Los implantes sólidos farmacéuticamente activos
que proporcionan liberación sostenida de un ingrediente activo son
capaces de proporcionar una concentración relativamente uniforme de
ingredientes activos en el cuerpo. Los implantes son

La Figura 1A muestra el perfil de liberación de
un fármaco hidrófobo procedente de un sistema de administración de

Se consigue una liberación controlada de
fármacos mediante una formulación mejorada de implantes
biodegradables de liberación lenta. La velocidad de liberación de un
fármaco desde un implante se modula por adición de un modulador de
liberación al implante. La liberación de un agente hidrófobo se
aumenta incluyendo un acelerador en el implante. El modulador de
liberación puede ser fisiológicamente inerte. Las formulaciones de
interés incluyen fármacos antiinflamatorios, por ejemplo,
glucocorticoides, NSAIDS, etc., combinados con un agente
oftalmológicamente activo.
La velocidad de liberación del agente
terapéuticamente activo se controla por la velocidad de transporte a
través de la matriz polímera del implante, y de la acción del
modulador. Modulando la velocidad de liberación, el agente se libera
a una velocidad en el intervalo de la dosis terapéutica, durante el
periodo de tiempo deseado, de manera que la velocidad de liberación
no variará en más del 100% durante el periodo de tiempo deseado, más
normalmente en no más del 50%. El agente se hace disponible para
el(los) sitio(s) específico(s) en
el(los) que se necesita el agente, y se mantiene a una dosis
efectiva.
El transporte del fármaco a través de la barrera
polímera también se afecta por la solubilidad del fármaco, carácter
hidrófilo del polímero, grado de reticulado del polímero, expansión
del polímero después de la absorción de agua de manera que haga a la
barrera polímera más permeable al fármaco, geometría del implante, y
similar. A elevadas cargas de fármaco, es decir, a una concentración
de carga por encima del umbral teórico de percolación, la teoría de
la percolación predice el potencial para el lixiviado del fármaco
procedente de la matriz del sistema de administración del fármaco.
En tales casos, los moduladores de liberación son útiles para
disminuir el proceso de lixiviado.
El modulador de liberación es un agente que
altera de una manera definida la liberación de un fármaco desde un
implante biodegradable. Para los agentes hidrófobos, es un
acelerador. Los aceleradores serán agentes hidrófilos, que se usan
en combinación con agentes hidrófobos para aumentar la velocidad de
liberación. Agentes hidrófilos son aquellos compuestos que tienen
una solubilidad en agua a temperatura ambiente de al menos
aproximadamente 100 \mug/ml. Agentes hidrófobos son aquellos
compuestos que tienen una solubilidad en agua a temperatura ambiente
menor que 100 \mug/ml.
Los agentes terapéuticamente activos que se
benefician de formulaciones con un modulador de liberación pueden
proceder de, pero sin limitarse a, las siguientes categorías
terapéuticas: inhibidores ACE; citoquinas endógenas que influyen en
la membrana basal; agentes que influyen en el crecimiento de las
células endoteliales; agente agonista o de bloqueo adrenérgico;
inhibidor de la aldosa reductasa; analgésico; anestésico;
antialérgico; antibacteriano; antifibrótico; antifúngico, por
ejemplo, anfoteracina B; antiglaucoma; antihiper o antihipotensivo;
antiinflamatorio; antineoplásico, antiprotozooico; antitumoral;

fármacos hidrófobos, que típicamente tienen un perfil de liberación

ejemplo, vincristina y vinblastina; metotrexato, ácido retinoico;
ciertos antibióticos, por ejemplo, ansamicinas tal como rifampin;
nitrofuranos tal como nifuroxacida; fármacos antiinflamatorios no
esteroideos, por ejemplo, diclofenaco, ketorolaco, flurbiprofeno,
naproxeno, suprofeno, ibuprofeno, aspirina; esteroides, etc.
Los esteroides son de especial interés, en
particular los compuestos esteroides con actividad antiinflamatoria,
es decir, glucocorticoides. Los glucocorticoides incluyen los

Se sabe que estos derivados de la hidrocortisona
tienen efectos terapéuticos significativos que son beneficiosos en
el tratamiento de enfermedades inflamatorias oculares, variando su
potencia y biotolerabilidad en función de sus sustituciones
químicas.
