Description
- GEBIET DER ERFINDUNG
- Diese Erfindung betrifft die Verwendung von Botulinustoxin zur Herstellung eines Arzneimittels zur Behandlung von cholinergisch kontrollierten Sekretionen, z. B. Medikamenten zur Modulierung von Tränenbildung oder Schleimsekretion.
- HINTERGRUND DER ERFINDUNG
- Bisher wurden Botulinustoxine, insbesondere Botulinustoxin Typ A, für die Behandlung einer Anzahl von neuromuskulären Störungen und Zuständen, die muskuläre Spasmen mit sich bringen, verwendet; z. B. Strabimus, Blepharospasmus, spastischer Schiefhals (zervikale Dystonie), oromandibulare Dystonie und spasmische Dysphonie (laryngeale Dystonie) (siehe z. B. Jankovic et al., New England Journal of Medicine (1991) 1185). Das Toxin bindet schnell und stark an die präsynaptischen cholinergischen Nervenenden und inhibiert die Exocytose von Acetylcholin durch Verringern der Frequenz der Acetylcholin-Freisetzung. Dies führt zu lokaler Paralyse und daher Entspannung der Muskeln, die von Spasmen betroffen sind.
- Kondo et al. berichten in Specialia (1977) 751 über eine Modifikation der Wirkung von Pentagastrin auf die Säuresekretion durch Botulinustoxin. Ray et al. beschreiben in J. Biol. Chem. (1993) 11057 die Inhibierung von Arachidonsäure-Freisetzung, assoziiert mit Acetylcholin-Freisetzung aus PC12-Zellen, mit Botulinustoxin.

US-PS 5 053 005 von Borodic, die die Behandlung der Verbiegung der jugendlichen Wirbelsäule, d. h. Skoliose, mit einem Acetylcholin-Freisetzungsinhibitor, bevorzugt Botulinustoxin A, vorschlägt. - Für die Behandlung von Strabismus mit Botulinustoxin Typ A siehe J. S. - Spasmische Dysphonie wurde mit Botulinustoxin Typ A behandelt. Siehe Blitzer et al., Ann. Otol. Rhino. Laryngol, 1985, 94, 591–594. Linguale Dystonie wurde mit Botulinustoxin Typ A gemäss Brin et al., Adv. Neurol. (1987) 50, 599–608 behandelt. Schliesslich offenbart Cohen et al., Neurology (1987) 37(Suppl. 1), 123–4, die Behandlung von Schreibkrämpfen mit Botulinustoxin Typ A.
- Der Begriff Botulinustoxin ist ein generischer Begriff, der die Familie von Toxinen, die durch das anaerobe Bakterium Chlostridium botulinum hergestellt werden, umfasst und bisher wurden sieben immunologisch unterschiedliche Neurotoxine identifiziert. Diesen wurden die Bezeichnungen A, B, C, D, E, F und G gegeben. Für weitere Information bezüglich der Eigenschaften der verschiedenen Botulinustoxine wird auf den Artikel von Jankovic und Brin, The New England Journal of Medicine, Nr. 17, 1990, S. 1186–1194 und die Übersicht von Charles L. Hatheway in Kapitel 1 des Buches Botulinum Neurotoxin and Tetanus Toxin, L. L. Simpson, Herausgeber, veröffentlicht von Academic Press Inc., San Diego, Kalifornien, 1989, verwiesen, deren Offenbarungen hiermit durch Bezugnahme eingeschlossen sind.
- Der neurotoxische Bestandteil von Botulinustoxin hat ein Molekulargewicht von etwa 150 kD und von ihm wird angenommen, dass er eine kurze Polypeptidkette von etwa 50 kD umfasst, von der angenommen wird, dass sie für die toxischen Eigenschaften des Toxins verantwortlich ist, d. h. durch Stören der Exocytose von Acetylcholin, durch Erniedrigen der Frequenz der Acetylcholin-Freisetzung, und einer grösseren Polypeptidkette von etwa 100 kD, von der angenommen wird, dass sie notwendig ist, um das Toxin in die Lage zu versetzen, an die präsynaptische Membran zu binden.
- Die "kurze" und die "lange" Kette sind mittels einer einfachen Disulfidbrücke verbunden. (Es wird festgestellt, dass bestimmte Serotypen der Botulinustoxine, z. B. Typ E, in Form eines einkettigen, ungekerbten Proteins im Gegensatz zu einer Doppelkette existieren können. Die Einzelkettenform ist weniger aktiv, aber sie kann durch Kerben mit einer Protease, z. B. Trypsin, in die entsprechende Doppelkette umgewandelt werden. Sowohl die Einzel- als auch die Doppelkette sind in dem Verfahren der vorliegenden Erfindung nützlich.)
