表面抗原分類70(CD70)は、腫瘍壊死因子スーパーファミリーのメンバーであり、且つその発現は、活性化T及びBリンパ球及び成熟樹状細胞に限定される。CD70は、そのリガンドであるCD27との相互作用を通して、腫瘍細胞及び制御性T細胞の生存に関係づけられる。CD70及びその受容体であるCD27は、T細胞(活性化及びTreg)、及びB細胞を含む複数の細胞型における免疫機能において複数の役割を有する。これらの細胞型の全てにおいてCD70がどのように機能してアポトーシスなどの機能を制御しているのかは正確には不明であり、相反する役割を示す論文がある。例えば、CD70は、抗原負荷に依存してT細胞のアポトーシス又は生存を誘導することが報告されている(Wensveen,F.,et al.J.Immunol,Vol 188:4256−4267,2012)。

いくつかの実施形態では、操作されたT細胞は、破壊されたTRAC遺伝子を含む。この破壊は、TCRの機能喪失をもたらし、且つ操作されたT細胞に同種移植への非アロ反応性及び適合性を与え、移植片対宿主疾患のリスクを最小化する。いくつかの実施形態では、内在性のTRAC遺伝子の発現は、移植片対宿主反応を予防するために除去される。
いくつかの実施形態では、操作されたT細胞は、破壊されたβ2M遺伝子を含む。β2Mは、MHC I複合体の共通の(不変の)構成要素である。遺伝子編集によってその発現を破壊することは、宿主対治療用異質遺伝子型T細胞反応を予防することになり、異質遺伝子型T細胞の持続の増大をもたらす。いくつかの実施形態では、内在性のβ2M遺伝子の発現は、宿主対移植片反応を予防するために除去される。
PD−1は、活性化T細胞において下方制御される免疫チェックポイント分子であり、T細胞の応答を弱めるか又は止める働きをする。遺伝子編集によってPD−1を破壊すれば、より多くの持続的且つ/又は強力な治療的T細胞応答をもたらし、且つ/又は対象における免疫抑制を低減できるであろう。いくつかの実施形態では、操作されたT細胞は、破壊されたPD−1遺伝子を含む。いくつかの実施形態では、内在性のPD−1遺伝子の発現は、本開示のCAR T細胞の抗腫瘍効果を増強するめに除去される。
いくつかの実施形態では、細胞の集団内の1つ以上の遺伝子編集は、細胞の増殖能、細胞の枯渇、細胞の生存能、細胞溶解能(例えば、サイトカイン産生及び/又は放出を増加させる)、又はこれらの任意の組合せにおける変化と関連する表現型をもたらす。
遺伝子編集(ゲノム編集を含む)は、ヌクレオチド/核酸が、標的化された細胞のゲノム中などのDNA配列中に挿入され、欠失され、且つ/又は置換される一種の遺伝子操作である。標的化された遺伝子編集により、標的化された細胞のゲノム中(例えば、標的化された遺伝子又は標的化されたDNA配列中)の予め選択された部位での挿入、欠失、及び/又は置換が可能になる。内在性の遺伝子の配列が、例えば、ヌクレオチド/核酸の欠失、挿入又は置換によって編集されるとき、影響を受けた配列を含む内在性の遺伝子は、配列改変のためにノックアウト又はノックダウンされ得る。したがって、標的化された編集は、内在性の遺伝子発現を破壊するために使用され得る。「標的化された組込み」は、挿入部位での内在性の配列の欠失の有無にかかわらず、1つ以上の外来性の配列の挿入を含むプロセスを指す。標的化された組込みは、外来性の配列を含有するドナー鋳型が存在する場合、標的化された遺伝子編集から生じ得る。本明細書で使用する場合、「破壊された遺伝子」は、内在性の遺伝子と比較して、内在性の遺伝子から機能性タンパク質の発現が減少又は阻害されるような挿入、欠失又は置換を含む遺伝子を指す。本明細書で使用する場合、「遺伝子を破壊すること」は、内在性の遺伝子中で、内在性の遺伝子から機能性タンパク質の発現が減少又は阻害されるような少なくとも1つのヌクレオチド/核酸を挿入するか、欠失させるか又は置換する方法を指す。遺伝子を破壊する方法は、当業者に知られており、本明細書に記載される。
CRISPR−Cas9システムは、遺伝子編集のために使用されるRNA誘導型DNA標的化プラットフォームとして転用されてきた原核生物において天然に存在する防御機構である。それは、DNAの切断を標的化するDNAヌクレアーゼCas9、並びに2つの非コードRNAであるcrisprRNA(crRNA)及びトランス活性化RNA(tracrRNA)に依拠する。CRISPRは、クラスター化して規則的な配置の短い回文配列リピートの略称であり、例えば、原核生物を感染させたか又は攻撃したウイルスによって、細胞に以前に暴露した外来DNAと類似性を有するDNAの断片(スペーサーDNA)を含有する細菌及び古細菌のゲノムにおいて見出されるDNA配列のファミリーである。DNAのこれらの断片が原核生物によって使用されて、例えば、引き続く攻撃の間に同様のウイルスからの再導入時に同様の外来DNAを検出し、且つ破壊する。CRISPR座位の転写により、スペーサー配列を含むRNA分子の形成がもたらされ、これが、外来の外来性DNAを認識し、且つ切断できるCas(CRISPR関連)タンパク質と相互作用し、且つ標的化する。CRISPR/Casシステムのいくつかの種類及びクラスが記載されている(例えば、Koonin et al.,(2017)Curr Opin Microbiol 37:67−78を参照のこと)。
