Description
- Gebiet der Erfindung
- Die vorliegende Erfindung betrifft neue Verwendungen zur Behandlung verschiedener Erkrankungen und Zustände mit Botulinumtoxinen. Bedeutenderweise stellt die vorliegende Erfindung Medikamente bereit, die zur Linderung von Schmerzen durch Muskelaktivitäten oder -kontraktionen nützlich sind, und ist daher bei der Behandlung von z.B. muskulären Spasmen, wie einer Erkrankung des mandibulartemporalen Gelenks, Kreuzschmerzen, myofascialen Schmerzen, Schmerzen durch Spastizität und Dystonie, sowie Sportverletzungen und Schmerzen durch Kontraktion bei Arthritis von Vorteil.
- Hintergrund der Erfindung
- Bisher wurden Botulinumtoxine, insbesondere Botulinumtoxin Typ A, zur Behandlung einer Anzahl neuromuskulärer Erkrankungen und Zustände, die muskuläre Spasmen einschließen, verwendet; beispielsweise Strabismus, Bleopharospasmus, spastischer Schiefhals (zervikale Dystonie), oromandibuläre Dystonie und spastische Dysphonie (Kehlkopfdystonie). Das Toxin bindet rasch und stark an präsynaptische cholinerge Nervenenden und inhibiert die Exocytose von Acetylcholin durch Absenkung der Frequenz der Acetylcholinfreisetzung. Dies führt zu einer lokalen Lähmung und folglich zur Relaxation des von Spasmen befallenen Muskels.
- Als ein Beispiel der Behandlung neuromuskulärer Erkrankungen sei auf US-PS 5 053 005 (Borodic) verwiesen, worin die Behandlung der Krümmung der jugendlichen Wirbelsäule, d.h. Skoliose, mit einem Acetylcholin-Freisetzungsinhibitor, vorzugsweise Botulinumtoxin A, vorgeschlagen wird.

- Spasmische Dysphonie wurde mit Botulinumtoxin Typ A behandelt, siehe Blitzer et al., Ann. Otol. Rhino. Laryngol., 1985, 94, 591–594. Linguale Dystonie wurde gemäß Brin et al., Adv. Neurol. (1987) 50, 599–608, mit Botulinumtoxin Typ A behandelt. Schließlich offenbart Cohen et al., Neurology (1987) 37 (Forts. 1), 123-4, die Behandlung von Schreibkrampf mit Botulinumtoxin Typ A.
- Der Ausdruck "Botulinumtoxin" ist ein generischer Ausdruck, der die Familie der Toxine umfaßt, die durch das anaerobe Bakterium Clostridium botulinum erzeugt wird, und bisher wurden sieben immunologisch unterschiedliche Neurotoxine identifiziert. Diesen wurden die Bezeichnungen A, B, C, D, E, F und G verliehen. Bezüglich weiterer Informationen über die Eigenschaften der verschiedenen Botulinumtoxine wird auf den Artikel von Jankovic und Brin, The New England Journal of Medicine, Nr. 17, 1990, Seiten 1186-1194, sowie auf den Übersichtsartikel von Charles L. Hatheway in Kapitel 1 des Buches mit dem Titel Botulinum Neurotoxin und Tetanus Toxin, L.L. Simpson (Hg.), veröffentlicht von Academic Press Inc., San Diego, Kalifornien, 1989, verwiesen.
- Die neurotoxische Komponente von Botulinumtoxin hat ein Molekulargewicht von etwa 150 kD und es wird angenommen, daß sie eine kurze Polypeptidkette von etwa 50 kD umfaßt, von der angenommen wird, daß sie für die toxischen Eigenschaften des Toxins verantwortlich ist, d.h. durch Eingreifen in die Exocytose von Acetylcholin mittels einer Verringerung der Frequenz der Acetylcholinfreisetzung, sowie eine größere Polypeptidkette von etwa 100 kD, von der angenommen wird, daß sie notwendig ist, um das Toxin in die Lage zu versetzen, an die präsynaptische Membran zu binden.
