Description
Die vorliegende Erfindung betrifft neue, sterile wässerige Zusammensetzungen, die eine Öl-in-Wasser-Emulsion enthalten. Insbesondere betrifft sie intravenöse Fettemulsionen. Es ist erkannt worden, dass die mikrobielle Kontamination der parenteralen Flüssigkeiten eine der vielen Ursachen für die im Krankenhaus entstehende Infektion von ICU-Patienten ist. Daher sehen beispielsweise in den USA die allgemeinen Anforderungen der Federal Food and Drug Administratrion (FDA) vor, dass "Verabreichungsvorrichtungen" häufig gewechselt werden. Das Arbeitsklima auf Intensivstationen ist geschäftig und wie in anderen Teilen des Gesundheitswesens besteht der Zwang zur Kosteneindämmung. Das Auswechseln von "Verabreichungsvorrichtungen" mindestens alle 6 oder 12 Stunden ist für die sehr gut ausgebildete ICU-Schwester, den Intensivtherapiespezialisten oder den Anästhesisten verhältnismässig Zeit raubend. Dies ist insbesondere dann der Fall, wenn eine Anzahl von schwer kranken Patienten in einer ICU zur gleichen Zeit eine Infusion erhalten soll. In unserem GB-Patent 1 472 793 ist offenbart, dass Propofol-Formulierungen ggf. einen oder mehrere zusätzliche Bestandteile enthalten können, die unter Stabilisatoren, Konservierungsmitteln und Antioxidationsmitteln ausgewählt sind, beispielsweise Paraben-Derivate, beispielsweise p-Hydroxybenzolsäurepropylester, butylierte Hydroxytoluol-Derivate, Ascorbinsäure und Natriummetabisulfit, Metallionen komplexierende Mittel, beispielsweise Natriumedetat, und Entschäumer, beispielsweise ein Silikon-Derivat, z.B. Dimethicon oder Simethicon. Die Zugabe von bekannten Konservierungsmitteln zu Öl-in-Wasser-Emulsionen wie "Diprivan" ist mit Schwierigkeiten verbunden. "Diprivan" ist ein propofolhaltiges Anästhetikum, das zum Einleiten und zur Aufrechterhaltung einer allgemeinen Anästhesie und zur Sedierung verwendet wird. Die verabreichten Volumina können beträchtlich sein, insbesondere im Fall der Sedierung. Folglich können signifikante Volumina des Konservierungsmittels den unter Behandlung stehenden Patienten verabreicht werden. Daher muss der Zusatzstoff sehr sorgfältig ausgewählt werden, um die Anforderungen der Arzneimittelzulassungsbehörden zu erfüllen, insbesondere weil die Verwendung von Konservierungsmitteln in am Schluss sterilisierten, parenteral injizierbaren Einzeldosierungen nicht empfohlen wird und/oder der Gegenstand von Warnhinweisen in den verschiedenen Richtlinien, beispielsweise den US-, GB- und europäischen Arzneibüchern ist. Ausserdem gibt es ein besonderes Problem bei der Zugabe von Zusatzstoffen in eine Öl-in-Wasser-Emulsion zur parenteralen Verabreichung. Es wird angenommen, dass die antimikrobiellen Eigenschaften jedes Konservierungsmittels in der wässrigen Phase ausgeübt werden müssen, um wirksam zu sein. Daher wäre ein Konservierungsmittel mit lipophilen Eigenschaften nicht wirksam, wenn es in den üblichen Dosiermengen hinzugegeben wird, weil sich nicht genügend Material in der wässerigen Phase befinden würde, obwohl es zwischen den Phasen zu einer gewissen Verteilung kommt. Die Erhöhung der Gesamtmenge eines derartigen Konservierungsmittels würde zu unakzeptabel hohen Konservierungsmittelmengen in der Lipidschicht führen, was zumindest zu Toxizitätsproblemen führt. Andererseits führt auch die Zugabe eines Konservierungsmittels mit hydrophilen Eigenschaften, z.B. eines ionischen Materials, zu Problemen. Die Zugabe von ionischem Material zu einer Öl-in-Wasser-Emulsion kann die Emulsion destabilisieren. Bei höherer Ionenbeladung (d.h. Konzentration des ionischen Materials) kann sich die stabilisierende elektrische Ladung (Zeta-Potenzial) auf den ölartigen Tröpfchen ändern. Diese elektrischen Ladungsänderungen erhöhen die