Description
本発明は、概して、(a)TGFβRII、又はTGFβ結合能を有するその断片と、(b)プログラム死リガンド1(PD−L1)などの免疫チェックポイントタンパク質に結合する抗体、又はその抗原結合断片とを含む二機能性分子、かかる分子の(例えば癌を治療するための)使用、及びかかる分子の作製方法に関する。
癌治療では、長年、化学療法が高い毒性を伴い、抵抗性癌細胞変異体の出現につながり得ることが認識されてきた。腫瘍の生存及び成長に重要な過剰発現し又は活性化したオンコプロテインに対する標的療法であっても、癌細胞は常に変異し、余剰の経路を利用するなどして標的化された経路への依存を減らすように適合する。癌免疫療法は、癌細胞を標的化することに代えて、免疫系の活性化に重点を置く癌治療の新しいパラダイムである。その原理は、宿主の免疫応答、特に適応T細胞応答を備え直すことにより、癌細胞、詳細には他の形態の治療を逃れた最小の残存疾患を死滅させるための免疫監視機構を提供し、ひいては持続性の防御免疫を実現するというものである。
本発明は、TGFβ結合能を有するTGFβ受容体II(TGFβRII)の少なくとも一部分とヒトタンパク質プログラム死リガンド1(PD−L1)などの免疫チェックポイントタンパク質に結合する抗体又は抗原結合断片とを含有する二機能性タンパク質が、有効な抗腫瘍及び抗癌治療薬であり得るという発見に基づく。このタンパク質は、これらの2つの薬剤を別々に投与する効果と比較したとき、癌治療において相乗効果を呈し得る。
本発明は、特定の腫瘍細胞又は免疫細胞の外表面上に存在する細胞性免疫チェックポイント受容体を標的化する抗体部分に係留された可溶性サイトカイン受容体(TGFβRII)を使用してTGFβを捕捉することにより、腫瘍微小環境におけるTGFβの局所的な低減を可能にする。免疫チェックポイントタンパク質に対する本発明の抗体部分の例は、抗PD−L1である。この二機能性分子(本明細書では「抗体−サイトカイントラップ」と称されることもある)は、抗受容体抗体とサイトカイントラップとが物理的に結び付いているため正確に有効となる。この得られる利点(例えば抗体と受容体とを別個の分子として投与するのと比べたときの)は、一部には、サイトカインがオートクリン及びパラクリン機能によって主に局所環境で機能することが理由である。抗体部分がサイトカイントラップを腫瘍微小環境に仕向け、そこでサイトカイントラップは局所的免疫抑制オートクリン又はパラクリン効果を中和することによって最も有効となり得る。さらに、抗体の標的が抗体結合時にインターナライズされる場合には、サイトカイン/サイトカイン受容体複合体の有効なクリアランス機構がもたらされる。PD−L1について、抗体介在性の標的インターナリゼーションが示されている。これは、抗TGFβ抗体の使用と比べて特徴的な利点であり、なぜなら、第一に、抗TGFβ抗体は完全には中和しない可能性があり;及び第二に、この抗体はサイトカインの半減期を延長させる担体として機能することもあり、且つ多くの場合に抗体/サイトカイン複合体は、蓄積して最終的に解離することによりサイトカインを循環中に放出して戻す循環シンクとして機能するためである(Montero-Julian et al., Blood. 1995; 85:917-24)。サイトカイントラップを使用したリガンドの中和はまた、CSF−1の場合のような、抗体による受容体の遮断と比べても優れた戦略であり得る。CSF−1は受容体介在性エンドサイトーシスによって循環から取り除かれるため、抗CSF−1受容体抗体の遮断は循環CSF−1濃度の大幅な上昇を引き起こした(Hume et al., Blood. 2012;119:1810-20)。
