Den foreliggende opfindelse angår en dosisinhalator indrettet til at blive aktiveret under anvendelse af den luftstrøm der frembringes ved inhalering og beregnet til at blive anvendt til inhalering af farmakologisk aktiv forbin-5 delse i fast, mikroniseret form, hvilken dosisinhalator er af den i indledningen til krav 1 angivne art.
Der stilles specielle krav til doseringsinhalatorer beregnet til lokal indgift af lægemidler til luftvejene og lungerne. Da der som oftest skal indgives meget kraftige læge-10 midler må dosisnøjagtigheden være stor. Den dosis af en aktiv forbindelse,der skal indgives kan være så lille som 0,1 mg.
Det er også nødvendigt at de partikler der forlader dosisinhalatoren har en passende størrelsesfordeling, idet for store partikler har tendens til at blive aflejrede i munden.
15 Der kendes flere systemer til lokal indgift af lægemid ler til åndedrætskanalen og til lungerne. Blandt disse systemer kan nævnes nebulisatorer, under tryk værende aerosoler, pumpeinhalatorer og inhalatorer som aktiveres af den luftstrøm der frembringes ved inhalering, i det følgende betegnet 20 "pulverinhalatorer".
Der er flere typer pulverinhalatorer tilgængelige på markedet. De repræsenter et supplement til under tryk værende aerosoler, hvor lægemidlet er opløst eller suspenderet i en flydende drivmiddelblanding. Pulverinhalatorer har den fordel at de altid af-25 giver den aktive forbindelse når patienten indånder, idet partikelskyen tilvejebringes ved hjælp af den luftstrøm der opnås ved inhaleringen. Derved løses problemet med at koordinere aktiveringen af en dosis med indåndingen for at bringe den aktive forbindelse til åndedrætskanalen og til lungerne.
30 Et eksempel på en pulverinhalator'derer tilgængelige på mar kedet er "Spinhaler" ® . I "Spinhaler" anvendes den aktive forbindelse i mikroniseret form, indeholdt i en hård gelatinekapsel som perforeres før brug. Den hårde gelatinekapsel anbringes i et rør i et apparat som bringes til rotation af 35 den luftstrøm der frembringes ved inhalering, hvorved den mikroniserede forbindelse bevæges ind i luftstrømmen og bringes gennem luftstrømmen til patientens åndedrætskanal og lunger.

Imidlertid har de tidligere kendte pulverinhalatorer ulemper: 1) De kan ikke anvendes til nøjagtig og reproducerbar dispensering af mikroniseret aktiv forbindelse i mængder på 5 under 15-20 mg og de er derfor kun anvendelige til mindre aktive forbindelser eller til højaktive forbindelser i kombination med et fortyndingsmiddel, sædvanligvis laktose, 2) de er besværlige at fylde og rengøre, 3) sædvanligvis kræves flere inhaleringer til at tømme 0 en kapsel der indeholder en enhedsdosis, 4) de: er vanskelige at håndtere for patienter med reduceret åndedrætskapacitet eller med nedsat evne til at anvende deres hænder, 5) laktose, fortyndingsmidlet, er meget forstyrrende ved 5 inhalering og kan forårsage forøget hyppighed af caries.
Der er et behov for en effektiv pulverinhalator som aktiveres af den- luftstrøm- der frembringes ved inhalering, som er let at håndtere for patienten f som tillader dispensering af aktiv forbindelse i en mængde ned til 0,1 mg uden rj at det er nødvendigt at indbefatte noget fortyndingsmiddel, og som giver en hensigtsmæssig størrelsesfordeling for de indgivne partikler.
Den foreliggende opfindelse angår en dosisinhalator, en såkaldt "pulverinhalator", som aktiveres af den- luftstrøm -- som frembringes ved inhalering, og som gør det muligt at dis- . pensere fast aktivt stof i mikroniseret form i en passende størrelsesfordeling i en mængde fra 0,1 mg og mere uden behov for noget fortyndingsmiddel.. Dosisinhalatoren kan konstrueres til indgift af aktiv forbindelse i en mængde på op til 5 mg. j Den kan også, med en passende konstruktion af doseringsenhe den, anvendes til indgift af aktiv forbindelse i en mængde på 5-50 mg.
Dette behov opfyldes ifølge opfindelsen ved, at inhalatoren er ejendommelig ved det i den kendetegnende del af j krav 1 angivne, og fordelagtige træk fremgår af de efterføl gende uselvstændige krav.
