et at、 、 1984 5ci
ence;223:1412−1415)に広く記述さ
れている。IL−2は凍結乾燥粉末で得られ、バイアル
当り1.2mgの滅菌水で再形成された。それぞれのバ
イアルは約0.3111!11のIL−2(比活性、3
−5X106単位/Il1g)を含有した。リムルスア
メボサイト(カブトガニ アメーバ様細胞)試験で測定
したところ、バイアル当り0.04ナノグラム以下のエ
ンドトキシンが存在した。それぞれのバイアルは、また
、5%のマンニトール、おJ:ヒIL−2(7)11g
当り約130μq(7)ドデシ)1.を硫酸ナトリウム
を含有した。前述したように、組換えに由来しない源か
らのIL−2も当然使用できる。 しAKI および組 え■ −2の 与LAK細胞は
第8号の患者(表1参照)を除く全ての患者に、中心静
脈カテーテルを通して静脈内にまたは大きな末梢静脈内
に投与された。第8号の患者には、経皮カテーテルを通
して肝動脈への直接の注入により投与された。約106
1!t1の最初の注入に続いて5分後に、約20分以上
に亙り残りのi胞が注入された。注入ラインにはろ紙は
使用されなかまた。注入バッグは注入の間、5分毎に静
かに混合された。 組換え10−2は、5%のヒト血清アルブミンを含有す
る50mff1の通常の食塩水中に希釈され、8時間毎
に15分に亙り静脈内に注入された。 111江」1 これらの研究で使用したIし−2の力価は、文献(Ro
senber et al、 1978゜J、lm1u
no1.:121 :1946−1950)の方法を使
用して、試料がIL−2依存細胞株の増殖を維持する能
力を測定することによって、決定した。これらの実験で
使用した力価は、セタス社(Cetus Corpo
ration)により得られたもので、確認されたもの

IL−2標準に沿って、この物質について行った試験は
、ここで使用したような1単位は、およそ0.4国際単
位に対応することを示した。 全ての注入された細胞の一定部分は凍結保存され、各患
者治療の最侵において、これらの細胞は解凍され、そし
て文献(L」ユze et工a!。 1981、Cancer Res、;41 :4420
−4425>に記載されたような、新鮮なヒト腫瘍標的
(teraet )細胞に対する標準4時間クロム放出
(release )試験を使用して、LAK活性のた
めに試験された。加えて、血清および抹消白血球を免疫
学的試験のために一定明間で凍結保存した。 LAK細胞および組換えIL−2の投与LAK1B胞は
、新鮮な天然キラー耐性腫瘍標的細胞および天然キラー
感応性に562細胞株の細胞溶解のために慣例的に試験
された。第2号の患者からの細胞についてなされたしA
K試験の特徴的な結果を第1図に示す。最大LAKIB
胞溶解は、エフェクタ;標的の比が20:1又はこれ以
下において見られ、時々、最大溶解はエフェクタ:標的
の比が5:1において見られた。K562腫瘍胞株は新
鮮な腫瘍細胞試料よりも、LAK細胞溶解に対して一貫
してより敏感だった。しAKII[l胞世代は、各治療
サイクルで白血球搬出を通して比較的一定だった。 患者は、各患者に使用したプロトコルによって、10.
000単位/kg、30.000単位/kg。 または100.000単位/kaの量で8時間ごとにI
L−2のポーラスな注入を受けた。第11号の患者のI
L−2注入後の血清IL−2レベルの特徴的パターンを
第2図に示す。血清IL−2レベルは、文献(Dono
hue、 1983゜J、Immunol; 130

に、ヒトにおける組換えIL−2の初めの6乃至7分の
半減期と一致して、各IL−2ポーラス注入の完了後に
急激に落ちた。ioo、oo。 単位/kQの投与後、血清レベルは、一般に8時間後の
次の注入の時までに、1乃至5単位、/1まで落ちた。 血清IL−2のより低いレベルは、30゜OOo単位/
kgが投与された場合(第2図)に見られ、そして10
,000単位/kgの投与の後には少しのみのくもしあ
るとしたら)血清におけるIL−2レベルがH察できた
。マウスの腫瘍モデルが、IL−2の持続された血清レ
ベルが最適の治療学的効果を得るために必要であること
を示唆したので、治療を進めるとき、組換えIL−2の
増加した量が静脈に使用された。 治療の結果 12人の患者に使用した正確な治療計画を第3図に表わ
す。第10号、第11号および第12号の患者を除いて
、全ての患者は少なくとも2サイクルの白血球搬出を受
