Abstract
Foreliggende søknad omhandler en anvendelse av en blanding omfattende (1) et anti-tumor legemiddel konjugert til en vannløselig polymer valgt fra gruppen bestående av poly (l-glutaminsyre), poly (d-glutaminsyre), poly (dl-glutaminsyre), poly (l-asparginsyre), poly (d-asparaginsyre) og poly (dl-asparginsyre) og hvor (a) nevnte anti-tumor legemiddel er paclitaxel, docetaxel, etopsid, teniposid, camptothecin eller epithilon, og (b) nevnte polygutaminsyrer eller polyasparginsyrer har en molekylvekt på fra 5000 til 100 000 dalton og (2) et platina legemiddel for fremstilling av et legemiddelpreparat for behandling av (i) kreft, (ii) systemisk autoimmun sykdom eller (iii) arteriell restenose eller arteriell okklusjon i et individ. ?? ?? ?? ?? 1
Description
OPPFINNELSENS OMRÅDE
Den foreliggende oppfinnelse dreier seg om anvendelse av en blanding av et vannløselig anti-tumor legemiddel og et platina legemiddel for fremstilling av et legemiddelpreparat for behandling av sykdom, spesielt kreft.
OPPFINNELSENS BAKGRUNN
Paclitaxel, et anti-mikrotubul-middel ekstrahert fra nåler og bark fra stillehavs-barlindtreet, Taxus brevifolia, har vist en oppsiktsvekkende anti-neoplastisk virkning i human kreft i fase I studier og i tidlige fase II og III prøver (Horwitz et al., 1993). Dette har primært blitt rapportert i langtkommen eggstokk-og brystkreft. Signifikant aktivitet har blitt dokumentert i småcelle og ikke-småcelle lungekreft, hode- og nakkekreft, og i føflekk-kreft med spredning. Imidlertid har en betydelig vanskelighet i utvikling av paclitaxel med henblikk på klinisk prøvebruk, vært at forbindelsen er uløselig i vann.
Docetaxel er semisyntetisk produsert fra 10-deacetyl baccatin III, en ikke-cytotoksisk forløper som er ekstrahert fra nålene til Taxus baccata og forestret med en sidekjede som er kjemisk syntetisert (Cortes and Pazdur, 1995). Ulike kreftcelle-linjer, deriblant bryst-, lunge-, eggstokk- og tykktarm- og endetarmkreft og føflekk-kreft har blitt funnet å være følsomme ovenfor docetaxel. I kliniske prøver har docetaxel blitt benyttet for å oppnå komplette eller delvise responser i bryst-, eggstokk-, hode- og nakkekreft, og i ondartet føflekk-kreft.
Paclitaxel blir vanligvis formulert som en konsentrert løsning som inneholder 6 mg paclitaxel per milliliter Cremophor EL (polyoksyetylert lakserolje) og dehydrert alkohol (50% v/v) og må bli fortynnet videre før tilførsel (Goldspiel, 1994). Mengden Cremophor EL som er nødvendig for å levere de nødvendige doser av paclitaxel er betydelig høyere enn det som tilføres med noen andre medikamenter som formuleres i Cremophor. En rekke toksiske virkninger har blitt tilskrevet Cremophor, deriblant blodåreutvidelse, tung pust og lavt blodtrykk. Dette bæremiddel har også blitt vist å fremkalle alvorlig overfølsomhet i laboratoriedyr og mennesker (Weiss et al., 1990). Faktisk er den maksimale dose paclitaxel som kan tilføres til mus ved i.v. bolusinjeksjon, bestemt av den akutte dødelige toksisitet av Cremophor bæremidlet (Eiseman et al., 1994). I tillegg er det kjent at Cremophor EL, som er et overflate-aktivt middel, fører til utlekking av ftalat bløtgjørere slik som di(2-etylheksyl)ftalat (DEHP)fra polyvinylkloridposer og de intravenøse tilførselsslanger. DHEP vites å være levertoksisk hos dyr, og er karsinogene hos gangere. Denne preparasjon av paclitaxel er også vist å danne fast materiale i løpet av en viss tid, og filtrasjon er derfor nødvendig under tilførsel (Goldspiel, 1994). Spesielle tiltak er derfor nødvendige for å preparere og tilføre paclitaxel-løsninger for at man kan sikre en sikker levering av medikamentet til pasienter, og disse tiltak fører nødvendigvis til høyere kostnader.
