Description
Foreliggende oppfinnelse vedrører farmasøytisk preparat omfattende lavdose-entecavir, samt fremstilling derav.
Foreliggende oppfinnelse vedrører følgelig farmasøytisk preparat effektivt
for administrering én gang per dag for å behandle hepatitt B-virusinfeksjon, omfattende fra 0,01 mg til 5 mg av entecavir festet til overflaten av et farmasøytisk akseptabelt substrat. Det er videre beskrevet fremgangsmåte for fremstilling av et farmasøytisk preparat som angitt i krav 1 som omfatter: (a) å oppløse nevnte entecarvir og en klebende substans i et løsningsmiddel, nevnte løsningsmiddel er vann eller vann som har en sur eller basisk pH,
(b) å sprøyte nevnte løsning fra trinn (a) på et bærersubstrat mens
nevnte bærersubstrat er i bevegelse for å feste nevnte entecavir til overflaten av bærersubstratet,
(c) å tørke det belagte bærersubstrat fra trinn (b) for å fjerne
løsningsmidlet, og
(d) å kombinere det tørkede belagte bærersubstrat fra trinn (c) med
andre ønskede ingredienser for å danne det farmasøytiske preparat.
Entecavir, [1 S-(1 a, 3a, 4p)}-2-amino-1,9-dihydro-9-[4-hydroksy-3-(hydroksymetyl)-2-metylencyklopentyl]-6H-purin-6-on
er et antiviralt middel som for tiden gjennomgår klinisk evaluering for behandling av hepatitt B-virusinfeksjon.
Entecavir og dets anvendelser ved behandling av hepatitt B er beskrevet av Zahler et al. i US-patentskrift nr. 5 206 244. Dette patent åpenbarer at en effektiv antiviral dose for oral eller parenteral administrering sannsynligvis vil ligge i området fra ca. 1,0 til 50 mg/kg kroppsvekt, og at den ønskede dose kan administreres flere ganger daglig med passende intervaller.
Forbedrede fremgangsmåter for syntese av entecavir åpenbares i Bisacchi et al. i WO 98/09964.
Foreliggende oppfinnelse er rettet mot farmasøytiske preparater som inneholder en lav dose av entecavir.
Oppfinnelsen er også rettet mot farmasøytiske preparater for oral administrering som inneholder lave doser av en farmasøytisk aktiv substans. Dette resultat oppnås ved å feste partikler av den farmasøytisk aktive substans til overflaten av et bærersubstrat. Prosessen med å deponere den aktive substans på bærersubstratet reguleres for å minimalisere agglomereringen av partiklene av aktiv substans/bærersubstrat.
Oppfinnelsen er rettet mot farmasøytiske preparater som inneholder en lav dose av fra ca. 0,001 mg til ca. 1,0 mg av det aktive antivirale middel entecavir, for administrering én gang daglig for å behandle hepatitt B-virusinfeksjon hos en voksen human pasient. Foreliggende oppfinnelse er følgelig rettet mot farmasøytisk preparat hvor entecavir er til stede i fra 0,001 til 10 vekt-% av nevnte preparat, den nevnte klebende substans er til stede i fra 1 til 10 vekt-% av nevnte preparat, nevnte bærersubstrat er til stede i fra 80 til 95 vekt-% av nevnte preparat, nevnte smuldremiddel er til stede i fra 1 til 5 vekt-% av nevnte preparat, og nevnte smøremiddel er til stede i fra 0,1 til 5 vekt-% av nevnte preparat. Slike foretrukne farmasøytiske preparater administreres også én gang daglig for å behandle hepatitt B-virusinfeksjon hos en voksen pasient.
Betegnelsen "voksen human pasient" defineres som en pasient på ca. 16 år eller mer, og en vekt lik, eller større enn, ca. 50 kg. Farmasøytiske preparater som inneholder entecavir ved den nedre ende av de ovennevnte områder er egnet for administrering til pediatriske pasienter, eller voksne pasienter som veier mindre enn ca. 50 kg.
De lavdose-entecavirfarmasøytiske preparater som er beskrevet ovenfor for daglig administrering kan også administreres til visse pasienter mindre ofte. For eksempel kan pasienter som er blitt behandlet ved daglig administrering av lavdose-entecavirfarmasøytiske preparater, slik at deres hepatitt B-virusinfeksjon nå er under kontroll, bli satt på et vedlikeholdsregime for beskyttelse mot ytterligere infeksjon. Slik vedlikeholdsterapi kan innebære administrering av lavdose-entecavirpreparatet på en mindre enn daglig basis. For eksempel kan en enkelt dose administrert hver tredje eller fjerde dag på ukebasis være tilstrekkelig.
