Description
Oppfinnelsen vedrører forbindelsen 3-(4-(2,6-diokso-l,3-dipropyl-2,3,6,7-tetrahydro-lH-purin-8-yl)-bicyklo[2,2,2]okt-l-yl)-propionsyre som er en antagonist av adenosinreseptorer.
Adenosin er en intracellulær og ekstracellulær budbringer som genereres av alle celler i kroppen. Det genereres også ekstracellulært ved enzymatisk omdannelse. Adenosin bindes til og aktiverer syv transmembrane G-proteinkoblede reseptorer og utløser dermed diverse fysiologiske responser. Adenosin i og for seg, stoffer som etterligner virkningene av adenosin (agonister) og stoffer som antagoniserer dets virkninger, har viktige kliniske anvendelser. Adenosin-reseptorene deles opp i fire kjente undertyper (dvs. Ai, A2a/ A.2b og A3) . Disse undertyper utløser ensartede og iblant motsatte virkninger. Aktivering av adenosin-Ai-reseptor utløser f.eks. en hevet renal vaskulær resistens, mens aktivering av adenosin-A2a-reseptor utløser en nedsatt renal vaskulær resistens.
I de fleste organsystemer fører perioder med metabolsk stress til en betydelig hevet konsentrasjon av adenosin i vevet. F.eks. produserer og frigir hjertet adenosin for å mediere adaptive responser på stress, så som en nedsatt hjertehastighet og koronær vasodilatasjon. På lignende måte heves adenosinkonsentrasjonen i nyrene som respons på hypoksi, metabolsk stress og mange nefrotoksiske stoffer. Nyrene produserer også adenosin konstitutivt. Nyrene juste-rer mengden av konstitutivt produsert adenosin for å regu-lere den glomerulære filtrering og elektrolyttreabsorpsjo-nen. Med hensyn til reguleringen av den glomerulære filtrering, fører en aktivering av Ai-reseptorer til en sammen-trekning av tilførende arterioler, mens en aktivering av A2a-reseptorer fører til en utvidelse av tilførende arterioler. Aktivering av A2a-reseptorer kan også utøve en vasodilatorisk virkning på den tilførende arteriol. Til sammen er virkningen av en aktivering av disse glomerulære adenosinreseptorer å nedsette den glomerulære filtreringsrate. I tillegg er Ax-adenosinreseptorer plassert i proksi-male tubuli og distale tubuli. Aktivering av disse reseptorer stimulerer natriumreabsorpsjon fra tubulær lumen. Følgelig vil en blokkering av adenosinets virkning på disse reseptorer føre til en hevet glomerulær filtreringsrate og en hevet natriumutsondring.
SAMMENFATNING AV OPPFINNELSEN
Oppfinnelsen baserer seg på oppdagelsen at forbindelsen angitt i krav 1 er en uforventet sterk og selektiv inhibitor av bestemte undertyper av adenosinreseptorer. Adenosinantagonister kan være nyttige ved forebyggelse og/eller behandling av tallrike sykdommer, også omfattende hjerte- og sirkulasjonsforstyrrelser, degenerative forstyrrelser i det sentrale nervesystem, respiratoriske forstyrrelser og mange sykdommer som en diuretisk behandling er egnet for.
Forbindelsen kan f.eks. foreligge i form av en akiral forbindelse, et racemat, en optisk aktiv forbindelse, en ren diastereomer, en blanding av diastereomerer eller et farma-kologisk akseptabelt addisjonssalt.
Forbindelsen ifølge foreliggende oppfinnelse kan også modi-fiseres ved tilføyelse av egnede funksjonaliteter for å forsterke selektive biologiske egenskaper. Slike modifika-sjoner er kjent innen faget og omfatter slike som hever den biologiske penetrasjon inn i et gitt biologisk system (f.eks. blod, lymfesystemet, det sentrale nervesystem), hever den orale tilgjengelighet, hever løseligheten for å muliggjøre administrasjon ved injeksjon, endre metabolismen og/eller endre utsondringshastigheten.
Forbindelsen ifølge oppfinnelsen kan benyttes ved behandling av en hjerte- og sirkulasjonsforstyrrelse, en degenerativ forstyrrelse i det sentrale nervesystem, en respiratorisk forstyrrelse, en sykdom for hvilken en diuretisk behandling er indikert, hypertensjon, Parkinsons sykdom, depresjon, traumatisk hjerneskade, neurologisk mangel etter slag, respiratorisk depresjon, neonatalt hjernetrauma, dysleksi, hyperaktivitet, cystisk fibrose, cirrhotiske askitter, neonatal apné, nyresvikt, diabetes, astma, en ødematøs tilstand, kongestiv hjertesvikt eller renal dysfunksjon (f.eks. dysfunksjon forbundet med diuretisk anvendelse ved kongestiv hjertesvikt eller renal toksisitet grunnet behandling med kjemoterapeutiske midler).