Los siguientes son ejemplos de glucocorticoides
que se han usado en el tratamiento de la inflamación ocular, y que
son de interés para usar en la presente invención: fosfato sódico de
dexametasona; fosfato sódico de prednisolona; acetato de
prednisolone; acetato de fluorometolona; dexametasona;
fluorometalona; y medrisona. De estos, se cree que dexametasona es
el más potente, y es por lo tanto un buen candidato para usar en un
sistema de administración intraocular de fármacos, porque una
pequeña velocidad de liberación del fármaco es suficiente para
establecer niveles terapéuticos de concentración en el ojo.
Triamcinolona es otro fármaco de interés para la administración
intraocular con liberación sostenida.
Los aceleradores pueden ser fisiológicamente
inertes, polímeros solubles en agua, por ejemplo, metil celulosa de
bajo peso molecular. En la presente invención se emplea
hidroxipropil celulosa (HPMC).
La cantidad de agente activo empleado en el
implante, individualmente o en combinación, variará ampliamente
dependiendo de la dosis efectiva requerida y de la velocidad de
liberación desde el implante. Normalmente, el agente será al menos
10 por ciento en peso del implante, y normalmente no más de 80, más
normalmente no más de aproximadamente 40 por ciento en peso del
implante. La cantidad de modulador de liberación empleado dependerá
del perfil de liberación deseado, de la actividad del modulador, y
del perfil de liberación del agente activo en ausencia de modulador.
Un agente que se libera muy lentamente o muy rápidamente requerirá
cantidades relativamente altas de modulador. Generalmente, el
modulador será al menos 10, más normalmente al menos aproximadamente
20 por ciento en peso del implante, y normalmente no más de
aproximadamente 50, más normalmente no más de aproximadamente 40 por
ciento en peso del implante.
Cuando se ha de emplear una combinación de
agentes activos, se determina el perfil de liberación deseado para
cada agente activo. Se añade un modulador fisiológicamente inerte
para controlar exactamente el perfil de liberación. La liberación
del fármaco proporcionará un nivel terapéutico para cada agente
activo.
La proporción exacta de modulador y agente
activo se determinará empíricamente formulando varios implantes que
tienen cantidades variables de modulador. Se usará un método
aprobado por USP para el ensayo de disolución o liberación para
medir la velocidad de liberación (USP 23; NF 18 (1995) páginas
1790-1798). Por ejemplo, usando el método a dilución
infinita, se añade una muestra pesada del dispositivo de
administración de fármaco a un volumen medido de una disolución que
contienen cuatro partes en peso de etanol y seis partes en peso de
agua desionizada, en el que el volumen de la disolución será tal que
la concentración del fármaco después de la liberación es menor que
5% de la saturación. La mezcla se mantiene a 37ºC y se agita
lentamente para mantener los implantes en suspensión. Se siguió la
aparición del fármaco disuelto como una función del tiempo por
diversos métodos conocidos en la técnica, tales como
espectrofotométricamente, HPLC, espectroscopía de masas, etc. La
concentración del fármaco después de 1 h en el medio es indicativa
de la cantidad de fármaco libre no encapsulado en la dosis, mientras
que el tiempo requerido para liberar el 90% del fármaco se relaciona
con la duración esperada de la acción de la dosis in vivo.
Normalmente, la liberación deberá estar exenta de grandes
fluctuaciones sobre un valor medio que permita una liberación
relativamente uniforme.
Normalmente, el implante se formulará para
liberar el(los) agente(s) activo(s) durante un
periodo de al menos aproximadamente 3 días, más normalmente de al
menos aproximadamente una semana, y normalmente no más de
aproximadamente un año, más normalmente no más de aproximadamente
tres meses. Para la mayor parte, la matriz del implante tendrá un
tiempo de vida fisiológico en el sitio del implante al menos igual
al periodo deseado de administración, normalmente al menos doble del
periodo deseado de administración y puede tener tiempos de vida de 5
a 10 veces el periodo deseado de administración. El periodo deseado
de liberación variará con el estado que se ha de tratar. Por
ejemplo, los implantes diseñados para cirugía
post-cataratas tendrán un periodo de liberación de
aproximadamente 3 días a 1 semana; el tratamiento de la uveitis
puede requerir la liberación durante un periodo de aproximadamente 4
a 6 semanas; mientras que el tratamiento para la infección por
citomegalovirus puede requerir la liberación durante 3 a 6 meses, o
más.