Lavdose-entecavirfarmasøytiske preparater i henhold til oppfinnelsen kan tillages for å administrering på enhver passende måte. For eksempel kan preparater for oral administrering, hvilket foretrekkes, være i form av tabletter, kapsler, granulat eller pulver, eller i form av eliksirer, løsninger eller suspensjoner. De lavdose-entecavirfarmasøytiske preparater kan også tillages for parenteral, rektal, transdermal eller nasal administrering i henhold til metoder som er velkjente på fagområdet. Slike blandinger kan inkludere farmasøytisk akseptable eksipienter inklusive volumgivende midler, smøremidler, smuldremidler, bindemidler osv., som vanlig anvendt i slike preparater og blandinger kan også inneha forsinket frigivelse.
Overraskende nok er det funnet at administrering én gang daglig av de lavdose-entecavirfarmasøytiske preparater i henhold til oppfinnelsen, er effektive med hensyn til å behandle hepatitt B-virusinfeksjon uten uønskede bivirkninger som kan resultere fra administrering av det høydoseregimet som er beskrevet i US-patentskrift nr. 5 206 244.
Hepatitt B-virusinfeksjon kan behandles med lavdose-entecavirpreparater som beskrevet ovenfor, i kombinasjon med ett eller flere andre farmasøytisk aktive midler. Egnede farmasøytisk aktive midler for dette formål inkluderer ett eller flere antivirale midler, f.eks. didanosin, lamivudin, abacavir, adefovir, adefovir-dipivoxil, famiclovir, (2R, 4R)-4-(2,6-diamino-9H-purin-9-yl)-2-hydroksymetyl-1,3-dioksolan (DAPD), hepatitt B-immunmodulerende proteiner (ETH 899 fra Enzo Biochem.), emitricitabin, 1-(2-deoksy-2-fluor-p-D-arabinfuranosyl)-tymin (FMAU), GLQ-233 (forbindelse A, oc-trichosantin), epavudin (L-dT), epicitabin (L-dC), ribavirin, tenofovir (PMPA), 2', 3'-dideoksy-2', 3'-didehydro-p-L-(-)-5-fluorcytidin-[L (-) Fd4C], så vel som andre fluor L- og D-nukleosider. Egnede farmasøytisk aktive midler for dette formål inkluderer også én eller flere immunmodulatorer, f.eks. cc-interferon, P-interferon, pegylert interferon, tymosin-a og hepatitt B-vaksiner, f.eks. HBV/MF59, Hepagen og Theradigm-HBV.
Når det andre farmasøytisk aktive middel eller slike midler er egnet for oral administrering, kan de kombineres med den lave dose av entecavir til en enkelt
tablett eller kapsel. Hvis det eller de andre farmasøytisk aktive midler ikke er forlikelige med entecavir for ko-administrering fra en enkeltdoseringsform, hvis for eksempel administreringsmåten er annerledes eller hvis frekvensen av administrering er annerledes, så vil det eller de andre farmasøytisk aktive midler bli administrert separat. Mengden av andre midler som administreres er den som konvensjonelt anvendes i monoterapi, eller en redusert mengde som bestemt av den behandlende lege. De separate doseformer kan administreres på samme tid eller sekvensielt i henhold til et foreskrevet program.
Ko-infiserte pasienter kan også behandles med lavdose-entecavirpreparater som er beskrevet ovenfor. En ko-infisert pasient er en som er infisert med andre virale eller ikke-virale sykdommer i tillegg til hepatitt B. Spesielt er slik behandling mulig for hepatitt B-pasienter som er ko-infisert med hepatitt C eller HIV. Slike ko-infiserte pasienter blir fortrinnsvis behandlet med de lavdose-entecavirpreparater som er beskrevet ovenfor, i kombinasjon med ett eller flere andre farmasøytisk
aktive midler som beskrevet ovenfor. For eksempel kan en pasient som er ko-infisert med hepatitt B og hepatitt C behandles med lavdose-entecavirpreparater i tillegg til å bli behandlet med et regime av ribavirin og et interferon.
Et annet aspekt ved oppfinnelsen er som angitt ovenfor fremstilling av farmasøytiske preparater, spesielt tabletter og kapsler, som inneholder entecavir i en mengde av mindre enn, eller lik, ca. 10 mg. Slike preparater kan ikke fremstilles med god innholdsensartethet ved ganske enkelt å blande den aktive substans og eksipientene. De tradisjonelle metoder for granulering er heller ikke egnet for produkter som er aktive i så lave doser.