け、数人の患者は3サイクル受けた。全ての場合、LA
K細胞投与のときにIL−2投与を開始し、モしてLA
KII胞投与の後少なくとも数日間IL−2を続けるこ
とが試みられたくもし患者によって許された場合には)
。 各患者におけるLAK細胞および組換えIL−2の薬量
を表2に示す。転移性黒色腫をもつ患者での1つの腫瘍
の完全な後退を含めて、これらの12人の患者のうちの
6人は測定可能な腫瘍後退を経験した。 第1号の患者は、LAKIII胞およびIL−2での治
療後に、全ての転移黒色腫の皮下沈着の完全な後退をし
た。これらの病変は治療の間安定し、そして治療の完了
の後およそ4週間から、腫瘍小節はゆっくり後退し始め
た。治療の終わりの後4週でなされた生体組織検査は、
全ての腫瘍、慢性炎症、および線維症の凝固性の壊死を
示した(第4図)。これらの全ての病変は完全に後退し
、この患者は治療の終結後6週間病気がない。LAK細
胞とIL−2に関する、これらのいくつかの皮膚病変の
後退の時間変化を第5図に示す。 治療の前、および治療の機種々の間隔で切取った、別の
患者(第9号)の皮下黒色腫の連続生態組織検査は、L
AKII胞およびIL−2の投与に続く“活性化された
°゛リンパ様細胞を伴う広い浸潤を示した(第6図)。 もとの腫瘍は最小の壊死を示し、リンパ球をほとんど含
まなかったが、治療の終わりまでに、リンパ様の湿潤は
腫瘍縁で顕著で、生存可能で壊死性の腫瘍細胞の間に散
乱された。この患者において、数個の小節が大きさでの
後退を示したが、他の小節は拡大した。この患者(第9
号)は、こうして臨床的な反応はなかったと考えられた
。 第2号の患者は、5個の肺転移を伴う直腸癌であった。 これらの転移の3個は、LAIII胞およびIL−2で
の治療の後に、従来の胸部X線および直線断1i11!
影法によって示されたように完全に消失した。いく分よ
り大きな残りの2つは、太きさは小さくなったが消えな
かった。これらの後者の転移病変の1つのil瘍後退の
時間変化は第7図に見られる。部分的に後退した1つの
病変の、および完全に後退した2つの病変の治療前およ
び治療後のX線は、それぞれ第8および9図に示されて
いる。治療の最後までに消えた3つの病変は、治療後5
月間再出現していない。部分的に消失した2つの小節は
6週間後に再び成長し初め、この患者は治療の第2のコ
ースで、これらの小節の大きさの少しの減少をみた。こ
の患者は治療の第3のコースを受けている。興味深いの
はこの患者のCEA水準で、これは治療の初めは15っ
であり、治療の第1のコースの後は91で、治療の第2
コースの後は33であった。 第8号の患者は、超音波検査によって示されたように結
腸癌からの肝転移の部分的後退を経験し、第9号の信者
は、黒色腫からの肺転移の部分的後退をした。これらの
患者は、転移疾患のかさ高い部位を有し、そして病変の
体積の50%の減少が見られたけれども完全に消えた病
変はなかった。 第11号の患者は、治療の完了後、2週間内に腎臓細胞
からの肺転移の部分的後退を示しく第10図)、第12
号の患者は、治療後2週間内に、大きい先端肺の腺癌の
治療に対する部分的応答を示したく第11図)、これら
の患者は、注意深く追跡され続けられており、完全な寛
解が見られないなら再治療されるだろう。 プロトコルが進むときになされた治療法の変更において
大きな因子が含まれた。これらは(1)IL−2投与の
間、末梢血液からのLAK前駆体の急速な消失があり、
そしてこうして白血球搬出は、IL−2が与えられた間
、許容しうる細胞収量を生じなかった。このプロトコル
において報告された12人の患者の治療の前に治療した
4人の患者から、IL−2注入の間、リンパ球を収穫す
る試みがなされた。最小数の細胞が得られた。これはI
L−2投与の間、循環系においてLAK前駆体が存在し
ない理由による。そしてこれら4人の患者のいずれも総
量で2X101’より多い細胞を受けなかった。この理
由により、白血球搬出はIL−2投与が中止された後に
行った。(2)IL−2投与の中止後、循環系のリンパ
球の数ならびにLAK前駆体の数において、目立ったは
ね返りがあった。したがって、プロトコルを進めるとき
IL−2は数日間投与され、それから白血球搬出の開始
のおよそ36時間前にIL−2投与を中止した。第3号
の患者におけるIL−2投与の関数としてのリンパ球分
画における変動の例を第12図に示す。こうしてIL−
2の投与およびリンパ球搬出を繰返すことにより、収穫