本開示は、会合したポリペプチド(例えば、部位特異的ポリペプチド)の標的核酸内の特定の標的配列に対する活性を誘導できるゲノム標的化核酸を提供する。ゲノム標的化核酸は、RNAであり得る。ゲノム標的化RNAは、本明細書で「ガイドRNA」又は「gRNA」と呼ばれる。ガイドRNAは、目的の標的核酸配列、及びCRISPR反復配列にハイブリダイズする少なくとも1つのスペーサー配列を含む。II型システムにおいて、gRNAはまた、tracrRNA配列と呼ばれる第2のRNAを含む。II型gRNAにおいて、CRISPR反復配列及びtracrRNA配列は、互いにハイブリダイズして、二重鎖を形成する。V型gRNAにおいて、crRNAが、二重鎖を形成する。両方のシステムにおいて、二重鎖は、部位特異的ポリペプチドに結合し、その結果、ガイドRNA及び部位特異的ポリペプチドは、複合体を形成する。いくつかの実施形態では、ゲノム標的化核酸は、その部位特異的ポリペプチドとの会合により、複合体に標的特異性をもたらす。したがって、ゲノム標的化核酸は、部位特異的ポリペプチドの活性を誘導する。
いくつかの実施形態では、gRNAは、スペーサー配列を含む。スペーサー配列は、目的の標的核酸の標的配列(例えば、ゲノム標的配列などのDNA標的配列)を定義する配列(例えば、20個のヌクレオチド配列)である。いくつかの実施形態では、スペーサー配列は、15〜30個のヌクレオチドである。いくつかの実施形態では、スペーサー配列は、15、16、17、18、19、29、21、22、23、24、25、26、27、28、29、又は30個のヌクレオチドである。いくつかの実施形態では、スペーサー配列は、20個のヌクレオチドである。
本開示のgRNAは、インビトロ転写(IVT)、合成及び/又は化学合成法、又はこれらの組合せを含むが、これらに限定されない、当該技術分野において利用可能な好適な手段によって生成される。酵素的(IVT)、固相、液相、複合型の合成法、小領域合成、及びライゲーション法が利用される。一実施形態では、gRNAは、IVT酵素的合成法を使用して作製される。IVTによりポリヌクレオチドを作製する方法は、当該技術分野において知られており、国際出願PCT/US2013/30062号明細書において記載される。したがって、本開示はまた、ポリヌクレオチド、例えば、DNAコンストラクトを含み、ベクターは、本明細書に記載されるgRNAをインビトロ転写するために使用される。
いくつかの実施形態では、gRNA及びRNA誘導型ヌクレアーゼは、同時に又は逐次的に、別々に細胞に送達される。いくつかの実施形態では、gRNA及びRNA誘導型ヌクレアーゼは、細胞に一緒に送達される。いくつかの実施形態では、gRNA及びRNA誘導型ヌクレアーゼは、ともに予め複合体を形成して、リボ核タンパク質(RNP)を形成する。
(i)TRAC標的化RNP;
(ii)β2M標的化RNP;
(iii)CD70標的化RNP;又は
(iv)PD−1標的化RNP。いくつかの実施形態では、以下のRNPの少なくとも2つが、細胞に送達される:

キメラ抗原受容体は、腫瘍細胞によって発現される抗原を認識し、それに結合するように操作された人工の免疫細胞受容体を指す。一般に、CARは、T細胞のために設計され、T細胞受容体(TCR)複合体のシグナル伝達ドメイン及び抗原認識ドメイン(例えば、抗体の一本鎖断片(scFv)又は他の抗体断片)のキメラである(Enblad et al.,Human Gene Therapy.2015;26(8):498−505)。CARを発現するT細胞は、CAR T細胞と呼ばれる。CARは、MHCに制限されない様式で選択された標的に対するT細胞の特異性及び反応性を再指示する能力を有する。MHCに制限されない抗原認識は、CARを発現するT細胞に抗原プロセシングに非依存的な抗原を認識する能力を与え、それにより腫瘍エスケープの主要な機構をバイパスする。さらに、T細胞において発現されるとき、CARは、内在性のT細胞受容体(TCR)アルファ及びベータ鎖と有利に二量体化しない。CARは、それらが結合する抗原によって参照される場合が多い。例えば、「CD19 CAR」、「CD70 CAR」、「CD33 CAR」及び「BCMA CAR」は、それぞれCD19、CD70、CD33又はBCMAに特異的に結合する抗原結合ドメインを含むCARである。したがって、このような用語は、抗CD19 CAR、抗CD70 CAR、抗CD33 CAR及び抗BCMA CARと互換的である。抗原に特異的に結合するCARが、いずれかの用語法により参照され得ることは当業者によって理解されるであろう。
外部ドメインは、細胞外の体液に曝されるCARの領域であり、且ついくつかの実施形態において、抗原結合ドメイン、並びに任意選択によりシグナルペプチド、スペーサードメイン、及び/又はヒンジドメインを含む。いくつかの実施形態では、抗原結合ドメインは、短いリンカーペプチドと連結した免疫グロブリンのVL及びVHを含む一本鎖可変断片(scFv)である。リンカーは、いくつかの実施形態において、可動性のための一続きのグリシン及びセリン並びに可溶性を加えるための一続きのグルタミン酸及びリジンとともに親水性残基を含む。一本鎖可変断片(scFv)は、実際には抗体の断片ではないが、代わりに10〜約25個のアミノ酸の短いリンカーペプチドと連結した免疫グロブリンの重(VH)及び軽鎖(VL)の可変領域の融合タンパク質である。リンカーは通常、可動性のためにグリシン、及び溶解性のためにセリン又はスレオニンを豊富に含み、VHのN末端をVLのC末端に連結するか又はその逆のいずれかであり得る。このタンパク質は、定常領域の除去及びリンカーの導入にもかかわらず、元の免疫グロブリンの特異性を保持する。いくつかの実施形態では、本開示のscFvは、ヒト化されている。他の実施形態では、scFvは、完全ヒトである。さらに他の実施形態では、scFvは、(例えば、マウス及びヒト配列の)キメラである。