- Im allgemeinen sind vier physiologische Gruppen von C. Botulinum bekannt (I, II, III, IV). Die Organismen, die in der Lage sind, ein serologisch unterschiedliches Toxin herzustellen, können aus mehr als einer physiologischen Gruppe stammen. Zum Beispiel können Toxine des Typs B und F durch die Stämme der Gruppe I oder II hergestellt werden.
- Zusätzlich wurden andere Stämme von Clostridien-Spezies (C. baratii, Typ F; C. butyricum, Typ E; C. novyi, Typ C1 oder D) identifiziert, die Botulinusneurotoxine herstellen können.
- Immunotoxinkonjugate von Ricin und Antikörpern, die dadurch gekennzeichnet sind, dass sie durch Verbessern der Affinität zur Zelloberfläche verstärkte Cytotoxizität haben, sind in der europäischen Patentveröffentlichung 0 129 434 offenbart. Die Erfinder stellen fest, dass Botulinustoxine anstelle von Ricin verwendet werden können.
- Botulinustoxin wird kommerziell durch Herstellen und Wachsenlassen von Kulturen von C. botulinum in einem Fermenter und dann Ernten und Reinigen der fermentierten Mischung in Übereinstimmung mit bekannten Techniken erhalten.
- Botulinustoxin Typ A, der Toxin-Typ, der im allgemeinen zum Behandeln von neuromuskulären Zuständen verwendet wird, ist gegenwärtig kommerziell von verschiedenen Quellen erhältlich; z. B. von Porton Products Ltd. UK, unter dem Handelsnamen "DYSPORT", und von Allergan, Inc., Irvin, California, unter dem Markennamen BOTOX®.
- Es ist eine Aufgabe der Erfindung, neue Behandlungen von neuromuskulären Störungen und Zuständen mit verschiedenen Botulinustoxinarten bereitzustellen. Es ist eine weitere Aufgabe der Erfindung, mit verschiedenen Botulinustoxin-Typen Schmerzen zu vermindern.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Die vorliegende Erfindung betrifft die Verwendung von Botulinustoxinen für die Herstellung eines Arzneimittels zur Behandlung von cholinergisch kontrollierten Sekretionen, einschliesslich Tränenbildung und Schleimsekretionen. Das Botulinustoxin wird aus der Gruppe, bestehend aus Botulinustoxin Typ A, B, C, D, E, F und G, ausgewählt.
- Jeder Serotyp des Botulinustoxins wurde als immunologisch unterscheidbare Proteine durch die Verwendung von speziellen Antikörpern identifiziert. Zum Beispiel wenn der Antikörper (das Antitoxin) z. B. Typ A erkennt, d. h. die biologische Aktivität neutralisiert, wird es die Typen B, C, D, E, F, oder G nicht erkennen.
- Während alle Botulinustoxine Zinkendopeptidasen zu sein scheinen, erscheint der Wirkmechanismus von verschiedenen Serotypen, z. B. A und E, innerhalb des Neuron anders als der des Typs B zu sein. Zusätzlich scheint der neuronale Oberflächen-"Rezeptor" für die Toxine für die Serotypen verschieden zu sein.
- Im Gebiet der Verwendung der Botulinustoxine gemäss der vorliegenden Erfindung mit Bezug auf Organsysteme, die die Freisetzung von Neurotransmitter erfordern, wird erwartet, die Toxine A, B, C, D, E, F und G direkt durch lokale Injektionen einzuführen.
- DETAILLIERTE BESCHREIBUNG
- Die Botulinustoxine, die erfindungsgemäss verwendet werden, sind Botulinustoxine Typ A, B, C, D, E, F und G.
- Die physiologischen Gruppen von Clostridium botulinum-Typen sind in Tabelle I aufgeführt.
- Diese Toxin-Typen können durch Auswahl der entsprechenden physiologischen Gruppe von Clostridium botulinum-Organismen hergestellt werden. Die als Gruppe I bezeichneten Organismen werden normalerweise als proteolytisch bezeichnet und stellen Botulinustoxine der Typen A, B und F her. Die als Gruppe II bezeichneten Organismen sind saccharolytisch und stellen Botulinustoxine der Typen B, E und F her. Die als Gruppe III bezeichneten Organismen stellen nur Botulinustoxine Typ C und D her und unterscheiden sich von Organismen der Gruppen I und II durch die Herstellung von signifikanten Mengen Propionsäure. Organismen der Gruppe IV stellen nur Neurotoxine des Typs G her. Die Herstellung von jedem und allen der Botulinustoxin-Typen A, B, C, D, E, F und G sind in Kapitel 1 des oben zitierten Botulinum Neurotoxin and Tetanus Toxin und/oder in dort zitierten Literaturstellen beschrieben. Botulinustoxine Typ B, C, D, E, F und G sind auch aus verschiedenen Spezien von Clostridien erhältlich.