されたLAKII胞の数を非常に増加することが可能で
あった。1L−2の毒性は、与えることのできるIL−
2の量を制限した。動物モデルは、治療的効果が、注入
されたLAKの数および投与されたIL−2の量に直接
関係することを示した。IL−2投与に関係した毒性副
作用の理由により、(下記)、この臨床的プロトコルの
早い段階において治療されたほとんどの患者は、8時間
ごとに10.000単位/kQのIL−2を受けた。後
続の患者は30.000m位/kgを受け、第11号お
よび第12号の患者は、ioo、ooo単位/lagを
受けた。一般に、より多いtL−2薬量は、長期間良好
には耐えられないので、より少ない投与量で治療サイク
ルが行われた。 治療の毒性 これらの患者における治療の毒性を表3に示、す。 先の研究において、活性化されたキラー細胞だけの注入
は一時的な熱および寒けおよび肺拡散容最の一時的な減
少を伴うことが証明された( Rosenberg、
1984. J、 Biol 、 Re5p 、 M
od、 ;3:501−511 :Mazumder
、etal、1984゜Cancer ; 53 :
896−905) 、最近の研究における患者は、しば
しばIll胞注入の直後に寒けをもった。この寒けは、
メペリジン(25乃至50mg)又はモルフォリン(4
乃至1ng)の静脈投与によって、一般によく!IIl
[Iされる。 このプロトコルにおいて、IL−2投与に関係する主な
副作用は、おそらく毛管透過性の全身的増加から生ずる
体液保持による体重増加であった。 12人の患者の8人は開始時の体重の10%以上増加し
た。この体液保持は、柔らかい組織において最も深く、
目立った末梢水腫を生じたが、早期には肺を分けるよう
に見えた。IL−2の量を30、OOOおよびIOo、
000m位/kfll与、’した選択された患者の肺水
測定は、普通のυ1限内であった。しかしIL−2投与
のコースの遅くにおいて、体液保持は、しばしば、12
人の患者の9人において、放射性写真が呼吸困難に関係
する肺における間隙の水腫のように見える肋膜の流出お
よび腹水を生じた。これらの患者の2人(第1号および
第12号の患者)は、それぞれ1および4日の潜伏を要
するひどい呼吸窮迫を発生した。加えて、IL−2注入
は熱および倦怠に関係し、12人の患者の7人は全身の
紅斑性の皮膚発疹を発生した。熱、寒けおよび倦怠は、
アセトアミノフェノン(6時間毎に650m(1)およ
びインドメチシン(6時間毎に25mg)の使用によっ
て除去することができ、患者の多くはこれらの治療を受
けた。多くの患者は、それらの発疹の治療のための抗ヒ
スタミン剤であるヒドロキシジンヒドロクロリドをも受
けた。はとんどの患者に使用した睡眠薬はドキセビンで
あった。はとんどの患者は胃腸の出血の予防にラニチジ
ン(1日150ma経口)を続けた。低尿排出を生ずる
顕著な腎臓機能障害は、共通でない副作用で、12人の
患者の3人のみが2111(+/%以上の血清クレアチ
ン水準を発生した。12人の患者の5人は2 mQ/%
以上の一時的高ビリルリン血を発生した。好酸球増加は
共通で、12人の患者の11人が循環系好酸球を発生し
、これは全白血球(white cell)分画の5%
以上であり、数人の患者においては全循環系白細胞の8
0%以上はIL−2投与の高さで好酸球であった。IL
−2投与は造血抑制にも関係するようであった。全ての
患者は治療の間、貧血を直すために輸血を必要とし、1
2人の患者の4人は50゜OOO血小板/rAI11
以下の血小板減少症を発生した。 全ての患者において、不利となる効果はIL−2投与停
止後敏速に消失し、12人の患者は全て結局、追跡調査
のために退院を許された。一般に利尿は、1m−2の中
止後24時間以内に始まり、利尿剤の使用により促進す
ることができた。他の副作用は一般にIL−2を止めた
後4乃至5日以内に消失した。この報告において考慮し
ていない別の1人の患者は、細胞注入の第1のサイクル
の間に胸の痛みを発生し、治療は1サイクルの完了の前
に中止された。続きの研究は、心筋の虚血の診断を確認
しなかった。この患者は退院し再治療されなかった。 この研究は、癌患者の治療において、インターロイキン
−2(IL−2)と結合してのリンフォカイン活性化キ
ラー(LAK)細胞の使用の最初の報告である。この臨
床的試みに先立つものとして、相乗的しAKII胞およ
び組換え1m−2の投与が、2つの異なる系統のマウス
