Description
図32および図33は、一実施形態に係る特徴および利点を有する緑内障ステント装置30fを示す。小柱ステント30fのこの実施形態は、逆刺付きのまたはねじ切りしたねじ状であり定着用逆刺128を有する延長部分またはピン126を含む。逆刺付きピン126は、ステント30fの遠位またはベース部分130から延びる。
図34は、一実施形態に係る特徴および利点を有する緑内障ステント装置30gを示す。ステント30gは、ヘッドまたはシート38gと、ベースまたは遠位端132を有するシャンクまたはメインボディ40gとを有する。小柱ステント30gのこの実施形態は、ステント30gの、ヘッド38gより下のメインボディ40gの上に(ねじ山136を有する)深いねじ134を含む。ねじは、ベース132の最後まで延ばすことができ、または延ばさなくてもよい。

図39に、一実施形態に係る特徴及び利点を有する緑内障ステント装置30mを示す。小柱ステント30mのこの実施形態は、水を吸収することで膨張する親水性材料から製作される。望ましくは、これによって、小柱網21内のより小さな切開から装置30mを挿入可能となる。続いて起こる、ステント30mの(小さめの矢印154により示した)膨張によって、小柱網21内の所定位置にステント30mがラッチ留めされ得ると有利である。上述のように、ステント30mの正確な方向付けは、適切な基準マーク、しるしまたはその種の他のものによって、かつ予め載置されたアプリケータ内におけるステントの位置決めによって保証される。

図43に、一実施形態に係る特徴及び利点を有する弁付きチューブ型ステント装置30rを示す。これは、前眼房20と血管の多い脈絡膜17との間における流れのためのチャネルを提供するステント30rの実施形態である。臨床上、脈絡膜17は、眼10に望ましい圧力よりも低い圧力になり得る。したがって、このステント30rは、脈絡膜17と前眼房10との間における所望の圧力差と等しい開放圧力を有する弁、あるいは所望の圧力低下を提供する狭窄部を含んでいる。
図44は、一実施形態に係る特徴及び利点を有する浸透膜装置30sを示す。この実施形態は、前眼房20と血管の多い脈絡膜17との間の流れのためのチャネルを提供する。浸透膜30sは、脈絡膜17の内皮層の一部分に代えて使用される。脈絡膜17には血管が多いので、水は、眼10の前眼房20内ほど脈絡膜側に集中しない。したがって、浸透勾配によって、前眼房20から脈絡膜17内へ水が流れる。
図45に、一実施形態に係る特徴及び利点を有するアブ エクステルノ式処置を用いた、ステント30tの埋め込みを示す。図45のアブ エクステルノ式処置では、眼10内へ穿刺を行うアプリケータまたは送出装置100cで、ステント30tをシュレム管21内へ挿入する。
図47に、一実施形態に係る特徴及び利点を有する、標的を定めた薬物送達埋没物30vを示す。この図面は、標的を定めた薬物送達の概念を描写するものである。緩やかな放出用の埋没物30vは、小柱網21内に埋め込まれる。
プログラムされた(「標的を定めた」としても知られる)ステント留置とは、最適により多く流出する形で利点を提供するためにシュレム管内の特定の1つまたは複数の位置に、1つまたは複数のステントを計画的に配置することをさす。例えば、眼の房水流の特性を評価することを含む方法を提供し得る。そのような特性には、例えば、コレクタチャネルの分布、コレクタチャネルの流れの特性、流出抵抗、流出容量、シュレム管の形状、サイズ、曲がり、および他の要素が含まれ得るが、それらに限定はしない。本方法はまた、さらに最適なステント留置を決定するステップと、1つまたは複数の位置に、また処置でステントを埋め込むステップとを含み得る。例えば、所望のステントが、薬剤を装入した、遺伝子を装入した、または表面を処理した、かつ/またはどんな関連の薬剤治療であるかにかかわらず、所望のステント留置の決定は、コレクタチャネルの数および位置、患者の微細解剖データ、使用するステントの数、使用するステントのタイプ、既に埋め込まれたステントの位置に関する死体解剖のデータベースの考察を含み得る。