- Die "kurze" und die "lange" Kette sind mittels einer einfachen Disulfidbrücke aneinander gebunden (es wird darauf hingewiesen, daß bestimmte Botulinumtoxin-Serotypen, z.B. Typ E, anstelle einer Doppelkette in Form eines einkettigen ungeschnittenen Proteins vorliegen können. Die einkettige Form ist weniger aktiv, kann jedoch durch Zerschneiden mit einer Protease, z.B. Trypsin, in die entsprechende zweikettige Form umgewandelt werden. Sowohl die ein- als auch die doppelkettige Form sind in dem erfindungsgemäßen Verfahren verwendbar).
- Im allgemeinen sind vier physiologische C. Botulinumgruppen bekannt (I, II, III, IV). Die Organismen, die zur Erzeugung eines serologisch eindeutigen Toxins in der Lage sind, können aus mehr als einer physiologischen Gruppe kommen. Beispielsweise können die Typ B- und F-Toxine durch die Stämme der Gruppe I oder II erzeugt werden. Darüber hinaus wurden andere Clostridiumspeziesstämme identifiziert, die Botulinumneurotoxin erzeugen können (C. baratii, Typ F; C. butyricum, Typ E; C. novyi, Typ C1 oder D).
- Immunotoxinkonjugate von Ricin und Antikörper, die dadurch gekennzeichnet sind, daß sie durch Verbesserung der Zelloberflächenaffinität eine erhöhte Cytotoxizität besitzen, sind in EP-B 0 128 434 offenbar. Die Erfinder geben an, daß Botulinumtoxin anstelle von Ricin verwendet werden kann.
- Botulinumtoxin wird kommerziell erhalten durch Etablierung und Aufzucht von C. Botulinumkulturen in einem Fermenter und anschließendes Abernten und Reinigen der fermentierten Mischung nach bekannten Techniken.
- Botulinumtoxin von Typ A, dem Toxintyp, der üblicherweise zur Behandlung neuromuskulärer Zustände verwendet wird, ist gegenwärtig aus verschiedenen Quellen kommerziell erhältlich; beispielsweise von Porton Products Ltd., UK, unter dem Markennamen "DYSPORT" und von Allergan Inc., Irvine, Kalifornien, unter dem Markennamen BOTOX®.
- Es ist ein erfindungsgemäßes Ziel, neue Behandlungen von neuromuskulären Erkrankungen und Zuständen mit verschiedenen Botulinumtoxinen bereitzustellen. Es ist ein anderes erfindungsgemäßes Ziel, Schmerzen mit verschiedenen Botulinum-Mischtypen zu lindern.
- Zusammenfassung der Erfindung
- Die vorliegende Erfindung stellt die Verwendung der neurotoxischen Komponente von Botulinumtoxin zur Herstellung eines Medikaments zur Behandlung von Schmerzen im Zusammenhang mit Muskelaktivitäten oder -kontraktionen bereit.
- Durch Verwendung spezifischer Antikörper wurden alle Botulinumtoxin-Serotypen als immunologisch unterschiedliche Proteine identifiziert. Wenn der Antikörper (das Antitoxin) beispielsweise Typ A erkennt, d.h. seine biologische Aktivität neutralisiert, so erkennt es nicht die Typen B, C, D, E, F oder G.
- Während alle Botulinumtoxine Zinkendopeptidasen zu sein scheinen, scheint der Wirkmechanismus verschiedener Serotypen, beispielsweise A und E, innerhalb des Neurons von demjenigen von Typ B unterschiedlich zu sein. Darüber hinaus scheint der neuronale "Oberflächenrezeptor" für die Toxine für die Serotypen unterschiedlich zu sein.
- In dem erfindungsgemäßen Anwendungsbereich der Botulinumtoxine in bezug auf Organsysteme, die die Freisetzung von Neurotransmittern beinhalten, wird erwartet, daß die neurotoxische Komponente der Toxine A, B, C, D, E, F und G direkt durch lokale Injektionen eingeführt wird.
- Detaillierte Beschreibung
- Die erfindungsgemäß verwendeten Botulinumtoxine sind die Botulinumtoxine Typ A, B, C, D, E, F und G.
- Die physiologischen Gruppen der Clostridium botulinum-Typen sind in Tabelle 1 aufgeführt.