Los implantes son de dimensiones proporcionadas
con el tamaño y forma de la región seleccionada para sitio del
implante y no migrarán del sitio de inserción después de la
implantación. Los implantes pueden ser rígidos, o algo flexibles
para facilitar tanto la inserción del implante en el sitio diana
como la acomodación del implante. Los implantes pueden ser
partículas, láminas, parches, placas, fibras, microcápsulas y
similares, y pueden ser de cualquier tamaño o forma compatible con
el sitio de inserción seleccionado.
Los implantes pueden ser monolíticos, es decir,
que tienen el agente activo distribuido uniformemente por toda la
matriz polímera, o encapsulados, en los que se encapsula un
reservorio de agente activo por la matriz polímera. Debido a la
facilidad de fabricación, normalmente se prefieren los implantes
monolíticos sobre las formas encapsuladas. A pesar del mayor control
proporcionado por el tipo encapsulado, el tipo reservorio puede ser
beneficioso en algunas circunstancias en los que el nivel
terapéutico del fármaco cae dentro de una ventana estrecha. La
selección de la composición polímera que se ha de emplear variará
con el sitio de administración, el periodo de tratamiento deseado,
la tolerancia del paciente, la naturaleza de la enfermedad a tratar
y similar. Las características de los polímeros incluirán
biodegradabilidad en el sitio de la implantación, compatibilidad
con el agente de interés, facilidad de encapsulación, una semivida
en el ambiente fisiológico de al menos 7 días, preferiblemente mayor
de dos semanas, solubilidad en agua, y similar. El polímero
normalmente comprenderá al menos aproximadamente 10, más normalmente
al menos aproximadamente 20 por ciento en peso del implante, y puede
comprender tanto como aproximadamente 70 por ciento en peso o
más.
Las composiciones polímeras biodegradables que
se pueden emplear pueden ser ésteres o éteres orgánicos, que cuando
se degradan dan lugar a productos de degradación fisiológicamente
aceptables, incluyendo los monómeros. Pueden tener uso anhídridos,
amidas, ortoésteres o similares, por sí mismos o en combinación con
otros monómeros. Los polímeros serán polímeros de condensación. Los
polímeros pueden estar reticulados o no reticulados, normalmente no
más de un reticulado ligero, generalmente menos de 5%, normalmente
menos de 1%. Para la mayor parte, además de carbono e hidrógeno, los
polímeros incluirán oxígeno y nitrógeno, particularmente oxígeno. El
oxígeno puede estar presente como oxi, por ejemplo, hidroxi o éter,
carbonilo, por ejemplo, no oxo carbonilo, tal como éster del ácido
carboxílico, y similar. El nitrógeno puede estar presente como
amida, ciano y amina. Pueden tener uso los polímeros expuestos en
Heller, citado anteriormente.
Son de interés particular los polímeros de
ácidos carboxílicos hidroxialifáticos, homo- o
co-polímeros, y polisacáridos. Incluidos entre los
poliésteres de interés están los polímeros de ácido
D-láctico, ácido L-láctico, ácido
láctico racémico, ácido glicólico, policaprolactona, y sus
combinaciones. Empleando el L-lactato o
D-lactato, se consigue un polímero de biodegradación
lenta, mientras que la degradación se mejora sustancialmente con el
racémico. Son de particular interés los copolímeros de ácidos
glicólico y láctico, en los que la velocidad de biodegradación se
controla por la relación de ácido glicólico a láctico. El copolímero
que se degrada más rápidamente tiene aproximadamente cantidades
iguales de ácidos glicólico y láctico. Los homopolímeros, o
copolímeros que tienen relaciones distintas a la igualdad, son más
resistentes a la degradación.
Entre los polisacáridos estarán alginato
cálcico, y celulosas funcionalizadas, particularmente ésteres de
carboximetilcelulosa caracterizadas por ser insolubles en agua, un
peso molecular de aproximadamente 5 kD a 500 kD, etc. También se
pueden emplear hidrogeles biodegradables en los implantes de la
presente invención. Típicamente, los hidrogeles son un material
copolímero caracterizado por la capacidad de embeber un líquido.
Ejemplos de hidrogeles biodegradables que se pueden emplear están
descritos por Heller en: Hydrogels in Medicine and Pharmacy, N.A.
Peppes ed., Vol. III, CRC Press, Boca Raton, FL, 1987, páginas
137-149.