Tidligere forsøk på å fremskaffe vannløselig paclitaxel har bestått blant annet av preparering av paclitaxel pro-medikamenter ved å introdusere løsegjørende deler, slik som ravsyre og aminosyrer, ved 2'-hydroksylgruppen eller ved 7-hydroksyl-posisjonen (Deutsch et al., 1989; Mathew et al., 1992). Disse pro-medikamenter har imidlertid ikke vist seg å være så kjemisk stabile som nødvendig for utvikling. For eksempel rapporterer Deutsch et al. (1989) om et 2'-succinatderivat av paclitaxel, men vannløseligheten til natriumsaltet var bare ca. 0,1% og trietanolamin og N-metylglukaminsaltene var løselige bare ved ca. 1% konsentrasjon. I tillegg ble det rapportert at aminosyreesterne var ustabile. Lignende resultater ble rapportert av Mathew et al. (1992). Greenwald et al. rapporterte syntesen av meget vannløselig 2'- og 7-polyetylenglykolestere av taxol (Greenwald et al., 1994), men ingen data som beskrev anti-tumoraktivitet av disse forbindelser in vivo ble rapportert (Greenwald et al., 1995).
Andre forsøk på å løse disse problemer har dreiet seg om mikroenkapsulering av paclitaxel både i liposomer og i nanosfærer (Bartoni and Boitard, 1990). Det ble rapportert at liposomformuleringen var like effektiv som fri paclitaxel, men bare liposomformulering som inneholdt mindre enn 2% paclitaxel var stabile rent fysisk (Sharma and Straubinger, 1994). Dessverre viste det seg at den nanosfære formuleringen var toksisk. Det er derfor fremdeles et behov for en vannløselig paclitaxelformulering som kan levere effektive mengder av paclitaxel og docetaxel uten de ulemper som fremkalles av medikamentets uløselighet.
Et annet hinder for en utstrakt bruk av paclitaxel er de begrensede resursser som danner basis for paclitaxelproduksjon, noe som fører til at paclitaxelterapi er meget dyrt. En behandlingserie kan for eksempel koste flere tusen dollar. Videre responderer ikke alle tumorer til paclitaxelterapi, og denne ytterligere ulempe kan forårsakes av at paclitaxel ikke kommer inn i tumoren. Det er derfor et umiddelbart behov for effektive formuleringer av paclitaxel og relaterte medikamenter som er vannløselige, og som har lange halveringstider i serum, til behandling av tumorer, autoimmune sykdommer slik som kronisk leddgikt, og også for å forhindre restenose av blodårer som har vært utsatt for skade slik som angioplastikk og innføring av stent.
SAMMENDRAG AV OPPFINNELSEN
Den foreliggende oppfinnelse prøver å overvinne disse og andre problemer som er knyttet til tidligere fremgangsmåter, ved å frembringe anvendelse av blandinger som omfatter et kjemoterapeutisk og anti-angiogent medikament, slik som paclitaxel eller docetaxel, bundet til en vannløselig polymer slik som for eksempel polyglutaminsyre eller polyasparaginsyre. Disse blandinger blir her vist å være overraskende effektive som anti-tumormidler mot flere eksempler på tumormodeller, og forventes å være minst like effektive som paclitaxel eller docetaxel mot enhver av sykdommene eller tilstander der taxaner eller taxoider vites å være effektive. Oppfinnelsens blandinger gir vannløselige taxoider for å overvinne problemene som fremkalles av de uløselige medikamenter selv, og gir også fordelene med kontrollert frisetting slik at tumorer som vist her, kan fjernes i dyremodeller etter en enkel intravenøs tilførsel.
Det forstås også at den foreliggende oppfinnelses vannløselige konjugater kan anvendes sammen med et platinamedikament.
Konjugering av kjemoterapeutiske medikamenter til polymerer er en tiltrekkende fremgangsmåte for å redusere systemisk toksisitet og å forbedre den terapeutiske indeks. Polymerer som har molekylmasse større enn 30 kDa diffunderer ikke lett gjennom normale kapillærer og det glomerulære endotel i nyrene, noe som fører til at normalt vev ikke blir utsatt for irrelevant medikamentmediert toksisitet (Maeda and Matsumura, 1989; Reynolds, 1995). På den annen side er det godt beskrevet at ondartede tumorer ofte har ødelagt kapillært endotel og har større permeabilitet enn blodårene i normalt vev (Maeda and Matsumura, 1989; Fidler et al., 1987). Således kan et polymer medikamentkonjugat, som normalt ville forbli i blodbanen, selektivt lekke fra blodårer inn i tumorer, noe som resulterer i tumor-akkumulering av aktivt terapeutisk medikament. I tillegg kan polymermedikament-konjugater fungere som medikamentdepoter og gi langvarig frisetting, og derved føre til at tumorcellene blir eksponert for medikament over et lengre tidsrom. Til sist kan vannløselige polymerer bli benyttet for å stabilisere medikamentene, samt for å løsgjøre forbindelser som ellers er uløselige. For tiden har en rekke syntetiske og naturlige polymerer blitt undersøkt med henblikk på deres evne til å forbedre tumor-spesifikk levering av medikament (Kopecek, 1990, Maeda and Matsumura, 1989). Imidlertid er bare et fåtall for tiden under klinisk evaluering, deriblant SMANCS i Japan og HPMA-Dox i Storbritannia (Maeda, 1991; Kopecek and Kopeckova, 1993).