TGFβは、二重人格的な癌分子としてのその逆説的な役割に起因して、癌免疫療法における幾らか問題のある標的であった(Bierie et al., Nat Rev Cancer. 2006; 6:506-20)。他の幾つかのサイトカインと同様に、TGFβ活性は開発段階であり、コンテクスト依存的である。実際、TGFβは、腫瘍プロモーター又は腫瘍サプレッサーのいずれとしても機能することができ、腫瘍発生、進行及び転移に影響を及ぼし得る。このTGFβの二重の役割の根底にある機構は依然として不明のままである(Yang et al., Trends Immunol. 2010; 31:220-227)。Smad依存的シグナル伝達がTGFβシグナル伝達の成長阻害を媒介する一方、Smad非依存的経路がその腫瘍促進効果に寄与することが仮定されているが、Smad依存的経路が腫瘍進行に関わることを示すデータもある(Yang et al., Cancer Res. 2008; 68:9107-11)。
治療用抗体によって脱阻害するためT細胞阻害チェックポイントを標的化する手法は、精力的に研究されている領域である(レビューについては、Pardoll, Nat Rev Cancer. 2012; 12:253-264を参照)。1つの手法では、抗体部分又はその抗原結合断片がT細胞上のT細胞阻害チェックポイント受容体タンパク質、例えば:CTLA−4、PD−1、BTLA、LAG−3、TIM−3、及びLAIR1などを標的化する。別の手法では、抗体部分が抗原提示細胞及び腫瘍細胞上の対抗受容体(これらの細胞は自身の免疫回避のためこれらの対抗受容体の幾つかを取り入れる)、例えば:PD−L1(B7−H1)、B7−DC、HVEM、TIM−4、B7−H3、又はB7−H4などを標的化する。

抗体−サイトカイントラップタンパク質は、概して、そのタンパク質を発現するように操作された核酸を含む哺乳類細胞を使用して組換えで作製される。好適な細胞株及びタンパク質作製方法の一例を実施例1及び2に記載しているが、多種多様な好適なベクター、細胞株及びタンパク質作製方法を用いて抗体ベースのバイオ医薬品が作製されており、これらの抗体−サイトカイントラップタンパク質の合成に使用することができる。
本願に記載される抗PD−L1/TGFβ Trapタンパク質は、患者における癌の治療又は腫瘍成長の低減に使用することができる。例示的癌としては、結腸直腸、乳房、卵巣、膵臓、胃、前立腺、腎臓、子宮頸部の癌、骨髄腫、リンパ腫、白血病、甲状腺、子宮内膜、子宮、膀胱、神経内分泌、頭頸部、肝臓、上咽頭、精巣の癌、小細胞肺癌、非小細胞肺癌、黒色腫、基底細胞皮膚癌、扁平上皮細胞皮膚癌、隆起性皮膚線維肉腫、メルケル細胞癌、膠芽腫、神経膠腫、肉腫、中皮腫、及び骨髄異形成症候群が挙げられる。
本発明はまた、本明細書に記載されるタンパク質の治療有効量を含有する医薬組成物も特徴とする。本組成物は、種々の薬物送達システムにおける使用向けに製剤化することができる。本組成物には、適切な製剤化のため1つ以上の生理学的に許容可能な賦形剤又は担体もまた含まれ得る。本発明における使用に好適な製剤化は、Remington’s Pharmaceutical Sciences, Mack Publishing Company, Philadelphia, Pa., 17th ed., 1985に掲載されている。薬物送達方法の簡単なレビューについては、例えば、Langer (Science 249:1527-1533, 1990)を参照のこと。
本発明は、ここで概略的に説明されているが、以下の例を参照することによりさらに容易に理解されるであろう。以下の例は単に本発明の特定の態様及び実施形態を例示するために含まれるものであり、いかなる方法であれ本発明の範囲を限定することは意図されない。
抗PD−L1/TGFβ Trapは、抗PD−L1抗体−TGFβ受容体II融合タンパク質である。この分子の軽鎖は抗PD−L1抗体の軽鎖(配列番号1)と同一である。この分子の重鎖(配列番号3)は、可動性(Gly4Ser)4Glyリンカー(配列番号11)を介して可溶性TGFβ受容体II(配列番号10)のN末端に遺伝子融合した抗PD−L1抗体の重鎖(配列番号2)を含む融合タンパク質である。融合接合部において、抗体重鎖のC末端リジン残基をアラニンに突然変異させて、タンパク質分解切断を低減した。抗PD−L1/TGFβ Trapの発現のため、同じ発現ベクター又は別個の発現ベクターのいずれかにおける抗PD−L1軽鎖をコードするDNA(配列番号4)及び抗PD−L1/TGFβ受容体IIをコードするDNA(配列番号5)を使用して、一過性の又は安定的なトランスフェクションの標準プロトコルを用いて哺乳類細胞をトランスフェクトした。馴化培養培地を回収し、抗PD−L1/TGFβ Trap融合タンパク質を標準プロテインAセファロースクロマトグラフィーによって精製した。1つの抗PD−L1抗体と2つの可溶性TGFβ受容体II分子とを含むこの精製タンパク質(図1A)は、サイズ排除クロマトグラフィー及び非還元条件下におけるSDS−ポリアクリルアミド電気泳動で約190キロダルトンの推定分子量(MW)を有する。還元条件下では、軽鎖及び重鎖はそれぞれ28及び75キロダルトンの見かけのMWを有する(図1B)。