Se pueden preparar partículas en las que el
centro puede ser de un material y que la superficie tenga una o más
capas de la misma o diferente composición, en las que las capas
pueden estar reticuladas, ser de diferente peso molecular, diferente
densidad o porosidad, o similar. Por ejemplo, el centro podría
comprender un polilactato revestido con copolímero
polilactato-poliglicolato, de manera que aumente la
velocidad de la degradación inicial. La mayoría de las relaciones
empleadas de lactato a glicolato estarán en el intervalo de
aproximadamente 1:0,1 a 1:1. Alternativamente, el centro podría ser
poli(alcohol vinílico) revestido con polilactato, de manera
que con la degradación del polilactato, el centro se disolvería y se
arrastraría rápidamente fuera del sitio de la implantación.
Los implantes tiene uso en el tratamiento de
distintos estados en los que es conveniente emplear un depósito para
el agente activo, en el que el implante sirve como un depósito tal.
Por lo tanto, dependiendo del estado particular que se ha de tratar,
el implante se puede ser introducir en distintos lugares diferentes
del huésped en los que es conveniente tener un depósito de agente
activo, incluyendo en el ojo, sistema nervioso central, sistema
vascular, en los huesos, en la piel, en los músculos, en los oidos,
etc.
Es de particular interés la formulación de
implantes para usar en el tratamiento de estados, enfermedades,
tumores y trastornos oculares. Los implantes biodegradables pueden
implantarse en distintos sitios, dependiendo de la forma y
formulación del implante, el estado que se ha de tratar, etc. Sitios
adecuados incluyen la cámara anterior, cámara posterior, segmento
posterior, incluyendo la cavidad vítrea, espacio supracoroidal,
subconjuntiva, episcleral, intracorneal, epicorneal y esclera.
Sitios adecuado intrínsecos al vítreo comprenden el espacio
supracoroidal, la pars plana y similares. El supracoroide es un
espacio potencial que se encuentra entre la pared escleral interior
y el coroide opuesto. Los implantes que se introducen en el
supracoroide pueden administrar fármacos al coroide y a la retina
anatómicamente opuesta, dependiendo de la difusión del fármaco desde
el implante, la concentración del fármaco comprendido en el implante
y similar. Los implantes se pueden introducir sobre o en una región
avascular. La región avascular puede ser natural, tal como la pars
plana, o una región hecha avascular por métodos quirúrgicos y
químicos. Las regiones avasculares inducidas quirúrgicamente se
pueden producir en un ojo por métodos conocidos en la técnica tales
como ablación por láser, fotocoagulación, crioterapia, coagulación
por calor, cauterización y similar. Puede ser particularmente
deseable producir una región avascular tal sobre o cerca del sitio
de tratamiento deseado, particularmente cuando el sitio deseado de
tratamiento está distante de la pars plana o no es posible el
emplazamiento del implante en la pars plana. La introducción de los
implantes sobre una región avascular permitirá la difusión del
fármaco desde el implante y al interior del ojo y evitará la
difusión del fármaco en la corriente sanguínea.
Volviendo ahora a la Figura 2, se muestra una
vista de la sección transversal del ojo, ilustrando los sitios para
la implantación de acuerdo con la presente invención. El ojo
comprende un cristalino 16 y abarca la cámara vítrea 3. Adyacente a
la cámara vítrea 3 está la parte óptica de la retina 11. La
implantación puede ser intraretinal 11 o subretinal 12. La retina
está rodeada por el coroide 18. La implantación puede ser
intracoroidal o supracoroidal 4. Entre la parte óptica de la retina
y el cristalino, adyacente al vítreo, está la pars plana 19.
Rodeando el coroide 18 está la esclera 8. La implantación puede ser
intraescleral 8 o episcleral 7. La superficie externa del ojo es la
córnea 9. La implantación puede ser epicorneal 9 o
intra-corneal 10. La superficie interna del ojo es
la conjuntiva 6. Debajo de la conjuntiva está la cámara anterior 1,
debajo de la cual está el cristalino 16. La cámara posterior 2 rodea
el cristalino, como se muestra en la figura. Opuesto a la superficie
externa están los nervios ópticos, y las arterias y venas de la
retina. La implantación en los espacios meníngeos 13, el nervio
óptico 15 y el nervio intraóptico 14 permite la administración del
fármaco en el sistema nervioso central, y proporciona un mecanismo
mediante el cual se puede cruzar la barrera hematoencefálica.