膜貫通ドメインは、膜をまたがる疎水性アルファヘリックスである。膜貫通ドメインは、CARの安定性をもたらす。いくつかの実施形態では、本明細書に提供されるとおりのCARの膜貫通ドメインは、CD8膜貫通ドメインである。他の実施形態では、膜貫通ドメインは、CD28膜貫通ドメインである。さらに他の実施形態では、膜貫通ドメインは、CD8及びCD28膜貫通ドメインのキメラである。他の膜貫通ドメインが、本明細書に提供されるとおりに使用されてもよい。いくつかの実施形態では、膜貫通ドメインは、CD8a膜貫通ドメインである:
内部ドメインは、受容体の機能的な末端である。抗原認識の後、受容体クラスター及びシグナルは、細胞に伝達される。最も一般的に使用される内部ドメインの構成要素は、CD3−ゼータであり、これは、3つの免疫受容活性化チロシンモチーフ(ITAM)を含有する。これは、抗原が結合された後に活性化シグナルをT細胞に伝達する。多くの場合、CD3−ゼータは、十分に能力のある活性化シグナルをもたらさない場合があり、したがって、共刺激シグナル伝達が使用される。例えば、CD28及び/又は4−1BBが、CD3−ゼータ(CD3ζ)とともに使用されて、増殖/生存シグナルを伝達し得る。したがって、いくつかの実施形態では、本明細書に提供されるとおりのCARの共刺激分子は、CD28共刺激分子である。他の実施形態では、共刺激分子は、4−1BB共刺激分子である。いくつかの実施形態では、CARは、CD3ζ及びCD28を含む。他の実施形態では、CARは、CD3−ゼータ及び4−1BBを含む。さらに他の実施形態では、CARは、CD3ζ、CD28、及び4−1BBを含む。表4は、本明細書で使用され得る4−1BB、CD28及びCD3−ゼータに由来するシグナル伝達ドメインの例を提供する。
CD70
いくつかの実施形態では、本開示のT細胞は、CD70を標的化するように設計されたキメラ抗原受容体(CAR)により操作される。CD70は、T細胞の増殖及び生存に関与する共刺激受容体であるCD27についてのリガンドとして最初に同定された。CD70は、ウイルス感染中の流入領域リンパ節において少ないパーセンテージの活性化T細胞及び抗原提示細胞上においてのみ見出される。明細胞腎臓癌、乳癌、胃癌、卵巣癌、神経膠芽腫、及び血液悪性腫瘍などの固形癌を含むがこれらに限定されない多くのヒト腫瘍もまた、CD70を発現する。正常組織におけるその限定的な発現パターン及び多くの癌における過剰発現のために、CD70は、魅力的な治療標的である。
いくつかの実施形態では、本開示のT細胞は、BCMAを標的化するように設計されたCARにより操作される。B細胞成熟抗原(BCMA、CD269)は、腫瘍壊死因子受容体(TNF)スーパーファミリーのメンバーである。BCMAは、B細胞活性化因子(BAFF)及び増殖誘導リガンド(APRIL)に結合する。非悪性細胞の中で、BCMAは、形質細胞及び成熟B細胞のサブセットによって主に発現される。BCMAは、多発性骨髄腫細胞及び非ホジキンリンパ腫を含むB系列細胞によって選択的に発現され、したがって、BCMAもまた、魅力的な治療標的である。
いくつかの実施形態では、本開示のT細胞は、CD19を標的化するように設計されたCARにより操作される。表面抗原分類19(CD19)は、白血病前駆細胞上で検出可能な抗原決定基である。ヒト及びマウスのアミノ酸及び核酸配列は、GenBank、Uniprot及びSwiss−Protなどの公的データベースにおいて見出すことができる。例えば、ヒトCD19のアミノ酸配列は、UniProt/Swiss−Prot受入番号P15391として見出すことができ、ヒトCD19のヌクレオチド配列のコード化は、受入番号NM−001178098で見出すことができる。CD19は、例えば、急性リンパ芽球性白血病、慢性リンパ球白血病及び非ホジキンリンパ腫を含む大部分のB細胞系列癌上で発現される。それはまた、B細胞前駆体の早期のマーカーである。例えば、Nicholson et al.Mol.Immun.34(16−17):1157−1165(1997)を参照されたい。
いくつかの実施形態では、本開示のT細胞は、CD33を標的化するように設計されたCARにより操作される。Siglec3としても知られるCD33は、シアル酸に結合することが知られる骨髄系列の細胞上で発現される膜貫通受容体である。CD33は、癌細胞(例えば、急性骨髄性白血病)において発現されるため、CD33は、これらの悪性腫瘍を標的化するための細胞表面マーカーとなると考えられる。
抗体(複数の形態において互換的に使用される)は、免疫グロブリン分子の可変領域に位置する少なくとも1つの抗原認識部位を介して炭水化物、ポリヌクレオチド、脂質、ポリペプチドなどの標的に特異的に結合できる免疫グロブリン分子である。本明細書で使用する場合、用語「抗体」は、インタクトな(すなわち、全長)モノクローナル抗体だけでなく、抗原結合断片(Fab、Fab’、F(ab’)2、Fv)、一本鎖可変断片(scFv)、その変異体、抗体部分を含む融合タンパク質、ヒト化抗体、キメラ抗体、ダイアボディ、直鎖状抗体、一本鎖抗体、単一ドメイン抗体(例えば、ラクダ又はラマVHH抗体)、多重特異性抗体(例えば、二重特性抗体)並びに抗体のグリコシル化バリアント、抗体のアミノ酸配列バリアント、及び共有結合的に修飾された抗体を含む、要求される特異性の抗原認識部位を含む免疫グロブリン分子の任意の他の修飾された配置も包含する。