I den foreliggende beskrivelse blir et taxoid forstått å bety de forbindelser som inkluderer paclitaxel og docetaxel, og andre kjemikalier som har taxan-skjelettet (Cortes and Pazdur, 1995), og kan isoleres fra naturlige kilder slik som barlindtreet, eller fra cellekultur, eller kjemisk syntetiserte molekyler, og foretrukket er en kjemisk forbindelse med den generelle kjemisk formel C47H51NO14, deriblant [2aR-[2aa,4p, 4ap,6p,9a(aR*,pS*), 11 a, 12a, 12aa, 12ba]]-p-(benzoylamino)-a-hydroksybenzen-propansyre 6, 12b,bis(acetyloksy)-12-(benzoyloksy)-28,3,4,48,5,6,9,10,11,12,12a, 12b-dodecahydro-4,11 -dihydroksy-4a,8,13,13-tetrametyl-5-okso-7,11-metano-1 H-cyklodeca[3,4]benz-[1,2-b]okset-9-yl-ester. Det blir forstått at paclitaxel og docetaxel hver er mere effektiv enn den andre forbindelse mot visse tumortyper, og at i den foreliggende oppfinnelses anvendelse blir de tumorer som er mere følsomme overfor et spesifikt taxoid behandlet med det vannløselige taxoidkonjugat.
I visse utførelser av den foreliggende oppfinnelse kan paclitaxel eller docetaxel være konjugert til en vannløselig polymer, og fortrinnsvis blir polymeren konjugert til hydroksyl i 2' eller 7-posisjon, eller begge, i paclitaxel- eller docetaxel-molekylet. Således blir, når funksjonelle grupper blir benyttet for konjugering av medikament, slik som over med C2'-hydroksylet i paclitaxel, en degraderbar binding, i dette tilfelle en ester, benyttet for å sikre at det aktive medikament blir frigjort fra den polymere bærer. Foretrukne polymerer inkluderer poly(l-glutaminsyre), poly(d-glutaminsyre), poly(dl-glutaminsyre), poly(l-asparaginsyre), poly(d-asparaginsyre), poly(dl-asparaginsyre), polyetylen-glykol, kopolymerer av de ovenfor opplistede polyaminosyrer med polyetylenglykol, polykaprolakton, polyglykolsyre og polymelkesyre, samt også polyakrylsyre, poly(2-hydroksyetyl-1- glutamin), karboksymetyl-dekstran, hyaluronsyre, humant serum-albumin og alginsyre, med polyetylenglykol, poly-asparaginsyrer og polyglutamin-syrer som de spesielt foretrukne. Polyglutaminsyrene eller polyasparaginsyrene fra den foreliggende oppfinnelse har fortrinnsvis en molekylvekt på 5000 til 100000, med ca. 20000 til ca. 80000 eller til og med ca. 30000 til ca. 60000 som de mest foretrukne.
Det blir forstått at den foreliggende oppfinnelses blandinger kan bli dispergert i en farmasøytisk akseptabel bærerløsning som beskrevet nedenfor. En slik løsning ville være steril eller aseptisk og kan bestå av vann, buffere, isotone midler eller andre ingredienser kjent for de som er øvet i faget, som ikke ville fremkalle allergiske eller andre skadelige reaksjoner når de ble tilført til et dyr eller til et menneske.
Den foreliggende oppfinnelse kan også i visse utførelser bli beskrevet som en anvendelse for fremstilling av medikamenter for å behandle kreft i et individ. Denne anvendelsen inkluderer å fremskaffe en blanding som omfatter et kjemoterapeutisk medikament slik som paclitaxel eller docetaxel, konjugert til en vannløselig polymer og finfordelt i en farmasøytisk akseptabel løsning, og tilføring av løsningen til individet i en mengde som effektivt kan behandle tumor. Foretrukne blandinger omfatter paclitaxel eller docetaxel konjugert til polyglutaminsyrer eller polyasparaginsyrer, og mer fordelaktig til poly(l-glutaminsyre) eller poly(l-asparaginsyre). Oppfinnelsens blandinger er underforstått å være effektive mot enhver type av kreft der det ikke-konjugerte taxoid er vist å være effektivt, og ville inkludere brystkreft, eggstokk-kreft, ondartet føflekk-kreft, lungekreft, mavesekk-kreft, tykktarms-kreft, hode- og nakke-kreft eller leukemi.
Anvendelsen ved å behandle en tumor kan inkludere et estimat av paclitaxel eller docetaxel opptak i tumor før tilførsel av en terapeutisk mengde av legemidlet eller pro-legemidlet. Denne fremgangsmåte kan bestå av enhver av de billedskapende teknikkene, der et metall bundet til et paclitaxel-chelatorkonjugat blir tilført et individ og påvist i en tumor. Dette trinn gir en kostnads-effektiv måte for å bestemme at en spesiell tumor ikke ville forventes å respondere på DTPA-paclitaxelterapi i de tilfeller der legemidlet ikke kommer inn i tumoren. Det antas at dersom en billedteknikk kan benyttes for å forutse responsen til paclitaxel og for å identifisere pasienter som trolig ikke vil respondere, kan betydelige utgifter og viktig tid spares for pasienten. Antagelsen er at dersom det ikke er noen rimelig mengde av kjemoterapeutisk middel nedfelt i tumoren, er sannsynligheten for at tumoren skal respondere på midlet relativt liten.
