Abstract
Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem koji sadrži formulaciju matrice s produljenim otpuštanjem, koji je stvrdnut na temperaturi od najmanje oko 60 °C tijekom vremenskog perioda od najmanje oko 1 minute, te formulacija matrice s produljenim otpuštanjem sadrži pripravak koji je naznačen time da sadrži barem: najmanje jedan polietilen oksid koji ima, prema reološkim mjerenjima, molekularnu težinu od najmanje 1,000,000; tenajmanje jedno aktivno sredstvo odabrano od opioidnih analgetika; tepri čemu pripravak sadrži barem oko 80 % (težinski) polietilen oksida. Patent sadrži još 40 patentnih zahtjeva.
Claims (41)
1. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem koji sadrži formulaciju matrice s produljenim otpuštanjem, koji je stvrdnut na temperaturi od najmanje oko 60 °C tijekom vremenskog perioda od najmanje oko 1 minute, te formulacija matrice s produljenim otpuštanjem sadrži pripravak koji je naznačen time da sadrži barem:
(1) najmanje jedan polietilen oksid koji ima, prema reološkim mjerenjima, molekularnu težinu od najmanje 1,000,000; te
(2) najmanje jedno aktivno sredstvo odabrano od opioidnih analgetika; te
pri čemu pripravak sadrži barem oko 80 % (težinski) polietilen oksida.
2. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 1, naznačen time da opioidni analgetik je oksikodon hidroklorid ili hidromorfon hidroklorid i pripravak sadrži više od 5% (težinski) oksikodon hidroklorida ili hidromorfon hidroklorida.
3. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 1, naznačen time da pripravak sadrži najmanje oko 80 % (težinski) polietilen oksida koji ima, prema reološkim mjerenjima, približnu molekularnu težinu od najmanje 1,000,000.
4. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 1, naznačen time da pripravak sadrži 10 mg oksikodon hidroklorida, i najmanje oko 85 % (težinski) polietilen oksida.
5. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 1, naznačen time da pripravak sadrži 15 mg ili 20 mg oksikodon hidroklorida.
6. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem koji sadrži formulaciju matrice sa produljenim otpuštanjem koji je stvrdnut na temperaturi od najmanje oko 60 °C tijekom vremenskog perioda od najmanje oko 1 minute, te formulacija matrice s produljenim otpuštanjem sadrži pripravak koji je naznačen time da sadrži barem:
(1) najmanje jedan polietilen oksid koji ima, prema reološkim mjerenjima, približnu molekularnu težinu od najmanje 1,000,000; te
(2) 40 mg oksikodon hidroklorida; i
pri čemu pripravak sadrži barem oko 65 % (težinski) polietilen oksida.
7. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem koji sadrži formulaciju matrice sa produljenim otpuštanjem koji je stvrdnut na temperaturi od najmanje oko 60 °C tijekom vremenskog perioda od najmanje oko 1 minute, te formulacija matrice s produljenim otpuštanjem sadrži pripravak koji je naznačen time da sadrži barem:
(1) najmanje jedan polietilen oksid koji ima, prema reološkim mjerenjima, približnu molekularnu težinu od najmanje 1,000,000; i
(2) 60 mg ili 80 mg oksikodon hidroklorida; i
pri čemu pripravak sadrži barem oko 60 % (težinski) polietilen oksida.
8. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 1, naznačen time da pripravak sadrži 8 mg hidromorfon hidroklorida, te najmanje oko 94 % (težinski) polietilen oksida.
9. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 1, naznačen time da pripravak sadrži 12 mg hidromorfon hidroklorida, te najmanje oko 92 % (težinski) polietilen oksida.
10. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 1, naznačen time da pripravak sadrži 32 mg hidromorfon hidroklorida, te najmanje oko 90 % (težinski) polietilen oksida.
11. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem koji sadrži formulaciju matrice sa produljenim otpuštanjem koji je stvrdnut na temperaturi od najmanje oko 60 °C tijekom vremenskog perioda od najmanje oko 1 minute, te formulacija matrice s produljenim otpuštanjem sadrži pripravak koji je naznačen time da sadrži barem:
(1) najmanje jedno aktivno sredstvo koje je odabrano od opioidnih analgetika;
(2) najmanje jedan polietilen oksid koji ima, prema reološkim mjerenjima, približnu molekularnu težinu od najmanje 1,000,000; te
(3) najmanje jedan polietilen oksid koji ima, prema reološkim mjerenjima, približnu molekularnu težinu od manje od 1,000,000.
12. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 11, naznačen time da pripravak sadrži najmanje oko 80 % (težinski) polietilen oksida.
13. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 11, naznačen time da pripravak sadrži najmanje:
1. 15 do 30 % (težinski) polietilen oksida koji ima, prema reološkim mjerenjima, molekularnu težinu od najmanje 1,000,000; i 65 do 80 % (težinski) polietilen oksida koji ima, prema reološkim mjerenjima, molekularnu težinu od manje od 1,000,000; ili
2. najmanje oko 20% (težinski) ili najmanje oko 30 % (težinski) ili najmanje oko 50 % (težinski) polietilen oksida koji ima, prema reološkim mjerenjima, molekularnu težinu od najmanje 1,000,000.
14. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 13, naznačen time da je gustoća formulacije matrice sa produljenim otpuštanjem jednaka ili manja od oko 1.20 g/cm3, poželjno jednaka ili manja od oko 1.19 g/cm3.
15. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 14,
pri čemu formulacija matrice sa produljenim otpuštanjem nakon što je pohranjena na 25 °C i 60 % relativne vlažnosti (RH) tijekom najmanje 1 mjeseca daje brzinu otapanja, koja kada je mjerena na USP aparatu 1 (košara) kod 100 okr/min. u 900 ml simulirane želučane tekućine bez enzima (SGF) na 37° C, naznačen time da postotak količine oslobođenog aktivnog sredstva kod 1,4 i 12 sati otapanja nema odstupanje veće od oko 15 % točaka u odnosu na odgovarajuću in-vitro brzinu otapanja referentne formulacije prije skladištenja.
16. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 15, naznačen time da je formulacija matrice sa produljenim otpuštanjem bila pohranjena na 40 °C i 75 % relativne vlažnosti (RH).
17. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 16, naznačen time da formulacija matrice sa produljenim otpuštanjem nakon što je pohranjena na 25 °C i 60 % relativne vlažnosti (RH) tijekom najmanje 1 mjeseca sadrži količinu od najmanje jednog aktivnog sredstva u % (težinski) vezano uz deklaracijski zahtjev aktivnog sredstva za formulaciju matrice s produljenim otpuštanjem tako da odstupanje nije veće od oko 10 % točaka u odnosu na odgovarajuću količinu aktivnog sredstva u % (težinski) relativno prema deklaracijskom zahtjevu aktivnog sredstva za formulaciju matrice s produljenim otpuštanjem referentne formulacije prije pohranjivanja.
18. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema zahtjevu 17, naznačen time da je formulacija matrice sa produljenim otpuštanjem bila pohranjena na 40 °C i 75 % relativne vlažnosti (RH).
19. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 18, naznačen time da oblik doziranja daje brzinu otapanja, koja kada je mjerena na USP aparatu 1 (košara) kod 100 okr/min. u 900 ml simulirane želučane tekućine bez enzima (SGF) na 37° C, je između 12.5 i 55% (težinski) otpuštene aktivne supstance nakon 1 sata, između 25 i 65% (težinski) otpuštene aktivne supstance nakon 2 sata, između 45 i 85% (težinski) otpuštene aktivne supstance nakon 4 sata i između 55 i 95% (težinski) otpuštene aktivne supstance nakon 6 sati.

Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 19, naznačen time da je aktivno sredstvo oksikodon hidroklorid i pri čemu oblik doziranja kada je ispitivan u komparativnoj kliničkoj studiji je bioekvivalentan sa komercijalnim proizvodom OxyContin™.
21. Oralni farmaceutski oblik doziranja u krutom stanju sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 20, naznačen time da je aktivno sredstvo oksikodon hidroklorid i pri čemu oblik doziranja sadrži 10 mg oksikodon hidroklorida kada je ispitivan u komparativnoj kliničkoj studiji je bioekvivalentan sa referentnom tabletom koja sadrži 10 mg oksikodon hidroklorida u matričnoj formulaciji koja sadrži:
a) Oksikodon hidroklorid: 10.0 mg/tableta
b) Laktozu (osušena raspršivanjem): 69.25 mg/tableta
c) Povidon : 5.0 mg/tableta
d) Eudragit® RS 30D (krutina) : 10.0 mg/tableta
e) Triacetin®: 2.0 mg/tableta
f) Stearil alkohol: 25.0 mg/tableta
g) Talk: 2.5 mg/tableta
h) Magnezijev stearat: 1.25 mg/tableta,
i pri čemu je referentna tableta pripremljena u slijedećim koracima:
1. Eudragit® RS 30D i Triacetin® pomiješani su tijekom prolaska kroz sito 60 mesh, te su pomiješani uz nisko smicanje tijekom približno 5 minuta ili dok nije uočena jednolika disperzija.
