- Technisches Gebiet
- Biologisch abbaubare Implantate, welche für eine gesteuerte, verzögerte Arzneimittelfreisetzung gestaltet wurden.
- Hintergrund der Erfindung
- Feste, pharmazeutisch wirksame Implantate, welche eine verzögerte Freisetzung eines Wirkstoffs ermöglichen, können eine relativ einheitliche Konzentration an Wirkstoffen im Körper bereitstellen. Implantate sind besonders nützlich zur Bereitstellung einer hohen lokalen Konzentration an einer bestimmten Zielstelle für längere Zeiträume. Diese Formen einer verlängerten Freisetzung verringern die zu verabreichende Anzahl der Dosierungen des Arzneimittels und vermeiden die Spitzen und Senken der Arzneimittelkonzentration, welche bei traditionellen Arzneimitteltherapien zu finden sind. Die Verwendung eines biologisch abbaubaren Systems der Arzneimittelzufuhr weist den weiteren Vorteil auf, dass das verbrauchte Implantat nicht von der Zielstelle entfernt werden muss.
- Viele der erwarteten Vorteile von Implantaten mit einer verzögerten Freisetzung hängen von einer verzögerten Freisetzung mit einem relativ konstanten Spiegel ab. Aber Formulierungen von hydrophoben Arzneimitteln mit biologisch abbaubaren Matrizen können ein Freisetzungsprofil aufweisen, welches eine geringe oder keine Freisetzung zeigt, bis eine Abtragung der Matrix auftritt, zu diesem Zeitpunkt kommt es zu einer Ausschüttung des Arzneimittels.
- Bei der Formulierung von implantierbaren Arzneimitteln ist das Auge von besonderem Interesse, da man das Ausmaß der erforderlichen chirurgischen Manipulation verringern kann und wirksame Mengen eines Arzneimittels spezifisch im Auge bereitstellen kann. Wenn eine Lösung direkt in das Auge injiziert wird, wird das Arzneimittel schnell ausgewaschen oder wird vom Innern des Auges in den allgemeinen Kreislauf abgereichert. Vom therapeutischen Standpunkt aus kann dies ebenso nutzlos sein wie wenn überhaupt kein Arzneimittel verabreicht wird. Aufgrund dieser inhärenten Schwierigkeit der Zufuhr von Arzneimittel in das Auge ist die erfolgreiche medizinische Behandlung von Augenerkrankungen unzureichend.
- Verbesserte Formulierungen mit einer verzögerten Freisetzung, welche eine konstante Freisetzungsrate des Arzneimittels ermöglichen, sind für medizinische und veterinärmedizinische Verwendungen von beträchtlichem Interesse.
- Relevante Literatur
- Die US-Patente 4,997,652 und 5,164,188 offenbaren biologisch verträgliche Implantate zum Einbringen in eine Vorderkammer oder ein hinteres Segment eines Auges zur Behandlung einer Augenerkrankung.
- Heller, "Biodegradable Polymers in Controlled Drug Delivery" beschreibt in "CRC Critical Reviews in Therapeutic Drug Carrier Systems", Vol. 1, CRC Press, Boca Raton, FL, 1987, S. 39–90 eine Einkapselung für eine gesteuerte bzw. regulierte Arzneimittelzufuhr. - Anderson et al. beschreiben in "Contraception" (1976) 13: 375 und Miller et al. beschreiben in J. Biomed. Materials Res. (1977) 11: 711 verschiedene Eigenschaften von Poly(dL-milchsäure). Im US-Patent 5,075,115 werden Formulierungen für eine verzögerte Freisetzung mit Milchsäurepolymeren und -copolymeren offenbart.
- Di Colo beschreibt 1992 in "Biomaterials" 13: 850–856 eine gesteuerte bzw. regulierte Arzneimittelfreisetzung aus hydrophoben Polymeren.

EP 0 311 065 undUS 4,304,765 beschrieben. - Zusammenfassung der Erfindung
- Es werden Zusammensetzungen und Verfahren für biologisch abbaubare Implantate bereitgestellt, welche formuliert sind, um eine gesteuerte bzw. regulierte, verzögerte Arzneimittelfreisetzung bereitzustellen. Die Freisetzungsrate wird angepasst durch Kombination von hydrophoben und hydrophilen Mitteln in dem Implantat. Erfindungsgemäß wird ein Implantat bereitgestellt, wie in Anspruch 1 definiert.

