Claims (1)
- 【特許請求の範囲】 1.患者の上行大動脈を冠状動脈心門と腕頭動脈との間で分割するためのカテー テル装置であって、遠位端と、近位端と、その両者の間に伸びる第1内部管腔 と、上記第1内部管腔と流体連通する上記遠位端にある開口とを有する長く伸 びたシャフトと、心臓の収縮と膨張により上行大動脈を通って流れる全ての血 液流を実質的に遮断するように冠状動脈心門と腕頭動脈との間で上行大動脈を 閉塞するための、上記シャフトの遠位端近くで上記第1管腔の上記開口より近 位にある、そして第2の側よりも大きい寸法に膨張する第1の側と上記第2の 側とを有するように偏心的に膨張可能である拡張可能手段とから成るカテーテ ル装置。 2.上記長く伸びたシャフトが、概ね患者の動脈弓と合致し遠位端が上行大動脈 内に位置することができるように形成された予め成形された遠位部を有するこ とを特徴とする上記請求項1に記載のカテーテル装置。 3.上記長く伸びたシャフトの上記予め成形された遠位部が内側部曲線と外側部 曲線を持つ湾曲を有し、上記拡張可能手段が上記拡張可能手段のより大きな第 1の側が曲線の外側を向き上記拡張可能手段の第2の側が曲線の内側を向くよ うに上記カテーテルシャフト上で向けられていることを特徴とする上記請求項 2に記載のカテーテル装置。 4.上記長く伸びたシャフトの予め成形された遠位部の湾曲が約135°ないし 約225°の角度を含むことを特徴とする上記請求項3に記載のカテーテル装 置。 5.上記拡張可能手段が膨張可能な弾性バルーンから成ることを特徴とする上記 請求項1に記載のカテーテル装置。 6.上記長く伸びたシャフトが上記膨張可能な弾性バルーン内の内部空間と流体 滑らかなコーティングが施されていることを更に含む、上記請求項87に記載 のカニューレ。 101.上記カニューレ本体の少なくとも遠位部の外表面上に滑らかなコーティ ングが施されていることを更に含む、上記請求項87に記載のカニューレ。 102.上記カニューレ本体が、上記カニューレ本体の近位外径よりも小さな遠 位外径を有するテーパの付いた遠位部を有することを特徴とする上記請求項8 7に記載のカニューレ。 103.上記カニューレ本体の少なくとも遠位部が、上記カニューレ本体の壁に 埋め込まれたコイル状のワイヤで補強されていることを特徴とする上記請求項 87に記載のカニューレ。 104.患者の大動脈を冠状動脈心門と腕頭動脈との間で閉塞するために、遠位 端が上記カテーテル挿入管腔を通過できる寸法となっている大動脈分割カテー テル手段と組み合わされた上記請求項87に記載のカニューレ。 105.カニューレを血管内に設置するための近位取り付け部品用のアダプター であって、近位端と、遠位端と、両者の間の第1管腔とを有する第1脚と、近 位端と、遠位端と、両者の間の第2管腔とを有し、第2管腔が第1管腔と液体 連通するように第2腕の遠位端が第1腕に取り付けてある第2脚と、第1管腔 を通る液体流を選択的に遮断するための、第1腕の遠位部にある第1止血手段 と、第1管腔を通る液体流を選択的に遮断するための、第1腕の近位部にある 第2止血手段と、アダプターをカニューレの近位端に取り付けるための手段と から成るアダプター。 106.長く伸びた医療器具を選択された位置に固定するための再配置可能固定 装置であって、上記長く伸びた医療器具をその中に滑動可能に収容し、その内 表面上に高摩擦材を有する管腔を内部に通して有する管状部材と、上記管状部 材を上記長く伸びた医療器具に対し摩擦固定するための上記長く伸びた医療器 具の周囲に上記管状部材を押しつけるための手段とから成る再配置可能固定装 置。 107.上記管状部材が、上記管状部材を上記長く伸びた医療器具の周囲に押し つけ易くする、少なくとも上記管状部材の部分に沿って上記管腔と連通する長 手方向のスロットを有することを特徴とする上記請求項106に記載の再配置 可能固定装置。 108.上記高摩擦材が、上記管腔の内表面と上記長く伸びた医療器具との間の 摩擦を増やすため、上記管腔の内表面に接着した粒子から成ることを特徴とす る上記請求項106に記載の再配置可能固定装置。 109.上記長く伸びた医療器具の周囲に上記管状部材を押しつけるための上記 手段が、上記管状部材の外表面の周囲に結ばれた縫合糸又は結紮糸から成るこ とを特徴とする上記請求項106に記載の再配置可能固定装置。 110.上記管状部材の上記外表面が、上記縫合糸又は結紮糸を上記管状部材に 固定するための手段を更に含むことを特徴とする上記請求項106に記載の再 配置可能固定装置。 111.上記縫合糸又は結紮糸を上記管状部材に固定するための上記手段が、上 記管状部材の外表面の少なくとも一本の円周状の溝から成ること特徴とする上 記請求項106に記載の再配置可能固定装置。 112.上記管状部材が、上記管状部材を上記長く伸びた医療器具の周囲に押し つけ易くする、少なくとも上記管状部材の部分に沿って上記管腔と連通する長 手方向のスロットを有し、上記高摩擦材が、上記管腔の内表面と上記長く伸び た医療器具との間の摩擦を増やすため、上記管腔の内表面に接着した粒子から 成り、上記長く伸びた医療器具の周囲に上記管状部材を押しつけるための上記 手段が、上記管状部材の外表面の周囲に結ばれた縫合糸又は結紮糸から成り、 上記管状部材が上記縫合糸又は結紮糸を上記管状部材に固定するための上記管 状部材の外表面の少なくとも一本の円周状の溝から成ること特徴とする上記請 求項106に記載の再配置可能固定装置。