Observasjonen at paclitaxel også hemmer restenose etter ballong-angioplastikk indikerer at de vannløselige paclitaxeler og docetaxeler fra den foreliggende oppfinnelse kan finne en rekke applikasjonsområder i tillegg til direkte parenteral tilførsel (WO 9625176). For eksempel forventes det at vannløselig paclitaxel vil være hensiktsmessig som en overflatedekking på implanterte medisinske utstyrsenheter, slik som slanger, shunter, katetere, kunstige implantater, nagler, elektriske implantater slik som pacemakere, og spesielt for arterielle eller venøse stenter, deriblant ballong-ekspanderbare stenter. I disse utførelser forventes det at vannløselig paclitaxel kan være bundet til en implanterbar medisinsk utstyrs-enhet, eller alternativt at det vannløselige paclitaxel kan være passivt adsorbert til overflaten av denne implanterbare enhet. For eksempel kan stenter dekkes med polymer-legemiddelkonjugater ved at stenten blir dyppet i polymer-legemiddel-løsning eller at stenten blir sprayet med denne løsning. Passende materialer for det implanterte utstyr bør være biokompatibelt og ikke-toksisk, og kan velges fra metaller slik som nikkel-titaniumlegeringer, stål, eller biokompatible polymerer, hydrogeler, polyuretaner, polyetylener, etylvinylacetat-kopolymerer, etc. Det vannløselige paclitaxel, spesielt et PG-paclitaxelkonjugat, dekkes på en stent som skal føres inn i en arterie eller vene etter ballong-angioplastikk.
Dersom ikke definert på annen måte har alle tekniske og vitenskapelige begreper som benyttes her, den samme betydning som de vanligvis har og blir forstått av en med vanlig kunnskap i faget som denne oppfinnelse hører til.

DETALJERT BESKRIVELSE AV OPPFINNELSEN
Den foreliggende oppfinnelse kommer fra oppdagelsen av nye, vannløselige formuleringer av paclitaxel og docetaxel, og den overraskende effektivitet disse formuleringer har mot tumorceller in vivo. Paclitaxel konjugert til poly(l-glutaminsyre) (PG-paclitaxel) tilført ti/ mus som har eggstokk- karsinom ( OCa- l) ga betydelig forsinket tumorvekst sammenlignet med den samme dose paclitaxel uten PG. Mus behandlet med paclitaxel alene eller med en kombinasjon av fritt paclitaxel og PG, viste forsinket tumorvekst initialt, men tumorene vokste tilbake til nivåer som var sammenlignbare med en ubehandlet kontrollgruppe etter 10 dager. Ved den maksimalt tolererbare dose (MTD) av PG-paclitaxel-konjugatet (160 mg ekvivalent paclitaxel/kg) var videre tumorveksten totalt supprimert, tumorene skrumpet inn, og mus som ble observert i 2 måneder etter behandling forble tumorfri (MTD: definert som den maksimale dose som produserte 15% eller mindre av kroppsvekttap innen 2 uker etter en enkel i.v. injeksjon). I en parallell studie ble anti-tumoraktiviteten til PG-paclitaxel undersøkt i rotter med rotte brystadenokarsinom (13762F). Igjen ble komplett tumorutryddelse observert ved 40 til 60 mg ekvivalent paclitaxel/kg av et PG-paclitaxelkonjugat. Disse overraskende resultater demonstrerer at polymer legemiddelkonjugatet PG- paclitaxel effektivt utrydder veletablerte faste tumorer i både mus og rotter etter en enkel intravenøs injeksjon. Videre er PG-paclitaxel, med en halv-levetid på 40 dager ved pH 7,4, et av de mest stabile vannløselige paclitaxel-derivater som er kjent (Deutsch et al., 1989; Mathew et al., 1992; Zhao and Kingston, 1991).
De nye forbindelser og fremgangsmåter fra den foreliggende oppfinnelse gir betydelige fremskritt i forhold til tidligere fremgangsmåter og blandinger, siden de vannløselige paclitaxeler forventes å forbedre effektiviteten av paclitaxel-basert anti-kreftterapi, ved å gi vannløselige paclitaxel-avledede blandinger med kontrollert frisetting. Slike blandinger eliminerer behovet for løsemidler som assosieres med bivirkninger som sees med tidligere paclitaxelblandinger
Paclitaxel kan bli gjort vannløselig på to måter: ved å konjugere paclitaxel til vannløselige polymerer som fungerer som legemiddelbærere, og ved å derivatisere anti-tumorforbindelsen med vannløselige chelaterende midler. Den siste tilnærming gir også en mulighet for å merke med radioaktive nuklider (for eksempel 111ln,<9>0Y,1<66>Ho,68Ga,<99m>Tc) med henblikk på billedfremstilling og/eller for strålebehandlings-studier. Strukturene til paclitaxel, polyetylenglykol-paclitaxel (PEG-paclitaxel), polyglutaminsyre-paclitaxelkonjugat (PG-paclitaxel) og dietylentriamin-pentaeddik-syre-paclitaxel (DTPA-paclitaxel) blir vist i FIG. 1.
