Description
Bakgrunn for oppfinnelsen
Den farmasøytiske industri anvender forskjellige metoder for å sammensette farmasøy-tiske midler i tablettformuleringer. I særdeleshet er våtgranulering en av de mest rådende metoder.
Et uttall av bisfosfonsyrer er blitt beskrevet å være anvendbare ved behandling og forhindring av sykdommer innbefattende benresorpsjon. Representative eksempler kan finnes i følgende US patentskrift nr. 3 962 432; US patentskrift nr. 4 054 598; US patentskrift nr. 4 267 108; US patentskrift nr. 4 327 039; US patentskrift nr. 4 621 077; US patentskrift nr. 4 624 947; US patentskrift nr. 4 746 654; US patentskrift nr. 4 922 077; og EPO patentpublika-sjon nr. 0 252 504. Standardmetoder for tablettformulering av bisfosfonsyrer lider imidlertid av alvorlige vanskeligheter.
I særdeleshet kan bisfosfonsyrer som bærer en basisk nitrogenholdig funksjonalitet inter-agere med laktosen i standardformuleringer resulterende i misfarging, ustabilitet og styrketap. Denne nedbrytning av den aktive bestanddel er særlig uttalt i nærvær av vann og/eller forhøyet temperatur. Det spekuleres på om denne uforenlighet spesifikt skyldes Maillard (eller "bru-nings")-reaksjonen hvori den frie aminogruppe av bisfosfonsyren reagerer med den "glykosi-diske" hydroksylgruppe av et sukker (slik som laktose), som sluttelig resulterer i dannelsen av brunpigmenterte nedbrytningsprodukter. Selv om dette problemet kan unngås ved eliminering av laktose, er anvendelse av laktose som et inert fortynningsmiddel generelt ønskelig.
Foreliggende oppfinnelse løser dette problemet ved å tilveiebringe en fremgangsmåte som unngår slik interaksjon mellom bisfosfonsyren og laktosen i formuleringen. I tillegg tilveie-bringer også foreliggende oppfinnelse en prosessfordel, da den bare krever blanding av bestand-delene uten granulering eller tilsetning av vann før sammenpressing.
Beskrivelse av oppfinnelsen
Foreliggende oppfinnelse angår således en fremgangsmåte for fremstilling av en tablett inneholdende som aktiv bestanddel 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre, eller et farmasøytisk akseptabelt salt derav, som er kjennetegnet ved at den omfatter dannelse av en blanding av den aktive bestanddel med: et fortynningsmiddel som er vandig, hurtigstrømmende laktose, et tørt bindemiddel, et oppbrytende middel, og eventuelt én eller flere ytterligere bestanddeler valgt fra gruppen bestående av; sammenpressingshjelpestoffer, smaksgivende midler, smaksforøkere, søtningsmidler og konserveringsmidler; smøring av blandingen med magnesiumstearat; og direkte sammenpressing av den resulterende smurte blanding til en ønsket tablettform.
Et foretrukket tørt bindemiddel er cellulose. I særdeleshet er mikrokrystallinsk cellulose foretrukket. Mikrokrystallinsk cellulose er tilgjengelig kommersielt under varemerket "Avicel" fra FMC Corporation.
Det oppbrytende middel kan være én av flere modifiserte stivelser eller modifiserte cellu-losepolymerer, i særdeleshet er crosscarmellosenatrium foretrukket. Crosscarmellosenatrium NF, type A, er kommersielt tilgjengelig under varemerket "Ac-di-sol".
Foretrukne smøremidler innbefatter magnesiumstearat.

I særdeleshet kan metoder for fremstilling av 4-amino-l-hydroksybutyliden-1,1 -bisfosfonsyre og 4- amino-1 -hydroksybuty liden-1,1 -bisfosfonsyremononalriumsalttrihydrat finnes i US patentskrift nr. 4 407 761 og US patentskrift nr. 4 922 077.
De farmasøytiske akseptable salter av 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre kan også anvendes i foreliggende oppfinnelse. Eksempler på basesalter av bisfosfonsyrer innbefatter ammoniumsalter, alkalimetallsalter slik som kalium- og natrium(innbefattende mono-, di- og trinatrium)-salter (som er foretrukne), jordalkalimetallsalter slik som kalsium- og magnesiumsalter, salter med organiske baser slik som disykloheksylaminsalter, N-metyl-D-glukamin og salter med aminosyrer slik som arginin, lysin osv. De ikke-toksiske, fysiologisk akseptable salter er foretrukne. Saltene kan fremstilles ved metoder kjent innen faget, slik som for eksempel i US patentskrift nr. 4 922 077.