の免疫原的及び非免疫原的肉腫、メラノーマ性及び非メ
ラノーマ性黒色腫、マウス結腸腺癌、及びマウスぼうこ
う癌を含めて種々の移植可能マウスll11!における
樹立された肺及び肝臓の転移の後退を行うことが可能で
あるという証明がある(Mule 、 et at。 1984、5cience; 225 : 1487−
1489 ;Mule、 et at、1985. J
、 Immunol、 :135 :646−65
2)。加えて、ヒトにおいてこれらの治療を結合する前
に、26人の癌患者は活性化キラー細胞でのみ治療され
(L旺江旺1゜丁984. J、 8io1 、 Re
5p 、 Med、 ; 3 : 501−511
:Mazumder 、 et al、1984Can
cer :53:896−905>、そして39人の癌
患者はIL−2だけで治療された(L吐阻et at、
1985 J、 Immunol、 ; 134
:157−166)。このプロトコルで治療された1
2人の患者のうち、6人は彼らの転移路の客観的な寛解
を経験した。全ての患者は転移性感が進展し、外科、化
学療法及び放射線治療を含めて標準の治療に失敗してい
た。 試験管中の研究は、LAK細胞が広い範囲のヒトのl1
ll!標的を溶解することができるが、通常の細胞を溶
解しないことを証明してきた。ヒト腫瘍の41の連続単
細胞懸濁物の研究において、胃、卵巣、すい臓、及び結
腸並びに種々の肉腫及び黒色腫の腺癌を含めて36がL
AK細胞によって著しく溶解された(Raner e
tal、1985Cancer :55:1327−1
333)、LAK細胞によって認識された抗原について
はほとんど知られていないが、特定の理論に限定される
ことなく、それは形質変換された細胞上に遍在して分布
し新鮮な正常細胞上には分布しないと考えられる。マウ
ス及びヒトの両者におけるLAKwJ胞の前駆体は、非
T1非B゛ヌル″リンパ球であるらしい。マウスにおい
てLAK前駆体細胞はThy−1−1Ia−であり、ヒ
トでは前駆体細胞は1eu−1−1OKT−3−11e
u −7−7−1OK 1である。これらののLAKI
胞前駆体はしたがって細胞表面表現型においても及びそ
れらの溶解特異性においても天然キラー細胞と異なる(
Q rimmet at、 1982 J、 EX
I)、 Med、 :155 :1823−1841
)。天然キラー細胞は培養された標的細胞を溶解し、新
鮮な!!瘍標的への影響はもしあるとしても非常に少な
い。しかししAKエフェクタ細胞は、前駆体1胞と異な
り、Tm胞であるらしい。マウスにおいてThy−1抗
原を有するからである。こうしてLAK前駆体細胞は、
IL−2の影響下でT細胞に分化することができるT細
胞系統における原始細胞であるらしい。これらの細胞は
、形質転換された細胞に対する天然の免疫監視における
役割のための良い候補をつくったが、2正常なヒトにお
けるこれらLAKIII胞の生理的機能は知られていな
い。 マウスモデルは、LAK細胞及び組換えIL−2の両者
が抗腫瘍効果を行うのに必要であることを予言した(M
ule、etal、1984 5cienCe:225
: 1487−1489 :Mule、et at。 1985、J、Iia+uno1.; 135 :64
6−652)。非常に大愚のIL−2がマウスにおいて
抗腫瘍効果を媒介したが(Rosenberg et
al。 1985、J、Exa、Med、:161 :1169
−1188)、IL−2投与に関係する毒性の理由によ
りIL−2のそれらの薬量を達成することは不可能であ
る。こうして、マウスにおいて、しAK細胞のみの投与
及び組換えIL−2のみの少量の投与は、抗腫瘍効果が
なかった。これは先のヒトの研究で確証された。そこで
は、26人がLAK細胞だけを受け、39人はIL−2
だけを受けた(16人は天然ヒトIL−2を受け、23
人は組換えIL−2を受けた)。抗腫瘍効果は見られな

7−166)、マウスモデルが体内ではIL−2への連
続的接触が必要であることを予言したので、ヒトでは8
時間毎の静脈注射を含む投与方法が選ばれた。IL−2
の連続的注入を伴う早期の研究は、許された最大の投与
層においても循環系には検出できるレベルのIL−2が
見出されないことを示した。しかしポーラスな投与が8
時間毎に与えられた場合、高い血清レベルが数時間、見
られたく第2図〉。 ココテ示したように、LAKIlfm及ヒI L−2を
同時に与えた場合、患者に著しい抗腫瘍効果が見られた