ヒト胎児腎臓293(HEK)細胞の一過性のトランスフェクションによって作製した抗PD−L1/TGFβ Trapは、様々な程度のクリッピング種を含有することが分かり、これらの種は還元条件下のSDS−PAGEで見かけのMWが約60kDの薄いバンドとして現れた(図1B)。このバンドは、融合接合部に近いTGFβRIIのN末端部分内のある部位で切断された抗PD−L1/TGFβ Trapの重鎖であると確認された。
ヒトPD−L1を発現するように安定にトランスフェクトしたHEK細胞に対する抗PD−L1抗体及び融合タンパク質の結合を、以下の手順を用いて試験した。
a.試験試料の50μlの段階希釈液をFACS緩衝液にセットアップした。
b.ヒトPD−L1を発現するように安定にトランスフェクトした50μlのHEK細胞を5×106細胞/mlで試験試料が入ったウェルに分注し、混合した。
c.1つ又は複数のプレートを暗所において氷上で1時間インキュベートした。
d.細胞を300×gで5分間ペレット化した。
e.上清をデカントし、細胞を300μlのFACS緩衝液に再懸濁し、再び300×gで5間ペレット化した。
f.試料のリンスを繰り返した。
g.細胞を、DyLight 488コンジュゲート全IgGヤギ抗ヒトIgG、Fcγ(1:300希釈)を含有する100μlのFACS緩衝液に再懸濁した。
h.1つ又は複数のプレートを暗所において氷上で45分間インキュベートした。
i.細胞を300×gで5間ペレット化した。
j.上清をデカントし、細胞を300μlのFACS緩衝液に再懸濁し、再び300×gで5分間ペレット化した。
k.試料のリンスを繰り返し、細胞を最終的に200μlのFACS緩衝液に再懸濁した。
l.FACS Caliberでデータを取得し、Microsoft Excelを使用して分析した。Graphpad Prism5で非線形回帰(シグモイド用量反応)を使用してEC50を計算した。
抗PD−L1/TGFβ TrapがTGFβを中和する能力を、SMAD3−ルシフェラーゼレポーターを有する4T1細胞を使用して決定した。以下に詳述するこのアッセイでは、SMAD3プロモーターの制御下にあるルシフェラーゼレポーターを使用してTGFβ誘導性SMAD3リン酸化の阻害を計測した。
1.試験前日、SMAD3−ルシフェラーゼレポーターを有する4T1細胞を供給した。
2.0日目、Biocoatの96ウェルプレートに100μlの新鮮培地中5×104細胞/ウェルの濃度で細胞をプレーティングし、37℃及び5%CO2で一晩インキュベートした。
3.1日目:
i.指示濃度の抗PD−L1/TGFβトラップ被験試料又はその対照が入った50μlの新鮮完全培地をウェルに加え、1時間インキュベートした。試料は全て、トリプリケートで試験した。
ii.20ng/mlヒトTGFβが入った50μlの新鮮完全培地を各ウェルに加え、試料を一晩インキュベートした(ウェルの最終濃度は5ng/mlである)。
4.2日目:
i.100μ1の培養上清を取り出し、150μg/ml D−ルシフェリンを含有する100μlの新鮮完全培地を加え、試料を少なくとも5分間インキュベートした。
ii.Envision 2104プレートリーダーを使用してCPMを記録することにより、ルミネセンスを計測した。
5.MS Excel又はGraphpad prism 5を使用してデータを分析した。ルシフェラーゼ活性はCPMとして記録した。阻害活性(%)を、以下の式を用いて計算した:

6.Graphpad prism 5のシグモイド用量反応(傾き可変)を使用して非線形回帰フィッティングを行った。IC50値を計算した。