Tablett- og kapselblandinger som inneholder fra ca. 0,001 mg til ca. 10 mg av entecavir fremstilles i henhold til de følgende metoder som sikrer høy potens og god ensartethet ved produktet. Preparatene fremstilles ved først omhyggelig å avsette entecavir på overflaten av bærersubstratpartiklene. Dette trinn fullføres ved å danne en løsning av entecavir i et løsningsmiddel sammen med en klebende substans, ved temperaturer som varierer fra ca. 25 til 80 °C, og påfører løsningen som en dusj eller en strøm mens bærersubstratpartiklene er i bevegelse. Betingelsene styres for å minimalisere partikkelagglomerering. Etterpå blir løsningsmidlet fjernet fra bæreroverflaten og etterlater da entecavirpartiklene festet til overflaten av bærersubstratet. Dette forhindrer separering av entecavir fra substratet og minimaliserer tapet av entecavir under påfølgende prosessering.
Etter tørking blandes de entecavirbelagte bærersubstratpartikler med eventuelle andre ingredienser som skal inkluderes i preparatet, for eksempel et smuldremiddel og/eller smøremiddel. Det resulterende pulver blir deretter komprimert til tabletter eller fylt i kapsler.
Bærersubstratpartiklene holdes i bevegelse under dusjetrinnet ved hjelp av mekanisk eller luftstrømsagitering. I den mekaniske agiteringsprosess blir bærersubstratet anbrakt i en mekanisk mikser (med høy skjærekraft) og agitert. En løsning inneholdende entecavir og klebesubstans holdt ved en temperatur på fra ca. 25 til ca. 80 °C blir sprøytet på bærersubstratpartiklene i en styrt hastighet og ved et atomiseringstrykk (0 - 2 bar). For å maksimalisere mengdene av entecavir som avsettes på bæreren, blir posisjonen av sprøyteutstyret justert for at man skal være sikker på at sprøytemønsteret bare omfatter bæreren. Avsetningshastigheten og sprøytemønsteret styres for å minimalisere partikkelagglomerering. Så snart den entecavirholdige løsning er avsatt, blir de våte entecavir/bærersubstratpartikler overført til en tørker, enten en brett-tørker eller fluidsjikttørker er egnet. Løsningsmidlet fjernes ved forhøyet temperatur. Når løsningsmidlet er vann eller pH-justert vann, er en temperatur på fra ca. 50 til ca. 80 °C passende.
I luftstrømagiteringsmetoden blir bærersubstratet anbrakt i en bolle med en finmasket sikt på bunnen. Den innkommende luftstrøm blir justert slik at substrat-partikkelbevegelsen er konstant og fluid. Bærermaterialet blir likevektsinnstilt til en temperatur på fra ca. 25 til ca. 80 °C. En løsning som inneholder entecavir og klebesubstans holdt ved en temperatur på fra ca. 25 til ca. 80 °C, blir sprøytet på bærersubstratpartiklene ved styrt hastighet og atomiseringstrykk som beskrevet ovenfor. Igjen blir posisjonen av sprøyteutstyret justert for at man skal være sikker på at sprøytemønsteret bare omfatter bæreren, og avsetningshastigheten blir styrt for å minimalisere partikkelagglomerering. Så snart entecavirløsningen er avsatt, blir temperaturen forhøyet for fjerning av løsningsmidlet. Når løsningsmidlet er vann eller pH-justert vann, er en temperatur på fra ca. 50 til ca. 80 °C passende. I fremgangsmåten med luftstrømsagitering utføres både avsetningen av entecavir på bærersubstrat og .fjerning avJøsningsmiddel i en enkelt enhet mens den mekaniske agiteringsmetode krever en operasjon med to enheter.
De ovennevnte prosesser har den ytterligere fordel å redusere eksponering av produksjonspersonalet for entecavir i atmosfæren på stedet.
Mens de ovennevnte metoder er beskrevet for fremstilling av farmasøytiske preparater som inneholder fra ca. 0,005 mg til ca. 10 mg entecavir, kan de også anvendes for å fremstille farmasøytiske preparater som inneholder lave doser av enhver løselig farmasøytisk aktiv substans.