。ここで報告した6つの客観的応答は、黒色腫、結腸直
腸癌、腎臓細胞癌及び肺の腺癌を含む4つの異なる組織
的タイプの腫瘍において生じた。体外におけるLAKI
胞の広い細胞溶解特性の理由により、この治療様式が多
数の組織的サブタイプの癌への適用可能性を有すること
は明らかである。6人の患者の5人は客観的部分的寛解
を達成しく腫瘍の体積が少なくとも50%減少)、1人
の患者は病気の完全な寛解を達成し、そして散在した黒
色腫の3つの治療の後6週間M瘍がない。5つの肺の黒
色腫の3つが完全に後退した第2号の患者において、こ
の消失した3つの転移も戻っていないが、部分的にしか
後退しなかった2つの転移は、およそ1月後再び成長を
始めた。これらの結果は、1IWA沈積の消失は長期の
II!瘍制開制御ずるが、部分的な腫瘍消失は、その後
腫瘍がすぐに成長することを示している。マウスモデル
において、拡張された調査が、LAK細胞溶解に抵抗す
るI!瘍の存在のためになされ、そしてそのような細胞
が存在することを証明することはできなかった。マウス
腫瘍モデルにおいて、LAK及びIL−2治療にもかか
わらず生き残った転移は、管内及び体内の両者で元のI
!IIIのようにLAK細胞溶解を受けやすい。 この治療を改良する他の方向がマウスモデルによって示
唆された。最少の騰瘍縁はバルクの疾患(bulk d
isease)よりも治療的LAK治療を受けやすく、
外科処置の直後のアジュバントとしてのこの治療の使用
−が示される。LAKIl[l胞及びIL−2での治療
は、宿主免疫適格に依存しないので(Mule et
al、1985. J、 l01iuno1.。 135:646−652)、本治療様式は化学治療及び
放射線治療との組合わせのために理想的であろう。加え
て、マウス実験は、同腫のLAKil[I胞が同系の細
胞のように治療的に効果的であること(Mule et
al、1985. J 、 l a+muno1.。 135 :646−652>、及びLAK細胞の直接経
口注入がLAK細胞の全身投与よりも効果的であるかも
しれないことを示している。さらに、本発明はLAK細
胞に限定されないことに注意すべきである。もちろん任
意の他の適当な免疫細胞も同様にすることができた。加
えて、これらのLAK又は同様な性質を有する免疫細胞
もまた、組織培養に広げることができ、そして本発明に
したがって使用することができた。さらに、単クローン
抗体は、LAK[l胞及びIL−2の両者を腫瘍部位に
向けるためにも使用することができた。 上記のような変更は限定的ではなく当業者の1人には容
易に明らかで提案される。 ここで説明したようなIL−2と結合した゛LAK細胞
の投与は、広い種類の腫瘍又は免疫感応条件への将来適
用可能性を伴う、癌治療への新しいアプローチである。 ヒトにおいて得られた結果のマウスとの類似性は、癌及
び他の免疫関連機能障害又は疾患の制御のための有効な
治療様式として、採用される免疫治療の利用性を高める
。 ここで説明した例及び態様は、説明の目的だけであるこ
と、及びこれを考慮すると種々の変形及び変化が当業者
には示唆され、それは本出願の精神及び範囲、及び特許
請求の範囲の範囲(SCOpe )に含まれることが理
解される。

Claims (8)
- (1)薬剤的に許容できる担体中の単離されたリンフォ
カイン活性化リンパ球を包含する組成物であつて、この
リンパ球が腫瘍を罹患するヒトにリンフォカインと共に
投与されたときこの腫瘍に対して反応的である組成物。 - (2)上記のリンパ球の数が1ml当り約 10^5から10^7細胞までの範囲に亙る特許請求の
範囲第1項記載の組成物。 - (3)上記のリンフォカインがインターロイキン2であ
る特許請求の範囲第1項記載の組成物。 - (4)癌に罹患するヒトに、有効量のリンフォカインお
よびリンフォカイン活性化リンパ球を投与することを包
含する癌の治療方法。 - (5)上記のリンフォカインがインターロイキン2であ
る特許請求の範囲第4項記載の方法。 - (6)上記の癌が、黒色腫、骨肉腫、肺腺癌、腎細胞癌
、結腸癌、直腸癌または肉腫である特許請求の範囲第5
項記載の方法。 - (7)日に多数回投与するインターロイキン2の量が、
体重1キログラム当り約1,000から10^6単位の
範囲に亙る特許請求の範囲第6項記載の方法。 - (8)リンパ球の量が、約10^6から10^1^2細
胞の範囲に亙る特許請求の範囲第6項記載の方法。