- Eine gesteuerte Arzneimittelfreisetzung wird erreicht durch eine verbesserte Formulierung von biologisch abbaubaren Implantaten mit langsamer Freisetzung. Die Freisetzungsrate eines Arzneimittels aus einem Implantat wird angepasst durch Zugabe eines Freisetzungsmodulators zum Implantat. Die Freisetzung eines hydrophoben Mittels wird erhöht durch Einschluss eines Beschleunigers in das Implantat. Der Freisetzungsmodulator kann physiologisch inert sein. Interessante Formulierungen umfassen entzündungshemmende Arzneimittel, z.B. Glukocorticoide, NSAIDS usw., kombiniert mit einem ophthalmischen Wirkstoff.
- Die Freisetzungsrate des therapeutischen Wirkstoffs wird gesteuert durch die Rate des Transports durch die polymere Matrix des Implantats und die Wirkung des Modulators. Durch Modulation der Freisetzungsrate wird das Mittel mit einer Rate innerhalb eines therapeutischen Dosierungsbereichs über den gewünschten Zeitraum freigesetzt, so dass die Freisetzungsrate über den gewünschten Zeitraum nicht um mehr als 100% variiert, insbesondere nicht um mehr als 50%. Das Mittel wird verfügbar an der speziellen Stelle (den speziellen Stellen), an welchen das Mittel benötigt wird und wird in einer wirksamen Dosierung aufrechterhalten.
- Der Transport des Arzneimittels durch die Polymerbarriere wird auch beeinflusst durch die Löslichkeit des Arzneimittels, die Hydrophilizität des Polymers, das Ausmaß der Vernetzung des Polymers, die Ausdehnung des Polymers bei Wasserabsorption, so dass die Polymerbarriere für das Arzneimittel permeabler wird, durch die Geometrie des Implantats und dergleichen. Bei hohen Arzneimittelbeladungen, d.h. bei einer Beladungskonzentration oberhalb des theoretischen Perkolationsschwellenwerts, sagt die Perkolationstheorie die Möglichkeit des Auslaufens des Arzneimittels aus der Matrix des Arzneimittelzufuhrsystems voraus. In solchen Fällen sind Freisetzungsmodulatoren nützlich, um den Auslaufprozess zu verlangsamen.
- Der Freisetzungsmodulator ist ein Mittel, welches die Freisetzung eines Arzneimittels aus einem biologisch abbaubaren Implantat auf definierte Art und Weise verändert. Bei hydrophoben Mitteln ist es ein Beschleuniger. Beschleuniger sind hydrophile Verbindungen, welche in Kombination mit hydrophoben Mitteln verwendet werden, um die Freisetzungsrate zu steigern. Hydrophile Mittel sind diejenigen Verbindungen, welche bei Umgebungstemperatur in Wasser eine Löslichkeit von mindestens etwa 100 μg/ml aufweisen. Hydrophobe Mittel sind diejenigen Verbindungen, welche bei Umgebungstemperatur in Wasser eine Löslichkeit von weniger als 100 μg/ml aufweisen.
- Therapeutische Wirkstoffe, welche von einer Formulierung mit einem Freisetzungsmodulator profitieren, können aus den folgenden therapeutischen Klassen stammen, sind aber nicht darauf beschränkt: Ace-Inhibitor, endogene Cytokine, welche die Basalmembran beeinflussen, Mittel, welche das Wachstum von Endothelialzellen beeinflussen, adrenerge Agonisten oder Blocker, Aldosereductoseinhibitor, Analgetikum, Anästhetikum, Antiallergikum, antibakterielles Mittel, antifibrotisches Mittel, Antipilzmittel, z.B. Amphoteracin B, Antiglaukommittel, antihypertensives oder hypotensives Mittel, entzündungshemmendes Mittel, antineoplastisches Mittel, antiprotozoisches Mittel, Antitumormittel, Antivirusmittel, Carbonanhydraseinhibitor, chelatbildende Mittel, cholinerge Mittel, Cholinesteraseinhibitor, CNS-Stimulationsmittel, Verhütungsmittel, Dopaminrezeptoragonist oder -antagonist, Östrogen, Glukocorticoid, Glucosidaseinhibitor, Freisetzungsfaktor, Wachstumshormoninhibitor, Wachstumsstimulationsmittel, hämolytisches Mittel, Heparinantagonist, Immunmodulator, Immunsuppressionsmittel, LH-RH-Agonist, antimitotisches Mittel, NSAID, Progesteron, thrombolytisches Mittel, Vasodilatator, vasopressorisches Mittel und Vitamin. Unter den hydrophoben Arzneimitteln, welche normalerweise ein langsames Freisetzungsprofil aufweisen und deswegen von einer Formulierung mit einem Freisetzungsbeschleuniger profitieren, sind Cyclosporine, z.B. Cyclosporin A, Cyclosporin G usw., Vinca-Alkaloide, z.B. Vincristin und Vinblastin, Methotrexat, Retinolsäure, bestimmte Antikörper, z.B. Ansamcyine wie etwa Rifampin, Nitrofurane wie etwa Nitrofuroxazid, nicht steroide entzündungshemmende Arzneimittel, z.B. Diclofenac, Keterolac, Flurbiprofen, Naproxen, Suprofen, Ibuprofen, Aspirin, Steroide usw.