- Derzeit werden 14 Spezien von Chlostridien als pathogen angesehen. Die meisten der pathogenen Stämme stellen Toxine her, die für die verschiedenen pathologischen Zeichen und Symptome verantwortlich sind. Organismen, die Botulinustoxine herstellen, wurden aus Botulinus-Ausbrüchen in Menschen (Typ A, B, E und F) und Tieren (Typ C und D) isoliert. Ihre Identität wurde durch die Verwendung von spezifischen Antitoxinen (Antikörpern) beschrieben, die gegen die früheren Toxine entwickelt wurden. Toxin des Typs G wurde im Boden gefunden und hat eine niedrige Toxigenizität. Es wurde jedoch aus Autopsieproben isoliert, aber bis jetzt gibt es keinen adäquaten Beweis, dass Typ G-Botulismus in Menschen aufgetreten ist.
- Bevorzugt ist das Toxin in die Region der neuromuskulären Kontaktstellen zu verabreichen, obwohl alternative Arten der Verabreichung (z. B. subkutane Injektion), die das Toxin direkt in die betroffene Region liefern können, wo angemessen, eingesetzt werden können. Die Toxine können als sterile pyrogenfreie wässrige Lösung oder Dispersion oder als ein steriles Pulver zur Wiederherstellung in eine sterile Lösung oder Dispersion vorgelegt werden.
- Wo gewünscht, können den Tonus einstellende Mittel, wie Natriumchlorid, Glycerin und verschiedene Zucker, zugegeben werden. Stabilisatoren, wie menschliches Serumalbumin, können ebenfalls beigefügt werden. Die Formulierung kann mittels eines geeigneten pharmazeutisch annehmbaren Konservierungsmittels, wie Paraben, konserviert werden, obwohl sie bevorzugt unkonserviert ist.
- Es ist bevorzugt, dass das Toxin in Form einer Einheitsdosis formuliert ist; z. B. kann es als sterile Lösung in einer Ampulle oder als eine Ampulle oder ein Säckchen, enthaltend ein lyophilisiertes Pulver zum Wiederherstellen mit einem geeigneten Transportmittel, wie Salzlösung zur Injektion, bereitgestellt werden.
- In einer Ausführungsform ist das Botulinustoxin als eine Lösung, enthaltend Salzlösung und pasteurisiertes menschliches Serumalbumin, das das Toxin stabilisiert und Verlust durch nicht-spezifische Absorption minimiert, formuliert. Die Lösung wird steril gefiltert (0,2 μm-Filter), in individuelle Ampullen gefüllt und dann vakuumgetrocknet, um ein steriles lyophilisiertes Pulver zu ergeben. Zum Verwenden kann das Pulver durch die Zugabe von steriler unkonservierter normaler Salzlösung (Natriumchlorid 0,9% zur Injektion) wiederhergestellt werden.
- Die Dosis des an den Patienten zu verabreichenden Toxins hängt von der Schwere des Zustands, dem Alter und der Grösse des Patienten und der Wirksamkeit des Toxins ab. Die Wirksamkeit des Toxins wird als ein Vielfaches des LD50-Werts für die Maus ausgedrückt, eine Einheit (U) des Toxins wird als die äquivalente Menge des Toxins, die 50% einer Gruppe von 18 bis 20 weiblichen Swiss-Webster-Mäusen, die jeweils 20 g wiegen, tötet, definiert.
- Typischerweise kann die einem Patienten zu verabreichende Dosis von etwa 0,01 bis etwa 1.000 Einheiten sein; z. B. bis zu etwa 500 Einheiten, und bevorzugt im Bereich von etwa 80 bis etwa 460 Einheiten pro Patient pro Behandlung, obwohl kleinere oder grössere Dosen unter angemessen Umständen verabreicht werden können, wie bis zu etwa 50 Einheiten für die Verminderung von Schmerzen und zur Kontrolle der cholinergischen Sekretion.
- So wie die Ärzte mit der Verwendung dieses Produkts vertrauter werden, kann die Dosis verändert werden. In dem Botulinustoxin Typ A, erhältlich von Porton, DYSPORT, enthält 1 Nanogramm (ng) 40 Einheiten. 1 ng des Botulinustoxins Typ A, erhältlich von Allergan, Inc., d. h. BOTOX®, enthält 4 Einheiten. Die Wirksamkeit des Botulinustoxins und seine lange Wirkdauer bedeuten, dass die Dosis dazu tendiert, auf einer seltenen Basis verabreicht zu werden. Letztlich liegen jedoch sowohl die Menge des verabreichten Toxins als auch die Frequenz seiner Verabreichung in der Verantwortung des für die Behandlung verantwortlichen Arztes und werden in Übereinstimmung mit den Fragen der Sicherheit und der Wirkungen, die durch das Toxin geliefert werden, sein.