Otros sitios de implantación incluyen la
administración de fármacos antitumorales para lesiones neoplásicas,
por ejemplo, tumor o área lesionada, por ejemplo, tejidos
circundantes, o en aquellas situaciones en las que se ha extirpado
la masa tumoral, tejido adyacente al tumor previamente extirpado y/o
en la cavidad resultante después de la extirpación del tumor. Los
implantes se pueden administrar de distintas formas, incluyendo
medios quirúrgicos, inyección, trocar, etc.
Se pueden emplear otros agentes en la
formulación para diversos propósitos. Por ejemplo, se pueden emplear
agentes tampón y conservantes. Los conservantes solubles en agua
que se pueden emplear incluyen bisulfito sódico, bisulfato sódico,
tiosulfato sódico, cloruro de benzalconio, clorobutanol, timerosal,
acetato de fenilmercurio, nitrato de fenilmercurio, metilparabén,
poli(alcohol vinílico) y alcohol feniletílico. Estos agentes
pueden estar presentes en cantidades individuales de
aproximadamente 0,001 a aproximadamente 5% en peso, y
preferiblemente aproximadamente 0,01 a aproximadamente 2%. Los
agentes tampón solubles en agua adecuados que se pueden emplear son
carbonato sódico, borato sódico, fosfato sódico, acetato sódico,
bicarbonato sódico, etc., como los aprobados por la FDA para la vía
de administración deseada. Estos agentes pueden estar presentes en
cantidades suficientes para mantener el pH del sistema entre 2 y 9,
y preferiblemente 4 a 8. Un agente tampón tal puede ser tanto como
5% sobre la base de peso a peso de la composición total. Cuando el
agente tampón o el potenciador es hidrófilo, también puede actuar
como un acelerador de liberación, y tendrá un efecto acumulativo con
el(los) otro(s) modulador(es).
Los implantes pueden ser de cualquier geometría,
incluyendo fibras, láminas, películas, microesferas, esferas,
discos circulares, placas y similar. El límite superior para el
tamaño del implante se determinará por factores tales como la
tolerancia al implante, limitaciones de tamaño en la inserción,
facilidad de manejo, etc. Cuando se emplean láminas o películas,
para facilidad de manejo las láminas o películas estarán en el
intervalo de al menos aproximadamente 0,5 mm x 0,5 mm, normalmente
aproximadamente 3-10 mm x 5-10 mm
con un espesor de aproximadamente 0,25-1,0 mm.
Cuando se emplean fibras, el diámetro de la fibra estará
generalmente en el intervalo de 0,05 a 3 mm. La longitud de la
fibra estará generalmente en el intervalo de 0,5-10
mm. Las esferas estarán en el intervalo de 2 \mum a 4 mm en
diámetro, con volúmenes comparables para otras partículas
conformadas.
El tamaño y forma del implante se puede usar
para controlar la velocidad de liberación, periodo de tratamiento,
y concentración del fármaco en el sitio de la implantación. Los
implantes grandes administrarán una dosis proporcionalmente mayor,
pero dependiendo de la relación superficie a masa, pueden tener una
velocidad de liberación más lenta. Se elegirá el tamaño y geometría
particular de un implante que mejor se adapte al sitio de la
implantación. Las cámaras, por ejemplo, la cámara anterior, cámara
posterior y cámara vítrea, son capaces de acomodar implantes
relativamente grandes de diversas geometrías, que tienen diámetros
de 1 a 3 mm. Es preferible una lámina o disco circular para la
implantación en el espacio supracoroidal. El espacio restringido
para la implantación intraretinal requiere implantes relativamente
pequeños, que tienen diámetros de 0,05 a 1 mm.
En algunas situaciones, se pueden utilizar
mezclas de implantes empleando los mismos o diferentes agentes
farmacológicos. De esta forma, se alcanza un cóctel de perfiles de
liberación, dando una liberación bifásica o trifásica con una única
administración, en el que el modelo de liberación puede variar
mucho.
Se pueden emplear diversas técnicas para
producir los implantes. Las técnicas útiles incluyen métodos de
evaporación del disolvente, métodos de separación de fases, métodos
interfaciales, métodos de extrusión, métodos de moldeo, métodos de
moldeo por inyección, métodos de prensado en caliente y similares.
Los métodos específicos se tratan en la patente de EE.UU. 4.997.652.
En una realización preferida, los métodos de extrusión son útiles
para evitar la necesidad de disolventes en la fabricación. Cuando se
usan métodos de extrusión, el polímero y el fármaco se eligen para
que sean estables a las temperaturas requeridas para la fabricación,
normalmente al menos aproximadamente
85ºC.