- Diese Toxintypen können durch Auswahl der geeigneten physiologischen Gruppe von Clostridium botulinum-Organismen erzeugt werden. Die als Gruppe I bezeichneten Organismen werden im allgemeinen als proteolytisch bezeichnet und erzeugen Botulinumtoxine der Typen A, B und F. Die als Gruppe II bezeichneten Organismen sind saccharolytisch und erzeugen Botulinumtoxine der Typen B, E und F. Die Organismen der Gruppe III erzeugen nur Botulinumtoxin der Typen C und D und unterscheiden sich von den Organismen der Gruppen I und II durch die Erzeugung signifikanter Mengen Propionsäure. Die Organismen der Gruppe IV erzeugen nur Neurotoxin vom Typ G. Die Erzeugung beliebiger und aller Botulinumtoxine der Typen A, B, C, D, E, F und G ist in Kapitel 1 von Botulinum Neurotoxin and Tetanus Toxin, wie oben zitiert und/oder in den darin zitierten Literaturstellen beschrieben. Botulinumtoxine der Typen B, C, D, E, F und G sind darüber hinaus aus verschiedenen Clostridiumspezies erhältlich.
- Derzeit werden 14 Clostridiumspezies als pathogen angesehen. Die meisten pathogenen Stämme erzeugen Toxine, die für verschiedene pathologische Anzeichen und Symptome verantwortlich sind. Organismen, die Botulinumtoxine erzeugen, wurden aus Botulismusausbrüchen in Menschen (Typen A, B, E und F) und Tieren (Typen C und D) isoliert. Ihre Identitäten wurden durch Verwendung spezifischer Antitoxine (Antikörper) beschrieben, die gegen die früheren Toxine entwickelt wurden. Typ G-Toxin wurde im Boden gefunden und besitzt eine geringe Toxigenität. Es wurde jedoch aus Autopsieproben isoliert, obwohl bisher keine ausreichenden Anzeichen vorliegen, daß Typ G-Botulismus bei Menschen aufgetreten ist.
- Das Toxin soll vorzugsweise durch intramuskuläre Injektion direkt in einen lokalen Bereich, wie beispielsweise einen spastischen Muskel, verabreicht werden, vorzugsweise in die Region der neuromuskulären Verbindung, obwohl auch alternative Verabreichungstypen (z.B. subkutane Injektion) angewandt werden können, durch die das Toxin direkt in die beeinträchtigte Region zugeführt werden kann, soweit dies angezeigt ist. Das Toxin kann als sterile, pyrogenfreie, wäßrige Lösung oder Dispersion und als ein steriles Pulver zur Rekonstituierung zu einer sterilen Lösung oder Dispersion dargereicht werden.
- Soweit gewünscht, können Tonizitätseinstellmittel, wie beispielsweise Natriumchlorid, Glycerin und verschiedene Zucker, zugegeben werden. Ferner können auch Stabilisatoren, wie beispielsweise Humanserumalbumin, eingeschlossen werden. Die Formulierung kann mittels eines geeigneten pharmazeutisch annehmbaren Konservierungsmittels, wie einem Paraben, konserviert werden, obwohl sie vorzugsweise nicht konserviert wird.
- Es ist bevorzugt, daß das Toxin in Einheitsdosierungsform formuliert wird; beispielsweise kann es als eine sterile Lösung in einer Ampulle oder als eine Ampulle oder ein Beutel bereitgestellt werden, die/der ein lyophilisiertes Pulver zur Rekonstituierung in einem geeigneten Träger, wie beispielsweise Kochsalzlösung für Injektionszwecke, enthält.
- Gemäß einer Ausführungsform wird die neurotoxische Komponente des Botulinumtoxins in einer Lösung formuliert, die Kochsalzlösung und pasteurisiertes Humanserumalbumin, das das Toxin stabilisiert und Verluste durch nichtspezifische Adsorption minimiert, enthält. Die Lösung wird steril filtriert (0,2 μm-Filter), in einzelne Ampullen abgefüllt und dann vakuumgetrocknet, wodurch ein steriles lyophilisiertes Pulver erhalten wird. Zur Anwendung kann das Pulver durch Zugabe steriler, nichtkonservierter, normaler Kochsalzlösung (0,9 % Natriumchlorid für Injektionszwecke) rekonstituiert werden.
- Die dem Patienten zu verabreichende Toxindosis hängt von der Schwere des Zustands ab; z.B. von der Anzahl der eine Behandlung erfordernden Muskelgruppen, vom Alter und der Größe des Patienten und von der Stärke des Toxins. Die Stärke des Toxins wird ausgedrückt als ein Vielfaches des LD50-Wertes für Mäuse, wobei eine Toxineinheit (U) definiert ist als eine äquivalente Toxinmenge, die 50 % einer Gruppe von 18–20 weiblichen Swiss-Webster-Mäusen tötet, die jeweils 20 g wiegen.