Wahrscheinlichkeit von Tröpfchenzusammenstössen und erhöhen die physikalische Instabilität der Emulsion. Es wurde die Möglichkeit der Zugabe eines einer Anzahl von antimikrobiellen Mitteln zu der Öl-in-Wasser-Emulsion untersucht. Ein derartiges Mittel dürfte keine signifikant nachteilige Wirkung auf die physikalische und chemische Stabilität der Emulsion haben. Ausserdem müsste ein derartiges Mittel die gewünschte antimikrobielle Wirkung ergeben. Es wurde festgestellt, dass eine Anzahl von potenziellen Mitteln zur Instabilität der Emulsion führten. Andere potenzielle Mittel besassen nicht die gewünschte antimikrobielle Wirkungsstärke. Ausserdem wurde nach einem Mittel gesucht, das diese Wirkungsstärke bei einer möglichst niedrigen Konzentration besitzt, um das Risiko einer physikalischen Instabilität zu minimieren, und um die Sicherheitsvorkehrungen zu minimieren. Trotz beträchticher Anstrengungen, die auch die Untersuchung der bekannten Konservierungsmittel Phenylquecksilberacetat, Phenylquecksilbernitrat, Benzylalkohol, Chlorbutanol, Chlorcresol und Phenol und die Untersuchung der bekannten Konservierungsmittel Natriummetabisulphit, Natriumsulphit, Natriummethylhydroxybenzoat und Natriumpropylhydroxybenzoat einschlossen, war es nicht möglich, ein Konservierungsmittel zu finden, das diesen Anforderungen genügt. Dann wurde die mögliche Verwendbarkeit von anderen Mitteln untersucht, welche die gewünschte Wirkung haben könnten. Es wurde unerwartet befunden, dass Edetat, welches nicht als antimikrobielles Mittel mit breitem Spektrum angesehen wird, das einzige Mittel ist, das diese Anforderungen erfüllt. Wie oben angegeben, ist das Edetat in Form des Natriumsalzes in unserem GB-Patent 1 472 793 als mögliches Metallionen-Komplexierungsmittel erwähnt. Natriumedetat ist in zwei der vielen Cremophor enthaltenen Beispielen dieses Patents enthalten. Erfindungsgemäss wird eine sterile wässerige Zusammensetzung zur parenteralen Verabreichung bereitgestellt, die eine Öl-in-Wasser-Emulsion aufweist, in der ein mit Wasser nicht mischbares Lösungsmittel mit Wasser emulgiert und mit einem grenzflächenaktiven Mittel stabilisiert ist, und die ausserdem eine Edetat-Menge enthält, die ausreicht, um für mindestens 24 Stunden (im Fall einer zufälligen Kontaminierung von aussen) signifikantes Mikroorganismenwachstum zu verhindern. Mit einer Öl-in-Wasser-Emulsion ist ein distinktes Zweiphasensystem gemeint, das sich im Gleichgewicht befindet, und das insgesamt im Ergebnis kinetisch stabil und thermo-dynamisch unstabil ist. Dies ist grundverschieden von einer mizellaren Formulierung, beispielsweise mit Cremophor EL, die thermodynamisch stabil ist. Mit dem Begriff "Edetat" sind Ethylendiamintetraessigsäure (EDTA) und deren Derivate gemeint, beispielsweise ist das Dinatrium-Derivat als Dinatriumedetat bekannt. Im Allgemeinen handelt es sich bei den geeigneten Edetaten der Erfindung um diejenigen Salze, die eine niedrigere Affinität für EDTA haben als Calcium. Konkrete Derivate, die in der Erfindung verwendet werden können, sind beispielsweise Trinatriumedetat, Tetranatriumedetat und Dinatriumcalciumedetat. Die Beschaffenheit des Edetats ist nicht kritisch, sofern es seine Funktion erfüllt, nämlich im Falle einer zufälligen, von aussen kommenden Kontamination, mindestens 24 Stunden lang signifikantes Mikroorganismenwachstum zu ver hindern (z.B. vorzugsweise eine Vermehrung um das Zehnfache nach einer von aussen kommenden Kontamination mit einer geringen Menge, z.B. 10-10<3> koloniebildenden Einheiten, bei Temperaturen im Bereich von 20-25 DEG C). Aus dem experimentellen Abschnitt ergibt sich, dass Natriumcalciumedetat gegenüber anderen Zusätzen einige Vorteile aufweist, dass Natriumedetat aber