I foreliggende oppfinnelse er bisfosfonsyren 4-amino-l-hydroksybutyliden-l,l-bisfosfonsyre. Mer foretrukket er det at bisfosfonsyren er et natriumsalt av 4-amino-l-hydroksybutyliden-l,l-bisfosfonsyre, i særdeleshet 4-amino-l-hydroksybutyliden-l,l-bisfosfonsyremononatriumsalttrihydrat.
En annen utførelsesform av foreliggende oppfinnelse er en tablett inneholdende som aktiv bestanddel 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre eller et farmasøytisk akseptabelt salt derav, kjennetegnet ved at tabletten er fremstilt ved fremgangsmåten ifølge oppfinnelsen.
Generelt omfatter disse tablettene på vektbasis ca. 0,5-40 vekt% av 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre, eller et farmasøytisk akseptabelt salt derav som aktiv bestanddel; og fra ca. 60 til 99,5 vekt% av bearbeidelseshjelpestoffer, uten noe vann tilsatt. Nærmere bestemt er bearbeidelseshjelpestoffene et fortynningsmiddel, et tørt bindemiddel, et oppbrytende middel og et smøremiddel. Foretrukne bearbeidelseshjelpestoffer innbefatter; vandig, hurtigstrømmende laktose; mikrokrystallinsk cellulose; crosscarmellosenatrium; og magnesiumstearat.
Foretrukne tabletter omfatter ca. 0,5-40 vekt% av 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre, eller et farmasøytisk akseptabelt salt derav som aktiv bestanddel; ca. 10-80 vekt% vandig, hurtigstrømmende laktose; ca. 5-50 vekt% mikrokrystallinsk cellulose; ca. 0,5-10 vekt% crosscarmellosenatrium; og ca. 0,1-5 vekt% magnesiumstearat.
Tablettene kan for eksempel være fra 50 mg til 1,0 g i nettovekt, fortrinnsvis 100-500 mg nettovekt, og helst 200-300 mg nettovekt.
Mer foretrukne tabletter ifølge foreliggende oppfinnelse omfatter: ca. 0,5-25 vekt% av 4-amino-l-hydroksybutyliden-1,1-bisfosfonsyre, eller et farmasøytisk akseptabelt salt derav; ca. 30-70 vekt% vandig, hurtigstrømmende laktose ; ca. 30-50 vekt% mikrokrystallinske cellulose; ca. 0,5-5 vekt% crosscarmellosenatrium; og ca. 0,1-2 vekt% magnesiumstearat.
Foretrukne tabletter man forestiller seg for kommersiell utvikling, er som følger.
Tabletter med 2,5 mg styrke av fri syre:
ca. 1,63 vekt% 4-arnmo-l-hydroksybutyliden-l,l-bisfosfonsyremononatirumsalttirhydrat;
ca. 56,87 vekt% laktose; ca. 40 vekt% mikrokrystallinsk cellulose; ca. 1 vekt% crosscarmellosenatrium; og ca. 0,5 vekt% magnesiumstearat.
Tabletter med 5 mg styrke av fri syre:
ca. 3,25 vekt% 4-ammc-l-hydroksybutyliden-l,l-bisfosfomyremononatiriunsalttirhydrat;
ca. 55,25 vekt% laktose; ca. 40 vekt% mikrokrystallinsk cellulose; ca. 1 vekt% crosscarmellosenatrium; og ca. 0,5 vekt% magnesiumstearat.
Tabletter med 25 mg styrke av fri syre:
ca. 16,4 vekt% 4-ammo-l-hydroksybutyliden-M ca. 42,1 vekt% laktose; ca. 40 vekt% mikrokrystallinsk cellulose; ca. 1 vekt% crosscarmellosenatrium; og ca. 0,5 vekt% magnesiumstearat.
Tabletter med 50 mg styrke av fri syre:
ca, 21,8 vekt% 4-amino-l-hydroksybutyliden-l,l-bisfosfoiisyremononatri ca. 36,7 vekt% laktose; ca. 40 vekt% mikrokrystallinsk cellulose; ca. 1 vekt% crosscarmellosenatrium; og ca. 0,5 vekt% magnesiumstearat.