Se midió la liberación del fármaco hidrófobo
dexametasona desde un sistema de administración de fármacos de
liberación prolongada. Se fabricó el sistema de administración de
fármacos con dexametasona y copolímero de poli(ácido
láctico)/poli(ácido glicólico). Se mezcló minuciosamente polvo de
dexametasona y un polvo de copolímero de poli(ácido
láctico)/poli(ácido glicólico) (PLGA) en una relación 50/50. El
polvo bien mezclado se colocó en una extrusora, se calentó durante
1 hora a 95ºC, y se extruyó a través de un orificio de calibre 20.
Se cortaron de los filamentos extruidos seis DDS de aproximadamente
100-120 \mug para la valoración de la liberación
del
fármaco.

Se colocó cada DDS individual en un vial de
vidrio relleno con medio receptor (NaCl al 9% en agua). Para
permitir las condiciones de "dilución infinita", se eligió el
volumen medio del receptor de manera que la concentración nunca
excediera el 5% de la saturación. Para minimizar el fenómeno de
transporte secundario, por ejemplo, polarización de la
concentración en la capa límite estancada, cada uno de los viales se
colocó en un baño de agua agitada a 37ºC. Se tomaron muestras para
análisis por HPLC de cada vial a puntos temporales definidos. El
método de HPLC fue como se describe en el documento USP 23 (1995)
páginas 1791-1798. Se usaron los valores de
concentración para calcular los perfiles de liberación acumulativos.
El perfil de liberación se muestra en la Figura 1A. Se ve que la
liberación del fármaco es muy lenta con este DDS. La liberación
apreciable del fármaco comienza en la cuarta semana después de la
iniciación, aproximadamente en el momento de la desintegración del
polímero.
Se fabricó un sistema de administración de
fármacos como se describió anteriormente, excepto que se incluyeron
diversas concentraciones de hidroxipropilmetilcelulosa (HPMC)
hidrófila como un modificador de liberación. Se usaron las
combinaciones de fármaco, polímero y HPMC mostrados en la Tabla
1.
Se ensayó la liberación del fármaco como se
describió anteriormente. Los datos se muestran en la Figura 1B. Se
ve que con la adición de HPMC hay un aumento pronunciado en la
velocidad de liberación. Se observa liberación próxima a orden cero
para XT014 y XT015, en los que la relación de modulador de
liberación a fármaco es 0,3 a 0,4. Por selección del polímero y
modificador de liberación apropiados, se puede diseñar a medida del
paciente la liberación del fármaco y el intervalo de administración
para proporcionar un perfil de liberación que está acelerado o
retardado.
Es evidente por los resultados anteriores que
los implantes biodegradables formulados con un agente activo y un
modulador de liberación proporcionan cinéticas de liberación en las
que el fármaco se libera a una velocidad constante durante un
periodo largo de tiempo, evitando la necesidad de que se administre
al paciente los fármacos de formas mucho menos efectivas, tal como
tópicamente. Los implantes proporcionan un método mejorado para
tratar los estados oculares y otros, evitando los picos y simas de
la liberación de fármacos.
Claims (4)
1. Un implante para la liberación sostenida de
fármacos que comprende:
- un polímero biodegradable farmacológicamente aceptable en una cantidad de al menos 10 por ciento en peso del implante;
- un agente terapéuticamente activo a una concentración de 10 a 80 por ciento en peso del implante; y
- hidroxipropilmetilcelulosa a una concentración de 10 a 50 por ciento en peso del implante;
- en el que dicho agente terapéuticamente activo se libera en una dosis terapéutica que no varía en más del 100% durante un periodo de al menos 3 días, y
- en el que dicho agente terapéuticamente activo es hidrófobo en el sentido de que tiene una solubilidad en agua a temperatura ambiente menor que 100 \mug/ml.
2. Un implante según la reivindicación 1, en el
que dicho polímero biodegradable farmacológicamente aceptable es un
poliéster de ácido láctico y ácido glicólico.
3. Un implante según la reivindicación 2, que
comprende
- el poliéster de ácido láctico y ácido glicólico en una concentración de al menos 20 por ciento en peso del implante,
- un agente terapéuticamente activo a una concentración de 10 a 50 por ciento en peso del implante, en el que el agente terapéuticamente activo es un glucocorticoide.
4. Un implante según la reivindicación 3, en el
que el glucocorticoide es dexametasona.
