La invención proporciona el uso de la toxina
botulínica para la fabricación de un medicamento para el tratamiento
de secreciones con control colinérgico, por ejemplo medicamentos
para modular el lagrimeo o la secreción mucosa.
Hasta ahora, las toxinas botulínicas, en
particular la toxina botulínica tipo A, han sido utilizadas en el
tratamiento de varios trastornos neuromusculares y estados que
implican espasmos musculares; por ejemplo, estrabismo,
blefaroespasmos, tortícolis espasmódica (distonía cervical),
distonía oromandibular y disfonía espasmódica (distonía laríngea).
La toxina se une rápida y fuertemente a las terminales presinápticas
colinérgicas del nervio e inhibe la exocitosis de acetilcolina
mediante la disminución de la frecuencia de liberación de
acetilcolina. Esto da como resultado una parálisis local y por lo
tanto una relajación del músculo afectado por el espasmo.
Kondo et at, en Specialia (1977) 751, reportan
una modificación de la acción de la pentagastrina en la secreción de

La expresión toxina botulínica es una expresión
genérica que abarca las familias de las toxinas producida por la
bacteria anaeróbica Clostridium botulinum y, hasta la fecha,
7 neurotoxinas inmunológicamente distintas han sido identificadas.
Estas han recibido las denominaciones A, B, C, D, E, F y G. Para más

and Tetenus Toxin, L.L. Simpson, Ed., publicado por Academic
Press Inc. de San Diego, California 1989, las descripciones de los
cuales se incorporan aquí por la referencia.
El componente neurotóxico de la toxina botulínica
tiene un peso molecular de aproximadamente 150 kiloDaltons y se
piensa que comprende una corta cadena polipeptídica de alrededor de
50 kD que se considera responsable de las propiedades tóxicas de la
toxina, por ejemplo, al interferir en la exocitosis de la
acetilcolina, disminuyendo la frecuencia de liberación de
acetilcolina, y en una cadena polipeptídica más larga de cerca de
100 kD que se cree es necesaria para permitir la unión de la toxina
a la membrana presináptica.
Las cadenas "corta" y "larga" están
unidas entre sí por medio de puentes simples de disulfuro. (Cabe
destacar que ciertos serotipos de toxina botulínica, por ejemplo,
del tipo E, pueden existir en forma de proteína de una sola cadena y
sin cortes, en lugar de una doble cadena. La forma de una sola
cadena es menos activa, pero puede convertirse en la correspondiente
cadena doble mediante cortes con una proteasa, por ejemplo,
tripsina. Ambas, la cadena sencilla y la doble, son útiles en el
método de la presente invención).
En general, se conocen cuatro grupos fisiológicos
de C. Botulinum (I, II, III, IV). Los organismos capaces de
producir una toxina serológicamente distinta pueden provenir de más
de un grupo fisiológico. Por ejemplo, las toxinas de tipo B y F
pueden ser producidas por cepas del Grupo I o II. Además, han sido
identificadas otras cadenas de especies de clostridial (C.
Baratii, tipo F; C. Butyricum, tipo E; C. Novyi,
tipo C_{1} o D) que pueden producir neurotoxinas
botulínicas.
Los conjugados con inmunotoxina de ricina y
anticuerpos, que se caracterizan por tener aumentada su
citotoxicidad mediante la mejora de la afinidad a la superficie
celular, están descritas en la memoria de Patente Europea 0129434.
Los inventores advierten que la toxina botulínica puede ser
utilizada en lugar de la ricina.

La toxina botulínica se obtiene comercialmente
mediante la creación y crecimiento de cultivos de C.
Botulinum en un fermentador y recogiendo y purificando más tarde
la mezcla fermentada de acuerdo con técnicas conocidas.
La toxina botulínica tipo A, el tipo de toxina
generalmente utilizado para tratar estados neuromusculares, está en
la actualidad comercialmente disponible en distintas compañías; por
ejemplo, en Porton Products Ltd. Reino Unido, bajo el nombre
comercial "DYSPORT", y en Allergan, Inc., Irvine, California,
bajo el nombre comercial BOTOX®.
Es un objeto de la invención proporcionar nuevos
tratamientos de trastornos y enfermedades neuromusculares con
diversos tipos de toxinas botulínicas. Es otro objeto de la presente
invención aliviar el dolor con diversos tipos de toxinas
botulínicas.
La presente invención se refiere al uso de la
toxina botulínica para la fabricación de un medicamento para el
tratamiento de secreciones con control colinérgico, incluyendo el
lagrimeo y la secreción mucosa. La toxina botulínica se selecciona
del grupo que consiste en las toxinas botulínicas de tipos A, B, C,
D, E, F y G.
