【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明はリドカイン含有外用貼付
剤、さらに詳しくは薬効成分としてリドカインまたはそ
の塩を水溶性高分子に分散または溶解して含有し、該薬
効成分を安定的にかつ長時間にわたって経皮吸収させる
ことができる外用貼付剤に関する。
【０００２】
【従来技術と解決すべき課題】近年、老人に比較的多く
発症する帯状疱疹神経痛及び帯状疱疹後神経痛に対して
神経ブロック療法、針治療、イオントフォレーシス療法
、中枢性鎮痛薬または抗うつ薬投与などが行われている
。神経ブロック療法の薬物として、代表的なものにリド
カインがある。リドカインは局所麻酔剤として開発され
、表面・浸潤・伝達麻酔作用を有し、主に歯科領域にお
ける表面麻酔等に使用されている。また、期外収縮、急
性心筋梗塞や心臓外科手術に伴う心室性不整脈治療の第
一選択薬として広く用いられている薬物である。
【０００３】このリドカインを帯状疱疹神経痛または帯
状疱疹後神経痛に適用するに際しては、例えば神経ブロ
ック療法の場合、ブロック針を用いて硬膜外腔への薬液
注入等が考えられるが、このような方法では下記の如き
問題がある。 （１）　　薬物は速やかに患部組織に浸透されるが、血
圧、心拍数及び全身状態をモニターしながら治療する必
要がある。 （２）　　高齢者、心疾患及び肝疾患を有している患者
について全身的に副作用が発現する可能性が高くなる。 （３）　　自宅で治療することが不可能なため、治療時
ごとに通院もしくは入院しなければならない。 （４）　　一回の治療に長い時間を要する。
【０００４】また、経皮吸収を目的として治療する手段
として、軟膏を塗布することも考えられる。この方法で
は外用貼付剤では貼付しにくい部位（例えば頭部、顔面
等）に塗布できる利点もあるが、その反面、下記の如き
問題がある。 （１）　　薬物の定量的投与性に欠け、また手、指、衣
類などを汚すことが少なくない。これを防止するために
、塗布部をカーゼなどで覆っておく方法もあるが煩雑と
なる。 （２）　　製剤中の溶媒が揮散する結果、薬物の結晶が
析出し経皮吸収性が低下する。これを防止するために、
適当なフィルムなどで覆う方法もあるが、この場合、皮
膚角質層の水分が過剰となり、湿疹、蕁麻疹などの皮膚
アレルギー症状が現われることが予測される。 （３）　　帯状疱疹神経痛または帯状疱疹後神経痛の治
療において、疾患部位に常に薬物が供給されていること
が望まれるが、リドカインは代謝が早いため軟膏では１
日数回の投与が必要である。
【０００５】また、最近注目を浴びているイオントフォ
レーシス療法についても、無痛的局所療法として利点も
多々あるがなお下記の如き問題点がある。 （１）　　特殊な機械類を使用するため、治療時ごとに
通院しなければならない。また、治療時において手間と
時間を要する。 （２）　　一回の治療範囲の大きさが限定され、また、
個人差により通電条件を変えなければいけない。 （３）　　ヒト皮膚に対して通電させるため、稀に熱傷
など二次的な副作用が起こる可能性がある。
【０００６】そこで、本発明者らは、経皮投与に適する
外用貼付剤としてリドカインを適用することを検討すべ
く鋭意研究を行なった結果、リドカインを水溶性高分子
に分散または溶解させることにより、粘着性の優れた、
また、薬物の優れた放出制御機能を発揮し、かつ、薬物
を長時間にわたって経皮吸収させることができ、これに
より経皮吸収型の外用貼付剤が得られることを見出し、
本発明を完成させるに至った。
【０００７】
【課題を解決するための手段】本発明は、水溶性高分子
物質、水及び保水剤を必須成分とする粘着性ゲル基剤に
リドカインまたはその塩を１〜１０％（重量％、以下同
様）配合してなる薬物保持層を支持体上に設けたことを
特徴とするリドカイン含有外用貼付剤を提供するもので
ある。
【０００８】本発明で用いる粘着性ゲル基剤は、水溶性
高分子物質、水及び保水剤を必須成分とする。上記水溶
性高分子物質としては、ゼラチン、デンプン、寒天、マ
ンナン、アルギン酸、ポリアクリル酸、ポリアクリル酸
塩、デキストリン、メチルセルロース、メチルセルロー
スナトリウム、カルボキシメチルセルロース、カルボキ