2. Oksikodon HCl, laktoza, i povidon su postavljeni u fluidiziranom sloju u posudu granulatora/sušilice (FBD), te je suspenzija raspršena u prah u fluidiziranom sloju.
3. Nakon raspršivanja, granulat je proveden kroz sito #12 ako je potrebno smanjiti grudice.
4. Suhi granulat je postavljen u miješalicu.
5. U međuvremenu, je na temperaturi od približno 70 °C otopljena tražena količina stearil alkohola.
6. Otopljeni stearil alkohol je pomiješan u granulat tijekom miješanja.
7. Granulat u obliku voska prebačen je u granulator/sušilicu u fluidiziranom sloju ili u posude i ostavljen da se ohladi do ili ispod sobne temperature.
8. Ohlađeni granulat je zatim proveden kroz sito #12.
9. Granulat u obliku voska postavljen je u miješalicu i podmazivan je sa traženim količinama talka i magnezijevog stearata tijekom približno 3 minute.
10. Granulat je komprimiran u tablete od 125 mg na pogodnom stroju za tabletiranje.
22. Oblik doziranja sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 21, naznačen time da je opioidni analgetik odabran iz skupine koja sadrži alfentanil, alilprodin, alfaprodin, anileridin, benzilmorfin, bezitramid, buprenorfin, butorfanol, klonitazen, kodein, desomorfin, dekstromoramid, dezocin, diampromid, diamorfon, dihidrokodein, dihidromorfin, dimenoksadol, dimepheptanol, dimetiltiambuten, dioksafetil butirat, dipipanon, eptazocin, etoheptazin, etilmetiltiambuten, etilmorfin, etonitazen, etorfin, dihidroetorfin, fentanil i njegove derivate, hidrokodon, hidromorfon, hidroksipetidin, izometadon, ketobemidon, levorfanol, levofenacilmorfan, lofentanil, meperidin, meptazinol, metazocine, metadon, metopon, morfin, mirofin, narcein, nikomorfin, norlevorfanol, normetadon, nalorfin, nalbufen, normorfin, norpipanon, opijum, oksikodon, oksimorfon, papaveretum, pentazocin, fenadokson, fenomorfan, fenazocin, fenoperidin, piminodin, piritramid, profeptazin, promedol, properidin, propoksifen, sufentanil, tilidin, tramadol, njihove farmaceutski prihvatljive soli, hidrati i solvati, te bilo koje smjese prethodno navedenih analgetika.
23. Oblik doziranja sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 22, naznačen time da je opioidni analgetik odabran iz skupine koja sadrži kodein, morfin, oksikodon, hidrokodon, hidromorfon, ili oksimorfon ili njihove farmaceutski prihvatljive soli, hidrati i solvati, te bilo koje smjese prethodno navedenih analgetika.
24. Oblik doziranja sa produljenim otpuštanjem prema zahtjevu 23, naznačen time da opioidni analgetik je oksikodon hidroklorid i oblik doziranja sadrži od oko 5 mg do oko 500 mg oksikodon hidroklorida.
25. Oblik doziranja sa produljenim otpuštanjem prema zahtjevu 24, naznačen time da oblik doziranja sadrži 5 mg, 7.5 mg, 10 mg, 15 mg, 20 mg, 30 mg, 40 mg, 45 mg, 60 mg, ili 80 mg, 90 mg, 120 mg ili 160 mg oksikodon hidroklorida.
26. Oblik doziranja sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 25, naznačen time da opioidni analgetik je oksikodon hidroklorid koji ima razinu 14-hidroksikodeinona od manje od oko 25 ppm, poželjno od manje od oko 15 ppm, manje od oko 10 ppm, ili manje od oko 5 ppm.