De farmasøytiske preparater av paclitaxel eller docetaxel som er passende for injiserbar bruk, inkluderer sterile vandige løsninger eller dispersjoner og sterile pulvere som kan brukes for preparering av sterile injiserbare løsninger eller dispersjoner. I alle tilfeller må formen være steril og må være i form av væske til injeksjon. Den må være stabil under betingelsene for produksjon og lagring, og må bli bevart mot forurensende effekter av mikroorganismer, slik som bakterier og sopp. Bæreren kan være et løsemiddel eller et dispergeringsmedium som inneholder for eksempel vann, etanol, polyol (foreksempel glycerol, propylenglykol, og flytende polyetylenglykol, og lignende), eller passende blandinger av disse, og vegetabilske oljer. Beskyttelse mot mikroorganisme-effekter kan fremkalles ved hjelp av ulike antibakterielle og anti-sopp midler, for eksempel parabener, klorobutanol, fenol, sorbinsyre, timerosal og lignende. I mange tilfeller vil det være fordelaktig å inkludere isotone midler, for eksempel sukker eller natriumklorid.
Sterile injiserbare løsninger blir preparert ved å inkorporere de aktive forbindelser i den ønskede mengde i det passende løsningsmiddel med ulike av de andre ingredienser som er opplistet ovenfor etter behov, etterfulgt av sterilfiltrering. Generelt blir dispersjoner preparert ved å inkorporere de ulike steriliserte aktive ingredienser inn i et sterilt bærermedium som inneholder det basale dispersjons-medium og de nødvendige andre ingredienser fra de som er beskrevet over. I tilfeller med sterile pulvere til preparering av sterile injiserbare løsninger, er de foretrukne preparasjonsmetoder vakuumtørking og frysetørkingsteknikker, som gir et pulver av den aktive ingrediens pluss enhver ytterligere ønsket ingrediens fra en tidligere sterilfiltrert løsning av denne.
Som brukt her inkluderer «farmasøytisk akseptabel bærer» alle typer løsningsmidler, dispergeringsmidler, dekkingsmidler, antibakterielle og antisoppmidler og isotone midler og lignende. Bruk av slike medier og midler i farmasøytisk aktive substanser er vel kjent i faget. Dersom ikke enhver type konvensjonelle medier eller midler er inkompatible med den aktive ingrediens, blir deres bruk vurdert i de terapeutiske blandinger. Ytterligere aktive ingredienser kan også inkorporeres inn i blandingene.
Frasen «farmasøytisk akseptabel» henviser også til molekylære enheter og blandinger som ikke gir en allergisk eller lignende uønsket reaksjon når de blir tilført et dyr eller et menneske.
Løsningen bør være passende buffret dersom det er nødvendig, og det flytende fortynningsmediet bør først bli gjort isotont med tilstrekkelig saltvann eller glukose for parenteral tilførsel i for eksempel en vandig løsning. Disse spesielle vandige løsninger er spesielt passende for intravenøs og intraperitoneal tilførsel. I denne sammenheng vil sterile vandige løsninger som kan benyttes, være kjent til de med øvelse i faget, i lys av de foreliggende opplysninger.
De følgende eksempler blir inkludert for å vise foretrukne utførelser av oppfinnelsen. Det bør forstås av de med øvelse i faget at teknikkene beskrevet i eksemplene som følger, representerer teknikker som oppfinneren har oppdaget funksjonerer bra i utførelsen av oppfinnelsen, og således kan vurderes å utgjøre foretrukne måter for å praktisere denne. Polyglutaminsyre-paclitaxel
Det foreliggende eksempel demonstrerer konjugering av paclitaxel til en vannløselig polymer, poly(l-glutaminsyre) (PG). Potensialet av vannløselige polymerer som legemiddelbærere er vel etablert (Kopecek, 1990; Maeda and Matsumura, 1989). I tillegg til dets evne til å løse opp legemidler som ellers er uløselige, fungerer legemiddel-polymerkonjugatet også som et langsomt-frisettende depot med henblikk på kontrollert frisetting av legemiddel.
Syntese av PG-paclitaxel
PG ble valgt som bærer for paclitaxel siden det lett kan nedbrytes av lysosomale enzymer, det er stabilt i plasma og inneholder tilstrekkelig med funksjonelle grupper som kan brukes til å binde legemiddel. Flere antitumorlege-midler, deriblant adriamycin (Van Heeswijk et al., 1985; Hoes et al., 1985), cyklo-fosfamid (Hirano et al., 1979) og Ara-C (Kato et al., 1984) har blitt konjugert til PG.