図50Aは、眼10の一部分の拡大断面図であり、自然な状態の小柱網21、シュレム管22、およびコレクタ管23を解剖学的に示している。図50Bに、コレクタ管23内へ延びてコレクタ管23を膨張させるステント229Cを示す。
図51Aは、複数のステントを展開するよう構成されたステント送出アプリケータ201の斜視図である。送出アプリケータ201は、ステント管腔249を画定する注入シース246と、遠位ステント保持部259と、ハンドル205とを備える。
Claims (18)
- 眼の病気を治療するためのインプラントを送出する器具であって、当該器具は、
自己閉鎖される眼の切開部位から眼内へ導入される大きさである延伸本体と、
前記延伸本体内に配置され、かつ連続して配列される複数のインプラントと、を備え、
前記複数のインプラントの各々は、眼の流体流出経路に流体を輸送するように形成され、
前記複数のインプラントの少なくとも1つは、眼の前眼房から眼の前記流体流出経路に流体を輸送するように形成され、
さらに、前記延伸本体は、前記複数のインプラントの各々を眼の組織内に埋め込むために前記延伸本体から連続して供給するアクチュエータを含み、
前記器具は、
(1)前記器具を眼から取り外すことなく、すなわち、前記インプラントを複数埋め込みする間に眼内に挿入するための更なる切開部を形成することなく、前記延伸本体から前記複数のインプラントを連続して供給すること、
(2)前記インプラントを眼の組織内に埋め込むために、前記複数のインプラントの少なくとも1つの出口部が前記インプラントの各々の入り口部に先立つ状態で前記眼の前眼房から前記インプラントを前進させること、
上記(1)または(2)の少なくとも1つを行うために構成された器具。 - 前記延伸本体は、管を含む請求項1記載の器具。
- 前記複数のインプラントは、前記管内において端と端とを接して配置される請求項2に記載の器具。
- 前記延伸本体は、カッティング部を含む請求項1記載の器具。
- 前記延伸本体は、管を含み、前記カッティング部は、前記管の端部を含む請求項4に記載の器具。
- 前記延伸本体は、管を含み、前記器具は、さらに前記管内のトロカールを含む請求項1に記載の器具。
- 前記複数のインプラントは、各々が管腔を有し、前記トロカールは、前記管腔を貫通する請求項6に記載の器具。
- 前記アクチュエータは、プッシャー部材を含む請求項1〜7のいずれかに記載の器具。
- 前記アクチュエータは、ロッドまたは管を含む請求項1〜8のいずれかに記載の器具。
- 眼の前眼房内に留められるように形成される少なくとも1つのインプラントの入り口部は、眼の前眼房から眼の前記流体流出経路に流体を流出すべく眼の前眼房内に配置されるように形成される入口開口部、とを有する請求項1〜9のいずれかに記載の器具。
- 前記流体は、眼房水である請求項1〜10のいずれかに記載の器具。
- 眼の前記流体流出経路は、シュレム管である請求項1〜11のいずれかに記載の器具。
- 前記複数のインプラントの少なくとも1つは、治療薬を溶出するように形成される請求項1〜12のいずれかに記載の器具。
- 前記複数のインプラントの少なくとも1つは、カッティング部を含む請求項1〜13のいずれかに記載の器具。
- 前記治療薬は、前記複数のインプラントの少なくとも1つの一部分である請求項13に記載の器具。
- 前記複数のインプラントと前記延伸本体は、眼の流体流出経路に隣接する眼の組織に位置するように大きさと寸法が作られた請求項1〜15のいずれかに記載の器具。
- 前記眼の組織は、小柱網である請求項1〜16のいずれかに記載の器具。
- 少なくとも前記インプラントの1つの前記入口部は、それぞれの眼の前記流体流出経路のそれぞれの出口部と流体連結し、
前記出口部は、眼の前記流体流出経路に流体を輸送すべく前記眼の流体流出経路内に配置されるように形成される出口開口部、を有する請求項1〜17に記載の器具。