aussergewöhnlich ist. Daher ist das Edetat vorzugsweise Dinatriumedetat. Üblicherweise ist das Edetat in den Zusammensetzungen der Erfindung in einer molaren Konzentration (bezüglich der freien Säure EDTA) im Bereich von 3 x 10<-5> bis 9 x 10<-4> enthalten. Vorzugsweise ist das Edetat im Bereich von 3 x 10<-5> bis 7,5 x 10<-4>, beispielsweise im Bereich von 5 x 10<-5> bis 5 x 10<-4>, noch bevorzugter im Bereich von 1,5 x 10<-4> bis 3,0 x 10<-4>, und am bevorzugtesten in einer Konzentration von etwa 1,5 x 10<-4> enthalten. Das mit Wasser nicht mischbare Lösungsmittel ist zweckmässigerweise in einer Menge von bis zu 30 Gew.-% der Zusammensetzung, noch geeigneter 5-25% vorzugsweise 10-20% und insbesondere etwa 10% enthalten. Es kann eine grosse Zahl mit Wasser nicht mischbarer Lösungsmittel in den Zusammensetzungen der vorliegenden Erfindungen verwendet werden. Üblicherweise handelt es sich bei dem mit Wasser nicht mischbaren Lösungsmittel um ein Pflanzenöl, beispielsweise Sojabohnen-, Saflor-, Baumwollsamen-, Mais-, Sonnenblumen-, Erdnuss-, Rizinus- oder Olivenöl. Vorzugsweise handelt es sich bei dem Pflanzenöl um Sojabohnenöl. Alternativ handelt sich bei dem mit Wasser nicht mischbaren Lösungsmittel um einen Ester einer mittleren oder langkettigen Fettsäure, beispielsweise ein Mono-, Di- oder Triglycrid, oder es handelt es sich um ein chemisch modifiziertes oder hergestelltes Material wie Ölsäurethylester, Myristinsäureisopropylester, Palmitinsäureisopropylester, einen Glycerolester oder hydriertes Polyoxyl-Rizinusöl. Alternativ kann es sich bei dem mit Wasser nicht mischbaren Lösungsmittel um Seetieröl handeln, beispielsweise Lebertran oder ein anderes, von Fisch stammendes Öl. Geeignete Lösungsmittel sind ferner beispielsweise fraktionierte Öle, z.B. fraktioniertes Kokosnussöl oder modifiziertes Sojabohnenöl. Ausserdem können die Zusammensetzungen der vorliegenden Erfindung ein Gemisch aus zwei oder mehreren der obigen mit Wasser nicht mischbaren Lösungsmittel enthalten. Die Zusammensetzung der Erfindung wird geeigneterweise so formuliert, dass sich ein physiologisch neutraler pH-Wert ergibt, der üblicherweise im Bereich von 6,0-8,5 liegt, und zwar ggf. mithilfe von Alkali wie Natriumhydroxid. Geeignete grenzflächenaktive Mittel sind beispielsweise synthetische nichtionische grenzflächenaktive Mittel, beispielsweise ethoxylierte Ether und Ester und Polypropylen-Polyenthylen-Block-Copolymere und Phosphatide, beispielsweise natürlich vorkommende Phosphatide, wie Ei- und Sojaphosphatide, und modifizierte oder künstlich manipulierte Phosphatide (beispielsweise durch physikalische Fraktionierung und/oder Chromatographie hergestellt), oder Gemische davon. Bevorzugte grenzflächenaktive Mittel sind Ei- und Sojaphosphatide. Die Zusammensetzung der Erfindung kann durch die Zugabe eines geeigneten Tonizitätsreglers, beispielsweise Glycerol, mit Blut isotonisch gemacht werden. Bei den Zusammensetzungen der vorliegenden Erfindung handelt es sich üblicherweise um sterile wässerige Formulierungen, die nach üblichen Herstellungsverfahren hergestellt werden, beispielsweise unter Einhaltung aseptischer Bedingungen oder indem sie am Schluss durch Autoklavieren sterilisiert werden. Intravenöse Fettemulsionen (auch als parenterale Ernährungsemulsionen bekannt) werden gewöhnlich durch Infusion an Patienten verabreicht, die zusätzliche Kalorien und angemessene Ernährung benötigen, wenn dies oral oder durch andere Mittel nicht erwünscht oder nicht möglich ist. Die intravenösen Fettemulsionen halten üblicherweise eine positive Stickstoffbilanz aufrecht und stellen eine angemessene Energiequelle (z.B. in Form von Fett), Vitamine und Spurenelemente zur Verfügung. Derartige Emulsionen werden