Den mikroniserede aktive forbindelse fastholdes i skivens perforeringer ved en kombination af at den mikroniserede

3 forbindelse er tvangsmæssigt indført i perforeringsåbningerne, hvorved den presses mod perforeringsåbningens sidevægge, ligesom den mikroniserede forbindelse har en tendens til at klæbe til den hosliggende flade.
5 En udførelsesform for dosisinhalatoren ifølge opfindel sen skal nu beskrives mere detaljeret under henvisning til tegningen, på hvilken fig. 1 viser et længdesnit gennem en dosisinhalator ifølge opfindelsen, 10 fig. 2 viser skrabere i forrådskammeret, hvilke skrabe re anvendes til at indføre aktiv forbindelse i den perforerede membrans perforeringer, og fig. 3 viser hvorledes den aktive forbindelse fødes fra forrådsenheden ind i perforeringerne i den perforerede 15 membran under anvendelse af de nævnte skrabere.
Dosisinhalatoren omfatter en manøvreenhed 1 som benyttes til fødning af doser af den aktive forbindelse, en dyse 2 som kan være forsynet med roterende organer 3 beregnet til at sønderslå sådanne aggregatpartikler af aktiv forbindel-20 se som måtte være dannet, en doseringsenhed 10 til afmåling af den tiltænkte dosis af aktiv forbindelse og et forrådskammer 5 for fast mikroniseret aktiv forbindelse.
Dosisinhalatoren omfatter også en luftledning 6 beregnet til passage af den luft der skal inhaleres. Dysen 2 kan 25 forsynes med roterende organer 3 beregnet til at sønderslå sådanne aggregatpartikler af aktiv forbindelse som måtte være dannet. Sønderdelingen af eventuelle partikelaggregater lettes ved hjælp af luftindløb 7 arrangeret på hver side af dysen 2.
30 Doseringsenheden 10 omfatter et forrådskammer 5 for den aktive forbindelse, en perforeret membran 4, en holder 9 for den perforerede membran, og doseringsorganer 8, skematisk vist på fig. 1, til indføring af den aktive forbindelse i den perforerede membrans 4 perforeringer. Doseringsenheden 35 10 og den perforerede membran 4 er arrangeret forskydeligt i forhold til hinanden mellem en første stilling, hvor aktiv forbindelse ved hjælp af doseringsorganerne 8 indføres i perforeringerne i en del af den perforerede membrans 4 areal, 4

og en anden stilling, hvor det nævnte delareal af den ladede membran 4 er blevet indført i luftledningen 6 i dosisinhalatoren. I den første stilling af membranen 4 bringes den aktive forbindelse fra forrådskammeret 5 ind i perforeringerne i 5 membranen' 4. Når en del af membranens 4 areal, som indeholder sådanne perforeringer fyldt med aktiv forbindelse,derefter indsættes i luftledningen 6 vil den aktive forbindelse indeholdt i perforeringerne blive ført med ved .inhalering og bragt gennem dysen 2 til patientens åndedrætskanal og lunger.
0 Indføringen af aktiv forbindelse i perforeringerne i den perforerede membran 4 sker i den beskrevne udførelsesform ved hjælp af mekaniske organer 8 bestående af·elastiske,, fjederbelastede skrabere 15, monteret i en holder 17 i forrådskammeret 5. Se fig. 2 og 3.
5 I en foretrukket udførelsesform er den perforerede membran 4 arrangeret forskydeligt i forhold til forrådskammeret 5.
I en anden foretrukket udførelsesform er den perforerede membran 4 drejeligt lejret i forhold til forrådskammeret 5. q I en yderliggere foretrukket udførelsesform omfatter do seringsudstyret 8 elastiske fjederbelastede skrabere 15 arrangeret i forrådskammeret 5 for at indføre aktiv forbindelse i perforeringerne i den perforerede membran 4.
I en anden foretrukket udførelsesform er perforeringer-5 ne i den perforerede membran 4 i form af keglestubbe med deres største åbning rettet mod dysen.
Skraberne 15 er hensigtsmæssigt fremstillet i et elastisk materiale, fx gummi eller formstof. Skraberne er hensigtsmæssigt arrangeret på en sådan måde at de,når den akti-0 ve forbindelse indføres i perforeringerne i membranen 4,rører overfladen af membranen- med- en vinkel som er mindre end 90°.
Når organerne til indføring af aktiv forbindelse i perforeringerne i membranen er arrangeret på denne måde opnås der den yderligere fordel at eventuelle aggregater af aktiv forbindel-5 se desintegreres før indgift.