Cada serotipo de toxina botulínica ha sido
identificado como proteínas inmunológicamente diferentes a través
del uso de anticuerpos específicos. Por ejemplo, si el anticuerpo
(antitoxina) reconoce, esto es, neutraliza la actividad biológica
de, por ejemplo, el tipo A, no reconocerá los tipos B, C, D, E, F o
G.
Aunque todas las toxinas botulínicas parecen ser
zinc-endopeptidasas, el mecanismo de acción de los
distintos serotipos, por ejemplo, A y E dentro de la neurona,
parecen ser diferentes al del tipo B. Además, el "receptor" de
la superficie neuronal para la toxina, parece ser diferente en cada
serotipo.
En el área de uso de las toxinas botulínicas de
acuerdo con la presente invención, con respecto a los sistemas
orgánicos que implican la liberación de neurotransmisor, se espera
introducir las toxinas A, B, C, D, E, F y G directamente mediante
inyección local.
Las toxinas botulínicas utilizadas de acuerdo
con la presente invención son las toxinas botulínicas tipo A, B, C,
D, E, F y G.

Estos tipos de toxinas pueden ser producidos por
selección del grupo fisiológico apropiado de organismos
Clostridium botulinum. Los organismos designados como Grupo
I, hacen referencia normalmente a proteolíticos y producen toxinas
botulínicas de los tipos A, B, y F. Los organismos designados como
Grupo II son sacarolíticos y producen toxinas botulínicas de los
tipos B, E y F. Los organismos designados como Grupo III producen
sólo toxinas botulínicas de los tipos C y D, y se distinguen de los
organismos de los Grupos I y II por la producción de cantidades
significativas de ácido propiónico. Los organismos del Grupo IV
sólo producen neurotoxina tipo G. La producción de una cualquiera y
de todos los tipos de toxina botulínica A, B, C, D, E, F y G está
descrita en el Capítulo I de "Botulinum Neurotoxin and Tetanus
Toxin", citado anteriormente, y/o las referencias allí
citadas. Los tipos de toxina botulínica B, C, D, E, F y G también
se pueden obtener de varias especies de clostridia.
Actualmente, catorce especies de clostridia son
consideradas patógenas. La mayoría de las cepas patógenas producen
toxinas que son responsables de varios signos y síntomas
patológicos. Han sido aislados organismos que producen toxinas
botulínicas a partir de epidemias de botulismo en humanos (tipos A,
B, E y F) y en animales (tipos C y D). Sus identidades fueron
descritas a través del uso de antitoxinas (anticuerpos)
específicas, desarrolladas contra unas primeras toxinas. La toxina
tipo G fue encontrada en el suelo y tiene baja toxigenicidad. Sin
embargo, aunque ha sido aislada de muestras de autopsia, todavía no
se ha encontrado evidencia adecuada de que el botulismo tipo G se
haya producido en seres humanos.
Preferentemente, la toxina ha de ser administrada
en la región de unión neuromuscular, aunque puedan ser empleadas
formas alternativas de administración (por ejemplo, inyección
subcutánea) que pueden transportar la toxina directamente a la
región afectada cuando se considere apropiado. La toxina puede ser
presentada como una solución o dispersión acuosa estéril apirética,
y como polvo estéril para reconstitución en una solución o
dispersión estéril.
Si se desea, pueden ser añadidos agentes
ajustadores de la tonicidad tal como cloruro sódico, glicerol y
distintos azúcares. Pueden también ser incluidos estabilizadores
tales como albúmina sérica humana. La formulación se puede conservar
por medio de un conservante adecuado, farmacéuticamente
conveniente, tal como un parabeno, aunque es preferible que no
tenga conservantes.
Es preferible que la toxina se formule en una
forma de dosificación unitaria; por ejemplo, se puede proporcionar
en forma de una solución estéril en un vial o como un vial o sobres
que contengan polvo liofilizado para reconstituir con un vehículo
adecuado tal como solución salina para inyección.
En una realización, la toxina botulínica se
formula en una solución que contiene albúmina sérica humana
pasteurizada y solución salina, que estabiliza la toxina y minimiza
las pérdidas mediante adsorciones no específicas. La solución se
filtra en condiciones estériles (filtro de 0,2 micrómetros), se
carga en viales individuales y se seca al vacío, para dar un polvo
liofilizado estéril. Para su uso, el polvo puede ser reconstituido
mediante la adición de suero salino normal estéril sin conservantes
(cloruro sódico al 0,9% para inyección).
La dosis de toxina a ser administrada al paciente
dependerá de la gravedad de los estados, de la edad y talla del
paciente y de la potencia de la toxina. La potencia de la toxina se
expresa como múltiplo del valor DL_{50} para el ratón,
definiéndose una unidad (U) de la toxina como la cantidad de toxina
equivalente que provoca la muerte al 50% de un grupo de 18 a 20
ratones Swiss-Webster hembras, que pesan
aproximadamente 20 gramos cada uno.