- Die in humantherapeutischen Anwendungen verwendeten Dosierungen sind der Muskelmasse grob proportional, in die injiziert wird. Typischerweise kann die dem Patienten verabreichte Dosis von etwa 0,01 bis zu 1.000 Einheiten betragen, beispielsweise bis zu etwa 500 Einheiten, und vorzugsweise etwa 80 bis etwa 460 Einheiten pro Patient und Behandlung, obwohl unter geeigneten Umständen kleinere oder größere Dosen verabreicht werden können, wie etwa 50 Einheiten zur Linderung von Schmerzen und zur Steuerung der cholinergen Sekretionen.
- Wenn die Ärzte mit der Verwendung dieses Produkts vertrauter werden, kann die Dosierung verändert werden. In dem von Porton erhältlichen Botulinumtoxin Typ A, DYSPORT, enthält 1 Nanogramm (ng) 40 Einheiten. 1 ng des Botulinumtoxins Typ A, das von Allergan Inc. erhältlich ist, d.h. BOTOX®, enthält 4 Einheiten. Die Stärke des Botulinumtoxins und dessen lange Wirkungsdauer legen nahe, daß eine Neigung dazu besteht, die Dosen auf unregelmäßiger Basis zu verabreichen. Schlußendlich ist jedoch sowohl die Menge des verabreichten Toxins als auch die Häufigkeit der Verabreichung im Ermessen des für die Behandlung verantwortlichen Arztes und wird den Fragen der Sicherheit und der durch das Toxin hervorgebrachten Wirkungen entsprechen.
- In einigen Fällen insbesondere bei der Linderung von Schmerzen im Zusammenhang mit Sportverletzungen, wie beispielsweise Muskelrissen oder -zerrungen nach Sportverletzungen ("Charleyhorse"), ist Botulinum Typ F bevorzugt, das eine geringe Aktivitätsdauer besitzt.
- Die Erfindung wird nachfolgend durch Bezugnahme auf die folgenden, nicht beschränkenden Beispiele dargestellt.
- In jedem der Beispiele werden geeignete Bereiche jedes Patienten mit einer sterilen Lösung injiziert, die das verschriebene Botulinumtoxin (confirmation of botulinum toxin) enthält. Die Gesamtdosen pro Patient liegen im Bereich von etwa 0,01 bis 460 Einheiten. Vor der Injektion in irgendeine Muskelgruppe wird die Anatomie der Muskelgruppe sorgfältig in Betracht gezogen, wobei das Ziel die Injektion in den Bereich mit der höchsten Konzentration neuromuskulärer Verbindungen ist, sofern diese bekannt ist. Vor der Injektion in den Muskel wird die Nadelposition in dem Muskel dadurch festgestellt, daß der Muskel seinen Bewegungsbereich durchläuft und die resultierende Bewegung des Nadelendes beobachtet wird. In Abhängigkeit vom Alter des Patienten, der Anzahl der zu injizierenden Stellen und den besonderen Bedürfnissen des Patienten werden Vollnarkose, Lokalanästhesie und Ruhigstellung angewandt. Zur Erzielung des gewünschten Ergebnisses können mehr als eine Injektion und/oder Injektionsorte erforderlich sein. Ferner können in Abhängigkeit von dem zu injizierenden Muskel einige Injektionen die Verwendung feiner teflonbeschichteter Hohlnadeln erfordern, die durch Elektromyografie geführt werden.
- Nach der Injektion wurde festgestellt, daß keine systemischen oder lokalen Nebenwirkungen auftreten und von keinem der Patienten wurde gefunden, daß er eine extensive lokale Hypotonizität entwickelte. Die Mehrzahl der Patienten zeigt sowohl subjektiv als auch objektiv gemessen eine Funktionsverbesserung.