Skraberne 15 er hensigtsmæssigt arrangeret således at de presser mod overfladen af membranen 4. Dette opnås som vist på fig. 1 ved at skraberne 15 er belastet ved hjælp af 5

en fjeder 12. Fjederen 12 er som vist på fig. 1 arrangeret i forrådskammeret 5.
Alternativt kan en fjeder være arrangeret således at membranen 4 presses mod skraberne 15. Fjederen kan i så 5 fald være anbragt på ydersiden af forrådskammeret 5, hensigtsmæs sigt monteret i manøvreenheden 1. Yderligere variationer er mulige. Det essentielle element er at skraberne 15 presser mod membranen 4 på en sådan måde at aktiv forbindelse indføres i perforeringerne i membranen 4 når manøvreenheden betje-10 nes. Se fig. 2 og 3.
I manøvreenheden 1, er der umiddelbart-ved siden af membranen 4 arrangeret en plade 1.3, som modvirker, at aktiv forbindelse passerer gennem perforeringerne i membranen 4, idet pladen 13 udgør bunden af perforeringerne.
^ Luftledningen 6 passerer gennem manøvreenheden 1. Ved inhalering gennem dysen 2 vil luftstrømmen passere dels gennem luftindløbene 7 arrangeret på siden af dysen 2, og dels gennem luftledningen 6. Luften i luftledningen 6 passerer gennem dens åbning 14 i manøvreenheden 1. I den på fig. 1 viste udførelsesform er en perforeret plade 18 arrangeret som et filter i åbningen 14 for at modvirke at uønskede partikler ler af større størrelse indtræder i luftledningen 6.
Når der anvendes en membran i form af et vandret plan som vist på fig. 1 kan membranen monteres således at den kan 25 bevæges ved at den roteres, idet de øvrige dele af doseringsinhalatoren er faste i forhold til hinanden.
For at sikre at den mængde aktiv forbindelse/ som er blevet fyldt ind i perforeringerne som indsættes i luftledningen 6, frigøres fra perforeringerne og medføres ved inhalering, 30 er perforeringerne i membranen hensigtsmæssigt udformet som keglestubbe, se fig. 3, med den bredeste åbning rettet mod dysen. Perforeringer i form af keglestubbe letter tømningen af perforeringerne idet den aktive forbindelse lettere frigøres. Når forrådskammeret 5 er anbragt mellem membranen 35 og dysen vil fyldningen af perforeringerne desuden lettes hvis perforeringerne er udformet som vist på fig. 3.
Perforeringerne i den perforerede membran kan være af vilkårlig udformning, de kan være cirkulære, firkantede,

6 elliptiske, rektangulære eller kan have en hvilken som helst anden geometrisk form. Arealet af perforeringerne i membranen kan være en større eller mindre del af membranarealet, fx fra 1-95%, idet betegnelsen "membranareal" refererer til den 5 del af membranens areal som indsættes i luftledningen. Antal let af perforeringer i membranarealet kan variere i afhængighed af sådanne faktorer som den mængde aktivt stof som skal indgives pr. dosis, de fysiske egenskaber af det aktive stof osv. I en foretrukket udførelsesform har perforeringerne ko-10 nisk form.
Den perforerede membran kan fremstilles i et hvilket som helst egnet materiale, fx metal eller formstof. Størrelsen af dosis for den aktive forbindelse,der skal indgives,bestemmes af størrelsen af perforeringerne i membranen, membranens tykkel-15 se, antallet af perforeringer i membranen og af størrelsen af luftledningen. En enkelt perforering kan være tilstrækkelig til dispensering af en given dosis af den aktive forbindelse. Nøjagtigheden af doseringen vil hovedsagelig afhænge af nøjagtigheden ved fremstillingen af membranen. Eksempler på per-20 forerede membraner som kan anvendes er metalnet som fremstil les af Veco Beheer B.V., Eerbeck, Holland. Disse net kan fås med forskellige perforeringsstørrelser. De kan formes på ønsket måde, fx i tromleform eller de kan anvendes i form af vandrette, plane membraner. Også vævede net af metal, fibre 25 eller af andre materialer kan benyttes. Den vigtige faktor er den doseringsnøjagtighed der kan opnås.
Manøvre enheden 1 er i den på fig. 1 viste udførelsesform arrangeret lige op.til doseringsenheden 10. Manøvreringsenheden kan være fremstillet i form af en ring med 30 indhak som det er vist på fig. 2, hvor fjederbelastede bolte 11, se fig. 1, anvendes til at tilvejebringe nøjagtige stillinger for den perforerede membran når den fremføres ved betjening af manøvreringsenheden 1. Manøvreringsenheden kan også arrangeres på anden måde, fx ved at indrette den til 35 at virke direkte på den perforerede membran.
