Abstract
Description
本出願は、2008年4月28日出願の発明の名称が「Absorbing Fluids in a Surgical Access Device」である米国特許第12/110,724号、2008年4月28日出願の発明の名称が「Scraping Fluid Removal in a Surgical Access Device」(代理人参照番号100873−242(END6366USNP1))である米国特許第12/110,727号、2008年4月28日出願の発明の名称が「Wicking Fluid Management in a Surgical Access Device」(代理人参照番号100873−243(END6366USNP3))である米国特許第12/110,742号、及び2008年4月28日出願の発明の名称が「Fluid Removal in a Surgical Access Device」(代理人参照番号100873−241(END6366USNP3))である米国特許第12/110,755号の一部継続出願であり、それらのすべては、参照することにより、その全容を援用する。
本発明は、外科手術を行うための方法及び装置に関し、具体的には、外科手術中に可視性を維持するための方法及び装置に関する。
(1) 近位ハウジングと、遠位カニューレと、手術用器具を受容するように寸法決定され構成される、前記近位ハウジング及び前記遠位カニューレを通って延在する作業チャネルと、
手術用器具が前記作業チャネルを貫通して配置されていない場合に、前記作業チャネルを封止するように構成される、前記近位ハウジング内に配置されるシールであって、前記シールが、内部を貫通して延在し、前記作業チャネルを通過する手術用器具を受容するように位置付けられる開口部を有する、シールと、
前記シールの遠位に位置付けられる流体除去装置であって、前記流体除去装置が、内部を貫通して延在し、前記作業チャネルを通過した手術用器具を受容するように位置付けられる開口部を有し、器具が、前記流体除去装置内の前記開口部を閉塞する場合に、前記流体除去装置が、送気ガスを前記遠位カニューレから前記流体除去装置の近位側に通過させ、それによって、前記流体除去装置の近位側及び遠位側の圧力を均一にするように構成される、流体除去装置と、を備える、手術用アクセス装置。
(2) 前記流体除去装置が、前記近位ハウジング内に形成された送気ポートから流動する送気ガスの経路内にあるように構成されるように位置付けられる、実施態様1に記載の手術用アクセス装置。
(3) 前記流体除去装置が、その少なくとも一部分に形成される切り欠きを含み、前記切り欠きが、器具が前記開口部を閉塞する場合に、送気ガスを送気ポートから前記切り欠きを通して前記遠位カニューレ内に通過させるように構成される、実施態様1に記載の手術用アクセス装置。
(4) 前記流体除去装置が、前記開口部を通過する手術用器具から流体を擦り取るように構成されるスクレーパを含む、実施態様1に記載の手術用アクセス装置。
(5) 前記流体除去装置が、前記スクレーパによって除去された流体を収着するように位置付けられる収着材を含む、実施態様4に記載の手術用アクセス装置。
(6) 前記収着材が、前記送気ポートの遠位に位置付けられる、実施態様5に記載の手術用アクセス装置。
(7) 前記収着材が、実質的に円筒の部材の周囲に配置される、実施態様5に記載の手術用アクセス装置。
(8) 前記送気ポートが、内部を貫通して延在し、長手方向軸を画定する管腔を有し、前記スクレーパが、前記長手方向軸の遠位に位置付けられる、実施態様4に記載の手術用アクセス装置。
(9) 前記流体除去装置の少なくとも一部分が、前記送気ポートの遠位に位置付けられる、実施態様1に記載の手術用アクセス装置。

(19) 内部を貫通して手術用器具を受容するように構成される開口部を有する、シールと、
前記シールに関連し、前記開口部、及び前記開口部を通過する手術用器具のうちの少なくとも1つから流体を収着するように構成される収着材と、を備える、手術用アクセス装置に使用するためのアセンブリ。
(20) 前記収着材が、吸着性要素である、実施態様19に記載のアセンブリ。
(22) 前記シールに隣接して位置付けられ、前記シール内の前記開口部を通って延在する手術用器具から流体を擦り取るように構成される、スクレーパ要素を更に備える、実施態様19に記載のアセンブリ。
(23) 前記スクレーパ要素の少なくとも一部分が、親水性である、実施態様22に記載のアセンブリ。
(24) 前記スクレーパに関連し、手術用器具が前記開口部を通過している場合に、前記開口部の近傍で収集された流体を吸い上げるように構成される、ウィッキング要素を更に備える、実施態様22に記載のアセンブリ。
(25) 前記シールに関連し、手術用器具が前記開口部を通過している場合に、前記開口部の近傍で収集された流体を吸い上げるように構成される、ウィッキング要素を更に備える、実施態様19に記載のアセンブリ。