En "antagonist" er et molekyl som bindes til en reseptor uten å aktivere reseptoren. Det konkurrerer med den endogene ligand om dette bindingssete og nedsetter dermed evnen av den endogene ligand til å stimulere reseptoren.
En "selektiv antagonist" er en antagonist som bindes med høyere affinitet til en bestemt undertype adenosinreseptor enn til andre undertyper.
Oppfinnelsen tilveiebringer tallrike fordeler. Forbindelsen kan lett fremstilles fra lett tilgjengelige utgangsstoffer, med et relativt lite antall trinn. Som spesifikk antagonist har forbindelsen bred medisinsk nytte. Ettersom forbindelsen er en meget sterk og spesifikk antagonist, kan den (1) brukes i lave doser for å minimere sannsynligheten for bivirkninger og (2) innlemmes i tallrike doseringsformer omfattende piller, tabletter, kapsler, aerosoler, suppositorier, flytende formuleringer for inntak eller injeksjon, kosttilskudd eller topiske preparater. I tillegg til medisinske anvendelser, kan antagonistforbin-delsen brukes ved behandling av fe og kjeledyr.
Hvis intet annet er nevnt, har alle tekniske og vitenskape-lige uttrykk som anvendes heri, den samme betydning som vanligvis forstås av én med vanlige kunnskaper innen faget som denne oppfinnelsen vedrører. Selv om lignende eller likeverdige metoder og materialer som dem som beskrives heri, kan brukes ved utøvelse eller testing av foreliggende oppfinnelse, beskrives egnede fremgangsmåter og materialer i det følgende.

Anvendelse av adenosinantagonistforbindelsen
Aktivering av adenosinreseptorer utløser mange fysiologiske responser, også omfattende nedsettelse av den renale blod-strøm, nedsettelse av den glomerulære filtreringsrate og hevet natriumreabsorpsjon i nyrene. Aktivering av adenosinreseptorer nedsetter hjertehastigheten, nedsetter led-ningshastigheten og nedsetter kontraktiliteten. Disse og andre virkninger av en aktivering av adenosinreseptorer i andre organer er normale regulatoriske prosesser. Imidler-tid blir disse virkninger patologiske i mange sykdoms-tilstander. Dermed finner adenosinantagonister bred anvendelse både ved forebyggelse og behandling av sykdom. Sykdommer som kan forebygges og/eller behandles med adenosinreseptorantagonister, omfatter hvilke som helst tilstander som (a) kjennetegnes ved at det foreligger et abnormalt nivå av adenosin og/eller (b) krever en inhibering eller stimulering av adenosinproduksjonen og/eller -frigivningen for å behandles. Slike tilstander omfatter kongestiv hjertesvikt, kardiopulmonær gjenopplivning, hemorrhagisk sjokk og andre hjerte- og sirkulasjonsforstyrrelser; degenerative forstyrrelser i det sentrale nervesystem; respiratoriske forstyrrelser (f.eks. bronchial astma, allergiske lungesykdommer); og mange sykdommer for hvilke en diuretisk behandling er indikert (f.eks. akutt og kronisk nyresvikt, renal insuffisiens, hypertensjon). Degenerative sykdommer så som Parkinsons sykdom, depresjon, traumatisk hjerneskade, neurologisk mangel etter slag, neonatalt hjernetrauma, dysleksi, hyperaktivitet og cystisk fibrose er alle blitt satt i sammenheng med adenosinreseptoraktiviteten. Andre tilstander hvor behandling med adenosinreseptorantagonister kan ha terapeutisk nytte, omfatter cirrhotiske askitter, neonatal apné, nyresvikt forbundet med tradisjonell diuretisk terapi, diabetes og astma.
I tillegg har oppfinnerne oppdaget at en administrasjon av høyt selektive og sterke adenosin-Ai-reseptorantagonister for eksempel kan utløse en diuretisk respons når de administreres alene, og kan forsterke den diuretiske respons på tradisjonelle diuretika. I tillegg forsterker en administrasjon av adenosinreseptorantagonister med tradisjonelle diuretika nedsettelsen av den glomerulære filtreringsrate som induseres av tradisjonelle diuretika. Forbindelsen som kreves beskyttet, kan anvendes f.eks. i ødematøse tilstander, så som kongestiv hjertesvikt og askitt.