- Steroide sind von besonderem Interesse, insbesondere Steroidverbindungen mit einer entzündungshemmenden Wirkung, z.B. Glukocorticoide. Glukocorticoide umfassen die folgenden:
- Es ist erkannt worden, dass diese Hydrocortisonderivate signifikante therapeutische Wirkungen aufweisen, welche bei der Behandlung von entzündlichen Augenerkrankungen vorteilhaft sind, welche in ihrer Potenz und biologischen Verträglichkeit als Funktion ihrer chemischen Substitutionen variieren.
- Die folgenden Beispiele sind Beispiele für Glukocorticoide, welche bei der Behandlung einer Augenentzündung verwendet worden sind und für eine Verwendung im Gegenstand der Erfindung von Interesse sind: Dexamethasonnatriumphosphat, Prednisolonnatriumphosphat, Prednisolonacetat, Fluormetholonacetat, Dexamethason, Fluormethalon und Medryson. Es wird angenommen, dass Dexamethason von diesen das wirksamste ist und deshalb ein guter Kandidat für die Verwendung in einem intraokularen Arzneimittelzufuhrsystem, da eine kleine Arzneimittelfreisetzungsrate ausreichend ist, um innerhalb des Auges therapeutische Konzentationsmengen zu etablieren. Triamcinolon ist ein anderes interessantes Arzneimittel für eine intraokulare Verabreichung mit verzögerter Freisetzung.
- Beschleuniger können physiologisch inerte, wasserlösliche Polymere sein, z.B. Methylcellulose mit niedrigem Molekulargewicht. Erfindungsgemäß wird Hydroxypropylmethylcellulose (HPMC) verwendet.
- Die in dem Implantat verwendete Menge an Wirkstoff, einzeln oder in Kombination, variiert in Abhängigkeit von der erforderlichen wirksamen Dosierung und der Freisetzungsrate aus dem Implantat. Normalerweise macht das Mittel mindestens 10 Gewichtsprozent des Implantats aus, und normalerweise nicht mehr als 80, insbesondere nicht mehr als etwa 40 Gewichtsprozent des Implantats. Die Menge an verwendetem Freisetzungsmodulator ist abhängig von dem gewünschten Freisetzungsprofil, der Wirksamkeit des Modulators und dem Freisetzungsprofil des Wirkstoffs ohne Modulator. Ein Mittel, welches sehr langsam oder sehr schnell freigesetzt wird, erfordert relativ hohe Mengen an Modulator. Im Allgemeinen macht der Modulator mindestens 10, insbesondere mindestens etwa 20 Gewichtsprozent des Implantats aus, und normalerweise nicht mehr als etwa 50, insbesondere nicht mehr als etwa 40 Gewichtsprozent des Implantats.
- Wenn eine Kombination an Wirkstoffen verwendet wird, wird das gewünschte Freisetzungsprofil jedes Wirkstoffs bestimmt. Um das Freisetzungsprofil genau zu steuern/regulieren, wird ein physiologisch inerter Modulator zugegeben. Die Freisetzung des Arzneimittels stellt eine therapeutische Menge jedes Wirkstoffs bereit.
- Die genauen Anteile von Modulator und Wirkstoff werden empirisch bestimmt durch Formulieren von mehreren Implantaten mit unterschiedlichen Mengen an Modulator. Es wird ein USP-zugelassenes Verfahren für einen Auflösungsversuch oder Freisetzungsversuch verwendet, um die Freisetzungsrate zu messen (USP 23, NF 18 (1995) S. 1790–1798). Beispielsweise wird unter Verwendung der "infinite sink method" eine gewogene Probe der Arzneimittelzufuhrvorrichtung zu einem gemessenen Volumen einer Lösung zugegeben, welche 4 Gewichtsteile Ethanol und 6 Gewichtsteile entionisiertes Wasser enthält, wobei das Lösungsvolumen so ist, dass die Arzneimittelkonzentration nach der Freisetzung kleiner ist als 5% Sättigung. Das Gemisch wird bei 37°C gehalten und langsam gerührt, um die Implantate in Suspension zu halten. Das Auftreten des gelösten Arzneimittels als eine Funktion der Zeit kann durch verschiedene im Fachgebiet bekannte Verfahren nachvollzogen werden, wie etwa beispielsweise spektrophotometrisch, durch HPLC, durch Massenspektrometrie usw. Die Arzneimittelkonzentration im Medium nach 1 h weist auf die Menge des freien nicht verkapselten Arzneimittels in der Dosis hin, während die Zeit, welche erforderlich ist, damit 90% des Arzneimittels freigesetzt sind, auf die erwartete Wirkungsdauer der Dosis in vivo bezogen wird. Normalerweise ist die Freisetzung frei von größeren Fluktuationen im Hinblick auf den Mittelwert, was eine relativ einheitliche Freisetzung ermöglicht.