PG-natriumsalt (MW 34 K, Sigma, 0,35 g) ble løst opp i vann. pH til den vandige løsning ble justert til 2 ved hjelp av 0,2M HCI. Presipitatet ble samlet, dialysert mot destillert vann og frysetørket for å gi 0,29 g PG.
Til en løsning av PG (75 mg, gjentagende enhet FW 170, 0,44 mmol) i
tørr DMF (1,5 ml_) ble tilsatt 20 mg paclitaxel (0,023 mmol, molar ratio PG/ paclitaxel = 19), 15 mg dicykloheksylkarbodiimid (DCC) (0,073 mmol) og en spor-mengde av dimetylaminopyridin (DMAP). Reaksjonen ble tillatt å gå ved rom-temperatur i 4 timer. Tynnskiktskromatografi (TLC, kiselgel) viste komplett omdannelse av paclitaxel (Rf = 0,55) til polymerkonjugat (Rf = 0, mobil fase, CHCl3/MeOH = 10:1). Reaksjonsblandingen ble helt over i kloroform. Det resulterende bunnfall ble samlet og tørket i vakuum for å gi 65 mg polymer-legemiddelkonjugat. Ved å endre vekt-ratioene av paclitaxel i forhold til PG i startmaterialene kan polymere konjugater med ulike paclitaxel-konsentrasjoner bli syntetisert.
Natriumsaltet til PG-paclitaxelkonjugatet ble fremskaffet ved å løse opp produktet i 0,5M NaHC03. Den vandige løsningen av PG-paclitaxel ble dialysert mot destillert vann (MWCO 1000) for å fjerne lavmolekylære vektsforurensninger og overskudd NaHC03-salt. Frysetørking av dialysatet ga 88,6 mg hvitt pulver. Paclitaxel-innholdet i dette polymerkonjugat ble bestemt ved hjelp av UV (beskrevet under) til å være 21% (vekt/vekt). Utbytte (overføring til polymer-bundet paclitaxel, UV): 93%. PG-paclitaxel med høyere innhold av paclitaxel (opp til 35%) kan bli syntetisert ved hjelp av denne metode ved enkelt å øke ratio paclitaxel til PG som benyttes.
<1>H-NMR (GE modell GN 500 spektrometer, 500 MHz i D20): 5 = 7,75 til 7,36 ppm (aromatiske komponenter av paclitaxel), 5 = 6,38 ppm (C10-H), 5,97 ppm (C13-H), 5,63 og 4,78 ppm (C2'-H), 5,55-5,36 ppm (C3'-H og C2-H, m), 5,10 ppm (C5-H), 4,39 ppm (C7-H), 4,10 (C20-H), 1,97 ppm (OCOCH3); og 1,18-1,20 ppm (C-CH3) blir identifisert som alifatiske komponenter av paclitaxel. Andre resonanser i paclitaxel ble dekket over av resonanser fra PG. PG-resonanser ved 4,27 ppm (H-a), 2,21 ppm (H-y), og 2,04 ppm (H-p) er i overensstemmelse med spektrum av rent PG. Koblingen av polymerkonjugert paclitaxel er separert for dårlig til å kunne måles med tilstrekkelig nøyaktighet. Løseligheten i vann var
>20 mg paclitaxel/mL.
Karakterisering av PG-paclitaxel
Ultrafiolette spektra (UV) ble fremskaffet på et Beckman DU-640 spektro-fotometer (Fullerton, CA). Innhold av paclitaxel konjugert PG ble estimert ved UV, basert på en standard kurve som var laget med kjente konsentrasjoner av paclitaxel i metanol ( X = 228 nm), der man antok at polymerkonjugatet i vann og det frie lege-middel i metanol hadde de samme molare ekstinksjonskoeffisienter og at begge fulgte Lambert-Beer's lov. Som vist ved dets UV-spektrum har PG-paclitaxel karakteristisk paclitaxel-absorpsjon med X skift fra 228 til 230 nm. Konsentrasjon av paclitaxel i PG-paclitaxel ble estimert basert på standardkurver produsert med kjente konsentrasjoner av paclitaxel i metanol ved absorpsjon ved 228 nm, der man antar at polymerkonjugatet i vann ved 230 nm og det frie legemiddel i metanol ved 228 nm, har de samme molare ekstinksjoner og begge følger Lambert-Beer's lov.