Administrasjon av adenosinantagonistforbindelsen
Forbindelsen kan administreres til et dyr (f.eks. et pat-tedyr så som et menneske, ikke-human primat, hest, hund, ku, svin, sau, geit, katt, mus, rotte, marsvin, kanin, hamster, ørkenrotte, frett, øgle, reptiler eller fugler). Forbindelsen kan administreres på hvilken som helst måte som er egnet for administrasjon av farmasøytiske forbindelser, også omfattende, men ikke begrenset til, piller, tabletter, kapsler, aerosoler, suppositorier, flytende formuleringer for inntak eller injeksjon eller for anvendelse som øye- eller øredråper, kosttilskudd og topiske preparater. Forbindelsen kan administreres oralt, intranasalt, transdermalt, intradermalt, vaginalt, intraauralt, intraokulært, bukkalt, rektalt, transmukosalt eller via inhalasjon, implantasjon (f.eks. kirurgisk) eller intravenøs administrasjon.
Valgfritt kan forbindelsen administreres sammen med en ikke-adenosinmodifiserende farmasøytisk sammensetning (f.eks. i kombinasjon med en ikke-adenosinmodifiserende diuretisk forbindelse slik det f.eks. beskrives i den sam-tidig svevende søknad PCT/US99/08879 innlevert den 23. april 1999).

Til reaksjonsblandingen tilsettes vann (150 ml), og det hele inndampes for å gi en olje, som løses opp i etylacetat/heksan (5/1, 500 ml). Blandingen vaskes med salin (2 x 200 ml), tørkes over vannfritt natriumsulfat og inndampes for å gi tittelforbindelsen, NMR (400 MHz, CDC13) 8 7,39; 5,11; 3,28; 1,84 og 1,41.

Oksalylklorid (C0C1)2 (16,5 ml, 188 mmol) i metylenklorid (150 ml) avkjøles til -63°. DMSO (18 ml, 362 mmol) tilsettes deretter dråpevis. Den dannede blanding omrøres i 30 minutter, og deretter tilsettes en blanding av 4-hydroksymetylbicyklo[2,2,2]oktan-l-karboksylsyrebenzylester, referanseeksempel 6, 34,5 g, 125 mmol) i metylenklorid (100 ml) i løpet av 15 minutter. 3-[ 4-( 2, 6- Diokso- l, 3- dipropyl- 2, 3, 6, 7- tetrahydro- lff- purin-8- yl) bicyklo[ 2, 2, 2] okt- l- yl] propionsyre
3-[4-(6-Amino-2,4-diokso-l,3-dipropyl-l,2,3,4-tetrahydro-pyrimidin-5-ylkarbamoyl)bicyklo[2,2,2]okt-l-yl]propionsyre-metylester (referanseeksempel 11, 1,95 g, 4,35 mmol) løses opp i 2-propanol (15 ml), og kaliumhydroksid (2N, 15 ml) tilsettes. Den dannede blanding oppvarmes til svakt til-bakeløp i en time og avkjøles deretter til 20-25°C. Vann (15 ml) tilsettes, og blandingen vaskes med metylenklorid (3 x 15 ml). Det vandige sjikt surgjøres til pH ca. 2 med konsentrert saltsyre. Felningen samles ved filtrering og tørkes i en vakuumovn i 16 timer for å gi tittelforbindelsen; HPLC-analyse viste at renheten var >96%.

Assaymetode
Forbindelsen ifølge oppfinnelsen ble fremstilt, som har strukturene som oppgis på fig. 1. For denne forbindelsen bestemte man Ki-verdiene for rotte- og humane adenosin-Ax-reseptorer og for humane adenosin-A2a-reseptorer i henhold til den følgende bindingsassayprotokoll. Forholdet A2a/Ai ble også beregnet.
Materialer
Adenosindeaminase og HEPES ble ervervet fra Sigma (St. Louis, MO). Hams F-12-cellekulturmedium og føtalt bovint serum ble ervervet fra GIBCO Life Technologies (Gaithersburg, MD) . Antibiotikum G-418, Falcon 150 rtiM kulturplater og Costar 12-brønners kulturplater ble ervervet fra Fisher (Pittsburgh, PA). [<3>H]CPX ble ervervet fra DuPont-New Eng-land Nuclear Research Products (Boston, MA). Penicillin/streptomycin-antibiotikablanding ble ervervet fra Mediatech (Washington, DC). Sammensetningen av HEPES-bufret Hank's oppløsning var: 130 mM NaCl, 5,0 mM Cl, 1,5 mM CaCl2, 0,41 mM MgS04, 0, 49 mM Na2HP04, 0,44 mM KH2P04, 5,6 mM dekstrose og 5 mM HEPES (pH 7,4).