- Normalerweise ist das Implantat so formuliert, dass der Wirkstoff (die Wirkstoffe) über einen Zeitraum von mindestens etwa 3 Tagen, insbesondere mindestens etwa 1 Woche und normalerweise nicht mehr als etwa 1 Jahr, insbesondere nicht mehr als etwa 3 Monaten, freigesetzt wird. In den meisten Fällen besitzt die Matrix des Implantats an der Implantationsstelle eine physiologische Lebensdauer, welche mindestens gleich ist wie der gewünschte Verabreichungszeitraum, normalerweise mindestens das Zweifache des gewünschten Verabreichungszeitraums, und normalerweise weist sie Lebensdauern auf, welche das Fünf- bis Zehnfache des gewünschten Verabreichungszeitraums betragen. Die gewünschte Freisetzungsdauer variiert mit der zu behandelnden Erkrankung. Implantate, welche beispielsweise für eine Verwendung nach einer Kataraktchirurgie gestaltet sind, weisen eine Freisetzungsdauer von etwa 3 Tagen bis 1 Woche auf, die Behandlung von Uveitis kann eine Freisetzung über einen Zeitraum von etwa 4–6 Wochen erfordern, während die Behandlung einer Zytomegalievirusinfektion eine Freisetzung über 3–6 Monate oder länger erfordern kann.
- Die Implantate haben eine Größenordnung, welche der Größe und Form der Region entspricht, welche als Implantationsstelle ausgewählt ist, und wandern nach der Implantation nicht von der Insertionsstelle weg. Die Implantate können steif oder etwas flexibel sein, so dass sowohl die Insertion des Implantats an der Zielstelle als auch die Anpassung des Implantats erleichtert werden. Die Implantate können Partikel, Folien, Stücke, Platten, Fasern, Mikrokapseln und dgl. sein und können eine beliebige Größe oder Form aufweisen, welche mit der ausgewählten Insertionsstelle kompatibel ist.
- Die Implantate können monolithisch sein, d.h. dass der Wirkstoff homogen in der Polymermatrix verteilt ist, oder sie können verkapselt sein, wobei ein Reservoir an Wirkstoff durch die Polymermatrix verkapselt wird. Aufgrund der einfachen Herstellung werden monolithische Implantate normalerweise gegenüber verkapselten Formen bevorzugt. Aber die größere Steuerung/Regulierung, welche durch den Typ des verkapselten Reservoirs gewährleistet wird, kann unter manchen Umständen von Vorteil sein, bei welchen der therapeutische Arzneimittelspiegel in ein enges Fenster fällt. Die Auswahl der verwendeten polymeren Zusammensetzung variiert mit der Verabreichungsstelle, der gewünschten Behandlungsdauer, der Toleranz des Patienten, der Art der zu behandelnden Erkrankung und dergleichen. Die Charakteristika der Polymere umfassen die biologische Abbaubarkeit an der Implantationsstelle, die Verträglichkeit mit dem interessierenden Mittel, die Leichtigkeit der Verkapselung, die Halbwertszeit in der physiologischen Umgebung von mindestens 7 Tagen, bevorzugt mehr als 2 Wochen, die Wasserlöslichkeit und dergleichen. Das Polymer umfasst normalerweise mindestens etwa 10, insbesondere mindestens etwa 20 Gewichtsprozent des Implantats, und kann mehr als etwa 70 Gewichtsprozent oder mehr umfassen.
- Biologisch abbaubare polymere Zusammensetzungen, welche verwendet werden können, können organische Ester oder Ether sein, welche beim Abbau physiologisch akzeptable Abbauprodukte ergeben, einschließlich der Monomere. Anhydride, Amide, Orthoester oder dergleichen können als solche oder in Kombination mit anderen Monomeren Verwendung finden. Die Polymere sind Kondensationspolymere. Die Polymere können vernetzt oder nicht vernetzt sein, normalerweise sind sie nicht mehr als leicht vernetzt, im Allgmeinen weniger als 5%, normalerweise weniger als 1%. Meistens umfassen die Polymere neben Kohlenstoff und Wasserstoff Sauerstoff und Stickstoff, insbesondere Sauerstoff. Der Sauerstoff kann als Oxy vorliegen, z.B. als Hydroxy oder Ether, als Carbonyl, z.B. Nicht-Oxo-Carbonyl, wie etwa als Carbonsäureester und dergleichen. Der Stickstoff kann als Amid, Cyano und Amino vorliegen. Es können die Polymere, welche von Heller (s. oben) angegeben sind, Verwendung finden.