(26) 前記シールが、前記開口部を通過する手術用器具から流体を擦り取るように構成され、前記収着材が、前記シールによって前記手術用器具から擦り取られた流体を収着するように構成される、実施態様19に記載のアセンブリ。
(27) 前記収着材が、前記シールと一体に形成される、実施態様19に記載のアセンブリ。
(28) 前記シールが、器具が内部を貫通して挿入されている場合に、封止を形成するように構成される器具シールを備える、実施態様19に記載のアセンブリ。
(29) 前記シールが、第1のシールを備え、前記シールアセンブリが、第2のシールを更に備える、実施態様19に記載のアセンブリ。
(30) 前記収着材が、前記第1のシール及び前記第2のシールの遠位に位置付けられる、実施態様29に記載のアセンブリ。
(32) 前記内部側壁が、その中に形成され、前記中央開口部の長手方向軸に平行に延在する、複数の溝を含む、実施態様31に記載の流体除去装置。
(33) 前記複数の繊維が、前記中央開口部の長手方向軸に対して、長手方向に配向される、実施態様31に記載の流体除去装置。
(34) 前記収着部材の前記多角形の断面形状が、四角形を含む、実施態様31に記載の流体除去装置。
(35) 前記収着部材の前記多角形の断面形状が、三角形を含む、実施態様31に記載の流体除去装置。
(36) 前記収着部材の少なくとも一部分が、親水性である、実施態様31に記載の流体除去装置。
(37) シール開口部から流体を除去するための方法であって、
手術用器具に、アクセス装置内のシール内の開口部を通過させることであって、前記器具上の流体が、前記アクセス装置内の収着要素によって収着される、ことを含む、方法。
(38) 前記収着要素が、前記器具によって前記シール上に被着された流体を収着する、実施態様37に記載の方法。
(39) 前記収着材が、流体を吸着する、実施態様37に記載の方法。
(40) 前記収着材が、流体を吸収する、実施態様37に記載の方法。
(42) 前記器具上の流体が、スクレーパによって前記器具から擦り取られ、前記収着要素が、前記スクレーパから流体を収着する、実施態様37に記載の方法。
(43) 医療器具から流体を除去するための装置であって、
スクレーパを通って延在する中央開口部を有する、実質的に平面構造を有するスクレーパであって、前記中央開口部が、拡張して、そこを通って挿入された手術用器具に係合し、手術用器具から流体を擦り取るように構成され、前記スクレーパが、中に形成された複数のチャネルを有する遠位表面を有し、前記チャネルが、前記中央開口部を通って挿入された器具から擦り取られた流体が、前記チャネルを通って流動することができるように、前記中央開口部から半径方向外向きに延在する、スクレーパを備える、装置。
(44) 医療器具から流体を除去するための装置であって、
冠部と、
前記冠部に連結された蓋と、
前記冠部と前記蓋との間で捕捉されたスクレーパであって、そこを通って形成された中央開口部を有し、拡張して、そこを通って挿入された器具に係合し、前記器具から流体を擦り取るように構成される、スクレーパと、を備える、装置。
(45) 複数のピンが、前記冠部と前記蓋との間に延在して、前記冠部と前記蓋を噛合し、前記複数のピンが、前記スクレーパ内に形成された複数の開口部を通って延在する、実施態様44に記載の装置。
Claims (9)
- 手術用器具を受容するように寸法決定され、構成された作業チャネルを画定する、ハウジングと、
前記ハウジング内に配置されるシールであって、前記作業チャネルを通過した手術用器具を受容するように位置付けられた開口部を有する、シールと、
前記ハウジング内に配置され、流体を収着して、流体が前記作業チャネルを通過した手術用器具上に再被着されるのを防止するように構成される、収着材と、
を備える、手術用アクセス装置。 - 前記収着材が、前記シール内の前記開口部から流体を収着するように位置付けられる、請求項1に記載の手術用アクセス装置。
- 前記収着材が、吸収剤を含む、請求項1に記載の手術用アクセス装置。
- 前記収着材が、吸着剤を含む、請求項1に記載の手術用アクセス装置。
- 前記ハウジングが、前記シールを含有する近位部分と、前記近位部分から遠位に延在し、体腔内に挿入されるように構成される遠位カニューレと、を含む、請求項1に記載の手術用アクセス装置。
- 前記シールが、第1のシールを備え、前記アクセス装置が、前記ハウジング内に配置される第2のシールを更に備え、前記収着材が、前記第1及び前記第2のシールの遠位に位置付けられる、請求項5に記載の手術用アクセス装置。
- 前記ハウジング内に配置され、作業チャネルを通過する手術用器具から流体を擦り取るように構成されるスクレーパを更に備える、請求項1に記載の手術用アクセス装置。
- 前記収着材が、前記スクレーパによって擦り取られた流体を収着するように構成される、請求項7に記載の手術用アクセス装置。
- 前記ハウジング内に配置され、前記作業チャネルを通過する手術用器具から流体を吸い上げるように構成されるウィッキング要素を更に備える、請求項1に記載の手術用アクセス装置。