Membranpreparering
Rotte- Ax- reseptor: Membraner ble fremstilt fra rotte-cere-bral cortex isolert fra nylig eutaniserte rotter. Vevene ble homogenisert i buffer A (10 mM EDTA, 10 mM Na-HEPES, pH 7,4) supplementert med proteaseinhibitorer (10 ug/ml benz-amidin, 100 uM PMSF og 2 ug/ml hver av aprotinin, pepstatin og leupeptin) og sentrifugert ved 20.000 x g i 20 min. Pel-letene ble gjensuspendert og vasket to ganger med buffer HE (10 mM Na-HEPES, 1 mM EDTA, pH 7,4, plus proteaseinhibitorer) . De endelige pelleter ble gjensuspendert i buffer HE, supplementert med 10% (vekt/vol) sakkarose og proteaseinhibitorer og frosset i alikvoter ved -80°C. Protein-konsentrasjonene ble målt ved bruk av BCA-proteinassaysett (Pierce).
Human Ax- reseptor: Human Al-adenosinreseptor-cDNA ble til-veiebragt ved RT-PCR og delklonet inn i pcDNA3.1 (Invitro-gen). En stabil transfeksjon av CHO-Kl-celler ble utført ved bruk av LIPOFECTAMIN-PLUS (GIBCO-BRL), og koloniene ble utvalgt i 1 mg/ml G418 og testet ved bruk av radioligandbindingsassayer. For membranpreparering ble CHO-Kl-celler som vokste som monosjikt i fullstendig medium (F12 + 10% FCS + 1 mg/ml G418), vasket i PBS og samlet i buffer A supplementert med proteaseinhibitorer. Cellene ble homogenisert, sentrifugert og vasket to ganger med buffer HE som beskrevet ovenfor. De endelige pelleter ble lagret i alikvoter ved -80°C.
Radioligandbindingsassayer
Membraner (50 ug membranprotein for rotte-AlAR'er og 25 ug CHO-Kl-membranprotein for humane AlAR'er), radioligander og forskjellige konsentrasjoner av konkurrerende ligander ble inkubert in triplo i 0,1 ml buffer HE plus 2 enheter/ml adenosindeaminase i 2,5 h ved 21°C. Radioligand [<3>H]DPCPX (112 Ci/mmol fra NEN, endelig konsentrasjon: 1 nM) ble brukt for konkurransebindingsassay på AiAR'er. Den ikke-spesifikke binding ble målt i nærvær av 10 uM BG9719. Bindingsassayene ble avsluttet ved filtrering over Whatman GF/C-glassfiberfiltre ved bruk av en celleinnsamlingsanord-ning fra BRANDEL. Filtrene ble skylt tre ganger med 3-4 ml iskald 10 mM Tris-HCl, pH 7,4, og 5 mM MgCl2 ved 4°C. Fil-terpapiret ble overført til et glass, og 3 ml scintilla-sjonsblanding ScintiVersell (Fisher) ble tilsatt. Radioaktiviteten ble telt i en p-teller fra Wallac.
Analyse av bindingsdataen
For Kj- bestemmelser: Konkurransebindingsdataen ble anpasset til en enkeltsete-bindingsmodell og plottet ved bruk av Prizm GraphPad. Cheng-Prusof f-lignings-Ki = IC50/ (1+ [I] /KD) ble brukt for å beregne Ki-verdiene fra IC5o-verdiene, hvor Kj er affinitetskonstanten for den konkurrerende ligand,
[I] er konsentrasjonen av den frie radioligand og KD er affinitetskonstanten for radioliganden.
For % binding: For énpunkts bindingsassayene ble dataen presentert som % av den samlede spesifikke binding ved 1 uM konkurrerende forbindelse: % av totalen = 100 x (spesifikk binding med 1 uM konkurrerende forbindelse / samlet spesifikk binding).
Resultater
Den testede forbindelse oppviste en rotte-Ai-Ki-verdi mellom 0,6 og 433, 8 nM, humant Ai-Ki-verdi mellom 1,6 og 1000 nM og humant A2a-Ki-verdi mellom 132 og 49930 nM. Forbindelsen hadde A2a/A1-forhold over 10.