シメチルセルロースナトリウム、ポリビニルアルコール
、ポリビニルピロリドン、メチルビニルエーテル−無水
マレイン酸共重合体、アラビアガム、トラガントガム、
カラヤガム、ローカストビーンガムなどが挙げられ、ま
た、これらの金属塩、並びに有機、無機の架橋剤により
架橋されたものも用いることができる。これらの水溶性
高分子物質は、粘着性ゲル基剤中に使用される他の原料
の物性及び所望の物性を出現させるために用いられる。 実際に使用するに当たっては、一種または二種以上の組
み合わせにより用いられ、粘着性ゲル基剤中の配合量と
しては０．５〜５０％（重量％、以下同じ）が好ましく
、より好ましくは５〜２５％の範囲で選定すれば良い。
【０００９】上記粘着性ゲル基剤中の水は、皮膚角質層
の膨潤及び薬物の透過性を向上させるもので、その水分
量は１０〜７０％が好ましく、より好ましくは２０〜５
０％の範囲で選定すればよい。
【００１０】上記保水剤は、粘着性ゲル基剤中の水分量
を一定に保持せしめ、目的貼付剤の保存中あるいは使用
中の水分揮散による皮膚への薬物放出率に対する悪影響
を抑えるもので、例えばエチレングリコール、ジエチレ
ングリコール、ポリエチレングリコール、グリセリン、
ソルビトール、マルチトール、プロピレングリコール、
１，３−ブチレングリコールなどのグリコール類、糖類
などが挙げられ、これらの一種または二種以上組み合わ
せて用いてよい。粘着性ゲル基剤中の配合量としては１
〜７０％が好ましく、より好ましくは１０〜６０％の範
囲で選定すれば良い。
【００１１】なお、保水性を一層向上させるために、超
高吸水性高分子物質を配合することができる。具体例と
してはデンプン−アクリロニトリルグラフト体、デンプ
ン−アクリル酸グラフト体、デンプン−スチレンスルホ
ン酸グラフト体、デンプン−ビニルスルホン酸グラフト
体、ポリビニルアルコール架橋体、アクリル酸−酢酸ビ
ニルケン化物、ポリエチレングリコールジアクリレート
架橋物等が挙げられる。これらの超高吸水性高分子物質
の配合量は、０〜２０％が好ましく、より好ましくは０
．０１〜１０％の範囲で選定すればよい。
【００１２】さらに必要に応じて通常の吸収助剤（例え
ばサリチル酸、ヒアルロン酸、オレイン酸、Ｎ，Ｎ−ジ
エチル−ｍ−トルアミド、ステアリン酸ｎ−ブチル、ベ
ンジルアルコール、ミリスチン酸イソプロピル、パルミ
チン酸イソプロピル、ポリプロピレングリコール、クロ
タミトン、ジエチルセバケート、Ｎ−メチルピロリドン
、Ｎ−エチルピロリドン、ラウリルアルコール等）及び
該吸収助剤をゲル基剤中の水に乳化させるための界面活
性剤（例えばポリオキシエチレンソルビタンモノオレー
ト、ポリオキシエチレンソルビタンモノステアレート、
ソルビタンモノオレート、ソルビタンモノパルミテート
等）が使用できる。その他、防腐剤、酸化防止剤等を適
量配合してもよい。なお、酸化防止剤や防腐剤を用いる
場合、薬物の放出制御機能や皮膚刺激の影響に留意すべ
きである。
【００１３】かかる成分からなる粘着性ゲル基剤のｐＨ
は皮膚刺激性及び薬物の安定性を考慮してｐＨ５〜９に
調整するのが好ましく、この場合アルカリ性物質（例え
ば水酸化ナトリウムまたはトリエタノールアミン、ジイ
ソプロパノールアミンなどのアミン類）などでｐＨ調整
してもよい。
【００１４】本発明にかかるリドカイン含有外用貼付剤
は、上述の粘着性ゲル基剤に有効量の薬物すなわちリド
カインまたはその塩（塩酸リドカインなど）を配合して
、これを適当な支持体に塗工して薬物保持層（薬物含有
量０．５〜１５．０％、好ましくは２．５〜１０．０％
）を含有することにより得られる。該薬物含有基剤の塗
布量は、通常、５００〜２０００ｇ／ｍ２の範囲である
。
【００１５】必要に応じ、薬物保持層の水分揮発を防止
して該層を保護する目的で、その表面に適当な材質のラ
イナーを添着してもよい。上記支持体としては、人体の
動作にある程度追随し易い柔軟なものが好ましく、例え
ば各種の不織布、織布、スパンデックス、ネル、または
これらにポリエチレンフィルム、ポリエチレンテレフタ
レートフィルム、ポリ塩化ビニルフィルム、エチレンビ
ニルアセテートフィルム、ポリウレタンフィルム等をラ
ミネート加工したものが挙げられる。