18匹の雄C57BL/6マウス、5〜6週齢を3群(N=6匹/群)に無作為に割り当て、各群に3つのタンパク質(抗PD−L1/TGFβ Trap、抗PD−L1(mut)/TGFβ Trap、及び抗PD−L1)のうちの1つを投与した。投与前にマウス体重を記録した。加熱灯の下で短時間温めた後、各マウスに、その体重に関わらず200μl中120μgのタンパク質を尾静脈から静脈内(IV)投与した。同じタンパク質を投与した各群を2つのサブ群(n=3匹)にさらに分割した。血液試料を2つのサブ群の各々から交互に採取し、即ち1時間、24時間、72時間、及び168時間の時点で血液試料のため1つのサブ群を抜き取り、一方、7時間、48時間、120時間、及び240時間の時点で血液試料のためもう1つのサブ群を抜き取った。各時点につき、ヘパリン処理マイクロガラス毛細管(容量100μl)を使用して各マウスの尾静脈から約50μlの血液試料を採取した。次にヘパリンLiでプレコーティングされたチューブに血液試料を移し、4℃で保管した。採取から10分以内に血液試料を14,000rpmで10分間スピンした。新しい一組の予め表示を付したチューブに少なくとも20μlの血漿試料を移し、分析日まで−20℃で保存した。
受容体介在性エンドサイトーシスについて、Alexa Fluor 488クエンチ技法を用いて製造者のプロトコル(Life Technologies、Carlsbad、CA)に従い試験した。簡潔に言えば、PD−L1を発現するHEK細胞(HEK/PD−L1細胞)を10μg/mlのAlexa Fluor 488コンジュゲート抗PD−L1/TGFβ Trapと共に氷上で約1時間インキュベートし、冷培地で4回洗浄した。次に洗浄した細胞を37℃で0.25、0.5、0.75、1、1.5、2、3及び4時間パルスしてインターナライズさせた。次に各時点の細胞試料を2つの部分に分割した。一方の部分は氷上でインキュベートし、細胞表面に結合し及びインターナライズしたAlexa Fluor 488コンジュゲート抗PD−L1/TGFβ Trapからの全蛍光を計測した;他方の部分は抗Alexa Fluor 488と共に4℃で約1時間インキュベートし、インターナライズしたAlexa Fluor 488コンジュゲート抗PD−L1/TGFβ Trapからのクエンチ不可能な蛍光を計測した。37℃における抗PD−L1/TGFβ Trapのクエンチ不可能な蛍光強度平均値(MFI)及び全MFIの時間経過を示すグラフを図6Aに示す。受容体介在性インターナリゼーション動態は抗PD−L1抗体(図6Bに示す)と極めて類似している。抗Alexa Fluor 488によるクエンチが100%でないことを説明する以下の式を使用した、37℃で種々の時点におけるHEK/PD−L1細胞に対する抗PD−L1/TGFβ Trap及び抗PD−L1の受容体介在性インターナリゼーション率を図6Cに示す。

等モル用量で、抗PDL1/TGFβ Trapは同所性EMT−6乳癌モデルにおいて抗PD−L1とTGFβ Trap対照との併用と同程度の効力を有した(実施例9)。以下の試験では、均等な曝露となるように投与した抗PDL1/TGFβ Trap又は抗PD−L1とTGFβ Trap対照との併用の効力を調べた。
実施例10の結果から、抗PD−L1(mut)/TGFβ Trap対照の生体内曝露が抗PD−L1/TGFβ Trapの生体内曝露の約3倍であるにも関わらず(実施例5)、抗PD−L1とTGFβ Trap対照との併用と比べて等モル用量の抗PD−L1/TGF−β Trapがより優れた抗腫瘍効力を有することが示唆された。フォローアップ試験において、等しい曝露に基づく抗PD−L1/TGFβ Trap及び抗PD−L1と抗PD−L1(mut)/TGFβ Trapとの併用の抗腫瘍効力を比較した。実施例10より低い用量を投与して近飽和レベルの投与を回避した。