- Von besonderem Interesse sind Polymere von hydroxyaliphatischen Carbonsäuren, entweder Homo- oder Copolymere und Polysaccharide. Umfasst von den interessanten Polymeren werden Polymere von D-Milchsäure, L-Milchsäure, racemischer Milchsäure, Glykolsäure, Polycaprolacton und Kombinationen davon. Durch Verwendung des L-Lactats oder D-Lactats wird ein langsam biologisch abbaubares Polymer erreicht, während der Abbau im Wesentlichen mit dem Racemat verstärkt wird. Copolymer von Glykol- und Milchsäure sind von besonderem Interesse, wobei die Rate der biologischen Abbaubarkeit durch das Verhältnis von Glykol- zu Milchsäure gesteuert bzw. reguliert wird. Das am schnellsten abgebaute Copolymer besitzt ungefähr gleiche Mengen von Glykol- und Milchsäure. Homopolymere oder Copolymere mit Verhältnissen, welche nicht gleich sind, sind gegenüber einem Abbau resistenter.
- Unter den Polysacchariden ist Calciumalginat und funktionalisierte Cellulosen, insbesondere Carboxymethylcelluloseester, gekennzeichnet dadurch, dass sie wasserunlöslich sind, mit einem Molekulargewicht von etwa 5 kDa bis 500 kDa usw. Es können auch biologisch abbaubare Hydrogele in den Implantaten der vorliegenden Erfindung verwendet werden. Hydrogele stellen normalerweise ein Copolymermaterial dar, gekennzeichnet durch die Fähigkeit, eine Flüssigkeit aufzunehmen. Beispielhafte biologisch abbaubare Hydrogele, welche verwendet werden können, werden von Heller in "Hydrogels in Medicine and Pharmacy", N. A. Peppes Ed., Vol. III, CRC Press, Boca Raton, FL, 1987, S. 137–149 beschrieben.
- Es können Partikel hergestellt werden, wobei die Mitte aus einem Material bestehen kann und die Fläche eine oder mehrere Schichten mit gleicher oder unterschiedlicher Zusammensetzung aufweisen kann, wobei die Schichten vernetzt sein können, ein unterschiedliches Molekulargewicht, eine unterschiedliche Dichte oder Porosität oder dergleichen aufweisen können. Beispielsweise kann die Mitte ein Polylactat umfassen, beschichtet mit einem Polylactat-Polyglykolat-Copolymer, um so die Rate des anfänglichen Abbaus zu verstärken. Die meisten verwendeten Verhältnisse von Lactat zu Glykolat liegen im Bereich von etwa 1:0,1 bis 1:1. Alternativ kann die Mitte ein Polyvinylalkohol sein, beschichtet mit Polylactat, so dass sich das Zentrum beim Abbau des Polylactats auflöst und schnell aus der Implantationsstelle herausgewaschen wird.
- Die Implantate finden Verwendung bei der Behandlung einer Vielzahl von Erkrankungen, bei welchen es dienlich ist, ein Depot für den Wirkstoff zu verwenden, wobei das Implantat als solches als ein Depot dient. In Abhängigkeit von der bestimmten zu behandelnden Erkrankung kann das Implantat deshalb an eine Vielzahl von unterschiedlichen Orten des Wirtes eingebracht werden, wo es dienlich ist, ein Wirkstoffdepot zu besitzen, einschließlich im Auge, im zentralen Nervensystem, im Gefäßsystem, in den Knochen, in der Haut, in den Muskeln, in den Ohren, usw.