- Beispiel 1
- Verwendung von Botulinumtoxin Typen A bis G zur Behandlung von Muskelkrämpfen und zur Schmerzbekämpfung im Zusammenhang mit Muskelkrämpfen bei einer Erkrankung des mandibulartemporalen Gelenks
- Eine 35-jährige Frau wird durch Verabreichung von insgesamt 0,1 bis 50 Einheiten Botulinumtoxin behandelt. Die Verabreichung erfolgt in die Muskeln, die das Schließen des Kiefers kontrollieren. Überaktive Muskeln können durch EMG (Elektromyografie)-führung identifiziert werden. Linderung der mit den Muskelkrämpfen verbundenen Schmerzen, mögliche Reduktion des Kieferverkrampfens tritt in etwa 1 bis 3 Tagen auf.
- Beispiel 2
- Verwendung von Botulinumtoxin Typen A bis G zur Behandlung von Muskelkrämpfen und zur Schmerzbekämpfung im Zusammenhang mit Muskelkrämpfen bei Zuständen nach Sportverletzungen (Charleyhorse)
- Ein 20-jähriger Mann mit schweren Krämpfen im Oberschenkel nach einer Sportverletzung wird durch Verabreichung eines Toxins mit kurzer Wirkdauer, möglicherweise einer niedrigen Dosis (0,1 bis 25 Einheiten) von bevorzugt Typ F in den Muskel und die benachbarten Muskeln, die kontrahiert ("verkrampft") sind, behandelt. Linderung der Schmerzen tritt in 1 bis 7 Tagen auf.
- Beispiel 3
- Verwendung von Botulinumtoxin Typen A bis G zur Behandlung von Muskelkrämpfen und zur Schmerzbekämpfung im Zusammenhang mit Muskelkrämpfen bei Störungen der glatten Muskulatur, wie, den Gastrointestinalmuskeln
- Eine 35-jährige Frau mit spastischer Kolitis wird mit 1 bis 100 Einheiten Botulinumtoxin, aufgeteilt in verschiedene Bereiche, Klistier (1 bis 5 Einheiten) verabreicht im Standardklistiervolumen, Titrationsdosis, ausgehend von der geringsten Dosis, behandelt. Die Injektion findet in den Mastdarm oder unteren Kolon statt, oder ein Klistier mit niedriger Dosis kann eingesetzt werden. Die Krämpfe und Schmerzen im Zusammenhang mit dem spastischen Kolon werden 1 bis 10 Tagen gelindert.
- Beispiel 4
- Verwendung von Botulinumtoxin Typen A bis G zur Behandlung von Muskelkrämpfen und zur Schmerzbekämpfung im Zusammenhang mit Muskelkrämpfen bei spastischen Zuständen infolge von Schlaganfall oder traumatischen Gehirn- oder Rückenmarksverletzungen
- Einem 70-jährigen Mann werden nach einem Schlaganfall oder einen cerebralvaskulären Vorfall 50 bis 300 Einheiten Botulinumtoxin in die Hauptmuskeln, die am festen Schließen der Hand und Anwinkeln von Handgelenk und Unterarm beteiligt sind, oder in die Muskeln, die am Schließen der Beine beteiligt sind, so daß der Patient und das Pflegepersonal Schwierigkeiten mit der Hygiene haben, injiziert. Eine Linderung dieser Symptome tritt nach 7 bis 21 Tagen auf.
Claims (6)
- Verwendung der neurotoxischen Komponente von Botulinumtoxin zur Herstellung eines Medikaments zur Behandlung von Schmerzen im Zusammenhang mit Muskelaktivitäten oder -kontraktionen.
- Verwendung gemäß Anspruch 1, wobei die neurotoxische Komponente ein Molekulargewicht von etwa 150 kD hat.
- Verwendung gemäß Anspruch 1 oder Anspruch 2, wobei die Schmerzen Kreuzschmerzen, myofasciale Schmerzen, Schmerzen durch Spastizität und/oder Dystonie sowie Sportverletzungen und Schmerzen durch Kontraktion bei Arthritis sind.
- Verwendung gemäß Anspruch 1 oder Anspruch 2, wobei die Schmerzen Spannungskopfschmerz oder Folgeerscheinungen eines Schlaganfalls, traumatischer Gehirnverletzungen oder Verletzungen des Rückenmarks sind.
- Verwendung gemäß einem der voranstehenden Ansprüche, worin die neurotoxische Komponente die neurotoxische Komponente von Botulinumtoxin Typ A, B, C, D, E, F oder G ist.
- Verwendung gemäß einem der vorstehenden Ansprüche, wobei das Medikament zur lokalen Verabreichung ist.