いくつかの実施形態では、本明細書に記載される操作されたT細胞は、腫瘍抗原標的化CARを含む。いくつかの実施形態では、腫瘍抗原は、通常、全く発現されない場合があるか、又は低レベルでのみ発現される場合がある非腫瘍細胞より腫瘍細胞においてより高レベルで発現されるタンパク質などの免疫原性分子を参照させる「腫瘍関連抗原」である。いくつかの実施形態では、腫瘍を内部に持つ宿主の免疫系によって認識される腫瘍関連構造は、腫瘍関連抗原と呼ばれる。いくつかの実施形態では、腫瘍関連抗原は、大部分の腫瘍によって広範に発現される場合、普遍的な腫瘍抗原である。いくつかの実施形態では、腫瘍関連抗原は、分化抗原、変異性抗原、過剰発現された細胞抗原又はウイルス抗原である。いくつかの実施形態では、腫瘍抗原は、腫瘍細胞に固有のタンパク質などの免疫原性分子を指す「腫瘍特異抗原」又は「TSA」である。腫瘍特異抗原は、腫瘍細胞において排他的に発現される。いくつかの実施形態では、腫瘍抗原は、CD70ではない。
いくつかの実施形態では、本明細書に記載される操作されたT細胞は、CD19 CAR、抗CD19 CAR又は抗CD19 CAR T細胞とも本明細書で呼ばれるCD19標的化CARを含む。いくつかの実施形態では、抗CD19 CARは、(i)抗CD19抗原結合ドメインを含む外部ドメイン、(ii)膜貫通ドメイン、及び(iii)少なくとも1つの共刺激ドメインを含む内部ドメインを含む。

ドナー鋳型
CARをコードする核酸は、ドナー鋳型と本明細書で呼ばれる(ドナーポリヌクレオチドとも呼ばれる)ものを含むT細胞に送達され得る。ドナー鋳型は、目的のゲノム位置で効率的なHDRを可能にする2つの相同な領域に隣接したCARをコードする核酸などの非相同配列を含有し得る。或いは、ドナー鋳型は、DNA中の標的化された位置と相同な領域を有しない場合があり、標的部位での切断後のNHEJ依存的末端連結によって組み込まれ得る。

ヌクレアーゼ及び/又はドナー鋳型は、プラスミドベクター、DNA小環、レトロウイルスベクター、レンチウイルスベクター、アデノウイルスベクター、ポックスウイルスベクター;ヘルペスウイルスベクター及びアデノ関連ウイルスベクター、及びその組合せを含むが、これらに限定されないベクター系を使用して送達され得る。
CARコンストラクトをコードするドナー核酸は、アデノ随伴ウイルス(AAV)を使用して細胞に送達され得る。AAVは、宿主ゲノムに部位特異的に組込み、したがって、CARなどの導入遺伝子を送達できる小型のウイルスである。逆位末端配列(ITR)は、AAVゲノム及び/又は目的の導入遺伝子に隣接して存在し、複製開始点として機能する。また、AAVゲノムには、転写されたときにAAVゲノムをカプセル化して標的細胞に送達するためのカプシドを形成するrepタンパク質及びcapタンパク質も存在する。これらのカプシド上の表面受容体は、AAVの血清型を付与し、カプシドが主にどの標的臓器に結合するかを決定し、したがって、AAVが最も効率的に感染することになる細胞を決定する。現在知られているヒトAAVの血清型は12種類である。いくつかの実施形態では、AAVは、AAV血清型6(AAV6)である。
CARをコードするドナー核酸は、相同組換え修復(HDR)によって標的遺伝子座位に挿入される。標的部位でのDNAの両方の鎖が、CRISPR Cas9酵素によって切断される。次に、HDRが発生して、二重鎖切断(DSB)を修復し、ドナーDNAを挿入する。これが正しく発生するために、ドナー配列は、標的遺伝子のDSB部位を取り囲む配列に相補的な隣接している残基(以下、「相同性アーム」)を有するように設計される。これらの相同性アームは、DSB修復のための鋳型として機能し、本質的に誤りのない機構であるHDRを可能にする。相同組換え修復(HDR)の割合は、変異部位と切断部位の間の距離の関数であり、そのため、重複している配列又は近くの標的を選択することが重要である。鋳型は、相同領域に隣接した余分な配列を含んでもよいし、ゲノム配列と異なる配列を含有してもよく、したがって、配列編集が可能になる。
本開示の操作された(遺伝子編集された)CAR T細胞は、自己由来(「自己」)の又は非自己由来(「非自己」、例えば、異質遺伝子型、同一遺伝子又は異種間)であり得る。「自己由来」は、同じ対象からの細胞を指す。「異質遺伝子型」は、対象と同じ種であるが、対象中の細胞と遺伝的に異なる細胞を指す。いくつかの実施形態では、T細胞は、哺乳動物から得られる。いくつかの実施形態では、T細胞は、ヒトから得られる。
いくつかの実施形態では、本明細書に記載される操作されたT細胞は、細胞のゲノムを改変することによって生成される。いくつかの実施形態では、標的遺伝子中の部位での二重鎖切断(DSB)が誘導される。いくつかの実施形態では、DSBは、1つ以上の内在性のDNA修復経路を使用して修復される。いくつかの実施形態では、DNA修復経路は、相同配列を必要としない(例えば、非相同末端連結経路又はNHEJ経路)。いくつかの実施形態では、修復経路は、相同配列を必要とする(例えば、相同組換え経路又はHDR経路)。
本明細書では、いくつかの実施形態において、癌を治療するための方法が提供される。本明細書に提供されるとおりに治療され得る癌の非限定的な例としては、多発性骨髄腫、白血病(例えば、T細胞白血病、B細胞急性リンパ性白血病(B−ALL)、及び/又は慢性リンパ性白血病(C−CLL))、リンパ腫(例えば、B細胞非ホジキンリンパ腫(B−NHL)、ホジキンリンパ腫、及び/又はT細胞リンパ腫)、及び/又は明細胞腎細胞癌(ccRCC)が挙げられる。いくつかの実施形態では、方法は、本開示のCAR T細胞(例えば、抗BCMA、抗CD19、抗CD33及び/又は抗CD70 CAR T細胞)を、多発性骨髄腫、白血病、又はリンパ腫を有する対象に送達することを含む。本明細書に提供されるとおりに治療され得る癌(例えば、固形腫瘍)の他の非限定的な例としては、膵臓癌、胃癌、卵巣癌、子宮頸癌、乳癌、腎臓癌、甲状腺癌、上咽頭癌、非小細胞肺(NSCLC)、神経膠芽腫、及び/又は黒色腫が挙げられる。