üblicherweise auf Intensivstationen verwendet, aber auch in anderen Krankenhaus-einrichtungen und im privaten Bereich. Beispiele für derartige intravenöse Fettemulsionen sind Intralepid (von Pharmacia vertrieben), Lipofundin (Braun) und Travamulsion (Baxter). Intralipid, Lipofundin and Travamulsion sind jeweils Warenzeichen. Nach einem weiteren Aspekt der vorliegenden Erfindung wird eine intravenöse Fettemulsion bereitgestellt, die eine Menge an Edetat enthält, die ausreicht, um mindestens 24 Stunden signifikantes Mikroorganismenwachstum zu verhindern. Insbesondere stellt die vorliegende Erfindung eine sterile wässerige Zusammensetzung zur parenteralen Verabreichung bereit, die eine Öl-in-Wasser-Emulsion aufweist, in der ein mit Wasser nicht mischbares Lösungsmittel mit Wasser emulgiert und mit einem grenzflächenaktiven Mittel stabilisiert ist, und die ausserdem eine Menge Edetat enthält, die ausreicht, um mindestens 24 Stunden signifikantes Mikroorganismenwachstum zu verhindern. Darüber hinaus ist vorgeschlagen worden, dass verschiedene Arzneimittel in Form von Öl-in-Wasser-Emulsionen verabreicht werden können, vgl. beispielsweise das US-Patent 4 168 308. Geeignete therapeutische oder pharmazeutische Mittel sind solche, die parenteral in einer Öl-in-Wasser-Emulsion verabreicht werden können. Üblicherweise handelt es sich bei derartigen Mitteln um lipophile Verbindungen, beispielsweise antifungale Mittel, Anästhetika, antibakterielle Mittel, Antikrebsmittel, Antiemetika, Mittel, die auf das zentrale Nervensystem wirken, beispielsweise Diazepam, Steroide, Barbiturate und Vitamin-Präparationen. Claims (10)
1. Sterile wässerige Zusammensetzung zur parenteralen Verabreichung, die eine Öl-in-Wasser-Emulsion aufweist, in der ein mit Wasser nicht mischbares Lösungsmittel mit Wasser emulgiert und mit einem grenzflächenaktiven Mittel stabilisiert ist, und die ausserdem eine Menge Edetat enthält, die ausreicht, um nach zufälliger Kontamination von aussen mindestens 24 Stunden signifikantes Mikroorganismenwachstum zu verhindern.
2. Zusammensetzung nach Anspruch 1, dadurch gekennzeichnet, dass die Menge Edetat eine molare Konzentration im Bereich von 3 x 10<-5> bis 9 x 10<-4> ist.
3. Zusammensetzung nach Anspruch 1, in Form einer intravenösen Fettemulsion.
4. Intravenöse Fettemulsion nach Anspruch 3, dadurch gekennzeichnet, dass die Edetat-Quelle Dinatriumedetat ist.

Intravenöse Fettemulsion nach Anspruch 3 oder 4, dadurch gekennzeichnet, dass sie bis zu 30 Gew.-%, beispielsweise 10 bis 20 Gew-%, eines mit Wasser nicht mischbaren Lösungsmittels enthält.
6. Intravenöse Fettemulsion nach einem der Ansprüche 3 bis 5, dadurch gekennzeichnet, dass das grenzflächenaktive Mittel ein natürlich vorkommendes Phosphatid, beispielsweise Eiphosphatid oder Sojaphosphatid, ist.
7. Intravenöse Fettemulsion nach einem der Ansprüche 3 bis 6, dadurch gekennzeichnet, dass der pH-Wert zwischen 6,0 und 8,5 liegt.
8. Intravenöse Fettemulsion nach Anspruch 7, dadurch gekennzeichnet, dass Natriumhydroxid vorhanden ist.
9. Intravenöse Fettemulsion nach einem der Ansprüche 3 bis 8, dadurch gekennzeichnet, dass sie mit Blut isotonisch ist, beispielsweise dass sie durch Zugabe von Glycerin mit Blut isotonisch gemacht ist.

Sterile wässerige Zusammensetzung nach einem der Ansprüche 1 bis 9, dadurch gekennzeichnet, dass das mit Wasser nicht mischbare Lösungsmittel aus pflanzlichen Ölen, beispielsweise aus Sojabohnen-, Saflor-, Baumwollsamen-, Mais-, Sonnenblumen-, Erdnuss-, Rizinus- oder Olivenöl; aus Seetierölen, beispielsweise aus Lebertran oder einem anderen, von Fisch stammenden Öl; und aus den fraktionierten Ölen, beispielsweise aus fraktioniertem Kokosnussöl oder modifiziertem Sojabohnenöl; ausgewählt ist.