【発明の効果】本発明の貼付剤によれば薬物を定量的に
放出させることができ、また使用方法が簡便で、かつ長
時間にわたる貼付が可能であり、さらに具体的には以下
の利点が奏せられる。 （１）　　肝臓における初回通過効果を回避することが
でき、生物学的利用率を高くすることができる。 （２）　　貼付剤化により薬物の長時間の持続放出がで
き常に安定な薬効を期待できる。また投与回数を少なく
することができ、患者のコンプライアンスの改善につな
がる。 （３）　　帯状疱疹後神経痛は時として風、衣服の摩擦
によっても痛みを生じる場合があり、この場合、皮膚の
表面を貼付剤で覆うことにより痛みを軽減できる。 （４）　　患者の年齢、健康状態、胃腸等の消化器官あ
るいは肝機能の状態、食物の影響に関係なく投与が可能
である。

【実施例】次に実施例及び比較例を挙げて本発明をより
具体的に説明するが、本発明はこれらに限定されるもの
ではない。なお、実施例または比較例中「部」は特に断
わらない限り「重量部」を意味する。

え混合する。更にプロピレングリコール、リドカインを
混合溶解した液を加えて混合する。次いでグリセリンに
ポリアクリル酸ナトリウム、カルボキシメチルセルロー
スナトリウム、水酸化アルミニウム、パラオキシ安息香
酸、パラオキシ安息香酸プロピルを分散させた液を加え
て均一になるまで十分に混合する。得られた基剤を不織
布上に１０００ｇ／ｍ２で展延し、シリコン加工したポ
リエチレンテレフタレート製のライナーを添着し、所望
の大きさに裁断する。これによりｐＨ７でリドカインを
５ｍｇ／ｃｍ２含有するリドカイン含有外用貼付剤を得
る。

ロースナトリウム、水酸化アルミニウム、パラオキシ安
息香酸メチル、パラオキシ安息香酸プロピルを分散させ
た液を加えて均一になるまで十分に混合する。得られた
基剤を不織布上に１０００ｇ／ｍ２で展延し、シリコン
加工したポリエチレンテレフタレート製のライナーを添
着し、所望の大きさに裁断する。これによりｐＨ６でリ
ドカインを５ｍｇ／ｃｍ２含有するリドカイン含有外用
貼付剤を得る。

ラオキシ安息香酸メチル、パラオキシ安息香酸プロピル
を加え、室温にて固まるまでかき混ぜ製する。これによ
りｐＨ７でリドカインを５０ｍｇ／ｇ含有するリドカイ
ン含有軟膏を得る。