- Die Formulierung von Implantaten für eine Verwendung bei der Behandlung von Augenerkrankungen, Leiden, Tumoren und Krankheiten ist von besonderem Interesse. Die biologisch abbaubaren Implantate können an verschiedenen Stellen implantiert werden, in Abhängigkeit von der Form und Formulierung des Implantats, der zu behandelnden Erkrankung usw. Geeignete Stellen umfassen die Vorderkammer, die hintere Kammer, das vordere Segment, einschließlich des Glaskörpers, den Suprachoroidalraum, den subkonjunktivalen, episkleralen, intracornealen, epicornealen Bereich und die Sklera. Geeignete Stellen außerhalb des Glaskörpers umfassen den Suprachoroidalraum, die Pars plana und dergleichen. Das Suprachoroid ist ein möglicher Raum, welcher zwischen der inneren Sklerawand und dem gegenüberliegenden Choroid liegt. Implantate, welche in das Suprachoroid eingebracht werden, können Arzneimittel zu dem Choroid und zu der anatomisch gegenüberliegenden Retina zuführen, in Abhängigkeit von der Diffusion des Arzneimittels aus dem Implantat, der Konzentration des im Implantat enthaltenen Arzneimittels und dergleichen. Implantate können über oder in eine avaskuläre Region eingebracht werden. Die avaskuläre Region kann natürlich vorkommend sein, wie etwa die Pars plana, oder es kann eine Region sein, welche durch chirurgische und chemische Verfahren avaskulär gemacht wurde. Chirurgisch induzierte avaskuläre Regionen können in einem Auge erzeugt werden durch Verfahren, welche im Stand der Technik bekannt sind, wie etwa Laserablation, Photokoagulation, Kryotherapie, Wärmekoagulation, Kauterisieren und dergleichen. Es kann besonders wünschenswert sein, solch eine avaskuläre Region über oder nahe der gewünschten Behandlungsstelle zu erzeugen, insbesondere wenn die gewünschte Behandlungsstelle von der Pars plana entfernt ist oder eine Platzierung des Implantats an der Pars plana nicht möglich ist. Das Einbringen von Implantaten über eine avaskuläre Region ermöglicht die Diffusion des Arzneimittels aus dem Implantat und in das innere Auge und vermeidet eine Diffusion des Arzneimittels in den Blutstrom.
- Nun wird auf
2 hingewiesen, worin ein Querschnitt des Auges gezeigt wird, wobei die Implantationsstellen in Übereinstimmung mit der vorliegenden Erfindung veranschaulicht werden. Das Auge umfasst eine Linse16 und umfasst den Glaskörper3 . Benachbart zum Glaskörper3 ist der optische Teil der Retina11 . Eine Implantation kann intraretinal11 oder subretinal12 erfolgen. Die Retina ist vom Choroid18 umgeben. Eine Implantation kann intrachoroidal oder suprachoroidal4 erfolgen. Zwischen dem optischen Teil der Retina und den Linsen, benachbart zum Glaskörper, liegt die Pars plana19 . Die Sklera8 umgibt das Choroid18 . Eine Implantation kann intraskleral8 oder episkleral7 erfolgen. Die äußere Fläche des Auges stellt die Cornea9 dar. Eine Implantation kann epicorneal9 oder intracorneal10 erfolgen. Die Innenfläche des Auges stellt die Konjunctiva6 dar. Hinter der Cornea befindet sich die Vorderkammer1 , hinter welcher die Linse16 ist. Die hintere Kammer2 umgibt die Linse, wie in der Figur gezeigt wird. Gegenüber der äußeren Fläche ist der optische Nerv, und die Arterien und Venen der Retina. Eine Implantation in die Meningealräume13 , den optischen Nerv15 und den intraoptischen Nerv14 ermöglicht eine Arzneimittelzufuhr in das zentral Nervensystem und stellt einen Mechanismus bereit, wobei die Blut-Hirn-Schranke überquert werden kann. - Andere Implantationsstellen umfassen die Zufuhr von Antitumorarzneimitteln zu neoplastischen Läsionen, z.B. einem Tumor, oder einer Läsionsstelle, z.B. umgebendes Geweben, oder in den Situationen, in welchen eine Tumormasse entfernt worden ist, Gewebe benachbart zu dem zuvor entfernten Tumor und/oder in die nach der Entfernung des Tumors verbleibende Höhle. Die Implantate können auf eine Vielzahl von Wegen verabreicht werden, einschließlich chirurgischer Mittel, einer Injektion, einem Trokar, usw.
- In der Formulierung können andere Mittel für eine Vielzahl von Zwecken verwendet werden. Beispielsweise können Puffermittel und Konservierungsmittel verwendet werden. Wasserlösliche Konservierungsmittel, welche verwendet werden können, umfassen Natriumbisulfit, Natriumbisulfat, Natriumthiosulfat, Benzalkoniumchlorid, Chlorbutanol, Thimerosal, Phenylquecksilberacetat, Phenylquecksilbernitrat, Methylparaben, Polyvinylalkohol und Phenylethylalkohol. Diese Mittel können in unter schiedlichen Mengen von etwa 0,001 bis etwa 5 Gewichtsprozent, und bevorzugt etwa 0,01 bis etwa 2% vorliegen. Geeignete wasserlösliche Puffermittel, welche verwendet werden können, sind Natriumcarbonat, Natriumborat, Natriumphosphat, Natriumacetat, Natriumbicarbonat, usw., wie von der FDA für die gewünschte Verabreichungsart zugelassen. Diese Mittel können in Mengen vorliegen, welche ausreichend sind, um einen pH des Systems von zwischen 2 und 9 und bevorzugt 4 und 8 aufrechtzuerhalten. Als solche können die Puffermittel bis 5 Gewichtsprozent bezogen auf das Basisgewicht der Gesamtzusammensetzung ausmachen. Wenn das Puffermittel oder der Verstärker hydrophil ist, kann er auch als ein Freisetzungsbeschleuniger wirken und weist einen kumulativen Effekt mit einem anderen Modulator (anderen Modulatoren) auf.