Foretrukne løsningsmidler i de ovenfor beskrevne metoder er vann og pH-justert vann. Løseligheten av entecavir i vann kan økes ved å senke vannets pH-verdi ved tilsetning av en syre, for eksempel saltsyre, eller ved å heve vannets pH-verdi ved tilsetning av en base, for eksempel ammoniumhydroksyd.
Klebesubstansen er fortrinnsvis et polymert materiale som har en høy grad av klebrighet. Egnede materialer inkluderer povidon, metylcellulose, hydroksymetylcellulose, hydroksypropylmetylcellulose, hydroksypropylcellulose, hydroksyetylcellulose, gelatin, guargummi og xantangummi og blandinger derav, idet povidon foretrekkes. Den klebende substans er fortrinnsvis til stede i sluttblandingen med fra ca. 0,01 % til ca. 10 % i vekt av hele blandingen.
Bærersubstratet er en farmasøytisk akseptabel substans som lett kan bli sprøytebelagt og allikevel ikke lett vil agglomerere. Eksempel 8
Tryggheten og den antivirale aktivitet hos entecavir gitt i 28 dager til personer med kronisk hepatitt B-virusinfeksjon, ble studert i et randomisert, dobbeltblindt, placebostyrt, doseeskalerende forsøk. Entecavir viste potent antiviral aktivitet ved alle de testede doser. Den gjennomsnittlige logg-reduksjon i hepatitt B-virus-DNA-virale nivåer i blodet ved dag 28 var henholdsvis 2,21, 2,25, 2,81 og 2,42 for de 0,05, 0,1, 0,5 og 1,0 mg én gang per dag doser av entecavir. Entecavir ble tålt godt.
Eksempel 9
Tryggheten og den antivirale aktivitet til tre doser av entecavir (0,01 mg, 0,1 mg og 0,5 mg) gitt én gang daglig i 24 uker, ble studert hos voksne personer med kronisk hepatitt B i et randomisert, dobbeltblindt, lamivudin (100 mg QD) styrt forsøk. Alle tre doser av entecavir viste potent antiviral aktivitet. De to høyere doser av entecavir ga signifikant større reduksjoner i hepatitt B-virus-DNA-virale nivåer i blod sammenlignet med lamivudin. Entecavir ble tålt godt i alle doser.
Claims (21)
1. Farmasøytisk preparat effektivt for administrering én gang per dag for å behandle hepatitt B-virusinfeksjon, omfattende fra 0,01 mg til 5 mg av entecavir festet til overflaten av et farmasøytisk akseptabelt substrat.
2. Preparat som angitt i krav 1, hvor nevnte entecavir er til stede i ca. 0,01 mg.
3. Preparat som angitt i krav 1, hvor nevnte entecavir er til stede i ca. 0,05 mg.
4. Preparat som angitt i krav 1, hvor nevnte entecavir er til stede i ca. 0,1 mg.
5. Preparat som angitt i krav 1, hvor nevnte entecavir er til stede i ca. 0,5 mg.
6. Preparat som angitt i krav 1, hvor nevnte entecavir er til stede i ca. 1,0 mg.
7. Preparat som angitt i krav 1, i form av en tablett eller kapsel.
8. Farmasøytisk preparat som angitt i krav 1, omfattende: fra 0,05 mg til 1,0 mg av entecavir festet til overflaten av et bærersubstrat, hvor nevnte bærersubstratet er valgt blant laktose, mikrokrystallinsk cellulose, kalsiumfosfat, dekstrin, dekstrose, dekstrater, mannitol, sorbitol og sukrose og blanding derav, og nevnte entecavir er festet til nevnte substrat ved en farmasøytisk akseptabel klebesubstans som er et polymert materiale valgt blant povidon, metylcellulose, hydroksymetylcellulose, hydroksypropylmetylcellulose, hydroksypropylcellulose, hydroksyetylcellulose, gelatin, guargummi og xantangummi og blandinger derav.
9. Preparat som angitt i krav 8, som inkluderer et smøremiddel og et smuldremiddel.
10. Preparat ifølge krav 9, hvor smøremidlet er valgt blant magnesiumstearat, stearinsyre, natriumstearylfumarat og natriumlaurylsulfat og blandinger derav, og hvor smuldremidlet er valgt blant krospovidon, kroskarmellosenatrium, natriumstivelseglykolat, pregelatinert stivelse og maisstivelse og blandinger derav.
