Abstract

(57)【要約】 鉗子は、各々が前縁部(12)および基部壁(14)を備えた２個のシェル半体(10a、10b)であって第1の開き形状および第2の閉じ形状をとり得る如き様式にて支持要素(16)上に取付けられた２個のシェル半体(10a、10b)を備える。該鉗子は複数の生検サンプルを受容する内部キャビティ(20)を有する少なくともひとつの容器要素(22)を備える。容器要素(22)は好適にはシェル半体(10a、10b)の一方(10b)と結合される。

Description

【発明の詳細な説明】
【０００１】 本発明は医療用、特に、内視鏡生検、腹腔鏡検査、婦人科用途などの生検(bio
psy)用の鉗子(forceps)に関する。
【０００２】 より詳細には、習用のこの形式の鉗子は各々が縁部および基部壁を備えた２個
のシェル半体であって第1の開き形状および第2の閉じ形状をとり得る如き様式に
て支持要素上に取付けられた２個のシェル半体を備える。
【０００３】 たとえば消化器系の内視鏡検査などにおける使用に際し、鉗子は上記閉じ形状
とされて内視鏡の操作ダクトに通され、生検されるべき（たとえば食道、胃、十
二指腸または結腸などの)器官の内部粘膜と接触せしめられる。
【０００４】 上記鉗子が正しく位置決めされたなら各シェル半体は、粘膜のサンプルを切断
すべく開かれてから閉じられ、該シェル半体間には上記粘膜が保持される。利用
可能なスペースは本来的に限られていることから、閉じられた上記各シェル半体
が保持し得るのはせいぜい２〜３個の生検サンプルである。
【０００５】 しかし、胃潰瘍における如く、または、結腸の深刻な慢性疾患である潰瘍性全
大腸炎(ulcerative pancolitis)の場合の形成異常もしくは新形成における如く
、多数のサンプルが採取されるべきである病理学的状態が生ずることは多い。
【０００６】 これらの場合、各シェル半体の内側のスペースが生検サンプルで満たされる度
に鉗子は内視鏡ダクトを介して引抜かれ、空にされ、操作対象部位へと再導入さ
れねばならない。故に、上記ダクトの大量の摩耗および変形が生じ、破断による
体液(liquid)の侵入のリスクがあるが、これは内視鏡に対する極めて深刻な損傷
を引き起こし得る。
【０００７】 複数回に亙る鉗子の導入および引抜きを実施する必要性に伴う別の問題は、特
にシェル半体を空にするのに必要な手順の時間が長いことである。採取されるべ
きサンプルのサイズは小さいことから、この操作を行う上で操作者は相当の時間
を要する。しかし実際の操作においては、たとえば鎮静薬を服用し得ない非常に
不耐性(intolerant)の患者などにより一定の条件において利用可能な時間は極め
て限られることから、習用の鉗子を用いることは困難である。
【０００８】 上述の先行技術の問題を回避すべく、本発明の主題は、本明細書の冒頭に記述
された形式の鉗子であって、複数の生検サンプルを受容するキャビティを有する
少なくとも一個の容器要素を備えることを特徴とする鉗子である。
【０００９】 故に本発明の鉗子は、一回の導入および引抜きを実施するだけで所定の病理学
的状態に対して必要なサンプルの全てを連続的に採取すべく使用され得ることか
ら、操作に必要な時間を相当に短縮すると共に内視鏡ダクトの摩耗を相当に減少
する。
【００１０】 更に、引抜き段階の間において上記容器要素はサンプルを上記ダクトから隔離
して保持することから、該ダクト内に存在し得る不純物または他の採取サンプル
の残存断片による一切の混合が防止されるという利点が在る。
【００１１】 本発明の更なる利点および特徴は、添付図面を参照して非限定的な実施例とし
て提供される以下の詳細な説明から明らかとなろう。
【００１２】 たとえば内視鏡生検用の生検用鉗子は、各々が前縁部12および基部壁14を有す
る２個のシェル半体10a、10bを備える(図１乃至図３)。シェル半体10a、10bは、
(各図中に示された)第1の開き形状と、該２個のシェル半体10a、10bの各前縁部1
2が相互に接触するという第2の閉じ形状とをとり得る様に、たとえば支持要素16
上に習用の様式で取付けられる。
【００１３】 シェル半体10bはその基部壁14において孔18を有するが、該孔18は、自身に結
合されたビーカー形状の容器要素22の内側のキャビティ20であってその壁部に複
数の貫通開口24を有するキャビティ20と連通する。
【００１４】 本発明の範囲において容器要素22を構成する材料は重要で無く、医療分野で使
用されるに適した材料から特別な制限なしで選択され得る。
【００１５】 一方、シェル半体10aは自身高さの約１／３まで充填材料25により充填される
。
【００１６】 使用に際して上記鉗子は先ずシェル半体10a、10bを閉じ形状として、生検され
るべき器官の内部粘膜に近い所望位置に該鉗子が来るまで(各図には示されてい
ない)内視鏡ダクトを通過せしめられる。次にシェル半体10a、10bは対応個数の
生検サンプルを切断すべく連続的に開閉される。
【００１７】 これらのサンプルは１個ずつ採取され、シェル半体10aにより及ぼされる圧力
により容器要素22の基部の方へ付勢される。
【００１８】 各開口24によれば、容器要素22のキャビティ20内に捕捉された空気および一切
の器官体液が排出され得ることから、容器要素22の内側における生検サンプルの
移動が更に容易となる。
【００１９】 各サンプルの切断の完了時にシェル半体10a、10bは再び閉じられ、上記鉗子は
その容器要素22と共に内視鏡ダクトを通して引抜かれ得る。故に、キャビティ20
の内側に封入された各生検サンプルは、組織学的汚染の潜在的原因を構成し得る
上記ダクトとの実質的な接触は一切行わない。
【００２０】 図４乃至図７は本発明に係る生検用鉗子の各シェル半体の代替実施例を示すが
、これらのシェル半体は図１乃至図３に関して記述されたのと同様の手法で支持
要素上に取付けられ得る。
【００２１】 この実施例において、容器要素20が結合されないシェル半体10aは、実質的に
前縁部12のレベルまで隆起された中央部分26と、環状で凹状とされた周縁部28と
を備えた基部壁14を有する(図４および図６)。
【００２２】 一方、シェル半体10b(図５および図７)はその基部壁14において孔18を有する
が、該孔18に対しては、ビーカー形状の容器要素22の端部であって開放されると
共に絞られたネックを有する端部が環状接着剤層30により固定される。故に容器
要素22の内部キャビティ20は、シェル半体10bの内側のスペースの連続部を構成
する。
【００２３】 容器要素22はまた、その基部壁および側壁にも開口24を有する。
【００２４】 直上に記述された各シェル半体を有する上記鉗子を操作する原理は、図１乃至
図３に関して記述されたのと実質的に同様である。シェル半体10aを特定形状と
したことから、各生検サンプルには更に大きな圧力が付与されて容器要素22の内
部キャビティ20内へと付勢され得る。
【００２５】 図８および図９はシェル半体10bに対して容器要素22を固定する変更例を示し
ているが、これは上述の接着剤層の使用の代替例である。この場合に要素22の開
放端部は、シェル半体10bの孔18の相補形状縁部に係合するためのフック縁部を
有している。この係合は好適には、スプリング・リング32などの機械的要素によ
り固定される。
【００２６】 本発明の範囲内に収まる原理、構成の詳細ならびに実施例の形態は、例示的な
ものとしてのみ記述されたそれらに関し本発明の有効範囲から逸脱することなく
広範囲に変更され得る。特に上記シェル半体は両者ともに、自身の容器要素を有
し、且つ／又は、相互から独立して、たとえば歯付きの、もしくは鋸歯状の縁部
などの任意の形状を有し得る。また上記容器要素は、実質的に任意の形状および
／または長さを有し、且つ／又は、結合されたシェル半体の縁部に直接的に係合
されたネットの形態とされ得る。更に、容器要素は等しく良好に夫々のシェル半
体に対して固着的にまたは解除可能に固定され得る。