Gelfiltreringskromatografistudier av PG-paclitaxel
Den relative molekylvekt av PG-paclitaxel blekarakterisert vedgelfiltrerings-kromatografi (GPC). GPC-systemet besto av to LDC modell III pumper koblet med en LDC gradient-danner, en PL gel GPC-søyle, og en Waters 990 fotodiode detektor. Elueringsmidlet (DMF) ble pumpet ved en hastighet på I, 0 mL/min. med UV deteksjon satt ved 270 nm. Konjugering av paclitaxel til PG resulterte i en økning av molekylvekten til PG-paclitaxel, som vist ved skift av retensjonstid fra 6,4 min. av PG til 5,0 min. ved PG-paclitaxelkonjugat, som analysert ved GPC. Den kalkulerte molekylvekt av PG-paclitaxel som inneholdt 15-25% paclitaxel (vekt/vekt) er i området på 45-55 kDa. Det urene produkt inneholdt en forurensning med liten molekylvekt (retensjonstid 8,0 til 10,0 min., og II, 3 min.) som kan effektivt fjernes ved å konvertere PG-paclitaxel til dets natriumsalt, etterfulgt av dialyse.
Hydrolytisk degradering av PG-paclitaxelkonjugat
PG-paclitaxel ble oppløst i fosfatbuffrede løsninger (PBS, 0,01 M) ved pH 6,0, pH 7,4 og pH 9,6, ved en ekvivalent paclitaxelkonsentrasjon på 0,4 mM. Løsningene ble inkubert ved 37°C med forsiktig omristing. Ved utvalgte tidsintervaller ble deler (100 uL) fjernet, blandet med en lik mengde metanol og analysert ved hjelp av høytrykks væskekromatografi (HPLC). HPLC-systemet besto av en reversert fase kiselgelsøyle (Nova-Pak, Waters, CA), en mobil fase av metanol-vann (2:1, volum/volum) levert med en strømningshastighet på
1,0 mL/min., og en fotodiode detektor. Konsentrasjonen av PG bundet paclitaxel, fritt paclitaxel og andre degraderings-produkter i hver prøve, ble kalkulert ved å sammenligne topp-områdene med en separat fremskaffet standardkurve, preparert fra paclitaxel, idet det antas at de molare ekstinksjonskoeffisienter i hver topp ved 228 nm er de samme som den til paclitaxel. Halveringstiden til konjugatet, estimert til å være 132, 40 og 4 dager ved henholdsvis pH 6,0, 7,4 og 9,6, ble bestemt ved hjelp av lineær minstekvadrat regresjonsanalyse. HPLC-analyse viste at inkubering av PG-paclitaxel i PBS-løsninger ga paclitaxel og flere andre komponenter, deriblant en som er mere hydrofob enn paclitaxel (metabolitt-1). Mengde metabolitt-1, som sannsynligvis representerte
7-epipaclitaxel, som ble gjenvunnet i PBS ved pH 7,4, overgikk faktisk mengde paclitaxel etter 100 timer inkubering (FIG. 6).
In vitro studier
Prøver tatt fra PBS-løsning ved pH 7,4 ble analysert ved hjelp av en tubulin-polymeriseringsanalyse. Tubulin-polymeriseringsreaksjonen ble utført ved 32°C i PEM-buffer (pH 6,9), med tubulin (fra bovin hjerne, Cytoskelleton, Inc., Boulder, CO) i en konsentrasjon på 1 mg/mL (10 u.M) i nærvær av prøver (1,0 u.M ekvivalent paclitaxel) og 1,0 mM GTP. Tubulin-polymerisering ble fulgt ved å måle absorbans av løsningen ved 340 nm som funksjon av tid. Etter 15 minutter ble kalsiumklorid (125 mM) tilsatt for å måle kalsiumklorid-indusert depolymerisering av mikrotubuler. Mens PG-paclitaxel ferskt oppløst i PBS, ikke hadde aktivitet i å produsere mikro-tubuler, hadde prøver av PG-paclitaxel som var inkubert i 3 dager, evne til å gi tubulin-polymerisering. De mikrotubuler som var dannet var stabile mot kalsiumklorid-indusert depolymerisering.
Effekten av PG-paclitaxel på cellevekst ble også undersøkt ved hjelp av tetrazoliumsalt- (MTT) analyse (Mosmann, 1983). MCF-7-celler eller 13762F-celler ble sådd ut i mengde på 2 x 10<4>celler/mL i en 96 brønns mikrotiterplate som ble behandlet 24 timer senere med ulike konsentrasjoner av PG-paclitaxel, paclitaxel eller PG, og inkubert i ytterligere 72 timer. MTT-løsning (20 uL, 5 mg/mL) ble deretter tilsatt til hver brønn og inkubert i 4 timer. Supernatanten ble fjernet, og det MTT formazan som var dannet av metabolske levedyktige celler, ble målt ved hjelp av en mikroplate fluorescensleser ved en bølgelengde på 590 nm. I løpet av en 3 dagers periode hemmet PG-paclitaxel tumorcelleproliferasjon like mye som fritt paclitaxel gjorde. De resulterende ICso-verdierfor den humane brystkreftcellelinje MCF-7 var for paclitaxel 0,59 u.M og 0,82 u.M for PG-paclitaxel (målt i paclitaxel ekvivalente enheter). I en 3762F-cellelinje var sensitivitet for PG-paclitaxel (IC50= 1,86 u.M) sammenlignbar med den overfor paclitaxel (IC50= 6,79 u.M). IC50av PG alene var større enn 100 uJvl i begge cellelinjer.