Típicamente, la dosis a ser administrada al
paciente puede ser desde 0,01 hasta 1.000 unidades; por ejemplo,
hasta 500 unidades, y preferiblemente en un intervalo desde 80
hasta 460 unidades por paciente y por tratamiento, aunque dosis
menores y mayores pueden ser administradas en circunstancias
apropiadas, tales como hasta 50 unidades para el alivio del dolor y
para controlar las secreciones colinérgicas.
Conforme los médicos se familiaricen con el uso
de este producto, la dosis puede cambiarse. En la toxina botulínica
tipo A, disponible por Porton, DYSPORT, 1 nanogramo (ng) contiene
40 unidades. 1 ng de toxina botulínica tipo A, disponible por
Allergan, Inc., es decir, BOTOX®, contiene 4 unidades. La potencia
de la toxina botulínica y su largo tiempo de acción, hacen que las
dosis tiendan a ser administradas de forma poco frecuente. En
última instancia, sin embargo, tanto la cantidad de toxina
administrada como la frecuencia en su administración será
responsabilidad del médico encargado del tratamiento y estará de
acuerdo con las cuestiones de seguridad y los efectos producidos
por la toxina.
El invento será ilustrado ahora con referencia al
siguiente ejemplo no limitante.
En el ejemplo, las zonas apropiadas de cada
paciente son inyectadas con una solución estéril que contiene la
confirmación de la toxina botulínica. Las dosis totales del
paciente varían desde aproximadamente 0,01 unidades hasta 460
unidades. La anestesia general, la anestesia local y la sedación se
usan de acuerdo con la edad del paciente, el número de puntos que
serán inyectados, y las necesidades particulares de cada paciente.
Puede ser necesaria más de una inyección y/o puntos de inyección
para alcanzar el resultado deseado.
Tras la inyección, cabe destacar que no hay
efectos secundarios sistémicos ni locales y en ninguno de los
pacientes se ha visto que desarrollase hipotonicidad local
extensiva. La mayoría de los pacientes muestra una mejora en la
función tanto subjetivamente y, cuando se mide, también
objetivamente.
Un hombre, de 65 años, con transpiración excesiva
unilateral se trató mediante la administración de 0,01 a 50
unidades de toxina botulínica, dependiendo del grado del efecto
deseado. Cuanto mayor sea la dosis, generalmente será mayor la
extensión y duración del efecto. Inicialmente se usan pequeñas
dosis. En esta indicación se puede utilizar una toxina de cualquier
serotipo, sola o en combinación. La administración se realizará, o
al plexo de nervios de la glándula, o al ganglio, o a la médula
espinal o al sistema nervioso central, dependiendo del conocimiento
del médico sobre la anatomía y fisiología de las glándulas a tratar
y de las células secretoras. Además, el nivel apropiado de médula
espinal o área del cerebro pueden ser inyectados con la toxina
(aunque esto pudiera causar varios efecto, incluyendo debilidad
general). De esta manera, la glándula (si es accesible) o el plexo
de nervios, o el ganglio, serán los objetivos de la elección. El
exceso de transpiración, lagrimeo, secreciones mucosas o
gastrointestinales del paciente están positivamente influenciados
por el sistema nervioso colinérgico. La transpiración y el lagrimeo
están bajo un mayor control colinérgico que la secreción mucosa o
gástrica, y responderían mejor al tratamiento con toxina. Sin
embargo, las secreciones mucosas y gástricas podrían ser moduladas
a través del sistema colinérgico. Todos los síntomas serían
reducidos o eliminados con una terapia con toxina en
1-7 días. La duración sería de unas semanas a
varios meses.
Claims (7)
1. El uso de una toxina botulínica para la
fabricación de un medicamento para el tratamiento de una secreción
con control colinérgico o con influencia colinérgica.
2. El uso de una toxina botulínica para la
fabricación de un medicamento para la modulación del lagrimeo o de
una secreción mucosa.
3. El uso de una toxina botulínica según la
reivindicación 1 o la reivindicación 2 para la fabricación de un
medicamento para el tratamiento del lagrimeo.
4. El uso de una toxina botulínica según la
reivindicación 1 o la reivindicación 2 para la fabricación de un
medicamento para modular una secreción mucosa.
5. El uso según una cualquiera de las
reivindicaciones 1 a 4, en el que la toxina botulínica ha de ser
administrada directamente a un área afectada de un paciente.
6. El uso según una cualquiera de las
reivindicaciones 1 a 5, en el que la toxina botulínica ha de ser
administrada mediante administración local a una glándula.
7. El uso según la reivindicación 6, en el que la
toxina botulínica de tipo A es para ser administrada a un paciente
en una cantidad entre alrededor de 0,01 y alrededor de 50
unidades.