27. Oblik doziranja sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 22, naznačen time da opioidni analgetik je oksimorfon hidroklorid i oblik doziranja sadrži od oko 1 mg do oko 500 mg oksimorfon hidroklorida.
28. Oblik doziranja sa produljenim otpuštanjem prema zahtjevu 27, naznačen time da oblik doziranja sadrži 5 mg, 7.5 mg, 10 mg, 15 mg, 20 mg, 30 mg, 40 mg, 45 mg, 60 mg, ili 80 mg, 90 mg, 120 mg ili 160 mg oksimorfon hidroklorida.
29. Oblik doziranja sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 22, naznačen time da opioidni analgetik je hidromorfon hidroklorid i oblik doziranja sadrži od oko 1 mg do oko 100 mg hidromorfon hidroklorida.
30. Oblik doziranja sa produljenim otpuštanjem prema zahtjevu 29, naznačen time da oblik doziranja sadrži 2 mg, 4 mg, 8 mg, 12 mg, 16 mg, 24 mg, 32 mg, 48 mg ili 64 mg hidromorfon hidroklorida.
31. Oblik doziranja sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 30, naznačen time da je u obliku tablete oblikovane pomoću direktnog prešanja pripravka, te koja je stvrdnuta barem podvrgavanjem navedene tablete temperaturi od najmanje oko 60°C ili najmanje oko 62°C u vremenskom periodu od najmanje oko 1 minute, poželjno najmanje oko 5 minuta ili najmanje oko 15 minuta.
32. Oblik doziranja sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 31, naznačen time da je u obliku tablete, te da je obložen slojem polietilen oksida u prahu tako da tvori tabletu koja ima jezgru tablete i sloj polietilen oksida koji obavija jezgru tablete.
33. Oblik doziranja sa produljenim otpuštanjem prema bilo kojem od zahtjeva 1 do 32, da je u obliku zbijene dvo ili višeslojne tablete, naznačen time da jedan od slojeva sadrži formulaciju s produljenim otpuštanjem i jedan od ostalih slojeva sadrži formulaciju s trenutnim otpuštanjem.
34. Oblik doziranja sa produljenim otpuštanjem prema zahtjevu 33, naznačen time da formulacija s produljenim otpuštanjem i formulacija s trenutnim otpuštanjem sadrže ista ili različita aktivna sredstva.
35. Oblik doziranja sa produljenim otpuštanjem prema zahtjevu 34, naznačen time da formulacija s produljenim otpuštanjem sadrži opioidni analgetik i formulacija s trenutnim otpuštanjem sadrži neopioidni analgetik.
36. Uporaba oblika doziranja prema bilo kojem od zahtjeva 1 do 35 naznačen time da je za proizvodnju lijeka za liječenja bola, pri čemu oblik doziranja sadrži opioidni analgetik.
37. Farmaceutska tableta prema bilo kojem od prethodnih zahtjeva naznačena time da ima silu loma od najmanje 110 N, poželjno 120 N, poželjnije 130 N, te najpoželjnije 140 N, kada je podvrgnuta indentacijskom testu.
38. Farmaceutska tableta prema bilo kojem od prethodnih zahtjeva naznačena time da ima dubinu prodiranja s razmakom pukotine od najmanje 1.0 mm, poželjno 1.2 mm, poželjnije 1.4 mm, te najpoželjnije 1.6 mm, kada je podvrgnuta indentacijskom testu.
39. Farmaceutska tableta prema bilo kojem od prethodnih zahtjeva naznačena time da se može oduprijeti radu od najmanje 0.06 J bez loma.
40. Farmaceutska tableta prema prethodnim zahtjevima naznačena time da ima silu loma od najmanje 110 N, poželjno 120 N, poželjnije 130 N, te najpoželjnije 140 N, kada je podvrgnuta indentacijskom testu; (b) dubinu prodiranja s razmakom pukotine od najmanje 1.0 mm, poželjno 1.2 mm, poželjnije 1.4 mm, te najpoželjnije 1.6 mm, kada je podvrgnuta indentacijskom testu; te (c) da se može oduprijeti radu od najmanje 0.06 J bez loma.
41. Farmaceutska tableta prema bilo kojem od prethodnih zahtjeva 37 do 40 naznačena time da ima gustoću od manje od 1.20 g/cm3, poželjno manje od 1.19 g/cm3.