In vivo anti-tumoraktivitet
Alt arbeid med forsøksdyr ble utført ved dyrestallen ved M.D. Anderson Cancer Center i overensstemmelse med institusjonens retningslinjer. C3H/Kam- mus ble avlet og oppstallet i en patogenfri dyrestall i avdelingen for eksperimentell strålingsonkologi. Enkelt-tumorer ble produsert i høyre lårmuskel hos C3H/Kam-hunnmus (25-30 g) ved å sprøyte inn 5 x 10<5>mus-eggstokk-kreftceller (OCa-l), brystkreft (MCa-4), leverkreft (HCa-l) eller fibrøst sarkom (FSa-ll). I en parallell studie fikk Fischer 344 hunnrotter (125-150 g) injisert 1,0 x 105 levedyktige 13762F tumorceller i 0,1 ml_ PBS. Behandlingene ble påbegynt når tumorene hos mus hadde vokst til 500 mm<3>(10 mm i diameter), eller når tumorene hos rotter hadde vokst til 2400 mm<3>(gjennomsnittsdiameter på 17 mm). En enkel dose av PG-paclitaxel i saltvann eller paclitaxel i Cremophor EL bæremiddel ble gitt i doser som varierte fra 40 til 160 mg ekvivalent paclitaxel/kg kroppsvekt. I kontrolleksperimenter ble saltvann, Cremophor bæremiddel [50/50 Cremophor/etanol fortynnet med saltvann (1:4)], PG- (MW 38K) løsning i saltvann, og en paclitaxel/PG-blanding benyttet. Tumorvekst ble bestemt hverdag (FIG. 7A, 7B, 7C, 7D og 7E) ved å måle tre ortogonale tumordiametere. Tumorvolum ble kalkulert ifølge formel A x B x C)/2. Absolutt vekstforsinkelse (AGD) hos mus blir definert som tiden målt i dager som tumorer behandlet med ulike legemidler bruker for å vokse fra 500 til 2000 mm<3>hos mus minus tiden målt i dager for tumorer behandlet med saltvannskontroll trenger for å vokse fra 500 til 2000 mm<3>. Tabell 1 sammenfatter den akutte toksisitet av PG-paclitaxel hos rotter sammenlignet med paclitaxel/Cremophor. Tabell 2 sammenfatter data vedrørende effekten av PG-paclitaxel mot MCa-4, FSa-ll og HCa-l tumorer hos mus. Disse data blir også sammenfattet i FIG. 7A-FIG. 7E.

Claims (12)
1. Anvendelse av en blanding omfattende (1) et anti-tumor legemiddel konjugert til en vannløselig polymer valgt fra gruppen bestående av poly (l-glutaminsyre), poly (d-glutaminsyre), poly (dl-glutaminsyre), poly (I-asparginsyre), poly (d-asparaginsyre) og poly (dl-asparginsyre) og hvor (a) nevnte anti-tumor legemiddel er paclitaxel eller docetaxel, og (b) nevnte polygutaminsyrer eller polyasparginsyrer har en molekylvekt på fra 5000 til 100 000 dalton og (2) et platinalegemiddel for fremstilling av et legemiddelpreparat for behandling av kreft i et individ.
2. Anvendelse i følge krav 1, hvor anti-tumor legemiddelet er paclitaxel.
3. Anvendelse i følge krav 1, hvor anti-tumor legemiddelet er docetaxel.
4. Anvendelse i følge krav 1, ytterligere omfattende polykaprolakton, polyglykolsyre, polymelkesyre, polyakrylsyre, poly (2-hydroksyetyl I-glutamin), karboksymetyldekstran, hyaluronsyre, human serum albumin, alginsyre eller kombinasjoner derav.
5. Anvendelse i følge krav 1, hvor nevnte polymer har en molekylvekt på fra omtrent 20 000 til omtrent 80 000 dalton.
6. Anvendelse i følge krav 1, hvor nevnte polymer har en molekylvekt på fra omtrent 30 000 til omtrent 60 000 dalton.
7. Anvendelse ifølge krav 1, hvor nevnte vannløselige polymer er konjugert til T- og/eller 7-hydroksyl av paclitaxel eller docetaxel.
8. Anvendelse i følge krav 1, hvor nevnte vannløselige polymer er poly (I-glutaminsyre).
9. Anvendelse i følge krav 1, hvor nevnte vannløselige polymer er poly (I-asparginsyre).
10. Anvendelse i følge krav 1, hvor nevnte kreft er brystkreft, eggstokk-kreft, malignt melanom, lungekreft, gastrisk kreft, kolonkreft, hode og halskreft eller leukemi.
11. Anvendelse i følge krav 1, hvor nevnte kreft er brystkreft.
12. Anvendelse i følge krav 1, hvor nevnte kreft er eggstokk-kreft.