- Die Implantate können von einer beliebigen Geometrie sein, einschließlich Fasern, Folien, Filmen, Mikrospheren, Spheren, runde Scheiben, Platten und dergleichen. Die obere Grenze für die Implantatgröße wird bestimmt von Faktoren wie etwa der Tolerierung des Implantats, Größenbeschränkungen bei der Insertion, der leichten Handhabung, usw. Wenn Platten oder Folien verwendet werden, sind die Platten oder Folien im Bereich von mindestens etwa 0,5 mm × 0,5 mm, normalerweise etwa 3–10 mm × 5–10 mm, mit einer Dicke von etwa 0,25 bis 1,0 mm zur leichteren Handhabung. Wenn Fasern verwendet werden, liegt der Durchmesser der Faser im Allgmeinen im Bereich von 0,05 bis 3 mm. Die Länge der Faser liegt im Allgemeinen im Bereich von 0,5 bis 10 mm. Spheren liegen im Bereich von 2 μm bis 4 mm Durchmesser, wobei andere geformte Partikel vergleichbare Volumina aufweisen.
- Die Größe und Form des Implantats kann verwendet werden, um die Freisetzungsrate, die Behandlungsdauer und die Arzneimittelkonzentration an der Implantationsstelle zu steuern/regulieren. Größere Implantate führen eine proportional größere Dosis zu, aber in Abhängigkeit vom Verhältnis von Fläche zu Masse können sie eine langsamere Freisetzungsrate aufweisen. Die bestimmte Größe und Geometrie eines Implantats wird so ausgewählt, dass sie am besten zur Implantationsstelle passt. Die Kammern, z.B. vordere Kammer, hintere Kammer und Glaskörper, können relativ große Implantate mit unterschiedlichen Geometrien aufnehmen, mit Durchmessern von 1 bis 3 mm. Eine Platte oder eine runde Scheibe ist bevorzugt für eine Implantation in den Suprachoroidalraum. Der beschränkte Raum bei einer Intraretinalimplantation erfordert relativ kleine Implantate mit Durchmessern von 0,05 bis 1 mm.
- In einigen Situationen können Gemische von Implantaten verwendet werden, wobei gleiche oder unterschiedliche pharmakologische Mittel verwendet werden. Auf diese Art wird ein Cocktail von Freisetzungsprofilen erreicht, was eine biphasische oder triphasische Freisetzung bei einer einzelnen Verabreichung ergibt, wobei das Muster der Freisetzung in großem Maße variiert werden kann.
- Zur Produktion der Implantate können verschiedene Techniken verwendet werden. Verwendbare Techniken umfassen Lösungsmittelevaporationsverfahren, Phasentrennungsverfahren, Grenzflächenverfahren, Extrusionsverfahren, Schmelzverfahren, Spritzgussverfahren, Wärmepressverfahren und dergleichen. Spezielle Verfahren werden im US-Patent 4,997,652 diskutiert. In einer bevorzugten Ausführungsform werden Extrusionsverfahren verwendet, um das Erfordernis für Lösungsmittel bei der Herstellung zu vermeiden. Wenn Extrusionsverfahren verwendet werden, werden das Polymer und das Arzneimittel so gewählt, dass sie bei den für die Herstellung erforderlichen Temperaturen, normalerweise mindestens etwa 85°C, stabil sind.
- Das folgende Beispiel wird zur Veranschaulichung angeführt.
- Experimenteller Teil
- Beispiel 1
- Herstellung und Untersuchung eines Arzneimittelzufuhrsystems (DDS "Drug Delivery System") ohne Freisetzungsmodulator
- Es wurde die Freisetzung des hydrophoben Arzneimittels Dexamethason aus einem Arzneimittelzufuhrsystem mit verlängerter Freisetzung gemessen. Das Arzneimittelzufuhrsystem wurde mit Dexamethason und Polymilchsäure/Polyglykolsäure-Copolymer hergestellt. Dexamethasonpulver und ein Pulver aus einem Polymilchsäure-Polyglykolsäure(PLGA)-Copolymer wurde im Verhältnis von 50/50 gründlich gemischt. Das gut gemischte Pulver wurde in einen Extruder gefüllt und für 1 h bei 95°C erwärmt, dann durch eine Öffnung mit dem Maß 20 extrudiert. Sechs DDS mit ungefähr 100 bis 120 μg wurden zur Bewertung der Arzneimittelfreisetzung aus den extrudierten Filamenten herausgeschnitten.