Cellekultur
CHO-celler som stabilt uttrykte den rekombinante humane AiAdoR (CHO:AiAdoR-celler) ble fremstilt slik som beskrevet (Kollias-Barker et al., J. Ph arma. Exp. Ther. 281 (2), 761, 1997) og dyrket som CHO:villtype-celler. CHO-cellene ble dyrket som monosjikt på plastskåler i Hams F-12-medium supplementert med 10% føtalt bovint serum, 100 U penicillin G og 100 ug streptomycin i en fuktet atmosfære av 5% C02/95% luft ved 37°C. Tettheten av [<3>H]CPX-bindingssetene
i CHO-cellene var 26+2 (n=4) fmol/mg protein. Cellene ble delkultivert to ganger ukentlig etter løsning ved bruk av 1 mM EDTA i Ca<2+->Mg<2+->fritt HEPES-bufret Hank' s oppløsning. Tre forskjellige kloner av CHO:AiAdoRceller ble brukt for eksperimentene, og alle resultatene ble bekreftet med cells fra to eller tre kloner. Tettheten av AiAdoR'er i disse celler var 4000-8000 fmol/mg protein, bestemt ved assay av den [<3>H]CPX-spesifikke binding.
Radioligandbinding
CHO-celler som var blitt dyrket på 150 mm kulturskåler, ble skylt med HEPES-bufret Hanks oppløsning, deretter fjernet med en celleskraper og homogenisert i iskald 50 mM Tris-HC1, pH 7,4. Cellemembranene ble pelletert ved sentrifugering av cellehomogenatet ved 48.000 x g i 15 minutter. Membranpelleten ble vasket to ganger ved gjensuspensjon i fersk buffer og sentrifugering. Den endelige pellet ble gjensuspendert i et lite volum av 50 mM Tris-HCl, pH 7,4, og lagret i alikvoter på 1 ml ved -80°C inntil de ble brukt 1 assayer.
For å bestemme tettheten av A;iAdoR'er i CHO-cellemembraner, ble 100 ul alikvoter av membraner (5 ug protein) inkubert i 2 timer ved 25°C med 0,15-20 nM [<3>H]CPX og adenosindeaminase (2 U/ml) i 100 ul 50 mM Tris-HCl, pH 7,4. Inkubasjo-nene ble avsluttet ved fortynning med 4 ml iskald 50 mM Tris-HCl-buffer og en umiddelbar innsamling av membranene på glassfiberfiltre (Schleicher og Schuell, Keene, NH) ved vakuumfiltrering (Brandel, Gaithersburg, MD). Filtrene ble vasket raskt tre ganger med iskald buffer for å fjerne ubundet radioligand. Filterplater som inneholdt innfanget membranbundet radioligand, ble plassert i 4 ml Scintiverse BD (Fisher), og radioaktiviteten ble kvantifisert ved bruk av en væskescintillasjonsteller. For å bestemme den ikke-spesifikke binding av [<3>H]CPX, ble membranene inkubert som beskrevet ovenfor, og 10 uM CPT ble tilsatt til inkuba-sjonsbufferen. Den ikke-spesifikke binding ble definert som [<3>H]CPX bundet i nærvær av 10 uM CPT. Den spesifikke binding av radioliganden til A]AdoR ble bestemt ved å subtra-here den ikke-spesifikke binding fra den samlede binding. Den ikke-spesifikke binding viste seg å øke lineært med en stigende [<3>H]CPX-konsentrasjon. Assayene ble utført in triplo ved hver testet konsentrasjon av [<3>H]CPX.
For å bestemme affinitetene av antagonister av AiAdoR'er for human rekombinant AiAdoR uttrykt i CHO-celler, målte man bindingen av 2 nM [<3>H]CPX i nærvær av økende konsentrasjoner av antagonist. Alikvoter av CHO-cellemembraner (100 ul: 5 ug protein), [<3>H]CPX, antagonist (0,1 nM - 100 uM) og adenosindeaminase (2 U/ml) ble inkubert i 3 timer ved 25°C i 200 ul 50 mM Tris-HCl-buffer (pH 7,4). Assayene ble avsluttet som beskrevet ovenfor.
Dataanalyse
Bindingsparametrene (dvs. Bmax, Kd, IC50, Ki og Hill-koeffi-sientene) ble bestemt ved bruk av radioligand-bindings-analyseprogrammet LIGAND 4.0 (Elsevier-Biosoft).