11. Farmasøytisk preparat som angitt i krav 10, hvor: entecavir er til stede i fra 0,001 til 10 vekt-% av nevnte preparat, den nevnte klebende substans er til stede i fra 1 til 10 vekt-% av nevnte preparat, nevnte bærersubstrat er til stede i fra 80 til 95 vekt-% av nevnte preparat, nevnte smuldremiddel er til stede i fra 1 til 5 vekt-% av nevnte preparat, og nevnte smøremiddel er til stede i fra 0,1 til 5 vekt-% av nevnte preparat.
12. Preparat som angitt i krav 11, hvor: den klebende substans er valgt blant povidon, metylcellulose,
hydroksymetylcellulose, hydroksypropylmetylcellulose, hydroksypropylcellulose, hydroksyetylcellulose, gelatin, guargummi og xantangummi og blandinger derav, nevnte bærersubstrat er valgt blant mikrokrystallinsk cellulose, kalsiumfosfat, dekstrin, dekstrose, dekstrater, mannitol, sorbitol og sukrose og blandinger derav, nevnte smuldremiddel er valgt blant krospovidon, kroskarmellosenatrium, natriumstivelseglykolat, pregelatinert stivelse og maisstivelse, samt blandinger derav, og nevnte smøremiddel er valgt blant magnesiumstearat, stearinsyre, natriumstearylfumarat og natriumlaurylsulfat og blandinger derav.
13. Preparat som angitt i krav 12, hvor den klebende substans er povidon.
14. Preparat ifølge krav 12, hvor nevnte bærersubstrat er mikrokrystallinsk cellulose.
15. Preparat ifølge krav 12, hvor nevnte smuldremiddel er krospovidon.
16. Preprat ifølge krav 12, hvor nevnte smøremiddel er magnesiumstearat
17. Preparat ifølge krav 12, som er et tablettpreparat omfattende: 0,01 % entecavir, 93,24 % mikrokrystallinsk cellulose 4,0 % krospovidon, 2,50 % povidon og
ca. 0,25 % magnesiumstearat, idet prosentene er på vektbasis; eller 1,0 % entecavir, 90,0 % mannitol, 4,0 % kroskarmellosenatrium, 2,50 % metylcellulose og 2,50 % stearinsyre, idet prosentene er på vektbasis; eller 0,5 % entecavir, 60,0 % laktosemonohydrat, 32,50 % mikrokrystallinsk cellulose, 4,0 % krospovidon, 2,50 % povidon og 0,50 % magnesiumstearat, idet prosentene er på vektbasis; eller 0,1 % entecavir, 60,0 % laktosemonohydrat, 35,39 % mikrokrystallinsk cellulose, 4,0 % krospovidon, 0,01 % povidon og 0,5 % magnesiumstearat, idet prosentene er på vektbasis.
18. Preparat ifølge krav 12, som har et ytre filmbelegg.
19. Preparat ifølge krav 12, som er et kapselpreparat som omfatter: 10,0 % entecavir, 82,03 % mikrokrystallinsk cellulose, 4,00 % krospovidon, 2,50 % povidon, 0,25 % magnesiumstearat og 1,22 % saltsyre, idet prosentene er på vektbasis; eller 0,05 % entecavir, 93,20 % dikalsiumfosfat, 4,00 % krospovidon, 2,50 % hydroksypropylcellulose og 0,25 % magnesiumstearat, idet prosentene er på vektbasis.
20. Fremgangsmåte for fremstilling av et farmasøytisk preparat som angitt i krav 1 som omfatter: (a) å oppløse nevnte entecarvir og en klebende substans i et løsningsmiddel, nevnte løsningsmiddel er vann eller vann som har en sur eller basisk pH, (b) å sprøyte nevnte løsning fra trinn (a) på et bærersubstrat mens nevnte bærersubstrat er i bevegelse for å feste nevnte entecavir til overflaten av bærersubstratet, (c) å tørke det belagte bærersubstrat fra trinn (b) for å fjerne løsningsmidlet, og (d) å kombinere det tørkede belagte bærersubstrat fra trinn (c) med andre ønskede ingredienser for å danne det farmasøytiske preparat.
21. Fremgangsmåte i henhold til krav 20, hvor nevnte bærersubstratet holdes i bevegelse under sprøytetrinn (b) ved mekanisk agitering, og det belagte bærersubstrat tørkes i trinn (c) i en brett-tørker eller fluidsjikttørker; eller bærersubstratet holdes i bevegelse under sprøytetrinn (b) ved luftstrømagitering, og det belagte bærersubstrat tørkes i trinn (c) også ved hjelp av luftstrømagitering.