この実施例は、エクスビボでCRISPR/Cas9遺伝子編集を使用して一次ヒトT細胞におけるCD70遺伝子の効率的な編集を記載する。第1の3つのタンパク質をコードするエクソンを含有するCD70遺伝子のゲノムセグメントは、gRNA設計ソフトウェアにおける入力として使用された。ゲノムセグメントはまた、隣接するスプライス部位アクセプター/ドナー配列を含んだ。所望のgRNAは、CD70のアミノ酸配列を破壊し、アウトオブフレーム/機能喪失アレル(「CD70ノックアウト」アレルと呼ばれる)をもたらす、コード配列における挿入又は欠失を引き起こすことになるものであった。CD70遺伝子を標的化するインシリコで同定されたgRNAスペーサー配列中の7つ全てが合成され、gRNAは、表5において示されるとおり、特異的に修飾された。表5におけるgRNAは2’−O−メチルホスホロチオエート修飾で修飾されたが、未修飾gRNA、又は他の修飾を有するgRNAが使用されてもよい。また、この参照により全体として本明細書に組み込まれる、2018年5月11日に出願されたPCT/IB2018/001619号を参照されたい。
オンターゲットアンプリコン分析は、CD70遺伝子:
GCTTTGGTCCCATTGGTCGC(配列番号160;PAM配列番号114を有する標的配列)を標的化するT7ガイド(配列番号26;配列番号36)を使用する遺伝子編集の後にCD70座位で実施された。
この実施例は、2、3つ又は4つの遺伝子の発現を同時に欠くヒトT細胞を生成するCRISPR/Cas9遺伝子編集技術の使用を記載する。具体的には、T細胞受容体(TCR)遺伝子(TCRアルファ定常(TRAC)領域において編集された遺伝子)、2−ミクログロブリン(β2M)遺伝子、表面抗原分類70(CD70)遺伝子及び/又はプログラム細胞死1(PD−1又はPD1)遺伝子が、列挙された遺伝子の2つ以上においてT細胞欠損を生成するCRISPR/Cas9遺伝子編集によって編集された。以下の略称が、簡潔さ及び明確さのために図において使用される:

多重ノックアウトを有する抗CD70 CAR T細胞の生成
この実施例は、TCR遺伝子、β2M遺伝子、CD70遺伝子及び/又はPD1遺伝子の発現を欠き、且つCD70を標的化するキメラ抗原受容体(CAR)を発現する異質遺伝子型ヒトT細胞の産生を記載する。これらの細胞は、図においてTCR−/β2M−/CD70−/抗CD70 CAR+又は3X KO(CD70)CD70 CAR+;TCR−/β2M−/PD1−/抗CD70 CAR+又は3X KO(PD1)CD70 CAR+;TCR−/β2M−/PD1−/CD70−/抗CD70 CAR+又は4X KO CD70 CAR+と呼ばれる。
この実施例は、TCR遺伝子、β2M遺伝子、PD−1遺伝子、及び/又はCD70遺伝子の発現を欠き、且つB細胞成熟抗原(BCMA)を標的化するキメラ抗原受容体(CAR)も発現する異質遺伝子型ヒトT細胞の産生を記載する。
CD19を標的化するキメラ抗原受容体(CAR)を発現し、且つTCR遺伝子、β2M遺伝子、及び任意選択によりCD70遺伝子の発現を欠く、異質遺伝子型ヒトT細胞が生成された。
CD33を標的化するキメラ抗原受容体(CAR)を発現し、且つT細胞受容体(TCR)遺伝子(TCRアルファ定常(TRAC)領域において編集された遺伝子)、β2−ミクログロブリン(β2M)遺伝子、及び任意選択によりCD70遺伝子の発現を欠く、異質遺伝子型ヒトT細胞が生成された。
CD33は、T細胞上で発現され得るが、培養されたCD4細胞上においてCD8細胞より高いレベルで観察される。抗CD33 CAR−T細胞を生成する経過中、CD4細胞は、兄弟殺しのために実質的に減少する。図8に示されるとおり、インタクトなCD70を有する抗CD33 CAR−T細胞培養物は、3週間の培養期間にわたってCD4+細胞において97%の減少を示したが、破壊されたCD70を有する細胞の培養物は、この時間経過にわたってわずか61%の減少を示した。したがって、CD70遺伝子を破壊することは、抗CD33 CAR−T細胞培養物において観察される兄弟殺しを減少させるように見える。理論に束縛されるものではないが、この効果は、CD70/CD27相互作用によって増強され得る免疫刺激機能を介して生じる可能性があり、CD70の遺伝子破壊は、悪性腫瘍における治療効果により最適であり得るよりバランスのとれたCD4/CD8比をもたらす。
インビトロでの抗CD33 CAR T細胞の細胞増殖に対するCD70 KOの効果
サイトカイン含有培地(IL−2+IL−7)において細胞の増殖する能力を評価するため、抗CD33 CAR T細胞が利用された。具体的には、二重ノックアウト(TRAC−/B2M−)又は三重ノックアウト(TRAC−/B2M−/CD70−)を含む5×106個の総抗CD33 CAR T細胞が、実施例3に記載されるとおりに生成され、蒔かれ、10mL体積のサイトカイン含有培地中で増殖させられた。1週間後に細胞が計数された。次に、前の培養由来の5×106細胞が、10mL体積(新鮮なサイトカイン含有培地)に再び蒔かれ、1週間後、細胞の総数が数え上げられた。CD70遺伝子における破壊を含有する異質遺伝子型抗CD33 CAR−T細胞は、第1週及び第2週の再播種においてより高いレベルまで増殖した(図9)。これらのデータは、CD70ノックアウトが、培養においてより高い細胞収量をもたらし得ることを示す。