YW243.55S70は、ヒト及びマウスの両方のPD−L1を認識するヒト抗体である(米国特許出願公開第2010/0203056 Al号)。標準的な分子生物学的技法によってその重鎖の可変領域配列(VH)及び軽鎖の可変領域配列(VL)(それぞれ配列番号14及び配列番号13として提供される)を使用して実施例1に記載する抗PD−L1/TGFβ Trapの対応する可変領域配列を置換し、抗PD−L1(YW)/TGFβ Trapを得た。抗PD−L1(YW)/TGFβ TrapをコードするDNAの構築後、この抗体融合タンパク質を実施例1に記載するとおり発現させた。マウス腫瘍モデルにおける効力の比較のため、抗PD−L1抗体YW243.55S70も同様に発現させる。

この試験では、本発明者らは、抗PD−1とTGFβ Trapとの併用治療がEMT−6同所性モデルにおいて何らかの相加的抗腫瘍効果をもたらすかどうかを調べた。ピジルズマブ(pidiluzumab)としても知られるCT−011は、血液悪性腫瘍の治療に関して臨床で試験されたヒト化抗ヒトPD1抗体である(Berger et al, Clin Cancer Res. 2008; 14:3044-3051)。これは海洋生物PD−1も認識し、同系腫瘍モデルにおいてシクロホスファミド及びワクチン治療と相乗作用する抗腫瘍活性を示している(Mkrtichyan et al., Eur J Immunol. 2011; 41:2977-86)。CT−011のVH及びVL配列を使用して、標準的な分子生物学的技法によりヒトIgG1/κ定常領域を含む組換え抗体を作製した。

Claims (12)
- タンパク質であって、
a)少なくとも、ヒトタンパク質プログラム死リガンド1(PD−L1)に結合する抗体の重鎖の可変領域と;
b)ヒトトランスフォーミング増殖因子β受容体II(TGFβRII)、又はTGFβ結合能を有するその断片と
を含む第1のポリペプチドと、
少なくとも、PD−L1に結合する抗体の軽鎖の可変領域と
を含む第2のポリペプチドを含み、
前記第1のポリペプチドが配列番号3のアミノ酸配列を含み、
前記第2のポリペプチドが配列番号1のアミノ酸配列を含み、
前記第1のポリペプチドの重鎖と第2のポリペプチドの軽鎖が、組み合わされてPD−L1に結合する抗原結合部位を形成する、タンパク質。 - 2つのポリペプチドであって、各々が配列番号3のアミノ酸配列からなるアミノ酸配列を有する、2つのポリペプチドと、2つのポリペプチドであって、各々が配列番号1のアミノ酸配列からなるアミノ酸配列を有する、2つのポリペプチドとを含む、請求項1に記載のタンパク質。
- 請求項1に記載の第1のポリペプチドをコードする第1のヌクレオチド配列と、請求項1に記載の第2のポリペプチドをコードする第2のヌクレオチド配列とを含む、核酸。
- 請求項3に記載の核酸を含む単離された細胞。
- a)TGFβRII、又はTGFβ結合能を有するその断片と、b)ヒトタンパク質プログラム死リガンド1(PD−L1)に結合する抗体、又はその抗原結合断片とを含むタンパク質を作製する方法であって、前記タンパク質の発現を許容する条件下で、請求項4に記載の細胞を維持するステップと、を含む方法。
- さらに前記タンパク質を回収するステップを含む、請求項5に記載の方法。
- 治療に使用するための医薬の製造における、請求項1又は2に記載のタンパク質の使用。
- 請求項1又は2に記載のタンパク質の治療有効量を含む医薬組成物。
- 治療に使用するための医薬の製造における、請求項1又は2に記載のタンパク質の使用であって、前記医薬が
(i)患者の腫瘍成長を阻害するためか、あるいは、
(ii)患者の癌を治療するために使用される、使用。 - 前記癌患者が放射線に曝露されるか、及び/又は
化学療法薬の投与に曝露されることをさらに含む、請求項9に記載の使用。 - 前記腫瘍又は癌が、結腸直腸、乳房、卵巣、膵臓、胃、前立腺、腎臓、子宮頸部の癌又は腫瘍、骨髄腫、リンパ腫、白血病、甲状腺、子宮内膜、子宮、膀胱、神経内分泌、頭頸部、肝臓、上咽頭、精巣の癌又は腫瘍、小細胞肺癌、非小細胞肺癌、黒色腫、基底細胞皮膚癌、扁平上皮細胞皮膚癌、隆起性皮膚線維肉腫、メルケル細胞癌、膠芽腫、神経膠腫、肉腫、中皮腫、及び骨髄異形成症候群からなる群から選択される、請求項9又は10に記載の使用。
- 前記タンパク質がヒトPD−L1とヒトPD−1受容体との間の相互作用を遮断する、請求項1又は2に記載のタンパク質。
