Abstract
1. Claims (20)
1. Фармацевтическая композиция, содержащая антитело или его антигенсвязывающий фрагмент, который специфически связывает пропротеин конвертазы субтилизина/кексина типа 9 человека (hPCSK9), в количестве от приблизительно 40 мг до приблизительно 500 мг на одну дозу, вместе с фармацевтически приемлемым эксципиентом или носителем.
2. Фармацевтическая композиция по п.1, содержащая приблизительно 50 мг, приблизительно 100 мг, приблизительно 150 мг, приблизительно 200 мг, приблизительно 250 мг, приблизительно 300 мг, приблизительно 350 мг, приблизительно 400 мг, приблизительно 450 мг или приблизительно 500 мг антитела или его антигенсвязывающего фрагмента.
3. Фармацевтическая композиция по п.1, содержащая эффективную дозу антитела или его антигенсвязывающего фрагмента, который специфически связывает hPCSK9, где такая доза достаточна для устойчивого снижения уровней липопротеинов низкой плотности (LDL-C) в течение по меньшей мере 14 дней, по меньшей мере 15 дней, по меньшей мере 16 дней, по меньшей мере 17 дней, по меньшей мере 18 дней, по меньшей мере 19 дней, по меньшей мере 20 дней, по меньшей мере 21 дня, по меньшей мере 22 дней, по меньшей мере 23 дней или по меньшей мере 28 дней после введения.
4. Фармацевтическая композиция по п.1, где антитело или его антигенсвязывающий фрагмент характеризуется одним или несколькими признаками из следующих:
а. снижение уровней липопротеинов низкой плотности (LDL-C), на величину по меньшей мере от приблизительно 25% до приблизительно 40%, по сравнению с уровнем до введения дозы, при устойчивом снижении в течение периода по меньшей мере 14 дней после введения индивиду дозы, приблизительно равной 50 мг, в режиме введения один раз в две недели;
b. снижение уровней LDL-C на величину по меньшей мере от приблизительно 50% до приблизительно 65%, по сравнению с уровнем до введения дозы, при устойчивом снижении в течение периода по меньшей мере 14 дней после введения дозы, приблизительно равной 100 мг, в режиме введения один раз в две недели;
с. снижение уровней LDL-C на величину по меньшей мере от приблизительно 60% до по меньшей мере приблизительно 75%, по сравнению с уровнем до введения дозы, при устойчивом снижении в течение периода по меньшей мере 14 дней после введения дозы, приблизительно равной 150 мг, в режиме введения один раз в две недели;
d. снижение уровней LDL-C на величину по меньшей мере от приблизительно 40% до приблизительно 75%, по сравнению с уровнем до введения дозы, при устойчивом снижении в течение периода по меньшей мере 28 дней после введения дозы, приблизительно равной 200 мг, в режиме введения каждую четвертую неделю;
е. снижение уровней LDL-C на величину по меньшей мере от приблизительно 50% до приблизительно 75%, по сравнению с уровнем до введения дозы, при устойчивом снижении в течение периода по меньшей мере 28 дней после введения дозы, приблизительно равной 300 мг, в режиме введения каждую четвертую неделю;
f. увеличение уровней сывороточного HDL холестерина по меньшей мере на 2-5,5% по сравнению с уровнем до введения дозы, при введении дозы приблизительно равной 150 мг в режиме введения каждую вторую неделю;
g. наличие небольшого или нерегистрируемого эффекта на уровни тропонина, при введении дозы, приблизительно равной 50-300 мг, в режиме введения каждую вторую-четвертую неделю;
h. увеличение одного или нескольких из следующего: уровней общего холестерина, уровней Apo-B, уровней холестерина, не относящегося к HDL-С или соотношения Apo-B/ApoA-1, при введении индивиду.
5. Фармацевтическая композиция по п.1, где антитело или его антигенсвязывающий фрагмент способен устранить устойчивость к статинам при введении индивиду с гиперхолестеринемией, устойчивой к действию статинов.
6. Фармацевтическая композиция по п.1, где антитело или его антигенсвязывающий фрагмент содержит последовательности CDR тяжелой цепи и CDR легкой цепи из аминокислотной последовательности пары HCVR и LCVR, имеющей последовательность пары SEQ ID NO:90/92.
7. Фармацевтическая композиция по п.1, где композиция представляет собой жидкий состав.
8. Фармацевтическая композиция по п.7, где жидкий состав представляет собой раствор для инъекции, содержащий от приблизительно 40 мг до приблизительно 200 мг антитела или антигенсвязывающего фрагмента в 1 мл раствора.
9. Фармацевтическая композиция по п.8, где раствор для инъекции содержит приблизительно 40 мг, приблизительно 50 мг, приблизительно 75 мг, приблизительно 100 мг, приблизительно 150 мг или приблизительно 200 мг антитела или антигенсвязывающего фрагмента в 1 мл раствора.
10. Лекарственная форма, содержащая фармацевтическую композицию по любому из пп.1-9.
11. Лекарственная форма по п.10, содержащая приблизительно 40 мг, приблизительно 50 мг, приблизительно 75 мг, приблизительно 100 мг, приблизительно 150 мг, приблизительно 200 мг, приблизительно 250 мг, приблизительно 300 мг, приблизительно 350 мг, приблизительно 400 мг, приблизительно 450 мг или приблизительно 500 мг антитела или антигенсвязывающего фрагмента.
12. Лекарственная форма по п.10 или 11, содержащая антитело или антигенсвязывающий фрагмент, в виде жидкого состава в герметично закрытом контейнере.
13. Лекарственная форма по п.12, где контейнер выбран из группы, состоящей из флакона, саше, предварительно заполненного шприца, предварительно заполненного автоинъектора, картриджа, многоразового шприца и многоразового аппликатора.
14. Лекарственная форма по п.10, где количество антитела или антигенсвязывающего фрагмента указано на герметично закрытом контейнере.
15. Изделие, содержащее фармацевтическую композицию по любому из пп.1-9 и контейнер.
16. Изделие по п.15, содержащее один или несколько из следующих компонентов:
a. одну или несколько лекарственных форм по любому из пп.10-14;
b. инструкции по применению; и
c. устройство для введения антитела, такое как шприц.
17. Изделие по п.16, дополнительно содержащее одну или несколько лекарственных форм, сождержащих ингибитор HMG-CoA-редуктазы.
18. Изделие по п.16, содержащее лекарственные формы в количестве, достаточном для режима введения один раз в неделю, один раз в две недели или один раз в четыре недели.
19. Фармацевтическая композиция по любому из пп.1-18 для применения при лечении заболевания или состояния, которое вызывается воздействием экспрессии или активности PCSK9.
20. Фармацевтическая композиция по п.19, где заболевание или состояние выбрано из группы, состоящей из: повышенных уровней общего холестерина, повышенных уровней холестерина, относящегося к липопротеинам низкой плотности (LDL-C), гиперлипидемии, дислипидемии, атеросклероза, гиперхолестеринемии, первичной гиперхолестеринемии, семейной гиперхолестеринемии и гиперхолестеринемии, которая не контролируется статинами.