- Jedes einzelne DDS wurde in einem Glasgefäß platziert, welches mit einem Rezeptormedium (9% NaCl in Wasser) gefüllt war. Um die "infinite sink"-Bedingungen zu ermöglichen, wurde das Rezeptormediumvolumen so gewählt, dass die Konzentration niemals eine Sättigung von 5% überstieg. Um sekundäre Transportphänomene, beispielsweise eine Konzentrationspolarisation in der stillstehenden Grenzschicht zu minimieren, wurde jedes der Glasgefäße in einem Schüttelwasserbad bei 37°C platziert. Von jedem Gefäß wurden zu definierten Zeitpunkten Proben für eine HPLC-Analyse entnommen. Das HPLC-Verfahren war wie in USP 23 (1995), S. 1791–1798 beschrieben. Die Konzentrationswerte wurden verwendet, um die kumulativen Freisetzungsprofile zu berechnen. Das Freisetzungsprofil ist in
1A gezeigt. Es ist zu sehen, dass die Arzneimittelfreisetzung mit diesem DDS sehr langsam ist. Eine merkliche Arzneimittelfreisetzung beginnt in der vierten Woche nach Beginn, ungefähr zum Zeitpunkt der Polymerauflösung. - Herstellung und Untersuchung eines DDS mit einem HPMC-Freisetzungsmodifikationsmittel
- Ein Arzneimittelzufuhrsystem wurde wie oben beschrieben hergestellt, außer dass verschiedene Konzentrationen der hydrophilen Hydroxypropylmethylcellulose (HPMC) als Freisetzungsmodifikationsmittel enthalten waren. Es wurden die Kombinationen von Arzneimittel, Polymer und HPMC, welche in Tabelle 1 gezeigt sind, verwendet.
- Die Freisetzung des Arzneimittels wurde wie oben beschrieben untersucht. Die Daten sind in
1B gezeigt. Es ist zu sehen, dass bei der Zugabe von HPMC eine ausgesprochene Steigerung der Freisetzungsrate vorliegt. Bei XT014 und XT015 ist eine Freisetzung in der Größenordnung nahe 0 zu beobachten, wobei das Verhältnis von Freisetzungsmodifikationsmittel zu Arzneimittel 0,3 bis 0,4 beträgt. Durch Auswahl des geeigneten Polymers und Freisetzungsmodifikationsmittel können die Arzneimittelfreisetzung und das Zufuhrintervall maßgeschneidert werden, um ein Freisetzungsprofil bereitzustellen, welches beschleunigt oder verlangsamt ist. - Aus den obigen Ergebnissen ist ersichtlich, dass biologisch abbaubare Implantate, welche mit einem Wirkstoff und einem Freisetzungsmodulator formuliert werden, eine Freisetzungskinetik ermöglichen, wobei das Arzneimittel mit einer konstanten Rate über längere Zeiträume freigesetzt wird, wobei vermieden wird, dass einem Patienten Arzneimittel auf weniger wirksamen Wegen, wie etwa topisch, verabreicht werden müssen. Die Implantate stellen ein verbessertes Verfahren zur Behandlung von Augenerkrankungen und anderen Erkrankungen bereit, durch Vermeidung von Spitzen und Senken bei der Freisetzung eines Arzneimittels.
Claims (4)
- Implantat zur verzögerten Freisetzung eines Wirkstoffs, umfassend: ein pharmakologisch akzeptables, biologisch abbaubares Polymer in einer Menge von mindestens 10 Gewichtsprozent des Implantats; einen therapeutischen Wirkstoff mit einer Konzentration von 10 bis 80 Gewichtsprozent des Implantats; und Hydroxypropylmethylcellulose mit einer Konzentration von 10 bis 50 Gewichtsprozent des Implantats; worin der therapeutische Wirkstoff in einer therapeutischen Dosierung freigesetzt wird, welche für einen Zeitraum von mindestens 3 Tagen um nicht mehr als 100% variiert, und worin der therapeutische Wirkstoff in dem Sinn hydrophob ist, dass er bei Umgebungstemperatur in Wasser mit weniger als 100 μm/ml löslich ist.
- Implantat nach Anspruch 1, worin das pharmakologisch akzeptable, biologisch abbaubare Polymer ein Polyester von Milchsäure und Glykolsäure ist.
- Implantat nach Anspruch 2, umfassend den Polyester von Milchsäure und Glykolsäure in einer Konzentration von mindestens 20 Gewichtsprozent des Implantats; einen therapeutischen Wirkstoff mit einer Konzentration von 10 bis 50 Gewichtsprozent des Implantats, worin der therapeutische Wirkstoff ein Glucocorticoid ist.
- Implantat nach Anspruch 3, worin das Glucocorticoid Dexamethason ist.