Oppfinnelsen gjelder således en tablett som er laget ved direkte kompresjon, kjennetegnet ved at den omfatter på vektbasis 0,5-40 vekt% av en aktiv bestanddel
4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre eller et farmasøytisk akseptabelt salt derav, og fra 60 til 99,5 vekt% av eksipienser bestående hovedsakelig av: vandig, hurtigstrømmende laktose; mikrokrystallinsk cellulose; crosscarmellosenatrium og magnesiumstearat.
Den gjelder også en tablett og som er laget ved direkte kompresjon, kjennetegnet ved at den omfatter på vektbasis 0,5-40 vekt% av en aktiv bestanddel 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre eller et farmasøytisk akseptabelt salt derav; 10-80 vekt% vandig, hurtigstrømmende laktose, 5-50 vekt% mikrokrystallinsk cellulose; 0,5-10 vekt% crosscarmellosenatrium; og 0,1-5 vekt% magnesiumstearat.
I tillegg gjelder oppfinnelsen en tablett som er laget ved direkte kompresjon kjennetegnet ved at den omfatter fra 0,5-40 vekt% av 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre eller et farmasøytisk akseptabelt salt derav, og fra 60-99,5 vekt% av eksipiensen, hvilke eksipienser omfatter et fortynningsmiddel som er vandig Fast Flo laktose, et bindemiddel, et oppbrytende middel og magnesiumstearat.
Tablettene ifølge oppfinnelsen kan også inneholde én eller flere ytterligere formuleringsbestanddeler som kan velges fra et vidt utall av eksipienser kjent innen det farmasøytiske formuleringsfag. I henhold til de ønskede egenskaper til tabletter kan ethvert antall bestanddeler velges, alene eller i kombinasjon, basert på deres kjente anvendelser ved fremstilling av tablettpreparater. Slike bestanddeler innbefatter, men er ikke begrenset til, fortyn-ningsmidler, sammenpressingshjelpestoffer, oppbrytende midler, smøremidler, bindemidler, smaksgivende midler, smaksforøkere, søtningsmidler og konserveringsmidler.
Uttrykket "tablett" som anvendt her, er beregnet på å omfatte sammenpressede farmasøy-tiske doseringsformuleringer av alle former og størrelser, enten belagt eller ubelagt. Substanser som kan anvendes for belegging, innbefatter hydroksypropylmetylceilulose, hydroksy-propylcellulose, titandioksid, talkum, søtningsmidler og fargestoffer.
Tablettene ifølge oppfinnelsen er anvendbare ved terapeutisk eller profylaktisk behandling av sykdommer innen kalsium- eller fosfatmetabolismen og assosierte sykdommer. Disse sykdommer kan oppdeles i to kategorier: 1. Unormale (ektopiske) avsetninger av kalsiumsalter, for det meste kalsiumfosfat, patologisk hardning av vev og benmisdannelser. 2. Tilstander som kan høste fordel av en reduksjon i benresorpsjon. En reduksjon i benresorpsjon vil forbedre balansen mellom resorpsjon og dannelse, redusere bentap eller resultere i benøkning. En reduksjon i benresorpsjon kan endre smerten assosiert med osteolytiske lesjoner og redusere forekomsten og/eller veksten av disse lesjoner.
Disse sykdommer innbefatter: osteoporose (innbefattende østrogenmangel, immobilise-ring, glukokortikoidfremkalt og senil), osteodystrofi, Pagets sykdom, myositis ossificans, Bechterews sykdom, ondartet hyperkalsemi, metastatisk bensykdom, periodontal sykdom, chole-lithiasis, nephrolithiasis, urolithiasis, urincalculus, hardning av arteriene (sklerose), artritt, bursitt, neuritt og tetani.
Forøkt benresorpsjon kan ledsages av patologisk høye kalsium- og fosfatkonsentrasjoner i plasma, hvilke vil kunne lindres ved anvendelse av foreliggende farmasøytiske preparater.
De etterfølgende eksempler er gitt med det formål å illustrere foreliggende oppfinnelse.