Forrådskammeret er i den foretrukne udførelsesform arrangeret mellem den perforerede membran og dysen. Se fig. 1.
Forrådskammeret kan arrangeres til at rumme en varie-

7 rende mængde af den aktive forbindelse. I en dosisinhalator hvor forrådskammeret er udformet således at man ikke kan genfylde med aktiv forbindelse kan det indeholde en mængde af aktiv forbindelse svarende til fx ca. 200 doseringsenheder.
5 I en doseringsinhalator hvor forrådskammeret er beregnet til at blive genfyldt,kræves der en indretning der kan arrangeres på toppen eller på siderne af forrådskammeret. Fx kan en skrue eller prop 16 arrangeres i forrådskammeret 5 som vist på fig.
1.
10 Luftledningen 6 kan have et areal på 25-350 mm . Luft ledningen kan være cirkulær eller kan have en anden geometrisk form. Hvis den er cirkulær kan diameteren være fra ΒΙΟ mm.
Blandt forbindelsesgrupper og specifikke forbindelser 15 der er velegnede til indgift med en pulverinhalator ifølge opfindelsen kan nævnes - betareceptorstimulerende midler såsom adrenalin, isoprenalin, orciprenalin, salbutamol og terbutalin, - steroider til inhalering såsom budesonid, 20 - substanser beregnet til nasal indgift.
Særligt nyttige er terbutalin og budesonid.
Den aktive forbindelse kan indgives i mikroniseret form uden yderligere ingredienser eller i farmaceutisk modifi-seret mikroniseret form for at opnå forbedrede strømningsegen-25 skaber.
Claims (6)
1. Dosisinhalator som aktiveres af den luftstrøm, som frembringes ved inhalering, og som er beregnet til inha-5 lering af en forud bestemt dosis på 0,1-50 mg fast, ufor-tyndet farmakologisk aktiv forbindelse i mikroniseret form pr indgift, hvilken inhalator omfatter en dyse (2), en luftledning (6) forbundet med dysen til passage af den luft, som skal inhaleres, en doseringsenhed (10) omfatten-10 de et forrådskammer (5) for den aktive forbindelse og et forskydeligt element (4) med doseringsåbning(er) til afgivelse af nævnte forbindelse fra forrådskammeret (5) ind i luftledningen (6) og en manøvreringsenhed (1) til forskydning af elementet (4) i forhold til forrådskamme-15 ret, hvilket udleveringselement (4) er en perforeret membran anbragt i en holder (9) i doseringsenheden (10), og en plade (13) anbragt umiddelbart ved siden af membranen (4) for at forhindre, at aktiv forbindelse passerer gennem membranens (4) perforeringer, når den aktive forbin-20 delse indføres deri, kendetegnet ved at omfatte organer (8) til tvangsmæssig indføring af aktiv forbindelse i den perforerede membrans (4) perforeringer, idet membranen er forskydeligt anbragt i forhold til organerne (8), således at fast aktiv forbindelse i mikroniseret form i en 25 første stilling bliver indført i perforeringerne i en del af membranens (4) areal, og således at de af den nævnte del af membranens (4) areal omfattede perforeringer, hvori den forudbestemte mængde aktiv forbindelse fastholdes, i en anden stilling indføres på tværs i luftledningen (6).
2. Dosisinhalator ifølge krav 1, kendetegnet ved, at den perforerede membran (4) er roterbar.
3. Dbsisinhalator ifølge krav 1 og/eller 2, k ende-tegnet ved, at organerne (8) omfatter fjederbelastede skrabere (15) anbragt i forrådskammeret (5).
4. Dosisinhalator ifølge krav 3, kendetegnet ved, at den perforerede membran (4) og skraberne (15) er presset mod hinanden ved hjælp af en fjeder.
5. Dosisinhalator ifølge krav 2, 3 eller 4, k e n d e - DK 159052 B tegnet ved, at perforeringerne i den perforerede mem-bran (4) har form som keglestubbe med deres største åbning vendt mod dysen (2).
6. Dosisinhalator ifølge et hvilket som helst af kravene 5 1-5/kendetegnet ved, at perforeringernes stør relse og antal i hver del af membranen (4), som er beregnet til afgivelse af en dosis, vælges således, at dosisstørrelse b-l-iver 0,1-5 mg af den aktive forbindelse.