サイトカイン含有培地(IL−2+IL−7)において細胞の増殖する能力をさらに評価するため、抗CD19 CAR T細胞が利用された。具体的には、二重ノックアウト(TRAC−/B2M−)又は三重ノックアウト(TRAC−/B2M−/CD70−)を含む5×106個の総抗CD19 CAR T細胞が、実施例3に記載されるとおりに生成され、蒔かれ、10mL体積のサイトカイン含有培地中で増殖させられた。1週間後に細胞が計数された。次に、前の培養由来の5×106細胞が、10mL体積(新鮮なサイトカイン含有培地)に再び蒔かれ、1週間後、細胞の総数が数え上げられた。CD70遺伝子における破壊を含有する異質遺伝子型抗CD19 CAR−T細胞は、CD70遺伝子破壊を伴わない対照細胞と比較して、第1週及び第2週の再播種においてより高いレベルまで増殖した(図10)。これらのデータは、CD70ノックアウトが、培養においてより高い細胞収量をもたらし得ることを示す。
サイトカインに駆動される増殖。細胞増殖に対するCD70及び/又はPD1ノックアウトの効果を評価するため、抗BCMA CAR T細胞が利用された。抗BCMA CAR T細胞は、実施例3に記載されるとおりに生成された。以下の編集されたT細胞の群が生成された:

CAR T細胞においてCD70遺伝子を破壊することの影響をさらに評価するため、抗CD70 CAR T細胞が、実施例3に記載されるとおりに生成された。具体的には、3X KO(TRAC−/β2M−/CD70−)抗CD70 CAR T細胞が、異なるgRNA(T7(配列番号36及びT8(配列番号37))を使用して生成された。エレクトロポレーションの後、細胞増殖が、生存細胞を計数することによって実施例2に記載されるとおりに評価された。図15は、三重ノックアウトTRAC−/β2M−/CD70−/T7又はT8 gRNAのいずれかで生成された抗CD70 CAR+ T細胞は、二重ノックアウトTRAC−/β2M−/抗CD70 CAR+ T細胞と比較して高い細胞増殖を示した。これらのデータは、CD70遺伝子をノックアウトすることが、抗CD70 CAR+ T細胞に細胞増殖の優位性をもたらすことを示唆する。
CD70ノックアウトを有する抗CD19 CAR T細胞の細胞を死滅させる機能
実施例3に記載されるとおりの編集された抗CD19 CAR T細胞の作製の後、CAR T細胞の機能活性を、フローサイトメトリーに基づく細胞障害性アッセイを使用して検証した。抗CD19 CAR T細胞(TRAC−/β2M−/CD19 CAR+及びTRAC−/β2M−/CD70−/CD19 CAR+)は、2つのCD19発現癌細胞株(標的細胞):Nalm6(ATCC crl3273)又はRaji(ATCC ccl−86)の1つと共培養された。標的細胞は、5μM efluor670(eBiosciences)で標識され、洗浄され、TRAC−/β2M−/抗CD19 CAR+、又はTRAC−/β2M−/β2M−/CD70−/抗CD19 CAR+との様々な比(0.01、0.05、0.1、0.5、1:1 T細胞:標的細胞)での共培養物中においてインキュベートされた。標的細胞は、96ウェル、U底プレートにおいてウェル当たり50,000細胞で播種された。共培養物は、一晩インキュベートされた。インキュベーションの後、ウェルは洗浄され、培地は、5mg/mL DAPI(Molecular Probes)の1:500希釈物を含有する200μLの培地で置き換えられた。次に、25μLのCountBrightビーズ(Life Technologies)が、各ウェルに加えられ、細胞培養物が、フローサイトメトリーによって細胞生存率について分析された(すなわち、生存細胞はDAPI染色について陰性である)。

細胞死滅アッセイを使用して、TRAC−/β2M−/CD70−/抗CD70 CAR+細胞及びTRAC−/β2M−/PD−1−/CD70−/抗CD70 CAR+細胞のCD70+接着腎細胞癌(RCC)由来細胞株(A498細胞)を死滅させる能力を評価した。接着細胞を、ウェル当たり50,000細胞で96ウェル中に播種し、37℃で一晩置いた。翌日、編集された抗CD70 CAR T細胞を、指定の比率で標的細胞を含有するウェルに加えた。指定されたインキュベーション期間の後、CAR T細胞を、吸引によって培養物から除去し、100μLのCell titer−Glo(Promega)をプレートの各ウェルに加えて、残留している生存細胞の数を評価した。次に、ウェル当たりの放出された光の量を、プレートリーダーを使用して定量化した。細胞は、24時間の共インキュベーション後にRCC由来細胞の強力な細胞の死滅を示した(図19)。抗CD70 CAR T細胞は、CD70がノックアウトされた場合により高い効力を示したが、これは、低いT細胞:A498比(1:1及び0.5:1)で明白に目に見え、ここで、細胞溶解は、TRAC−/β2M−/CD70−/抗CD70 CAR+については90%を超えたままであるが、TRAC−/β2M−//抗CD70 CAR+については細胞溶解が90%未満に落ちる。このことは、CD70遺伝子をノックアウトすることが、抗CD70 CAR+ T細胞により高い細胞死滅の能力をもたらすことを示唆する。
CD70ノックアウトは、連続的な再チャレンジ時に抗CD33 CAR+ Tによる細胞の死滅を増進する
抗原発現細胞(例えば:標的細胞)によるチャレンジ及び再チャレンジ後に細胞の増殖する能力を評価するため、抗CD33 CAR T細胞が、実施例3において記載されるとおりに生成され、且つ利用された。具体的には、合計5×106個のT細胞が、5×106個の照射された標的細胞(MV−4−11)の存在下で蒔かれ、10mL体積において増殖させられた。1週間後、細胞は計数され、続いて、前の培養物からの5×106細胞が、新鮮な一定分量の5×106個の照射された標的細胞とともに10mL体積において再度蒔かれ、1週間後に細胞の総数が数え上げられた。この工程が示されるとおりに繰り返され、各再チャレンジは5×106細胞で開始された。生存細胞の数は、実施例2において示されるとおりに計数された。CD70遺伝子における破壊を含有する異質遺伝子型抗CD33 CAR−T細胞は、MV−4−11細胞による2回のチャレンジの後の第2週においてより高いレベルまで増殖した(図20)。これらのデータは、CD70が、抗原発現細胞の存在下でT細胞増殖を制限でき、その減少が、抗原刺激後により高い細胞増殖をもたらすことができることを示す。