Eksempel 1
Prosedyre for fremstilling av tabletter med 5 mg styrke av 4- amino- l- hvdroksvbutvliden- l. l-bisfosfonsvre
Den aktive bestanddel (ekvivalent med 5 mg vannfri, fri syre pr. tablett) ble forblandet med en tredjedel av den mikrokrystallinske cellulose NF og halvparten av den vannfrie laktose NF i en bomblander i 5 minutter ved 20 omdreininger pr. minutt. Til forblandingen ble det tilsatt de gjenværende to tredjedeler av den mikrokrystallinske cellulose NF og den gjenværende halv-part av den vannfrie laktose NF. Dette ble blandet i 10 minutter ved 20 omdreininger pr. minutt. Crosscarmellosenatrium ble tilsatt til de blandede pulvere og ble blandet i 5 minutter ved 20 omdreininger pr. minutt. Sluttelig ble magnesiumstearat tilsatt til blandingen ved føring gjennom en 90 mesh sikt og ble blandet ytterligere i 5 minutter ved 20 omdreininger pr. minutt. Den smurte blanding ble presset under dannelse av tabletter med 5 mg aktiv bestanddel.

Eksempel 7
Stabilitetsstudier
Tablettformuleringer av den aktive bestanddel (ekvivalent med 5 mg vannfri, fri 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre pr. tablett) ble fremstilt under forskjellige betingelser med forskjellige eksipienser. De direkte sammenpressingstabletter ble fremstilt i henhold til prosedyren ifølge eksempel 1, og våtgranulerte tabletter ble fremstilt i henhold til prosedyren angitt nedenfor. Tablettene ble underkastet stabilitetsstudier under åpne skålbetingelser ved 40 °C/75 % relativ fuktighet. Følgende observasjoner ble observert:
1. Tablettmisfarging oppstod innen 2 uker i formuleringer som ble fremstilt ved våtgranulering og inneholdt vannfri laktose. 2. Tablettmisfarging oppstod innen 4 uker i formuleringer som ble fremstilt ved våtgranulering og inneholdt vandig laktose. 3. Det var ingen tablettmisfarging etter 4 uker i formuleringer som ble fremstilt som en direkte sammenpressings(tørrblanding)formulering. Undersøkelser av den aktive bestanddel bekreftet at det ikke var noe tap av styrke eller dannelse av nedbrytningsprodukter over den samme periode. Tabell I viser stabilitetskarakteristikkene for den direkte sammenpressings-
formulering sammenlignet med en våtgranulert formulering.

Prosess for våtgranuleringstabletter
1) Vannfri laktose, alendronatnatrium og mikrokrystallinsk cellulose ble blandet i en blander av egnet størrelse. 2) Blandingen ble forsert gjennom mesh nr. 30, etterfulgt av reblanding i et ytterligere tidsrom. 3) Pulverblandingen ble granulert med en tilstrekkelig mengde vann inntil klump-dannelse oppstod.
4) Den måte masse ble ført gjennom en sikt nr. 5.
5) Den våtsiktede granulering ble tørket i en drevet lufttørker ved 40-50 °C inntil tapet ved tørking var mindre enn 2 % ved 105 °C.
6) Tørrgranuleringen ble siktet gjennom en egnet sikt.
7) Den tørre, siktede granulering ble blandet med crosscarmellosenatrium, etterfulgt av magnesiumstearat. 8) Tabletter ble sammenpresset under anvendelse av den merkede granulering.
Claims (11)
1. Fremgangsmåte for fremstilling av en tablett inneholdende som aktiv bestanddel 4-amino-l-hydroksybutyliden-l, 1-bisfosfonsyre, eller et farmasøytisk akseptabelt salt derav, karakterisert ved at den omfatter dannelse av en blanding av den aktive bestanddel med: et fortynningsmiddel som er vandig, hurtigstrømmende laktose, et tørt bindemiddel, et oppbrytende middel, og eventuelt én eller flere ytterligere bestanddeler valgt fra gruppen bestående av; sammenpressingshjelpestoffer, smaksgivende midler, smaksforøkere, søtningsmidler og konserveringsmidler; smøring av blandingen med magnesiumstearat; og direkte sammenpressing av den resulterende smurte blanding til en ønsket tablettform.
2. Fremgangsmåte ifølge krav 1,
karakterisert ved at den aktive bestanddel er: 4-amino-1 -hyrdoksybutyliden-1,1 -bisfosfonsyre-mononatriunisalttirhydrat.