- Die Erfindung wird nun mit Bezug auf das folgende Beispiel illustriert.
- In dem Beispiel wird eine sterile Lösung, von der bestätigt ist, dass sie Botulinustoxin enthält, in geeignete Bereiche jedes Patienten injiziert. Die Gesamtdosis pro Patient erstreckt sich von etwa 0,01 Einheiten bis 460 Einheiten. Allgemeine Anästhesie, Lokalanästhesie und Dämpfung (Sedierung) werden gemäss dem Alter des Patienten, der Anzahl der Gebiete, in die injiziert wird, und den besonderen Bedürfnissen des Patienten verwendet. Mehr als eine Injektion und/oder ein Injektionsgebiet kann notwendig sein, um das gewünschte Ergebnis zu erreichen.
- Es wird festgestellt, dass es infolge der Injektion keine systemischen oder lokalen Nebenwirkungen gibt und dass bei keinem Patienten gefunden wird, dass er extensiven lokalen Tonusmangel entwickelt. Die Mehrheit der Patienten zeigt eine Verbesserung der Funktion sowohl subjektiv als auch wenn objektiv gemessen.
- BEISPIEL 1
- Verwendung von Botulinustoxin Typ A bis G zur Behandlung von übermässigem Schwitzen, Tränenbildung oder Schleimsekretion oder andere cholinergisch kontrollierte Sekretionen
- Ein Mann, Alter 65, mit übermässigem unilateralem Schwitzen wird durch Verabreichen von 0,01 bis 50 Einheiten Botulinustoxin abhängig vom Grad der gewünschten Wirkung behandelt. Je grösser die Dosis um so grösser normalerweise die Ausbreitung und Dauer der Wirkung. Kleine Dosen werden anfänglich verwendet. Jeder Serotyp des Toxins allein oder in Kombination konnte bei dieser Indikation verwendet werden. Die Verabreichung erfolgt an das Drüsennervengeflecht, Ganglion, Rückenmark oder zentrale Nervensystem, zu bestimmen durch das Wissen des Arztes von der Anatomie und Physiologie der Zieldrüsen und sekretorischen Zellen. Zusätzlich können die angemessene(n) Rückenmarkshöhe oder Gehirnbereiche mit dem Toxin injiziert werden (obwohl dies viele Wirkungen, einschliesslich allgemeiner Schwäche, hervorrufen würde). Daher sind die Drüse (falls zugänglich) oder das Nervengeflecht oder das Ganglion die Ziele der Wahl. Übermässiges Schwitzen, Tränen (Tränenbildung), Schleimsekretion oder gastrointestinale Sekretion werden durch das cholinergische Nervensystem positiv beeinflusst. Schwitzen und Tränen sind unter stärkerer cholinergischer Kontrolle als Schleim- oder gastrische Sekretion und würden daher besser auf die Toxin-Behandlung antworten. Schleim- und gastrische Sekretionen können jedoch durch das cholinergische System moduliert werden. Alle Symptome würden unter Toxintherapie in etwa 1 bis 7 Tagen reduziert oder eliminiert werden. Die Dauer wäre Wochen oder einige Monate.
Claims (7)
- Verwendung eines Botulinustoxins zur Herstellung eines Arzneimittels zur Behandlung einer cholinergisch kontrollierten oder einer cholinergisch beeinflussten Sekretion.
- Verwendung eines Botulinustoxins zur Herstellung eines Arzneimittels zur Modulierung von Tränenbildung oder von Schleimsekretion.
- Verwendung eines Botulinustoxins gemäss Anspruch 1 oder 2 zur Herstellung eines Arzneimittels zur Behandlung von Tränenbildung.
- Verwendung eines Botulinustoxins gemäss Anspruch 1 oder 2 zur Herstellung eines Arzneimittels zur Modulierung von Schleimsekretion.
- Verwendung gemäss einem der Ansprüche 1 bis 4, worin das Botulinustoxin direkt in die befallenen Gebiete eines Patienten zu verabreichen ist.
- Verwendung gemäss einem der Ansprüche 1 bis 5, worin das Botulinustoxin durch lokale Verabreichung an eine Drüse zu verabreichen ist.
- Verwendung gemäss Anspruch 6, worin das Botulinustoxin vom Typ A einem Patienten in einer Menge zwischen ca. 0,01 bis ca. 50 Einheiten zu verabreichen ist.