抗原発現細胞(例えば:標的細胞)によるチャレンジ及び再チャレンジ後に細胞の増殖する能力をさらに評価するため、抗CD19 CAR T細胞が、実施例3において記載されるとおりに生成され、且つ利用された。具体的には、合計5×106個のT細胞が、5×106個の照射された標的細胞(Nalm6)の存在下で蒔かれ、10mL体積において増殖させられた。1週間後、細胞は計数され、続いて、前の培養物からの5×106細胞が、新鮮な一定分量の5×106個の照射された標的細胞とともに10mL体積において再度蒔かれ、1週間後に細胞の総数が数え上げられた。この工程が示されるとおりに繰り返され、各再チャレンジは5×106細胞で開始された。生存細胞の数は、実施例2において示されるとおりに計数された。CD70遺伝子における破壊を含有する異質遺伝子型抗CD19 CAR−T細胞は、最初の2回のチャレンジ中に同様の量まで増殖した。しかしながら、3回のチャレンジで、CD70遺伝子における破壊を含有する異質遺伝子型抗CD19 CAR−T細胞は、Nalm6細胞による3回目のチャレンジにおいてより高いレベルまで増殖した(図21)。これらのデータは、CD70の存在が、抗原発現細胞の存在下でT細胞増殖を制限でき、その減少が、繰り返された抗原刺激後により高い細胞増殖をもたらすことができることを示す。
上で生成された抗CD70 CAR+ T細胞は、CD70+腎臓癌細胞株A498で連続的に再チャレンジされ、それらのCD70+腎臓癌細胞株A498又はACHNを死滅させる能力について評価された。
実施例3に記載されるとおりに生成される抗BCMA CAR+ T細胞は、連続的に再チャレンジされ、且つBCMA+多発性骨髄腫細胞株MM.1S(ATCC CRL−2974)を死滅させるそれらの能力について評価された。CAR結合を介する連続的なT細胞活性化時にサイトカインを分泌する能力もまた、各再チャレンジの後に測定された。MM.1S細胞は、5μM eFlour670で標識され、2つ(TRAC−/β2M−)又は4つ((TRAC−/β2M−/PD−1−/CD70−)のgRNA編集のいずれかを含有する1×106個の抗BCMA CAR+ T細胞を有する6ウェルの組織培養皿において2:1(MM.1S対T細胞)の比で混合された。MM.1S細胞に対する暴露の1日後、一定分量の培養物を採取し、CAR−T細胞による標的細胞の死滅及びIFN−g分泌の両方について分析した。サイトカイン放出を測定するため、T細胞及び標的細胞を、指定の比で24時間共インキュベートした。上清培地を、製造業者の指示書(RD Systems)に従って新しいプレート上でのIL−2又はIFNγELISA(RD Systems)における使用のために回収した。細胞の死滅を定量するため、細胞は洗浄され、培地は、5mg/mL DAPI(Molecular Probes)の1:500希釈物を含有する200μLの培地で置き換えられた(死/瀕死細胞を数え上げるため)。最後に、25mLのCountBrightビーズ(Life Technologies)を、各ウェルに加えた。次に、細胞は、フローサイトメトリーによって処理された。

多重ノックアウト抗CD70 CAR+ T細胞のA498−PD−L1腎癌細胞への効果の比較
細胞死滅アッセイ。多重に遺伝子編集された抗CD70 CAR+細胞のPD−L1を過剰発現するA498腎癌細胞を死滅させる能力は、本明細書に記載される細胞死滅アッセイを使用して判定された。PD−L1(CD274)を過剰発現する細胞を作製するため、A498細胞を、PD−L1 cDNA及びピューロマイシン耐性遺伝子をコードするレンチウイルス(Genecopoeia)で感染させた。ピューロマイシンによる選択の後、細胞を抗PD−L1抗体で染色して、PD−L1の発現を評価した。PD−L1を発現するA498細胞は、A498−PD−L1とも呼ばれ、記載される機能アッセイにおいて使用された。
抗CD70 CART細胞におけるCD70及びPD1ノックアウトの有効性:NOGマウスにおける皮下腎細胞癌腫瘍異種移植モデル
小型の腫瘍モデルにおける治療
高レベルのCD70を発現する腎癌細胞を除去するCD70 CARを発現するT細胞の能力は、マウスにおける皮下腎細胞癌(A498)腫瘍異種移植モデルを使用してインビボで評価された。
より大型の腎細胞癌腫瘍に対する抗CD70 CAR T細胞のインビボでの有効性が調査された。上のとおり、CRISPR/Cas9及びAAV6を使用して、CD70を標的化するCARコンストラクト(配列番号45)を使用してTRAC座位からの同時の発現を伴ってTCR、β2M、CD70及び/又はPD1の発現を欠くヒトT細胞を作製した。この実施例において、活性化T細胞はまず、TRAC(配列番号40)、β2M(配列番号41)、PD1(配列番号42)、及びCD70(配列番号36又は37)を標的化するsgRNAを含有する2、3、又は4種の異なるCas9:sgRNA RNP複合体により電気穿孔された。TRAC座位でのDNA二重鎖切断は、キメラ抗原受容体カセット(遺伝子発現に関する−/+調節エレメント)に隣接するTRAC座位に対して右及び左の相同性アームを含有するAAV6に送達されるDNA鋳型(配列番号43)(配列番号5のアミノ酸配列を含む抗CD70 CARをコードする)による相同組換え修復によって修復された。