００部ＳＩＳ以下の各成分を加熱下において溶解し、プ
ロピレングリコールに溶解させたリドカイン溶液を加え
混合する。次に、適当な不織布上に５００ｇ／ｍ２にな
るように展延し、冷却した。この基剤にポリエチレンテ
レフタレートフィルムをラミネートし、所望の大きさに
切断した。これによりリドカインを５ｍｇ／ｃｍ２含有
するリドカイン含有外用貼付剤を得る。
【００２４】試験例１ 　　フランツ拡散セルを用いてラット腹部摘出皮膚を透
過するリドカインの量をＨＰＬＣにより定量した。実施
例１〜３、比較例２、３について直径１．７ｃｍの円形
（リドカイン１１．３５ｍｇ、または２２．７ｍｇ含有
）に打ち抜き、拡散セルのラット皮膚上に貼付した。レ
セプター側にｐＨ６．８−リン酸緩衝液を用い、経時的
にサンプルを採取し、薬物透過量を測定した。なお、比
較例１については直径１．７ｃｍの円内に塗布し、以下
同様に試験を行なった。 （結果）結果を図１に示す。（ｎ＝３）図１から明らか
なように、実施例１〜３及び比較例１について薬物は経
時的にラット皮膚を透過し、その透過性は比較例２、３
に比べて優れていた。
【００２５】試験例２ 　　ラットの背部を除毛した後、これに実施例１〜３、
比較例２、３について４ｃｍ×４ｃｍ（リドカイン８０
．０ｍｇまたは１６０．０ｍｇ含有）の大きさに裁断し
たものを貼付した。この状態で経時的に血液を０．５ｍ
ｌずつ採取し、蛍光偏光イムノアッセイ法にて血清中の
リドカイン量を測定した。なお、比較例１については４
ｃｍ×４ｃｍの大きさのガーゼに１．６ｇ（リドカイン
８０．０ｍｇ含有）塗布し、以下同様に試験を行なった
。 （結果）結果を図２に示す。（ｎ＝４）図２から明らか
なように、実施例１〜３について薬剤投与後６〜８時間
で血中濃度は一定状態に達した。以後２４時間まで持続
し、比較例に比べて高い血中濃度を示した。また、薬剤
除去後は速やかに消失した。比較例１については薬剤投
与後速やかに血中濃度は上昇するが、持続性がなく、薬
剤投与後４〜５時間でピークとなり、以後減少した。比
較例２、３については経皮吸収性が悪く、血中濃度は低
かった。
【００２６】試験例３ 　　モルモットの背部を除毛した後、マンドリン線で一
定強度の刺激を与え、表皮に正常な収縮反応の起こるこ
とを確認後、実施例１〜３、比較例２、３について４ｃ
ｍ×４ｃｍ（リドカイン８０．０ｍｇまたは１６０．０
ｍｇ含有）の大きさに裁断したものを貼付した。貼付後
経時的に薬剤を除去し貼付部位をマンドリン線で６回刺
激し、３回以上皮膚収縮反応のなかったものを局所麻酔
作用陽性とした。なお、比較例１については４ｃｍ×４
ｃｍの大きさのガーゼに１．６ｇ（リドカイン８０．０
ｍｇ含有）塗布し、以下同様に試験を行なった。 （結果）結果を表１に示す（ｎ＝５） 表１から明らかなように、実施例１〜３について薬剤投
与後４時間で効果が認められ、以後薬剤を除去するまで
持続し、高い効果率が得られた。また、薬剤除去後速や
かにその効果は消失した。比較例１について薬剤投与後
４〜８時間で高い効果率が得られたものの、持続性がな
かった。比較例２については薬剤投与後４時間で効果が
認められたものの、低い効果率であり、また比較例３に
ついては全く効果が認められなかった。

Claims (3)

    【特許請求の範囲】
  1. 【請求項１】　　水溶性高分子物質、水及び保水剤を必
    須成分とする粘着性ゲル基剤に、リドカインまたはその
    塩を１〜１０重量％配合してなる薬物保持層を支持体上
    に設けたことを特徴とするリドカイン含有外用貼付剤。
  2. 【請求項２】　　粘着性ゲル基剤が薬物の放出制御機能
    を有し、そのｐＨ値が５〜９である請求項第１項記載の
    外用貼付剤。
  3. 【請求項３】　　粘着性ゲル基剤中の水分量が２０〜７
    ０重量％である請求項第１項または第２項記載の外用貼
    付剤。