Claims (10)

    【特許請求の範囲】
  1. 【請求項１】 各々が前縁部(12)および基部壁(14)を備えた２個のシェル半
    体(10a、10b)であって第1の開き形状および第2の閉じ形状をとり得る如き様式に
    て支持要素(16)上に取付けられた２個のシェル半体(10a、10b)を備えた医療用の
    特に生検用の鉗子において、 該鉗子は複数の生検サンプルを受容するキャビティ(20)を有する少なくともひ
    とつの容器要素(22)を備えることを特徴とする、鉗子。
  2. 【請求項２】 前記容器要素(22)は前記シェル半体(10a、10b)の少なくとも
    一方(10b)に結合されることを特徴とする、請求項１記載の鉗子。
  3. 【請求項３】 前記容器要素(22)が結合されないシェル半体(10a)は、実質
    的に前記前縁部(12)のレベルまで隆起された中央部分(26)と、環状で凹状とされ
    た周縁部(28)とを有する基部壁(14)を備えることを特徴とする、請求項１または
    ２のいずれか一項に記載の鉗子。
  4. 【請求項４】 前記シェル半体(10a、10b)の少なくとも一方(10b)はその基
    部壁(14)にて、該シェル半体(10b)に結合された容器要素(22)のキャビティ(20)
    と連通する孔(18)を有することを特徴とする、請求項１から３のいずれか一項に
    記載の鉗子。
  5. 【請求項５】 ビーカー形状の容器要素(22)の開放端部は、該容器要素(22)
    の内部キャビティ(20)が前記シェル半体(10b)の内側のスペースの連続部を構成
    する如き様式で前記孔(18)に固定されることを特徴とする、請求項４記載の鉗子
    。
  6. 【請求項６】 前記開放端部は環状接着剤層(30)により前記シェル半体(10b
    )に固定されることを特徴とする、請求項５記載の鉗子。
  7. 【請求項７】 前記開放端部は前記シェル半体(10b)の前記孔(18)の相補形
    状縁部に係合するフック縁部を有することを特徴とする、請求項５記載の鉗子。
  8. 【請求項８】 前記シェル半体(10b)の前記孔(18)の前記縁部に対する前記
    容器要素(22)の前記開放端部の前記縁部の前記係合はスプリング・リング(32)な
    どの機械的要素により固定されることを特徴とする、請求項７記載の鉗子。
  9. 【請求項９】 前記容器要素(22)はビーカー形状であると共にその基部また
    は側壁に、もしくはその基部および側壁に少なくとも一個の開口(24)を有するこ
    とを特徴とする、請求項１から８のいずれか一項に記載の鉗子。