NOGマウスにおける皮下MM.1S腫瘍異種移植モデルに対するTRAC−/β2M−/抗BCMA(4−1BB共刺激)CAR+ T 細胞、TRAC−/β2M−/PD−1−/抗BCMA(4−1BB共刺激)、TRAC−/β2M−/CD70−/抗BCMA(4−1BB共刺激)、及びTRAC−/β2M−/PD−1−/CD70−/抗BCMA(4−1BB共刺激)CAR+ T細胞の有効性が評価された。簡潔には、25頭の5〜8週齢雌、CIEA NOG(NOD.Cg−PrkdcscidI12rgtm1Sug/JicTac)マウスは、試験の開始の5〜7日前に病原体を含まない条件下で維持された、換気されたマイクロアイソレーターケージにおいて個別に飼育された。1日目において、25頭のマウスは、50%マトリゲル/マウス中にある5×106個のMM.1S細胞の右側腹部における皮下接種を受けた。平均腫瘍体積が、75と125mm3の間に達したとき、マウスは、5つの治療群(N=5)に分けられ、表18に示されるとおりの約50%の抗BCMA CAR+ T細胞を含むT細胞集団を投与された。
CD70ノックアウトを伴うもの及び伴わないものの両方のいくつかの抗BCMA CAR+ T細胞遺伝子型の有効性は、NOGマウスにおける皮下RPMI−8226腫瘍異種移植モデルに対して評価された。簡潔には、85頭の5〜8週齢雌、CIEA NOG(NOD.Cg−PrkdcscidI12rgtm1Sug/JicTac)マウスは、試験の開始の5〜7日前に病原体を含まない条件下で維持された、換気されたマイクロアイソレーターケージにおいて個別に飼育された。1目において、マウスは、10×106個のRPMI−8226細胞の皮下接種を受けた。RPMI−8226細胞による摂取の10日後、マウスは、17種の治療群(N=5)に分けられ、表20に示されるとおりの約80%の抗BCMA CAR+ T細胞を含むT細胞集団を投与された。
サイトカイン依存性。遺伝子編集が、制御されない細胞増殖などの有害な特性を有する細胞を生成し得る望まれないオフターゲット編集をもたらしたかどうかを判定するため、サイトカイン及び/又は血清の非存在下で増殖する遺伝子編集されたCAR T細胞の能力が評価された。
抗CD70 CAR+ T細胞の機能に対する多重ノックアウトの効果

枯渇マーカーPD−1及びLAG3のレベルは、上の実施例において使用されるTRAC−/β2M−/抗CD70 CAR+(2X KO、CD70 CAR+)、TRAC−/β2M−/PD−1−/抗CD70 CAR+(3X KO(PD−1)、CD70 CAR+)、TRAC−/β2M−/CD70−/抗CD70 CAR+(3X KO(CD70)、CD70 CAR+)及びTRAC−/β2M−/PD−1−/CD70−/抗CD70 CAR+(4X KO、CD70 CAR+)T細胞に対して評価された。CD4+ T細胞は、PD−1発現(図38)について評価され、CD8+ T細胞及びCD4+ T細胞の両方が、フローサイトメトリーによってLAG3発現(それぞれ図39A及び39B)について評価された。
様々な癌細胞株におけるCD70発現。相対的なCD70発現を様々な癌細胞株において測定して、様々な癌型を死滅させる抗CD70 CAR+ T細胞の能力をさらに評価した。CD70発現は、Alexa Fluor 647抗ヒトCD70抗体(BioLegend Cat.No.355115)を使用するFACS分析によって測定された。図40A(左のグラフ)は、他の腎臓癌細胞株A498、786−O、cacki−1及びCaki−2と比較した、FACSによって測定されるとおりのACHN細胞における相対的なCD70の発現を示す。さらに、非腎臓癌細胞株は、Alexa Fluor 647抗ヒトCD70抗体(BioLegend Cat.No.355115;図40A、右のパネル)又は図40BにおけるFITC抗ヒトCD70抗体(BioLegend Cat.No.355105)のいずれかを使用するFACS分析(表22、図40A及び図40B)によってCD70発現について評価された。SNU−1(腸癌細胞)は、A498と同様の高レベルのCD70発現を示した(図40A、右のパネル)。SKOV−3(卵巣)、HuT78(リンパ腫)、NCI−H1975(肺)及びHs−766T(膵臓)細胞株は、ACHNと同様であるか又はより高いが、A498より低いCD70発現のレベルを示した(表22、図40B)。
卵巣腫瘍モデルにおける治療
中程度のレベルのCD70を発現する卵巣腺癌細胞を除去する抗CD70 CARを発現するT細胞の能力は、マウスにおける皮下卵巣癌(SKOV−3)腫瘍異種移植モデルを使用してインビボで評価された。
中程度のレベルのCD70を発現する肺腺癌細胞を除去するCD70 CARを発現するT細胞の能力は、マウスにおける皮下肺癌(NCI−H1975)腫瘍異種移植モデルを使用してインビボで評価された。
中程度のレベルのCD70を発現する膵臓癌細胞を除去するCD70 CARを発現するT細胞の能力は、マウスにおける皮下膵臓癌(Hs 766T)腫瘍異種移植モデルを使用してインビボで評価された。
T細胞リンパ腫を除去する抗CD70 CARを発現するT細胞の能力は、マウスにおける皮下T細胞リンパ腫(Hu T78)腫瘍異種移植モデルを使用してインビボで評価された。

- 前記RNA誘導型ヌクレアーゼ及びgRNAが、リボ核タンパク質粒子(RNP)中で複合体を形成する、請求項111〜120のいずれか一項に記載の方法。
- 標的細胞に対する免疫療法のための操作されたT細胞を生成するための方法であって、
(a)T細胞中のCD70遺伝子を破壊すること、及び
(b)前記標的細胞上で発現される抗原(前記抗原は、CD70ではない)に結合するCARを発現させることを含む方法。 - 前記標的細胞が、癌細胞である、請求項122に記載の方法。
